Introduction: A hospital information system (HIS) is a comprehensive, integrated information system designed to manage the administrative, financial and clinical aspects of a hospital. As an area of medical informatics, the introduction of a HIS into a hospital was purported to reduce the time spent on administrative and clinical activities by electronic data processing. However, adoption has been slow, and a key concern has been that staffs will require more time to complete their work using HIS. And also Most of available HIS have lots of deficiencies in data gathering, so hospital managers decided to keep their paper records and same time use HIS. Most previous studies addressing this issue have been done with concentration on clinical staff activities. Methods: Comparative observational of the impact of a HIS on Medical Records Department time in four units: Admission Discharge and Transfer/ADT, Medical Filing and Retrieval, Medical coding/billing, and Medical Statistics. Measurements: We used a time-motion method to measure staff time spent in predefined activities. Medical Records staff monitored while using HIS and paper based system. 15 clerical medical records staff were observed treating 300 documents while still using paper-based records, and same persons were observed treating 400 patients’ records with HIS. Results: Following HIS implementation, the use of the computerized system decreased the average total workload by 61.35% (from 66.41 to 23.93 m). Conclusion: Hospital Information System (HIS) reduces three nonclinical activities time noticeably which is vital to decision making and plays a crucial role in the success of the organization.

Introduction: Medical records department‘s duties includes: collecting, editing, resorting the patients files, and it is one of the important departments in hospitals. Evaluation of medical records department in Iran shows that these departments have not achieved desirable standards and high quality services. The aim of present research was to evaluate the medical records department services (reception, statistical, coding, and filing) of Qom,s University of Medical Sciences Teaching-Curative hospitals. Methods: Present research is a cross-sectional descriptive study in which the medical records departments services in Qom,s University of Medical Sciences Teaching-Curative hospitals ( AL Zahra, Ezadi, Her-Majesty Masomeh, Fatemi, Sahamiye, Kamkar , and Nekooyee) have been evaluated. Data have been collected by check lists completed by interviews and observations. Personals Demographic information for medical records departments also have been collected by questioner and face to face interviews. Results: Our findings showed that the average score of medical records departments of the Masomeh hospital 10.54, Izadi 11.79, Al- Zahra 12.21, Nekoyee 15.34, Kamkar 15.73, and Fatemi sahameyee 16.44, were of 20, in which their scores have been evaluated, Al Zahra , Izadi, and Her-Majesty Masomeh were Intermediate, Kamkar and Nekoyee Good, and Fatemyee sahamyee Very Good. Conclusions:Half of these hospital have a under desirables scores on their services, so in order to optimizing these departments services, it is necessary that first of all the medical records departments officers in studied hospital pay close attention to allocate resources for each departments according to relevance standards and secondly pay attention to present guidelines and documented procedures of each units, providing documented job-descriptions to staffs ,with definitions and work standards in case of each units activities, applying educated personal and suitable distribution of them in units.

Introduction: Supervision over main and complementary parts of planning and their evalvation, comprise the planning cycle which emphasizes on the qualitative as wellas quantitative procedures being carried out. Pervision makes the necessary reforms on the input, while evaluation takes care of midterm and longterm plans and procedures. The objective of this study was to evalvate the fonetion of medicalrecords department at Mashhad school of Medicine in 1381. Methods: This is a cross - seetional descriptive survey study.Data collection was done through four check lists using Interviewond observation methods. Indesigning the check lists. Likret criteria was used to evaluate the checklist questions. Findings: Non of the hospitals understudy bene fited from a M.R.A graduate. Medical Record department at Imam Reza hospital scord 94% (55.29) While Hasheminejad hospital scord 50 (29.41) the results of the surrey showed that the medical recods of Mashhad theaching hospitals with the average seove of 71.77 (42.42) had average performance range. Results: The results of the study showed that this is highly important that spcial instructions be written for very unit of Medical Records department, for job description, on the job training courses exployement of medical records graduates, resover provision, equipment and suitable envorinment, and also periodic evalution of the function of medical records departments for the improvement of the department.

Efficiency and effectiveness of the services of the organizations such as medical records departments depends on a dynamic management. The major objective of this study is to compare the management of medical records departments of general teaching hospitals affiliated to Iran, Tehran, and Shahid Beheshti Universities of Medical Sciences. In this descriptive, cross-sectional case and comparative study, the major variables are: planning, organization, implementation, and evaluation. Managers, experts, and heads of the four major units of the hospitals were examined. The results indicated that the managers of medical records units with an average of 2.53, their rate of management usage in the department estimated in a high level. But, the experts and the heads of the wards with the averages of 1.75 and 1.38 have estimated in a low level. The involvement of the medical records managers of IUMS in planning was highest, the involvement of the same managers of IUMS in organization and implementation showed highest, and involvement of these managers at SHBUMS in evaluation was in highest level.

The transaction of human beings with one another and life in the present age especially with the contemporary technology advances have become so complicated that unless we describe a specific standard for each item and not obligate people to regard those specific standards, the lifetime will be tiresome. So the more the standardization and higher commitment to meeting the standards, the more the advancement and elevation of the society will be. Materials and Methods: This descriptive research was conducted cross-sectionally. The statistical universe of research was medical records department of 16 teaching hospitals affiliated to Iran and Kermanshah universities of medical sciences. Data gathering tools were five checklists adjusted by ISO 9001- 2000 international standards. Then the data was analyzed using indices of descriptive statistics. Results: The results showed that in the hospitals related to Iran university of medical sciences the criteria of resource management with 77% had the highest score, and the measurement of criteria analysis and process improvement with 47 % had the least score and in hospitals related to Kermanshah university of medical sciences , resources management with 61% indicated the highest score and quality management system with 33% had the least score. And the total mean of the standard criteria in the hospitals related to Iran university of medical sciences was 60% and in hospitals related to Kermanshah university of medical sciences was 46%. Conclusion: The rate of final conformity of medical records system by the criteria of the ISO 9001-2000 standards in hospitals related to Iran university of medical sciences was greater than in hospitals related to Kermanshah university of medical sciences. And total conformity rate of medical records system in Kermanshah hospitals was low. So the regulation of medical records department with ISO quality management standards can help to elevate its quality.
