Background: The anterior cruciate ligament is composed of two functional bundles and is crucial for knee function. There is limited understanding of the role of each individual bundle and the influence on length pattern due to difference in bone tunnel position under loading conditions throughout the range of motion. We measured point to point length between the femoral and tibial footprints of the ligament throughout the range of motion in normal knees, under normal loading conditions, and investigated length pattern changes secondary to differences in the femoral footprint. We hypothesized that anteromedial and posterolateral bundles have complementary roles, and the ligament length pattern is influenced by the footprint position., Methods: We studied the squat movements of six healthy knees and measured point to point footprint distance. The footprint distances were measured after changing them to be 10% lower, 10% shallower, and both 10% lower and shallower than the defined anatomical femoral footprint., Results: Average length changes of 12.0 and 14.1 mm from maximum extension (10°) to deep flexion (150°) were observed when the anteromedial and posterolateral bundles were defined by the default anatomical position. Maximum and minimum length were reached during full extension and flexion for both the anteromedial and posterolateral bundles, respectively. At 10% lower, length increased 2.2 mm over the default value in both the anteromedial and posterolateral lengths. At 10% shallower, decreases of 4.1 mm and 3.9 mm were observed compared with the default anteromedial and posterolateral lengths, respectively. In the 10% lower and 10% shallower position, anteromedial and posterolateral lengths decreased 2.1 mm and 1.9 mm compared with the default value, respectively., Conclusions: The anteromedial and posterolateral bundles have a complementary role. Femoral footprint position defined in the lower direction leads to stronger tension during extension, while the higher and shallower direction leads to isometry during flexion, and the deeper direction leads to laxity during flexion. The target bone tunnel position is that the anteromedial bundle should not to be too low and too deep to maintain function of bundle with less change in length. In addition, the posterolateral bundle should be somewhat lower and/or deeper than the anteromedial, with the expectation that it will function to induce stronger tension at the extended position. However, we should avoid lower position when we cannot prepare a sufficient diameter of reconstructed bundle to avoid re-injury due to excessive tension., (Copyright © 2018 The Japanese Orthopaedic Association. Background: Proper soft tissue balance is crucial for a successful clinical outcome after total knee arthroplasty (TKA). Bi-cruciate substituting (BCS)-TKA has been developed to more closely approximate normal knee characteristics. The purpose of the present study was to evaluate midflexion laxity before and after BCS-TKA using a navigation system, and assess the correlation between intraoperative laxity and the maximum flexion angle after surgery., Methods: Fifty-one knees in 46 patients with osteoarthritis replaced with BCS prosthesis were assessed. Manual mild passive internal-external rotational and varus-valgus stress was applied to the knees, and the maximum total laxity was measured automatically by a navigation system before and after TKA. The correlations with the range of motion (ROM) were evaluated using Spearman's correlation coefficients (ρ)., Results: Internal-external stress assessment revealed no statistically significant difference at each flexion angle before and after BCS-TKA. In contrast, the varus-valgus stress assessment revealed that BCS-TKA had significantly decreased varus-valgus laxity from preoperative levels at 20-120° flexion angles. Furthermore, the maximum flexion angle at six months after surgery significantly correlated with the intraoperative laxity at deep flexion range., Conclusion: BCS-TKA stabilized varus-valgus laxity to better than preoperative levels at midflexion range., (Copyright © 2018 The Author(s). Purpose: To evaluate changes in midflexion rotational laxity before and after posterior-stabilized (PS)-total knee arthroplasty (TKA)., Methods: Twenty-nine knees that underwent PS-TKA were evaluated. Manual mild passive rotational stress was applied to the knees, and the internal-external rotational angle was measured automatically by a navigation system at 30°, 45°, 60°, and 90° of knee flexion., Results: The post-operative internal rotational laxity was statistically significantly increased compared to the preoperative level at 30°, 45°, 60°, and 90° of flexion. The post-operative external rotational laxity was statistically significantly decreased compared to the preoperative level at 45° and 60° of flexion. The post-operative internal-external rotational laxity was statistically significantly increased compared to the preoperative level only at 30° of flexion. The preoperative and post-operative rotational laxity showed a significant correlation at 30°, 45°, 60°, and 90° of flexion., Conclusion: Internal-external rotational laxity increases at the initial flexion range due to resection of both the anterior or posterior cruciate ligaments and retention of the collateral ligaments in PS-TKA. Preoperative and post-operative rotational laxity indicated a significant correlation at the midflexion range. This study showed that a large preoperative rotational laxity increased the risk of a large post-operative laxity, especially at the initial flexion range in PS-TKA., Level of Evidence: III.

Introduction: Midflexion stability can potentially improve the outcome of total knee arthroplasty (TKA). The purpose of this study was to evaluate the correlation between varus-valgus stability at 0° of extension and 90° of flexion and that at the midflexion range in posterior-stabilized (PS)-TKA., Materials and Methods: Forty-three knees that underwent PS-TKA were evaluated. Manual mild passive varus-valgus stress was applied to the knees, and the postoperative maximum varus-valgus stability was measured every 10° throughout range of motion, using a navigation system. Correlations between the stability at 0°, 90° of flexion, and that at each midflexion angle were evaluated using Spearman's correlation coefficients., Results: The stability of 0° modestly correlated with that of 10°-20°, but it did not significantly correlate with that of 30°-80°. However, the stability of 90° strongly correlated with that of 60°-80°, modestly correlated with that of 40°-50°, weakly correlated with that of 20°-30°, and did not correlate with that of 10°., Conclusions: The present study confirmed the importance of acquiring stability at 90° flexion to achieve midflexion stability in PS-TKA. However, initial flexion stability did not strongly correlate with the stability at either 0° or 90°. Our findings can provide useful information for understanding varus-valgus stability throughout the range of motion in PS-TKA. Attention to soft tissue balancing is necessary to stabilize a knee at the initial flexion range in PS-TKA.

Background: Restoration of normal knee kinematics is key to improving patient satisfaction and functional outcomes after total knee arthroplasty (TKA). However, the effect of preoperative varus-valgus kinematics due to knee osteoarthritis on the postoperative kinematics is unclear. The function of the knee ligament contributes to both knee stability and kinematics. The aim of this study was to evaluate changes in varus-valgus kinematics before and after TKA using a navigation system, in addition to comparing the pre- and postoperative changes in kinematic patterns between cruciate-retaining (CR)- and posterior-stabilized (PS)-TKAs., Methods: Forty knees treated with TKA were evaluated (CR-TKA 20; PS-TKA 20). Manual mild passive knee flexion was applied while moving the leg from full extension to flexion. The varus-valgus angle was automatically measured by a navigation system at every 10° of the flexion angle, and the kinematics were evaluated., Results: Kinematic patterns throughout flexion can be classified into five types. The pre- and postoperative kinematic patterns were similar in 60% of patients who underwent CR-TKA, whereas they were similar in only 25% of those who underwent PS-TKA. The mean change in the size of the varus-valgus angle throughout flexion did not differ between CR-TKA and PS-TKA. However, the distribution of changes in the size of the varus-valgus angle differed between CR-TKA and PS-TKA., Conclusions: We obtained the following results: 1) some patterns of varus-valgus kinematics are noted under unloading conditions despite recovery of neutral alignment in extension and 2) the preoperative varus-valgus kinematic pattern persisted more strongly after CR-TKA than after PS-TKA., (Copyright © 2015 Elsevier B.V. Purpose: In vivo fluoroscopic analyses have revealed the kinematics after total knee arthroplasty (TKA), including femoral condylar lift-off. This study asked whether differences in static varus-valgus laxity or coronal limb alignment after TKA affect lift-off under weight-bearing conditions. It was hypothesised that there is a correlation between coronal laxity or alignment and lift-off during walking., Methods: The current study analysed nineteen subjects undergoing cruciate-retaining TKA performed by the measured resection technique. The varus-valgus laxity at knee extension was measured using a 150 N stress radiograph. The mechanical axis was measured using a full-standing radiograph. Continuous radiological images were taken while the subject walked on a treadmill, and the images during single-leg stance were analysed to determine the lift-off using a 3D-to-2D image-to-model registration technique., Results: The average angle in varus/valgus stress was 6.8 ± 1.8°/6.6 ± 2.1°. No statistically significant differences were observed between the varus and valgus laxity. The average amount of lift-off was 0.7 ± 0.4 mm. The static varus-valgus laxity (n. s.) or the differences in the laxities (n. s.) on the stress radiograph did not influence lift-off. The weight-bearing ratio was achieved within the middle third of the knee in 90 % of subjects. Two outliers with valgus alignment (68 ± 1 %) demonstrated no significant difference in lift-off in comparison with the majority of the subjects (46 ± 9 %)., Conclusion: The static coronal laxity and alignment did not influence the lift-off under dynamic weight-bearing conditions after well-balanced and aligned cruciate-retaining TKA. Measured resection technique can produce sufficient coronal stability and alignment without significant lift-off during walking.

In total knee arthroplasty (TKA), the patella is significantly associated with range of motion and gait performance. Currently, no highly accurate methods are available that can measure the 3D in vivo behavior of the TKA patellar component, as the component is made of x-ray-permeable ultra-high molecular weight polyethylene. Previously, we developed a computer simulation that matches CT scan and unidirectional radiographic images using image correlations, and applied it to kinematic studies of natural and TKA knees. The examination of the measurement accuracy for the patellar bone of a fresh-frozen pig knee joint yielded a root mean square error of 0.2 mm in translation and 0.2° in rotation. In this study, we recruited four patients who had a TKA and investigated 3D movements of the patellar component during squatting. We could visualize the patellar component using the position of the holes drilled for the component peg, and estimated and visualized the contact points between the patellar and femoral components. The principles and the utility of the simulation method are reported. Purpose: We aimed to evaluate the feasibility and safety of selective arterial embolization for refractory knee hemarthrosis., Materials and Methods: Transcatheter arterial embolization for refractory knee hemarthrosis was performed on five female patients (median age, 77 years; range, 71-80 years) between May 2009 and September 2012. Selective arterial embolization of the feeding artery was performed using a 2.5 F microcatheter coaxially advanced from a 5 F catheter with its tip positioned in the superficial femoral artery. One- or two-millimeter gelatin particles were used as embolic agents., Results: In all patients, transarterial embolization performed successfully after arteriography showed nontumorous staining around the knee joint. The feeding arteries were the lateral superior genicular artery in all five patients, the lateral inferior genicular artery in four patients, the medial superior genicular artery in one patient, the medial inferior genicular artery in one patient, the middle genicular artery in one patient, and the descending genicular artery in one patient. In all five patients, staining was remarkably diminished around the knee joint after the embolization procedure. No complication was observed. The hemarthrosis improved after the embolization, and the postoperative course has been uneventful with no recurrence in any patient., Conclusion: These results suggest that selective arterial embolization for refractory hemarthrosis of the knee is safe and useful.

Purpose: Lateral radiographic views can be easily taken and have reveal considerable information about the patella. The purpose of this study was to obtain sagittal plane patellar kinematics data through the entire range of knee flexion under weight-bearing conditions., Methods: Patellar flexion angles relative to the femur and tibia and anterior-posterior and proximal-distal translations of the patella relative to the femur and tibia were measured from 0 to 165° knee flexion in nine healthy knees using dynamic radiographic images., Results: The patella flexed relative to the femur and tibia by two thirds times and one third times the knee flexion angle, respectively. The patella translated in an arc relative to the femur and tibia as the knee flexed. In early flexion, the superior and centroid points translated anteriorly and then the patella translated posteriorly relative to the femur. All three points of the patella translated posteriorly relative to the tibia during a full range of flexion. An average of four and three millimetres proximal patellar translation relative to the tibia was demonstrated from 0 to 20° and 140 to 160° knee flexion, respectively., Conclusions: Physiological sagittal plane patellar kinematics, including patellar flexion angles and translations relative to the femur and tibia, showed generally similar patterns for each subject. Measurements of dynamic radiographic images under weight-bearing activities may enhance the opportunity to identify patellar pathological conditions.

There are several methods for evaluating stability of the joint during total knee replacement (TKR). Activities of daily living demand mechanical loading to the knee joint, not only in full extension, but also in mid-flexion. The purpose of this study was to compare the varus-valgus stability throughout flexion in knees treated with either cruciate-retaining or posterior-stabilised TKR, using an intra-operative navigation technique. A total of 34 knees underwent TKR with computer navigation, during which the investigator applied a maximum varus-valgus stress to the knee while steadily moving the leg from full extension to flexion both before and after prosthetic implantation. The femorotibial angle was measured simultaneously by the navigation system at every 10° throughout the range of movement. It was found that posterior-stabilised knees had more varus-valgus laxity than cruciate-retaining knees at all angles examined, and the differences were statistically significant at 10° (p = 0.0093), 20° (p = 0.0098) and 30° of flexion (p = 0.0252).

Healthy knee kinematics during dynamic full flexion were evaluated using 3D-to-2D model registration techniques. Continuous knee motions were recorded during full flexion in a lunge from 85° to 150°. Medial and lateral tibiofemoral contacts and femoral internal-external and varus-valgus rotations were analyzed as a function of knee flexion angle. The medial tibiofemoral contact translated anteroposteriorly, but remained on the center of the medial compartment. On the other hand, the lateral tibiofemoral contact translated posteriorly to the edge of the tibial surface at 150° flexion. The femur exhibited external and valgus rotation relative to the tibia over the entire activity and reached 30° external and 5° valgus rotations at 150° flexion. Kinematics' data during dynamic full flexion may provide important insight as to the designing of high-flexion total knee prostheses.

After total knee arthroplasty, it is common for flexion contracture to exist during the early stages of postoperative course. We retrospectively investigated whether the early postoperative contracture would finally disappear, in 104 osteoarthritic knees after surgery with posterior-stabilized prostheses. The knees were divided into 5 groups based on their contracture 3 months after surgery (group I: no contracture, group II: 5°, group III: 10°, group IV: 15°, group V: ≥20°). The proportion of patients with residual contracture (≥5°) 2 years after surgery was 1/34 in group I, 4/30 in group II, 6/23 in group III, 6/6 in group IV, and 11/11 in group V. Dynamic knee kinematics were analyzed for medial osteoarthritic (OA) knees in three activities, including two types of maximum knee flexion. Continuous x-ray images of kneeling, squatting, and stair climbing motions were taken using a large flat panel detector. CT-derived bone models were used for the model registration-based 3D kinematic measurements. Three-dimensional joint kinematics and contact locations were determined using two methods: bone-fixed coordinate systems and by interrogation of CT-based bone model surfaces. The femur exhibited gradual external rotation with knee flexion for kneeling and squatting activities, and gradual internal rotation with knee extension for stair climbing. From 100 degrees to 120 degrees flexion, contact locations showed a medial pivot pattern similar to normal knees. However, knees with medial OA displayed a femoral internal rotation bias and less posterior translation when compared with normal knees. A classic screw-home movement was not observed in OA knees near extension. Decreased variability with both activities and methods of calculation were demonstrated for all three activities. In conclusion, the weight-bearing kinematics of patients with medial OA differs from normal knees. Pathological changes of the articulating surfaces and the ligaments correspond to observed abnormalities in knee kinematics.

We evaluated the postoperative alignment of 37 primary total knee arthroplasties performed using a computed tomography-based navigation system (Vector Vision Knee 1.5; Brain Lab, Germany) with a new 3-dimensional analysis. The mean coronal femoral angle was 89.0 degrees +/- 1.4 degrees (85.5 degrees -92.8 degrees ), and the coronal tibial component was 89.2 degrees +/- 1.0 degrees (87.4 degrees -91.6 degrees ). The hip-knee-ankle angle was observed to be 178.2 degrees +/- 1.5 degrees (173.9 degrees -181.8 degrees ). The external rotational alignment of the femoral component relative to the surgical epicondylar axis was -0.5 degrees +/- 1.7 degrees (-3.2 degrees to 3.4 degrees ). The results demonstrated that a computed tomography-based navigation system provided a reasonably satisfactory component alignment. The discrepancy between the 2-dimensional and 3-dimensional evaluations was 1.0 degrees +/- 0.9 degrees (0.1 degrees -3.4 degrees ). Three-dimensional analysis is necessary to evaluate the accuracy of the navigation system.

This study evaluated the mid-term results of total knee arthroplasty (TKA) following high tibial osteotomy (HTO), comparing posterior cruciate-retaining prostheses to posterior stabilized prostheses. The Knee Society score for the entire group (20 knees) improved significantly from 62 (median) preoperatively to 87 at the latest follow-up. The postoperative Knee Society score of 85 in posterior cruciate-retaining prostheses (8 knees) was significantly inferior to the 94 score in posterior stabilized prostheses (12 knees). Of Knee Society score, Stability and ROM scores (17 and 21, respectively) in posterior cruciate-retaining TKA were inferior to those in posterior stabilized TKA (25 and 24, respectively). Since postoperative knee instability due to posterior cruciate ligament (PCL) insufficiency is thought to contribute to the inferior results of posterior cruciate-retaining prostheses after HTO, PCL-substituting TKA would be suitable for use after HTO.

The expectation of returning to sports activities after total knee arthroplasty (TKA) has become more important to patients than ever. To our knowledge, no studies have been published evaluating the three-dimensional knee joint kinematics during sports activity after TKA. Continuous X-ray images of the golf swing and stationary cycling were taken using a large flat panel detector for four and eight post-arthroplasty knees, respectively. The implant flexion and axial rotation angles were determined using a radiographic-based, image-matching technique. Both the golf swing from the set-up position to the top of the backswing, and the stationary cycling from the top position of the crank to the bottom position of the crank, produced progressive axial rotational motions (p = 0.73). However, the golf swing from the top of the backswing to the end of the follow-through produced significantly larger magnitudes of rotational motions in comparison to stationary cycling (p < 0.01). Excessive internal-external rotations generated from the top of the backswing to the end of the follow-through could contribute to accelerated polyethylene wear. However, gradual rotational movements were consistently demonstrated during the stationary cycling. Therefore, stationary cycling is recommended rather than playing golf for patients following a TKA who wish to remain physically active.

We compared the alignment of 39 total knee replacements implanted using the conventional alignment guide system with 37 implanted using a CT-based navigation system, performed by a single surgeon. The knees were evaluated using full-length weight-bearing anteroposterior radiographs, lateral radiographs and CT scans. The mean hip-knee-ankle angle, coronal femoral component angle and coronal tibial component angle were 181.8 degrees (174.2 degrees to 188.3 degrees), 88.5 degrees (84.0 degrees to 91.8 degrees) and 89.7 degrees (86.3 degrees to 95.1 degrees), respectively for the conventional group and 180.8 degrees (178.2 degrees to 185.1 degrees), 89.3 degrees (85.8 degrees to 92.0 degrees) and 89.9 degrees (88.0 degrees to 93.0 degrees), respectively for the navigated group. The mean sagittal femoral component angle was 85.5 degrees (80.6 degrees to 92.8 degrees) for the conventional group and 89.6 degrees (85.5 degrees to 94.0 degrees) for the navigated group. The mean rotational femoral and tibial component angles were -0.7 degrees (-8.8 degrees to 9.8 degrees) and -3.3 degrees (-16.8 degrees to 5.8 degrees) for the conventional group and -0.6 degrees (-3.5 degrees to 3.0 degrees) and 0.3 degrees (-5.3 degrees to 7.7 degrees) for the navigated group. The ideal angles of all alignments in the navigated group were obtained at significantly higher rates than in the conventional group. Our results demonstrated significant improvements in component positioning with a CT-based navigation system, especially with respect to rotational alignment.

Dynamic knee kinematics were analyzed for normal knees in three activities, including two different types of maximum knee flexion. Continuous X-ray images of kneel, squat, and stair climb motions were taken using a large flat panel detector. CT-derived bone models were used for model registration-based 3D kinematic measurement. Three-dimensional joint kinematics and contact locations were determined using three methods: bone-fixed coordinate systems, interrogation of CT-based bone model surfaces, and interrogation of MR-based articular cartilage model surfaces. The femur exhibited gradual external rotation throughout the flexion range. Tibiofemoral contact exhibited external rotation, with contact locations translating posterior while maintaining 15 degrees to 20 degrees external rotation from 20 degrees to 80 degrees of flexion. From 80 degrees to maximum flexion, contact locations showed a medial pivot pattern. Kinematics based on bone-fixed coordinate systems differed from kinematics based on interrogation of CT and MR surfaces. Knee kinematics varied significantly by activity, especially in deep flexion. No posterior subluxation occurred for either femoral condyle in maximum knee flexion. Kneeling is an important function of the knee for many activities of daily living. In this study, we evaluated the in vivo kinematics of kneeling after total knee arthroplasty (TKA) using radiographic based image-matching techniques. Kneeling from 90 to 120 degrees of knee flexion produced a posterior femoral rollback after both cruciate-retaining and posterior-stabilized TKA. It could be assumed that the posterior cruciate ligament and the post-cam mechanism were functioning. The posterior-stabilized TKA design had contact regions located far posterior on the tibial insert in comparison to the cruciate-retaining TKA. Specifically, the lateral femoral condyle in posterior-stabilized TKA translated to the posterior edge of the tibial surface, although there was no finding of subluxation. After posterior-stabilized TKA, the contact position of the post-cam translated to the posterior medial corner of the post with external rotation of the femoral component. Because edge loading can induce accelerated polyethylene wear, the configuration of the post-cam mechanism should be designed to provide a larger contact area when the femoral component rotates., (c) 2007 Orthopaedic Research Society. We compared a group of 20 patients who had 24 minimally invasive total knee arthroplasties with a similar group of 21 patients who had 25 standard medial parapatellar approach total knee arthroplasties. We wanted to clarify whether the minimally invasive group had an advantage over the standard group in muscle strength, pain level, postoperative recovery, and clinical results and whether the patients were prone to radiographically poor results, more operative time, and increased complications. The extensor and flexor torque, visual analog scale, pace of rehabilitation, Knee Society scores, radiographic findings, operative time, and complications of each group were examined. The minimally invasive group showed higher extensor torque values, higher ratios of postoperative to preoperative extensor torque, and lower average visual analog scale scores at 1 and 2 weeks. The patients in this group achieved straight leg raising, 90[degrees] knee flexion, and T-cane gait earlier. There was no component malalignment, but the tibial component shifted to a more medial position. The mean operative time was 56 minutes longer in the minimally invasive group. We encountered no major perioperative complications in either group. We believe the minimally invasive technique positively contributes to the early restoration of quadriceps strength and a speedy return to normal function., ((C) 2007 Lippincott Williams & Wilkins, Inc.)

The present study investigated factors associated with knee pain and functional limitation in knee OA patients. Subjects were 109 Japanese males who were newly diagnosed with knee OA at three university hospitals over a 1-year period. Knee pain and functional limitation in walking and climbing and/or descending stairs were selected as outcome measures. To assess factors associated with outcomes, we calculated odds ratios (OR) using logistic regression analysis. Taller height (> or =163 vs. <163 cm) showed a negative association with knee pain, "Pain on walking": OR = 0.08, 95% confidence interval = 0.01-0.79; "Pain on stairs": 0.25, 0.08-0.82. A significant characteristic related to a lesser degree of functional disability was alcohol consumption ("Waking distance": 0.34, 0.14-0.84; "Help on stairs": 0.21, 0.09-0.51). In the present study, knee pain was associated with shorter height in male Japanese patients with knee OA. Functional limitation was associated with no consumption of alcohol.

Background: Understanding the normal kinematics of the joints is important for reconstructive surgery. However, only a few extensive studies have been done on medial and lateral laxity of the normal knee., Methods: Radiographs of 50 normal knees were obtained under varus and valgus stress in both extension and flexion and the relative angle of the articular surface was measured., Results: In extension, the mean angle was 4.9 degrees in varus stress and 2.4 degrees in valgus stress. In flexion, the mean angle was 4.8 degrees in varus stress and 1.7 degrees in valgus stress. Lateral laxity was significantly greater than medial laxity in both extension and flexion., Conclusions: Lateral laxity may be necessary for the medial pivot movement of the normal knee. There is some disagreement regarding the importance of pursuing the perfect rectangular gaps during total knee arthroplasty (TKA). The methods for measuring the tension of soft tissues during the operation are not accurate and do not always reflect the postoperative tensions of dynamic phases, such as walking and standing. Slight lateral laxity can be accepted with TKA, and further studies are necessary to determine whether prosthesis lift-off occurs in the replaced knee with slight lateral laxity similar to that in the normal knee.

We have previously developed a radiographic technique, the oblique posterior condylar view, for assessment of the posterior aspect of the femoral condyles after total knee arthroplasty. The purpose of this study was to confirm the validity of this radiographic view based upon intra-operative findings at revision total knee arthroplasty. Lateral and oblique posterior condylar views were performed for 11 knees prior to revision total knee arthroplasty, and radiolucent lines or osteolysis of the posterior aspect of the femoral condyles were identified. These findings were compared with the intra-operative appearance of the posterior aspects of the femoral condyles. Statistical analysis showed that sensitivity and efficacy were significantly better for the oblique posterior condylar than the lateral view. This method can, therefore, be considered as suitable for routine follow-up radiographs of the femoral component and in the pre-operative planning of revision surgery.

The wear phenomenon of ultra-high molecular weight polyethylene (UHMWPE) in knee and hip prostheses is one of the major restriction factors on the longevity of these implants. Especially in retrieved knee prostheses with anatomical design, the predominant types of wear on UHMWPE tibial components are delamination and pitting. These fatigue wear patterns of UHMWPE are believed to result from repeated plastic deformation owing to high contact stresses. In this study, the elasto-plastic contact analysis of the UHWMPE tibial insert, based on geometrical measurement for retrieved knee prosthesis, was performed using the finite element method (FEM) to investigate the plastic deformation behaviour in the UHMWPE tibial component. The results suggest that the maximum plastic strain below the surface is closely related to subsurface crack initiation and delamination of the retrieved UHMWPE tibial component. The worn surface whose macroscopic geometrical congruity had been improved due to wear after joint replacement showed lower contact stress at macroscopic level.

The relationship among three anatomic landmarks and the mechanical axis of the tibia was evaluated using magnetic resonance imaging in 30 normal knees in 15 healthy volunteers. These three relationships are calculated for flexion and extension. The angles between the tibial mechanical axis and a line perpendicular to the surgical epicondylar axis, a line perpendicular to the clinical epicondylar axis, and the anteroposterior axis were 3.9 degrees varus, 0.6 degrees varus, and 0.2 degrees valgus, respectively. The mechanical axis of the femur was 3.0 degrees varus relative to the mechanical axis of the tibia. The results of the current study suggest that the surgical epicondylar axis rather than the clinical epicondylar axis or the anteroposterior axis can maintain a more predictable orientation with respect to the mechanical axis of the tibia when moving from flexion into extension.

In total knee arthroplasty (TKA) following failed high tibial osteotomy, the mechanical axis does not intersect the center of the tibial component if the tibia has been resected perpendicular to the anatomical axis. Therefore, tibial resection referencing the predicted postoperative mechanical axis instead of the tibial shaft axis is advocated. To obtain the optimal tibial resection, characteristics of the tibial proximal deformity were measured radiographically and predicted postoperative lower limb alignment was calculated using full-length, weight-bearing, lower limb anteroposterior radiographs. Two finite element analysis models also were examined. The proximal tibia was resected perpendicular to the tibial shaft axis in model 1, and perpendicular to the predicted postoperative tibial mechanical axis in model 2. When the proximal tibia was resected perpendicular to the tibial shaft axis, the predicted lower limb mechanical axis was significantly shifted medially to the center of the tibial joint surface. The results of the finite element analysis reflected the medial shift of the lower limb mechanical axis in model 1, where stresses were increased in the medial tibial compartment. Tibial resection referencing the predicted postoperative tibial mechanical axis, instead of the tibial shaft axis, should be performed, especially in cases with a deformed tibia.

Simulation of the femoral cut for total knee arthroplasty was performed in 44 knees using three-dimensional computed tomography. The three-dimensional images were measured, and the shape of the femur was compared to the provided femoral prosthesis. The ratio between the medial-lateral and anteroposterior dimensions of the three-dimensional images did not always match the prosthesis. The widths of the medial condyles of the three-dimensional images tended to be larger than those of the prosthesis. The lengths of the lateral posterior condyles of the three-dimensional images tended to be shorter than those of the prosthesis. These results suggest that a new prosthesis should be designed to achieve a better anatomical fit.

Patellar tracking and femoral condylar geometry in deep knee flexion were evaluated using magnetic resonance imaging. The patellar tilting angle, patellar shift, and patellar anteroposterior translation from 0 degrees to 135 degrees flexion were measured. The depth of the femoral condylar articular surface and the curvature of the femoral condylar articular surface also were measured at 135 degrees flexion. The patella shifted laterally, tilted medially, and sank deeply into the intercondylar notch during deep knee flexion. The articular surface of the lateral condyle, existing deep within the intercondylar notch, began to curve steeply at a point farther from the center of the intercondylar notch than did the medial condyle. The geometry of the femoral condyle is adequate to fit the patellar geometry. Results of the current study suggest that the geometry of the lateral femoral condyle allows the patella to track smoothly with a larger patellofemoral contact area and less patellofemoral pressure during deep flexion.

In order to obtain high reproducibility and sensitivity in the evaluation of joint space in osteoarthritic knees, we used new equipment, and established an anteroposterior weight-bearing radiograph with both knees in semiflexion taking into account our investigation of the posterior slope angle. We compared our method with the Rosenberg method, and the coefficient of variation of our method showed higher reproducibility than that of the Rosenberg method. The most essential issue is whether the bilateral leg loading radiograph can show the same sensitivity as the homolateral leg loading radiograph. We compared standing radiographs of the knee in 26 degrees flexion with those of 26 degrees flexion in both knees. We assessed the widths of the narrowest points of the joint spaces in the medial and lateral compartments, the condylar plateau angle, and the femorotibial angle, in 47 knees in 24 patients. The data were analyzed statistically by single regression and each correlation was of statistical significance. Our method has better reproducibility and is more comfortable for the subject than conventional methods.

The purpose of this study is to clarify the clinical features of the knee affected by multiple epiphyseal dysplasia. Thirty-one cases of multiple epiphyseal dysplasia were reviewed. Of the patients, 11 were male and 20 were female. The average age at onset of symptoms was 22.5 years. The average age at initial visit to the authors' hospital was 28.9 years. Radiographic findings showed epiphyseal abnormality of the knee in all but two (93%) cases. Irregularity, segmentation of the epiphysis, widening of the joint space, and genu valgum deformity were the dominant findings before epiphyseal closure. After epiphyseal closure, the most characteristic finding was a shallow femoral trochlear groove, which was observed in 56.5% of the cases. Other findings in adult patients included early onset osteoarthritic change, genu valgum, depression of the lateral tibial plateau, and multiple free bodies. However, there still is a possibility that multiple epiphyseal dysplasia exists, even if the patient lacks a shallow femoral trochlear groove. If genu valgum or varum, free bodies, and premature osteoarthritis are observed, one should evaluate other joints, keeping a diagnosis of multiple epiphyseal dysplasia in mind. Patients with knees that have a femoral trochlear groove of normal or near normal shape do exist, and premature osteoarthritic changes may develop in such patients.

Anteroposterior knee laxity was evaluated in 14 patients (19 knees) who had posterior cruciate ligament retaining total knee arthroplasty using the Miller Galante I prosthesis. The followup ranged from 87 to 118 months (average, 105.9 months), and the measurements were done using the KT-2000 arthrometer. The mean anteroposterior displacement with the knees with Miller Galante I prostheses was 10.1 mm at 30 degrees flexion and 8.1 mm at 75 degrees flexion. In the 15 knees with Miller Galante I prostheses with flexion greater than 90 degrees, seven had less stability at 75 degrees than at 30 degrees flexion. These knees were considered to have a nonfunctional posterior cruciate ligament, and they had a worse Knee Society score (81.1) than did the other eight knees with Miller Galante I prostheses (89.9). There were four knees in which the flexion was less than 90 degrees. In this study, approximately half of the knees with posterior cruciate ligament retaining total knee arthroplasty did not have good anteroposterior stability in flexion an average of 9 years after surgery.

Changes in limb alignment after total knee arthroplasty were evaluated in 20 knees replaced with the Miller Galante knee system. The mean follow-up period was 87.4 months. Seventeen of the 20 knees were in the varus position on the initial postoperative radiographs, but the alignment significantly changed to become even more aligned toward varus during the follow-up period. The thickness of the ultra-high-molecular-weight polyethylene (UHMWPE) also decreased significantly in the medial femorotibial joint. The wear of the UHMWPE possibly changed the alignment, and the postoperative alignment had a positive correlation with the wear rate. The components should be implanted so that the mechanical axis intersects the center of the components to prevent worsening of alignment as well as to minimize any such wear.

Objective: To assess the usefulness of a new axial radiographic technique in knees with patellofemoral arthritis (PF-OA)., Design and Patients: After a marking wire had been attached to the skin on the tibial tuberosity so that the wire matched the width of the patellar tendon, an axial radiograph was taken at 30 degrees of flexion in 16 normal knees and 14 PF-OA knees in which computed tomographic analysis had revealed a laterally positioned tibial tuberosity at 30 degrees of flexion. The distance of the marking wire from the lateral condyle and from the patellar groove was compared between the two groups., Results: The marking wire was located significantly laterally in PF-OA knees compared with normal knees., Conclusion: An axial radiograph with a marking wire on the tibial tuberosity is useful for assessing the position of the tibial tuberosity in PF-OA knees.

To evaluate whether or not the combination of sagittal and coronal spin-echo (SE) T1 and sagittal gradient echo (GRE) T2* weighted imaging is adequate to diagnose internal derangement of the knee, a study was done in 300 knees of 293 consecutive patients who underwent both MR imaging of the knee and arthroscopy. In prospective analysis, the MR imaging had an accuracy, sensitivity and specificity of more than 90% in the evaluation of medial meniscus (MM) and of 97% in the evaluation of anterior cruciate ligament (ACL). In the evaluation of lateral meniscus (LM), accuracy was 83%, sensitivity 76%, and specificity 88%. In a review of false-negative MR diagnoses (n = 42) of menisci, the most significant lesion was not in the meniscus but in the ACL in 32 knees (76%). Conservative treatment was selected in 25 knees (60%), rather than partial meniscectomy (n = 13) or meniscoresis (n = 4). There were 21 false-positive MR diagnoses of menisci even in retrospective analysis, and 17 of them (81%) had findings of tear in the posterior zone, which is known to be a difficult area for arthroscopic examination. The results were similar to those of other authors' with SE T1 and/or proton density and T2 weighted images. The combination of sagittal and coronal SE T1 and sagittal GRE T2* weighted images could be sufficient as a routine MR protocol for diagnosing the internal derangement of the knee.

Appropriate tension of the posterior cruciate ligament, which often is tight in deep flexion, is difficult to achieve after posterior cruciate ligament retaining total knee arthroplasty. Kinematics and maximum flexion after partial release of the posterior cruciate ligament were evaluated in this study. A partial release improved the maximum flexion angle and maintained anteroposterior stability without causing undesirable changes in kinematics, whereas full resection of the posterior cruciate ligament caused unfavorable anteroposterior instability. Partial posterior cruciate ligament release eliminated excessive rollback movement caused by a tight posterior cruciate ligament and also shifted the point of articular surface contact anteriorly. These results indicate that partial release of the posterior cruciate ligament may improve knee function in patients with a tight posterior cruciate ligament after total knee arthroplasty.

Femoral condyle geometry was evaluated in 30 normal and 30 varus knees using magnetic resonance imaging. In the sagittal view, the distal part of the medial condyle was deformed in the varus knees, but there was no significant difference in the posterior part of the medial condyle between the normal and varus knees. In the transverse view, the transepicondylar axis (a reliable rotational landmark) showed approximately 6 degrees external rotation relative to the posterior condylar axis in the varus knees and the normal knees. The results of this study suggest that no hypoplasia exists at the posterior part of the medial condyle in varus knees. In addition, the posterior condylar axis may be a reliable rotational landmark but is in relative internal rotation in varus knees when total knee arthroplasty is performed.

Objective: To assess the mechanisms contributing to the induction of patellofemoral arthritis (PF-OA)., Design and Patients: A computed tomography scan was taken at three levels of the lower extremity in full extension and at 30 degrees of flexion. The cuts were superimposed and 12 parameters were compared in 17 PF-OA knees and 27 normal knees to assess the rotation angle of the tibial tubercle., Results: Although the tibial tubercle was in almost the same position in full extension in the normal and PF-OA knees, it was positioned significantly laterally at 30 degrees of flexion in PF-OA knees. Also the articular surface of the lateral femoral condyle was significantly narrower or steeper in PF-OA knees., Conclusion: Anatomic variations and mechanical abnormalities were identified in the PF-OA knees.

The etiology of spontaneous hemarthrosis of the knee in elderly patients with osteoarthritis is still unknown. This report discusses six cases, one that was managed with conservative measures and five in which arthroscopic evaluation and treatment were performed. These patients were elderly individuals whose chief complaint was knee pain and swelling about the joint, without an obvious history of trauma. Each of the five operative patients underwent arthroscopic surgery after repeated hemorrhage into the joint that did not respond to serial aspirations of large amounts of blood and immobilization, using a knee immobilizer. The roentgenograms of each involved knee demonstrated lateral compartment osteoarthritis. Arthroscopic examination revealed a degenerative flap tear of the posterior horn of the lateral meniscus in the five operative cases. These patients underwent arthroscopic resection of the injured lateral meniscus and did not experience a recurrence of the hemarthrosis. The origin of the bleeding was most likely from the peripheral arteries of the posterior horn of the lateral meniscus.

The use of standing radiographs to determine correction angles for high tibial osteotomy is not appropriate because the relative angle of the articular surfaces (condylar-plateau angle) in the weight-bearing knee changes after the osteotomy. This may give unpredictable results postoperatively. We found that the condylar-plateau angle in postoperative standing films is very similar to that seen in non-weight-bearing supine views, and suggest that these latter radiographs be used for pre-operative planning. We describe our early results, using a special osteotomy jig, in 140 knees.
