Knee collateral ligaments play a vital role in providing frontal-plane stability in post-total knee arthroplasty (TKA) knees. Finite element models can utilize computationally efficient one-dimensional springs or more physiologically accurate three-dimensional continuum elements like the Holzapfel-Gasser-Ogden (HGO) formulation. However, there is limited literature defining subject-specific mechanical properties, particularly for the HGO model. In this study, we propose a co-simulation framework to obtain subject-specific material parameters for an HGO-based finite element ligament model integrated into a rigid-body model of the post-TKA knee. Our approach achieves comparable accuracy to spring formulations while significantly reducing coefficient calibration time and demonstrating improved correlation with reference knee kinematics and ligament strains throughout the tested loading range.

Objectives: The idea to aim for an "individualized" alignment, whereby the constitutional alignment is restored, has gained much interest among knee surgeons. This requires insight into the prediseased, natural alignment of our patients' knees. The aim of this study is (1) to determine how the hip-knee-ankle (HKA) angle is influenced during the arthritic process and (2) to investigate the correlation between joint line changes and the progression of osteoarthritis (OA). It is our hypothesis that the most pronounced coronal parameter changes appear at the proximal tibia and at the joint line., Methods: One hundred sequential full-length X-rays with a minimum follow-up of 1 year were retrospectively reviewed from a radiographic joint database. Patients had to be at least 50 years of age needed to have an HKA angle of more than 1.3° varus to be included. Patients with ipsilateral total hip arthroplasty, femoral or tibial fracture, osteotomy, or ligamentous repair were excluded. Fifteen alignment parameters were investigated on the sequential full-length X-rays. Moreover, the relationship between the alignment parameters and the Kellgren-Lawrence grade (KL grade) was determined by using linear mixed models., Results: A progressive KL grade is associated with an increase of the HKA (p < 0.001). Mostly, HKA differs due to decrease of the medial tibial plateau (MPTA) angle (0.93°) and an increase of the joint line angle (JLCA) (0.86°). The mLDFA demonstrated the most pronounced changes in the beginning of OA (KL grade 1-2) (p = 0.049). In particular, the MPTA becomes considerably smaller (p = 0.004) in the later stage of OA (KL grade 3). Also, a progressive increase of the JLCA (p < 0.001) is observed upwards of KL grade 3., Conclusion: By comparing consecutive full-length X-rays in the same patients, it is possible to define the coronal alignment changes during the arthritic process. The HKA angle increases according the arthritic progression, whereby the most pronounced changes appear at the proximal tibia (MPTA) and at the joint line (JLCA).The alignment changes in varus OA knees can be divided in three stadia: (1) erosion of the distal medial femoral condyle, (2) erosion of the medial tibial plateau, and (3) a progressive increase of the joint line angle., Level of Evidence: Therapeutic Study, Level III., (Copyright © 2023 The Author(s). Aims: Higher osteoblastic bone activity is expected in aseptic loosening and painful unicompartmental knee arthroplasty (UKA). However, insights into normal bone activity patterns after medial UKAs are lacking. The aim of this study was to identify the evolution in bone activity pattern in well-functioning medial mobile-bearing UKAs., Methods: In total, 34 patients (13 female, 21 male; mean age 62 years (41 to 79); BMI 29.7 kg/m 2 (23.6 to 42.1)) with 38 medial Oxford partial UKAs (20 left, 18 right; 19 cementless, 14 cemented, and five hybrid) were prospectively followed with sequential 99mTc-hydroxymethane diphosphonate single photon emission CT (SPECT)/CT preoperatively, and at one and two years postoperatively. Changes in mean osteoblastic activity were investigated using a tracer localization scheme with volumes of interest (VOIs), reported by normalized mean tracer values. A SPECT/CT registration platform additionally explored cortical tracer evolution in zones of interest identified by previous experimental research., Results: Significant reduction of tracer activity from the preoperative situation was found in femoral and anteromedial tibial VOIs adjacent to the UKA components. Temporarily increased osteoblastic bone activity was observed in VOIs comprising the UKA keel structure at one year postoperatively compared to the preoperative activity. Persistent higher tracer uptake was found in the posterior tibial cortex at final follow-up. Multivariate analysis showed no statistical difference in osteoblastic bone activity underneath cemented or cementless components., Conclusion: Well-functioning medial mobile-bearing UKAs showed distinct changes in patterns of normalized bone tracer activity in the different VOIs adjacent to the prosthetic components, regardless of their type of fixation. Compared to the preoperative situation, persistent high bone activity was found underneath the keel and the posterior tibial cortex at final follow-up, with significant reduced activity only being identified in femoral and anteromedial tibial VOIs. Skin-attached inertial sensors are increasingly used for kinematic analysis. However, their ability to measure outside-lab can only be exploited after correctly aligning the sensor axes with the underlying anatomical axes. Emerging model-based inertial-sensor-to-bone alignment methods relate inertial measurements with a model of the joint to overcome calibration movements and sensor placement assumptions. It is unclear how good such alignment methods can identify the anatomical axes. Any misalignment results in kinematic cross-talk errors, which makes model validation and the interpretation of the resulting kinematics measurements challenging. This study provides an anatomically correct ground-truth reference dataset from dynamic motions on a cadaver. In contrast with existing references, this enables a true model evaluation that overcomes influences from soft-tissue artifacts, orientation and manual palpation errors. This dataset comprises extensive dynamic movements that are recorded with multimodal measurements including trajectories of optical and virtual (via computed tomography) anatomical markers, reference kinematics, inertial measurements, transformation matrices and visualization tools. Aims: Altered alignment and biomechanics are thought to contribute to the progression of osteoarthritis (OA) in the native compartments after medial unicompartmental knee arthroplasty (UKA). The aim of this study was to evaluate the bone activity and remodelling in the lateral tibiofemoral and patellofemoral compartment after medial mobile-bearing UKA., Patients and Methods: In total, 24 patients (nine female, 15 male) with 25 medial Oxford UKAs (13 left, 12 right) were prospectively followed with sequential 99mTc-hydroxymethane diphosphonate single photon emission CT (SPECT)/CT preoperatively and at one and two years postoperatively, along with standard radiographs and clinical outcome scores. The mean patient age was 62 years (40 to 78) and the mean body mass index (BMI) was 29.7 kg/m2 (23.6 to 42.2). Mean osteoblastic activity was evaluated using a tracer localization scheme with volumes of interest (VOIs). Normalized mean tracer values were calculated as the ratio between the mean tracer activity in a VOI and background activity in the femoral diaphysis., Results: Significant reduction of normalized tracer activity was observed one year postoperatively in tibial and femoral VOIs adjacent to the joint line in the lateral compartment. Patellar VOIs and remaining femoral VOIs demonstrated a significant, diminished normalized tracer activity at final follow-up., Conclusion: The osteoblastic bone activity in the native compartments decreased significantly after treatment of medial end-stage OA with a UKA, implying reduced stress to the subchondral bone in the retained compartments after a UKA. Knee joint laxity or instability is a common problem that may have detrimental consequences for patients. Unfortunately, assessment of knee joint laxity is limited by current methodologies resulting in suboptimal diagnostics and treatment. This paper presents a novel method for accurately measuring non-invasive knee joint laxity in four degrees-of-freedom (DOF). An arthrometer, combining a parallel manipulator and a six-axis force/moment sensor, was developed to be used in combination with a low-dose biplanar x-ray system and 3D image data to reconstruct tibiofemoral position and orientation of laxity measurements. As proof-of-concept, four cadaveric knees were tested in the device. Each cadaveric knee was mounted in the device at approximately 30° of flexion and twelve monoplanar anteroposterior, mediolateral and internal/external load cases were applied. Additionally, four biplanar load cases were applied, consisting of different combinations of anteroposterior and internal/external loads. The arthrometer was limited to four DOF to address the specific measurements. For validation purposes, the pose reconstructions of tibia and femur were compared with pose reconstructions of bone pin marker frames mounted on each bone. The measurements from the arthrometer in terms of translation and rotations displayed comparable values to what have previously been presented in the literature. Furthermore, the measurements revealed coupled motions in multiple planes, demonstrating the importance of multi DOF laxity measurements. The validation displayed an average mean difference for translations of 0.08 mm and an average limit of agreement between -1.64 mm and 1.80 mm. The average mean difference for rotations was 0.10° and the limit of agreement was between -0.85° and 1.05°. Purpose: This study was set up to identify the native trochlear geometry and define its relationship with the rotational landmarks of the distal femur., Methods: The rotational landmarks of the distal femur were analysed on CT-scans of 281 patients with end-stage knee osteoarthritis., Results: The anterior trochlear line (ATL) was on average 4.3° (SD 3.3°) internally rotated relative to the surgical transepicondylar axis (sTEA). The ATL was on average 2.1° (SD 3.0°) internally rotated relative to the posterior condylar line (PCL). The relationship between the ATL and the sTEA was statistically different in the different coronal alignment groups (p = 0.004): 3.9° (SD 3.0°) in varus knees, 4.0° (SD 2.9°) in neutral knees and 5.4° (SD 3.8°) in valgus knees. The lateralisation of the trochlea, represented by the distance between the perpendicular to PCL and the perpendicular to the posterior parallel line to the sTEA, was on average 2.2 mm (SD 1.8 mm)., Conclusion: The ATL was on average 4.3° (SD 3.3°) internally rotated relative to the sTEA and 2.1° (SD 3.0°) internally rotated relative to the PCL. The ATL is more externally orientated in varus knees and more internally rotated in valgus knees. The trochlear groove is lateralised by only 2.2 mm when the femoral component is externally rotated., Level of Evidence: III.

Background: In a typical osteoarthritic knee with varus deformity, distal femoral resection based off the worn medial femoral condyle may result in an elevated joint line. In a setting of fixed flexion contracture, the surgeon may choose to resect additional distal femur to obtain extension, thus purposefully raising the joint line. However, the biomechanical effect of raising the joint line is not well recognized., Questions/purposes: (1) What is the effect of the level of the medial joint line (restored versus raised) on coronal plane stability of a TKA? (2) Does coronal alignment technique (mechanical axis versus kinematic technique) affect coronal plane stability of the knee? (3) Can the effect of medial joint-line elevation on coronal plane laxity be predicted by an analytical model?, Methods: A TKA prosthesis was implanted in 10 fresh frozen nonarthritic cadaveric knees with restoration of the medial joint line at its original level (TKA0). Coronal plane stability was measured at 0°, 30°, 60°, 90°, and 120° flexion using a navigation system while applying an instrumented 9.8-Nm varus and valgus force moment. The joint line then was raised in two steps by recutting the distal and posterior femur by an extra 2 mm (TKA2) and 4 mm (TKA4), downsizing the femoral component and, respectively, adding a 2- and a 4-mm thicker insert. This was done with meticulous protection of the ligaments to avoid damage. Second, a simplified two-dimensional analytical model of the superficial medial collateral ligament (MCL) length based on a single flexion-extension axis was developed. The effect of raising the joint line on the length of the superficial MCL was simulated., Results: Despite that at 0° (2.2° ± 1.5° versus 2.3° ± 1.1° versus 2.5° ± 1.1°; p = 0.85) and 90° (7.5° ± 1.9° versus 9.0° ± 3.1° versus 9.0° ± 3.5°; p = 0.66), there was no difference in coronal plane laxity between the TKA0, TKA2, and TKA4 positions, increased laxity at 30° (4.8° ± 1.9° versus 7.9° ± 2.3° versus 10.2° ± 2.0°; p < 0.001) and 60° (5.7° ± 2.7° versus 8.8° ± 2.9° versus 11.3° ± 2.9°; p < 0.001) was observed when the medial joint line was raised 2 and 4 mm. At 30°, this corresponds to an average increase of 64% (3.1°; p < 0.01) in mid-flexion laxity with a 2-mm raised joint line and a 111% (5.4°; p < 0.01) increase with a 4-mm raised joint line compared with the 9-mm baseline resection. No differences in coronal alignment were found between the knees implanted with kinematic alignment versus mechanical alignment at any flexion angle. The analytical model was consistent with the cadaveric findings and showed lengthening of the superficial MCL in mid-flexion., Conclusions: Despite a well-balanced knee in full extension and at 90° flexion, increased mid-flexion laxity in the coronal plane was evident in the specimens where the joint line was raised., Clinical Relevance: When recutting the distal and posterior femur and downsizing the femoral component, surgeons should be aware that this action might increase the laxity in mid-flexion, even if the knee is stable at 0° and 90°.

Purpose: Balancing mobile-bearing (MB) unicondylar knee arthroplasty (UKA) is challenging. If performed improperly, potential complications include pain, implant loosening, or progression of osteoarthritis in the preserved compartment. The purpose of this study was to document effects of improper balancing on knee kinematics and joint contact stress. It was hypothesized that over-stuffing would lead to more valgus and higher lateral contact force., Methods: Seven fresh-frozen cadaver legs were mounted in a kinematic rig that applied three motion patterns to the specimens: passive flexion-extension, open chain extension, and squatting. During testing, an infrared camera system recorded the trajectories of markers rigidly attached to femur and tibia, while a pressure sensor measured contact pressure in the lateral compartment. Prior computer tomography scans allowed identification of coordinate frames of the bones and calculations of anatomical rotations and translations. Collateral ligament strains were calculated, and quadriceps forces recorded. Following testing on the native knee, a medial MB UKA was implanted in each specimen and all motion trials were repeated. Three inlay thicknesses were tested to simulate optimal balancing as well as under- (1 mm thinner) and over-stuffing (1 mm thicker) of the medial compartment relative to the optimal thickness., Results: Under-stuffing of the medial compartment leads to kinematics closest to the native knee. Subjectively balanced and over-stuffed MB UKA knees were in more valgus. Lateral peak contact stress was higher from mid- to deep flexion following UKA in all three tested states; however, these results were not significant. Peak strain in the superficial medial collateral ligament (sMCL) was significantly higher in MB UKA, regardless of the inlay thickness mainly in mid-flexion. Inlay thickness had no significant impact on measured quadriceps force during squatting., Conclusion: The results underline the importance of optimal balancing. Over-stuffing should be avoided as it results in the largest kinematic changes relative to the native condition and induces higher strains in the sMCL. Based on the kinematic findings, it is advisable to use thinner inlays, as long as this is not compromising stability or risking inlay luxation.

Introduction: Fixed-bearing unicompartmental knee arthroplasty (UKA) closely replicates native knee kinematics. As few studies have assessed kinematics following mobile-bearing (MB) UKA, the current study aimed to investigate whether MB UKA preserves natural knee kinematics., Materials and Methods: Seven fresh-frozen full-leg cadaver specimens were prepared and mounted in a kinematic rig that allowed all degrees of freedom at the knee. Three motion patterns, passive flexion-extension (0°-110° flexion), open-chain extension (5°-70° flexion) and squatting (30°-100° flexion), were performed pre- and post-implantation of a medial MB UKA and compared in terms of rotational and translational knee joint kinematics in the different anatomical planes, respectively., Results: In terms of frontal plane rotational kinematics, MB UKA specimens were in a more valgus orientation for all motion patterns. In the axial plane, internal rotation of the tibia before and after UKA was consistent, regardless of motion task, with no significant differences. In terms of frontal plane, i.e., inferior-superior, translations, the FMCC was significantly higher in UKA knees in all flexion angles and motor tasks, except in early flexion during passive motion. In terms of axial plane, i.e., anteroposterior (AP), translations, during open-chain activities, the femoral medial condyle center (FMCC) tended to be more posterior following UKA relative to the native knee in mid-flexion and above. AP excursions of the FMCC were small in all tested motions, however. There was substantial AP translation of the femoral lateral condyle center during passive motion before and after UKA, which was significantly different for flexion angles > 38°., Conclusions: Our study data demonstrate that the kinematics of the unloaded knee following MB UKA closely resemble those of the native knee while relative medial overstuffing with UKA will result in the joint being more valgus. However, replacing the conforming and rigidly fixed medial meniscus with a mobile inlay may successfully prevent aberrant posterior translation of the medial femoral compartment during passive motion and squatting motion.

Objectives: The aim of this study was to provide information about the mechanical properties of grafts used for knee ligament reconstructions and to compare those results with the mechanical properties of native knee ligaments., Methods: Eleven cadaveric knees were dissected for the semitendinosus, gracilis, iliotibial band (ITB), quadriceps and patellar tendon. Uniaxial testing to failure was performed using a standardized method and mechanical properties (elastic modulus, ultimate stress, ultimate strain, strain energy density) were determined., Results: The elastic modulus of the gracilis tendon (1458±476MPa) (P<0.001) and the semitendinosus tendon (1036±312MPa) (P<0.05) was significantly higher than the ITB (610±171MPa), quadriceps tendon (568±194MPa), and patellar tendon (417±107MPa). In addition, the ultimate stress of the hamstring tendons (gracilis 155.0±30.7MPa and semitendinosus 120.1±30.0MPa) was significantly higher (P<0.001, respectively P<0.05), relative to the ITB (75.0±11.8MPa), quadriceps tendon (81.0±27.6MPa), and patellar tendon (76.2±25.1MPa). A significant difference (P<0.05) could be noticed between the ultimate strain of the patellar tendon (24.6±5.9%) and the hamstrings (gracilis 14.5±3.1% and semitendinosus 17.0±4.0%). No significant difference in strain energy density between the grafts was observed., Conclusions: Material properties of common grafts used for knee ligament reconstructions often differ significantly from the original knee ligament which the graft is supposed to emulate., (Copyright © 2017 Elsevier B.V. Background: The aim of this study was to provide a characterization of the tensile properties of the medial collateral ligament (MCL), lateral collateral ligament (LCL), anterolateral ligament (ALL) and medial patellofemoral ligament (MPFL). Our hypothesis was that extra-articular knee ligaments are heterogeneous in nature and possess distinct material properties., Methods: MCL (n=12), LCL (n=11), MPFL (n=12) and ALL (n=19) samples from fresh frozen human cadaveric knees were subjected to uniaxial tensile testing to failure and analyzed for their material properties. The elastic modulus (slope of the linear portion of the stress/strain curve), ultimate stress (stress at failure), ultimate strain (strain at failure) and strain energy density (area under the stress/strain curve) were calculated., Results: The MCL had the highest elastic modulus (441.8±117.2MPa) and was significantly greater than the MPFL (294.6±190.4MPa) and LCL (289.0±159.7MPa) (P<0.05) as well as the ALL (173.7±91.8MPa) (P<0.001). The ultimate stress was significantly higher (P<0.05) for the LCL (83.6±38.1MPa) and MCL (72.4±20.7MPa), relative to the MPFL (49.1±31.0MPa) and ALL (46.4±20.1MPa). The ultimate strain of the LCL (41.0±9.9%) and ALL (37.8±7.9%) were significantly higher (P<0.05) compared to the MCL (22.9±2.5%) and MPFL (22.2±5.6%). The strain energy density of the LCL (15.2±6.4MPa) was significantly greater (P<0.05) than all other ligaments (ALL 7.8±3.1MPa, MCL 7.5±2.9MPa and MPFL 5.0±2.9MPa)., Conclusions: Extra-articular knee ligaments are a heterogeneous group with respect to material characteristics. Introduction: Medial knee instability is a key clinical parameter for assessing ligament injury and arthroplasty success, but current methods for measuring stability are typically either qualitative or involve ionizing radiation. The purpose of this study was to perform a preliminary analysis of whether ultrasound (US) could be used as an alternate approach for quantifying medial instability by comparing an US method with an approach mimicking the current gold standard fluoroscopy method., Materials and Methods: US data from the medial knee were collected, while cadaveric lower limbs (n = 8) were loaded in valgus (10 Nm). During post-processing, the US gap width was measured by identifying the medial edges of the femur and tibia and computing the gap width between these points. For comparison, mimicked fluoroscopy (mFluoro) images were created from specimen-specific bone models, developed from segmented CT scans, and from kinematic data collected during testing. Then, gap width was measured in the mFluoro images based on two different published approaches with gap width measured either at the most medial or at the most distal aspect of the femur., Results: Gap width increased significantly with loading (p < 0.001), and there were no significant differences between the US method (unloaded: 8.7 ± 2.4 mm, loaded: 10.7 ± 2.2 mm) and the mFluoro method that measured gap width at the medial femur. In terms of the change in gap width with load, no correlation with the change in abduction angle was observed, with no correlation between the various methods. Inter-rater reliability for the US method was high (0.899-0.952)., Conclusions: Ultrasound shows promise as a suitable alternative for quantifying medial instability without radiation exposure. However, the outstanding limitations of existing approaches and lack of true ground-truth data require that further validation work is necessary to better understand the clinical viability of an US approach for measuring medial knee gap width.

Background: The recently reintroduced bicruciate retaining Total Knee Arthroplasty (BCR TKA) is an effort to reproduce kinematics closer to the native knee. However, there is no data on appropriate balancing with this implant. Balancing is crucial and challenging as medial and lateral polyethylene (PE) inlays are modular, which allows for placement of different thicknesses in the medial and lateral compartments. This study aimed at providing a detailed kinematic view on balancing BCR TKA., Methods: Seven fresh frozen cadaver legs were mounted in a kinematic rig that applied squatting under application of physiologic quadriceps and hamstring forces. Additionally, specimen laxity was assessed using Lachman tests and varus/valgus stress tests. Following testing on the native knee, a BCR TKA was implanted in each specimen and all trials were repeated. Using one millimeter increments, five inlay thicknesses were tested to simulate optimal balancing, symmetric under-, and overstuffing, valgus constellation, and varus constellation., Results: Overall, knee kinematics following BCR TKA seem to be very close to the native knee. The changes as introduced to tibiofemoral kinematics through over- or understuffing the polyethylene inserts are affecting the system only to a minor degree and generally lack statistical significance. Reproduction of the tibial varus via PE-Inlays did not lead to kinematics much closer to the native knee., Conclusions: The changes introduced to tibiofemoral kinematics by removal of the conforming meniscus and cartilage and replacement with a flat PE insert and femoral component are of more impact than different inlay sizes and their combinations for a BCR TKA., (Copyright © 2017 Elsevier B.V. Purpose: To characterize the tensile and histologic properties of the anterolateral ligament (ALL), inferior glenohumeral ligament (IGHL), and knee capsule., Methods: Standardized samples of the ALL (n = 19), anterolateral knee capsule (n = 15), and IGHL (n = 13) were isolated from fresh-frozen human cadavers for uniaxial tensile testing to failure. An additional 6 samples of the ALL, capsule, and IGHL were procured for histologic analysis and determination of elastin content., Results: All investigated mechanical properties were significantly greater for both the ALL and IGHL when compared with capsular tissue. In contrast, no significant differences between the ALL and IGHL were found for any property. The elastic modulus of ALL and IGHL samples was 174 ± 92 MPa and 139 ± 60 MPa, respectively, compared with 62 ± 30 MPa for the capsule (P = .001). Ultimate stress was significantly lower (P < .001) for the capsule, at 13.4 ± 7.7 MPa, relative to the ALL and IGHL, at 46.4 ± 20.1 MPa and 38.7 ± 16.3 MPa, respectively. The ultimate strain at failure was 37.8% ± 7.9% for the ALL and 39.5% ± 9.4% for the IGHL; this was significantly greater (P = .041 and P = .02, respectively) for both relative to the capsule, at 32.6% ± 8.4%. The strain energy density was 7.8 ± 3.1 MPa for the ALL, 2.1 ± 1.3 MPa for the capsule, and 7.1 ± 3.1 MPa for the IGHL (P < .001). The ALL and IGHL consisted of collagen bundles aligned in a parallel manner, containing elastin bundles, which was in contrast to the random collagen architecture noted in capsule samples., Conclusions: The ALL has similar tensile and histologic properties to the IGHL. The tensile properties of the ALL are significantly greater than those observed in the knee capsule. CLINICAL RELEVANCE: The ALL is not just a thickening of capsular tissue and should be considered a distinct ligamentous structure comparable to the IGHL in the shoulder. Purpose: The purpose of this study was to investigate and describe buckling of the patellar tendon., Methods: Healthy young adults (28±4years, 10F/10M) underwent passive knee flexion/extension during the simultaneous measurement of knee angle and collection of cine ultrasound from the patellar tendon. Patellar tendon buckling was observed visually in ultrasound images, and the corresponding knee angle at which evidence of buckling disappeared was identified., Results: All subjects showed evidence of distal buckling which occurred on average at 23±8° flexion. Proximal buckling was only observed in fourteen subjects (10F/4M) at an average of 15±8°. Buckling patterns varied between subjects, but with high within-subject consistency. Buckling magnitude increased with age (p=0.03) and decreased with more weekly exercise (p=0.02)., Discussion: The patellar tendon exhibited significant buckling in knee extension suggesting that buckling is a component of healthy knee function. Like tendon crimp, buckling may serve as a protective mechanism, allowing the tissue to unwrinkle prior to undergoing pure strain. The links between increased buckling magnitude and both age and reduced activity suggest that excessive buckling may be maladaptive, though future work is necessary to elucidate this relationship. Buckling is relevant to consider when estimating tendon length, as buckling can lead to significant underestimation of resting length and thus overestimation of strain., Conclusion: This study demonstrates the complexity of tendon behavior even in healthy adults undergoing passive motion, suggesting that buckling may be relevant to an improved understanding of tendon health and pathology., (Copyright © 2016 Elsevier B.V. Purpose: It is assumed that unicondylar knee arthroplasty (UKA) features kinematics close to the natural knee. Clinical studies have also shown functional benefits for UKA. There is to date only little biomechanical data to support or explain these findings. The purpose of this study was to investigate whether UKA is able to preserve natural knee kinematics or not., Methods: Six fresh frozen full leg cadaver specimens were prepared to be mounted in a kinematic rig with six degrees of freedom for the knee joint. Three motion patterns were applied before and after medial UKA: passive flexion-extension, open chain extension, and squatting. During the loaded motions, quadriceps and hamstrings muscle forces were applied. Infrared cameras continuously recorded the trajectories of marker frames rigidly attached to femur, tibia, and patella. Prior computer tomography allowed identification of coordinate frames of the bones and calculations of anatomical rotations and translations., Results: Native kinematics was reproduced after UKA in all the specimens. In the unloaded knee and during open chain extension, femoral rollback patterns after UKA were very close to those in the native knee. During squatting, the medial femoral condyle after UKA tended to be more posterior and superior with flexion and there was less tibial internal rotation. The tibia was found to be more in valgus after UKA during all motion patterns., Conclusion: As ligaments, lateral compartment and patellofemoral anatomy are preserved with UKA; the unloaded knee closely resembles native kinematics. The slight kinematic changes that were found under load are probably due to loss of the conforming medial meniscus and to the mismatch in geometry and stiffness introduced by UKA. These patterns resemble those found in knees with significant loss of function of the medial meniscus.

Purpose: Modular bicompartmental knee arthroplasty (BKA) for treatment of medio-patellofemoral osteoarthritis (OA) should allow for close to normal kinematics in comparison with unicondylar knee arthroplasty (UKA) and the native knee. Three motion patterns were applied with the native knee and after sequential implantation of medial UKA and patellofemoral joint replacement (PFJ): passive flexion-extension, open chain extension, and squatting. During the loaded motions, quadriceps and hamstrings muscle forces were applied. Infrared cameras continuously recorded the trajectories of marker frames rigidly attached to femur, tibia and patella. Prior computer tomography allowed identification of coordinate frames of the bones. Strains in the collateral ligaments were calculated from insertion site distances., Results: UKA led to a less adducted and internally rotated tibia and a more strained medial collateral ligament (MCL). Addition of a patellofemoral replacement led to a more posterior position of both femoral condyles, a more dorsally located tibiofemoral contact point and higher MCL strain with squatting., Conclusion: In comparison to UKA modular BKA leads to a more dorsal tibial contact point, a medial femoral condyle being located more posteriorly, and more MCL strain. The availability of detailed knee kinematic data during various activities can facilitate clinical studies of this joint. To describe in detail normal knee joint rotations in all three anatomical planes, 25 healthy subjects (aged 22-49 years) performed eleven motor tasks, including walking, step ascent and descent, each with and without sidestep or crossover turns, chair rise, mild and deep squats, and forward lunge. Kinematic data were obtained with a conventional lower-body gait analysis protocol over three trials per task. To assess the repeatability with standard indices, a representative subset of 10 subjects underwent three repetitions of the entire motion capture session. Extracted parameters with good repeatability included maximum and minimum axial rotation during turning, local extremes of the flexion curves during gait tasks, and stride times. These specific repeatable parameters can be used for task selection or power analysis when planning future clinical studies.

This paper compares geometry-based knee axes of rotation (transepicondylar axis and geometric center axis) and motion-based functional knee axes of rotation (fAoR). Two algorithms are evaluated to calculate fAoRs: Gamage and Lasenby's sphere fitting algorithm (GL) and Ehrig et al.'s axis transformation algorithm (SARA). Calculations are based on 3D motion data acquired during isokinetic dynamometry. AoRs are validated with the equivalent axis based on static MR-images. We quantified the difference in orientation between two knee axes of rotation as the angle between the projection of the axes in the transversal and frontal planes, and the difference in location as the distance between the intersection points of the axes with the sagittal plane. Maximum differences between fAoRs resulting from GL and SARA were 5.7° and 15.4mm, respectively. Maximum differences between fAoRs resulting from GL or SARA and the equivalent axis were 5.4°/11.5mm and 8.6°/12.8mm, respectively. Differences between geometry-based axes and EA are larger than differences between fAoR and EA both in orientation (maximum 10.6°).and location (maximum 20.8mm). Knee joint angle trajectories and the corresponding accelerations for the different knee axes of rotation were estimated using Kalman smoothing. For the joint angles, the maximum RMS difference with the MRI-based equivalent axis, which was used as a reference, was 3°. For the knee joint accelerations, the maximum RMS difference with the equivalent axis was 20°/s(2). Functional knee axes of rotation describe knee motion better than geometry-based axes.
