Objective:  The objective of this prospective study was to evaluate the use of ultrasonography in the diagnosis of medial coronoid process disease in unclear cases., Study Design:  Fifteen elbows (on thirteen dogs) for which radiography and computed tomography did not lead to a clear diagnosis of medial coronoid process disease were included. On each elbow, ultrasonography was performed with a high frequency linear transducer (12-18Hz). Then, arthroscopic examination of the joint was performed by a surgeon who was unaware of ultrasonographic findings to confirm medial coronoid process disease., Results:  At least one ultrasonographic lesion was detected in 13 out of 15 elbows. The main reported ultrasonographic lesions were joint effusion (10/15 elbows) and an abnormal shape of the medial coronoid process (irregular, ill-defined or fragmented) (9/15)., Conclusion:  Ultrasonography can be a helpful additional diagnostic tool to confirm medial coronoid process disease of the elbow joint before performing arthroscopy in unclear cases. Objective: To compare the attenuation of the medial coronoid process (MCP) in dogs with and without arthroscopically confirmed evidence of medial coronoid disease (MCD)., Animals: The database at our institution was searched for cases with thoracic limb lameness, diagnosed with MCD by arthroscopic examination that had CT as part of their investigation and compared with a control group of elbow joints from cadavers euthanized for reasons unrelated to MCD. A total of 84 elbow joints were included that met these criteria., Procedures: Following CT, a standardized measurement of the MCP was obtained from apex to base and the mean attenuation, SD, and total area were recorded. A comparative measurement was obtained from the proximal radial cortex at the level of the nutrient foramen. Elbow joint arthroscopy was carried out using standard portals, and the modified Outerbridge score was (MOS) used to score elbow joint cartilage. Descriptive and inferential statistics were carried out using MLwiN and R., Results: Attenuation of the MCP was reduced in dogs with MCD compared with those with no MCD (P < .002). No significant differences were observed in the attenuation between categories of severity (MOS). There was good inter- and intraobserver agreement between measurements (intraclass correlation coefficient = 0.89 and 0.95, respectively)., Clinical Relevance: MCP attenuation is reduced in dogs with MCD compared with dogs with no evidence of MCD. This finding may be a useful tool for early detection of MCD, but there is no relationship with arthroscopic lesion severity.

Background: Post-traumatic elbow arthrofibrosis (PEA) and its associated limitations to elbow range of motion (ROM) are a recognized consequence of trauma to the pediatric elbow. Closed manipulation under anesthesia (CMUA) of the elbow can be performed in pediatric patients as a nonoperative attempt to improve dysfunctional ROM. Minimal outcome data to support CMUA exist. The study evaluates the efficacy of CMUA for PEA in pediatric patients., Methods: Patients younger than 18 years who underwent CMUA (Current Procedural Terminology code 24300) for PEA between 2005 and 2015 at 3 institutions were included. A retrospective chart review was performed to collect demographic data and ROM premanipulation and at last follow-up. Paired 2-tailed t tests were used to compare pre- and postmanipulation elbow ROM., Results: Thirteen patients with a mean age of 12.2 ± 2.6 years (range 6.7-15.6 years) met the inclusion criteria. Median time to CMUA from initial surgery was 4.2 months (interquartile range [IQR] 3.6-8.4, range 1.4-19.7 months). Median follow-up time was 6 months with an IQR of 3.3-10.0 months. At last follow-up, there was significant improvement in elbow flexion of 22° ± 17° (P < .001) and extension of 29° ± 21° (P < .001). The average premanipulation motion arc of 60° ± 24° significantly increased to 110° ± 22° at final assessment (P < .001)., Conclusion: CMUA appears to be a valuable alternative and reliable procedure for improving PEA in pediatric patients who exhaust nonoperative interventions., (Copyright © 2019 Journal of Shoulder and Elbow Surgery Board of Trustees. Arthroscopic management of elbow stiffness offers several potential advantages over conventional open procedures. Projected increased joint visualization, decreased tissue disruption and faster recovery have yielded growing interest for this procedure. Elbow arthroscopy remains a challenging procedure and reported complication rate is higher than in other joints. Sound local anatomy knowledge is required to ensure a safe procedure. This article covers the use of radiographs and MR imaging for diagnosing elbow disorders. It concentrates on these techniques, but other imaging techniques including computed tomography, ultrasound, and nuclear medicine are discussed when clinically important. The article covers the use of imaging for specific symptoms including lateral, medial, anterior, posterior and generalized elbow pain, focal swelling, restriction of movement, locking, and neurologic symptoms. Radiographs have been the mainstay of initial imaging of the elbow, while ultrasound examination is often used for soft tissue disease. Synovial plicae are vestigial folds of synovium in a joint, most widely recognized in the knee and also in the elbow and hip joints. The most commonly shared theory on the origin of plicae is that they are the remnants of the membranes that divide the synovial cavitations during normal joint development. Synovial folds do not generally cause any symptoms. However, they can become inflamed, thickened, fibrosed, and also impinged in the joint, leading to mechanical symptoms and chondral damage. This article presents an overview of the embryological origin of plicae, their anatomy, pathologies, and appearances on imaging. Synovial plica is rarely diagnosed as cause of elbow pain. Impingemnt of posterolateral plicae in the radiocapitellar joint tends to be usually overlooked. The purpose of this study was to present outcomes of arthroscopic treatment in relatively large number of cases and propose reliable diagnostic test for posterolateral plicae of the radiocapitellar joint.From January 2000 to December 2010, 24 cases diagnosed with pathologic posterolateral radiocapitellar plica on arthroscopic finding were retrospectively reviewed. Magnetic resonance imaging (MRI) evaluation and preoperative physical examination were performed. The posterolateral radiocapitellar plica test newly proposed by the present study was also conducted. To measure postoperative clinical outcomes, the disabilities of the arm, shoulder, and hand (DASH) score and Mayo elbow performance score (MEPS) were employed. Minimum duration of follow up was 24 months.According to the preoperative MRI, pathologic radiocapitellar plica was identified in 17 cases (70.8%). Preoperatively, maximal tender point was present on the radiocapitellar joint line in 20 cases (83.3%) and mechanical symptoms were observed in 9 cases (37.5%). 6 cases (25%) demonstrated pain at terminal extension and limitation of extension. 20 (83.3%) cases tested positive for posterolateral radiocapitellar plica test. The sensitivity and specificity of the posterolateral radiocapitellar plica test were 83.3% and 87.5%, respectively. The accuracy value was 86.3%. Arthroscopic debridement of pathologic plica in the radiocapitellar joint demonstrated clinical improvements: DASH score was from 36.6 to 8.9 and MEPS was from 56.9 to 95.6 at the latest follow-up.Symptomatic impingement by the pathologic posterolateral plica of the radiocapitellar joint should be considered when posterolateral elbow pain which is refractory to conservative treatment, and other prevalent diseases are excluded. The posterolateral radiocapitellar plica test and radiocapitellar joint line tenderness could be recommended as reliable examination maneuvers to obtain accurate diagnosis. Arthroscopic debridement was an effective method for treating symptomatic plicae.

Background: Work-related musculoskeletal disorders at the elbow are a common health problem, which highly impacts workers' well-being and performance. Besides existing qualitative information, there is a clear lack of quantitative information of physical risk factors associated with specific disorders at the elbow (SDEs)., Objective: To provide evidence-based quantitative measures of physical risk factors associated with SDEs., Methods: Studies were searched from 2007 to 2017 in Medline, EMBASE, and Cochrane Work. The identified risk factors were grouped in main- and sub-categories of exposure using the Grading of Recommendations, Assessment, Development and Evaluation (GRADE) framework for rating evidence., Results: 133 different risk-factor specifications were identified in 10/524 articles and were grouped into 5 main- and 16 sub-categories of exposure. The risk factors were significantly associated with lateral epicondylitis, medial epicondylitis, or ulnar neuropathy. Significant risk factors such as wrist angular velocity (5°/s, with increasing prevalence ratio of 0.10%/(°/s), or forearm supination (≥45° and ≥5% of time combined with forceful lifting) were found., Conclusions: This review delivers a categorization of work-related physical risk-factor specifications for SDEs with a special focus on quantitative measures, ranked for evidence. These results may build the base for developing risk assessment methods and prospective preventive measures.

Purpose: The purpose of this study was to compare different grades of elbow stiffness and investigate demographic, injury, and treatment factors potentially associated with the development of severe elbow stiffness., Methods: We performed a retrospective study involving 169 patients with post-traumatic elbow stiffness between June 2014 and June 2016. Patient demographics, injury, and treatment details were reviewed. Patients were classified into three groups according to the elbow motion. Ordinal regression analyses were performed to examine the independent factors., Results: Patients were classified into: mild (49 patients), moderate (59 patients), and severe (61 patients) groups. Patients with severe stiffness had a significantly worse elbow functional performance. Univariate ordinal regression revealed that severe elbow stiffness was associated with high-energy injury (odds ratio [OR] 4.73), olecranon fracture (OR 1.92), fracture-dislocation (OR 2.28), and open fracture (OR 3.24). Multivariate regression showed that higher-energy injuries were associated with severe stiffness (OR 4.45, p = 0.003)., Conclusions: Elbow stiffness after fracture surgery often results in joint stiffness. Severe stiffness often resulted in more significant functional impairment. Our study suggested that high-energy injuries were associated with the development of severe elbow stiffness.

Introduction: Repeated haemarthrosis is widely accepted as the triggering cause of synovitis and haemophilic arthropathy. A first-line treatment of chronic synovitis is radiosynoviorthesis (RS). The aim of this study was to evaluate the RS effects on the progression of arthropathy and on a reduction in bleeding in patients with haemophilia., Methods: An observational-retrospective study was performed. Bleeding episodes in the 12 months following and in the 12 months preceding RS was compared. The arthropathy was clinically and radiologically analysed by age range, joint and subject, comparing those undergoing RS (Radiosynoviorthesis Group, RSG) against those not undergoing this treatment (Non-Radiosynoviorthesis Group, Non-RSG)., Results: One hundred and seventy-four RS were performed in 71 patients ( 90 Y in Knees and 186 Re in elbows/ankles/shoulder). RS resulted in significant reduction in bleeding (582 preintervention and 168 postintervention, P < .001). In general, the level of arthropathy measured clinically and radiologically was greater with age increase in both groups (RSG and Non-RSG), especially in the 25-40 age range. A significant increase (P < .05) in the progression of arthropathy was also observed, both globally by patient and specifically for each joint, in non-RSG and RSG group., Conclusion: RS is an effective method to reduce the number of haemarthrosis episodes in chronic synovitis. Moreover, RS can positively affect arthropathy by slowing down its progression. Purpose: Posteromedial olecranon impingement (PMOI) is the most common diagnosis in baseball players with throwing-induced elbow injuries. CT imaging manifestations of PMOI have seldom been previously reported. Our purpose is to investigate the additional value of CT imaging in the evaluation of PMOI., Materials and Methods: From July 2010 to December 2013, 47 baseball players with throwing-induced elbow pain received imaging studies (CT and/or MRI) of the elbow at our institution. After retrospectively reviewing the clinical records and imaging studies, 31 of the 47 players were diagnosed with PMOI by the criteria of posteromedial elbow pain with clinical consistency for PMOI and characteristic osteoarthrosis at the posteromedial ulnotrochlear articulation (posteromedial olecranon space, PMOS) detected by CT and/or MRI. The imaging modalities of these 31 players including radiography, CT, and MRI were reviewed by 2 experienced musculoskeletal radiologists., Results: The most common imaging manifestations of PMOI by CT or MRI include joint space narrowing, subchondral sclerosis, and osteophytes at the PMOS. CT was superior to MRI with a statistically significant (P<0.05) difference in detecting joint space narrowing, medial olecranon subluxation, as well as the number of loose bodies. In contrast, bone marrow edema and associated soft tissue injuries in PMOI are more readily observed on MRI., Conclusions: CT is superior in identifying some imaging features of PMOI. Objective: To evaluate magnetic resonance imaging (MRI) features of tubercular involvement of elbow joint., Methods: Fourteen patients with tubercular arthritis of elbow were evaluated. The following findings were recorded on MRI (n=12): bones involved; erosions, intraosseous abscesses, synovial thickening, and effusion; hypointensity within the thickened synovium on T2-weighted images; and periarticular soft tissue changes., Results: The common abnormalities on MRI were synovial thickening and bone erosions (n=12), soft tissue (n=9)/intraosseous (n=5) abscesses, and T2 hypointensity within the thickened synovium (n=5)., Conclusions: A diagnosis of elbow tuberculosis should be considered if there is a combination of findings involving bone and soft tissue., (Copyright © 2016 Elsevier Inc. The elbow is one of the most complex joints of the human body. Bony, ligamentous and muscular constraints ensure elbow stability. During recent years elbow arthroscopy has become more and more popular resulting from technical and surgical innovations. The diagnostic and therapeutic elbow arthroscopy following traumatic elbow dislocation is the best example. Functional outcomes after elbow dislocation significantly depend on sufficient evaluation of elbow stability, possible accompanying soft tissue injuries and on the initiation of adequate therapy. Elbow arthroscopy after traumatic elbow dislocation allows visualization of ligament ruptures and cartilaginous lesions, the resection of loose bodies and flushing of the hemarthrosis. Moreover, elbow stability can be tested directly. Concerning therapy, elbow arthroscopy represents an additional diagnostic tool and an aid for possible surgical procedures. In this article the basic requirements and special techniques for elbow arthroscopy are described. Using the examples of an elbow dislocation and arthrofibrosis, arthroscopical standard views, arthroscopical stability test and arthroscopical arthrolysis are explained.

An acutely painful and swollen isolated joint has a broad differential. We present the case of an elderly male with four-month progressive, painful swelling of the right elbow. After an initial workup for inflammatory arthropathies was nondiagnostic and a trial of conservative management failed to relieve his pain, a diagnostic arthroscopy was performed. Biopsy revealed metastatic colon adenocarcinoma isolated to the synovial tissue of the elbow. This case provides an example of the presentation and progression of synovial metastasis from a visceral cancer and can potentially guide physicians in the diagnosis and management of similar cases.

Imaging of the elbow joint places high demands on the quality of imaging due to the challenging anatomy and the sometimes subtle findings. For the diagnosis of periarticular soft tissues, ligamentous structures and in individual cases for fracture and tumor diagnosis, magnetic resonance imaging (MRI) is mostly groundbreaking and allows a reliable diagnosis in most cases. This review article discusses the complex imaging anatomy and anatomical variants of this joint and the most common osseous and ligamentous injuries of the elbow joint are presented. The typical MRI findings and indications are illustrated and discussed and possible pitfalls are pointed out. The various examination techniques and MRI sequences are also addressed.

The elbow is a complex joint that supports forearm movement and consequently is at risk for various injuries and disorders. Elbow disorders can range from chronic to acute problems, many of which can be debilitating. This article explains the functional anatomy of the elbow joint and discusses the most common elbow disorders and injuries. It also presents the most common diagnostic imaging choices, along with typical acquisition methods.

Objective: The purpose of this article is to review the embryologic development, anatomy, and histology of the synovial plicae of the elbow. The pathophysiologic features, clinical manifestations, imaging findings, and treatment of elbow synovial fold syndrome will also be reviewed., Conclusion: Elbow synovial fold syndrome is an uncommon entity that is often confused with lateral epicondylitis. Knowledge of the clinical and imaging diagnosis of this entity is essential for the appropriate management of patients.

Objective: The purpose of this video article is to aid examiners in clinical practice in identifying intraarticular and extraarticular pathologic changes in the elbow, including intraarticular structures, fluid in the joint, arthritis, synovitis, and loose bodies., Conclusion: Common pathologic findings can be identified with a standardized ultrasound technique based on landmarks, and intraarticular injection and aspiration, dynamic imaging, and ulnar stress maneuvers can be performed. Extraarticular pathologic findings are evaluated in an approach based on quadrants.

Background/aims: The elbow is among the most common joints that are aspirated and/or injected. An intra-articular approach should be a convenient and a safe procedure with minimal risk of complications. Several approaches to access the elbow joint have been outlined in the literature, but a comparative study is lacking. This study evaluates the technical feasibility of the lesser-performed posterior transtriceps approach with MR arthrography and compares it to the classic lateral radiocapitellar approach., Patients and Methods: Using fluoroscopy guidance, MR arthrographies of the elbow were performed in 51 consecutive patients from 2006 to 2011. A classical lateral radiocapitellar approach was performed in 29 and a posterior transtriceps approach in 22 elbows. Studies were retrospectively reviewed with special attention to the extent of extra-articular contrast extravasation. This was a level IV diagnostic study., Results: Contrast leakage occurred in 12 radiocapitellar approaches, which caused a diagnostic dilemma in one subject. There was only a minimal amount of contrast leakage in five subjects using the transtriceps approach and no diagnostic dilemmas occurred. Results show no significant differences between the approaches. No complications occurred in the posterior transtriceps group and all MR arthrographies were diagnostic., Conclusions: The posterior transtriceps approach is a technical feasible procedure, is easy to perform, and avoids a diagnostic dilemma in presumed injuries to the lateral collateral ligament complex. Our results show a tendency of even lesser amount of contrast leakage, further promoting a more widespread usage of the posterior transtriceps approach.

This case report is of a symptomatic posterior-lateral elbow plica in a child who presented with elbow locking. MR images demonstrated thickening of a posterior-lateral plica between the radius and capitellum of the elbow. Surgery confirmed a thickened and inflamed posterior-lateral plica, which was resected with subsequent improvement of the child's symptoms. This case shows the clinical importance of identifying thickening of posterior-lateral plicae in children and suggesting the diagnosis of plica syndrome, an entity that has not been previously reported in children in the radiologic literature.

Magnetic resonance (MR) arthrography has, to a great extent, replaced conventional and CT arthrography as the standard of care imaging tool for indications including medial elbow pain in the throwing athlete, chondral and osteochondral lesions in the elbow, and intra-articular bodies. There are still rare specific incidences where CT arthrography is indicated. Although the disease was first described in the hip, reports of chondrolysis in nearly all diarthrodial joints have since emerged with considerable variations in the literature.Despite speculation among clinicians and researchers about the implicit causal pathways and etiologic contributors associated with chondrolysis, definitive answers remain elusive.The term chondrolysis has been applied to varied levels of joint cartilage destruction from focal chondral defects to diffuse cartilage loss, revealing a lack of consistency in the application of diagnostic criteria to guide differential disease classification.Differentiating between the various potential etiologies associated with chondrolysis provides opportunities for the prevention of the disease.

To evaluate the inter-observer reliability of radiological assessment systems for haemophilic arthropathy, three senior orthopaedic surgeons with expertise in haemophilia independently evaluated a total of 527 joint radiographs of adult haemophilia patients, without any knowledge of the clinical data. This study was the largest study to evaluate the reliability of radiological assessment systems. As for the results, the Arnold-Hilgartner staging system showed moderate reliability (kappa value: κ = 0.44, P = 0.000), and the De Palma grading system and the Pettersson scoring system showed fair reliability (κ = 0.40, P = 0.000) and slight reliability (κ = 0.12, P = 0.000) respectively. As for the reliability of the eight findings in the Pettersson scoring system, three findings, which were 'narrowing of joint space' (κ = 0.70 P = 0.000), 'irregular subchondral surface' (κ = 0.58, P = 0.000) and 'erosion of joint margins' (κ = 0.56, P = 0.000), showed substantial or moderate reliability. Other findings showed fair or less reliability. The traditional radiological assessment systems showed poor inter-observer reliability. Both progressive scales showed higher reliability than the additive scale, and the three findings in the Pettersson scoring system showed good reliability. Subchondral bone surface geometry of the medial coronoid process was examined in 20 grossly normal elbow joints of adult German Shepherd dogs (GSDs) and compared with results obtained from 10 joints of adult GSDs and 12 joints of adult Rottweilers affected with bilateral fragmentation of the medial coronoid process (FMCP). Additionally, seven dogs (4 GSDs and 3 Rottweilers) with unilateral FMCP were investigated. The subchondral contour of the ulnar trochlear notch was digitised to obtain the 3D coordinates of its surface points. Geometry was investigated in two defined section planes along the longitudinal and transverse axes of the medial coronoid process. The coordinates of the surface points in the section planes were normalised along one axis and geometry was estimated by the position of the surface points along its third coordinate and by calculation of the angle of inclination of the medial coronoid process. Subchondral bone surface geometry was not related to the type of fragmentation (i.e. single sagittal versus multiple sagittal or transverse fragmentation). No differences were noted when comparing joints of dogs affected unilaterally with FMCP and their contralateral normal joints. In addition, no differences in geometry were seen between GSDs affected with fragmentation and normal GSDs. The most evident differences were found between Rottweilers affected with FMCP and normal GSDs as well as FMCP affected GSDs. The results suggest that subchondral bone geometry (geometric incongruity) plays no role in the development of FMCP. Lateral pain of the elbow is frequently reported. Impingement of the humeroradial fold or a synovial fringe is a seldom evoked cause, sometimes ignored. Nonetheless, this etiology should be remembered in young adults performing sport activities and in heavy workers. Pain can be acute and inflammatory. Elective pain is usually elicited by the pressure on the humeroradial joint space, beneath the lateral epicondyle. Snapping, locking and restricted mobility can be observed. MRI-arthrogram is the best radiological examination to perform. Symptoms can be improved by conservative treatment but may warrant an arthroscopical resection.

Objective: The purpose of our study was to evaluate the effectiveness of conventional radiography and CT for explaining the osseous causes of elbow stiffness., Materials and Methods: Two independent readers analyzed loose bodies and osteophytes on conventional radiography and CT (or CT arthrography) of the elbow in 94 consecutive patients (71 men, 23 women; mean age, 41 years; range, 18-68 years). Arthroscopic or surgical correlation was available in 58 (62%) patients. In all 94 patients, the expected restriction of motion was measured on images and correlated (Pearson's correlation) with the clinical restriction of motion. Kappa statistics were performed for interobserver agreement., Results: Accuracy for detecting loose bodies was 67% with conventional radiography and 79% with CT. Differences in accuracy were most pronounced for detecting loose bodies in the posterior joint space (64% for conventional radiography vs 79% for CT). Accuracy for detecting osteophytes was 69% with conventional radiography and 76% with CT. Expected restriction of motion on conventional radiography correlated significantly with clinical restriction for only one reader for flexion (R = 0.21, p = 0.04). Expected restriction of extension on CT correlated significantly with clinical restriction of motion by both readers (R = 0.34 and 0.33, p = 0.001 and 0.001, respectively). Expected restriction of flexion on CT correlated significantly by one reader (R = 0.24, p = 0.02). Interobserver agreement with regard to detection of both loose bodies and osteophytes was higher for CT (kappa = 0.83 and 0.76) than for conventional radiography (0.64 and 0.60)., Conclusion: CT is more effective than conventional radiography in explaining the osseous causes of elbow stiffness.

Musculoskeletal outcome remains the major hallmark of haemophilia. The purpose of the study was to assess joint status using a new musculoskeletal assessment tool in children with haemophilia and describe the development of haemophilic arthropathy during childhood and puberty focussing on the age of remarkable changes. The prospective study involved Lithuanian patients aged 4-17 years with severe haemophilia A and B, no signs of inhibitors and treatment on-demand. Patients were subdivided into two groups according to actual age. Group I patients were 4-9 years and group II patients 10-17 years of age. The musculoskeletal status was measured using the Haemophilia Joint Health Score (HJHS). We report on 20 patients with a mean age of 11.5 years (SD 4.3, range 4-17.2 years). The mean HJHS score was 24.5 (SD 14.5, range 5-50). The most affected joints were ankles, followed by knees and elbows. Mean HJHS score in age group I (n = 7) was 11.6 (SD 6.5); in group II (n = 13) the score was significantly higher - mean 31.5 (SD 12.8) (P = 0.0002). Ankles, knees and elbows were significantly more impaired based on the HJHS scores in older patients as compared with younger ones. The HJHS appears to be a useful tool in evaluating musculoskeletal outcome of patients receiving treatment on-demand. Children > or =10 years of age had significantly higher HJHS scores as a sign of progressing haemophilic arthropathy. We conclude that the most aggravating development of haemophilic joint damage seems to occur from the age of 10 and onwards.

Elbow pain is frequently encountered in clinical practice and can result in significant morbidity, particularly in athletes. Magnetic resonance imaging (MRI) is an excellent diagnostic imaging tool for the evaluation of soft tissue and osteochondral pathology around the elbow. Recent advances in magnetic field strength and coil design have lead to improved spatial resolution and superior soft tissue contrast, making it ideal for visualization of complex joint anatomy. This article describes the normal imaging appearances of anatomy around the elbow and reviews commonly occurring ligamentous, myotendinous, neural, and bursal pathology around the elbow., (Copyright 2010 Wiley-Liss, Inc.)

Arthropathy is considered as an irreversible and progressive complication in patients with haemophilia, even in children on prophylaxis. To estimate the progression of haemophilic arthropathy, 85 joints of 24 boys with severe (n = 18) and moderate (n = 6) haemophilia (A: 22, B: 2) were investigated with clinical examination, X-rays and magnetic resonance imaging (MRI) at two time periods (time 0 and 1). Patients' age at time 0 was 10.5 +/- 3.6 years and time elapsed to time 1 was 3.8 +/- 1.4 years. At time 0: all investigated joints had more than three bleeds. Sixteen boys were on secondary prophylaxis for 5.4 +/- 2.8 years. Clinical score (a modification of World Federation of Haemophilia's scale): 2.0 +/- 3.6, X-ray score (Pettersson): 2.1 +/- 2.8, MRI score (Denver): 4.5 +/- 3.8. After the first evaluation, prophylaxis was intensified in 11 children and initiated in four. At time 1: clinical score: 1.5 +/- 3.1, X-ray: 1.7 +/- 2.7, MRI score: 5.1 +/- 4.1. On average, the clinical and X-ray scores showed a significant improvement (26% and 40% of the joints respectively, P < 0.01) and the number of haemarthroses evidenced a threefold reduction from time 0 to 1 (P < 0.01), findings that could be associated with the modification of prophylaxis after time 0. MRI findings showed deterioration in 34% of the joints. Conversely, 14 joints (16.5%) with mild or moderate synovitis without cartilage degradation at time 0 showed an improvement at time 1. The information carried by the three scales could be divided into information shared by the three scores and information specific to each score, thus giving a more complete picture of joint damage caused by bleedings.

Elbow dysplasia is a major welfare issue for medium and large-breed dogs, including Labrador Retrievers, Rottweilers and Mastiffs. It is common to see older dogs with chronic lameness and bilateral pain and stiffness of the elbow, and managing the condition is challenging. We analysed scores of elbow dysplasia following the IEWG protocol from the official screening programme of the Allgemeiner Deutscher Rottweiler Club (ADRK). The data set included X-rays from 5100 Rottweiler dogs born between 1995 and 2004. Out of these 5,100 dogs, 46.9% were free from ED, 9.8% showed borderline signs (ED-UG), 31.8% ED-grade 1 (mild ED), 10% ED-grade 2 (moderate ED) und 1.6% ED-grade 3 (severe ED). Male dogs were more often affected by ED-grade 2 and 3 than female dogs. Traits analysed were ED-grade (dogs free from ED and dogs with ED-grades 1 to 3) and borderline ED (ED-UG). Birth year, birth season and inbreeding coefficient were significant for ED-grade. Higher inbreeding coefficients were associated with higher ED scores. ED-UG was significantly influenced by birth year and the interaction of birth year and birth season. A bivariate linear animal model was employed to estimate heritabilities using Residual Maximum Likelihood (REML) for ED-grade and ED-UG. Heritability estimates and their standard errors were 0.387 +/- 0.028 for ED-grade and 0.017 +/- 0.009 for ED-UG. The additive genetic correlation between ED-grade and ED-UG was -0.5. Heritabilities for ED-grade in female and male dogs were 0.350 +/- 0.033 and 0.497 +/- 0.047. We do not recommend use of ED-UG in breeding work because of the low heritability estimate and the negative additive genetic correlation with ED-grade.

The elbow is one of the most commonly injured joints in sports activities. In particular, weight lifters, golfers, tennis players, and pitchers are affected. Injuries in sports involving overhead throwing are commonly based on the pathophysiologic model of valgus extension overload syndrome. The injuries are commonly complex and demand a good knowledge of the symptoms, the exact anatomy, and the biomechanics to arrive at a precise radiologic diagnosis. The characteristic patterns of injury that occur in specific sports activities are related to a combination of increased varus or valgus and extension or flexion overload that results in tensile forces and/or compression and shear stress. Acute symptoms are frequently based on chronic degeneration of the tendons and ligamentous structures due to repetitive microtrauma from overuse syndrome.

The official screening results of the Belgian National Committee for Inherited Skeletal Disorders, an affiliate of the Belgian Kennel Club, have been used to estimate the prevalence of hip dysplasia, elbow dysplasia and humeral head osteochondrosis in the dog breeds in Belgium, and these have been compared with reported prevalence data from other countries. In some breeds, the prevalence of hip and elbow dysplasia is very high, both in Belgium and in other countries. Comparisons of the prevalence of hip dysplasia are not always feasible because different systems are used to evaluate the quality of the hips and because there is no strict consensus on what should be considered a diseased hip joint.

Objective: To determine if anthropometric measurements of the Labrador scapula, humerus, ulna and radius, or their ratios, are related to the presence of elbow dysplasia (ED)., Methods: Digital calliper measurements of the lengths of the left scapula, humerus, radius and ulna, and their ratios, were analysed by gender in 103 volunteer Labradors (41 dogs, 62 bitches) against the ED radiological scores derived by the International Elbow Working Group (IEWG). The IEWG score is an umbrella score used to classify for ED and includes fragmented coronoid process, osteochondritis dessicans, incongruity and ununited anconeal process, the last of which occurs rarely in Labradors., Results: Of the 103 Labradors studied, 31 were diagnosed radiographically with ED (20 bitches (32%), 11 (27%) dogs). Scapula length was significantly shorter for bitches with ED (P = 0.02), but not for dogs with ED. However, dogs showed a trend for a difference in the ulna:radius ratio (P = 0.06), which bitches did not. Although a greater percentage of bitches than dogs had ED in this study, the difference was not statistically significant., Conclusions: Labrador bitches diagnosed with ED have a shorter scapula, which is a new finding associated with this condition. The difference in presentation associated with gender is unexpected and further research is recommended.

Aim: To determine reliable magnetic resonance imaging (MRI) features differentiating three paediatric intra-articular congenital or neoplastic synovial lesions that contain blood products, from post-traumatic or haemorrhagic inflammatory processes., Materials and Methods: This was a retrospective review of MRI findings of 22 paediatric intra-articular congenital or neoplastic synovial lesions, including venous malformation (VM) (n=12), pigmented villonodular synovitis (PVNS; n=8), and synovial sarcoma (SS; n=2). These MRI features were compared with 22 paediatric post-traumatic or inflammatory intra-articular processes containing blood products and producing mass effect. The following imaging features were assessed: presence of a discrete mass, extension, extra-articular oedema, susceptibility, joint effusion, and size. Fisher's exact test was used and results were considered statistically significant when p<0.05., Results: The three intra-articular synovial lesions, compared with controls, were more likely to directly invade osseous structures when a discrete mass was present (13/16, 81.3% versus 1/9, 11.1%; p<0.002) and extend into extra-articular soft tissues (13/21, 61.9% versus 2/17, 11.8%; p<0.003), but were less likely to show extra-articular oedema (3/22, 13.6% versus 13/22, 59.1%; p<0.004), a joint effusion (10/22,45.5% versus 19/22, 86.4%, p<0.01), susceptibility within a joint effusion (0/22, 0% versus 11/22, 40.9%; p=0.00), osseous oedema (3/16, 18.8% versus 7/9, 77.8%; p<0.009), and synovial enhancement (8/21, 38.1% versus 14/16, 87.5%; p<0.003). VMs had characteristic tubular vessels with internal fluid-fluid levels (11/12) that extended into bone (10/12) and extracapsular soft tissues (11/12)., Conclusion: Our study indicates that, despite the overlapping presence of haemorrhagic products, intra-articular VM, PVNS, and SS show MRI features that permit distinction from acquired post-traumatic and haemorrhagic inflammatory lesions.

Baseball pitcher throwing biomechanics are important to understanding the pathophysiology and magnetic resonance (MR) imaging appearances of injuries in baseball pitchers. Baseball pitchers experience repetitive excessive valgus forces at the elbow. Typical injuries are secondary to medial joint distraction, lateral joint compression, and rotatory forces at the olecranon. MR imaging is useful for evaluation of the elbow in baseball pitchers.

Fragmented medial coronoid process (FMCP) is the most common cause of forelimb lameness in juvenile medium and large breed dogs; however methods of assessing the disruption to their gait remain subjective. The purpose of this study was to objectively quantify the mechanical disruptions to gait in dogs with arthroscopically confirmed unilateral FMCP. Seven dogs underwent full inverse dynamic analysis at the time of diagnosis. Kinematic and force data were collected from both forelimbs at trot. Stance phase joint angles, net joint moments and net joint powers were calculated using custom software. There were gross differences in kinetic and kinematic patterns between FMCP affected and compensating forelimbs. Stance time was 0.24 sec on the lame side and 0.26 sec on the compensating side. The shoulder and the elbow were more flexed at ground contact, and elbow, carpal and MCP joints had smaller ranges of motion on the lame side. Net joint moments were significantly reduced (P < 0.05) in the elbow, carpal and MCP joints of the FMCP affected limb. Net joint powers were likewise significantly smaller (P < 0.05). However, the overall moment and power patterns persisted. Total limb support moment was significantly smaller on the affected side (P < 0.05). Total limb power was significantly reduced on the affected side (P < 0.05) being most affected in its propulsive phase in the second half of stance. Inverse dynamic analysis of this clinical condition is an objective means by which to assess the mechanical disruption to gait.

Carpal-tarsal osteolysis is a rare condition that manifests as the progressive resorption of carpal and tarsal bones in young children. The diagnosis of this condition is often difficult and delayed as the initial clinical presentation is non-specific. Radiographic changes occur gradually, are often not seen at presentation and depend on recognising loss of bone in the ossification centres of the carpus and tarsus. MRI demonstrates morphological abnormalities in the cartilaginous, as well as the osseous components, of the developing carpal and tarsal bones and therefore may be helpful in predating the radiographic changes. Ultrasound appears to contribute little to the diagnosis and may be misleading. Exclusion of other conditions, particularly juvenile idiopathic arthritis, is important in making the diagnosis. MRI can be useful in excluding an inflammatory arthropathy, and suggesting the diagnosis of carpal-tarsal osteolysis.

Growth and differentiation factor 5 (GDF5) has been implicated in chondrogenesis and joint formation, and an association of GDF5 and osteoarthritis (OA) has been reported recently. However, the in vivo function of GDF5 remains mostly unclarified. Although various human GDF5 mutations and their phenotypic consequences have been described, only loss-of-function mutations that cause brachypodism (shortening and joint ankylosis of the digits) have been reported in mice. Here, we report a new Gdf5 allele derived from a large-scale N-ethyl-N-nitrosourea mutagenesis screen. This allele carries an amino acid substitution (W408R) in a highly conserved region of the active signaling domain of the GDF5 protein. The mutation is semi-dominant, showing brachypodism and ankylosis in heterozygotes and much more severe brachypodism, ankylosis of the knee joint and malformation with early-onset OA of the elbow joint in homozygotes. The mutant GDF5 protein is secreted and dimerizes normally, but inhibits the function of the wild-type GDF5 protein in a dominant-negative fashion. This study further highlights a critical role of GDF5 in joint formation and the development of OA, and this mouse should serve as a good model for OA.

We present a retrospective study of 25 patients treated by open arthrolysis of the elbow for post-traumatic stiffness. The mean follow-up was for 7.8 years (5 to 10.8). The range of movement of the elbow, pain scores and functional outcomes were recorded pre- and postoperatively. An improvement in the mean range of movement from 55 degrees (0 degrees to 95 degrees ) to 105 degrees (55 degrees to 135 degrees) was obtained in our patients at one year. This improvement was maintained over the mean follow-up period of 7.8 years (5 to 10.8). Improvement in pain, function and patient satisfaction was recorded in 23 of the 25 patients at final follow-up. On the basis of this study, we believe that the results of open arthrolysis for post-traumatic stiffness of the elbow are durable over the medium term.

Syringomyelia can occur in patients presenting bone and joint diseases of various origins. When joint destruction of the shoulder or elbow produces little pain, a neurological cause might be involved. In this case, the disease history can be of utmost importance because an initial diagnosis of rheumatoid polyarthritis, polyosteoarthritis, or destructive joint disease can be misleading before the syringomyelic origin of the bone and joint disease becomes patent. We report two cases illustrating this association and the diagnostic pitfalls which can delay recognition of the syringomyelia. Better awareness of the prevalence of this condition should be helpful in establishing the diagnosis and in selecting patients who can benefit from neurosurgical treatment. The two cases presented here suggest that syringomyelia could be underdiagnosed in certain patients with an initially atypical presentation. A review of the current knowledge of syringomyelia suggests that arthroplasty is generally not advisable for destroyed dislocated syringomyelic joints.

Painful snapping of the elbows is rare. We report on a 12-year-old boy with a painful snap in both elbows. High-resolution magnetic resonance imaging of the elbow using microscopy coils detected a synovial fold interposed in each humeroradial joint and was very helpful in establishing the cause of symptoms. Resection of the synovial folds was performed with subsequent relief of symptoms.

Unlabelled: Measurement of joint health is critically important when assessing children with haemophilia. Few measures exist; they lack sensitivity to small changes, don't account for normal development and were never formally validated. To address these concerns, the Hemophilia Joint Health Score (HJHS) was developed by modifying existing scores., Objective: To test the inter-observer and test-retest reliability of the HJHS., Methods: Using a fully factorial design, four physiotherapists (from Canada, the United States and Sweden) examined eight boys with severe haemophilia A on two consecutive days using the HJHS. The boys ranged in age from 4-12 years and presented with variable joint damage. Six index joints (elbows, knees and ankles) were assessed on 11 impairment items including swelling, flexion and extension loss and gait. Concordance was measured by the intra-class correlation co-efficient., Results: Reliability of the HJHS was excellent with an inter-observer co-efficient of 0.83 and a test-retest of 0.89., Conclusion: This study is the first in a series to assess the psychometric properties of the HJHS, a promising new measure of joint health in boys with haemophilia.

The utility of magnetic resonance imaging of the elbow has noticeably improved over the past decade. Advances in equipment and refinement of pulse sequences has allowed for superior visualization of many pathologic processes involving the elbow joint and its surrounding structures. At this time, magnetic resonance imaging is most valuable in detecting occult fractures, osteochondritis dissecans, collateral ligament tears, nonossified intra-articular loose bodies, and soft tissue masses. It may also aid in the diagnosis of tendinous injuries, compression neuropathies, and synovial disorders. This article serves as an educational update for hand surgeons and illustrates the features of normal anatomy and specific disorders of the elbow.
