Objective: This study compared the achieved tooth movement to that predicted in the preliminary simulated digital treatment plan between adults and teenagers. Materials and methods: Records of 60 patients (30 adults; mean age: 36.6 ± 11.36 years, and 30 teenagers; mean age: 16.23 ± 2.25 years) were randomly selected. Initial and predicted models were obtained from the initial simulated treatment plan. The first model of the refinement scan was labeled as achieved. SlicerCMF software (version 3.1; https://www.slicer.org) was used to superimpose the achieved and predicted digital models over the initial ones with regional superimposition on relatively stable first molars. 600 teeth were measured in each group for horizontal, vertical, angular movements, and transverse dimensions. Accuracy was defined as the achieved amount of movement minus predicted and was calculated for each individual and compared between teens and adults. Results: The mandibular inter-canine width accuracy was statistically significant between groups (p = 0.050). Significant under-correction in horizontal movements was noted for mandibular first premolars in teenagers compared to adults (p = 0.037). There was considerable over-correction in horizontal movements for mandibular central and lateral incisors between groups (p < 0.05). No significant difference was observed between groups in the vertical plane. Rotations were underachieved for maxillary first premolars, more in adults than teenagers (p = 0.017). Conclusion: The accuracy of achieved versus predicted tooth movement between adults and teenagers was significant for the inter-canine width. Mandibular central and lateral incisors showed significantly greater over-correction in adults in the horizontal plane. The accuracy of rotations and vertical movements was comparable. Objectives: Whether gemination or fusion, double teeth are rare worldwide, including Africa based on few published data. New cases from the continent are tallied, and anomalies potentially associated with double teeth are identified. These findings should interest a range of dental researchers. Methods: The presence of double teeth was recorded in 97 modern and premodern North and sub‐Saharan African samples (5631 inds.). They and coexistent anomalies are described relative to published examples. Prevalence was estimated as possible, using a Poisson model for 95% confidence intervals (CI). Results: Three maxillary double teeth were identified: a primary left lateral incisor in a Nubian child (1938–1756 BC), permanent left central incisor in an adult Egyptian (3650–3500 BC), and permanent right central incisor in a modern (19th century) adult from Guinea. Each co‐occurs, respectively, with a talon cusp, peg lateral incisor and, in the latter individual, second premolar crown variation with rotation, and third molar dens evaginatus. Double tooth prevalence is 0.048% (CI 0.001%–0.270%), with regional variation, in premodern, and 0.000% in modern North Africans. It is 0.000% for premodern and 0.048% for modern sub‐Saharan Africans (0.008%–1.714%). Conclusions: The double incisors are comparable to other global examples, indicative of common developmental processes during odontogenesis. Prevalence is lower than published modern rates, to suggest some exceptionality in Africans as reported earlier for other dental variants. Finally, though circumstantial, double teeth and accompanying anomalies may share an etiology. The palatal anchorage device (PAD) is commonly used in fixed appliance for anchorage, but the biomechanics of PAD in patients treated with clear aligners(CAs)remains poorly understood, especially in its elastic mode and force magnitude. This study aimed to assess the biomechanical effects of using PAD for retraction during clear aligner treatment (CAT) with the extraction of two maxillary first premolars. Four finite element models were created: (1) Incisors retraction (IR) with active contraction of the clear aligner only; (2) IR with PAD using different forces (50,75,100 g) or not; (3) PAD without IR (forces of 50,75,100 g), and (4) Different elastic directions (D1, D2, D3) from aligner to PAD without IR. IR caused lingual tipping and extrusion of the central incisors, as well as mesial and buccal tipping of the first molar. PAD during IR provided strong anchorage for the first molar, with elastics slightly increasing the lingual tipping and extrusion of the central incisors. Elastics applied to the U-shaped arch led to central incisor intrusion and reduced lingual retraction. The highest stress was observed on the mesial attachment of the canine. PAD effectively reinforced posterior anchorage in CAT, while anterior positioning facilitated anterior teeth intrusion. Background: The aim of the investigation was to evaluate if the inclination of the lower anterior teeth can be controlled reliably after lower premolar extraction for Class III camouflage treatment with completely customized lingual appliances (CCLAs). Treatment outcome was tested against the null hypothesis that lower premolar extractions for non-surgical camouflage treatment of a Class III malocclusion will lead to further compensation by retroclining mandibular incisors during CCLA treatment. Methods: This retrospective study included 25 patients (f/m 12/13; mean age 20.7 years, SD 9.5 years) with uni- or bilateral Class III molar relationship and a Wits value of ≤ -2 mm. In all consecutively debonded patients, lower premolars were extracted to correct the sagittal relationship with a non-surgical camouflage approach. Lateral head films prior to (T1) and at the end of lingual orthodontic treatment (T2) were used to evaluate skeletal and dentoalveolar effects. A paired t-test with alpha = 5% was used to define differences between the endpoints. The linear correlation between the inclination of the mandibular incisors at T1 and the achieved correction was measured with the Pearson correlation coefficient (PCC). A Schuirmann's TOST equivalence test was used to check if the final lower incisor inclination was within the defined norms. Results: The null hypothesis was rejected as the mean lower incisor inclination was improved by 1.8° despite lower premolar extractions (T1: 86.8°/ T2: 88.6°). There was a strong correlation (-0.75) between the lower incisor inclination at T1 and the achieved correction indicating a controlled correction towards the norm regardless of the initial incisor position. At T2, the interincisal angle as well as the lower incisor inclination were within the norm. Conclusion: Lower premolar extractions for non-surgical camouflage treatment of a Class III malocclusion will not lead to undesired retroclining of mandibular incisors during CCLA treatment even in severe cases. Background: Mandibular second premolar agenesis is a common problem in orthodontics and is often treated in conjunction with maxillary counterbalancing extractions. However, in cases without maxillary crowding or dental protrusion, space closure may pose challenges leading to compromised occlusal results or patient profile. Multiple techniques have been described to treat these patients; nevertheless, there is a paucity of data comparing effectiveness of space closure utilizing various anchorage techniques. The goal of this study is to assess the effectiveness of the Herbst device during mandibular molar protraction and compare it to the use of temporary anchorage device (TADs) in patients with mandibular second premolar agenesis. Materials and methods: This retrospective study included 33 patients with mandibular premolar agenesis treated without maxillary extractions. Of these patients, 21 were treated with protraction Herbst devices and 12 with TADs. Changes in molar and incisor positions, skeletal base positions and occlusal plane angulations were assessed on pretreatment (T0) and post-treatment (T1) lateral cephalograms. Scans/photographs at T0 and T1 were used to evaluate canine relationship changes representing anchorage control. Space closure and breakage/failure rates were also compared. Data was analyzed with paired and unpaired t-tests at the significance level of 0.05. Results: Within the Herbst group, changes in mandibular central incisor uprighting and mandibular molar crown angulations were statistically significant. However, no significant differences were noted between the Herbst and TAD groups. Protraction rates as well as overall treatment times were comparable (0.77 mm/month vs. 0.55 mm/month and 3.02 years vs. 2.67 years, respectively). Canine relationships were maintained or improved toward a class I in 82.85% of the Herbst sample, compared to in 66.7% of the TAD sample. Emergency visits occurred in 80.1% of the Herbst group, with cementation failures or appliance breakages as the most common reasons. Conclusion: The Herbst device could be a viable modality in cases with missing mandibular premolars where maximum anterior anchorage is desired, or if patients/parents are resistant to TADs. Furthermore, they could be beneficial in skeletal class II patients with mandibular deficiency who also need molar protraction. Tooth transposition is the positional interchange of two adjacent teeth, which may be incomplete or complete. The root apex may remain in a normal position despite a severely displaced crown. The aim of this paper is to provide a literature review on the incidence, aetiology and treatment options for pseudo-transposed mandibular incisors. This paper describes the orthodontic treatment of two cases that were successful in winning the William Houston Gold Medal at the Membership in Orthodontics Examinations of the Royal College of Surgeons of Edinburgh in 2022. Both cases discuss the management of palatally impacted maxillary canines and treatment was initiated during the emerging COVID-19 crisis and completed over a 27-month period. The first case describes the management of a 15-year-old female with a Class II division 1 incisor relationship complicated by a palatally impacted upper left canine, moderate upper and lower arch crowding, and a lower centreline discrepancy with an associated right lateral displacement on closure. Treatment involved open surgical exposure of the impacted canine, a quad helix appliance, premolar extractions, and upper and lower fixed appliances. The second case describes the management of a 14-year-old female with a Class III incisor relationship complicated by bilaterally impacted maxillary canines, impeded eruption of the lower right second molar, moderate lower and severe upper arch crowding, and an upper centreline discrepancy. These case reports aim to demonstrate the high standards of care achieved by Orthodontic Specialty Trainee Registrars in the UK and provide aspirational standards for future trainees. Objectives: This study aims to compare the achieved and predicted root movements in adults after four first‐premolar extractions and Invisalign treatment. Materials and Methods: Thirty‐three consecutive adults (22 Class I, 9 Cusp‐to‐cusp Class II and 2 Cusp‐to‐cusp Class III) from a single clinical division who completed the first series of aligners after premolar extractions were included in this retrospective study. A pretreatment cone‐beam computed tomography model was registered onto the pretreatment surface‐scanned dental model (SSDM) to locate the pretreatment root apices of the whole dentition. These were copied and transferred to the predicted and achieved post‐treatment SSDMs to acquire the locations of the predicted and achieved post‐treatment root apices. The differences between predicted and achieved root movements (DPARMs) were tested using the paired t‐test or Wilcoxon signed rank test. Results: In the anteroposterior direction, posterior root movements of maxillary and mandibular anterior teeth were poorly achieved (3.24–5.74 mm DPARMs, p <.05). In the vertical direction, roots of maxillary anterior teeth achieved greater intrusion (0.70–0.93 mm DPARMs, p <.05), while those of mandibular incisors achieved less intrusion (0.57–0.65 mm DPARMs, p <.05) than predicted. In the mediolateral direction, lateral incisor roots did not move distally (−0.65 to −0.96 mm DPARMs, p <.05), while those of canines did not move buccally, compared with the prediction (−0.75 mm DPARMs, p <.05). Conclusions: In the four first‐premolar extraction treatments with Invisalign, root movements were not achieved as predicted, particularly for anterior teeth in the anteroposterior direction. Objective: To evaluate the accuracy of dental rotational movements using clear aligners with different attachment configurations. Materials and Methods: This retrospective study analysed 212 teeth from 89 patients undergoing Invisalign treatment. Digital models were analysed after the virtual treatment plan (ST1) and after the first treatment phase (ET1) to evaluate the effective clinical rotational movement. The rotational movements of incisors, canines, and bicuspids were measured using data from the Clincheck Movements Table. ST1 and ET1 were compared to determine the actual rotational movement achieved (ST1‐ET1). The presence or absence of attachments (rectangular or optimized) on teeth was analysed. The accuracy of rotational movements among attachment types was compared using the Kruskal‐Wallis test. Multiple linear regressions were conducted with accuracy as the dependent variable and tooth type, gender, and age as predictors. Results: Optimized attachments had the highest median accuracy (70%), followed by rectangular (65%), and without attachment (63%), with no significant differences (p =.5). There were no significant differences across age groups, genders, or tooth types. Baseline accuracy was 68.62% (95% CI: 56.03–81.20, p <.001). Age was a significant predictor (estimate = −0.30, 95% CI: −0.58 – −0.03, p =.032), indicating decreased accuracy with increasing age. The model's R2 was 0.046, with an adjusted R2 of 0.003, indicating minimal variance explained. Conclusion: The addition of attachment configurations to clear aligners improves rotational accuracy, but not significantly. Background: Pattern of dental anomalies encountered in cleft patients shows subtle signs of genetic involvement. This study aimed to evaluate the prevalence and pattern of tooth agenesis and supernumerary teeth in Thai cleft population according to the cleft type. Methods: Data collected from patients with cleft lip and palate, who had been treated at Tawanchai Cleft Center, Khon Kaen University, Thailand, available during year 2012–2022, were investigated. Records from 194 patients with non-syndromic clefts met the inclusion criteria. Standard dental records, and at least either orthopantomogram (OPG) or cone beam computed tomography (CBCT), were examined. Statistical analysis was performed using chi-square and binominal test (p ≤ 0.05). Results: Prevalence of tooth agenesis was higher (77.3%) than that of supernumerary teeth (5.7%) and was more common in bilateral cleft lip and palate (BCLP) (88.1%) than in unilateral cleft lip and palate (UCLP) (72.6%) (p = 0.017). The upper lateral incisor was more frequently affected (46.4%), followed by the upper second premolar. The number of missing teeth observed on the left side was significantly higher. Patients with left UCLP (ULCLP) had the highest prevalence of tooth agenesis. A total of 41 tooth agenesis code (TAC) patterns was found. The prevalence of supernumerary teeth was comparable with 6.6% of ULCLP, 5.1% of BCLP, and 4.5% of URCLP. Tooth-number anomalies were observed more often in the BCLP and were most likely to occur on the left side of the maxilla. Both types of anomalies could be featured in a small proportion of cleft patients. Conclusions: More than half of the patients with non-syndromic cleft lip and palate in this study, presented with tooth-number anomalies. Tooth agenesis was approximately 10-time more prevalent than supernumerary teeth. Tooth agenesis was likely to appear on the left-side of the maxilla regardless of the laterality of the cleft. Background: Posttreatment changes after orthodontic treatment are challenging. One of the main reasons for such a phenomenon is the lack of patient compliance with removable retainers especially in the maxillary arch, due to palatal coverage, deterioration of speech, decreased masticatory efficiency, and loss of retainers. Fixed retainers have been introduced to overcome patient compliance and provide longer stable results. However, teeth still show movements when a six-unit fixed retainer is in place. Thus, in this study, an eight-unit fixed retainer was evaluated in an attempt to eliminate unwanted movements. The aim of this research: was to assess short-term positional changes associated with an eight-unit extended maxillary fixed retainer. Materials and methods: A single-arm clinical trial was conducted to address the aim of the study. This research was approved by the institutional review board of the Faculty of Dentistry, Alexandria University (IORG:0008839, No-0479–8/2022). The registration date of this study was 5/06/2023. Twenty-eight patients (19.8 ± 4.5 years) who had finished the active orthodontic phase and started retention had an eight-unit extended maxillary fixed retainer that was bonded to the palatal surface of the maxillary incisors, canines, and the first premolars or the second premolars. Pre-retention and one-year post-retention intra-oral scans were made to produce STL files that were superimposed to determine the amount of tooth change. Additionally, analysis of digital casts and lateral cephalometric radiographs was performed. Results: Statistically significant changes in all planes and the rotation of teeth after one year of retention were found. The upper right lateral incisor exhibited the most evident change in the vertical plane, while the upper right central incisor exhibited the greatest change overall. Minimal changes in the cast measurements were observed. Lateral cephalometric measurements showed minimal changes after one year of retention, and these changes were not statistically significant except in the interincisal angle and the angle between the upper incisor and the line connecting the A-point to the pogonion. Conclusion: Increasing the extension of maxillary fixed retainers did not eliminate unwanted tooth movement in the first year of retention. [ABSTRACT FROM AUTHOR]

Objective: Assess color alteration and hydrogen peroxide (HP) penetration in human and bovine teeth using various in‐office bleaching protocols with different application times. Materials and Method s : Thirty healthy human premolars and 30 healthy bovine incisors were divided into five groups and subjected to different bleaching protocols: 2 × 15 min, 1 × 30 min, 2 × 20 min, or 1 × 40 min, with a control group for each tooth type. All teeth were treated with 35% HP gel. Color alteration was measured using digital spectrophotometry before and 1 week after bleaching. HP concentration within the pulp was determined via UV–Vis spectrophotometry. Statistical analysis included one‐way ANOVA, Tukey's, and Dunnett's tests (α = 0.05). Results: All groups exhibited significant color alteration, with no statistically differences among them (p > 0.05). However, significant differences were observed when compared with their respective control groups (p < 0.05). HP penetration into the pulp was evident in all bleached teeth compared to the control groups (p < 0.05), with the 2 × 20 group showing the highest HP levels within the pulp cavity, irrespective of tooth type (p < 0.05). Conclusion: A simplified 1 × 30‐min protocol can be recommended as it effectively maintains color alteration and HP penetration, irrespective of whether human or bovine teeth. Clinical Significance: This study suggests that an in‐office dental bleaching protocol using a 1 × 30‐min session is recommended, as it ensures both effective color change and no increase in the amount of HP penetration. Background: This retrospective clinical study was undertaken to comparatively evaluate the number of restorative treatments, endodontic treatments, and tooth extractions performed for patients under general anesthesia due to dental anxiety or special needs between 2015 and 2022 and to examine the pain, bleeding, nausea, and vomiting data of those patients. Methods: In total, 1165 patients underwent dental treatment under general anesthesia in the faculty hospital. Those under the age of 15 and with no endodontic procedure planned (n = 918) were excluded, followed by those with incomplete data (n = 25) and those without endodontic treatment (n = 25). Patients who underwent at least one endodontic treatment were finally included in the study (n = 184). Patients were divided into two groups: healthy and with special needs. Dental treatments were recorded as endodontic, restorative, and teeth extractions. Endodontic treatments were classified according to the tooth type (premolar, molar, and incisors). The composite restorations were classified as anterior, occlusal (O), occluso-distal (OD) or occluso-mesial (OM), and mesio-occluso-distal (MOD) restorations and patients' post-treatment pain, nausea, vomiting, and bleeding were recorded. The data were analyzed statistically. Results: Among the 184 patients included in the study, 70 (38%) were healthy, and 114 (62%) had special needs. Postoperative bleeding was observed more in patients with special needs (χ2 = 4.189, p < 0.05), whereas pain was observed more in healthy patients (U = 2922.00, p < 0.05). While the number of anterior, O, and MOD restorations was higher in patients with special needs, the number of OD or OM restorations was higher in healthy patients (χ2 = 74.877, p < 0.05). Conclusions: Patients with special needs undergo a greater number of restorative treatments compared to control patients, which may be associated with the inadequate oral hygiene care of such patients. However, restorative treatment is mostly indicated for such patients in our faculty hospital, which may indicate that a conservative approach is taken. Additionally, the finding that postoperative bleeding was more severe in this group of patients compared to the control group in this study may emphasize the need to consider more possible complications after general anesthesia in these patients. Purpose: To evaluate the adaptability between posts and post spaces and the rationality of cores fabricated by two digital custom post‐and‐core processes. Materials and Methods: Titanium post‐and‐cores were fabricated by digital scanning impression technology or digital scanning wax‐pattern technology on tooth defect molds of incisors, premolars, and molars, with traditional lost‐wax casts of these teeth as the controls. Micro‐CT and a laboratory scanner were used to determine intervals between post wall and root canal wall of the root apex, middle, and cervix of each sample in cross‐, sagittal, and coronal sections; intervals between the end of post and tooth; diameters of cervical, middle, and incisal part at cross‐, sagittal, and coronal sections of each sample, as well as shoulder widths. Results: The three fabrication processes showed significant differences in intervals between post‐and‐core prostheses and root canal walls, diameters of all parts of cores, and shoulder widths. Scanning impressions showed significant advantages in the main part of post‐and‐cores in incisors and premolars, while the scanning wax‐pattern process showed obvious inferiorities in premolars and molars. As to core spatial size, values of measured sites in the scanning impression process were closer to the standard than those of the traditional process, while differences between the measured value of the scanning wax‐pattern process were much more obvious than in the traditional process. Conclusions: The use of digital custom post‐and‐core scanning impressions improved the rationality and precision of post‐and‐core dimensions compared with two other processes. [ABSTRACT FROM AUTHOR]

Objectives: To evaluate the feasibility of using the pulp volume (Pv) to total volume (Tv) ratio (Pv:Tv), obtained from cone beam computed tomography (CBCT) scans of single-rooted teeth, for age estimation in a Brazilian population sample. Methods: After obtaining approval from the ethics committee, the study commenced by applying inclusion criteria to screen CBCT scans, resulting in a probability-based sample of participants aged 18 years and older (ranging from 18 to 82 years, with a mean age of 46.44 years). A total of 517 single-rooted teeth, including maxillary central incisors (CI), mandibular canines (C), and mandibular first premolars (FP), were chosen based on excellent agreement values (> 0.9). Pv and Tv measurements were conducted using semi-automatic segmentation with ITK-SNAP 3.8 software. Statistical analysis was performed using Jamovi software, with a significance level set at 5% (α = 0.05). Results: A strong negative correlation (r > −0.7) was observed between chronological age and the Pv:Tv ratio across all examined teeth. However, when conducting regression analysis with Pv:Tv data and chronological age as the independent variable, only the mandibular FP teeth exhibited a normal distribution. The resulting linear model demonstrated moderate predictive value (approximately 64%) in explaining the variance in chronological age, but caution should be exercised when interpreting these findings. Conclusions: The method of measuring individual tooth volume using CBCT to estimate chronological age via Pv:Tv has been demonstrated as effective and reproducible within the Brazilian population sample. Introduction: This study aimed to compare achieved movements with predicted movements after 28‐week use of Invisalign Lite aligners. Settings and Sample Population: The digital impressions of 21 subjects treated with Invisalign Lite at a private practice and in the dental clinic (Milan, Italy) were taken and analysed. Subjects were Caucasian with a mean age of 20.1 years. Methods: Patients were analysed at two time points: at T0, before starting therapy, and at T1, after 28 weeks of treatment with Invisalign clear aligners, with a 2‐week change interval. The changes that occurred between T0 and T1 were compared to the predicted changes between T0 and Ts (setup/ClinCheck). Tooth movement performance was estimated through variables calculated as the difference between obtained and planned movements. Results: In both maxillary and mandibular arches, the teeth that exhibited the least accurate expression of torque were the central incisors. Tip was not accurate on maxillary central incisors and canines, mandibular central incisors, lateral incisors, first premolars, second premolars and first molars. Rotations were under‐expressed on maxillary lateral incisors, canines and second premolars and on mandibular central incisors, canines, first premolars, second premolars and first molars. The overall angular changes showed a tendency to underperformance. Transverse linear changes were accurate with a significant overperformance on maxillary and mandibular first molars. Conclusions: Torque correction of maxillary central incisors, as well as rotational correction of most of the teeth, showed significant differences between what was planned and what was obtained. Introduction: Orthodontic treatment of severe Bi-maxillary protrusion cases involves extractions of all four premolars to provide space for anterior teeth retraction. Over the years different methods of retraction have been developed for individual tooth retraction and en-masse anterior teeth retraction utilising different types of mechanics for retraction purposes. Anchorage consideration has been a problem because of anchor loss, but it has been overcome by utilising absolute anchorage with Temporary Anchorage Devices (TAD). Need for the study: The TAD devices mini plates and miniscrew have been Known for providing absolute anchorage for retraction purposes but there has been limited evidence to prove which TAD device is better in terms of time taken for retraction and external root resorption. Therefore, the present clinical study aims to determine the superior treatment method using the two devices based on duration in terms of time taken and amount of retraction achieved in the specified time. Aim: Evaluation and comparison of the effectiveness of miniplate and mini-screw for retraction of anterior teeth in severe bimaxillary protrusion cases with extraction of Ist bicuspids. Materials and Methods: A split-mouth randomised controlled trial will be carried out in the Department of Orthodontics and Dentofacial Orthopedics, Sharad Pawar Dental College and Hospital, Sawangi, Wardha, from October 2024 to March 2026 with a total of 11 subjects. The sample will be selected from the Outpatient Department based on the study's inclusion and exclusion criteria. Informed written consent will be gathered from all participants. The patient sample will be divided into two groups: Group-A: will receive a mini screw (control group) and Group-B: will receive a mini-plate (experimental group). The statistical analysis for comparing the en-mass rate of retraction of anterior teeth (mm) in the allocated groups will be done using the Mann-Whitney test, and student's t-test, with the significance level set at 5%. Background: CBCT is widely used in dentistry and is used for a variety of purposes. The presence of different artifacts can reduce the quality of images. Since the implants mainly contain metals, they can cause many artifacts in CBCT images. Due to the importance of CBCT and the effects of artifacts on the final image quality, this study was conducted to investigate the effect of FOV size on the artifacts. Methods: Three human skulls with unbroken mandible and maxilla were selected. CBCT images were prepared under normal exposure conditions and with three ranges of FOV (4 × 5 cm2, 5 × 8 cm2, 8 × 8 cm2). Then an implant was placed in the empty dental sockets in the incisor, canine, and premolar areas, and CBCT images were taken with similar conditions. The Hounsfield Unit obtained before and after implant placement was compared, and their difference was considered as metal artifact. Data were analyzed using two-factor variance analysis. Results: In this study, the mean difference of artifacts with a large FOV in both jaws and in the mandible for medium and small radiation ranges from the central to the premolar side was always decreasing. However, in the maxilla for the medium and small radiation ranges, the mean difference increased from the central side to the canine, but again decreased relatively significantly from the canine to the premolar. The mean interaction between the dental areas and the FOV sizes was not significant (p = 0.997), and it was found that the amount of artifacts in the studied areas does not change with changing the size of the FOV. Also there was no significant difference of artifacts between the three types of FOV size (p = 0.730), but there was a significant difference between the dental areas (p = 0.020). Conclusions: According to the findings of this study, it can be concluded that the amount of metal artifacts is not related to the size of FOV. Background: Cleidocranial dysplasia (CCD) is an autosomal dominant hereditary disorder. Besides skeletal abnormalities, CCD is often associated with dental complications, such as multiple supernumerary teeth and permanent teeth impaction or delayed eruption. Methods: Supernumerary teeth of axial, sagittal and coronal CBCT view was characterized in detail and 3D image reconstruction was performed. Number and location of teeth, morphology of supernumerary teeth, positional relationship between supernumerary and adjacent permanent teeth, direction of supernumerary teeth in CCD patients were analyzed. Results: The mean age of the 3 CCD patients in this study was 16.7 years. Among 36 supernumerary teeth, the majority of them were identified as apical side located and lingual side located. Normal orientation was the most common type in this study, followed by sagittal orientation, and horizontal orientation. Horizontal orientation teeth were all distributed in the mandible. Supernumerary teeth exhibited significantly shorter crown and dental-root lengths, as well as smaller crown mesiodistal and buccolingual diameters (P < 0.01). There was no difference in the number of supernumerary teeth between the maxilla and mandible, and the premolars region had the largest number of supernumerary teeth and the incisor region had the smallest number. Conclusions: This study compares number and location of teeth, morphology of supernumerary teeth, positional relationship between supernumerary and adjacent permanent teeth and direction of supernumerary teeth, this study also provides a reference for the comprehensive evaluation of CCD patients before surgery. Background: There are different methods for determining the required space for unerupted teeth. However, the accuracy of these techniques varies depending on ethnic differences. Therefore, the current study was performed to compare the accuracy of four methods for estimating the mesiodistal width of unerupted canines and premolars in a population of northern Iran. Methods: The present cross-sectional study was conducted on 50 pairs of dental casts of patients aged 12–24 years old. The mesiodistal width of the teeth was measured with a digital caliper by two observers (ICC < 0.9), and the mean value was recorded. The space required for eruption of canines and premolars was obtained by the Tanaka-Johnson formula and the Moyers tables and compared with the actual value by paired t test. Results: The Tanaka-Johnson formula had overestimation in the maxilla and mandible, which was statistically significant (p < 0.001). The values obtained from the Moyers tables in different confidence levels were not accurate. However, the 65% level for the mandible had almost no difference from the actual value (P = 0.996 and r2 = 0.503). Furthermore, linear regression was obtained based on the total mesiodistal width of the maxillary first molar and mandibular central incisor (maxilla: Yx= 0.613X + 2.23 and mandible: Ym= 0.618X + 1.6) and the total mesiodistal width of the mandibular first molar and maxillary central incisor in each jaw (maxilla: Yx = 0.424X + 5.021 and mandible: Ym = 0.447X + 3.631). Conclusion: The Tanaka-Johnson method was overestimated in the population of northern Iran. The 85% and 75% confidence levels of the Moyers table have the best clinical results for the maxilla and mandible, respectively. Purpose: A jumping gap (JG) refers to the implant's future buccal wall; this study aims to estimate the jumping gap dimension in relation to virtual implant placement and subsequently link the implant diameter and implant position with the anatomical site. Methods: This observational study was conducted to analyze the maxillary teeth in the esthetic zone from cone-beam computed tomography (CBCT) scans using OnDemand software. The horizontal jumping gap dimension of each tooth was assessed by subtracting the calculated virtual implant diameter from the socket dimensions. Results: A total of 253 anterior and premolar maxillary teeth were analyzed from 52 CBCT scans. The estimated JG dimensions were 1.23 ± 0.59 mm, 1.80 ± 0.64 mm, 3.02 ± 0.69 mm, for central incisors, lateral incisors and canines respectively, 3.70 ± 0.68 mm for the first premolars showing the highest horizontal gap and 3.19 ± 0.88 mm for second premolars. The incisors showed the smallest horizontal gap compared to the canines and premolars. Conclusions: In terms of JG, immediate implant placement is more favorable at the canine and premolar sites. By contrast, the incisors sites should be handled with extreme caution, where the use of narrower implants is advisable when necessary. [ABSTRACT FROM AUTHOR]

Background: Dental development assessment is an important factor in dental age estimation and dental maturity evaluation. This study aimed to develop and evaluate the performance of an automated dental development staging system based on Demirjian's method using deep learning. Methods: The study included 5133 anonymous panoramic radiographs obtained from the Department of Pediatric Dentistry database at Seoul National University Dental Hospital between 2020 and 2021. The proposed methodology involves a three-step procedure for dental staging: detection, segmentation, and classification. The panoramic data were randomly divided into training and validating sets (8:2), and YOLOv5, U-Net, and EfficientNet were trained and employed for each stage. The models' performance, along with the Grad-CAM analysis of EfficientNet, was evaluated. Results: The mean average precision (mAP) was 0.995 for detection, and the segmentation achieved an accuracy of 0.978. The classification performance showed F1 scores of 69.23, 80.67, 84.97, and 90.81 for the Incisor, Canine, Premolar, and Molar models, respectively. In the Grad-CAM analysis, the classification model focused on the apical portion of the developing tooth, a crucial feature for staging according to Demirjian's method. Conclusions: These results indicate that the proposed deep learning approach for automated dental staging can serve as a supportive tool for dentists, facilitating rapid and objective dental age estimation and dental maturity evaluation. Objective: The aim of this study was to investigate the thickness of acellular extrinsic fibre cementum (AEFC) at four root positions of anterior and posterior teeth with special focus on functional aspects. Furthermore, the correlations between cementum thickness and chronological age and sex are investigated. Background: While numerous studies confirm continuous cementum apposition with age, masticatory forces as well as physiological and orthodontically induced tooth movements also have the potential to affect tooth cementum thickness. Materials and Methods: Undecalcified teeth were embedded in resin and transverse‐sectioned in the cervical third of the root. Two sections per root were selected, and digital images at four positions were obtained (mesial, distal, oral, and vestibular) using light microscopy. The AEFC thickness of 99 teeth (anterior = 66, posterior = 33, male = 54, female = 45) were measured in both sections. The differences in mean values between root positions and the association of root position variation with tooth type, age, sex, and subject as well as the overall effects of age and sex were analysed using a mixed model. Results: First incisors and canines showed the greatest mean AFEC thickness, in contrast to premolars which had the lowest values. Differences were found across the four root positions, with a pattern varying considerably between anterior and posterior teeth and between maxilla and mandible in the anterior teeth. An interaction between root position and subject pointed to the existence of an individual component in the variation of AEFC thickness across the four root positions. There was an age trend with an almost linear increase in cementum thickness of 1 μm per year. Overall, females tended to exhibit a significantly lesser AEFC thickness compared to males. Conclusions: Distinct differences in the pattern of thickness values across the four root positions in anterior and posterior teeth support the assumption that the AEFC is strongly affected by functional processes. The aim of this case series was to evaluate implants inserted in bone after guided bone regeneration (GBR). Fourteen patients with generalized aggressive periodontitis (GAP) who had lost one or two maxillary teeth in the incisor or premolar region were enrolled in the study. Due to bone resorption, the lateral width and vertical height of the bone were insufficient for implant placement. GBR was carried out in a staged approach using titanium-reinforced e-PTFE (expanded polytetrafluoroethylene) membranes. No bone grafts or bone substitute materials were used. After 6 to 8 months, turned-surface implants (n = 47) were inserted in augmented and nonaugmented bone sites and prosthetically treated with single crowns. All patients were examined during a 3- to 6-month recall schedule over a 10- to 20-year period, and clinical and radiographic examinations were performed. GBR yielded mean vertical and lateral bone gains of 4.5 and 7.0 mm, respectively. The implant survival rate was 100%, mucositis was present in 28.8% of sites, and peri-implantitis was not found. The annual bone loss at tooth sites was significantly higher than at implant sites in augmented bone (0.5% vs 0.2%, respectively; P = .000), and the adjacent teeth had significantly higher annual bone loss (0.8%; P = .000). Background: Vertical maxillary excess (VME) is one of the most common reasons for seeking orthodontic treatment. Total intrusion with aligners is a promising alternative to surgery in some cases. Considering the elastic deformation of aligners, this study aimed to evaluate the possible desirable and undesirable teeth displacements during full maxillary arch intrusion using clear aligners and temporary anchorage devices (TADs). Methods: The maxillary arch and clear aligners were modeled in SolidWorks. Four aligner brands including Leon, Duran, Duran Plus, and Essix Plus were selected based on their material properties. Anterior and posterior intrusion forces of 80 and 300 g were applied from attachments between the canines and first premolars and between the first and second molars, respectively. Vertical and anteroposterior tooth displacements were determined. Results: The greatest intrusion was recorded at the buccal of the second molar, followed by the first molar. The lowest value was measured at the palatal of the molars with all aligners except Duran, which indicated minimal intrusion in the central incisor. All teeth were mesially displaced at the incisal/occlusal except incisors that moved distally. All apices showed distal movement. Conclusions: Total intrusion using clear aligners may be accompanied by other tooth movements, including buccal tipping and mesial-in rotation of the molars, retrusion of incisors, and mesial movement of other teeth. This case report describes the treatment of two maxillary central incisors following a traumatic injury with tooth #8 developing replacement resorption and #9 developing inflammatory root resorption. A 10-year-old girl presented complaining of pain in her maxillary central incisors. Upon clinical examination, teeth #8 and #9 were tender to percussion and palpation of the buccal soft tissues. Thermal and electrical pulpal sensitivity tests for teeth #8 and #9 were negative. An intraoral periapical radiograph revealed resorptive defects in tooth #8, which were filled with bone-like tissue, while tooth #9 had radiolucent resorptive defects along the root surface and a periapical radiolucency. A diagnosis of replacement resorption was made for tooth #8 and external inflammatory root resorption for tooth #9. Tooth #8 was treated with a multidisciplinary approach utilizing a guided template for premolar autotransplantation with an immediate veneer restoration, while tooth #9 was managed with root canal treatment using a tricalcium silicate cement to fill the canal. At the 1, 4, 8, 12, and 24-month follow-ups, the patient remained asymptomatic, and there was no radiographic evidence of root or periapical pathosis on either tooth. The root-end of the donor tooth transplanted to the #8 site continued to develop. This case report highlights successful interdisciplinary management of two forms of root resorption using modern treatment strategies that provided immediate function and esthetics to the maxillary central incisors in a young patient following trauma. Tooth autotransplantation is one of the methods used for tooth loss rehabilitation in children. Premolars are usually used as autotransplants requiring esthetic alterations. The purpose of this paper is to present an innovative, alternative and inexpensive way to restore auto-transplanted teeth using the crown of the tooth of the recipient site. A seven-year-old male with a non-contributory medical history presented with an intrusion of his permanent maxillary right central incisor. The tooth underwent orthodontic extrusion using fixed appliances but eventually showed signs of ankylosis. At 10 years old, autotransplantation of a premolar to substitute the ankylosed incisor was undertaken. Six weeks after the autotransplantation, the premolar was restored to an incisor by modifying the extracted ankylosed tooth to a veneer for the transplanted tooth, providing functional and esthetic restoration. Statement of Problem: Emerging evidence indicates that the emergence profile and subsequent restorative angle play a crucial role in clinical outcomes and can potentially affect the development and progression of peri‐implant diseases. However, the traditional evaluation of the emergence profile and angle has been limited to mesial and distal sites using periapical x‐rays, without considering the buccal sites. Purpose: To describe a novel 3D method to estimate the emergence profile and restorative angles around single implant‐supported crowns including buccal sites. Materials and Methods: A total of 30 implant‐supported crowns (11 molars, 8 premolars, 8 central incisors and 1 canine) were extra‐orally scanned using an intraoral scanner and the STL files produced were imported into a 3D software. The crown/abutment interface of each crown was delineated, and apico‐coronal lines were automatically drawn following the shape of the crown. Three reference points were defined on the apico‐coronal lines at the transition edge of the biological (BC) and the esthetic zone (EC) and the resulting angles were then calculated. The reliability of the measurements (2D and 3D) were assessed using the intraclass correlation coefficient (ICC). Results: In anterior restorations, the mean angle of the esthetic zone amounted to 162 ± 14° at mesial sites, to 140 ± 10° at buccal sites and to 163 ± 11° at distal sites. The corresponding angles at the biological zones, amounted to 155 ± 13° at mesial sites, 139 ± 15° at buccal sites and 157 ± 5° at distal sites. In posterior restorations, the mean angle of the esthetic zone amounted to 162 ± 12° at mesial sites, to 157 ± 13 at buccal sites and to 162 ± 11 at distal sites. The corresponding angles at the biological zone, amounted to 158 ± 8 at mesial sites, 150 ± 15° at buccal sites and 156 ± 10 at distal sites. The ICC for all measurements ranged between 0.77 and 0.99 indicating a good intra‐examiner reliability. Conclusion: Within the limitations of the present study, the 3D analysis seems to be a reliable and applicable method for the quantitative evaluation of the emergence profile in daily practice. Future randomized clinical trials are needed to assess whether a 3D analysis with the ensuing the emergence profile serves as a predictor for clinical outcomes. Clinical Significance: The development and implementation of a 3D workflow will provide technicians and dentists with the ability to assess the restorative angle of implant‐supported restorations during the provisional phase and the final restoration. Objective The aim of this study was to assess the frequency of mandibular second premolar (MnP2) distoangulation among orthodontic patients and the angular changes after a longitudinal follow-up. Methods The sample was collected from the orthodontic records of 865 patients. The distoangulation group was composed of 42 patients (mean age 9.29 ± 1.24 years, 16 male, 26 female) with distoangulation of MnP2. The control group was composed of 32 patients (mean age 9.38 ± 1.10 years, 15 male, and 17 female) without distoangulation of MnP2. Panoramic radiographs taken in the mixed (T1) and the early permanent dentition (T2) were analyzed in both groups. Longitudinal angular changes (distal angle θ and premolar-molar angle γ), degree of root formation, second premolar depth, and prevalence of associated dental anomalies were analyzed. Intergroup comparison was performed with Mann–Whitney, t-tests, and chi-square tests (P  < 0.05). Results The prevalence of MnP2 distoangulation in the mixed dentition was 4.85%. The distoangulation group showed a smaller initial distal angle (59.34o ± 8.41) when compared to control group (79.88o ± 7.60). The spontaneous eruption of the MnP2 with distoangulation was observed in 76.57% of the sample. MnP2 distoangulation was significantly associated with agenesis of its antimere, small maxillary lateral incisors, and deciduous molar infraocclusion. Limitations Severe cases of MnP2 distoangulation were absent in this study. Conclusions The frequency of MnP2 distoangulation among orthodontic patients was 4.85%. Mild to moderate distoangulated Mnp2 spontaneously uprighted from the mixed to the permanent dentition. Small lateral incisors, second premolar agenesis, and infraocclusion of deciduous molar were frequently found in cases with MnP2 distoangulation. [ABSTRACT FROM AUTHOR]

Dental anomalies are clinically evident abnormalities that cause a variety of dental problems. Depending on the tooth location, dens evaginatus indicates a posterior tooth abnormality, while talon cusp explains an anterior tooth anomaly. Identifying the problem and initiating treatment requires careful observation and proper investigations. Here, the authors present the case of a nine-year-old female child who had an H-shaped talon cusp or barrel-shaped bilateral maxillary lateral incisors in the form of premolars. The report includes clinical and radiographic interpretation. Associated problems and various treatment options are also explained. Although dental anomalies affecting the teeth are relatively common, the presentation of barrel-shaped or premolar-shaped bilateral maxillary lateral incisors is uncommon. Knowledge about the prevalence and distribution of such anomalies would aid various dental experts in early identification and recognition of dental malformations, as well as in complete treatment planning.

Dentoalveolar trauma is an important etiologic factor for tooth loss. A multidisciplinary approach to treatment is necessary because the functional, esthetic, and psychosocial problems that result from dentoalveolartrauma can significantly influence the patient's quality of life. The objective of this article istodescribethe multidisciplinarytreatment of a 39-year-old man who experienced dentoalveolar trauma Ina bicycle accident. Clinical and tomographic examinations revealed the avulsion of the maxillary left central incisor as well as coronal fractures of the other incisorsand themaxillary right canine. The maxillary right central incisor had a subgingival fracture, and the treatment included endodontic treatment followed by orthodontic extrusion using a temporary intraradicular retention pin and an orthodontic elastic band attached to a provisional prosthesis foranchorage. A dental implant and a connectivetissue graft were placed at the site of the avulsed tooth. Subsequently, 5 zirconia crowns were fabricated using a digital workflow. In some cases of dentoalveolar trauma, orthodontic extrusion accomplished with theaid of the provisional prosthesis can be a conservative, fast, and economical option to preserve periodontal tissues and restore the biologic width. In the present case, the combination of orthodontic extrusion, implant placement, and a digital workflow ensured the precision, comfort, and esthetics of the final restorations, improving the quality of life of the patient. Aim: The purpose of this study was to evaluate the effect of the density and the thickness of the cortical and the cancellous bone at selected inter-radicular areas in subjects with different facial growth patterns using cone beam computed tomography (CBCT) in order to choose the optimal area for miniscrew insertion. Materials and methods: From 150 CBCT scans, 45 scans were included in the study. The subjects were categorized into three groups based on their skeletal growth pattern according to SN-GoMe angle and facial height index. Cortical and cancellous bone density and thickness were measured at the selected inter-radicular areas. Results: Compared to the other two groups, the hyperdivergent group had thinner cortical bone in the anterior region of the maxilla between the central and the lateral incisors on the buccal side at 4 mm from the alveolar crest (P-value: 0.012) and on the palatal side at 7 mm from the alveolar crest (P-value: 0.030). Cancellous bone density values in these areas were higher in subjects with hypodivergent and hyperdivergent growth pattern. Furthermore, in hyperdivergent group less dense cortical bone in the posterior region of the maxilla on the palatal side between the second premolar and the first molar (p-value: 0.020) and on the buccal side between the first molar and the second molar (p-value: 0.038 & 0.047) was observed. No significant differences were found in the mandible between the three groups. No significant differences were found between the male and the female subjects. Conclusion: Hyperdivegents presented thinner cortical bone in the anterior of the maxilla between the central and the lateral incisors. Less dense cortical bone was found between maxillary second premolar and first molar on the palatal side and also between the maxillary first molar and the second molar on the buccal side in this group too. Normal showed higher density values in the posterior of the maxilla compared to the other two groups. No significant differences were found among three groups in mandible. [ABSTRACT FROM AUTHOR]

Background: Perforation of the lingual cortex while placing dental implants in the interforaminal region of the mandible can cause severe hemorrhage. The purpose of this study was to evaluate the features of mandibular lingual foraminals (MLFs) and locational relationship between them and lingual mucosal vessels (LMVs) by CT/MRI fusion volumetric images. Methods: 37 images within complete anterior mandibular region using both MSCT and three-dimensional volumetric interpolated breath-hold examination (3D-VIBE) MRI were taken from our imaging archives. After exclusion of 11 for lesions or artifacts, 26 CT/MRI fusion volumetric images were included to evaluate the frequency, diameter, and position of MLFs. The anterior mandibular region was divided into 4 equal segments under each teeth, and 40 regions were got from C5 to D5. Furthermore, the positional relationship between MLFs and LMVs was analyzed in this coordinate system. Results: 62 MLFs (73.81%) were located below the incisors, followed by premolars (21.43%) and canines (4.76%). Female bias, the mean diameter of the female was 0.08 mm while the male was 0.21 mm. The total number of LMVs was most distributed on lingual side of C1 and D1. According to Spearman's correlation coefficient, the location of MLFs was related to LMVs. The MLFs in fourth segment of D1 were positively moderately correlated with LMVs in fourth segment of D4, while the MLFs in third segment of C1 showed a weak positive correlation with LMVs in third segment of D4. Conclusions: The features and the correlation between MLFs and LMVs in CT/MRI fusion volumetric images may offer reference to dentists when only MLFs can been seen on routine preoperative CT examination of implants. Introduction: Clear aligner therapy has become increasingly popular in recent years, although it has encountered several difficulties in premolar extraction treatment. These difficulties include anterior dentition, lingual tipping and extrusion. The design of the present clinical scheme usually set a tiny space between the anterior teeth before retraction in order to obtain an ideal outcome. The objective of our research was to analyze the effect of the existing spaces during retraction. Methods: Models including maxillary dentition without first premolars, maxilla, periodontal ligaments, gingiva, or aligners were constructed and imported to an ANSYS workbench. Five groups of models were created: without spaces and with 0.25, 0.50, 0.75 and 1.00 mm spaces between the anterior dentition. A 0.20 mm retraction step was applied to all the groups. Results: As the spaces between the anterior dentition increased, the bowing effect of the aligner caused by the passive forces decreased gradually. Accordingly, the degree of extrusion of the anterior dentition was alleviated significantly, while sagittal movement was reduced. However, the overall movement tended to be a bodily displacement rather than tipping. Meanwhile, maximum Von Mises stress of the periodontal ligaments (PDLs) was markedly decreased. Conclusion: These analyses indicate that spaces between the anterior dentition during anterior retraction are beneficial for decreasing the tendency for extrusion of the anterior dentition and require provision of anchorage. Dental anomalies are clinically evident abnormalities that cause a variety of dental problems. Depending on the tooth location, dens evaginatus indicates a posterior tooth abnormality, while talon cusp explains an anterior tooth anomaly. Identifying the problem and initiating treatment requires careful observation and proper investigations. Here, the authors present the case of a nine-yearold female child who had an H-shaped talon cusp or barrel-shaped bilateral maxillary lateral incisors in the form of premolars. The report includes clinical and radiographic interpretation. Associated problems and various treatment options are also explained. Although dental anomalies affecting the teeth are relatively common, the presentation of barrel-shaped or premolar-shaped bilateral maxillary lateral incisors is uncommon. Knowledge about the prevalence and distribution of such anomalies would aid various dental experts in early identification and recognition of dental malformations, as well as in complete treatment planning. The aim of this study was to show a case of an impacted canine in an adult patient with agenesis of the maxillary lateral incisor treated with clear aligners (CA). A 19-year-old male with a persistence of 5.3 and absence of 1.2 came to our department of the School of Orthodontics at the University of Federico II in Naples and asked for an aesthetic treatment. The Canine First approach was used to surgically expose the canine and pull it into the dental arch. In order to ensure long-term aesthetic, periodontal, and occlusal results, a treatment with CAs to close the space through the mesial placement of the canine and the enameloplasty of the tooth crown was performed. Background: Clinicians agree that obtaining and retaining good treatment results for missing maxillary central incisors owing to trauma is not easy. Management of adult patients with permanent maxillary central incisor loss who visit the clinic with high expectations for aesthetics and function pose a significant diagnostic dilemma. Therefore, esthetic and functional outcomes should be taken into consideration when deciding the proper treatment method. The treatment described in this study aimed to reestablish smile esthetics by proposing an effective multidisciplinary clinical approach that includes orthodontic-prosthetic-periodontal procedures, optimally reduced lip protrusion, center dental midlines, and establishment of stable occlusion. Case presentation: The patient was a 19-year-old adult female with bimaxillary arch protrusion who had been wearing removable dentures for several years since the loss of her maxillary central permanent incisors. A multidisciplinary treatment including the extraction of two mandibular primary premolars was adopted. The treatment plan consisted of orthodontic space closure by shifting the adjacent teeth towards the central incisor spaces combined with appropriate morphologic remodeling and gingival reshaping to obtain good aesthetic and functional results. The duration to complete the orthodontic treatment was 35 months. Clinical and radiographic results after treatment suggested smile harmony with an improvement in the facial profile, good function of the occlusion, and a positive effect on bone remodeling in the area of the missing incisors during orthodontic tooth movement. Conclusions: This clinical case illustrated the necessity for using multidisciplinary methods involving orthodontic, prosthodontic, and periodontic procedures to treat an adult female patient with bimaxillary arch protrusion and long-term absence of anterior teeth due to severe trauma. Background/Aim: There is a little research on the experiences and opinions of patients who have had autotransplantation of a tooth. The aim of the study was to assess the satisfaction of patients who underwent the autotransplantation of a developing premolar to replace a traumatised maxillary central incisor. Materials and methods: Eighty patients (with a mean age of 10.7‐years) and 32 parents were surveyed with 13 and 7 questions, respectively, to determine their opinions about the surgery, post‐operative period, orthodontic and restorative treatment they had received. Results: Patients and their parents were very satisfied with the outcomes of the autotransplantation treatment. The majority of patients and all parents declared that they would choose this treatment again if needed. Patients who already had aesthetic restoration of the transplanted toothscored significantly better in position, similarity to other teeth, alignment and aesthetics, when compared to patients before restoration of the premolar to the shape of incisor. Patients after orthodontic treatment considered the alignment of the transplanted tooth between the adjacent teeth as better when compared to patients before or during their orthodontic treatment. Conclusions: Autotransplantation of developing premolars to replace traumatized maxillary central incisors proved to be a well‐accepted treatment option. A delay of restoration of the transplanted premolars to the shape of the maxillary incisors did not have a negative impact on the satisfaction with the treatment. [ABSTRACT FROM AUTHOR]

Aims: To analyse the histological structure and histomorphometric characteristics of human hard palatal mucosa in order to determine the donor site of choice for connective tissue grafts from a histological point of view. Materials and Methods: Palatal mucosa samples from six cadaver heads were harvested at four sites: incisal, premolar, molar and tuberosity. Histological and immunohistochemical techniques were performed, as was histomorphometric analysis. Results: In the current study, we found that the density and size of cells were higher in the superficial papillary layer, whereas the thickness of the collagen bundles increased in the reticular layer. Excluding the epithelium, the mean percentage of lamina propria (LP) and submucosa (SM) was 37% and 63%, respectively (p <.001). LP thickness showed similar values in the incisal, premolar and molar regions, and a significantly greater thickness in tuberosity (p <.001). The thickness of SM increased from incisal to premolar and molar, disappearing in the tuberosity (p <.001). Conclusions: As dense connective tissue of LP is the tissue of choice for connective tissue grafts, the best donor site from a histological point of view is tuberosity because it is composed only of a thick LP without the presence of a loose submucosal layer. Aim: To evaluate the influence of the abutment material (zirconia vs. titanium) on the long‐term aesthetic and clinical outcomes of implant‐supported restorations. Materials and Methods: In 30 patients, a single implant‐supported restoration with either a zirconia or a titanium abutment was placed in the anterior maxilla (incisors, canines, and bicuspids). Aesthetic (Implant Crown Aesthetic Index or ICAI), clinical, radiographic, and patient‐centred outcomes were recorded at baseline (1 month after final restoration), 1 year, and 5 years of follow‐up. This study was registered in ClinicalTrials.gov (NCT02315794). Results: Twenty‐five subjects completed the follow‐up visits at 1 and 5 years. ICAI values showed statistically significant better aesthetic outcomes when zirconia abutments were used compared to titanium abutments. Between 1 and 5 years, the aesthetic sub‐analysis of the crown component worsened but the mucosal sub‐analysis improved. There were no significant changes in bone levels, but the plaque index, bleeding on probing, and probing depths worsened in both groups. Young patients with a severely traumatized maxillary central incisor have limited possibilities to restore its loss. Autotransplantation of immature premolars has been documented as a predictable long-term outcome with the potential for preservation and regeneration of alveolar bone. They can also be successfully reshaped to the central incisor's morphology using direct composite build-ups. However, tooth transplantation is a technique sensitive procedure and requires good case selection, an understanding of relevant tooth transplantation biological principles and careful execution of the optimal surgical and follow-up protocols. Orthodontists play an important role in cooperation with the surgeon before and after surgery. This includes selection of suitable candidates based on orthodontic evaluation of occlusion and indications for premolar extraction, opening adequate space before transplantation and post-surgical alignment of the transplant for an optimal reshaping and establishing a stable occlusion. The aim of the paper is to present clinical application of autotransplanation of immature premolars to replace a traumatized central incisor in growing patients and to discuss the role of an orthodontist in the interdisciplinary team. BACKGROUND: Transverse maxillary deficiency is one of the most common skeletal anomalies. The incidence of posterior crossbite caused by maxillary deficiency is between 2.7% and 23.3%. Unilateral posterior crossbite is more common than bilateral crossbite. The most common treatment for skeletal posterior crossbite is rapid maxillary expansion (RME), in which the base of the maxillary bone is expanded by separating the midpalatal suture. OBJECTIVE: This study compares the biomechanical effects of three different RME appliances, especially the effects on the midline, and evaluates the usability of the modified asymmetric RME (ARME) appliance for treating unilateral crossbites. METHODS: Three scenarios were created with skull models using three different appliances: (1) conventional-bonded RME appliance; (2) full-cap splint RME appliance, with all teeth covered with acrylic; and (3) ARME, with all teeth on the right side and premolars and molars on the left side covered with acrylic. The finite element method was used to assess stress levels and displacements in all models after applying a 5-mm horizontal displacement to the RME screw. RESULTS: The lateral transverse movement of the first molars was greater with the conventional RME appliance than with the full-cap splint RME appliance. The lateral transverse movement of the first molar was greater on the left than on the right side with the ARME. The lateral transverse movement of the central incisors was greater with the full-cap splint RME appliance than with the conventional RME appliance. The lateral transverse movement of the central incisor was greater on the right than on the left side with the ARME. CONCLUSION: Asymmetrical RME appliance increases unilateral expansion compared to other appliances. Therefore, it should be used in cases of unilateral posterior crossbite. The loss of an anterior tooth because of avulsion has been reported in up to 3% of dental injuries. Management alternatives, such as implant-supported restorations and a fixed partial denture, are contraindicated in growing patients because of the continuous growth of the alveolar process. At the same time, orthodontic treatment for gap closure will result in asymmetry and will require adjustment of the adjacent healthy teeth. Therefore, restoring a missing tooth imposes a treatment challenge, especially in children and young adults. Tooth autotransplantation is a treatment modality with high reported survival and success rates that overcome these mentioned limitations. It might also help to preserve the alveolar bone and the soft tissues. This case report describes a multidisciplinary approach for managing a 13-year-old boy who lost his right maxillary central incisor because of a fall. Management included autotransplantation of the left maxillary second premolar to the site of the lost right maxillary central incisor, management of external inflammatory resorption with an endodontic treatment, orthodontic treatment, and aesthetic restoration of the area with composite resin crowns that can be adapted to the expected changes of the jaws during the craniofacial growing period. Objectives: Wax typodonts are widely used as pre‐clinical teaching tools to test and research the complex force systems created by archwire activations, however, a limitation is the inability to quantify the resultant statically indeterminate tooth movements. The aim of this study was to develop an analogue‐to‐digital typodont system to quantify the effects of archwire activations on individual typodont teeth in three dimensions. Methods: The typodont system was developed using CAD/CAM technology. Posterior expansion, squared, tapered, asymmetrical arch forms and reversed curve of Spee activations were tested with three repeats. The resulting three‐dimensional movements of individual typodont teeth were assessed with six degrees of freedom (df). Cartesian coordinate systems were set for each tooth. Mesio‐distal, bucco‐lingual and intrusive–extrusive movements were assessed as changes in the location of the geometrically estimated centre of resistance in the x, y and z axes, respectively. Torque, tip and rotation movements were assessed as the rotations around the mesio‐distal, bucco‐lingual and intrusive–extrusive axes, respectively. Results: Individual typodont tooth displacements from each activation could reliably be described with six df. The transversal‐to‐sagittal movement ratio ranged from 2:1 to 7:1, depending on the activation. Asymmetrical arch form activations caused a midline shift and affected the lateral overjet. Reversing the curve of Spee led to intrusion of incisors and second molars, extrusion of premolars and first molars and pronounced first and third order effects. Conclusion: The digital typodont system is a promising teaching tool. The tested activations have implications in three dimensions, which should be considered when planning tooth movements. Objective: The objective of this study is to present a case report in which the injectable composite resin technique was used as a restorative alternative for dental re-anatomization in a patient with cleft lip and palate and aesthetic complaints. Materials and Methods: The treatment plan included the re-anatomization of the maxillary premolars and canines using a flowable composite resin. This resin was injected and cured through a transparent matrix, which was a copy of the diagnostic wax-up model. Some parameters such as application time and marginal adaptation were also observed when performing the restorations. Additionally, old composite resin restorations on the upper lateral incisors were replaced using the incremental technique with conventional resins, which helped to assess color stability and fracture/wear deterioration for both restorative techniques. Results: The clinical case report shows that the injectable technique was a simple and quick method for restoring the anatomy of teeth (shape and contour) in one session, since the injectable resin can be easily applied in interproximal areas without the need to manually sculpt the resin. In this case, no clinical, visual, or photographic differences were found in marginal discoloration, color stability, and fracture/wear deterioration for the two restorative techniques after one year of follow-up. Conclusions: The professional may have another clinical option for restorative treatment in the case of small re-anatomizations. In addition, the injectable technique seems to require less operator skill and chair time and better marginal adaptation in cases of small anatomical changes. [ABSTRACT FROM AUTHOR]
