Three-dimensional plane wave imaging (PWI) with a 2-D array has been studied for ultrafast volumetric imaging in medical ultrasound. Compared to 2-D PWI, 3-D PWI requires the transmission of an increased number of plane waves (PWs) to scan a volume of interest and achieve transmit dynamic focusing in both the lateral and elevational directions. To reduce the number of PW angles for a given 2-D angular range by mitigating the grating lobe level, we propose two aperiodic patterns of PW angles: concentric rings with a uniform radial interval and the well-known sunflower pattern. Both patterns are validated to provide uniform angle distributions without regular periodicity, and thereby reduce the grating lobe level compared to a periodic angle distribution with the same number of PW angles. Simulation studies show that the aperiodic patterns enhance the contrast of B-mode images by approximately 3-6 dB over all depths. This enhancement implies that the aperiodic angle sets can increase the volume rate by approximately 2-6 times compared to the periodic angle set at the same contrast and spatial resolution.

Objectives: The purpose of this study was to investigate the added value of diffuse optical tomographic categories combined with conventional sonography for differentiating between benign and malignant breast lesions., Methods: In this retrospective database review, we included 145 breast lesions (116 benign and 29 malignant) from 145 women (mean age, 46 years; range, 16-86 years). Five radiologists independently reviewed sonograms with and without a diffuse optical tomographic category. Each lesion was scored on a scale of 0% to 100% for suspicion of malignancy and rated according to the American College of Radiology Breast Imaging Reporting and Data System classification. Diagnostic performance was analyzed by comparing area under receiver operating characteristic curve values. Reader agreement was assessed by intraclass correlation coefficients., Results: In the multireader multicase receiver operating characteristic analysis, adding a diffuse optical tomographic category to sonography improved the diagnostic accuracy of sonography (mean areas under the curve, 0.923 for sonography alone and 0.969 for sonography with diffuse optical tomography; P = .039). The interobserver correlation was also improved (0.798 for sonography alone and 0.904 for sonography with diffuse optical tomography). The specificity increased for 4 reviewers from a mean of 19.5% to 45.8% (P < .001 for reviewers 1-4; P = .238 for reviewer 5) with no significant change in the sensitivity. When the diffuse optical tomographic category was applied strictly, the specificity increased for all reviewers from a mean of 19.5% to 68.3% (P < .001 for all reviewers) with no significant change in the sensitivity., Conclusions: The addition of diffuse optical tomographic categories to sonography may improve diagnostic performance and markedly decrease false-positive biopsy recommendations., (© 2015 by the American Institute of Ultrasound in Medicine.)

It is desired that the same imaging functional modules such as beamformation, envelope detection, and digital scan conversion (DSC) are employed for the efficient development of a cross-sectional photoacoustic (PA) and ultrasound (US) dual-modality imaging system. The beamformation can be implemented using either delay-and-sum beamforming (DAS-BF) or adaptive beamforming methods, each with their own advantages and disadvantages for the dual-modality imaging. However, the DSC is always problematic because it causes blurring the fine details of an image, e.g., edges. This paper demonstrates that the pixel based focusing method is suitable for the dual-modality imaging; beamformation is directly conducted on each display pixel and thus DSC is not necessary. As a result, the artifacts by DSC are no longer a problem, so that the proposed method is capable of providing the maximum spatial resolution achievable by DAS-BF. The performance of the proposed method was evaluated through simulation and ex vivo experiments with a microcalcification-contained breast specimen, and the results were compared with those from DAS-BF and adaptive beamforming methods with DSC. Background: The aim of this study was to evaluate the diagnostic utility of PAI at detecting thyroid microcalcifications at 700 nm laser wavelengths., Methods: This study included 36 resected samples in 18 patients. To evaluate the PA manifestation of microcalcifications in PAI, gray level histogram and co-occurrence matrix (COM) texture parameters were extracted from the 3 fixed ROI US and PA images, respectively, per sample. We compared the textural parameters obtained from specimen PAIs between samples with punctate microcalcifications on specimen radiography and those without microcalcifications., Results: On specimen US, the mean value (2748.4 ± 862.5) of samples with microcalcifications on specimen radiography was higher than that (1961.9 ± 780.2) of those without microcalcifications (P = 0.007). However, there were no significant differences in textural parameters obtained from specimen PAIs between samples with punctate microcalcifications on specimen radiography and those without when applying both the mean value of the three slices of thyroid specimens and the value of the thyroid specimen slice which had the highest value of the mean values in specimen US., Conclusion: PAI did not show significant PA contrast on thyroid microcalcifications indicating that the experimental setup and protocols should be enhanced, e.g., method of complete blood rejection from ex vivo specimens, the multi-wavelength spectroscopic PA imaging method which can solely extract the PA signal from microcalcifications even with high spectral interferences, or PA imaging with narrower slice thickness using 2-dimensional array transducer, etc.

In medical ultrasound imaging, lateral resolution is limited when using a fixed transmit focusing. Various synthetic aperture (SA) techniques, in which two-way dynamic focusing is enabled by utilizing prebeamformed radio-frequency (RF) data have been proposed for improving the spatial resolution. However, SA methods were not extensively evaluated in terms of their clinical performance. In this paper, a phantom and an in vivo evaluation of the SA method with bidirectional pixel-based focusing (BiPBF) is presented in comparison with the conventional beamforming. The performance of the proposed SA-BiPBF was assessed with a blind study and the established breast imaging-reporting and data system (BI-RADS), in addition to measuring contrast-to-noise ratio (CNR) and signal-to-noise ratio (SNR). Prebeamformed RF data were acquired from a tissue mimicking phantom (Model 040, CIRS Inc., Norfolk, VA, USA) and from patients with breast lesions by using a commercial ultrasound scanning system with a linear array transducer equipped with a research package and parallel data acquisition system (SonixTouch, SonixDAQ, and L14-5/38, Ultrasonix Corp., Canada). In phantom and in vivo experiments, a default setting of a breast preset was applied (e.g., the center frequency of 10 MHz and acoustic output of MI = 0.66). In phantom experiment, the SA-BiPBF method showed higher CNR and SNR values compared to the conventional method (3.4 and 23.9 dB versus 3.1 and 15.8 dB, respectively). In addition, the lateral resolution and penetration depth were increased by 95.4% and 40.3%, respectively. Consistent with the phantom experiment, in the in vivo experiment with ten patients, the CNR value for the SA method was 3.3 ± 0.5 compared to 2.8 ± 0.8 for the conventional method. Similarly, the SNR values with the SA-BiPBF and conventional methods were 34.0 ± 3.6 and 27.2 ± 3.4 dB, respectively. From the experiments, it was shown in side-by-side comparisons that the image quality of the SA-BiPBF method was considerably improved in both phantom and in vivo breast images. However, the SA-BiPBF image showed different features compared to the conventional one in the in vivo experiments. These features are resulting from the increased image quality of the SA-BiPBF method but are not always perceived as improvements by the radiologists.

An ultrasound microbubble contrast agent is a promising technique in clinical diagnosis because ultrasound in combination with microbubbles enhances the ultrasound backscatter to produce an increased contrast images. In this study, we developed phospholipid-based microbubbles showing a relatively narrow size distribution of 0.8-1.3 microm. The optimal resonance frequency of developed microbubbles was determined to be 2.5-3.0 MHz by measuring echo signals at various frequencies. Ultrasound harmonic imaging was performed in a vessel phantom at the optimal resonance frequencies. Microbubble contrast-enhanced ultrasound images visualized a vessel tube clearly and demonstrated much improved image quality, compared to the control. In conclusion, the ultrasonography in the harmonic mode is capable of maximally resonating micrbubbles with a narrow size distribution at a specific frequency for enhanced ultrasound imaging.

In this paper, a new compounded direct pixel beamforming (CDPB) method is presented to remove blurring artifacts introduced by ultrasound scan conversion. In CDPB, receive focusing is directly performed on each display pixel in Cartesian coordinates using the raw RF data from adjacent transmit firings so that artifacts from the scan conversion can be removed. In addition, the energy variations resulting from the distance between the transmit scanline and display pixel are compensated by utilizing the gain factor obtained from the ultrasound beam pattern. The proposed CDPB method was evaluated using simulation and in vivo liver data acquired by a commercial ultrasound machine equipped with a research package. The experimental results showed that the proposed CDPB method improved the information entropy contrast (IEC) by 23.6% compared with the conventional scan conversion method and it reduced the blocking artifacts factor (BAF) by 16.4% over the direct pixel-based focusing method. These results indicate the proposed new direct pixel beamforming method could be used to enhance image quality in medical ultrasound imaging.

Real-time photoacoustic (PA) imaging involves beamforming methods using an assumed fixed sound speed, typically 1540 m/s in soft tissue. This leads to degradation of PA image quality because the true sound speed changes as PA signal propagates through different types of soft tissues: the range from 1450 m/s to 1600 m/s. This paper proposes a new method for estimating an optimal sound speed to enhance the cross-sectional PA image quality. The optimal sound speed is determined when coherent factor with the sound speed is maximized. The proposed method was validated through simulation and ex vivo experiments with microcalcification-contained breast cancer specimen. The experimental results demonstrated that the best lateral resolution of PA images of microcalcifications can be achieved when the optimal sound speed is utilized.

An efficient ultrasound beamforming algorithm based on sigma-delta modulation (SDM) is proposed, which performs dynamic focusing at the Nyquist rate as in conventional delay-sum beamformers. Specifically, a block of consecutive SDM samples for each focal point is selected on each channel according to the focusing delay. The data blocks for all channels are then added up on a bit-to-bit basis and the resulting block of multibit samples is fed to the demodulation filter to produce the focused signal. Since the SDM samples in each data block are not altered at all, the proposed method does not cause any signal distortion. Two hardware structures are also proposed for minimum hardware implementation of the proposed error-free SDM beamforming method. It is verified that compared to the delay-sum beamformer using 10-bit ADCs, the proposed algorithm provides almost the same image quality with a greatly reduced hardware complexity. In addition, it can be implemented with a lower hardware complexity than that of the conventional SDM beamformer when the number of channels is 64 or less.

Harmonic coded excitation techniques have been used to increase SNR of harmonic imaging with limited peak voltage. Harmonic Golay coded excitation, in particular, generates each scan line using four transmit-receive cycles, unlike conventional Golay coded excitation method, thus resulting in low frame rates. In this paper we propose a method of increasing the frame rate of said method without impacting the image quality. The proposed method performs two transmit-receive cycles using QPSK code to ensure that the harmonic components of incoming signals are Golay coded and uses harmonic quadrature demodulation to extract compressed second harmonic component only. The proposed method has been validated through mathematical analysis and MATLAB simulation, and has been verified to yield a limited error of -52.08dB compared to the ideal case. Therefore, the proposed method doubles the frame rate compared to the existing harmonic Golay coded excitation method without significantly deteriorating the image quality.

Multi-pulse techniques like CPS (contrast pulse sequence) and TPS (triplet pulse sequence) are the most popular methods for separating the 3rd harmonic signals from received signal. Those two methods, however, transmit a pulse at least three times along each scanline with different phase and amplitude, which results in the frame rate reduction. In this paper, we propose a technique using two pulses whose phase difference is 90 degrees and a simple digital filter. The second harmonic signal is eliminated by summing two received signals as their phase difference becomes 180 degrees and then the fundamental signals are eliminated by using a digital filter. Computer simulations are performed for different values of signal bandwidths and filter specifications. The results show the maximum error is -35.5 dB compared to TPS.

The compression error of post-compression based coded excitation techniques increases with decreasing f-number, which causes the elevation of side-lobe levels. In this paper, a post-compression based coded excitation technique with reduced compression errors through dynamic aperture control is proposed. To improve the near-field resolution with no frame rate reduction, the proposed method performs simultaneous transmit multi-zone focusing using two mutually orthogonal complementary Golay codes. In the proposed method, the two mutually orthogonal sequences of length 16 are simultaneously transmitted toward two different focal depths, which are separately compressed into two short pulses on receive after dynamic focusing is performed. After carrying out the same transmit-receive operation for the same scan line with the complementary set of the orthogonal Golay codes, a single scan line with two transmit foci is obtained. The computer simulation results using a linear array with a center frequency of 7.5 MHz and 60% 6 dB bandwidth show that the range side-lobe level can be suppressed below -50 dB, when f-number is maintained not smaller than 3. The performance of the proposed scheme for a smaller f-number of 2 was also verified through actual experiments using a 3.85 MHz curved linear array with 60% 6 dB bandwidth. Both the simulation and experimental results show that the proposed method provides improved lateral resolution compared to the conventional pre-compressed and post-compression based coded excitation imaging using Golay codes.
