Intestinal ischemia/reperfusion (I/R) induces mucosal barrier dysfunction and bacterial translocation (BT). Neutrophil-derived oxidative free radicals have been incriminated in the pathogenesis of ischemic injury in various organs, but their role in the bacteria-containing intestinal tract is debatable. Primed neutrophils are characterized by a faster and higher respiratory burst activity associated with more robust bactericidal effects on exposure to a second stimulus. Hypoxic preconditioning (HPC) attenuates ischemic injury in brain, heart, lung and kidney; no reports were found in the gut. Our aim is to investigate whether neutrophil priming by HPC protects against intestinal I/R-induced barrier damage and bacterial influx. Rats were raised in normoxia (NM) or kept in a hypobaric hypoxic chamber (380 Torr) 17 h/day for 3 weeks for HPC, followed by sham operation or intestinal I/R. Gut permeability was determined by using an ex vivo macromolecular flux assay and an in vivo magnetic resonance imaging-based method. Liver and spleen homogenates were plated for bacterial culturing. Rats raised in HPC showed diminished levels of BT, and partially improved mucosal histopathology and epithelial barrier function compared with the NM groups after intestinal I/R. Augmented cytokine-induced neutrophil chemoattractant (CINC)-1 and -3 levels and myeloperoxidase activity correlated with enhanced infiltration of neutrophils in intestines of HPC-I/R compared with NM-I/R rats. HPC alone caused blood neutrophil priming, as shown by elevated production of superoxide and hydrogen peroxide on stimulation, increased membrane translocation of cytosolic p47(phox) and p67(phox), as well as augmented bacterial-killing and phagocytotic activities. Neutrophil depletion reversed the mucosal protection by HPC, and aggravated intestinal leakiness and BT following I/R. In conclusion, neutrophil priming by HPC protects against I/R-induced BT via direct antimicrobial activity by oxidative respiratory bursts and through promotion of epithelial barrier integrity for luminal confinement of enteric bacteria.

Ischemic preconditioning has been proved to reduce tissue damages and benefit subsequent organ transplantation. Chronic hypoxic preconditioning was found to increase the levels of lung antioxidants. This study was to test the hypothesis that levels of lung antioxidants might increase after hypoxia which may counteract the insults of free radicals. Female Wistar rats were kept in an altitude chamber (380 torr) 15 h a day for 4 weeks (hypoxia-adapted). Controls were kept at room air pressure (sea-level). After hypoxic preconditioning, no significant difference in the levels of the oxidative markers, malondialdehyde, thiobarbituric acid reactive substances and isoprostane was seen in the lungs of the hypoxia-adapted rats compared to the sea-level controls. Both the activity and protein level of manganese superoxide dismutase were higher in hypoxia-adapted lungs. Lung manganese superoxide dismutase mRNA levels, determined by real-time RT-PCR, were not significantly different in the two groups of rats. When isolated saline-perfused lungs were prepared and treated with xanthine (500 microM) and xanthine oxidase (5 mU/ml), and the levels of free radicals in the perfusate determined by chemiluminescence, less chemiluminescence was seen in the hypoxia-adapted lung perfusate. When the vascular response was determined in this same preparation before or 45 min after xanthine/xanthine oxidase treatment, the filtration coefficient was increased in the sea-level lungs but not in the hypoxia-adapted lungs. We conclude that an increase in superoxide dismutase activity and protein levels is one of the benefits of hypoxic preconditioning.

Objective: Hypoxic pulmonary vasoconstriction (HPV) is a well-known phenomenon to temporarily offset a ventilation/perfusion mismatch. Sustained HPV may lead to pulmonary hypertension. In this protocol, we studied the relationships between the HPV response and oxygen radical release after hypoxia/reoxygenation (H/R) challenge in an isolated perfused lung model., Materials and Methods: We used an in situ isolated rat lung preparation. Two hypoxic challenges (5% CO2-95% N2) were administered for 10 minutes each with administration of antioxidants of superoxide dismutase (SOD; 2 mg/kg), catalase (20,000 IU/kg), dimethylthiourea (DMTU; 100 mg/kg), dimethylsulfoxide (DMSO; 1 mL/kg), or allopurinol (30 mg/kg) between 2 challenges. We measured pulmonary arterial pressure changes before, during, and after H/R challenge. We measured blood concentration changes in hydroxyl radicals and nitric oxide (NO) before and after H/R. mRNA expressions of SOD and catalase in lung tissue were measured after the experiments., Results: Hypoxia induced pulmonary vasoconstriction by increasing pulmonary arterial pressure and consecutive hypoxic challenges did not show tachyphylaxis. Blood concentrations of hydroxyl radicals and NO increased significantly after H/R challenges. mRNA expressions of SOD and catalase increased significantly, however, neither SOD nor catalase showed attenuated effects on HPV responses. Small molecules of DMTU, DMSO, and allopurinol attenuated the HPV responses., Conclusions: H/R induced increases in the expressions of SOD and catalase in lung tissues. DMTU, DMSO, and allopurinol antioxidants attenuated the HPV responses by reducing the oxygen radical release.

Chronic hypoxic (CH) preconditioning reduces superoxide-induced renal dysfunction via the upregulation of superoxide dismutase (SOD) activity and contents. Endotoxaemia reduces renal antioxidant status. We hypothesize that CH preconditioning might protect the kidney from subsequent endotoxaemia-induced oxidative injury. Endotoxaemia was induced by intraperitoneal injection of lipopolysaccharide (LPS; 4 mg kg(-1)) in rats kept at sea level (SL) and rats with CH in an altitude chamber (5500 m for 15 h day(-1)) for 4 weeks. LPS enhanced xanthine oxidase (XO) and gp91phox (catalytic subunit of NADPH oxidase) expression associated with burst amount of superoxide production from the SL kidney surface and renal venous blood detected by lucigenin-enhanced chemiluminescence. LPS induced a morphologic-independent renal dysfunction in baseline and acute saline loading stages and increased renal IL-1beta protein and urinary protein concentration in the SL rats. After 4 weeks of induction, CH significantly increased Cu/ZnSOD, MnSOD and catalase expression (16 +/- 17, 128 +/- 35 and 48 +/- 21, respectively) in renal cortex, and depressed renal cortex XO (44 +/- 16%) and renal cortex (20 +/- 9%) and medulla (28 +/- 11%) gp91phox when compared with SL rats. The combined effect of enhanced antioxidant proteins and depressed oxidative proteins significantly reduced LPS-enhanced superoxide production, renal XO and gp91phox expression, renal IL-1beta production, and urinary protein level. CH also ameliorated LPS-induced renal dysfunction in the baseline and acute saline loading periods. We conclude that CH treatment enhances the intrarenal antioxidant/oxidative protein ratio to overcome endotoxaemia-induced reactive oxygen species formation and inflammatory cytokine release.

Background: Hypoxic preconditioning (HP) confers cytoprotection against ischemia/reperfusion (I/R) injury. This effect is in part attributable to the induction of heme oxygenase (HO)-1. This experiment evaluates liver cell damage after I/R injury in HP rats., Methods: HP rats were prepared by exposure (15 hr/day) to an altitude chamber (5500 m) for 2 weeks. Partial hepatic ischemia was produced in the left lobes for 45 min followed by 180 min of reperfusion. Zinc (Zn) protoporphyrin (PP), a specific inhibitor of HO enzymatic activity, was subcutaneously injected 1 hr before the I/R injury into separate groups of sea-level (SL) control and HP rats. Serum alanine aminotransferase (ALT) levels, liver HO-1 mRNA and protein, and HO enzymatic activity were measured., Results: HO-1 was induced in the livers of rats exposed to HP. The levels of HO-1 mRNA and protein were obviously overexpressed after 2 weeks of HP. HP diminished the elevation of serum ALT levels after I/R injury (83.7+/- 4.9 U/L) when compared with SL controls (280.8+/-19.4 U/L) and HP+ZnPP-pretreated groups (151.3+/-4.4 U/L). The HO activity in treated rats also was correlated with these results (237.9+/-19.8 pmol/mg of protein per hour for the HP group, 164.3+/-12.7 pmol/mg of protein per hour for the HP+ZnPP group, and 182.6+/-8 pmol/mg of protein per hour for the SL controls)., Conclusions: The authors' results indicated that the induction of HO-1 in hypoxic preconditioning played a protective role against hepatic I/R injury.

We explored whether hypoxic preconditioning minimizes oxidative injury induced by overdistension/emptying in the rat bladder. For hypoxic preconditioning, female Wistar rats were placed in a hypobaric chamber (380 Torr) 15 h day(-1) for 28 days. Overdistension was induced by infusion of two times the threshold volume of saline into the bladder and was maintained for 1 or 2 h, followed by drainage/emptying. During overdistension (ischaemia) and emptying (reperfusion) periods, a bursting increase of reactive oxygen species (ROS) from the bladder was originated from the large numbers of infiltrating leucocytes and scattered resident cells, including urothelial, submucosal, and smooth muscle cells. ROS impaired the voiding function by a reduction of bladder afferent and efferent nerve activity and bethanecol- or ATP-induced detrusor contraction. ROS enhanced pro-apoptotic mechanisms, including increases in the Bax/Bcl-2 ratio, CPP32 expression, and poly(ADP-ribose) polymerase (PARP) fragments with subsequent apoptotic cell formation in the insulted bladders. Hypoxia preconditioning up-regulated Bcl-2 expression in the bladder and significantly reduced the levels of ROS and apoptosis detected in the overdistension/emptying bladders and preserved partial voiding function. Bcl-2 up-regulation by hypoxia preconditioning contributes protection against overdistension/emptying-induced oxidative stress and injury in the bladder.

Renal ischaemia releases reactive oxygen species (ROS) in the kidneys. We hypothesized that the kidneys are more resistant to the insult of ROS in chronically hypoxic rats. We thus compared rats kept at sea level (SL) and those that had been adapted to hypoxia (hypoxia adapted, HA) by exposure to an altitude of 5500 m in an altitude chamber for 15 h day-1 for 4 weeks. Xanthine (X, 0.75 mg kg-1) and xanthine oxidase (XO, 24.8 mU kg-1) were injected intrarenally. A lucigenin-enhanced chemiluminescence method was employed to detect the amount of free radicals in renal venous blood samples and on the kidney surface. In the renal venous blood samples, 26.05 (+/- 4.36) x 104 and 10.98 (+/- 1.79) x 104 counts were detected in the SL and HA rats, respectively, after X-XO treatment; these figures were significantly different. On the kidney surface of the SL rats, the free radical count amounted to 12.77 (+/- 1.64) x 104, while that in the HA rats was 8.47 (+/- 0.42) x 104; these figures were also significantly different. There was a significant increase in urine volume and urinary excretion of Na+, K+ and protein after X-XO administration in both groups of rats. However, the effect was greater for the SL rats than for the HA rats. The lipid peroxidation of the kidneys was not significantly different in the two groups of rats. Finally, we found that the activity of superoxide dismutase (SOD) and SOD mRNA were higher in the renal tissue of HA rats. We conclude that the renal response to free radicals is attenuated after chronic hypoxia in rats, and that SOD might play an important role in protecting HA rats from oxidative stress.

One of the proposed pathologic actions underlying brain infarction is excess free radicals resulting from reoxygenation. In this paper we report an investigation of the neuroprotective effect of hypoxic preconditioning on transient focal ischemia-reperfusion injuries in rat brain. Female Wistar rats were subjected to 380 mmHg in an altitude chamber for 15 hours/day. Our ex vivo studies showed that auto-oxidation and iron-induced lipid peroxidation of brain homogenates of the four-week hypoxia-preconditioned rats were significantly lower than those of the normoxic rats. A focal infarction in the cerebral cortex of normoxic rats was consistently observed 24 hours after a 60-minute transient ischemic occlusion of the right middle cerebral artery and bilateral common carotid arteries. Hypoxic preconditioning in fact attenuated cortical infarction in a duration-dependent manner. Induction of the neuroprotection required two weeks of hypoxic preconditioning. Four weeks of hypoxic preconditioning significantly reduced the cortical infarcted area, the elevated lipid peroxidation, and resulted in an acute increase in cytosolic cytochrome c in the infarcted cortex of normoxic rats. The protective effect of four weeks of hypoxic preconditioning lasted seven days under a renormoxic condition. Our data suggest that oxidative stress may result in apoptosis in the transient focal ischemia-reperfusion injuries. Furthermore, hypoxic preconditioning attenuated cortical infarction in the rat brain. Although supplementation of antioxidants may encounter difficulty at the blood-brain barrier, hypoxic preconditioning is very likely to protect CNS targets from oxidative injuries without any barrier.

The neuroprotective effect of intermittent hypoxia on ferrous citrate (iron)-induced oxidative stress was investigated in the nigrostriatal dopaminergic system of rat brain. Female Wistar rats were subjected to 380 mm Hg in an altitude chamber for 15 h/day for 7, 14, or 28 days. Iron was locally infused in the substantia nigra of anesthetized rats. Seven days after infusion, lipid peroxidation was elevated in the infused substantia nigra and dopamine content and tyrosine hydroxylase-positive axons were decreased in the ipsilateral striatum in the normoxic rats. Intermittent hypoxic treatment prevented iron-induced oxidative injuries. Induction of the neuroprotection required 2 weeks. Intracerebroventricular infusion of L-buthionine-[S,R]-sulfoximine (L-BSO), which mimicked a reduced antioxidative condition, aggravated iron-induced oxidative injuries. Intermittent hypoxia ameliorated L-BSO-induced augmentation of iron-induced oxidative injuries. Basal GSH (glutathione) content, GSH/GSSG ratio, superoxide dismutase (SOD) and catalase activities in intact substantia nigra were not altered by intermittent hypoxia. Furthermore, intermittent hypoxia attenuated iron-induced reductions in GSH content, GSH/GSSG ratio, and SOD, iron-induced increase in catalase but had no effect on glutathione peroxidase. Our data suggest that intermittent hypoxia may protect the nigrostriatal dopaminergic system from iron-induced oxidative injuries. Moreover, antioxidative defensive systems may partially contribute to the neuroprotection by intermittent hypoxia.

We previously demonstrated that the pulmonary vascular response to substance P (SP) increased in chronically hypoxic rats. This study explored the temporal increase in reactivity of the pulmonary vascular response to SP and its underlying mechanisms. First, young female Wistar rats were exposed to sea level (SL) or simulated high altitude (HA) for 15 h/day for 3 days, 1 wk, 2 wk, and 4 wk. Lungs were isolated and perfused with 4% bovine serum albumin in Krebs-Henseleit buffer solution. SP (1.5 x 10(-4) M) induced significant increases in pulmonary arterial pressure (P(pa)), venous pressure (P(v)), capillary pressure (P(c)), arterial resistance (R(a)), and filtration coefficient (K(fc)) in SL lungs. Increases in P(pa) and R(a) were significantly augmented in HA lungs, with a temporal increase trend peaking at 2 wk of HA exposure. The selective neurokinin (NK) type 1 (NK1) receptor antagonist SR-14033 significantly attenuated SP-induced increases in P(pa), P(v), P(c), R(a), and K(fc) in SL lungs. In lungs exposed to HA for 2 wk, SR-14033 suppressed the effect of SP on P(pa). Also, chronic hypoxia induced significant increases in NK1 receptors and NK1 receptor mRNA, with a temporal trend. We conclude that chronic hypoxia temporally augments SP-induced vascular responses, which are closely associated with increases in NK1 receptors and gene expression.

Excessive production of nitric oxide (NO) may play a detrimental role in the process of hypoxia-related neuropathology. This study explored whether treatment with melatonin would attenuate the neuropathological changes in the vagal ganglia following a severe hypoxic insult. Thirty minutes prior to hypoxia treatment, young adult rats were pre-treated with melatonin at 5. 25 or 100 mg/kg injected intraperitoneally. Hypoxia was achieved by subjecting the rats to a barometric pressure of 0.2 atm (PO2 = 43 Torr) for 4 hr in an altitude chamber. Nicotinamine adenine dinucleotide phosphatediaphorase (NADPH-d) histochemistry combined with the neuronal nitric oxide synthase (nNOS) immunohistochemistry were used to detect the NADPH-d/nNOS reactivity in the nodose ganglion (NG) at various time points following the hypoxic exposure. In normal untreated rats, about 43% of the neurons in the NG displayed NADPH-d/nNOS reactivity. Following hypoxic exposure, both the percentage and the staining intensity of NADPH-d/nNOS positive neurons in the NG were markedly increased, but these were reduced in longer surviving animals. Quantitative analysis of cell counts revealed that about 17% of the neurons died at 14 days after hypoxia treatment. However, in hypoxic rats given different doses of melatonin pretreatment, neuronal death as well as the frequency and staining intensity of NADPH-d/nNOS reactivity of the nodose neurons were significantly decreased. The effect of melatonin on neuronal survival and NADPH-d/ nNOS expression was dose-dependent. It is therefore suggested that melatonin exerts a neuroprotective effect and may serve as a potential therapeutic strategy for prevention and/or reducing the susceptibility of nodose neurons to NO-mediated hypoxic neuropathy.

The sympathetic nervous system is activated in response to altitude hypoxia and activation of renal sympathetic nerves may cause vasoconstriction and fluid retention. However, renal excretion does not differ significantly between rats exposed to high altitude hypoxia and control rats. We hypothesize that renal response to norepinephrine (NE) is altered after chronic hypoxia. Female Wistar rats weighing 200 to 220 g were exposed to hypoxia in an altitude chamber (5,500 m, 380 torr) 15 hours/day for 4 weeks (HA, high altitude). Our findings showed that systemic infusion of NE (300 micrograms.kg-1.hr-1) produced less diuresis/natriuresis in HA rats that in sea level (SL) controls. With mechanical elevation of arterial blood pressure, both SL and HA rats showed no significant difference in their response to pressure diuresis. Direct intrarenal arterial NE (10 micrograms.kg-1.hr-1) administration reduced renal function more in HA rats than in SL rats. Intrarenal arterial administration of L-arginine (100 micrograms.kg-1.hr-1) did not alter the renal action of NE in HA rats. However, with intrarenal arterial infusion of phosphoramidon (100 micrograms.kg-1.hr-1), NE increased renal response in HA rats to almost the same level as that in SL rats. These results suggest that HA rats may have either an excess renal action of antidiuretic and antinatriuretic factors or an insufficient renal action of diuretic and natriuretic factors during NE administration.

Because chronic hypoxia increases the production of oxygen radicals, we hypothesized that antioxidants attenuate chronic hypoxic pulmonary hypertension. In part 1, we examined the temporal progress in chronic hypoxic pulmonary hypertension in 46 Wistar rats exposed to hypoxia from 0-3 weeks. In part 2, we tested whether antioxidants attenuated chronic hypoxic pulmonary hypertension in 82 rats divided into 10 groups: control, fullerenol-1, U-83836E, dimethylthiourea-1, dimethylthiourea-2, hypoxia, hypoxia + fullerenol-1, hypoxia + U83836E, hypoxia + dimethylthiourea-1, and hypoxia + dimethylthiourea-2. Control animals breathed room air and were injected intraperitoneally with saline for 2 weeks. Fullerenol-1, U-83836E, and dimethylthiourea are antioxidants and were administered intraperitoneally for 2 weeks, except that dimethylthiourea was given either on days 3, 5, and 7 (dimethylthiourea-1), or on days 8, 10, and 12 (dimethylthiourea-2). Hypoxic animals were placed into a hypobaric chamber with a barometric pressure of 380 Torr for 2 weeks. Hypoxia + antioxidant groups were administered antioxidants during hypoxic exposure. We observed a gradual increase in pulmonary artery pressure, the weight ratio of right ventricle to left ventricle plus septum, and hematocrit during the 3 weeks of chronic hypoxia. These hypoxia-induced alterations were significantly attenuated by U-83836E and dimethylthiourea, but not by fullerenol-1. Neither the temporal alterations nor the antioxidant effects can be explained by the change in either tracheal neutral endopeptidase activity or the lung or plasma substance P level, perhaps because of the time lag in sampling. These results indicate that oxygen radicals play an important role in the development of chronic hypoxic pulmonary hypertension.

Multifiber renal afferent nerve activity responds to volume expansion in sea level rats but not in chronically hypoxic (high altitude) rats. We performed single-unit recordings of renal afferent nerve activity to characterize renal sensory receptors and their responses to volume expansion in these animals. Hypoxia was induced by placing Wistar rats in an altitude chamber (380 Torr, 5,500 m) for 4 wk. Spontaneously firing renal R2 chemoreceptor and arterial, ureteropelvic, and venous mechanoreceptors were identified. The basal activity of each receptor was similar among these rats. In response to specific stimulus, the increasing impulse of R2 chemoreceptor was similar between two groups of rats, but the increasing activity of each mechanoreceptor was less in hypoxic rats. When challenged with saline load, R2 chemoreceptor activity decreased, but all mechanoreceptors activated in all rats. Despite similar increases of arterial, renal ureteropelvic, and venous pressure during saline load, the increasing activity of each mechanoreceptor was significantly less in hypoxic rats. These results indicated chronic hypoxia attenuates the sensitivity of renal mechanoreceptors in response to the stimulation of saline load.

Background: We studied the renal action of substance P (SP) in rats chronically exposed to hypoxia (high altitude, HA), compared to control rats kept at sea level (SL)., Methods: Hypoxia was induced by placing female Wistar rats (182-225 g) in an altitude chamber (5500 m) 15 h.d-1 for 4 weeks., Results: Intrarenal arterial infusion of substance P (60 ng.kg-1. h-1) increased the excretion of urine, sodium in both groups of rats, however, the excretion of kallikrein (KK) and the glomerular filtration rate (GFR) were not significantly altered. After aprotinin (10,000 kiu.kg-1. min-1) treatment, kallikrein was depleted, and substance P lost its diuretic action. Spantide, an SP antagonist (6000 ng.kg-1. h-1, I.V.) decreased urine, and urinary excretion of sodium and potassium in SL rats, but not in HA rats. Acute renal denervated diuresis in SL rats was not modified after Spantide administration. Finally, it was found that both SP and SP antagonist did not significantly change the renal parameters in either group of rats after chronic renal denervation., Conclusion: We made the following conclusions: a) endogenous renal action of SP was suggested in SL but not in HA rats; b) the renal action of SP might be through KK release, although urinary KK did not increase after SP administration; and c) SP action is renal nerve dependent in both groups of rats.

1. We have studied the role of kallikrein (KK) in the maintenance of renal function in chronic hypoxic rats (high altitude; HA), compared with control rats kept at sea level (SL). Hypoxia was induced by placing female Wistar rats (198-290 g) in an altitude chamber (5500 m) 15 h/day for 4 weeks. Experiments were also conducted to study the interaction of KK with renal nerve activity and endothelin (ET), two parameters previously shown to be altered in this model. 2. It was found that renal cortex tissue KK activity (TKA) was not significantly different in 10 SL and 10 HA rats. However, the urinary KK activity (UKA) was reduced nearly to half (from 35.2 +/- 4.6 to 18.5 +/- 1.7 pkat/min) in HA rats after 4 weeks of chronic hypoxia. 3. Acute renal denervated diuresis was accompanied by a significant increase in UKA (from 9 +/- 2 to 14 +/- 2 pkat/min in HA and denervated HA rats, respectively; P < 0.05) in HA rats. Intrarenal arterial pretreatment of aprotinin reduced the denervated diuresis. 4. Endothelin (600 ng/kg per h) reduced urine flow, sodium and potassium excretion in the ipsilateral kidney in another 10 SL and 10 HA rats. The extent of the drop of these parameters was significantly less in HA rats. Urinary KK activity was correlated significantly with the measured renal functional parameters (r ranging from 0.472 to 0.612) in SL rats, but was insignificant in HA rats (r ranging from 0.032 to 0.192). 5. We have demonstrated that chronic exposure to hypoxia decreases urinary KK excretion and that KK is involved in acute renal denervated diuresis generated in these animals. The present study suggests that KK plays a partial role in the maintenance of renal function in chronic hypoxic rats.

This report was designed to assess response of the renal nerve activity (RNA) during and after renal ischemia in chronic hypoxic rats. Hypoxia was induced by placing the female Wistar rats in an altitude chamber set at 5500 m for 4 weeks. Simultaneous recordings of left renal efferent (RENA) and afferent (RANA) nerve activity were performed in each pentobarbital-anesthetized rat throughout the experiment. Ischemic renal failure was induced by complete occlusion of the left renal artery for 45 min. During renal arterial occlusion (RAO), RENA gradually decreased while RANA enhanced initially and then this decreased gradually in both sea level (SL) controls and chronic hypoxic (high-altitude; HA) rats. During 45 min of reperfusion, RENA depressed more in comparison with RANA in both groups of animals. In addition, RANA returned to baseline level in SL rats, while it remained elevated in HA rats. In the second experiment, six groups of renal ischemic rats were challenged by rapid intravenous infusion of 10 ml of saline, and urine was collected for 90 min from the left ureter. Baseline RENA was low in rats 4 h after RAO of SL (4SL) and of HA (4HA) groups. The effects of saline loading on RENA and RANA were different in HA and SL rats. Saline loading significantly decreased RENA but increased RANA more in SL rats. Following saline loading, RENA in 4SL and 4HA rats, as well as animals 24 h after RAO of SL (24SL) and HA (24HA) were comparable to their respective SL or HA animals. In 4SL rats, RANA was significantly enhanced, and remained elevated during saline loading and the recovery period. In 4HA, 24HA and 24SL rats, RANA reduced significantly during saline loading, then its activity returned to the baseline value. The insulted kidneys showed increased renal excretion of water and sodium in 4SL and 4HA rats. Urinary excretion reduced significantly in 24SL rats but was almost normal in 24HA rats. These results indicate that a decrease in RENA may play a protective role in response to renal ischemia in both SL and HA rats. In response to renal ischemia and saline loading, different alterations of RANA in SL and HA rats may reflect a beneficial mechanism located in the hypoxia-pretreated kidney.

Capsaicin pretreatment was used to deplete tachykinins in order to study the role of tachykinins in chronic hypoxia-induced pulmonary hypertension. Forty three young Wistar rats weighing 235 +/- 4 g were randomly divided into four groups: control (n = 10); capsaicin pretreatment (n = 10); intermittent chronic hypoxia (n = 10); and capsaicin pretreatment + intermittent chronic hypoxia (n = 13). Control animals breathed room air. Rats in the capsaicin pretreatment groups were given capsaicin via subcutaneous injection over a three-day period. Hypobaric hypoxia was intermittently applied by placing animals into a hypobaric chamber with a barometric pressure of 380 Torr for two weeks. In the capsaicin pretreatment + intermittent chronic hypoxia group, rats were exposed to intermittent hypoxia for two weeks immediately after the last dose of capsaicin. Subsequently, pulmonary vascular function, as well as substance P (a tachykinin) level and neutral endopeptidase (NEP, the major degradation enzyme for tachykinins) activity in the lungs were measured. Chronic hypoxia caused significant increases in pulmonary artery pressure, right ventricle/(left ventricle + septum) weight ratio, hematocrit, and lung substance P level, as well as a significant decrease in lung NEP activity. All these chronic hypoxia-induced changes were significantly lessened by capsaicin pretreatment. Capsaicin pretreatment alone did not induce any significant alteration in vascular function. These results suggest that the chronic hypoxia causes an increase in lung tachykinin levels which, in turn, enhance the development of pulmonary hypertension.

The effect of aprotinin, a nonspecific kallikrein inhibitor on renal function was studied in chronic hypoxic rats (high altitude, HA), comparing to control rats kept at sea level (SL). Hypoxia was induced by placing female Wistar rats (195-230 g) in an altitude chamber (5,500 m) 15 hr per day for 4 weeks. Intrarenal arterial administration of aprotinin (300 kiu/kg/min), reduced the urinary kallikrein activity (UKA) in 9 SL and 10 HA anesthetized rats. Aprotinin did not significantly alter the urinary excretion of urine, sodium and potassium in SL rats, however, those parameters increased after aprotinin administration in HA rats. These data support the hypothesis that kallikrein participates in the regulation of renal function in chronic hypoxic rats.

The after effects of renal ischemia were studied in hypoxia-adapted rats. It was felt that chronic hypoxia animals which had already adapted to a low oxygen level might be more tolerant of renal ischemic insult; however, chronic hypoxia is always accompanied by polycythemia, which may cause severe RBC trapping and consequently might enhance renal damage after renal ischemia. Chronic hypoxic rats were prepared by exposure in an altitude chamber 15 h per day for 4 weeks. The plasma sodium, potassium, urea, and creatinine levels were determined to compare the changes in these parameters between the baseline and 3 h after a 45-min occlusion of both renal arteries in 12 sea-level (SLB) controls and in 12 chronic hypoxic (CHB) and 11 chronic hypoxic plus RBC pheresis (to reduce hematocrit: CH + P) rats. From the parameters measured, the CHB rats were found to be more tolerant of renal ischemia. However, this was not the case in the rats with pheresis. It is concluded that after chronic hypoxia, some humoral factors in the plasma may play an important role in reducing the renal damage after ischemic insult.

1. Chronic hypoxic rats are always polycythaemic. It is possible that an increase in packed cell volume may enhance erythrocyte trapping with a consequent increase in renal damage after renal ischaemia. These experiments were designed to assess renal functional changes after renal arterial occlusion in chronic hypoxic rats. 2. Chronic hypoxic rats were prepared by exposure (15h/day) to an altitude chamber (5486m) for 4 weeks. 3. After 45 min of left renal arterial occlusion, there were significant decreases in the excretion of potassium, p-aminohippurate and inulin and in the p-aminohippurate extraction ratio in 12 sea level ischaemic insulted kidneys. In 12 chronic hypoxic rats, the same parameters were changed after left renal ischaemia but only the p-aminohippurate ratio was significantly altered. 4. Administrations of 1 or 5 mg/kg phosphoramidon did not cause any significant improvement in the measured renal parameters in both kidneys and in both groups of rats after ischaemia. 5. In the second experiment, the rats were challenged by rapid infusion of 10 ml of saline intravenously, and urine was collected for 90 min from each ureter. Four hours after left renal arterial occlusion, the insulted kidney showed increased water and sodium excretion in both sea level and chronic hypoxic rats. However, 24 h after left renal ischaemia, the responses of sea level and chronic hypoxic rats were different. Urinary excretion was significantly reduced in sea level rats, but was almost normal in chronic hypoxic rats. 6. This report suggests that some beneficial factors after chronic hypoxia might play important roles in reducing the damage after renal ischaemia.

1. The direct renal effects of endothelin (ET) were studied in eight chronic hypoxic rats (HA) and eight sea level (SL) spontaneously hypertensive rats (SHR). 2. After 4 weeks of exposure to simulated 5486 m (18,000 ft) hypoxia, all HA rats were in apparently good health, and baseline renal function, except effective renal blood flow, was not significantly different from SL rats. 3. Intrarenal arterial administration of ET (600 ng/kg per h) reduced ipsilateral renal excretion of water, sodium and potassium, glomerular filtration rate and effective renal plasma flow in both SL and HA rats to almost the same extent. 4. Administration of ET antiserum, however, increased the renal excretion of water in HA rats. 5. It is concluded that ET may play a role in the renal regulation of chronic hypoxic SHR.

In an attempt to understand the changes of circulating neuropeptide Y (NPY) during hypoxia, the plasma level of NPY was investigated by radioimmunoassay. Exposure of rats to hypobaric hypoxia at an altitude of 18,000 ft for 4 weeks causes an increase of pulmonary pressure and an elevation of plasma NPY-like immunoreactivity (NPY-LI). However, the systemic blood pressure was not elevated by this chronic hypoxia. Also, plasma noradrenaline (NA) estimated by chromatographic analysis (HPLC-ECD) was not markedly raised. Failure of bretylium and guanethidine, sympathetic neuron blockers, in reducing the plasma NPY-LI level of these rats ruled out the participation of adrenergic nervous terminals. Adrenal medulla seems responsible for this elevation of plasma NPY-LI because this magnitude disappeared in adrenalectomized rats. These data suggest that chronic hypoxia induced an elevation of circulating NPY from the adrenal gland of rats.

This study was undertaken to investigate the effects of endothelin (ET) on cardiovascular function in chronic hypoxic rats (HA) and sea level controls (SL). Catheters were placed in femoral and pulmonary arteries for measurements of mean systemic arterial pressure (Psa) and mean pulmonary arterial pressure (Ppa). The cardiac output was measured by thermodilution method. It is very interesting to find that intravenous injection of ET (0.2 and 1 nmol/kg) caused a decrease in pulmonary arterial pressure (from 44 +/- 2.6 to 36 +/- 1.9 mmHg, p < 0.05) which sustained for about 10 min while the systemic arterial pressure rise (1 nmol/kg) or no significant change (0.2 nmol/kg) in the HA rats. The depressor action of ET is mainly resulted from the drop of cardiac output in HA rats, since the pulmonary vascular resistance resisted to the action of ET in those chronic hypoxic rats could be due to a simple loss of reaction to ET or the release of vascular dilators which balance the ET vasoconstrictive action. This report also found indomethacin and saralasin did not involve the action of ET on pulmonary arterial depression.

The present study was designed first to investigate the pulmonary hypertensive effects of chronic hypoxia in spontaneously hypertensive rats and second to compare the cardiovascular effects of atrial natriuretic factor on rats exposed to hypoxia and on control rats kept at sea level. Catheters were placed in the femoral and pulmonary arteries for measurement of mean systemic arterial pressure and mean pulmonary arterial pressure. The cardiac output was measured by thermodilution method. It was found that 4 weeks of simulated 18,000-foot hypoxia led to polycythemia, right ventricular hypertrophy, and pulmonary hypertension, which resulted from an increased pulmonary vascular resistance. However, systemic arterial pressure was not significantly different between the two groups of rats. Atrial natriuretic factor administration decreased systemic arterial pressure and pulmonary arterial pressure to a lesser extent in the hypoxic group compared with the sea level control group. It is concluded that these animals showed an impaired response to atrial natriuretic factor after long-term exposure to hypoxia.

The aftereffects of renal function were studied in rats exposed to hypoxia at 35,000 ft simulated altitude for 60 min. It was observed that the renal handling of a large volume of infused saline was retarded 4 hr after hypoxia, but it was nearly recovered by the end of 24 hr after exposure to severe hypoxia. However, the urine output, plasma and urine electrolytes, and renal hemodynamics were not significantly altered in hypoxic rats. A high mortality of rats exposed to such a low oxygen tension is also reported.

Healthy male rats were acclimatized by being placed in a decompression chamber at a simulated altitude of 18 000 feet (5486 m) for three hours daily for 84 days. The altitude acclimatized rats paired with unacclimatized rats were rapidly decompressed together. The range of decompression was performed from on atmospheric pressure to an ambient pressure of 30 mmHg in 0-2 seconds. It was found that in control rats, 14 of 20 lung (70%) exhibited pulmonary haemorrhage following rapid decompression. In altitude acclimatized rats, however, only 6 of 20 (30%) revealed decompression-induced haemorrhage. The difference was statistically significant. The present findings indicate that long-term intermittent exposures to hypoxia might increase the resistance of pulmonary tissue to rapid decompression, resulting in a decrease in frequency and severity of pulmonary haemorrhage. The possible mechanism of such a phenomenon is discussed.
