Objective: This study evaluated the effect of a formulation containing eicosapentaenoic acid and docosahexaenoic acid combined with soluble fibers (beta-glucan and guar gum) on fasting blood lipids used as coronary heart disease biomarkers of individuals classified into different levels of lipidemia by multivariate techniques., Methods: Serum total cholesterol, low-density lipoprotein cholesterol, high-density lipoprotein cholesterol, triacylglycerol, plasma glucose concentrations, body mass index, age, and waist circumference were determined in 99 healthy volunteers. Three clusters or subgroups were identified according to coronary heart disease risk levels. Within each cluster, individuals were randomly assigned to one of four experimental groups, with each group receiving samples of a functional formulation containing 460 mg of omega-3 polyunsaturated fatty acids and/or 580 mg of soluble fibers, and placebo to be consumed in one bottle per day (200 mL) for 6 wk., Results: No significant changes were observed for triacylglycerol (P = 0.281) and total cholesterol (P = 0.082) concentrations across the three subgroups. Soluble dietary fibers improved the sensory quality of the formulation containing eicosapentaenoic acid and docosahexaenoic acid. The efficiency of cluster analysis to discriminate individuals in subgroups was confirmed by one-way analysis of variance (P < 0.003)., Conclusion: The omega-3 polyunsaturated fatty acid supplementation equivalent to fish consumed 2.5 to 3 times per week by a functional food-containing soluble dietary fiber showed no beneficial result in terms of changes in blood lipids in individuals classified according to different levels of lipidemia. Small numbers of patients in each cluster and possibly the low dose of fish oil and soluble dietary fibers used in this study may have also contributed to the lack of these differences. Multivariate techniques proved to be a very efficient tool to solve the heterogeneity problem usually observed in human designs and to evaluate the results within subgroups categorized by n variables extracted from the same population.

Lipid peroxidation and lipid-derived oxidized products have been implicated in the pathogenesis of a variety of human diseases. To clarify the role of oxidative stress in essential hypertension and hypercholesterolemia the in vitro oxidative susceptibility of LDL, the antioxidant status and the lipid peroxide content of blood plasma were examined in hypercholesterolemic (HC), hypertensive (H), hypercholesterolemic/hypertensive (HH) and normolipidemic/normotensive subjects (N). Plasma ascorbate and lipid-soluble antioxidants were lower, while LDL oxidizability, CE-OOH and TL-OOH were higher in H, HC, and HH groups than in the N group. No difference was observed among groups for PL-OOH and isoprostanes. In summary, the results show that: 1) lipid- and water-soluble antioxidants are lower in hypercholesterolemic and hypertensive patients as compared to normal subjects, whereas the lipid peroxide content and the LDL susceptibility to oxidation were higher; 2) total cholesterol, LDL-cholesterol, apoB and CE-OOH were negatively correlated with the content of a-tocopherol; 3) there was a positive correlation between the content of lipid-soluble antioxidants and the resistance of LDL to oxidation; and 4) CE-OOH and TL-OOH were positively correlated with total cholesterol and LDL-cholesterol.

An artificial chylomicron-like lipid emulsion doubly labeled with tri[(N)3H]oleoylglycerol ([3H]TO) and cholesteryl [1-14C]oleate ([14C]CO) was infused intravenously into human subjects with the purpose of simultaneously measuring the plasma disappearance rates (residence time, RT) of [14C]CO, which represents solely the splanchnic organ uptake of the remnant chylomicron core, and of [3H]TO, which combines the remnant disappearance with the shedding off of chylomicron triglycerides by the action of lipoprotein lipase. Thus, the fraction of the particle triglyceride content that is removed before the remnant is taken up is expressed as a delipidation index (DI = 1 - RT of [3H]TO/RT of [14C]CO. The present procedure has an advantage over the use of chylomicrons labeled with retinyl ester or radioactive triglycerides alone that represent, respectively, the chylomicron remnant or the whole particle metabolism only. When normal subjects as well as primary hyperlipidemic subjects were studied, the plasma triglyceride concentration was directly related to [14C]CO RT and [3H]TO RT, but inversely related to the delipidation index. There may be different patterns of relations between these parameters of chylomicron metabolism in primary and in secondary hyperlipidemias, as well as under the action of drugs that influence the metabolism of lipoproteins.

Purpose: To compare the effects of lovastatin and gemfibrozil in patients with primary hyperlipidemias., Patients and Methods: Forty patients with cholesterolemia over 200 mg/dl and triglyceridemia not higher than 350 mg/dl, excluded secondary causes, were selected. Twenty patients received lovastatin and 20 gemfibrozil. In order to establish the lipid profile, blood samples were taken after 2 months without medication, after 4 weeks of diet and placebo and after 6 and 12 weeks of active treatment. Biochemical profile was determined before and after the treatment with active drug., Results: Thirty nine patients completed the study. Total and LDL-cholesterol were significantly reduced (p less than 0.05) by both drugs but lovastatin had greater effect. Only gemfibrozil reduced triglycerides significantly. Neither drug had significant effects on HDL-cholesterol. The tolerance was satisfactory; only one patient (using gemfibrozil) needed to stop the treatment due to gastrointestinal side effects. The biochemical profile did not present any significant alteration., Conclusion: Both drugs produced useful effects on the lipid profile. Lovastatin produced greater reductions of total and LDL-cholesterol, while gemfibrozil was more active reducing triglycerides. Neither drug changed significantly the HDL-cholesterol.
