Otorhinolaryngologists and head and neck surgeons are predestined to scientifically analyze and prioritize the medical needs in ENT medicine under the umbrella of the German Society of Otolaryngology, Head and Neck Surgery (DGHNO-KHC). This medical need assessment is important for targeting research and development of medical innovations to improve ENT patients' care with these needs and to trigger respective research and innovation funding programs at an early stage. This is to recommend that the DGHNO-KHC and its working groups and task forces in their specialty disciplines address the issue. The goal is a comprehensive medical need assessment for the fields of otolaryngology as well as head and neck surgery. The parameterization of medical needs is based on the unmet medical need (UMN) concept. Criteria for prioritization should follow the method of multi-criteria decision analysis (MCDA). The working groups of the DGHNO-KHC familiarize themselves with the concept of UMN. Subsequently, suggestions on ENT diseases with UMN in diagnostics and therapy will be collected considering the medical evidence. The suggestions are evaluated in a standardized way according to MCDA and a ranking is established. The results will be published and used to make research funding institutions aware of UMN in otorhinolaryngology, head and neck surgery. The work will contribute to the further development of the UMN concept and will also be able to provide a basis to investigate the evidence of the concept to define the UMN. Through this work, the DGHNO-KHC will be perceived as an important scientific player for the development of need-driven medical innovations in Germany. HNO-Ärztinnen und HNO-Ärzte sind prädestiniert dafür, den medizinischen Bedarf in der HNO-Heilkunde unter dem Dach der Deutschen Gesellschaft für Hals-Nasen-Ohren-Heilkunde, Kopf- und Hals-Chirurgie (DGHNO-KHC) wissenschaftlich zu analysieren und zu priorisieren. Diese Bedarfsanalyse ist wichtig, um gezielt Erforschung und Entwicklung medizinischer Innovationen zur besseren Versorgung von HNO-Patientinnen und HNO-Patienten auf diesen Bedarf auszurichten und entsprechende Programme zur Forschungs- und Innovationsförderung frühzeitig anzustoßen. Hiermit soll die Empfehlung ausgesprochen werden, dass die DGHNO-KHC und deren Arbeitsgemeinschaften und Arbeitsgruppen in ihren Spezialdisziplinen sich des Themas annehmen. Das Ziel ist ein umfassendes Die Arbeitsgemeinschaften und Arbeitsgruppen der DGHNO-KHC machen sich mit dem Konzept des UMN vertraut. Anschließend werden Vorschläge zu HNO-Erkrankungen mit UMN in Diagnostik und Therapie unter Beachtung der medizinischen Evidenz gesammelt. Die Ergebnisse werden veröffentlicht und genutzt, um Institutionen der Forschungs- und Innovationsförderung auf den UMN in der HNO-Heilkunde aufmerksam zu machen. German health policy has propagated an expansion of outpatient surgeries and procedures and initiated the discussion about this. Otorhinolaryngology, head and neck surgery offers opportunities to provide currently inpatient procedures on an outpatient basis.The German Society of Otorhinolaryngology, Head and Neck Surgery, and the German Professional Association of Otorhinolaryngologists established working groups to assess and evaluate the ENT-specific aspects of shifting services to the outpatient setting. The working groups were given the task of developing and considering organizational, structural and personnel definitions of quality assurance.Facts were determined in detail, which exclude an ambulatory operation in the ENT-specialty. This was based on both surgery-related and patient-related facts. Finally, operations were named which can be performed as outpatient operations.An evaluation was performed from the ENT specialist's perspective. A prerequisite for outpatient treatment is a reorganization of remuneration. The current DRG and EBM system do not provide a satisfactory framework for this, and the EBM does not allow for the economic provision of surgical interventions in otorhinolaryngology, head and neck surgery. The development of an appropriate financing model is as imperative for a successful implementation as the integration and financing of further training of ENT physicians.Die deutsche Gesundheitspolitik hat eine Ausweitung ambulanter Operationen und Verfahren propagiert und die Diskussion darüber eingeleitet. Die HNO-Heilkunde bietet Möglichkeiten, derzeit stationär durchgeführte Eingriffe ambulant zu erbringen.Die Deutsche Gesellschaft für HNO-Heilkunde, Kopf- und Hals-Chirurgie und der Deutsche Berufsverband der HNO-Ärzte haben Arbeitsgruppen ins Leben gerufen, die die HNO-fachlichen Aspekte der Leistungsverlagerungen in den ambulanten Bereich beurteilen und bewerten sollen. Die Arbeitsgruppen bekamen den Auftrag dabei organisatorische, strukturelle und personelle Definitionen der Qualitätssicherung zu erarbeiten und zu berücksichtigen.Es wurden ausführlich Tatbestände bestimmt, die eine ambulante Operation im HNO-Fachgebiet ausschließen. Grundlage bildeten hierbei sowohl operationsbedingte als auch patientenbedingte Tatbestände. Schließlich wurden Operationen benannt, die als ambulante Operationen durchgeführt werden können.Es erfolgte eine HNO-fachliche Bewertung. Voraussetzung für eine Ambulantisierung ist eine Neuordnung der Vergütung. Das bisherige DRG- und EBM-System bieten hierzu keinen befriedigenden Rahmen, der EBM ermöglicht keine wirtschaftliche Erbringung operativer Eingriffe im HNO-Bereich. Die Entwicklung eines adäquaten Finanzierungsmodells ist für einen Erfolg der Umsetzung genauso zwingend erforderlich wie die Integration und Finanzierung der HNO-ärztlichen Weiterbildung.

The inpatient care of children after ENT surgery is organized heterogeneously across Germany. In this statement, the working group on Pediatric ENT comments a national survey on the status quo of child care after ENT interventions in German hospitals. The statement should serve as a basis for further interdisciplinary discussion.Die stationäre Betreuung von Kindern nach Operationen im HNO-Bereich ist deutschlandweit heterogen organisiert. In dieser Stellungnahme kommentiert die Arbeitsgemeinschaft Pädiatrische HNO-Heilkunde eine nationale Umfrage zum Status quo der Kinderversorgung nach HNO-Eingriffen in deutschen Kliniken. Diese dient als Grundlage für die weitere interdisziplinäre Diskussion.

Since the beginning of the SARS-CoV-2 pandemic, swabs or other samples have increasingly been taken from the upper aero-digestive tract, since high viral loads exist here, especially in the early stages of the disease. As diagnostic options, swabs from the anterior nose, from the nasopharynx, from the oropharynx or the extraction of throat rinse water or saliva are possible. The laboratory methods available are antigen tests that can be read in a few minutes or more lengthy RT-PCR methods in a lab. Swabs are carried out by physicians, medical staff, laypeople and in the self-test, in each case according to prior instructions. Many of these factors therefore have an influence on the informative value and the sensitivity of the entire diagnostic process. The PCR laboratory method is more sensitive than the antigen method; the swabs from the nasopharynx are considered the most valid smear site; correct execution of a test can be achieved even with non-professional individuals with good instructions. Complications with such swabs are reported very rarely, given the assumed number of procedures performed. Short-term nosebleeds after traumatic smears can be assumed without publications about it being found. Broken parts of swabs had to be removed by an ENT doctor. There are only very few reports on injuries to the skullbase with CSF-leaks, including 2 times with anomalies such as meningoceles. The choice of a suitable diagnostic medium depends on many parameters such as availability, the timing of the result, a smear test by knowledgeable staff or a self-test, and a number of other practical considerations.Seit Beginn der SARS-CoV-2-Pandemie werden zunehmend Abstriche oder andere Beprobungen aus dem oberen Aerodigestivtrakt genommen, da hier vor allen Dingen in frühen Erkrankungsstadien hohe Viruslasten bestehen. Als diagnostische Möglichkeiten sind Abstriche aus der vorderen Nase, dem Nasopharynx oder dem Oropharynx sowie die Gewinnung von Rachenspülwasser oder Speichel möglich. Als Labormethoden stehen in einigen Minuten ablesbare Antigentests oder langwierigere RT-PCR-Methoden zur Verfügung. Abstriche werden von ärztlichem Personal, medizinischem Fachpersonal, Laien und im Eigentest, jeweils nach Instruktionen, durchgeführt. Auf die Aussagekraft und die Sensitivität des gesamten diagnostischen Prozesses haben somit viele dieser Faktoren einen Einfluss. Die PCR-Labormethode ist sensitiver als die Antigenmethode; der Abstrich aus dem Nasopharynx wird als der valideste Abstrichort angesehen. Eine korrekte Durchführung eines Tests kann auch bei nicht professionellen Personen mit guten Instruktionen erreicht werden. Komplikationen werden bei solchen Abstrichen, gemessen an der anzunehmenden Anzahl durchgeführter Prozeduren, sehr selten berichtet. Kurzfristiges Nasenbluten bei traumatischen Abstrichen ist anzunehmen, ohne dass darüber Publikationen aufgefunden werden konnten. Abgebrochene Abstrichträger mussten HNO-ärztlich entfernt werden. Verletzungen der Schädelbasis mit Liquorrhö wurden bisher sehr vereinzelt berichtet, davon 2-mal bei Anomalien wie Meningozelen. Die Wahl eines geeigneten diagnostischen Mediums hängt von vielen Parametern, wie Verfügbarkeit, zeitlicher Ablauf bis zum Ergebnis, Abstrich durch kundiges Personal oder Eigentest und etlichen anderen praktischen Erwägungen, ab.

For patients with severe chronic rhinosinusitis with nasal polyps (CRSwNP) which cannot be controlled by continuous therapy with intranasal corticosteroids (INCS) and systemic corticosteroids and/or surgical treatment, there were no approved curative options for a long time. For CRSwNP treatment with T2-addressing biologics is possible. On October 24, 2019, the European Commission granted extended approval for dupilumab as the first biological agent for treatment of insufficiently controlled severe CRSwNP. The Federal Joint Committee (G‑BA) evaluates the benefits of reimbursable drugs with new active ingredients. This includes assessment of the additional benefit and its therapeutic relevance.A meta-analysis was performed using individual patient data based on two phase III studies. Both studies examined the safety and efficacy of dupilumab as an add-on therapy to INCS for treatment of CRSwNP in adults inadequately controlled with systemic corticosteroids and/or surgery compared to INCS alone.Based on the present data, the G‑BA decided that there is an indication of a considerable additional benefit of dupilumab compared to mometasone furoate.For patients with severe CRSwNP inadequately controlled with INCS and systemic corticosteroids and/or surgery, there is an indication of a considerable additional benefit for the administration of dupilumab as an add-on therapy to INCS compared to mometasone furoate alone.HINTERGRUND: Für Patienten mit schwerer chronischer Rhinosinusitis mit Nasenpolypen (CRSwNP), die durch eine Dauertherapie mit intranasalen Kortikosteroiden (INCS) und Gabe von systemischen Kortikosteroiden und/oder eine chirurgische Behandlung nicht ausreichend kontrolliert werden konnten, gab es für lange Zeit keine zugelassenen kurativen Therapieoptionen. Bei der CRSwNP ist eine Behandlung mit T2-adressierenden Biologika möglich. Am 24.10.2019 hat die Europäische Kommission für Dupilumab als erstes Biologikum eine Zulassungserweiterung zur Therapie von unzureichend kontrollierter, schwerer CRSwNP ausgesprochen. Der Gemeinsame Bundesausschuss (G‑BA) bewertet den Nutzen von erstattungsfähigen Arzneimitteln mit neuen Wirkstoffen. Hierzu gehört insbesondere die Bewertung des Zusatznutzens und seiner therapeutischen Bedeutung.Auf der Grundlage zweier Phase-III-Studien wurde eine Metaanalyse anhand von individuellen Patientendaten durchgeführt. Vestibular schwannomas (VSs) are benign, slow-growing tumors that are classified by the Koos and Hannover grading scales. Despite extensive study, the management of large VS remains controversial.Between 2003 and 2018, 61 patients with VS grade IV were treated in our institution. Patient data and radiological images were retrospectively analyzed. Additionally, we performed a subgroup analysis of patients with and without fourth ventricle distortions.Most patients presented with multiple symptoms, like hearing loss in 55 patients (90 %), trigeminal nerve affection in 16 (26 %), facial nerve affection in 7 (12 %), ataxia in 27 (45 %), and symptoms associated with increased intracranial pressure caused by hydrocephalus in 4 patients (7 %). Patients with type IV b VS presented significantly more often with ataxia, tonsillar herniation, and hydrocephalus. Complete tumor resection was achieved in 48 patients (79 %) and near-total resection was achieved in 12 patients (20 %). During long-term follow-up, 90 % of the patients had favorable outcomes regarding facial nerve function (House and Brackman grade I-III). In six patients (10 %), a ventriculoperitoneal shunt was inserted. More than 90 % of the patients had 70 % functional impairment based on the Karnofsky Index.Large VS is often associated hydrocephalus, ataxia, multiple cranial nerve impairments, and clinical signs of increased intracranial pressure. Primary microsurgical resection, as the primary treatment option, is associated with good outcomes in patients with large VS.Vestibularisschwannome (VS) sind benigne Tumoren, die anhand der Hannover-Klassifikation bzw. der Koos-Klassifizierung eingeteilt werden. Trotz der umfangreichen Literatur sind die Klinik und die Behandlungskonzepte speziell bei großen VS selten beschrieben.Zwischen 2003 und 2018 wurden 61 Patienten mit VS Typ IV durch die Arbeitsgruppe Schädelbasischirurgie am Klinikum Dortmund behandelt. Die radiologischen und klinischen Daten wurden retrospektiv ausgewertet. Zudem erfolgte eine Subgruppenanalyse zwischen Patienten mit und ohne Kompression des IV. Ventrikels.Neben einer Hörminderung bei 55 Patienten (90 %) hatten die meisten Patienten multiple Symptome wie eine Trigeminusaffektion bei 16 (26 %), eine Fazialisparese bei 7 (12 %), eine Ataxie bei 27 (45 %) und Symptome eines Hirndruckanstiegs durch einen Hydrozephalus bei 4 Patienten (7 %). Bei Patienten mit einem VS Typ IVb wurde signifikant häufiger eine Ataxie, eine tonsilläre Herniation bzw. ein Hydrozephalus festgestellt. Eine komplette Resektion wurde in 48 Patienten (78 %) erreicht und eine weitestgehende Tumorentfernung in 12 Patienten (20 %). In der Langzeituntersuchung zeigten 90 % einen günstigen Outcome bezüglich des Nervus facialis (House-und-Brackman-Grad I–III). 6 Patienten (10 %) benötigten einen dauerhaften ventrikulo-peritonealen Shunt. Mehr als 90 % der Patienten erzielten einen Karnofsky-Index 70 %.VS Typ IV sind häufig assoziiert mit Hydrozephalus, Ataxie, multiplen Hirnnervenausfällen und gelegentlich Zeichen eines intrakraniellen Druckanstiegs. Die primäre mikrochirurgische Resektion ist weiterhin eine entscheidende Therapieoption.

An increased psychosocial workload can have an negative impact on health. An effective way to record this is the effort reward imbalance model postulated by Siegrist. Values on this topic from ENT residents are missing, which is why the concept and corresponding questions were included in the survey on the current situation in further education.An online survey on the current situation of the ENT residency including the recording of psychosocial workload was developed by ENT physicians on the basis of a well-known questionnaire of colleagues of the Alliance of Young Physicians. The short version of the validated questionnaire on the effort reward imbalance model according to Siegrist with 16 items was used. An online survey was carried out addressing all ENT residents in Germany known to the German society of Oto-Rhino-Laryngology, Head and Neck surgery. The survey was sent by e-mail and was available from April 192,3 % of the participants had an effort-reward imbalance. The mean value of effort reward imbalance was 1.57 ± 0.43, adjusted 2.16 ± 1.36. The effort scale was 10.71 ± 1.40 (3-12), adjusted 85.72 ± 15.52, reward scale 16.58 ± 2.86 (7-28), adjusted 45.61 ± 13.63, over commitment 17 ± 3.37, adjusted 61.14 ± 18.73. A high effort reward imbalance had positive significant correlations with regard to the duration of residency, the number of working hours per week and the number of duty hours per month.The effort and reward imbalance is comparable to other specialty physicians in residency. It is related to working hours, services and the progress of training. It can be improved through personal initiative and could be supplemented with the support of the hospital's internal stakeholders.Eine erhöhte psychosoziale Arbeitsbelastung kann negativen Einfluss auf die Gesundheit haben. Eine hohe ER-Ratio war positiv signifikant korreliert hinsichtlich der Dauer der Weiterbildungszeit, der Anzahl an Arbeitsstunden/Woche und der Anzahl an geleisteten Diensten im Monat.Die erfasste hohe psychosoziale Arbeitsbelastung ist vergleichbar mit Assistenzärzten anderer Fachrichtungen. Sie steht in Zusammenhang mit hoher Arbeitszeit (Stunden, Dienste) und mit dem Fortschritt der Weiterbildung. Die Belastung kann mithilfe von Selbstinitiative verbessert werden und könnte mit der Unterstützung der klinikinternen Akteure ergänzt werden.

Residents of ENT were asked about their situation in residency. A good and well structured training is the key for an attractive residency.Between April 25-August 1, 2019, 691 residents registered with the German Society of Otorhinolaryngology, Head- and Neck Surgery ENT society were invited to anonymously participate in an online survey using SurveyMonkeyThe response rate was 36 % (n = 249). The participants attested their further training an average result. Mainly they see deficits in the further education and training structure and culture with a desire for improved feedback and improved surgical training. Participants were more dissatisfied with advanced further training time, regardless of ownership of the institution and level of care.The survey proposes concrete advice for improvement of ENT medical training in Germany. Suggestions for improvement are further development of the associated training and continuing education programs in cooperation with professional associations, structured feedback and supervision as well as transparent rotation plans and reliable working conditions.Es wurden die Weiterbildungsassistenten im Fach HNO zur Situation der Weiterbildung und den Arbeitsbedingungen befragt. Die Weiterbildung ist der Schlüssel zur Ausbildung des Nachwuchses im Fachbereich.Vom 25.04.–01.08.2019 wurden alle 691 bei der Deutschen Gesellschaft für HNO-Heilkunde, Kopf- und Halschirurgie (DGHNO-KHC) registrierten Ärzte in Weiterbildung in der HNO zur anonymen Teilnahme an einer Online-Umfrage mittels SurveyMonkeyDie Rücklaufquote betrug 36 % (n = 249). Die Teilnehmer attestierten ihrer Weiterbildung ein durchschnittliches Ergebnis. Hauptsächlich sehen sie Defizite in der Weiter- und Ausbildungsstruktur und -kultur mit Wunsch nach verbessertem Feedback und verbesserter operativer Ausbildung. Mit fortgeschrittener Weiterbildungszeit waren die Teilnehmer unzufriedener, unabhängig von Trägerschaft und Versorgungstyp.Die Befragung der Weiterbildungsassistenten regt konkrete Verbesserungen der HNO-ärztlichen Weiterbildung an. Hierbei werden vorrangig die Zusammenarbeit mit der Fachgesellschaft zur Weiterentwicklung der Aus- und Fortbildungsstruktur, strukturiertes Feedback und Supervision, transparente Rotationspläne und verlässliche Arbeitsbedingungen genannt.

In Germany, about 5 million people of all ages suffer from dysphagia. Due to demographic change and improved medical care, the incidence of swallowing disorders is expected to increase. Dysphagia is associated with an increased morbidity and mortality and leads to a considerable financial burden on the health systems. The two most common causes of dysphagia are neurological disorders and head and neck cancer. Diagnostics and therapy have developed continuously over the past decades. In particular, the flexible endoscopic evaluation of swallowing (FEES) has become an established part of dysphagia diagnostics.The certificate "Diagnostics and Therapy of Oropharyngeal Dysphagia, incl. FEES" was developed by the German Society for Phoniatrics and Pedaudiology (DGPP) and the German Society for Otolaryngology, Head and Neck Surgery (DGHNO KHC) in cooperation with the German Professional Association for Phoniatrics and Pedaudiology and the German Professional Association of Otolaryngologists.It consists of three parts: the modules (A, B and C), the indirect supervision and a practical examination. Structure, detailed contents and requirements for obtaining the certificate are described in the following article. The qualification of the lecturers and auditors are also defined.The systematic training serves the quality assurance and establishment of standards in the diagnostics and therapy of oropharyngeal dysphagia in the area of phoniatrics and ear, nose and throat medicine.In Deutschland leiden ca. 5 Millionen Menschen jeden Lebensalters an einer Dysphagie. Durch den demografischen Wandel und die verbesserte medizinische Versorgung ist mit einer Zunahme der Inzidenz von Schluckstörungen zu rechnen. Die Dysphagie ist mit einer erhöhten Morbiditäts- und Mortalitätsrate assoziiert und führt zu einer erheblichen finanziellen Belastung der Gesundheitssysteme. Die zwei häufigsten Ursachen einer Schluckstörung sind neurologische Erkrankungen und Kopf-Hals-Tumore. Die Diagnostik und Therapie haben sich in den letzten Jahrzehnten kontinuierlich weiterentwickelt. Insbesondere die flexibel-endoskopische Beurteilung des Schluckaktes (FEES) ist zu einem festen Bestand in der Abklärung von Schluckstörungen geworden.Das Zertifikat „Diagnostik und Therapie oropharyngealer Dysphagien, inkl. FEES“ wurde von der Deutschen Gesellschaft für Phoniatrie und Pädaudiologie (DGPP) und der Deutschen Gesellschaft für Hals-Nasen-Ohren-Heilkunde, Kopf- und Hals-Chirurgie (DGHNO KHC) in Zusammenarbeit mit dem Deutschen Berufsverband für Phoniatrie und Pädaudiologie und dem Deutschen Berufsverband der Hals-Nasen-Ohrenärzte entwickelt.Es besteht aus drei Teilen: den Modulen (A, B und C), der indirekten Supervision und einer praktischen Prüfung. Aufbau, detaillierte Inhalte und die Anforderungen zur Erlangung des Zertifikates werden im folgenden Artikel beschrieben. Ebenso werden die Qualifikationen der Dozenten und Prüfer festgesetzt.Die systematische Ausbildung dient der Qualitätssicherung und Etablierung von Standards in der Diagnostik und Therapie von oropharyngealen Dysphagien in der Phoniatrie und Pädaudiologie und in der Hals-, Nasen- Ohrenheilkunde.

Acta Otolaryngol (Stockh) 1996; 116: 762-765.The etiology of inverted papilloma is believed to be of viral origin, as viruses or viral material is found in these tumors of the nose and paranasal sinuses. as the nose is the first defense line of the respiratory tract, we suspected an etiology of airborne pollution as well, especially occupational pollution. We examined a group of 47 patients treated in our department for nasal inverted papilloma, asking about lifelong professional history and occupational exposure. A matched control group of patients with non-malignant diseases was included in the study. We found a significantly higher degree of occupational exposure to different smokes, dusts, and aerosols in the case group. No similar results have been published to our knowledge. to obtain better proof of the role of possible noxious agents, a study technically investigating exposure is mandatory.

We saw a child after normal delivery with mild hematoma but a substantial deviation of the nose. The child had no respiratory distress nor in rest nor during effort. We decided not to perform a reposition of the nose but watched further development. After a few weeks the nose straightened significantly, and was nearly normal after 3 months. An exam after 1 year did not show any deviation. Recent studies only advise to perform surgical steps, if there is respiratory distress. This well documented case is an impressive example for the ability of the neonatal nose to help itself.

The present lecture is a personal report on changing working conditions for physicians in German hospitals over the last 20 years. Documentation, coding and other administrative requirements have constantly risen and will increase once again from 2004 onwards when the complete reimbursement system in German hospitals will be rearranged on the basis of diagnosis-related groups, though without providing an adequate workforce of professional coders. Moreover, the European Court ruled that any doctor's on-call hours spent in a hospital should be regarded as part of his full working time, which in Germany is presently restricted to an average 38.5 hours a week. Consequently, this will lead to a huge demand of personnel on the one hand and an increasing fragmentation of doctor-patient relationships on the other hand, as there will be a continuous change of doctors on duty. Here, it is outlined that this fragmentation will have a negative impact on treatment quality and education and training of future physicians.

The effects of high-frequency ventilation (HFV) on the rheological properties of tracheobronchial secretions were investigated in vitro. The mucus was obtained by suction from 16 patients after major surgery of the larynx or hypopharynx. After short centrifugation, the mucus was treated with high-frequency vibrations of 3 or 30 Hz for 10 min in a chamber saturated with water vapor. The mucus was separated by a nonpermeable membrane from the ventilator's gas stream. Maximal, minimal, and mean viscosity were determined by a modified capillary viscosimeter. Under high-frequency vibrations, maximal, minimal, and mean viscosity slightly increased (p less than or equal to .001). Conversely, the mean viscosity of control samples did not change significantly. These findings indicate that high-frequency vibrations do not improve mucous rheology. Observed beneficial effects of HFV on mucociliary clearance may be caused by changes in the mucous subphase during HFV.
