The cerebellum is involved in the acquisition and consolidation of learned fear responses. Knowledge about its contribution to extinction learning, however, is sparse. Extinction processes likely involve erasure of memories, but there is ample evidence that at least part of the original memory remains. We asked the question whether memory persists within the cerebellum following extinction training. The renewal effect, that is the reoccurrence of the extinguished fear memory during recall in a context different from the extinction context, constitutes one of the phenomena indicating that memory of extinguished learned fear responses is not fully erased during extinction training. We performed a differential AB-A/B fear conditioning paradigm in a 7-Tesla (7T) MRI system in 31 young and healthy men. On day 1, fear acquisition training was performed in context A and extinction training in context B. On day 2, recall was tested in contexts A and B. As expected, participants learned to predict that the CS+ was followed by an aversive electric shock during fear acquisition training. Skin conductance responses (SCRs) were significantly higher to the CS+ compared to the CS- at the end of acquisition. Differences in SCRs vanished in extinction and reoccurred in the acquisition context during recall indicating renewal. Fitting SCR data, a deep neural network model was trained to predict the correct shock value for a given stimulus and context. Event-related fMRI analysis with model-derived prediction values as parametric modulations showed significant effects on activation of the posterolateral cerebellum (lobules VI and Crus I) during recall. Since the prediction values differ based on stimulus (CS+ and CS-) and context during recall, data provide support that the cerebellum is involved in context-related recall of learned fear associations. Likewise, mean β values were highest in lobules VI and Crus I bilaterally related to the CS+ in the acquisition context during early recall. A similar pattern was seen in the vermis, but only on a trend level. Thus, part of the original memory likely remains within the cerebellum following extinction training. We found cerebellar activations related to the CS+ and CS- during fear acquisition training which likely reflect associative and non-associative aspects of the task. Cerebellar activations, however, were not significantly different for CS+ and CS-. Since the CS- was never followed by an electric shock, the cerebellum may contribute to associative learning related to the CS, for example as a safety cue. CA Batsikadze

Background and Purpose: The pathophysiology of development, growth, and rupture of intracranial aneurysms (IAs) is only partly understood. Cyclooxygenase 2 (COX-2) converts arachidonic acid to prostaglandin H 2 , which, in turn, is isomerized to prostaglandin E 2 . In the human body, COX-2 plays an essential role in inflammatory pathways. This explorative study aimed to investigate COX-2 expression in the wall of IAs and its correlation to image features in clinical (1.0T, 1.5T, and 3.0T) magnetic resonance imaging (MRI) and ultra-high-field 7T MRI. Methods: The study group comprised 40 patients with partly thrombosed saccular IAs. The cohort included 17 ruptured- and 24 unruptured IAs, which had all been treated microsurgically. Formaldehyde-fixed paraffin-embedded samples were immunohistochemically stained with a monoclonal antibody against COX-2 (Dako, Santa Clara, CA; Clone: CX-294). We correlated Perls Prussian blue staining, MRI, and clinical data with immunohistochemistry, analyzed using the Trainable Weka Segmentation algorithm. Results: Aneurysm dome size ranged between 2 and 67 mm. The proportion of COX-2 positive cells ranged between 3.54% to 85.09%. An upregulated COX-2 expression correlated with increasing IA dome size ( P =0.047). Furthermore, there was a tendency of higher COX-2 expression in most ruptured IAs ( P =0.064). At all field strengths, MRI shows wall hypointensities due to iron deposition correlating with COX-2 expression ( P =0.022). Conclusions: Iron deposition and COX-2 expression in IAs walls correlate with signal hypointensity in MRI, which might, therefore, serve as a biomarker for IA instability. Furthermore, as COX-2 was also expressed in small unruptured IAs, it could be a potential target for specific medical treatment.

OBJECTIVE: A new technique for 2D gradient-recalled echo echo-planar imaging (GE-EPI) termed 'variable slice thickness' (VAST) is proposed, which reduces signal losses caused by through-slice susceptibility artifacts, while keeping the volume repetition time (TR) manageable. The slice thickness is varied across the brain, with thinner slices being used in the inferior brain regions where signal voids are most severe.MATERIALS AND METHODS: Various axial slice thickness schemes with identical whole-brain coverage were compared to regular EPI, which may either suffer from unfeasibly long TR if appropriately thin slices are used throughout, or signal loss if no counter-measures are taken. Evaluation is based on time-course signal-to-noise (tSNR) maps from resting state data and a statistical group-level region of interest (ROI) analysis on breath-hold fMRI measurements.RESULTS: The inferior brain region signal voids with static B0 inhomogeneities could be markedly reduced with VAST GE-EPI in contrast to regular GE-EPI. ROI-averaged event-related signal changes showed 48% increase in VAST compared to GE-EPI with regular "thick" slices. tSNR measurements proved the comparable signal robustness of VAST in comparison to regular GE-EPI with thin slices.CONCLUSION: A novel acquisition strategy for functional 2D GE-EPI at ultrahigh magnetic field is presented to reduce susceptibility-induced signal voids and keep TR sufficiently short for whole-brain coverage.

Objectives To investigate whether a dose reduction of Gadobutrol for renal magnetic resonance angiography (MRA) at 7 Tesla (T) is feasible while preserving diagnostic image quality. Methods Ten healthy volunteers were enrolled for a renal MRA on a 7 T scanner. Fast low angle shot (FLASH) MRA data sets were obtained utilizing three different doses of Gadobutrol (0.1, 0.05 and 0.025 mmol/kg body weight [BW]). Contrast ratios (CR) were measured in the aorta as well as in the intra- and extraparenchymal arteries compared to the psoas muscle. Qualitative analysis regarding the delineation of vessel structures was performed using a four-point-scale. Results All doses of Gadobutrol allowed for a good delineation of the aorta and renal arteries. For the extra- and intraparenchymal segmental arteries higher values were observed for full and half dose in comparison to quarter dose. No significant difference was observed for full and half dose. A lower CR was observed for quarter compared to half dose (p Conclusions While best results were observed for half and full dose, a dose reduction to 0.025 mmol/kg BW is justifiable, maintaining a diagnostic image quality. This may be of high interest considering patients with renal impairment.

BACKGROUND AND PURPOSE: The pathophysiology of wall contrast enhancement in thrombosed intracranial aneurysms is incompletely understood. This in vivo study aimed to investigate wall microstructures with gadolinium-enhanced 7T MR imaging. MATERIALS AND METHODS: Thirteen patients with 14 thrombosed intracranial aneurysms were evaluated using a 7T whole-body MR imaging system with nonenhanced and gadolinium-enhanced high-resolution MPRAGE. Tissue samples were available in 5 cases, and histopathologic findings were correlated with 7T MR imaging to identify the gadolinium-enhancing microstructures. RESULTS: Partial or complete inner wall enhancement correlated with neovascularization of the inner wall layer and the adjacent thrombus. Additional partial or complete outer wall enhancement can be explained by formation of vasa vasorum in the outer aneurysm wall layer. The double-rim enhancement correlated with perifocal edema and wall histologic findings suggestive of instability. CONCLUSIONS: Two distinct aneurysm wall microstructures responsible for gadolinium enhancement not depictable at lower spatial resolutions can be visualized in vivo using high-resolution gadolinium-enhanced 7T MR imaging.

Purpose A 16-channel receive (16Rx) radiofrequency (RF) array for 7T ultra-high field body MR imaging is presented. The coil is evaluated in conjunction with a 16-channel transmit/receive (16TxRx) coil and additionally with a 32-channel transmit/receive (32TxRx) remote body coil for RF transmit and serving as receive references. Methods The 16Rx array consists of 16 octagonal overlapping loops connected to custom-built detuning boards with preamplifiers. Performance metrics like noise correlation, g-factors, and signal-to-noise ratio gain were compared between 4 different RF coil configurations. In vivo body imaging was performed in volunteers using radiofrequency shimming, time interleaved acquisition of modes (TIAMO), and 2D spatially selective excitation using parallel transmit (pTx) in the spine. Results Lower g-factors were obtained when using the 16Rx coil in addition to the 16TxRx array coil configuration versus the 16TxRx array alone. Distinct signal-to-noise ratio gain using the 16Rx coil could be demonstrated in the spine region both for a comparison with the 16TxRx coil (>50% gain) in vivo and the 32TxRx coil (>240% gain) in a phantom. The 16Rx coil was successfully applied to improve anatomical imaging in the abdomen and 2D spatially selective excitation in the spine of volunteers. Conclusion The novel 16-channel Rx-array as an add-on to multichannel TxRx RF coil configurations provides increased signal-to-noise ratio, lower g-factors, and thus improves 7T ultra-high field body MR imaging.

Objective To perform non-enhanced (NE) magnetic resonance imaging (MRI) of the small bowel at 7 Tesla (7 T) and to compare it with 1.5 Tesla (1.5 T). Material and methods Twelve healthy subjects were prospectively examined using a 1.5 T and 7 T MRI system. Coronal and axial true fast imaging with steady-state precession (TrueFISP) imaging and a coronal T2-weighted (T2w) half-Fourier acquisition single-shot turbo spin-echo (HASTE) sequence were acquired. Image analysis was performed by 1) visual evaluation of tissue contrast and detail detectability, 2) measurement and calculation of contrast ratios and 3) assessment of artifacts. Results NE MRI of the small bowel at 7 T was technically feasible. In the vast majority of the cases, tissue contrast and image details were equivalent at both field strengths. At 7 T, two cases revealed better detail detectability in the TrueFISP, and better contrast in the HASTE. Susceptibility artifacts and B 1 inhomogeneities were significantly increased at 7 T. Conclusion This study provides first insights into NE ultra-high field MRI of the small bowel and may be considered an important step towards high quality T2w abdominal imaging at 7 T MRI.

Improved real-time cardiac magnetic resonance (CMR) sequences have currently been introduced, but so far only limited practical experience exists. This study aimed at image reconstruction optimization and clinical validation of a new highly accelerated real-time cine SPARSE-SENSE sequence. Left ventricular (LV) short-axis stacks of a real-time free-breathing SPARSE-SENSE sequence with high spatiotemporal resolution and of a standard segmented cine SSFP sequence were acquired at 1.5 T in 11 volunteers and 15 patients. To determine the optimal iterations, all volunteers’ SPARSE-SENSE images were reconstructed using 10-200 iterations, and contrast ratios, image entropies, and reconstruction times were assessed. Subsequently, the patients’ SPARSE-SENSE images were reconstructed with the clinically optimal iterations. LV volumetric values were evaluated and compared between both sequences. Sufficient image quality and acceptable reconstruction times were achieved when using 80 iterations. Bland-Altman plots and Passing-Bablok regression showed good agreement for all volumetric parameters. 80 iterations are recommended for iterative SPARSE-SENSE image reconstruction in clinical routine. Real-time cine SPARSE-SENSE yielded comparable volumetric results as the current standard SSFP sequence. Due to its intrinsic low image acquisition times, real-time cine SPARSE-SENSE imaging with iterative image reconstruction seems to be an attractive alternative for LV function analysis. • A highly accelerated real-time CMR sequence using SPARSE-SENSE was evaluated. • SPARSE-SENSE allows free breathing in real-time cardiac cine imaging. • For clinically optimal SPARSE-SENSE image reconstruction, 80 iterations are recommended. • Real-time SPARSE-SENSE imaging yielded comparable volumetric results as the reference SSFP sequence. • The fast SPARSE-SENSE sequence is an attractive alternative to standard SSFP sequences.

Background Considering the currently reported association between a repetitive application and cumulative dosage of Gadolinium (Gd)-based contrast agents and Gd-deposition in brain tissue as well as the risk for the advent of nephrogenic systemic fibrosis (NSF), techniques allowing for a dose reduction become an important key aspect aside from non-enhanced magnetic resonance angiography (MRA) techniques. Thus, this study was focused on the reduction and/or complete omission of contrast agent for renal MRA at 7T. Purpose To evaluate the performance of time-of-flight MRA versus low-dose contrast-enhanced (CE) renal MRA at 7T. Material and Methods Ten healthy volunteers were examined on a 7T MR system comprising a TOF MRA and three-dimensional (3D) fast low angle shot spoiled gradient-echo sequence (FLASH) MRA after administration of one-quarter of clinical dose of gadobutrol. Qualitative image analysis was performed including overall image quality, artery delineation and presence of artifacts. Contrast ratio (CR), signal-to-noise ratio (SNR), and contrast-to-noise ratio (CNR) of the renal arteries were calculated. Results TOF MRA and low-CE MRA achieved comparable overall ratings, with slightly superior delineation of the main renal arteries in TOF MRA (TOF = 3.10 ± 0.75, low-CE = 2.95 ± 0.75). Segmental branches outside and inside the parenchyma were delineated significantly better on TOF MRA. Quantitative analysis demonstrated the superiority of TOF MRA, yielding higher scores for CR, SNR, and CNR. Conclusion The initial results of our study demonstrate the feasibility and comparable diagnostic performance of TOF and low-dose CE renal MRA at 7T.

The aim of the present study was to explore cerebellar contributions to the central executive in n-back working memory tasks using 7-T functional magnetic imaging (fMRI). We hypothesized that cerebellar activation increased with increasing working memory demands. Activations of the cerebellar cortex and dentate nuclei were compared between 0-back (serving as a motor control task), 1-back, and 2-back working memory tasks for both verbal and abstract modalities. A block design was used. Data of 27 participants (mean age 26.6 ± 3.8 years, female/male 12:15) were included in group statistical analysis. We observed that cerebellar cortical activations increased with higher central executive demands in n-back tasks independent of task modality. As confirmed by subtraction analyses, additional bilateral activations following higher executive demands were found primarily in four distinct cerebellar areas: (i) the border region of lobule VI and crus I, (ii) inferior parts of the lateral cerebellum (lobules crus II, VIIb, VIII, IX), (iii) posterior parts of the paravermal cerebellar cortex (lobules VI, crus I, crus II), and (iv) the inferior vermis (lobules VI, VIIb, VIII, IX). Dentate activations were observed for both verbal and abstract modalities. Task-related increases were less robust and detected for the verbal n-back tasks only. These results provide further evidence that the cerebellum participates in an amodal bilateral neuronal network representing the central executive during working memory n-back tasks.

OBJECTIVE Multiple cerebral cavernous malformations (CCMs) are rare lesions that occur in sporadic or familial form. Depending on the disease form, the natural history and treatment of the lesions strongly vary. Molecular analysis of an underlying germline mutation (CCM1–3) is the most sensitive screening method to distinguish between sporadic and familial cases. However, based on the different pathomechanisms that are believed to be involved in either form, significant distinctions in the CCM-associated cerebral venous angioarchitecture should be detectable. This has not been systematically studied. METHODS A consecutive series of 28 patients with multiple CCMs (681 total) diagnosed on 1.5-T MRI underwent genetic screening for CCM1–3 mutations and high-resolution susceptibility-weighted imaging (SWI) of the cerebral venous angioarchitecture with 7-T MRI. Imaging data were analyzed to examine the CCM-associated venous angioarchitecture. Results were correlated with findings of molecular analysis for CCM1–3 mutations. RESULTS Two different SWI patterns (sporadic and familial) were found. The presence of associated developmental venous anomalies correlated with negative screening for germline mutations (11 sporadic) in all cases. All patients with confirmed familial disease showed normal underlying venous angioarchitecture. Additionally, a very unusual case of a probable somatic mutation is presented. CONCLUSIONS The SWI results of the venous angioarchitecture of multiple CCMs correlate with sporadic or familial disease. These results are consistent with the theory that venous anomalies are causative for the sporadic form of multiple CCMs.

Background Cardiac magnetic resonance imaging (MRI) relies on correct ECG-gating, which is hindered in arrhythmia. Purpose To examine whether a prototype free-breathing real-time cine sequence using SPARSE-SENSE (SPARSE) improves left ventricular quantification in atrial fibrillation. Material and Methods On a 1.5T MR system left ventricular short-axis stacks were acquired of the SPARSE sequence and of a “reference” steady-state free precession (SSFP) sequence with arrhythmia rejection in 20 patients with atrial fibrillation. Two radiologists independently rated arrhythmia-caused artifact severity in both sequences using a 4-point scale. Coefficients of variation of myocardial signal intensity for both sequences were acquired. Volumetry was performed twice by one reader and once by another reader. Correlation between artifact severity and employed sequence was analyzed by modified Fisher’s exact test. Coefficients of variation and volumetric data were compared by paired t-test and intraclass correlation. Results Median arrhythmia-caused artifact severity was 2 in both readers for SSFP and 0 (reader 1)/1 (reader 2) for SPARSE, being significantly lower in SPARSE ( P Conclusion Real-time MRI with SPARSE data sampling is promising in atrial fibrillation because it reduces arrhythmia-caused artifacts.

Classical delay eyeblink conditioning is likely the most commonly used paradigm to study cerebellar learning. As yet, few studies have focused on extinction and savings of conditioned eyeblink responses (CRs). Saving effects, which are reflected in a reacquisition after extinction that is faster than the initial acquisition, suggest that learned associations are at least partly preserved during extinction. In this study, we tested the hypothesis that acquisition‐related plasticity is nihilated during extinction in the cerebellar cortex, but retained in the cerebellar nuclei, allowing for faster reacquisition. Changes of 7 T functional magnetic resonance imaging (fMRI) signals were investigated in the cerebellar cortex and nuclei of young and healthy human subjects. Main effects of acquisition, extinction, and reacquisition against rest were calculated in conditioned stimulus‐only trials. First‐level β values were determined for a spherical region of interest (ROI) around the acquisition peak voxel in lobule VI, and dentate and interposed nuclei ipsilateral to the unconditioned stimulus. In the cerebellar cortex and nuclei, fMRI signals were significantly lower in extinction compared to acquisition and reacquisition, but not significantly different between acquisition and reacquisition. These findings are consistent with the theory of bidirectional learning in both the cerebellar cortex and nuclei. It cannot explain, however, why conditioned responses reappear almost immediately in reacquisition following extinction. Although the present data do not exclude that part of the initial memory remains in the cerebellum in extinction, future studies should also explore changes in extracerebellar regions as a potential substrate of saving effects. Introduction Detailed knowledge of the biliary anatomy is essential to avoid complications in living donor liver transplantation. The aim of this study was to determine the optimal dosage of Gd-EOB-DTPA for contrast-enhanced magnetic resonance cholangiography (ce-MRC) with reference to contrast-enhanced CT cholangiography (ce-CTC). Materials and methods 30 potential living liver donors (PLLD) underwent both ce-CTC and ce-MRC. Ten candidates each received single, double or half-dose Gd-EOB-DTPA. Ce-MRC images with and without inversion recovery pulses (T1w ± IR) were acquired 20–30 min after intravenous contrast injection. Image data was quantitatively and qualitatively reviewed by two radiologists based on a on a 5-point scale. Data sets were compared using a Mann–Whitney- U -test or Wilcoxon-rank-sum-test. Kappa values were also calculated. Results All image series provided sufficient diagnostic information both showing normal biliary anatomy and variant bile ducts. Ce-CTC showed statistically significant better results compared to all ce-MRC data sets. T1w MRC with single dose Gd-EOB-DTPA proved to be superior to half and double dose in subjective and objective evaluation without a statistically significant difference. Conclusions Ce-MRC is at any dosage inferior to ce-CTC. As far as preoperative planning of bile duct surgery is focused on the central biliary anatomy, ce-MRC can replace harmful ce-CTC strategies, anyway. Best results were seen with single dose GD-EOB-DTPA on T1w MRC+IR.

Stress and additional load on the executive system, produced by a parallel working memory task, impair decision making under risk. However, the combination of stress and a parallel task seems to preserve the decision-making performance [e.g., operationalized by the Game of Dice Task (GDT)] from decreasing, probably by a switch from serial to parallel processing. The question remains how the brain manages such demanding decision-making situations. The current study used a 7-tesla magnetic resonance imaging (MRI) system in order to investigate the underlying neural correlates of the interaction between stress (induced by the Trier Social Stress Test), risky decision making (GDT), and a parallel executive task (2-back task) to get a better understanding of those behavioral findings. The results show that on a behavioral level, stressed participants did not show significant differences in task performance. Interestingly, when comparing the stress group (SG) with the control group, the SG showed a greater increase in neural activation in the anterior prefrontal cortex when performing the 2-back task simultaneously with the GDT than when performing each task alone. This brain area is associated with parallel processing. Thus, the results may suggest that in stressful dual-tasking situations, where a decision has to be made when in parallel working memory is demanded, a stronger activation of a brain area associated with parallel processing takes place. The findings are in line with the idea that stress seems to trigger a switch from serial to parallel processing in demanding dual-tasking situations.

Purpose: To assess the diagnostic accuracy of T2 mapping for the detection of myocardial edema in acute myocardial infarction (AMI), and to compare this diagnostic accuracy with that of the current standard for myocardial edema imaging, which is T2w dark-blood TSE imaging. Materials and Methods: 29 patients with AMI were examined at 1.5 T. For the visualization of myocardial edema, T2 maps, calculated from three T2w SSFP images, and T2w dark-blood TSE images were acquired in standard short- and long-axis views. Cine SSFP images were acquired for the analysis of left ventricular (LV) function and late gadolinium enhancement images (LGE) for the visualization of myocardial necrosis. The T2 maps as well as the T2w dark-blood TSE images were evaluated twice independently from the cine SSFP and LGE images. The presence or absence of myocardial edema was rated visually for each LV segment. As the standard of reference, the infarct zone was defined based on the cine SSFP and the LGE images. Results: In this segment-based analysis, T2 mapping showed a sensitivity of 82 % and a specificity of 94 % for the detection of edema in the infarct zone. T2w dark-blood TSE imaging revealed a sensitivity of 50 % and a specificity of 98 %. T2 mapping showed a higher intra-rater agreement compared to T2w dark-blood TSE imaging (κ: 0.87 vs. 0.76). Conclusions: T2 mapping allows for the visualization of myocardial edema in AMI with a high sensitivity and specificity, and features better diagnostic accuracy in terms of a higher sensitivity compared to T2w dark-blood TSE imaging. We investigated whether higher activation of the cerebellar cortex in unpredictable compared to predictable sequential finger movements reflects higher demands in motor response selection or also increases in demands on motor sequencing. Furthermore, we asked the question whether the cerebellar nuclei show a similar or reversed response profile as the cerebellar cortex. Ultra-high-field 7T functional magnetic resonance imaging was performed in nineteen right-handed, healthy young participants. Tasks involved finger tapping of a constant sequence, a random sequence, and with one finger at a time (no sequence). Conditions involved the same number of movements of fingers II-V. The three tasks were accompanied by the activation of the known hand areas within the cerebellar cortex and dentate nuclei. Activation of the cerebellar cortex and the dorsorostral dentate was significantly increased in the random-sequence condition compared to both the constant-sequence and the no-sequence conditions, with no significant difference between the constant-sequence and the no-sequence conditions. Error rate and movement frequency was not significantly different between conditions. Thus, differences between conditions cannot be explained by differences in motor execution. Because no difference was observed between the no-sequence and the constant-sequence conditions, increased cerebellar activation in the random-sequence condition likely reflects increased demands in motor response selection. Co-activation of cerebellar cortex and nuclei may be a consequence of excitatory afferent collaterals to the nuclei, "rebound-firing" of dentate neurons, and/or inhibitory synaptic input from Purkinje cells.

The aim of this study was to investigate the feasibility of dynamic contrast-enhanced 7 T MRI of the liver using an eight-channel radiofrequency (RF) transmit/receive body-coil. 16 healthy subjects were examined on a 7 T MR system utilizing a custom-built eight-channel RF body-coil suitable for RF-shimming. The following data were acquired: (1) steady state free precession imaging, (2) T2w turbo spin echo imaging, (3) T1w in and opposed-phase imaging, (4) T1w 3D FLASH images pre-contrast and in arterial, portal-venous and venous phase and (5) a fat-saturated pre- and post-contrast 2D FLASH sequence. Visual evaluation of (1) the delineation of liver vasculature, (2) the overall image quality, and (3) artifact presence and consequent image impairment was performed. SNR of the liver parenchyma was measured for the contrast-enhanced 2D and 3D FLASH sequences. For statistical analysis, a Wilcoxon-Rank Test was used. Best delineation of non-enhanced liver vasculature and overall image quality was found for 2D FLASH MRI, with only slight improvement in vessel conspicuity after the application of contrast media. T2-weighted TSE imaging remained strongly impaired, falling short of diagnostic relevance and precluding a clinical application. Our results demonstrate the feasibility and diagnostic potential of dedicated contrast-enhanced 7 T liver MRI as well as the potential for non-contrast-enhanced angiographic application.

Background: Accurate synchronization between magnetic resonance imaging data acquisition and a subject's cardiac activity ("triggering") is essential for reducing image artifacts but conventional, contact-based methods for this task are limited by several factors, including preparation time, patient inconvenience, and susceptibility to signal degradation. The purpose of this work is to evaluate the performance of a new contact-free triggering method developed with the aim to eventually replace conventional methods in non-cardiac imaging applications. In this study, the method's performance is evaluated in the context of 7 Tesla non-enhanced angiography of the lower extremities. Methods: Our main contribution is a basic algorithm capable of estimating in real-time the phase of the cardiac cycle from reflection photoplethysmography signals obtained from skin color variations of the forehead recorded with a video camera. Instead of finding the algorithm's parameters heuristically, they were optimized using videos of the forehead as well as electrocardiography and pulse oximetry signals that were recorded from eight healthy volunteers in and outside the scanner, with and without active radio frequency and gradient coils. Based on the video characteristics, synthetic signals were generated and the "best available" values of an objective function were determined using mathematical optimization. The performance of the proposed method with optimized algorithm parameters was evaluated by applying it to the recorded videos and comparing the computed triggers to those of contact-based methods. Additionally, the method was evaluated by using its triggers for acquiring images from a healthy volunteer and comparing the result to images obtained using pulse oximetry triggering. Results: During evaluation of the videos recorded inside the bore with active radio frequency and gradient coils, the pulse oximeter triggers were labeled in 62.5% as "potentially usable" for cardiac triggering, the electrocardiography triggers in 12.5%, and the proposed method's triggers in 62.5%. Evaluation of the angiography images demonstrated that under appropriate conditions the method is feasible to produce an image quality comparable to pulse oximetry. Conclusion: We conclude that cardiac triggering using the proposed method is technically feasible. However, for improved reliability the signal-to-noise ratio of the videos will have to be addressed by either replacing the camera sensor, improving the illumination, or by use of additional signal filtering techniques. One type of Internet addiction is excessive pornography consumption, also referred to as cybersex or Internet pornography addiction. Neuroimaging studies found ventral striatum activity when participants watched explicit sexual stimuli compared to non-explicit sexual/erotic material. We now hypothesized that the ventral striatum should respond to preferred pornographic compared to non-preferred pornographic pictures and that the ventral striatum activity in this contrast should be correlated with subjective symptoms of Internet pornography addiction. We studied 19 heterosexual male participants with a picture paradigm including preferred and non-preferred pornographic materials. Subjects had to evaluate each picture with respect to arousal, unpleasantness, and closeness to ideal. Pictures from the preferred category were rated as more arousing, less unpleasant, and closer to ideal. Ventral striatum response was stronger for the preferred condition compared to non-preferred pictures. Ventral striatum activity in this contrast was correlated with the self-reported symptoms of Internet pornography addiction. The subjective symptom severity was also the only significant predictor in a regression analysis with ventral striatum response as dependent variable and subjective symptoms of Internet pornography addiction, general sexual excitability, hypersexual behavior, depression, interpersonal sensitivity, and sexual behavior in the last days as predictors. The results support the role for the ventral striatum in processing reward anticipation and gratification linked to subjectively preferred pornographic material. Mechanisms for reward anticipation in ventral striatum may contribute to a neural explanation of why individuals with certain preferences and sexual fantasies are at-risk for losing their control over Internet pornography consumption.

To evaluate prospectively 7 Tesla time-of-flight (TOF) magnetic resonance angiography (MRA) and 7 Tesla non-contrast-enhanced magnetization-prepared rapid acquisition gradient-echo (MPRAGE) for delineation of intracerebral arteriovenous malformations (AVMs) in comparison to 1.5 Tesla TOF MRA and digital subtraction angiography (DSA). Twenty patients with single or multifocal AVMs were enrolled in this trial. The study protocol comprised 1.5 and 7 Tesla TOF MRA and 7 Tesla non-contrast-enhanced MPRAGE sequences. All patients underwent an additional four-vessel 3D DSA. Image analysis of the following five AVM features was performed individually by two radiologists on a five-point scale: nidus, feeder(s), draining vein(s), relationship to adjacent vessels, and overall image quality and presence of artefacts. A total of 21 intracerebral AVMs were detected. Both sequences at 7 Tesla were rated superior over 1.5 Tesla TOF MRA in the assessment of all considered AVM features. Image quality at 7 Tesla was comparable with DSA considering both sequences. Inter-observer accordance was good to excellent for the majority of ratings. This study demonstrates excellent image quality for depiction of intracerebral AVMs using non-contrast-enhanced 7 Tesla MRA, comparable with DSA. Assessment of untreated AVMs is a promising clinical application of ultra-high-field MRA. • Non-contrast-enhanced 7 Tesla MRA demonstrates excellent image quality for intracerebral AVM depiction. • Image quality at 7 Tesla was comparable with DSA considering both sequences. • Assessment of intracerebral AVMs is a promising clinical application of ultra-high-field MRA.

Catheter angiography (DSA) as gold standard for the evaluation of aneurysmal occlusion after coiling has now been largely replaced by MRI or CTA in long term observations. However, the first year after treatment is crucial because most recurrences occur in this time. Until now no guidelines exist concerning the imaging modality to adopt in this period. Aim of the study was to determine whether DSA could also be omitted in the first follow-up examination after coiling due to MRI results.489 consecutive half-year follow-up examinations consisting of DSA, CE-MRA and TOF-MRA at 1.5 or 3T were reviewed retrospectively. Visualization of residual or recurrent aneurysms in both MRA-techniques was compared to DSA by two experienced readers.Remnants/recurrences could be visualized in at least one of the three techniques in 190 (38.9%) aneurysms. Remnants/recurrences requiring retreatment (n=52) were detectable with at least one of the two MRI-techniques. In three cases (0.6%) remnants/recurrences were only visible on DSA but neither on CE-MRA nor on TOF-MRA. However, they were small (2mm) and therapy concept did not change. In one case (0.2%) they were only visible on the CE-MRA and TOF-MRA but not on the DSA, in five cases (1%) visible on DSA and TOF-MRA but not on the CE-MRA and in four cases (0.8%) not visible on the TOF-MRA but on both of the other imaging modalities.The combination of CE-MRA und TOF-MRA is also an appropriate alternative to DSA concerning the evaluation of residual or recurrent aneurysms in the crucial first follow-up.

To assess the diagnostic accuracy of phonocardiogram (PCG) gated velocity-encoded phase contrast magnetic resonance imaging (MRI).Flow quantification above the aortic valve was performed in 68 patients by acquiring a retrospectively PCG- and a retrospectively ECG-gated velocity-encoded GE-sequence at 1.5 T. Peak velocity (PV), average velocity (AV), forward volume (FV), reverse volume (RV), net forward volume (NFV), as well as the regurgitant fraction (RF) were assessed for both datasets, as well as for the PCG-gated datasets after compensation for the PCG trigger delay.PCG-gated image acquisition was feasible in 64 patients, ECG-gated in all patients. PCG-gated flow quantification overestimated PV (Δ 3.8 ± 14.1 cm/s; P = 0.037) and underestimated FV (Δ -4.9 ± 15.7 ml; P = 0.015) and NFV (Δ -4.5 ± 16.5 ml; P = 0.033) compared with ECG-gated imaging. After compensation for the PCG trigger delay, differences were only observed for PV (Δ 3.8 ± 14.1 cm/s; P = 0.037). Wide limits of agreement between PCG- and ECG-gated flow quantification were observed for all variables (PV: -23.9 to 31.4 cm/s; AV: -4.5 to 3.9 cm/s; FV: -35.6 to 25.9 ml; RV: -8.0 to 7.2 ml; NFV: -36.8 to 27.8 ml; RF: -10.4 to 10.2 %).The present study demonstrates that PCG gating in its current form is not reliable enough for flow quantification based on velocity-encoded phase contrast gradient echo (GE) sequences.Phonocardiogram gating is an alternative to ECG-gating in cardiac MRI. Phonocardiogram gating shows only limited reliability for velocity-encoded cardiac MRI. Further refinements of the post-processing algorithm are necessary.

Purpose: To investigate the feasibility of 7 Tesla (T) nonenhanced high field MR imaging of the renal vasculature and to evaluate the diagnostic potential of various nonenhanced T1-weighted (T1w) sequences. Materials and Methods: Twelve healthy volunteers were examined on a 7T whole-body MR system (Magnetom 7T, Siemens Healthcare Sector) using a custom-built eight-channel radiofrequency (RF) transmit/receive body coil. Subsequent to RF shimming, the following sequences were acquired (i) fat-saturated two-dimensional (2D) FLASH, (ii) fat-saturated 3D FLASH, and a (iii) fat-saturated 2D time-of-flight MR angiography (TOF MRA). SNR and CNR were measured in the aorta and both renal arteries. Qualitative analysis was performed with regard to vessel delineation (5-point scale: 5 = excellent to 1 = nondiagnostic) and presence of artifacts (5-point scale: 5 = no artifact present to 1 = strong impairment). Results: The inherently high signal intensity of the renal arterial vasculature in T1w imaging enabled moderate to excellent vessel delineation in all sequences. Qualitative (mean, 4.7) and quantitative analysis (SNRmean: 53.9; CNRmean: 28.0) demonstrated the superiority of TOF MRA, whereas 2D FLASH imaging provided poorest vessel delineation and was most strongly impaired by artifacts (overall impairment 3.7). The 3D FLASH MRI demonstrated its potential for fast high quality imaging of the nonenhanced arterial vasculature, providing homogeneous hyperintense vessel signal. Conclusion: Nonenhanced T1w imaging in general and, TOF MRA in particular, appear to be promising techniques for good quality nonenhanced renal artery assessment at 7 Tesla. J. Magn. Reson. To validate a phonocardiogram (PCG)-gated cine imaging approach for the assessment of left ventricular (LV) function.In this prospective study, cine MR imaging of the LV was performed twice in 79 patients by using retrospectively PCG- and retrospectively ECG-gated cine SSFP sequences at 1.5 T. End-diastolic volumes (EDV), end-systolic volumes (ESV), stroke volumes (SV), ejection fraction (EF), muscle mass (MM), as well as regional wall motion were assessed. Subgroup analyses were performed for patients with valvular defects and for patients with dysrhythmia.PCG-gated imaging was feasible in 75 (95%) patients, ECG-gating in all patients. Excellent correlations were observed for all volumetric parameters (r 0.98 for all variables analysed). No significant differences were observed for EDV (-0.24 ± 3.14 mL, P = 0.5133), ESV (-0.04 ± 2.36 mL, P = 0.8951), SV (-0.20 ± 3.41 mL, P = 0.6083), EF (-0.16 ± 1.98%, P = 0.4910), or MM (0.31 ± 4.2 g, P = 0.7067) for the entire study cohort, nor for either of the subgroups. PCG- and ECG-gated cine imaging revealed similar results for regional wall motion analyses (115 vs. 119 segments with wall motion abnormalities, P = 0.3652).The present study demonstrates that PCG-gated cine imaging enables accurate assessment of global and regional LV function in the vast majority of patients in clinical routine.Phonocardiogram-gating is an alternative to electrocardiographic-gating in cardiac MR. Phonocardiogram-gated imaging allows reliable assessment of global and regional left-ventricular function. Phonocardiogram-gating is feasible in patients with valvular lesions or cardiac dysrhythmia. Because phonocardiogram-gating is insensitive to magneto-hydrodynamic effects, it is suitable for ultra-high field.

Previous anatomical studies in monkeys have shown that forelimb motor representation is located caudal to hindlimb representation within the dorso‐rostral dentate nucleus. Here we investigate human dentate nucleus motor somatotopy by means of ultra‐highfield (7 T) functional magnetic brain imaging (fMRI). Twenty five young healthy males participated in the study. Simple finger and foot movement tasks were performed to identify dentate nucleus motor areas. Recently developed normalization procedures for group analyses were used for the cerebellar cortex and the cerebellar dentate nucleus. Cortical activations were in good accordance with the known somatotopy of the human cerebellar cortex. Dentate nucleus activations following motor tasks were found in particular in the ipsilateral dorso‐rostral nucleus. Activations were also present in other parts of the nucleus including the contralateral side, and there was some overlap between the body part representations. Within the ipsilateral dorso‐rostral dentate, finger activations were located caudally compared to foot movement‐related activations in fMRI group analysis. Likewise, the centre of gravity (COG) for the finger activation was more caudal than the COG of the foot activation across participants. A multivariate analysis of variance (MANOVA) on the x, y, and z coordinates of the COG indicated that this difference was significant (P = 0.043). These results indicate that in humans, the lower and upper limbs are arranged rostro‐caudally in the dorsal aspect of the dentate nucleus, which is consistent with studies in non‐human primates. The deep cerebellar nuclei (DCN) are a key element of the cortico-cerebellar loop. Because of their small size and functional diversity, it is difficult to study them using magnetic resonance imaging (MRI). To overcome these difficulties, we present here three related methodological advances. First, we used susceptibility-weighted imaging (SWI) at a high-field strength (7T) to identify the dentate, globose, emboliform and fastigial nucleus in 23 human participants. Due to their high iron content, the DCN are visible as hypo-intensities. Secondly, we generated probabilistic maps of the deep cerebellar nuclei in MNI space using a number of common normalization techniques. These maps can serve as a guide to the average location of the DCN, and are integrated into an existing probabilistic atlas of the human cerebellum (Diedrichsen et al., 2009). The maps also quantify the variability of the anatomical location of the deep cerebellar nuclei after normalization. Our results indicate that existing normalization techniques do not provide satisfactory overlap to analyze the functional specialization within the DCN. We therefore thirdly propose a ROI-driven normalization technique that utilizes both information from a T1-weighted image and the hypo-intensity from a T2*-weighted or SWI image to ensure overlap of the nuclei. These techniques will promote the study of the functional specialization of subregions of the DCN using MRI.

The combination of parallel acquisition (generalized autocalibrating partially parallel acquisitions) and time-resolved three-dimensional (3D) view-sharing techniques is a promising tool for dynamic contrast-enhanced 3D-magnetic resonance angiography (MRA). We evaluated the influence of different k-space acquisition strategies on image quality for a recently developed time-resolved echo-shared angiographic technique during a contrast-enhanced 3D-MRA of the thoracic vessels.In 20 patients (16 men, 4 women; range, 28-75 years), 2 dynamic MRA protocols with different k-space acquisition strategies were performed on a 1.5-T whole-body scanner (MAGNETOM Avanto, Siemens AG, Erlangen, Germany) during injection of 5 mL (flow-rate, 3 mL/s) gadobutrol. For protocol 1, the central-region which was updated with every cycle included 20% of the entire k-space (protocol 2: 10%), the peripheral-region was undersampled by a factor of 10 (protocol 2: 5%). Image quality and details were compared visually. Signal-to-noise ratio and sharpness of vessel borders were estimated.Morphologic and functional assessment of the pulmonary arteries and the aorta was significantly improved for protocol 1. The sharpness of vessel borders (3.3 mm vs. 4.1 mm; P = 0.001), image quality, and the visibility of image details were significantly improved for protocol 1 compared with protocol 2.The size of the central region that is updated for every frame seems to be more crucial for image quality of echo-shared angiographic techniques than the sampling density in the periphery of the k-space.

The present review focuses on recent developments in structural and functional magnetic resonance imaging (MRI) of the deep cerebellar nuclei (DCN), the main output structure of the cerebellum. The high iron content in the DCN allows for their visibility in T2*-weighted images. Spatial resolution has improved allowing the identification of DCN in individual cerebellar patients and healthy subjects. Based on findings in larger groups of healthy subjects, probabilistic MRI-based atlases of the deep cerebellar nuclei have been developed, which are important tools in human lesion and functional imaging studies. High iron content in the DCN, on the other hand, decreases the blood oxygenation level dependent-signal making functional imaging a difficult challenge. Compared to the vast amount of studies reporting activation of the cerebellar cortex, the number of studies demonstrating activation of the DCN is much less. Most studies report activation of the dentate nucleus. Dentate activations appear to be more reliable in more complex tasks for reasons currently unknown. As yet, few studies tried to show activations of functional subunits of the dentate nucleus. Increased signal-to-noise ratio and better spatial resolution using higher MR field strength together with recent progress in dentate normalization methods will allow identification of functional subunits and their interactions with the cerebellar cortex in future studies.

To evaluate a TGRAPPA (temporal parallel acquisition technique)-accelerated, single breath-hold multi-slice cine imaging approach for the assessment of left ventricular (LV) function.One hundred eleven patients were examined at 1.5 T. Cine imaging was performed with single-slice breath-hold acquisitions in short-axis orientation using a SSFP (TR 2.63 ms, TE 1.12 ms, FA 72°) sequence and a TGRAPPA SSFP (TR 2.66 ms, TE 1.11 ms, FA 72°, AF 3) sequence, which covered the entire LV in multiple short-axis slices during a single breath-hold. End-diastolic (EDV), end-systolic (ESV), stroke volumes (SV), ejection fraction (EF), muscle mass (MM) and regional wall motion were assessed for both data sets.Single breath-hold imaging was feasible in 108 patients. Excellent correlations were observed for all volumetric parameters derived from both data sets (all r 0.97). While EDV and ESV showed marginally lower values for single breath-hold imaging (EDV: -1.6 ± 7.9 ml; ESV: -1.8 ± 6.0 ml, p 0.05), no differences were observed for SV, EF, MM and regional wall motion assessment. Single breath-hold imaging required significant shorter acquisition times (28 ± 6 s vs. 335 ± 87 s).TGRAPPA-accelerated multi-slice SSPF imaging allows for fast and accurate assessment of regional and global LV function within a single breath-hold.

Purpose To assess potential cognitive deficits under the influence of static magnetic fields at various field strengths some studies already exist. These studies were not focused on attention as the most vulnerable cognitive function. Additionally, mostly no magnetic resonance imaging (MRI) sequences were performed. Materials and Methods In all, 25 right-handed men were enrolled in this study. All subjects underwent one MRI examination of 63 minutes at 1.5 T and one at 7 T within an interval of 10 to 30 days. The order of the examinations was randomized. Subjects were referred to six standardized neuropsychological tests strictly focused on attention immediately before and after each MRI examination. Differences in neuropsychological variables between the timepoints before and after each MRI examination were assessed and P-values were calculated Results Only six subtests revealed significant differences between pre- and post-MRI. In these tests the subjects achieved better results in post-MRI testing than in pre-MRI testing (P = 0.013–0.032). The other tests revealed no significant results. Conclusion The improvement in post-MRI testing is only explicable as a result of learning effects. MRI examinations, even in ultrahigh-field scanners, do not seem to have any persisting influence on the attention networks of human cognition immediately after exposure. J. Magn. Reson. To develop a transmit/receive radiofrequency (RF) array for MR imaging of the human spine at 7 T. The prototype is characterized in simulations and bench measurements, and the feasibility of high-resolution spinal cord imaging at 7 T is demonstrated in in vivo images of volunteers.The RF phased array consists of 8 overlapping surface loop coils with a dimension of 12 cm x 12 cm each. Bench measurements were obtained with a phantom made of body-simulating liquid and assessed with a network analyzer. For safety validation, numerical computations of the RF field distribution and the corresponding specific absorption rate were performed on the basis of 3 different human body models. In vivo images of 3 volunteers (2 with a documented scoliosis) were acquired using a 3D-FLASH sequence with a high spatial resolution of 0.57 mm isotropic.The 7 T transmit/receive RF coil could be easily integrated into the patient table for examinations of the cervicothoracic or thoracolumbosacral spine. Comparable results were found for all 3 numerical calculations using different human body models. Measurements of the g-factor indicated good image quality for parallel imaging acceleration factors up to 2.7 along the head-feet direction, which could be validated in the in vivo images. The in vivo images demonstrated very fine anatomic features such as the longitudinal ligaments or the venous drainage through the vertebral bodies. A largely homogeneous excitation over an extensive field-of-view of 40 cm could be obtained.These early results indicate that a multichannel transmit/receive phased array RF coil can be used for in vivo spine imaging at 7 T, thereby rendering high-resolution spine imaging a promising new application in 7 T clinical research.

BACKGROUND AND PURPOSE: Higher magnetic field strengths and continuous improvement of high-resolution imaging in multiple sclerosis (MS) are expected to provide unique in-vivo and non-invasive insights in pathogenesis and clinical monitoring. The purpose of this study was to investigate the potential of high-resolution imaging of MS lesions in vivo comparing 7T with conventional 1.5T. MATERIALS AND METHODS: Twelve consecutive patients with clinically definite MS were scanned on a 7T whole-body scanner and on a 1.5T Avanto. The 1.5T and 7T imaging protocol consisted of high-resolution axial proton density (PD) + T2-weighted turbo spin-echo and T2*-weighted gradient-echo (GRE), and sagittal T1-weighted 3D magnetization-prepared rapid acquisition of gradient echo. RESULTS: The sequence parameters at 7T had to be modified because of specific absorption rate (SAR) restrictions while keeping contrast parameters equivalent to 1.5T. White matter lesions were better detected and delineated from adjacent structures at 7T compared with 1.5T. There were 42% of the patients who showed additional lesions at 7T: there were 97 white matter lesions detected on 1.5T versus 126 lesions at 7T, an increase of 23%. The perivascular migration of MS lesions was well visualized on T2*-weighted GRE sequences. In larger lesions (10 mm), a multilayer structure was revealed on T2*-weighted GRE not seen at 1.5T. Because of the higher resolution, it was possible to differentiate between juxtacortical white matter lesions and cortical lesions. There were 44% of the subcortical lesions depicted at 7T that showed cortical involvement. CONCLUSIONS: Ultra-high-field imaging of patients with MS at 7T was well tolerated and provided better visualization of MS lesions in the gray matter and demonstrated structural abnormalities within the MS lesions themselves more effectively.

The human hippocampus plays a central role in various neuropsychiatric disorders, such as temporal lobe epilepsy (TLE), Alzheimer's dementia, mild cognitive impairment, and schizophrenia. Its volume, morphology, inner structure, and function are of scientific and clinical interest. Magnetic resonance (MR) imaging is a widely employed tool in neuroradiological workup regarding changes in brain anatomy, (sub-) volumes, and cerebral function including the hippocampus. Gain in intrinsic MR signal provided by higher field strength scanners and concomitant improvements in spatial resolution seem highly valuable. An examination protocol permitting complete, high-resolution imaging of the human hippocampus at 7 T was implemented. Coronal proton density, T2, T2*, and fluid-attenuated inversion recovery contrasts were acquired as well as an isotropic 3D magnetization-prepared rapid acquisition gradient-echo (500 microm isotropic voxel dimension, noninterpolated). Observance of energy deposition restrictions within acceptable scan times remained challenging in the acquisition of thin, spin-echo-based sections. At the higher resolution enabled by 7 T, demarcation of the hippocampus and some internal features including gray/white matter differentiation and depiction of the hippocampal mantle becomes much more viable when compared with 1.5 T; thus, in the future, this imaging technology might help in the diagnosis of subtle hippocampal changes.

Purpose 2- and 3-dimensional evaluation of quantitative pharmacokinetic parameters derived from the Tofts model modeling dynamic contrast enhancement of lesions in MR mammography. Materials and methods In 95 patients, MR mammography revealed 127 suspicious lesions. The initial rate of enhancement was coded by color intensity, the post-initial enhancement change is coded by color hue. 2D and 3D analysis of distribution of color hue and intensity, vascular permeability and extracellular volume were performed. Results In 2D, malignant lesions showed significant higher number of bright red, medium red, dark red, bright green, medium green, dark green and bright blue pixels than benign lesions. In 3D, statistical significant differences between malignant and benign lesions was found for all this parameters. Vascular permeability was significant higher in malignant lesions than in benign lesions. Regression model using the 3D data found that the best discriminator between malignant and benign lesions was combined number of voxels and medium green pixels, with a sensitivity of 79.4% and a specificity of 83.1%. Conclusions Quantitative analysis of pharmacokinetic variables of contrast kinetics showed significant differences between malignant and benign lesions. 3D analysis showed superior diagnostic differentiation between malignant and benign lesions than 2D analysis. The parametric analysis using a pharmacokinetic model allows objective analysis of contrast enhancement in breast lesions.

One prerequisite for transferring ultra-high-field magnetic resonance imaging (MRI) (3T) into clinical diagnostic workup is a low rate of side effects. To our knowledge, publications of subjective acceptance and willingness to undergo examinations at3T are rare. We present first results from our research site.Exposure to 7 T whole-body MRI of head, extremities, or breast was assessed in 102 subjects. They judged sources of discomfort (examination duration, room temperature) and physiological sensations (vertigo, light flashes) on a 10-point scale, differentiating between examination phases: table stationary or moving. For comparison, the same questionnaire was completed by 43 of these subjects after undergoing a 1.5 T examination. Vertigo was the most pronounced sensation at 7 T with 5% rating it as very unpleasant (none at 1.5 T). This should be compared with the fact that the lengthy exam duration was regarded as even more uncomfortable. Compared to 1.5 T, average study duration at 7 T was roughly doubled, and 7 T elicited a wider range of complaints.Although the number of side effects is increased at 7 T compared to 1.5 T, 7 T was well tolerated by the majority of subjects. Further data collection is necessary for better understanding of these effects.

OBJECTIVE Routine MR examinations of the cervical spine might be insufficient in the diagnosis of a stenotic problem of the spinal canal, especially when symptoms only occur in a specific position. Latest developments enable real-time dynamic MR examinations in the neutral position as well as anteflexion and retroflexion using a dynamic instrumentation system designed for the cervical spine. We report our experience using the dynamic NeuroSwing System. MATERIALS AND METHODS Twenty patients with symptoms of cervical spinal stenosis were included. All examinations were performed using a 1.5 Tesla MR scanner. The patients were measured in the neutral position, and in dynamic anteflexion and retroflexion. Patients set their individual limits of movement interactively during the scan. The dynamic scans were performed within 51 seconds using a real-time TrueFisp (steady-state free precession) sequence with a slice thickness of 4 mm and a temporal resolution of 2.5 images per second. RESULTS The averaged anteflexion and retroflexion were 42.3 degrees and 47.25 degrees, respectively. An increase in spinal stenosis during movement was found in 5 patients. CONCLUSION Real-time dynamic examination of the cervical spine might be a useful supplement to the conventional static examination and may allow a more precise allocation of symptoms in some patients.

This prospective study evaluates the feasibility of the magnetic resonance (MR)-guided vacuum-assisted breast biopsy with a handheld vacuum-biopsy system and documents the biopsy results with MR mammography 24 h after the procedure. MR-guided biopsy was undertaken in 33 patients with 34 lesions on dynamic MR mammography. The interventions were performed with the handheld 10-gauge Vacora vacuum-biopsy system. In all cases, dynamic MR mammography was performed 24 h after the procedure to determine the extent of the lesion removal and to identify the lesions that were missed. In 5/34 (14.7%) lesions, biopsy was not performed because no suspicious lesion was identified on the day of biopsy. In 25/29 (86.2%) lesions, the biopsy was successfully performed with a complete removal in 4/29 (13.8%) and a partial removal of 21/29 (72.4%) lesions. In 4/29 (13.8%) interventions the lesion was missed with the biopsy. In one case, histopathology after surgical excision revealed ductal carcinoma in situ. Histopathology revealed 9/29 (31%) malignant and 20/29 (68.9%) benign lesions. MR-guided vacuum-assisted breast biopsy with the handheld Vacora vacuum-biopsy system is technically feasible in most cases. MR mammography 24 h after the biopsy should be performed in those cases in which the biopsy success is unclear immediately after the procedure.

Loss of muscle mass and strength is a common symptom in the elderly population. This is partly a result of the structural changes that occur during the aging process. We applied diffusion tensor magnetic resonance imaging (DTMRI) to determine if water diffusivity in skeletal muscle changes with age.Thirty-eight healthy men, ranging from 27 to 67 years of age, were recruited for this study. The total population was grouped by age (Youngmean age 46.4: n = 20; Oldor = mean age 46.4: n = 18) and body mass index (BMI; Normal25: n = 20; Overor =25: n = 18). The principal, second, and third eigenvalues (lambda(1)or = lambda(2)or = lambda(3)), and fractional anisotropy (FA), were calculated from the diffusion tensor of the soleus, lateral and medial gastrocnemius, and anterior tibialis (AT). Analyses included comparison of groups and linear regressions.The older adults showed a significant reduction in the eigenvalues of the plantar flexors ( approximately 7%, p.05) and FA of AT ( approximately 10%, p.05) from that of the younger adults. No age differences were observed in the FA of the plantar flexors or eigenvalues of AT. lambda(1) and lambda(2) had a linear dependence on age in the plantar flexors, whereas AT showed age dependence in lambda(3) and FA.We demonstrated that DTMRI is sensitive to age-related changes in muscle, and that the effects of aging differ between the plantar flexors and AT.

To assess two compressed sensing cine magnetic resonance imaging (MRI) sequences with high spatial or high temporal resolution in comparison to a reference steady-state free precession cine (SSFP) sequence for reliable quantification of left ventricular (LV) volumes.LV short axis stacks of two compressed sensing breath-hold cine sequences with high spatial resolution (SPARSE-SENSE HS: temporal resolution: 40 msec, in-plane resolution: 1.0 × 1.0 mm(2) ) and high temporal resolution (SPARSE-SENSE HT: temporal resolution: 11 msec, in-plane resolution: 1.7 × 1.7 mm(2) ) and of a reference cine SSFP sequence (standard SSFP: temporal resolution: 40 msec, in-plane resolution: 1.7 × 1.7 mm(2) ) were acquired in 16 healthy volunteers on a 1.5T MR system. LV parameters were analyzed semiautomatically twice by one reader and once by a second reader. The volumetric agreement between sequences was analyzed using paired t-test, Bland-Altman plots, and Passing-Bablock regression.Small differences were observed between standard SSFP and SPARSE-SENSE HS for stroke volume (SV; -7 ± 11 ml; P = 0.024), ejection fraction (EF; -2 ± 3%; P = 0.019), and myocardial mass (9 ± 9 g; P = 0.001), but not for end-diastolic volume (EDV; P = 0.079) and end-systolic volume (ESV; P = 0.266). No significant differences were observed between standard SSFP and SPARSE-SENSE HT regarding EDV (P = 0.956), SV (P = 0.088), and EF (P = 0.103), but for ESV (3 ± 5 ml; P = 0.039) and myocardial mass (8 ± 10 ml; P = 0.007). Bland-Altman analysis showed good agreement between the sequences (maximum bias ≤ -8%).Two compressed sensing cine sequences, one with high spatial resolution and one with high temporal resolution, showed good agreement with standard SSFP for LV volume assessment. Spinocerebellar ataxia type 3, spinocerebellar ataxia type 6 and Friedreich's ataxia are common hereditary ataxias. Different patterns of atrophy of the cerebellar cortex are well known. Data on cerebellar nuclei are sparse. Whereas cerebellar nuclei have long been thought to be preserved in spinocerebellar ataxia type 6, histology shows marked atrophy of the nuclei in Friedreich's ataxia and spinocerebellar ataxia type 3. In the present study susceptibility weighted imaging was used to assess atrophy of the cerebellar nuclei in patients with spinocerebellar ataxia type 6 (n = 12, age range 41-76 years, five female), Friedreich's ataxia (n = 12, age range 21-55 years, seven female), spinocerebellar ataxia type 3 (n = 10, age range 34-67 years, three female), and age- and gender-matched controls (total n = 23, age range 22-75 years, 10 female). T1-weighted magnetic resonance images were used to calculate the volume of the cerebellum. In addition, ultra-high field functional magnetic resonance imaging was performed with optimized normalization methods to assess function of the cerebellar cortex and nuclei during simple hand movements. As expected, the volume of the cerebellum was markedly reduced in spinocerebellar ataxia type 6, preserved in Friedreich's ataxia, and mildy reduced in spinocerebellar ataxia type 3. The volume of the cerebellar nuclei was reduced in the three patient groups compared to matched controls (P-values < 0.05; two-sample t-tests). Atrophy of the cerebellar nuclei was most pronounced in spinocerebellar ataxia type 6. On a functional level, hand-movement-related cerebellar activation was altered in all three disorders. Within the cerebellar cortex, functional magnetic resonance imaging signal was significantly reduced in spinocerebellar ataxia type 6 and Friedreich's ataxia compared to matched controls (P-values < 0.001, bootstrap-corrected cluster-size threshold; two-sample t-tests). The difference missed significance in spinocerebellar ataxia type 3. Within the cerebellar nuclei, reductions were significant when comparing spinocerebellar ataxia type 6 and Friedreich's ataxia to matched controls (P < 0.01, bootstrap-corrected cluster-size threshold; two-sample t-tests). Susceptibility weighted imaging allowed depiction of atrophy of the cerebellar nuclei in patients with Friedreich's ataxia and spinocerebellar ataxia type 3. In spinocerebellar ataxia type 6, pathology was not restricted to the cerebellar cortex but also involved the cerebellar nuclei. Functional magnetic resonance imaging data, on the other hand, revealed that pathology in Friedreich's ataxia and spinocerebellar ataxia type 3 is not restricted to the cerebellar nuclei. There was functional involvement of the cerebellar cortex despite no or little structural changes.

There are controversies whether learning of conditioned eyeblink responses primarily takes place within the cerebellar cortex, the interposed nuclei, or both. It has also been suggested that the cerebellar cortex may be important during early stages of learning, and that there is a shift to the cerebellar nuclei during later stages. As yet, human studies have provided little to resolve this question. In the present study, we established a setup that allows ultra-high-field 7T functional magnetic resonance imaging (fMRI) of the cerebellar cortex and interposed cerebellar nuclei simultaneously during delay eyeblink conditioning in humans. Event-related fMRI signals increased concomitantly in the cerebellar cortex and nuclei during early acquisition of conditioned eyeblink responses in 20 healthy human subjects. ANOVAs with repeated-measures showed significant effects of time across five blocks of 20 conditioning trials in the cortex and nuclei (p< 0.05, permutation corrected). Activations were most pronounced in, but not limited to, lobules VI and interposed nuclei. Increased activations were most prominent at the first time the maximum number of conditioned responses was achieved. Our data are consistent with a simultaneous and synergistic two-site model of learning during acquisition of classically conditioned eyeblinks. Because increased MRI signal reflects synaptic activity, concomitantly increased signals in the cerebellar nuclei and cortex are consistent with findings of learning related potentiation at the mossy fiber to nuclear cell synapse and mossy fiber to granule cell synapse. Activity related to the expression of conditioned responses, however, cannot be excluded.

Our aim was to evaluate quantitative parametric analysis for characterization of breast lesions in contrast-enhanced magnetic resonance (MR) mammography. In 62 patients, contrast-enhanced MR mammography revealed 75 suspicious lesions, of which 18 were benign and 57 were malignant. The quantitative parametric analysis delineates signal intensity changes of contrast-enhanced lesions on a pixel-by-pixel basis. The initial rate of enhancement is coded by color intensity: a slow rate is coded to dark; a fast rate, bright. The postinitial enhancement change is coded by color hue: blue for increasing signal intensity, green for plateau, and red for decrease in signal intensity. Malignant lesions showed a significantly higher number of bright-red (P = 0.004) and medium-red (P0.001) pixels than benign lesions. Benign lesions showed significantly more blue pixels than did malignant lesions (P = 0.010). Of the 75 lesions, 72 (96%) showed heterogeneous distribution of pixel color hue. Quantitative parametric analysis of contrast kinetics in lesions can replace the subjective manual region of interest (ROI) method and makes a step toward standardization of MR mammography. It allows quantitative evaluation of different contrast kinetics parameters in contrast-enhanced breast lesions.

We sought to compare a three-dimensional, contrast-enhanced, magnetic resonance angiogram (3D CE MRA) sequence combining parallel-imaging (generalised autocalibrating partially parallel acquisitions (GRAPPA)) with a time-resolved echo-shared angiographic technique (TREAT) in an intraindividual comparison to a standard 3D MRA sequence. Four healthy volunteers (27-32 years), and 11 patients (11-82 years) with vascular pathologies of the hand were examined on a 1.5-Tesla (T) MR system (Magnetom Avanto, Siemens, Erlangen, Germany) using two multichannel receiver coils. Following automatic injection (flow rate 2.5 cc/s) of 0.1 mmol/kg gadoterate (Dotarem, Guerbet, Roissy, France), 32 consecutive 3D data sets were collected with the TREAT sequence (TR/TE: 4.02/1.31 ms, FA: 10 degrees, GRAPPA acceleration factor: R=2, TREAT factor: 5, voxel size: 1.0 x 0.7 x 1.3 mm(3)) and a T1-wwighted 3D gradient-echo sequence (TR/TE: 5.3/1.57 ms, FA: 30 degrees, GRAPPA acceleration factor: 2, voxel size: 0.71 x 0.71 x 0.71 mm(3,)). MR data sets were evaluated and compared for image quality and visualisation of vascular details. In the volunteer group, all MR imaging was successful while technical problems prevented acquisition of the standard protocol in two patients. For the corresponding segments, the number of visible segments was equal on both sequences. Overall image quality was significantly better on the standard protocol than on the TREAT protocol. TREAT MRA provided functional information in lesions with rapid blood flow, e.g. detection of feeding and draining vessels in an haemangioma. TREAT-MRA is a robust technique that combines morphological and functional information of the hand vasculature and deals with the very special physiological demands of vascular lesions, such as quick arteriovenous transit time.

Purpose To demonstrate the feasibility of using a multiecho phase-contrast (PC) gradient-echo sequence with motion-sensitizing gradient (MSG) to accelerate MR elastography (MRE) acquisitions in comparison to single-echo PC sequences. Materials and Methods The sequence was implemented and compared with a conventional single-echo sequence as the standard of reference in both agarose phantoms and in vivo in the biceps of three healthy volunteers. For reconstruction of the elasticity modulus, a local frequency estimation (LFE) algorithm was used. ETL factors of 1–16 were evaluated. Results Phantom experiments demonstrated excellent consistency between single-echo and multiecho measurements in terms of wave equivalency, SNR, and reconstructed shear modulus. Additionally, the in vivo MRE examinations showed an excellent correspondence to the single-echo results. Minor loss of wave amplitude was observed at higher ETL factors. Conclusion The results demonstrate that a multiecho sequence is suitable for accelerating MRE in nearly homogeneous tissue, such as muscle. It provides equivalent elasticity values in a significantly reduced scan time compared to a single-echo sequence. Purpose: To evaluate the three-time-point (3TP) method for diagnosis of breast lesions detected on contrast-enhanced MR mammography. Materials and Methods: MR imaging was performed in 40 women with 120 suspected breast lesions in mammography and/or sonography. The contrast kinetics was converted by 3TP software on a pixel-by-pixel basis into color-coded images. Lesion diagnosis was made by analysis of color intensity and color hue. The 3TP results were compared with the results of the region-of-interest (ROI) method. In 16 patients, we were able to correlate the results with histopathological findings. Results: The 3TP method could successfully be performed in all MR mammographies. Forty (33%) lesions had a diameter of less than 5 mm, 56 (47%) lesions between 5 and 10 mm, and 24 (20%) lesions were greater than 1 cm. Of all 120 lesions, 65 (54%) showed heterogeneous contrast enhancement. In 117 (97%) of all 120 lesions the results of ROI and the automated 3TP method were considered equivalent. However, in three lesions the manual ROI differed from the 3TP method. After a second, repeated manual ROI placement, we were able to confirm equivalent results with the 3TP images as well. Conclusions: The 3TP method automatically and reliably converts contrast kinetic information of the entire breast into a color-coded image. The 3TP method presents kinetic information of the entire dynamic series in an easy-to-interpret format and this automated method may allow to forego time-consuming and sometimes subjective manual ROI placements. This method displays the heterogeneity of the contrast enhancement pattern often observed in malignant lesions and makes it usable as diagnostic criterion.

Purpose To combine whole-body multi-station three-dimensional contrast enhanced magnetic resonance angiography (3D CE MRA) using a self-developed rolling table platform with parallel imaging strategies (PAT) in order to increase the spatial resolution of the 3D MRA data sets. Materials and methods Whole-body multi-station MRA was performed with a rolling table platform (AngioSURF) on 5 volunteers in two imaging series: 1) standard imaging protocol, 2) modified high-resolution protocol employing PAT using the generalized autocalibrating partially parallel acquisitions (GRAPPA) algorithm with an acceleration factor of 3. For an intra-individual comparison of the two MR examinations, the arterial vasculature was divided into 30 segments. Signal-to-noise ratios (SNR) and contrast-to-noise ratios (CNR) were calculated for all 30 arterial segments of each subject. Vessel segment depiction was qualitatively assessed applying a 5-point scale to each of the segments. Image reconstruction times were recorded for the standard as well as the PAT protocol. Results Compared to the standard protocol, PAT allowed for increased spatial resolution through a 3-fold reduction in mean voxel size for each of the 5 stations. Mean SNR and CNR values over all specified vessel segments decreased by a factor of 1.58 and 1.56, respectively. Despite the reduced SNR and CNR, the depiction of all specified vessel segments increased in PAT images, reflecting the increased spatial resolution. Qualitative comparison of standard and PAT images showed an increase in vessel segment conspicuity with more detailed depiction of intramuscular arterial branches in all volunteers. The time for image data reconstruction of all 5 stations was significantly increased from about 10 minutes to 40 minutes when using the PAT acquisition. Conclusion The implementation of PAT into the concept of whole-body MRA enabled to increase the spatial resolution in all examined territories, which resulted in more detailed MR angiograms.

Purpose To compare image quality, artifacts, and signal-to-noise ratio (SNR) in cardiac cine TrueFISP magnetic resonance imaging (MRI) with and without parallel acquisition techniques (PAT). Materials and Methods MRI was performed in 16 subjects with a TrueFISP sequence (1.5 T; Magnetom Sonata, Siemens): TR, 3.0 msec; TE, 1.5 msec; flip angle (FA), 60°. Three axes were scanned without PAT (no PAT) and using the generalized autocalibrating partially parallel acquisition (GRAPPA) and modified sensitivity encoding (mSENSE) reconstruction algorithms with an autocalibration mode to reduce scan time. A conventional spine array and a body flex array were used. Artifacts, image noise, and overall image quality were classified on a 4-point scale by an observer blinded to the implemented technique; for quantitative comparison, SNR was measured. Results With a PAT factor of two, acquisition time could be reduced by 39%. No PAT did not show artifacts, and GRAPPA revealed fewer artifacts than mSENSE. PAT provided inferior-quality scores concerning image noise and overall image quality. In quantitative measurements, GRAPPA and mSENSE (20.1 ± 6.2 and 15.6 ± 6.2, respectively) yielded lower SNR than no PAT (30.6 ± 20.1; P < 0.05) and P < 0.001). Conclusion Time savings in PAT are accompanied by artifacts and an increase in image noise. The GRAPPA algorithm was superior to mSENSE concerning image quality, noise, and SNR. J. Magn. Reson. Objectives: Preoperative evaluation of potential living liver donors (PLLDs) includes the assessment of the biliary anatomy to avoid postoperative complications. Aim of this study was to compare T2-weighted (T2w) and Gd-EOBDTPA enhanced T1-weighted (T1w) magnetic resonance cholangiography (MRC) techniques in the evaluation of PLLDs. Materials and Methods: 30 PLLDs underwent MRC on a 1.5 T Magnetom Avanto (Siemens, Erlangen, Germany) using (A) 2D T2w HASTE (Half Fourier Acquisition Single Shot Turbo Spin Echo) fat saturated (fs) in axial plane, (B) 2D T2w HASTE fs thick slices in coronal plane, (C) free breathing 3D T2w TSE (turbo spin echo) RESTORE (high-resolution navigator corrected) plus (D) maximum intensity projections (MIPs), (E) T2w SPACE (sampling perfection with application optimized contrasts using different flip angle evolutions) plus (F) MIPs and (G) T2w TSE BLADE as well as Gd-EOB-DTPA T1w images without (G) and with (H) inversion recovery. Contrast enhanced CT cholangiography served as reference imaging modality. Two independent reviewers evaluated the biliary tract anatomy on a 5-point scale subjectively and objectively. Data sets were compared using a Mann-Whitney-U-test. Kappa values were also calculated. Results: Source images and maximum intensity projections of 3D T2w TSE sequences (RESTORE and SPACE) proved to be best for subjective and objective evaluation directly followed by 2D HASTE sequences. Interobserver variabilities were good to excellent (k50.622-0.804). Conclusions: 3D T2w sequences are essential for preoperative biliary tract evaluation in potential living liver donors. Furthermore, our results underline the value of different MRCP sequence types for the evaluation of the biliary anatomy in PLLDs including Gd-EOB-DTPA enhanced T1w MRC. OA Förderung 2014
