Background Aspergillus fumigatus, the causal agent of aspergillosis in humans, is commonly present as a saprophyte in various organic substrates, such as spoiled silages. Aspergillosis is generally combated with demethylation inhibitor (DMI) fungicides, but the recent appearance of resistant medical and environmental strains made current treatment strategies less reliable. The goal of this study was to determine the evolution of A. fumigatus populations during the ensiling process of whole-crop corn, high moisture corn and wet grain corn, and to monitor the sensitivity of isolates from treated and untreated fields to one medical and one agricultural DMI fungicide. Results A. fumigatus was isolated from fresh forage at harvest at rather low concentrations (102 cfu g-1 ). The low frequency lingered during the silage process (at 60 and 160 days), whereas it significantly increased during air exposure (at 7 and 14 days of air exposure). Field treatment of corn with a mixture of prothioconazole and tebuconazole did not affect the sensitivity of A. fumigatus isolates. One of 29 isolates from the untreated plot was resistant to voriconazole. A unique amino acid substitution (E427K) was detected in the cyp51A gene of 10 of 12 sequenced isolates, but was not associated with DMI resistance. Conclusion A. fumigatus significantly increased during aerobic deterioration of ensilaged corn after silo opening, compared with the low presence in fresh corn and during ensiling. Field treatment of corn with DMI fungicides did not affect the sensitivity of A. fumigatus isolates collected from fresh and ensiled corn. © 2019 Society of Chemical Industry.

Perennial allergic rhinitis (PAR) is very common in children and has a relevant impact on their families. House dust mites (HDM) are the most relevant cause of PAR. The present pilot study aimed to evaluate whether hypertonic saline (3%) nasal spray as monotherapy is able to improve: nasal symptom severity and parental perception of rhinitis control, sleep, and school performance in HDM-mono-sensitized children with PAR. Globally, 25 children (13 males and 12 females; mean age 9.5±3.1 years) were treated for 3 weeks. They were visited at baseline, at the end of treatment, and after a 2-week follow-up. Hypertonic saline significantly reduced total symptom score, and improved the perception, according to their parents, of rhinitis control, sleep, and school performance. In conclusion, the present pilot study provided the first evidence that 3% hypertonic saline monotherapy was able to relieve nasal symptoms and parental perception of PAR impact as well as being safe and well tolerated.

Case report of an 89-year-old patient who had cardiorespiratory arrest during cemented hip hemiarthroplasty and required cardiopulmonary resuscitation. This complication occurred immediately after using the carbon dioxide-based lavage system known as CarboJet and was attributed to gas embolism once other entities were ruled out. The possible factors that may contribute to this surgical complication are discussed.

We report a case of a very large intraosseous ganglion in a patient with Down's syndrome. The lesion was located in the medial femoral condyle and was diagnosed with imaging techniques (X-rays, CAT scan, bone scan and magnetic resonance imaging) that allowed measuring it. Treatment consisted of curettage and filling of the defect with the skeletal repair system carbonated apatite resorbable cement which allowed for immediate early weight bearing during the postoperative period in a non-cooperative patient. We think the location and size of the lesion are of interest, as well as the peculiarities of the patient, which may help understand the pathophysiology of this entity. As far as we know, the use of resorbable cement to treat this kind of lesions has not been reported.

During a nursing conference of the Northeaster Piedmont Neonatal Intensive and Subintensive Neonatal Units the error in pediatrics and neonatology was discussed and a follow-up work was proposed with the aim to understand how many, what type of errors and what kind of adverse event they cause in our clinical practice.Through an anonymous "detection sheet" we detected the errors made between March 1 and April 30, 2010 in a NICU and 2 Subintensive therapies. The total number of patients was 166 for 2398 days of hospitalization.The total number of errors was 72, with a error of 0.43/patient. Forty-six patients had experienced at least 1 error (28% of patients) and more than a 16 (10% of our patients). There is a statistically significant correlation between days of hospitalization and the number of errors occurred (r=0.63 Sperman's correlation, P0.01); 48% and 53% of the errors in the NICU and Subintensive CU were related to medication administration.The severe damage in the NICU was caused by errors more frequently related to vascular access while the only mistake that led to a serious incident in subintensive CU was determined by a monitoring error. Errors were most frequently attributed to inattention-distraction, less frequently have been attributed to a lack of experience or a state of excessive fatigue. The data of our study were made available to all staff in order to make operators more aware of the importance of working safely.

Results of a subanalysis of data from the multinational risperidone trial (RIS-INT-2) are reported. Patients with chronic schizophrenia were treated with risperidone at 1 mg/day (n = 25), 4 mg/day (n = 27), 8 mg/day (n = 29), 12 mg/day (n = 31), or 16 mg/day (n = 29), or 10 mg/day of haloperidol for 8 weeks. According to the Positive and Negative Syndrome Scale (PANSS) total and subscale scores, improvements were noted in each treatment group from baseline to treatment endpoint. On each scale the magnitude of improvement was greater in the risperidone patients than in the haloperidol patients. The onset of action of risperidone was faster than haloperidol. By treatment week 2, over half of the patients receivingor = 4 mg/day of risperidone were clinically improved (or = 20% reduction in total PANSS scores). This rate of improvement was not seen until week 6 in the haloperidol patients. Severity of extrapyramidal symptoms (scores on the Extrapyramidal Symptom Scale) was significantly lower in patients receiving 1 or 4 mg/day of risperidone than in patients receiving higher risperidone doses and in haloperidol patients. The optimal dose of risperidone, in terms of both efficacy and safety, was 4 mg/day. These results confirm the findings of the controlled trials of risperidone conducted in North America and the multinational trial.

Effective and adequate therapy to control pain and stress are essential in managing children in Pediatric Intensive Care Unit (PICU) undergoing painful invasive procedures, this should be, but is not yet, one of our main aims. Aware that this difficult mission must be pursued in a systematic, multimodal and multitasking way, the Studying Group on Analgosedation in PICU from the Italian Society of Neonatal and Paediatric Anesthesia and Intensive Care (SARNePI) is providing its recommendations.

This case report concerns a newborn with bilateral adrenal massive hemorrhage complicated by inferior cava vein thrombosis, hypertension and encephalopathy to elaborate diagnostic and therapeutic problem of neonatal thromboembolic events. Increasing of risk population (extreme prematurity), improvement of imaging, laboratory and neonatologist care enhanced in recent years the diagnosis of this group of diseases. Even if literature about this subject is poor, we remark the significant effort of neonatologist to revise and update neonatal guidelines and the relevance of the institution of the National Registry of Neonatal Thrombosis (RITI).

We describe two cases of delayed delivery in dichorionic, diamniotic pregnancies, where we used an Endoloop ligature to clamp the umbilical cord with excellent maternal and fetal long-term outcome.

A 76 year old male patient with a history of implantation of a total hip arthroplasty Perfecta (Orthomet(®)), who presented with an iliac fossa mass, increased diameter of the thigh, and pain during hip flexion and extension. CT and ultrasound show the presence of a giant cystic mass in left iliac fossa about 7 cm in diameter next to the prosthesis. A pseudo-tumour secondary to wear debris after placement of a total arthroplasty is rare. We present a case of a large iliopsoas bursitis caused by polyethylene particles, which caused compression and thrombosis of the superficial femoral vein.

Community-acquired pneumonia (CAP) is a leading cause of childhood death. There are few published reports of radiographic findings among children with severe CAP.To describe chest X-ray (CXR) findings and assess association between these radiographic findings and pneumococcal isolation in children with severe CAP.A prospective, multicenter, observational study was conducted in 12 centers in Argentina, Brazil, and the Dominican Republic. Children aged 3-59 months, hospitalized with severe pneumonia, were included. On admission, blood and pleural effusion cultures were performed. Streptococcus pneumoniae was identified according to standard procedures in the respective national reference laboratory. Chest X-rays were taken on admission and read before the culture results were reported.Out of 2,536 enrolled patients, 283 (11.2%) had S. pneumoniae isolated, in 181 cases (7.1%) from blood. The follow radiographic patterns were observed: alveolar infiltrate (75.2%), pleural effusion (15.6%), and interstitial infiltrate (9.2%). Overall, pleural effusion was associated with pneumococcal isolation and pneumococcal bacteremia (P0.001). Infiltrates were unilateral (78.7%) or bilateral (21.3%), right-sided (76%) or left-sided (24%), in the lower lobe (53.6%) or the upper lobe (46.4%). Multivariate analysis including patients with affection of only one lobe showed that upper lobe affection and pleural effusion were associated with pneumococcal isolation (OR 1.8, 95% CI, 1.3-2.7; OR 11.0, 95% CI, 4.6-26.8, respectively) and with pneumococcal bacteremia (OR 1.7, 95% CI, 1.2-2.6; OR 3.1, 95% CI, 1.2-8.0, respectively).Three-quarters of the patients studied had alveolar infiltrates. Upper lobe compromising and pleural effusion were associated with pneumococcal invasive disease.

The identification of pre and postnatal risk factors as responsible of hypoxic-ischemic brain injury damage results very important in predicting the neurological outcome of newborns. The aim of this study was to investigate a possible correlation between pre and postnatal risk factors and severe acidosis at birth.We collected all data of 559 newborns in our Hospital with risk factors. Risk factors have been subdivided in: maternal, placental, fetal, umbilical and obstetrical risk factors.Of 559 newborns we collected umbilical cord pH and base excess (BE) in 411. Of these, 32 showed severe acidosis at birth: 29 full-term and 3 preterm. Fourteen out of 32 had almost one obstetric risk factor. Among all obstetric risk factors the application of the vacuum was statistically significant (9/59 applications of vacuum; P 0.003). No signs of brain injury were seen at cerebral echography in 29 cases of acidosis at birth. In all 3 premature neonates we found intraventricular hemorrhage, but without apparent neurological consequences at follow-up. Of 559 newborns with risk factors 21 have been intubated at birth. Only in premature deliveries a significant correlation between intubation at birth and acidosis was found.The only factor which seems to play a significant role in developing a severe acidosis at birth is the use of vacuum in full-term deliveries. Umbilical cord pH and BE are related to intraventricular hemorrhage and need of intubation in delivery room for preterm infants. However, in both groups, no significant effect of acidemia on neurological outcome could be demonstrated.

Since the discovery of neuroleptics and the reintegration of people with schizophrenia in the community, psycho-social reeducation became an essential part of the treatment. The Life Skills Profile is a tool of reference for assessment of the dimensions, which have an impact on the adaptation in community. Each item describes an observable behavior, written in common language, to allow the care-givers and the family to evaluate it without having a specific formation. The long version (39 items) is recommended for therapeutic interventions with a person and the short one (20 items) for large scale studies on outcome in community.In order to dispose of a measure of function and disability in schizophrenia, we have translated the Life Skills Profile (LSP) in French and tested the validity of this translation for the long version (39 items) and the short ones (16 and 20 items). The 4 dimensions of the 16-items version--"withdrawal", "self-care", "compliance" and "antisocial"--were used for people with mental disorders and the 20-items version enhanced with a fifth dimension "bizarre", especially useful for people with schizophrenia.175 people suffering of schizophrenia (DSM IV, codes F20.0 to F20.5) were evaluated by their caregivers in 3 settings: psychiatric hospital, ambulatory care and sheltered homes. Confirmatory factorial analyses were performed to test the dimensional models and their psychometric characteristics were established.The original structure in 5 dimensions of the long version (39 items) is not confirmed. However, the short versions in 16 and 20 items were confirmed, without any modification. The required psychometric qualities of reliability and validity of the 20-items version were fulfilled. The inter-rater reliability ranged from 0.65 to 0.75 for the 5 dimensions; the test-retest reliability ranged from 0.80 to 0.91 and the internal consistency from 0.67 to 0.81. The validity was evaluated by comparison of the LSP scores upon living arrangements (people living in sheltered homes had lesser scores) and pathologies (19 people with severe major depression had higher scores).The 39-items version of the Life Skills Profile was not validated in French. However, for clinical practice of social rehabilitation, this tool remains useful for a single person to check specific behaviors, which could hinder his/her integration into the community, to plan specific interventions and to evaluate changes, in addition with other scales. By the validation of the short version of the Life Skills Profile, an instrument is at disposal in French for outcome studies which allow to: 1) quickly assess the social functioning of person suffering of schizophrenia by a caregiver or a family member; 2) to detect insufficient skills in specific domains; and 3) to evaluate therapeutic efficiency.

Anxiolysis and pain control are a duty for physicians and must be treated very carefully in the Pediatric Intensive Care Units, although it is very difficult to assess them: in critically ill children sedatives and/or analgesic medications are routinely provided and titrated to obtain a satisfactory level of sedation, but different evaluation scores are needed to discriminate between light or inadequate and deep or excessive sedation, especially when the clinical examination is unavailable. It is usual to associate a benzodiazepin with an opioid, more often Midazolam and Morphine or Fentanyl; other drugs as Propofol, Clonidine and Ketamine have specific indications, brief painful procedures and weaning from long periods of sedation to avoid withdrawal. Sometimes it can be useful to add a neuromuscular blocking agent to help mechanical ventilation. Adverse sedation events are relatively frequent, associated with drug overdoses and drug interactions, particularly when 3 or more drugs are used: all class of medications and all routes of administration are involved.

The purpose of this study was to evaluate the prevalence of pathological gambling in the Swiss adult population before the introduction of new forms of gambling, and the link between pathological gambling and alcohol abuse.2526 telephone interviews were completed using standardized assessment instruments for identifying potential and probable pathological gamblers (SOGS) and alcohol abuse (CAGE).The current prevalence of probable and potential pathological gamblers were estimated to be 0.8% and 2.2%, respectively. The results also show a clear correlation between alcohol abuse and gambling behaviours.For the first time estimations are available of the Swiss prevalence rates of pathological gambling. Considering the link between gambling availability and increases in the prevalence of pathological gambling and the correlation between alcohol abuse and pathological gambling, the implications of these results for the prevention and treatment of this debilitating disorder are discussed.

We report a case of severe accidental hypothermia (24.8 degrees C) in a seven-year-old child due to prolonged exposure to low temperatures and temporary contact with river water. When the patient was seen in hospital, bradycardia (30.min-1), bradypnoea (5. min-1), scarcely reacting pupils, and Glasgow Coma Scale=3 were noted. For rewarming minimally invasive techniques (humidified warmed gases and intravenous solutions at 40 degrees C) were employed with a very successful outcome.

This study examines the diagnosis of adjustment disorder in DSM-III-R. In view of the methodological problems associated with this, we investigated clinical characteristics of patients to whom the diagnosis had been applied. The patients in question were treated at the University Psychiatric Outpatient Clinic in Lausanne, Switzerland (N = 1,308). Four groups of comparisons are made: between adjustment disorder, other diagnoses, and no diagnosis; between adjustment disorder with depressed mood and other mood disorders; between adjustment disorder with anxious mood and anxiety disorders; and between the clinical subdivisions of adjustment disorder. This diagnosis is characterized by a high number of marital stressors, low age, predominance of women, and short period of treatment. The clinical profile is marked by a predominance of depressive and mixed types, and the relationship between these is demonstrated. Finally, the course of a certain proportion of these disorders goes beyond the 6-month period stipulated by DSM-III-R. This result justifies the modifications introduced in DSM-IV.

Sixty-seven first episode schizophrenic patients (PSE-Catego criteria) have been included in this study in order to evaluate their prognosis and the factors predictive of their evolution. Potential predictive factors consisted of anamnestic and demographic data, scores on the Disability Assessment Schedule (DAS-WHO) and relatives' Expressed Emotion index (EE), measured by the Camberwell Family Interview (CFI). The outcome was assessed monthly by the Global Assessment Scale (GAS/DSM III-R). At four years, 39 patients (58%) were still being followed. 33% of the patients presented a good evolution (EGFor = 51) and 67% of the patients a bad evolution (EGF51). Four factors predictive of the psychosocial adaptation were extracted using regression analysis: premorbid psychosocial evolution, EE, sex and psychiatric family history. These 4 factors predicted correctly 85% of cases. Moreover, the monthly follow-up of these patients pointed to three types of evolution: the patients presenting a good and stable evolution (22%), those presenting a bad and stable evolution (33%) and those presenting an oscillating evolution which fluctuated between good and bad periods (44%). However, no predictive factors of the psychosocial adaptation of these oscillating patients could be identified through the statistical analysis. These results take all their importance regarding the treatment of schizophrenic patients, for whom the therapeutic plans which have to be settled should take into account their prognosis in the most precise manner. Moreover, the predictive value of EE on psychosocial adaptation for a 4 years period is confirmed.

Expressed Emotion (EE) proved to be the best single predictor of relapse in schizophrenia in 1972. Since then, studies on EE were oriented in three directions: Replication of the original study in different countries, improvement of the methodology and clinical applications. The analysis of different key studies shows that EE is a powerful predictor factor, although it is influenced by other variables such as culture.

The authors report their clinical experience of 15 cases of femoro-popliteal by-pass with PTFE; all patients underwent postoperative anti-platelet therapy, in 8 cases with ASA and dipyridamole, in 7 with indobufene in accordance with a double blind randomized work. The Authors report the results of a one year follow-up. They deal with patency duration results and tolerance of pharmacological treatment.

The Authors report their fifteen years experience, point out the clinical features of inflammatory abdominal aortic aneurysms and examine the different pathogenetic theories.

Fifty patients with 65 popliteal aneurysms underwent reconstructive procedures (mean age 64). Atherosclerosis is almost the exclusive cause of popliteal aneurysms. Most of the cases showed an acute ischemia, while he others were asymptomatic or presented a complication of venous occlusion or neural compression. Numerous cases were found incidentally at amputations (these were not included). The best operational method is ligation of the aneurysm sometimes with partial resection and autologous venous graft bypass. The immediate results were excellent in all cases, loss of limb was observed in only three.

Indications for a unilateral or bilateral axillofemoral and femorofemoral bypass, are presented. Our experience in 48 patients with axillofemoral and femorofemoral form the basis of this report. Indications for these extended bypasses included suppurative growing infection and obliterated common and profunda femoris arteries. These procedures were rapidly performed in critically ill patients. One patient died of recurrent artoduodenal hemorrhage three days following successful axillofemoral bypass. The other patients enjoyed patent grafts for variable periods, although some of them successfully underwent a declotting procedure.

A study of 36 French-speaking families in Geneva identified 66% of high-EE household. At nine-month follow-up, the relapse rate for the high-EE group (33%) was significantly higher than for the low-EE group (0%). These results seem to confirm the relevance of EE as predictor of relapse. However, in our sample, it appears that one component of EE, critical comments, was principally responsible for the prognostic validity.

Three typical cases of segmental portal hypertension due to occlusion of the splenic vein are reported. This syndrome may be asymptomatic for a very long time and then present suddenly in the form of a serious picture of high digestive haemorrhage due to rupture of the varices of the fundus of the stomach as a result of hypertrophized submucous collateral drainage circulation. Useful for diagnosis are oesophagogastroduodenoscopy, which points to stomach varices, and splenoportography or superselective arteriography of the splenic artery with venous phase, which highlight pathognomonic dilatation and tortuosity of the gastroepiploic veins. Surgical exploration typically shows: presence of large epipolic vessels, splenomegaly, absence of changes in the liver and in the portal and mesenteric circulation. Resolutive treatment of choice is splenectomy.

The present report is based on a review of literature and shows that violence is a rare manifestation, which results from the intrications of numerous factors and which is difficult to predict even in the short term. Preventive work should be based on the knowledge of some personal as well as relational risk factors.

A method for assessing the acid-base balance in patients admitted to the Intensive Care Department, making use of the Graphic Display option of a minicomputer is presented. Patient condition can be plotted on present graphs whose coordinates are the pH and PACO2 measured at successive moments. It is thus possible to visualise the behavior of acid-base balance over a period, as well as the extent of renal or respiratory compensation. The data relating to a group of patients are reported by way of illustration.

This work presents an analysis instrument and the first results obtained while applying it to 280 depressive patients compared to 496 non-depressive patients who were followed during a 4 year period. The treatment follow-up permits us to present the chronological "history" of the patient's contacts with different services and to establish a classification. One of the interests, for the clinician, consists in specifying the best therapeutical approach possible. A more detailed study should allow us to relate certain patterns with diagnostic subcategories and to determine if the high proportion of short patterns corresponds to the "successful" treatments or to the dropouts.

In spite of the many advances in diagnosis, pulmonary embolism is still hard to identify, one problem stemming from the similarity of its clinical signs with heart and lung conditions. The present study was carried out on 17 patients affected by massive pulmonary embolism. Clinical aspects and diagnostic problems are discussed, with particular regard to the data supplied by chest X-ray, catheterism of the right cardiac cavities and angiopneumographic investigation.

A case of total stripping of the femoral axis secondary to saphenectomy is reported. This type of accident is followed by amputation unless quickly recognised and corrected. A combined saphenous vein and PTFE graft interposed between the origin of the superficial femoral and the distal part of the posterior tibial remained patent for three months. Its eventual thrombosis, however, did not result in loss of the limb, because a sound compensatory circulation had been established in the meanwhile. It is therefore felt that repair surgery is always worth attempting, even in the case of extensive arterial lesions that will otherwise necessitate amputation.

Six cases of delayed rupture of the spleen in two stages are described. One case of occult rupture is also presented. A picture of acute anaemia was observed in the five two-stage cases, whereas the occult rupture was diagnosed as organised splenic haematoma following computerised scintiscanning, arteriography and tomography. Splenectomy was performed in all cases, under emergency conditions in the first five, and of choice in the sixth. No post-operative complications were noted.

The author investigates the evolution of relationships between epidemiology and diagnostic classifications. The scope of neurotic disorders is considerably broader than the scope of anxiety disorders in the DSM IIIR, despite the introduction of new concepts such as panic disorder. Epidemiological studies require today diagnostic instruments based on structured interviews. They provide more homogeneous groups of patients. The recognition of the importance of the anxiety disorder's prevalence is a prerequisite for improving both therapeutic and prophylactic interventions.

This paper reviews some of the important studies on the genetic aspects of anxiety, based on clinical, family and twin studies. Well-known methodological difficulties characteristic of earlier studies are at this time partly resolved. Researchers use specific criteria for defining subcategories of anxiety disorders. These are being applied in large epidemiological family studies and national twin registers. The part played by genetic or environmental factors varies greatly according to diagnostic sub-category. A genetic influence seems important for panic disorders and agoraphobia, but not for generalized anxiety disorder. A validation of these sub-categories could be obtained from discoveries of molecular genetics.
