Study design This is a prospective observational study. Objective The aim of this study is to determine the rate of occult infection after instrumented spine surgery in presumed aseptic patients. Summary of background data The reported incidence rate of delayed/occult infection determined by positive culture swabs after instrumented spine surgery in prospective studies is 0.2%-6.9%. However, this rate may be higher as delayed infections are challenging to diagnose. Fever can be absent and inflammatory markers are often normal. If indolent organisms exist in low concentrations surrounding the instrumentation, these organisms can possibly avoid detection and disrupt bone formation leading to instrumentation loosening, pain generation, and/or failure of a solid fusion. Materials and methods This study included 50 consecutive presumed aseptic patients undergoing a posterior revision requiring removal of instrumentation at least 6 months following their index procedure. Common markers of infection were examined preoperatively. Multiple culture swabs were taken directly from the removed instrumentation and cultured for 14 days. Results Of the 50 patients, 19 (38%) were culture-positive (CP) for bacteria upon removal of their instrumentation, with 14 patients (28%) having ≥2 positive specimens of the same organism. The average length of time between the index procedure and the revision surgery was 4.55 years (range: 0.53-21 y). Polymicrobial infections were found in 26% (5/19) of CP patients. The most prevalent microorganism found was Propionibacterium acnes, in 63% (12/19) of CP patients. There was no significant difference between CP and culture-negative patients regarding preoperative markers for infection, age, or length between index and revision procedures. Conclusions The results of this study indicate a positive culture rate of 38% in presumed aseptic patients who had previously undergone instrumented spine surgery. These results are consistent with other retrospective studies and are >6 times greater than any previous prospective study utilizing culture swabs. Level of evidence Level-III.

Summary: Background: Lumbar intervertebral disc (IVD) herniations are associated with significant disability. Discectomy is the conventional treatment option for IVD herniations but causes a defect in the IVD, which has low self-repair ability, thereby representing a risk of further IVD degeneration. An acellular, bioresorbable, and good manufacturing practice (GMP)-compliant in situ-forming gel, which corrects discectomy-associated IVD defects and prevents further IVD degeneration had been developed. However, this acellular matrix-based strategy has certain limitations, particularly in elderly patients, whose tissues have low self-repair ability. The aim of this study was to investigate the therapeutic efficacy of using a combination of newly-developed, ultra-purified, GMP-compliant, human bone marrow mesenchymal stem cells (rapidly expanding clones; RECs) and the gel for IVD regeneration after discectomy in a sheep model of severe IVD degeneration. Methods: RECs and nucleus pulposus cells (NPCs) were co-cultured in the gel. In addition, RECs combined with the gel were implanted into IVDs following discectomy in sheep with degenerated IVDs. Findings: Gene expression of NPC markers, growth factors, and extracellular matrix increased significantly in the co-culture compared to that in each mono-culture. The REC and gel combination enhanced IVD regeneration after discectomy (up to 24 weeks) in the severe IVD degeneration sheep model. Interpretation: These findings demonstrate the translational potential of the combination of RECs with an in situ-forming gel for the treatment of herniations in degenerative human IVDs. STUDY DESIGN This was a prospective cohort study. OBJECTIVE The objective of this study was to establish minimal clinically important difference (MCID) and substantial clinical benefit (SCB) thresholds for Patient-Reported Outcomes Measurement Information System (PROMIS) in cervical spine pathology. SUMMARY OF BACKGROUND DATA PROMIS enables improved psychometric properties with reduced questionnaire burden through computer adaptive testing. Despite studies showing good correlation with "legacy" outcome measures, literature on the clinical significance of changes in PROMIS scores is scarce. MATERIALS AND METHODS Adult patients undergoing cervical spine surgery at a single institution between 2016 and 2018 were prospectively enrolled. Patients completed questionnaires [Short Form-36 (SF-36), Neck Disability Index (NDI), Visual Analogue Scale Arm/Neck, and PROMIS Pain Interference (PI) and Physical Function (PF)] preoperatively and at 6 months postoperatively. MCID was calculated using distribution-based and SCB using anchor-based methods. The SF-36 Health Transition Item was utilized as an anchor with the cutoff values chosen using receiver operating characteristic curve analysis. RESULTS There were 139 patients meeting inclusion criteria, with a mean age of 56.4 years and diagnoses of myelopathy (n=36), radiculopathy (n=48) and myeloradiculopathy (n=49). Study design Randomized, double-blinded, controlled trial. Objective To investigate the effectiveness of local intraoperative corticosteroids at decreasing the severity of swallowing difficulty following multilevel anterior cervical discectomy and fusion (ACDF). Summary of background data Dysphagia is a common complication after ACDF, and while for most patients the symptoms are mild and transient, some patients can suffer from severe dysphagia resulting in significant postoperative morbidity. Previous studies investigating the local application of corticosteroids are limited. Methods This was a prospective, randomized, double-blinded, controlled trial of patients undergoing 2, 3, or 4 level ACDF for radiculopathy and/or myelopathy. Patients undergoing multilevel ACDF were randomized to receive local corticosteroid in the retropharyngeal space or placebo (no steroid). Dysphagia was assessed using validated outcomes including the Eating Assessment Tool-10 (Eat-10) and Swallowing Quality of Life (SWAL-QOL) Questionnaire both preoperatively and at 1 day (POD1), 2 days (POD2), and 1-month postoperatively. Results One-hundred nine patients had a complete dataset available for analysis. Eat-10 scores were significantly lower in the Steroid group on POD2 (8 vs. 16, P = 0.03) and 1-month postoperatively (2 vs. 5, P = 0.03). A comparison of the individual SWAL-QOL subscale scores demonstrated that patients in the Steroid group had better scores than the Control group in various subscales at all postoperative time points. Significant differences were noted (always in favor of the Steroid group) in 40% of subscales on POD1, 60% of subscales on POD2, and 50% of subscales at 1-month postoperatively. The Control group never had a better SWAL-QOL subscale score at any time point postoperatively. Conclusion Local administration of corticosteroid after multilevel ACDF can decrease postoperative severity and symptomatology of dysphagia during the immediate postoperative period to 1-month postoperatively. The long-term effects of local steroid administration on fusion and other complications will need to be established in future studies.Level of Evidence: 1.

Longitudinal cohort.The aims of this study were to measure concordance between patients' and surgeons' preoperative expectations of lumbar surgery, and determine which member of the dyad more closely predicted fulfillment of expectations, defined as patient-reported status postoperatively.Concordant patient-surgeon expectations reflect effective communication and should foster better outcomes.Preoperatively patients and surgeons completed identical surveys measuring expectations for improvement in symptoms and physical/psychosocial function. Responses ranged from "complete improvement" to "do not have this expectation"; scores for each survey ranged from 0 to 100 (greatest expectations). Concordance between pairs of patient-surgeon scores was measured with the intraclass correlation coefficient (ICC). Postoperatively, fulfillment of expectations was measured from patient-reported amount of improvement received and was calculated as the proportion of patient-reported postoperative score relative to patient-reported preoperative score, and surgeon-reported preoperative score (range 0 [no expectations fulfilled] to1.2 [expectations surpassed]). Clinical measures included patient-reported spine-related disability.For 402 patient-surgeon pairs, mean survey scores were 73 ± 19 (patients) and 57 ± 16 (surgeons); 84% of patients had higher scores than surgeons, mainly due to expecting complete improvement, whereas surgeons expected a lot/moderate/little improvement. The ICC for the entire sample was .31 (fair agreement); for subgroups, the greatest difference in ICC was for patients with more spine-related disability (ICC = .10, 95% confidence interval [CI]:0.00-0.23) versus less disability (ICC = .46, 95% CI: 0.34-0.56). 96% of patients were contacted ≥2.0 years postoperatively. Proportions of expectations fulfilled were 0.79 (0-3.00) (patients) and 1.01 (0-2.29) (surgeons). Thus patients were less likely to anticipate subsequent postoperative status (odds ratio [OR] 0.34, 95% CI 0.25-0.45) versus surgeons who were more likely to anticipate patient-reported postoperative status (OR 2.98, 95% CI: 2.22-4.00).Concordance between patients' and surgeons' expectations was fair; due mostly to patients expecting complete improvement whereas surgeons expected a lot/moderate/little improvement. Compared to patients' expectations, surgeons' expectations more closely coincided with patient-reported fulfillment of expectations 2 years postoperatively.Level of Evidence: 1.

Prospective observational study OBJECTIVE.: The aim of this study was to record daily opioid use and pain levels after 1-level lumbar decompression or microdiscectomy.The standardization of opioid-prescribing practices through guidelines can decrease the risk of misuse and lower the number of pills available for diversion in this high-risk patient population. However, there is a paucity of quantitative data on the "minimum necessary amount" of opioid appropriate for post-discharge prescriptions.At two institutions between September 2017 and 2018, we prospectively enrolled 85 consecutive adult patients who underwent one-level lumbar decompression or microdiscectomy. Patients with a history of opioid dependence were excluded. Daily opioid consumption and pain scores were collected using an automated text-messaging-based platform for 6 weeks or until consumption ceased. Refills during the study period were monitored. Patients were asked for the number of pills left over and the method of disposal. Opioid use was converted to oral morphine equivalents (OMEs). Results are also reported in terms of "pills" (oxycodone 5 mg equivalents) to facilitate clinical applications. Risk factors were compared between patients in the top and bottom half of opioid consumption.Total opioid consumption ranged from 0 to 118 pills, with a median consumption of 32 pills (236.3 OME). Seventy-five percent of patients consumed ≤57 pills (431.3 OME). Mean Numeric Rating Scale pain scores declined steadily over the first 2 weeks. By postoperative day 7 half of the study population had ceased taking opioids altogether. Only 22.4% of patients finished their initial prescription, and only 9.4% of patients obtained a refill.These data may be used to formulate evidence-based opioid prescription guidelines, establish benchmarks, and identify patients at the higher end of the opioid use spectrum.2.

STUDY DESIGN A cross-sectional, mixed methods analysis of sources of expectations of lumbar surgery. OBJECTIVE The aim of this study was to ascertain sources of expectations and compare them to clinical characteristics. SUMMARY OF BACKGROUND DATA Understanding where patients obtain expectations of lumbar surgery is necessary in order to develop interventions to foster appropriate expectations. METHODS This was a qualitative-quantitative study of 428 patients interviewed preoperatively with a 20-item validated survey, which asks patients how much improvement they expect per item. Patients then were asked open-ended questions about how they came to have these expectations and, using qualitative analysis, responses were grouped into themes representing different sources of expectations. The likelihood of citing various sources was then assessed with odds ratios (ORs) based on demographic and clinical characteristics. RESULTS Patients' mean age was 55 years, 80% had degenerative diagnoses and 24% had prior lumbar surgery. Patients volunteered multiple sources; most prevalent were current surgeon (83%), internet resources (55%), social network contacts (26%), other physicians (22%); and previous experience (65%) for the subgroup who had prior lumbar surgery. Patients were more likely to cite their surgeon if they had less disability [OR 2.8, confidence interval (CI) 1.3-5.8, P = 0.007], were treated with conservative care, such as physical therapy (OR 2.7, CI 1.6-4.7, P = 0.0003), and had symptoms for ≤12 months (OR 1.8, CI 1.1-3.0, P = 0.03). Study design Observational study. Objective To demonstrate use of magnetic resonance (MR) neurography to visualize the course of the lumbar plexus at the L4-L5 disc space. Summary of background data Risk of injury to the lumbar plexus during lateral transpsoas approach for lumbar interbody fusion (LLIF) is significant. We describe a new technique for preoperative mapping using magnetic resonance neurography to directly visualize the course of the plexus relative to the L4-L5 disc space. Methods Consecutive lumbar plexus MR neurograms (n=35 patients, 70 sides) were studied. Scans were obtained on a Siemens 3-Tesla Skyra magnetic resonance imaging scanner. T1- and T2-color-coded fusion maps were generated along with 3-dimensional models of the lumbosacral plexus with attention to the L4-L5 interspace. The position of the plexus and the shape of the psoas muscle at the L4-L5 interspace were evaluated and recorded. Results Direct imaging of the lumbar plexus using MR neurography revealed a substantial variability in the position of the lumbar plexus relative to the L4-L5 disc space. The left-side plexus was identified in zone 2 (5.7%), zone 3 (54.3%), and zone 4 (40%) (P = 0.0014); on the right, zone 2 (8.6%), zone 3 (42.9%) or zone 4 (45.7%), and zone 5 (2.9%) (P = 0.01). Right-left symmetry was found in 18 of 35 subjects (51.4%) (P = 0.865). There was no association between the position of the plexus and the shape of the overlying psoas muscle identified. In patients with an elevated psoas (n = 12), the lumbar plexus was identified in zone 3 in 75% and 66% (left and right) compared with patients without psoas elevation (n = 23), 30.4% and 43.5% (left and right). Conclusion The course of the lumbosacral plexus traversing the L4-L5 disc space may be more variable than has been suggested by previous studies. Magnetic resonance neurography may provide a more reliable means of preoperatively identifying the plexus when compared with current methods. Level of evidence 3.

STUDY DESIGN Retrospective case series. OBJECTIVE To determine the clinical and radiographic outcomes of patients undergoing minimally invasive lateral lumbar interbody fusion (LLIF) with a minimum 2-year follow-up. SUMMARY OF BACKGROUND DATA Minimally invasive LLIF is performed through a lateral, retroperitoneal, transpsoas approach. This procedure is characterized by the use of a tubular retractor to minimize tissue damage and real-time neuromonitoring to ensure safe passage through the psoas muscle. To date, advantages of minimal invasive LLIF, compared with open procedures, has been limited to early postoperative outcomes and complications, with the longest mean follow-up duration of 22 months. METHODS A total of 118 patients who underwent minimally invasive LLIF with a minimum of 2 years follow-up were included in this study. Clinical outcomes were determined by using Visual Analog Score for the degree of pain (trunk or lower extremity), and Oswestry Disability Index and Short Form-12 scoring methods for patient function. Radiographic evaluations included (i) disk height; (ii) segmental coronal angulation; (iii) segmental lordotic angulation; (iv) Cobb angle; (v) cage subsidence; and (vi) fusion status. Preoperative psychological symptoms predict surgical outcomes. The impact of surgical outcomes on psychological well-being, however, has not been delineated.This study aimed to compare pre- with postoperative depressive and anxiety symptoms based on success of surgery, defined as fulfilled expectations and improvement in disability and pain.A prospective 2-year longitudinal study in a tertiary care center was carried out.The sample consisted of 276 patients who underwent lumbar surgery.The Geriatric Depression Scale (GDS) and the Spielberger State-Trait Anxiety Inventory (STAI) were the outcome measures.Patients completed the following validated surveys several days before and again 2 years after surgery: the GDS with a set threshold for a positive screen for depression; the STAI with population norms used as threshold values; the Oswestry Disability Index (ODI); a numerical pain rating; and the Expectations Survey measuring amount of improvement expected. Dependent variables were pre- to postoperative within-patient change in GDS and STAI scores. Independent variables were three outcomes of surgery: proportion of expectations fulfilled, and changes in ODI scores and pain ratings. Analyses were conducted with GDS and STAI scores as continuous variables and according to threshold values, and for expectations, ODI and pain according to minimum clinically important differences (MCIDs).Mean age was 55, 56% were men, and 78% had degenerative diagnoses. For depressive symptoms, 41% screened positive preoperatively and 16% screened positive postoperatively; 72% had some improvement. In multivariable analysis adjusted for age, gender, comorbidity, diagnosis, and surgical invasiveness, depressive symptoms improved more for more expectations fulfilled (p.0001), more ODI improvement (p.0001), and more pain improvement (p=.001). For anxiety symptoms: 59% were worse than population norms preoperatively and 26% were worse postoperatively; 73% had some improvement. In adjusted multivariable analyses, anxiety symptoms improved more for more expectations fulfilled (p=.0002), more ODI improvement (p.0001), and more pain improvement (p=.03). Similar results were obtained according to threshold values and MCIDs.Substantial improvements in psychological well-being resulted after surgery among patients with favorable spine-specific outcomes.

STUDY DESIGN Multivariate analysis. OBJECTIVE The purpose of this study was to investigate risk factors for incidental durotomy (ID) in modern spine surgery techniques. SUMMARY OF BACKGROUND DATA ID, a relatively common complication of spine surgery, has been associated with postoperative complications such as durocutaneous fistulas, pseudomeningoceles, and arachnoiditis. Revision surgery may be necessary if the dural tear is not recognized and repaired during the initial procedure. METHODS ID was prospectively documented in patients who underwent spine surgery at a single institution during a 2-year period (n=4822). Patients with ID (n=182) from lumbar or thoracolumbar cases were matched 1:1 to a control cohort without ID. Demographic, diagnostic, and surgical procedure data were retrospectively collected and analyzed. Background context Lateral lumbar interbody fusion (LLIF) has become an increasingly common minimally invasive procedure for selective degenerative deformity correction, reduction of low-grade spondylolisthesis, and indirect foraminal decompression. Concerns remain about the safety of the transpsoas approach to the spine due to proximity of the lumbosacral plexus. Purpose To address risk factors for iatrogenic nerve injury in a large cohort of patients undergoing LLIF. Study design Retrospective analysis of 919 LLIF procedures to identify risk factors for lumbosacral plexus injuries. Methods The medical charts of patients who underwent transpsoas interbody fusion with or without supplemental posterior fusion for degenerative spinal conditions over a 6-year period were retrospectively reviewed. Patients with prior lumbar spine surgery or follow-up of less than 6 months were excluded. Factors that may affect the neurologic outcome were investigated in a subset of patients who underwent stand-alone LLIF. Results Four hundred fifty-one patients (males/females: 179/272) met the inclusion criteria and were followed for a mean of 15 months (range, 6–53 months). Average age at the time of surgery was 63 years (range, 24–90 years). Average body mass index was 29 kg/m2 (range, 17–65 kg/m2). A total of 919 levels were treated (mean, 2 levels per patient). Immediately after surgery, 38.5% of the patients reported anterior thigh/groin pain, whereas sensory and motor deficits were recorded in 38% and 23.9% of the patients, respectively. At the last follow-up, 4.8% of the patients reported anterior thigh/groin pain, whereas sensory and motor deficits were recorded in 24.1% and 17.3% of the patients, respectively. When patients with neural deficits present before surgery were excluded, persistent surgery-related sensory and motor deficits were identified in 9.3% and 3.2% of the patients, respectively. Among 87 patients with minimum follow-up of 18 months, persistent surgery-related sensory and motor deficits were recorded in 9.6% and 2.3% of the patients, respectively. Among patients with stand-alone LLIF, the level treated was identified as a risk factor for postoperative lumbosacral plexus injury. The use of recombinant human bone morphogenetic protein 2 was associated with persistent motor deficits. Conclusions Although LLIF is associated with an increased prevalence of anterior thigh/groin pain as well as motor and sensory deficits immediately after surgery, our results support that pain and neurologic deficits decrease over time. The level treated appears to be a risk factor for lumbosacral plexus injury.

STUDY DESIGN Retrospective cohort study. OBJECTIVE To assess the association between vertebral body Hounsfield unit (HU) measurements on quantitative computed tomography and the risk for incidental durotomy (ID). SUMMARY OF BACKGROUND DATA ID during spine surgery has been associated with adverse postoperative sequelae that may require prolonged hospital stay and reoperation. Previously identified independent risk factors include age, revision surgery, and lumbar surgery. METHODS ID was prospectively documented in spine surgery patients at a single institution during a 2-year period (incidence: 4%, 191/4,822). Patients with ID were matched 1:1 to a control cohort without ID based on age and sex. Inclusion criteria for both cohorts were primary lumbar surgery and quantitative computed tomographic scans within 1 year of date of surgery. Demographic, radiographical, and intraoperative data, along with dual x-ray absorptiometry t scores, were collected and analyzed. RESULTS A total of 71 patients with ID met the inclusion criteria (38 male, 33 female). Average age of patients with ID was 63.8 ± 12.9 years (range: 34-85 yr). Computed tomographic scans were acquired 1.5 ± 2.2 months away from date of surgery (range: 0-12 mo). Inter-rater reliability for HU measurements between a fellowship-trained spine surgeon and a research fellow was strong (r = 0.901, P < 0.001). HU values were significantly lower in patients with ID than controls (149.2 ± 60.7 [range: 44.5-301.5] versus 177.0 ± 81.4, [range: 62.0-524.0], respectively; P = 0.023). The area under the receiver operating characteristic curve was 0.603 (P = 0.034). A threshold of 169 HU optimized sensitivity (0.718) and specificity (0.451), and patients with HU value 169 or less were found to be at increased risk for ID (odds ratio: 2.092, 95% confidence interval: 1.042-4.201, P = 0.037). CONCLUSION Lower HU value is an accessible clinical marker for increased risk of ID. A threshold value of HU was defined (≤169) that may be used clinically to identify patients at elevated risk for ID, which may improve the informed decision making process prior to spinal surgery. Background context Traumatic fractures of the spine are most common at the thoracolumbar junction and can be a source of great disability. Purpose To review the most current information regarding the pathophysiology, injury pattern, treatment options, and outcomes. Study design Literature review. Methods Relevant articles, textbook chapters, and abstracts covering thoracolumbar spine fractures with and without neurologic deficit from 1960 to the present were reviewed. Results The thoracolumbar spine represents a unique system from a skeletal as well as neurological standpoint. The rigid rib-bearing thoracic spine articulates with the more mobile lumbar spine at the thoracolumbar junction (T10 - L2), the site of most fractures. A complete examination includes a careful neurologic examination of both motor and sensory systems. CT scans best describe bony detail while MRI is most efficient at describing soft tissues and neurological structures. The most recent classification system is that of the new Thoracolumbar Injury Classification and Severity Score. The different fracture types include compression fractures, burst fractures - both stable and unstable -, flexion-distraction injuries and fracture dislocations. Their treatment, both operative and non-operative depends on the degree of bony compromise, neurological involvement, and the integrity of the posterior ligamentous complex. Minimally invasive approaches to the care of thoracolumbar injuries have become more popular, thus, the evidence regarding their efficacy is presented. Finally, the treatment of osteoporotic fractures of the thoracolumbar spine is reviewed, including vertebroplasty and kyphoplasty, their risks and controversies, and senile burst fractures, as well. Conclusions Thoracolumbar spine fractures remain a significant source of potential morbidity. Advances in treatment have minimized the invasiveness of our surgery and in certain stable situations, eliminated it all together.

Study design Retrospective case series. Objective To report on the rare finding of motor deficits contralateral to the transpsoas approach in patients who underwent lateral lumbar interbody fusion (LLIF). Summary of background data Although sensorimotor deficits occurring ipsilaterally to a transpsoas approach have more fully been elucidated, there seems to be a paucity of data on motor deficits contralateral to an LLIF approach. Methods The electronic medical records and radiographical studies of 244 patients who underwent LLIF at a single institution between 2006 and 2009 were retrospectively reviewed for reports on motor deficits contralateral to the surgical approach. Results Of the patients reviewed, 2.9% (7/244) presented with a postoperative contralateral motor deficit, the most severe of which was a 1/5 weakness of the quadriceps muscle. An average of 3 levels (range: 2-4) was fused in 7 patients who developed a contralateral motor deficit, and in 3 of the 7 patients, an anterior lumbar interbody fusion (ALIF) was performed in addition to the LLIF. At 1 year follow-up, 3 patients presented with complete resolution of their muscle weakness, 1 patient still had mild weakness, 1 patient had decreased range of motion in the affected joint, and 1 patient had a 2/5 foot drop. One patient was lost to follow-up. Conclusion These data are among the largest reports of contralateral motor deficits after LLIF. Among possible underlying mechanisms are entrapment of the contralateral nerve root through translational correction of spondylolisthesis, front-to-back misalignment of the cage resulting in contralateral nerve root impingement, pressure on the contralateral peroneal nerve during positioning, and overdistraction neurapraxia when using ALIF at L5-S1 concomitantly. Awareness of the possibility of this rare complication can play an important role in surgical consideration and preoperative patient counseling. Level of evidence 4.

Study design A retrospective cohort study. Objective To present the technique, radiographic, and clinical outcomes of a series of patients who underwent a novel method for revision L5-S1 interbody fusion. Summary of background data Pseudoarthrosis at L5-S1 is a relatively common problem after long fusion to the sacrum. Revision approach to L5-S1 for cage or graft removal by anterior lumbar interbody fusion, posterior lumbar interbody fusion, or transforaminal interbody fusion is challenging and potentially dangerous which makes salvage by a posterior reamed transacral technique appealing. Methods Consecutive patients with symptomatic pseudoarthrosis at L5-S1, who underwent posterior reamed fluoroscopically guided fusion were identified over a 3-year period. Operative notes, medical records, preoperative and postoperative plain radiographs, computed tomography scans, Visual Analog Scores (VAS) (lower extremity and low back) preoperatively, and at most recent follow-up were studied. Results Ten patients (age, 53±2.8 y) with prior lumbar spinal operations (mean, 3.5±0.6) met the inclusion criteria. Prior procedures at L5-S1 were anterior lumbar interbody fusion (n=4), posterior lumbar interbody fusion (n=3), and posterolateral fusion (n=3). Mean Meyerding grade was 1.41 (range, 0-4). Reaming was performed between the S1 and S2 (n=9) or S2 and S3 (n=1) nerve roots and allowed fragmentation/removal of polyetheretherketone interbody grafts (n=3) or femoral ring allografts (n=3). Transacral Harms cage (n=8) or autograft (n=1) was passed through the reamed channel or a carbon fiber-reinforced polyetheretherketone directly into the interspace (n=1). VAS scores lower extremity (P=0.003) and low back (P=0.001) were improved at a mean follow-up of 13.5±3.6 months. No neurologic sequelae occurred and solid fusion was achieved in 9/10 (90%). Conclusions We report a series of patients who underwent a novel revision technique for symptomatic lumbosacral pseudoarthrosis. Despite the small numbers in this cohort, a salvage technique is presented that permitted fusion as confirmed by computed tomography scan and improved VAS scores in the majority of patients. The data suggest that this technique should be considered as an alternative to the revision anterior or posterior approaches to L5-S1 and merits further investigation.

Background context The lateral transpsoas approach to interbody fusion of the lumbar spine (lateral lumbar interbody fusion [LLIF]) with recombinant human bone morphogenetic protein-2 (BMP-2) augmentation has been increasingly performed in recent years. Potential side effects and adverse sequelae of BMP-2 in the acute setting remain to be fully elucidated. Purpose To review the literature for reports of complications related to BMP-2 implantation in lumbar spinal surgery and present a case of a contralateral psoas muscle seroma after LLIF with BMP-2 implantation. Study design Case report and literature review. Methods The PubMed database was searched for articles related to adverse events to BMP-2 in lumbar spinal surgery. We report the case of a 57-year-old woman who underwent routine right-sided transpsoas approach for LLIF with the use of BMP-2 at our institution and developed a left-sided psoas muscle fluid accumulation 2 weeks postoperatively. Results No reports of complications contralateral to an LLIF approach attributable to an inflammatory response to BMP-2 were identified in the English literature. In the presented patient, a large (4.2×6.5×2.7 cm) left-sided sterile intramuscular psoas fluid collection was seen on a magnetic resonance imaging study obtained on postoperative day 14. At a 6-month follow-up, left-sided L5 radiculopathy resulting in 4/5 foot drop was confirmed by electromyography. The patient reported here represents the only case of a contralateral psoas seroma with suspected association to BMP-2 utilization in LLIF encountered at our institution. Conclusions A serous psoas muscle fluid accumulation after BMP-2 implantation may rarely occur contralateral to the surgical approach for LLIF. Further characterization of complications related to BMP-2 implantation after lumbar spinal surgery will help guide preoperative informed decision making and the management of this unusual postoperative adverse event.

STUDY DESIGN Prospective retrieval analysis of Prodisc-C cervical total disc replacements (CTDRs) from 24 explanting surgeons during a 6-year period. OBJECTIVE To determine the in vivo mechanical performance and fixation to bone of explanted Prodisc-C CTDRs. SUMMARY OF BACKGROUND DATA The nature and quantity of damage sustained by an implanted device has proven to be important in the prediction of clinical longevity. We hypothesized that retrieval analysis of the Prodisc-C will display characteristic modes of wear consistent with increased posterior angulation and translation of the functional spinal unit after resection of the discoligamentous anatomy. METHODS Thirty CTDRs from 29 patients (mean age, 45.1 ± 1.9; range, 31-57 yr) after a mean length of implantation of 1.0 ± 0.2 years (range, 2 d-3.5 yr) were studied. Operative level was C4-C5 in 20% (6 of 30), C5-C6 in 47% (14 of 30), C6-C7 in 20% (6 of 30), and unknown in 13% (4 of 30). Polyethylene and metallic (cobalt chrome molybdenum [CoCrMo]) components were examined using light stereo-microscopy (6X-31X), scanning electron microscopy, and energy dispersive x-ray analysis. RESULTS CTDRs were explanted for indications of axial pain (n = 9), radicular symptoms (n = 6), atraumatic loosening (n = 6), trauma (n = 5), metal allergy (n = 1), myelopathy (n = 1), hypermobility (n = 1), and unknown (n = 1). Surface area of ongrowth (mean = 7.2 ± 1.4%) was not associated with operative level (P = 0.37), surgeon-reported axial pain (P = 0.56), or atraumatic loosening (P = 0.93). Burnishing consistent with metallic endplate impingement was present in 80% (24 of 30) of retrieved CTDRs, most commonly in the posterior quadrant (P < 0.001). There was no association between implant height (P = 0.19) or depth (P = 0.17) and posterior impingement. Backside wear was not observed on any of the disassembled implants (0 of 16). Third-body wear occurred in 23% (7 of 30) and the donor site was confirmed by scanning electron microscope/energy dispersive x-ray analysis to be the porous-coated surface of the CTDR. CONCLUSION Early clinical failures of Prodisc-C CTDRs display surface damage evidence of metal endplate-endplate impingement, most commonly posteriorly. Backside wear was not evident; however, third-body wear was found. Future studies will determine the clinical impact of these predominant modes of wear on long-term metal-on-polyethylene semiconstrained CTDR performance.

STUDY DESIGN A retrieval analysis of wear modes and fixation of lumbar total disc replacements (TDRs). Explanted Prodisc-L TDRs were prospectively collected during a 7-year period (2005-2011) and analyzed. OBJECTIVE To assess the in vivo modes of wear and fixation of lumbar TDR with the Prodisc-L device. SUMMARY OF BACKGROUND DATA Inferior clinical outcomes and failure of lumbar TDR may occur because of suboptimal component fixation, wear properties, and impingement in a subset of patients. Posterior component TDR impingement has been demonstrated radiographically; however, despite its widespread use, the in vivo mechanical performance and fixation of the Prodisc-L device remain unknown. METHODS Explanted polyethylene and metallic (CoCrMo) components of Prodisc-L devices were examined by light stereo-microscopy (6X-31X), scanning electron microscopy, and energy-dispersive x-ray analysis from an international retrieval registry, with 13 participating surgeons. RESULTS Nineteen ProDisc-L devices from 18 patients (age, 44.7 ± 2.9 yr) following an index TDR at L4-L5 (n = 6), L5-S1 (n = 11), and unknown level (n = 2) were explanted for pain (n = 8), prosthesis subluxation/migration (n = 4), end plate collapse/subsidence (n = 3), polyethylene dislodgement (n = 3), and unknown (n = 2) after a mean length of implantation of 13.0 ± 3.9 months. Surface area of bony ongrowth was 9.6 ± 2.9% (range, 0%-52.5%). TDR burnishing was observed posteriorly consistent with component impingement in extension in 53% (8/15) (P < 0.02), more commonly than anterior 20% (3/15) lateral 20% (n = 3) (3/15) patterns. Circumferential burnishing was not observed. Posterior impingement was associated with 6° lordotic implants (P < 0.05) and 10-mm polyethylene size (P < 0.05). Backside wear occurred in 75% (9 of 12) of the disassembled implants and third-body wear was observed in 33% (5 of 15). CONCLUSION Metallic end plate burnishing was evident in a large percentage of clinically failed Prodisc-L TDR devices, most commonly posteriorly, consistent with impingement in extension. Long-term follow-up studies will evaluate the effects of the observed backside wear, third-body wear, and end plate impingement on clinical outcomes.

STUDY DESIGN Prospective lumbar and cervical surgery cohorts. OBJECTIVE Compare fulfillment of expectations with traditional global outcomes and provide evidence for its validity. SUMMARY OF BACKGROUND DATA New lumbar and cervical spine surgery Expectations Surveys can be used to measure fulfillment of expectations and their performance should be compared with existing outcomes. METHODS Three hundred thirty-six lumbar and 133 cervical spine surgery patients preoperatively completed valid 20-item Expectations Surveys measuring symptoms, function, and psychological well-being. Approximately 2 years postoperatively patients rated how much improvement they received for each item. The proportion of expectations fulfilled was compared with traditional outcomes, including global satisfaction and change in standard spine questionnaires, with correlation coefficients (r) and areas under receiver operator characteristic curves (AUC). RESULTS Ninety percent of lumbar patients had some expectations fulfilled (24% had all expectations fulfilled completely or exceeded). Post-operative ileus is a recognized complication of surgery. Little is known about the incidence and risk factors for post-operative ileus following spinal fusion surgery. To report the incidence and to assess for independent risk factors of post-operative ileus after spinal fusion surgery. Retrospective single-centre cohort study. Patients with prolonged or recurrent post-operative ileus were identified by review of hospital stay documentation. Patients with post-operative ileus were matched 1:2 to a control cohort without post-operative ileus. Uni and multi variate analyses were performed on demographic, comorbidity, surgical indication, medication, and peri-operative details to identify risk factors for post-operative ileus. Two thousand six hundred and twenty five patients underwent spinal fusion surgery between January 2012 and December 2012. Forty nine patients with post-operative ileus were identified (1.9 %). Post-operative length of hospital stay was significantly longer for patients with post-operative ileus (9.3 ± 5.2 days), than control patients (5.5 ± 3.2 days) (p

Spondylotic degeneration in the cervical spine may result in static and/or dynamic spinal cord compression that can lead to the associated signs and symptoms of myelopathy. Clinical examination combined with appropriate imaging studies help to confirm the diagnosis. Classic natural history and basic science studies suggest a pernicious course of demyelination and neurologic decline in a large subset of patients. The characterization of disease severity and progression in patients with cervical spondylotic myelopathy has improved in recent years with imaging and data from prospective and multicenter studies. Additionally, advances in surgical techniques, implants, and imaging modalities have improved the identification of surgical candidates with cervical spondylotic myelopathy and associated treatment strategies. Surgical treatment, via an anterior, posterior, or a combined approach, is primarily intended to arrest neurologic progression, although it can improve function in many patients. Alignment and the characteristics and location of spinal cord compression help determine the ideal surgical approach. Distinct complications associated with each technique may be mitigated by appropriate patient selection and should be discussed preoperatively to ensure informed decision making.

OBJECT The unique and complex biomechanics of the atlantoaxial junction make the treatment of C1–2 instability a challenge. Several screw-based constructs have been developed for atlantoaxial fixation. The biomechanical properties of these constructs have been assessed in numerous cadaver studies. The purpose of this study was to systematically review the literature on the biomechanical stability achieved using various C1–2 screw constructs and to perform a meta-analysis of the available data. METHODS A systematic search of PubMed through July 1, 2013, was conducted using the following key words and Boolean operators: “atlanto [all fields]” AND “axial [all fields]” OR “C1–C2” AND “biomechanic.” Cadaveric studies on atlantoaxial fixation using screw constructs were included. Data were collected on instability models, fixation techniques, and range of motion (ROM). Forest plots were constructed to summarize the data and compare the biomechanical stability achieved. RESULTS Fifteen articles met the inclusion criteria. An average (± SD) of 7.4 ± 1.8 cadaveric specimens were used in each study (range 5–12). The most common injury models were odontoidectomy (53.3%) and cervical ligament transection (26.7%). The most common spinal motion segments potted for motion analysis were occiput–C4 (46.7%) and occiput–C3 (33.3%). Four screw constructs (C1 lateral mass–C2 pedicle screw [C1LM–C2PS], C1–2 transarticular screw [C1–C2TA], C1 lateral mass–C2 translaminar screw [C1LM-C2TL], and C1 lateral mass–C2 pars screw [C1LM–C2 pars]) were assessed for biomechanical stability in axial rotation, flexion/extension, and lateral bending, for a total of 12 analyses. The C1LM–C2TL construct did not achieve significant lateral bending stabilization (p = 0.70). All the other analyses showed significant stabilization (p < 0.001 for each analysis). Significant heterogeneity was found among the reported stabilities achieved in the analyses (p < 0.001; I2 > 80% for all significant analyses). The C1LM–C2 pars construct achieved significantly less axial rotation stability (average ROM 36.27° [95% CI 34.22°–38.33°]) than the 3 other constructs (p < 0.001; C1LM–C2PS average ROM 49.26° [95% CI 47.66°–50.87°], C1–C2TA average ROM 47.63° [95% CI 45.22°–50.04°], and C1LM–C2TL average ROM 53.26° [95% CI 49.91°–56.61°]) and significantly more flexion/extension stability (average ROM 13.45° [95% CI 10.53°–16.37°]) than the 3 other constructs (p < 0.001; C1LM–C2PS average ROM 9.02° [95% CI 8.25°–9.80°], C1–C2TA average ROM 7.39° [95% CI 5.60°–9.17°], and C1LM–C2TL average ROM 7.81° [95% CI 6.93°–8.69°]). The C1–C2TA (average ROM 5.49° [95% CI 3.89°–7.09°]) and C1LM–C2 pars (average ROM 4.21° [95% CI 2.19°–6.24°]) constructs achieved significantly more lateral bending stability than the other constructs (p < 0.001; C1LM–C2PS average ROM 1.51° [95% CI 1.23°–1.78°]; C1LM–C2TL average ROM −0.07° [95% CI −0.44° to 0.29°]). CONCLUSIONS Meta-analysis of the existing literature showed that all constructs provided significant stabilization in all axes of rotation, except for the C1LM–C2TL construct in lateral bending. There were significant differences in stabilization achieved in each axis of motion by the various screw constructs. These results underline the various strengths and weaknesses in biomechanical stabilization of different screw constructs. There was significant heterogeneity in the data reported across the studies. Standardized spinal motion segment configuration and injury models may provide more consistent and reliable results.

Because the spine and pelvis are integrated, changes in spine sagittal balance affect relative acetabulum position. A 1° change of the anterior pelvic plane changes acetabulum anteversion by 0.8°. Three-column spine osteotomies correct fixed sagittal plane deformity. Twenty patients with kyphotic deformity and associated sagittal imbalance underwent corrective 3-column osteotomy. We reviewed upright pelvic and spine radiographs preoperatively and postoperatively and documented the changes in angles. The average sagittal vertical axis was 11.07 cm preoperatively and 4.8 cm postoperatively. Lumbar lordosis changed (on average) from 39° preoperatively to 55° postoperatively (P.05). Sacral slope increased an average of 6.7° (P = .015). Pelvic tilt decreased by 5.4° (P = .001). The anterior pelvic plane increased by 8.23° (P.0001). This correction of the sagittal balance is associated with a concomitant increase in sacral slope, pelvic tilt, and the anterior pelvic plane angles. These changes will increase acetabulum anteversion by a predicted 6.54°. This increase will change acetabular cup position and must be considered in patients with spine and pelvic osteoarthritis that requires hip surgery.

Study design Retrospective case series. Objective To examine the reoperation rate, specifically the need for posterior decompression and/or fusion, in a cohort of patients who underwent stand-alone lateral lumbar interbody fusion for symptomatic spinal stenosis with instability or deformity. Summary of background data Lateral lumbar interbody fusion provides a minimally invasive means of achieving interbody arthrodesis and indirect foraminal decompression while avoiding the potential morbidity of traditional anterior or posterior approaches. The revision rate for formal posterior decompression after isolated lateral lumbar interbody fusion for spinal stenosis is unknown. Methods One hundred seventeen patients who underwent stand-alone lateral lumbar interbody fusion for symptomatic spinal stenosis with an indication for fusion were included in the analysis. Detailed demographic and intraoperative data were collected. Clinical evaluation was done both preoperatively and at the final follow-up, and radiographical evaluation was done preoperatively and with the first postoperative standing radiographs. Results A total of 10.3% of patients who underwent stand-alone lateral lumbar interbody fusion ultimately required revision surgery, most commonly for persistent radiculopathy and symptomatic implant subsidence. Average time to revision was 10.8 months. There was no difference in radiographical correction between patients who did and did not require revision surgery. Conclusion Lateral lumbar interbody fusion provides a minimally invasive means to treat lumbar spinal stenosis with an acceptable revision rate for formal posterior decompression at early follow-up. Study design Retrospective case series. Objective To evaluate the proportional trend over time of neurological deficits after lateral lumbar interbody fusion (LLIF) at a single institution. Summary of background data Because lumbar nerve roots converge to run as the lumbar plexus within or less frequently underneath the posterior part of the psoas muscle, they are prone to iatrogenic damage during the transpsoas approach in LLIF, and adverse postoperative neurological sequelae remain a major concern. Methods The electronic medical records and office notes of 451 patients who had consecutively undergone LLIF between March 2006 and April 2012 at a single institution were retrospectively reviewed for reports on postoperative neurological deficits. Results A total of 293 patients (173 females and 120 males) met the study inclusion criteria and were followed postoperatively for a mean period of 15.4 ± 9.2 months (range: 6-53 mo). The number of included patients who underwent LLIF at our institution was 47 in the years 2006 to 2008 (group A), 155 in 2009 to 2010 (group B), and 91 in 2011 to 2012 (group C). Our data indicate a decreasing proportional trend during the past 6 years for postoperative sensory deficits (SDs), motor deficits (MDs), and anterior thigh pain (TP). The decreasing trends were statistically significant for the proportion of SDs in the immediate postoperative setting (P = 0.018) and close to statistically significant for SDs at last follow-up (P = 0.126), TP immediately after surgery (P = 0.098), and TP at last follow-up (P = 0.136). Conclusion To the authors' best knowledge, this study constitutes the largest series of this sort to date, with regard to both sample size and study period. The present data indicate a decreasing proportional trend over time for SDs, MDs, and anterior TP, which can be considered a representation of an institutional learning curve during a 6-year time period of performing LLIF.

STUDY DESIGN Retrospective analysis of 73 standalone lateral lumbar interbody fusion (LLIF) procedures to identify any association between the amount of coronal curve correction and lumbosacral plexus injuries and/or postoperative pain. OBJECTIVE To address if there is any association between the amount of correction in both the coronal and sagittal planes and the development of postoperative neurological deficit and/or anterior thigh/groin pain. SUMMARY OF BACKGROUND DATA LLIF is a powerful tool for the restoration of spinal alignment including correction of small degenerative curves of the lumbar spine and increase of lumbar lordosis. Concerns remain about its safety regarding injuries of the lumbosacral plexus, which occur with a prevalence ranging from 0.7% to 23%. METHODS The medical records and spinal radiographs of patients undergoing standalone LLIF for symptomatic degenerative scoliosis of the lumbar spine were retrospectively reviewed during a 6-year period. RESULTS Thirty patients (73 levels) met the inclusion criteria and were followed for a mean of 21 months (range, 9-39 mo). Average age at the time of surgery was 67 years (range, 50-78 yr). Immediately after surgery, a motor deficit was recorded in 6 patients and a sensory deficit in 17 patients. Statistical analysis did not reveal any significant association between the amount of coronal curve correction, restoration of lumbar lordosis or increase in lumbar spine height, and the development of postoperative motor or sensory deficits. Seventeen patients complained of anterior thigh/groin pain immediately postoperative. A statistically significant association was identified between postoperative anterior thigh/groin pain and the magnitude of curve correction (P = 0.005), as well as the increase in lumbar lordosis (P = 0.040). CONCLUSION There is a strong association between the development of postoperative anterior thigh/groin pain and the amount of coronal curve correction, as well as the increase in lumbar lordosis. To report three cases of transient perioperative neurological deficit in the absence of direct cord insult following decompression of the severely stenotic thoracic spine.The clinical and radiographic electronic medical records of three patients who underwent decompression for severe midthoracic stenosis with transient neurological deficits perioperatively were reviewed. The cases are presented with consideration of possible underlying mechanisms and multimodality intraoperative monitoring (IOM) findings.Two patients had neurologic changes on IOM and Stagnara wake-up test, the remaining patient had absent motor and sensory potentials at baseline and throughout the case. IOM changes were observed immediately following decompression in the absence of direct cord insult or displacement. Postoperatively all patients experienced neurological motor deficits which presented as complete paralysis of the right lower extremity in two of the patients and the left lower extremity in one patient. The deficit was transient-improvement of motor strength occurred between 1 and 13 months of follow-up in all patients.Decompression of a severely stenotic region of the thoracic spinal cord may lead to a complete yet transient motor deficit in the perioperative period in the absence of direct mechanical cord insult. Potential etiologies include ischemia-reperfusion injury, microthrombi, and altered perfusion due to internal recoil of spinal cord architecture following decompression. IOM may show conspicuous findings in such events, however, may not be relied upon when baseline potentials are sub-optimal. Recognition of this short-lived neurological deficit following decompression of the severely stenotic thoracic spine will improve preoperative patient counseling and merits further study for determination of the precise pathophysiology.

STUDY DESIGN Retrospective analysis of prospective registry data. OBJECTIVE To determine the patient characteristics, risk factors, and fracture patterns associated with vertebral artery injury (VAI) in patients with blunt cervical spine injury. SUMMARY OF BACKGROUND DATA VAI associated with cervical spine trauma has the potential for catastrophical clinical sequelae. The patterns of cervical spine injury and patient characteristics associated with VAI remain to be determined. METHODS A retrospective review of prospectively collected data from the American College of Surgeons trauma registries at 3 level-1 trauma centers identified all patients with a cervical spine injury on multidetector computed tomographic scan during a 3-year period (January 1, 2007, to January 1, 2010). Fracture pattern and patient characteristics were recorded. Logistic multivariate regression analysis of independent predictors for VAI and subgroup analysis of neurological events related to VAI was performed. RESULTS Twenty-one percent of 1204 patients with cervical injuries (n = 253) underwent screening for VAI by multidetector computed tomography angiogram. VAI was diagnosed in 17% (42 of 253), unilateral in 15% (38 of 253), and bilateral in 1.6% (4 of 253) and was associated with a lower Glasgow coma scale (P < 0.001), a higher injury severity score (P < 0.01), and a higher mortality (P < 0.001). VAI was associated with ankylosing spondylitis/diffuse idiopathic skeletal hyperosteosis (crude odds ratio [OR] = 8.04; 95% confidence interval [CI], 1.30-49.68; P = 0.034), and occipitocervical dissociation (P < 0.001) by univariate analysis and fracture displacement into the transverse foramen 1 mm or more (adjusted OR = 3.29; 95% CI, 1.15-9.41; P = 0.026), and basilar skull fracture (adjusted OR = 4.25; 95% CI, 1.25-14.47; P= 0.021), by multivariate regression model. Subgroup analyses of neurological events secondary to VAI occurred in 14% (6 of 42) and the stroke-related mortality rate was 4.8% (2 of 42). Neurological events were associated with male sex (P = 0.024), facet subluxation/dislocation (crude OR = 9.00; 95% CI, 1.51-53.74; P = 0.004) and the diagnosis of ankylosing spondylitis/diffuse idiopathic skeletal hyperosteosis (OR = 40.67; 95% CI, 5.27-313.96; P < 0.001). CONCLUSION VAI associated with blunt cervical spine injury is a marker for more severely injured patients. High-risk patients with basilar skull fractures, occipitocervical dissociation, fracture displacement into the transverse foramen more than 1 mm, ankylosing spondylitis/diffuse idiopathic skeletal hyperosteosis, and facet subluxation/dislocation deserve focused consideration for VAI screening.

Background context Despite common use of intraoperative electrophysiologic neuromonitoring, injuries to the lumbar plexus during lateral lumbar interbody fusion (LLIF) have been reported. Emerging data suggest that recombinant human bone morphogenetic protein-2 (rhBMP-2) use during an anterior or transforaminal lumbar interbody fusion may be associated with an increased risk of neurological deficit. Clinical data on the sequelae of rhBMP-2 implantation in close proximity to the lumbosacral plexus during LLIF remains to be understood. Purpose The purpose of this study was to compare the incidence of neurologic deficits and pain in patients undergoing LLIF with and without rhBMP-2. Study design/setting Retrospective outcome analysis in controlled cohorts undergoing the lateral exposure technique for LLIF with and without rhBMP-2. Methods The electronic medical records of patients undergoing LLIF with and without supplemental posterior fusion for degenerative spinal conditions were retrospectively reviewed over a 6-year period. Patients with previous lumbar spine surgery or follow-up of less than 6 months were excluded. Patients were divided into 2 groups, Group 1 (rhBMP-2 use; n=72) and Group 2 (autograft/allograft use; n=72), and were matched according to the age at the time of surgery, gender, weight, body mass index, side of approach, total number of treated spinal segments, use of supplemental posterior fusion, and length of follow-up. Results Immediately after surgery, a sensory deficit was recorded in 33 patients in Group 1 and 35 patients in Group 2 (odds ratio [OR] 0.895; 90% confidence interval [CI] 0.516–1.550; p=.739). At last follow-up, a persistent sensory deficit was identified in 29 patients whose LLIF procedure was supplemented by rhBMP-2 and 20 patients in whom autograft/allograft was used (OR 1.754; 90% CI 0.976–3.151; p=.115). A motor deficit was recorded in 37 patients immediately after the rhBMP-2 procedure and 28 patients treated with autograft/allograft (OR 1.661; 90% CI 0.953–2.895; p=.133). A persistent motor deficit was recorded in 35 and 17 patients in Groups 1 and 2, respectively, at last follow-up (OR 3.060; 90% CI 1.681–5.571; p=.002). During the first postoperative examination, 37 patients in Group 1 and 25 patients in Group 2 complained of anterior thigh or groin pain (OR 1.987; 90% CI 1.133–3.488; p=.045). At last follow-up, there was a significantly higher number of patients in Group 1 who complained of persistent anterior thigh or groin pain than Group 2 (8 vs. 0 patients) (OR 16.470; 90% CI 1.477–183.700; p=.006). Conclusions Our results provide evidence of an increased rate of postoperative neurologic deficit and anterior thigh/groin pain after LLIF using rhBMP-2, when compared with matched controls without rhBMP-2 exposure. This study suggests a potential direct deleterious effect of rhBMP-2 on the lumbosacral plexus.

Background context Follow-up studies of patients undergoing anterior cervical discectomy and interbody fusion (ACDF) have demonstrated varying degrees of radiographic degeneration at adjacent levels, with most cases being asymptomatic (adjacent segment degeneration, ASDeg) and far fewer being symptomatic (adjacent segment disease, ASDz). Controversy remains as to whether these conditions are related to altered biomechanics or represent the natural history of cervical spondylosis at the adjacent segment. Purpose To provide an evidence-based analysis of the peer-reviewed literature on clinical studies of ASDeg and ASDz after ACDF. Study design/setting Systematic review of existing literature. Methods The MEDLINE database was queried for clinical studies reporting ASDeg and/or ASDz after ACDF. Articles written in the English language with a minimum follow-up of 2 years were independently reviewed and analyzed by two authors, and the level of evidence was assigned. Data were pooled to generate summary outcomes and organized by number of levels, technique, and graft/implants. Results Of the 238 articles returned from the MEDLINE database query, 14 met inclusion criteria. An average of 168 patients was enrolled per study with an average follow-up of 106.5 months. Graft materials, cage design, plate fixation system, and length of fusion varied widely. Additionally, no clear standard was seen for radiographic assessment modalities (eg, plain lateral radiograph, flexion-extension radiographs, computed tomography, or magnetic resonance imaging). Validated clinical outcome measures were used in 43% (6/14) of the studies. The average incidence of ASDeg was 47.33% (459.14/970) with a range from 16% to 96%. The frequency-weighted average for ASDz was 11.99% (263.70/2,199) with a range from 1.80% to 36.00%. Follow-up ranged from 24 to 296 months with no reliable commonalities, which prohibited a meta-analysis. Conclusions This review highlights the heterogeneous methodology of the peer-reviewed literature on ASDeg and ASDz after ACDF and the paucity of high-level clinical data published on these conditions. Despite the low level of evidence to define the incidence of ASDeg and ASDz, it is clear that radiographic ASDeg is more common than symptomatic ASDz, indicating that adjacent segment pathology remains subclinical in a large subset of patients. This analysis underscores the need for standardized radiographic measures in the assessment of ASDeg and validated clinical outcome measures for ASDz after ACDF. Consistent methodology and multi-surgeon collaboration may improve the quality of clinical data on ASDeg and ASDz and elucidate the true etiology and incidence of these conditions.

Background context Increased fusion rates have been reported with the addition of an anterior cervical plate (ACP) to anterior cervical discectomy and fusion (ACDF). Bioabsorbable implants have become increasingly used in orthopedic and spine surgical procedures. There are limited data regarding the outcomes of bioabsorbable ACP (bACP) with ACDF. Purpose To compare the clinical and radiographic outcomes of patients undergoing ACDF for single-level degenerative disorders with a bACP versus a conventional metal ACP (mACP). Study design Retrospective comparative cohort study. Patient sample Thirty-one patients undergoing ACDF for a single-level degenerative disorder (ie, disc herniation or spondylotic neural compression). Outcome measures Incidence of early (within 2 weeks) complications, postoperative sagittal alignment, Odom's criteria, and pseudarthrosis rate. Methods The authors retrospectively reviewed the results of a consecutive series of patients undergoing ACDF for symptomatic single-level disc herniation or spondylotic neural compression with either a bACP or an mACP over a 3-year period. Operative notes, clinical charts, and radiographs were analyzed. Radiographic outcomes were assessed for intersegmental alignment, graft subsidence, fusion rate, prevertebral soft-tissue shadow, and graft containment. Clinical outcome was evaluated by Odom's criteria. Results Fourteen patients underwent ACDF with a bACP and 15 with an mACP. Radiographic outcomes at the most recent follow-up demonstrated pseudarthrosis in 4 of 14 patients (29%) in the bACP group and 0 of 15 patients in the mACP group. Graft extrusion and anterior displacement was present in three of four pseudarthroses (75%). Comparing preoperative and final radiographs, cervical lordosis was maintained at the operative segment in only 3 of 14 bACP patients (21%) compared with 8 of 15 patients (53%) in the mACP group. The mean Cobb angle was 2.4°±1.9° lordosis in the mACP group and −2.7°±2.5° kyphosis in the bACP group (p=.12). In the mACP group, 14 of 15 patients had good or excellent results. In the bACP group, only 7 of 14 patients had good or excellent results. Conclusions Bioabsorbable ACP fixation was associated with a high rate of graft extrusion and early loss of intersegmental cervical alignment. Inferior clinical outcomes were observed in patients in the bACP group compared with the mACP group. Based on these findings, continued use of the bACP used in this study cannot be recommended.

Background— Cell transplantation for myocardial repair is limited by early cell death. Gene therapy with human Bcl-2 (hBcl-2) has been shown to attenuate apoptosis in the experimental setting. Therefore, we studied the potential benefit of hBcl-2 transgene expression on the survival of cardiomyoblast grafts in ischemic rat hearts. Methods and Results— H9c2 rat cardiomyoblasts were genetically modified to express both firefly luciferase and green fluorescent protein (mH9c2). The cells were then transduced with adenovirus carrying hBcl-2 (AdCMVhBcl-2/mH9c2). Lewis rats underwent ligation of the left anterior descending artery (LAD) to induce a sizable left ventricular (LV) infarct. Hearts were explanted and the infarcted region was restored using collagen matrix (CM) seeded with 1×10 6 mH9c2 cells (n=9) or AdCMVhBcl-2/mH9c2 cells (n=9). Control animals received CM alone (n=6) or no infarct (n=6). Restored hearts were transplanted into the abdomen of syngeneic recipients in a “working heart” model. Cell survival was evaluated using optical bioluminescence imaging on days 1, 5, 8, 14, and 28 after surgery. The left heart function was assessed 4 weeks postoperatively using echocardiography and magnetic resonance imaging. During 4 weeks after surgery, the optical imaging signal for the AdCMVhBCL2/mH9c2 group was significantly ( P P =0.04) and ejection fraction (AdCMVhBcl-2/mH9c2: 47.0±6.2; mH9c2: 48.7±6.1; control: 34.3±6.0; P =0.02) compared with controls. Importantly, no malignant cells were found in postmortem histology. Conclusion— Transduction of mH9c2 cardiomyoblasts with AdCMVhBcl-2 increased graft survival in ischemic rat myocardium without causing malignancies. Both AdCMVhBcl-2/mH9c2 and mH9c2 grafts improved LV function.

Objective: A major limitation of stem cell transfer is early donor-cell death. Here, we seek to enhance myocardial repair following injury through transplantation of cardiomyocyte-enriched human embryonic stem cells (hESC) and recipient treatment with cytoprotective (allopurinol + uricase) and anti-inflammatory (ibuprofen) agents. Methods: We injected 106 (15% hESC-derived cardiomyocytes) green fluorescent protein (GFP+) hESC in the infarcted area following left anterior descending artery (LAD)-ligation in SCID-beige mice. In Group I, 1.6 mg allopurinol and 0.2 mg of uricase were injected i.p. for 3 days prior to cell transplantation. In Group II, 0.35 mg/ml of ibuprofen were added to the drinking water before and after cell implantation. In Group III, the LAD was ligated and allopurinol/uricase was administered without cell treatment. In Group IV, ibuprofen was added to the drinking water and the LAD was ligated without additional cell treatment. In Group V, only cells were transplanted. Group VI involved infarcted controls and Group VII involved sham-operated mice (all groups: n = 5). We evaluated heart function (ejection fraction (EF)) by MRI (4.7 T) 3 weeks later. The hearts were harvested for histology. Results: Differentiated hESC formed clusters and expressed α-sarcomeric actin and Connexin 43. Cell treatment improved heart function, which was best in the ibuprofen- and allopurinol-treated groups (+cell transfer), compared to the infarcted controls [EF: Group I: 76.6 ± 8.6%, Group II: 78.6 ± 7.3%, Group III: 58.1 ± 5.7%, Group IV: 53.9 ± 5.2%, Group V: 57.7 ± 7.5%, Group VI: 43.5 ± 4.3%, and Group VII: 66.3 ± 7.8%]. We did not observe tumors in any group. Conclusions: Allopurinol/uricase and ibuprofen enhance differentiated hESC-engraftment and myocardial restoration following transplantation into the injured heart. © 2005 Elsevier B.V. Traumatic thoracic aortic injury (TAI) is traditionally treated with immediate surgery. Previously published studies have established the safety and efficacy of treating TAI with endovascular stents. Our hypothesis was that stents are supplanting operative repair as the primary therapy for TAI.Retrospective cohort.University level I trauma center.Blunt trauma patients admitted to a level I trauma center diagnosed with TAI between September 1997 and November 2003 were identified from an institutional trauma registry (N = 25). Data were abstracted from medical records and analyzed. Three groups were defined: surgical repair (cardiopulmonary bypass or clamp and sew) (n = 10); medical management (n = 8); and endovascular stent (n = 7).Prior to 2002, 9 (75%) of 12 patients were treated by surgical repair, 2 (17%) by medical management, and 1 (8%) by endovascular stent. Since 2002, 1 patient (8%) was treated by surgical repair, 6 (46%) by medical management, and 6 (46%) by endovascular stent. Injury Severity Scores were comparable between the surgical cohort (mean +/- SEM score, 34.9 +/- 3.4), stent placement (35.1 +/- 3.7), and medical management (29.9 +/- 2.8) (P = .48). Overall survival was 80% with no differences in morbidity or mortality. The stented group had shorter hospital lengths of stay compared with surgical management (28 vs 46 days) (P.05). The 1 operative case since 2002 was a combined arch/innominate injury that anatomically precluded stent placement.Initial reports suggested thoracic aortic stents as an alternative for injured patients with prohibitive operative risks. Our data suggest stent placement is quickly evolving into the primary therapy for TAI across all Injury Severity Score profiles.

Objective The optimal surgical procedure for the neck in patients with squamous head and neck cancers is controversial. Selective neck dissections have replaced modified radical neck dissections as the procedure of choice for the clinically negative (N0) neck and are now being considered for patients with early-stage neck disease. We report the long-term local recurrence rates in 100 consecutive patients undergoing a radical or modified radical neck dissection for clinically positive (N+) and N0 neck disease and review comprehensively the literature reporting and comparing regional control rates for both neck dissection types. Patients The clinical records of 100 consecutive patients who underwent a comprehensive neck dissection (levels I-V) for squamous head and neck cancers with a minimum of a 2-year follow-up were retrospectively reviewed for primary site of disease, clinical and pathologic neck status, histopathologic grade, neck dissection type, and the site and time of recurrence. Results Complete data were available for 97 patients on whom 99 neck dissections were performed. Three patients died from unknown causes. Seventy-six patients with N+ disease underwent a therapeutic neck dissection, while 24 patients with clinically N0 disease underwent an elective dissection. The overall neck recurrence rate in patients with controlled primary disease was 7%. The neck or regional failure rate for patients completing the recommended adjuvant radiotherapy was 4%. Six (25%) of 24 patients with clinically N0 disease had occult metastases. The recurrence rate for this group was 4%. Conclusion Further study is needed to determine the optimal surgical management of the N0 and limited N+ neck.
