FAST is essential to decide whether trauma patients need laparotomy, but it has a notable decrease in accuracy in patients with pelvic fracture. Our objective is to analyze the consequences of therapeutic decision-making based on the FAST results in trauma patients with pelvic fracture.Descriptive study that includes trauma patients older than 16 with a pelvic fracture admitted to the critical care area or who died. The FAST result was compared with a true positive or negative value according to the results of laparotomy or abdominal CT. We recorded diagnosis and treatment of each injury and resolution of the case, detailing the cause of death, among all variables.Over the 13-year period, we included 263 trauma patients with pelvic fracture, with a mean ISS of 31 and mortality of 19%. FAST had a sensitivity of 65.2%, specificity of 69%, false negative rate of 34.8% and false positive rate of 30.9%. Hemodynamically unstable patients died twice as many stable patients (27% vs 14%, P .05). Patients with positive FAST died more than negative FAST (43% vs 26%); and 4 out of 10 hemodynamically unstable patients who underwent non-therapeutic laparotomy after presenting a false positive FAST died from hypovolemic shock. The mortality rate fell from 60% to 20% when preperitoneal packing was performed before angio-embolization of the pelvis.FAST has low accuracy in polytraumatized patients with pelvic fracture. Patients with false positive FAST have higher mortality, which can be reduced notably by applying preperitoneal packing.

Introduction Vital signs indicate the presence of bleeding only after large amounts of blood have been lost, with high morbidity and mortality. The Shock Index (SI) is a hemorrhage indicator with a cut-off point for the risk of bleeding at 0.9. The aim of this study is to assess whether a cut-off of ≥0.8 is more sensitive for detecting occult bleeding, providing for early initiation of therapeutic maneuvers. Methods SI analytical validation study of severe trauma patients older than 16 years of age. Vital signs were recorded, and scales for predicting bleeding included: SI, Assessment of Blood Consumption score, and Pulse Rate Over Pressure score. The relationship between the SI and 5 markers for bleeding was analyzed: need for massive transfusion, angiographic embolization, surgical bleeding control, death due to hypovolemic shock, and the overall predictor “active bleeding” (defined as the presence of at least one of the 4 markers above). Results Data from 1402 trauma patients were collected prospectively over a period of 10 years. The mean Injury Severity Score was 20.9 (SD 15.8). The mortality rate was 10%. The mean SI was 0.73 (SD 0.29). “Active bleeding” was present in 18.7% of patients. The SI area under the ROC curve for “active bleeding” was 0.749. Conclusions An SI cut-off point ≥0.8 is more sensitive than ≥0.9 and allows for earlier initiation of resuscitation maneuvers in patients with occult active bleeding.

The terms 'development' and 'evolution' are both used to describe the unfolding of the carcinogenic process. However, there is increasing awareness of an essential difference in the meanings of these two terms with reference to cancer. We discuss evidence suggesting that the concepts of development and evolution are both pertinent to the description of carcinogenesis; however, they appropriately apply to distinct phases of a multistep process. Such a distinction bears important implications for the study and management of cancer.

Amniotic epithelial cells (AECs) represent a useful and noncontroversial source for liver-based regenerative medicine, as they can differentiate into hepatocytes upon transplantation into the liver. However, the possibility that AECs can differentiate into other liver cell types, such as hepatic sinusoidal endothelial cells (HSECs), has never been assessed. In order to test this hypothesis, rat- and human-derived AECs (rAECs and hAECs, respectively) were subjected to endothelial cell tube formation assay in vitro. Moreover, to evaluate differentiation in vivo, the retrorsine (RS) model of liver repopulation was used. Pyrrolizidine alkaloids (including RS) are known to target both hepatocytes and endothelial cells, inducing cell enlargement and inhibition of cell cycle progression. rAECs and hAECs were able to form capillary-like structures when cultured under proangiogenic conditions. For in vivo experiments, rAECs were obtained from dipeptidyl peptidase type IV (DPP-IV, CD26) donors and were transplanted into the liver of recipient CD26 negative animals pretreated with RS. rAEC-derived cells were engrafted in between hepatocytes and resembled HSECs as assessed by morphological analysis and the pattern of expression of CD26. Donor-derived CD26+cells coexpressed HSEC markers RECA-1 and SE-1, while they lacked expression of typical hepatocyte markers (i.e., cytochrome P450, hepatocyte nuclear factor 4α). As such, these results provide the first evidence that AECs can respond to proangiogenic signals in vitro and differentiate into HSECs in vivo. Furthermore, they support the conclusion that AECs possesses great plasticity and represents a promising tool in the field of regenerative medicine both in the liver and in other organs.

The aim of Regenerative Medicine is to replace or regenerate human cells, tissues or organs in order to restore normal function. Among all organs, the liver is endowed with remarkable regenerative capacity. Nonetheless, there are conditions in which this ability is impaired, and the use of isolated cells, including stem cells, is being considered as a possible therapeutic tool for the management of chronic hepatic disease. Placenta holds great promise for the field of regenerative medicine. It has long been used for the treatment of skin lesions and in ophthalmology, due to its ability to modulate inflammation and promote healing. More recently, cells isolated from the amniotic membrane are being considered as a possible resource for tissue regeneration, including in the context liver disease. Two cell types can be easily isolated from human amnion: epithelial cells (hAEC) and mesenchymal stromal cells (hAMSC). However only the first cell population has been demonstrated to be a possible source of proficient hepatic cells. This review will summarize current knowledge on the differentiation of hAEC into liver cells and their potential therapeutic application.

An important drawback of local surgery for lesions in the anal canal is the difficulty of achieving en bloc full-thickness resections. The aim of this study is to evaluate TEM/TEO in lesions of this type from the point of view of morbidity, mortality and the quality of the pathology specimen. This is an observational study with prospective data collection from June 2004 to July 2018. Two groups are defined: group A (rectal tumors with proximal margin between 0 and ≤4 cm from anal verge) and group B (distal margin > 4 cm from anal verge). A technical description is provided; resections and postoperative complications in both groups are compared. During the study period, 757 patients underwent TEM/TEO. Finally, 692 patients were included, 192 patients in group A and 500 patients in group B. An en bloc surgical specimen was obtained in 176/192 patients (91.7%), although the defect was completely sutured in 132 (68.8%). In the comparative analysis, group A did not present significantly greater fragmentation of the resected piece [16/192 (8.3%) vs. 36/500 (7.2%), p = 0.630], although group A was associated with greater involvement of the surgical margin [28/192 (14.6%), 32/500 (6.4%), p = 0.001] and clinically relevant morbidity [16/192 (8.3%), 20/500 (4%), p = 0.034]. There was no mortality. The use of TEM/TEO to remove lesions originating in the anal canal is feasible. But we have to take into account that there is an increase in complications, technical difficulties and affected margins resection.

The aim of this study was to evaluate the effect of cavity preparation with Er:YAG laser on dentin adjacent to restorations submitted to cariogenic challenge in situ, by subsuperficial microhardness analysis. Bovine incisors were sectioned, flattened, and polished, resulting in 40 dentin slabs. The slabs were randomly assigned to four groups (n = 10), according to the cavity preparation method: I-high-speed handpiece (control); II-Er:YAG laser (160 mJ; 3 Hz); III-Er:YAG laser (260 mJ; 3 Hz); IV-Er:YAG laser (300 mJ; 3Hz). Cavities were restored with composite resin, and the specimens were fixed in intra-oral appliances, which were worn by 10 volunteers for 14 days for simulating cariogenic challenge in situ. During the experimental period, 20% sucrose solution was dripped over each specimen 6 times a day. Samples were removed, sectioned, and examined for subsuperficial Knoop microhardness at 100, 200, and 300 μm from the restoration and at 30 μm from dentin surface. Split-plot analysis of variance showed no significant difference among the cavity preparation techniques (p = 0.1129), among distances (p = 0.9030), as well as no difference in the interaction between the main factors (p = 0.7338). It was concluded that the cavity preparation with Er:YAG laser did not influence on dentin microhardness submitted to cariogenic challenge in situ.

The aim of the present clinical randomized split-mouth study was to evaluate the effectiveness and efficiency of an Er:YAG laser for caries removal in primary molars, microbiological dentin analysis, and clinical restorations after 1 year in 29 children.The children's teeth were randomized into two groups: (I) an Er:YAG laser group and (II) a bur preparation group. The efficiency of the treatments (the time necessary for the removal of carious tissue) was evaluated based on the time spent on caries removal in the deciduous molars. The effectiveness (caries removal capacity) of the caries removal was determined by means of a blind test in which the examiner performed a tactile and visual examination of the dentin. Microbiological analysis was performed by counting the Streptococcus mutans and Lactobacillus sp in the remaining dentin. Clinical analysis of restorations was performed using the USPHS method in combination with photographs of restored teeth, 7 days after the restorative procedure and again after 1 year. All cavities were restored with the Adper Single Bond 2/Filtek Z350 system. The obtained data were analyzed with a significance level of 5 %.The Er:YAG laser was less effective and had the same efficacy as bur preparation during caries removal at the pulpal wall of deciduous molars. In the surrounding walls, bur preparation was the more effective method. Regardless of the method employed, the affected dentin in the pulpal wall had similar amounts of S. mutans and Lactobacillus sp. The restorations were clinically accepted by the USPHS method over a 1-year period.It can be concluded that caries removal with an Er:YAG laser has no influence on the clinical behavior of restorations.Irradiation with an Er:YAG laser is appropriate for caries removal in primary teeth.

The evidence linking aging and cancer is overwhelming. Findings emerging from the field of regenerative medicine reinforce the notion that aging and cancer are profoundly interrelated in their pathogenetic pathways. We discuss evidence to indicate that age-associated alterations in the tissue microenvironment contribute to the emergence of a neoplastic-prone tissue landscape, which is able to support the selective growth of preneoplastic cell populations. Interestingly, tissue contexts that are able to select for the growth of preneoplastic cells, including the aged liver microenvironment, are also supportive for the clonal expansion of normal, homotypic, transplanted cells. This suggests that the growth of normal and preneoplastic cells is possibly driven by similar mechanisms, implying that strategies based on principles of regenerative medicine might be applicable to modulate neoplastic disease.

This study evaluates, in situ, the effect of an anti-asthmatic medicine on the enamel and dentin of primary teeth. Twenty-eight specimens of enamel and dentin were prepared, selected, sterilized, and stored. Microhardness, microscopic morphology, and initial surface roughness were evaluated. Fourteen volunteers wore palatal devices containing two dentin specimens and two enamel specimens. Volunteers underwent surface treatment three times a day for 5 days, with the dripping of 5 ml of salbutamol sulfate for 1 min, only on specimens from one side of the device. At the end of the experiment the microscopic morphology, roughness, surface loss, and microhardness were ascertained. For enamel surfaces exposed to the medicine, erosion was observed in the microscopy images, along with a significant increase in roughness (p = .0325) and tissue loss (p = .03251) and a significant decrease in microhardness (p = .0325). For the dentin surfaces, erosion was observed in the microscopy images, but there was no significant increase in roughness (p = .593) or tissue loss (p = .285) nor any decrease in microhardness (p = .1088). In conclusion, the salbutamol sulfate had an in situ erosive effect on primary teeth enamel and this effect was observed morphologically on primary dentin. RESEARCH HIGHLIGHTS: Asthma is considered a global health problem, and its prevalence has increased in many countries, especially among children, as well as dental erosion. This study has clinical relevance because provides data on the erosive effect of a commonly prescribed anti asthmatic drug on deciduous teeth.

This study aimed to develop and to assess an algorithm to facilitate lip print visualization, and to digitally analyze lip prints on different supports, by superimposition. It also aimed to classify lip prints according to sex. A batch image processing algorithm was developed, which facilitated the identification and extraction of information about lip grooves. However, it performed better for lip print images with a uniform background. Paper and glass slab allowed more correct identifications than glass and the both sides of compact disks. There was no significant difference between the type of support and the amount of matching structures located in the middle area of the lower lip. There was no evidence of association between types of lip grooves and sex. Lip groove patterns of type III and type I were the most common for both sexes. The development of systems for lip print analysis is necessary, mainly concerning digital methods.

Background: It remains uncertain as to whether or not CO2 laser is able to hinder demineralization of enamel. The possibility to use bovine instead of human teeth on anticariogenic studies with laser has not yet been determined. Purpose: To compare the ability of CO2 laser and fluoride to inhibit caries-like lesions in human enamel and to test whether a similar pattern of response would hold for bovine enamel. Study Design: Ninety-six enamel slabs (2 × 2 × 4 mm) (48 from bovine and 48 from human teeth) were randomly distributed according to surface treatment (n = 12): CO2 laser, 5% sodium fluoride varnish (FV), 1.23% acidulated phosphate fluoride (APF) gel, or no treatment (control). Specimens were subjected to a 14-day in vitro cariogenic challenge. Microhardness (SMH) was measured at 30 μm from the surface. For ultrastructural analysis, additional 20 slabs of each substrate (n = 5) received the same treatment described earlier and were analyzed by SEM. Results: ANOVA and Tukey test ascertained that CO2 laser promoted the least mineral loss (SMH = 252a). Treatment with FV resulted in the second highest values (207b), which was followed by APF (172c). Untreated specimens performed the worst (154d). SEM showed no qualitative difference between human and bovine teeth. APF and control groups exhibited surfaces covered by the smear layer. A granulate precipitate were verified on FV group and fusion of enamel crystals were observed on lased-specimens. Conclusions: CO2 laser may control caries progression more efficiently than fluoride sources and bovine teeth may be a suitable substitute for human teeth in studies of this nature. Microsc. Res. This study ascertained whether under dental erosion models that closely mimics the real-life situation enamel and root dentin from bovine origin would be reliable substitutes for human counterparts. Through a 2x2 crossover design, in a first trial, 14 volunteers wore a palatal device containing slabs of bovine and human enamel. Half of the participants ingested (4x daily, for 10 days) orange juice first, crossing over to mineral water, while the remainder received the reverse sequence. In a second trial, volunteers wore devices with slabs of bovine and human root dentin. Except for the duration of each intraoral phase, which lasted 2 rather 10 days, the experiment with root dentin run exactly as for enamel. Dental substrates were analyzed for surface microhardness. Two-way ANOVAs (α=0.05) indicated no difference between the microhardness values recorded for human and bovine enamel (p=0.1350), but bovine root dentin had lower microhardness compared to its human counterpart (p=0.0432). While bovine enamel can reliably substitute its human counterpart in in situ dental erosion models, bovine root dentin does not seem to be a viable alternative to the corresponding human tissue.

Dental erosion is a type of wear caused by non bacterial acids or chelation. There is evidence of a significant increase in the prevalence of dental wear in the deciduous and permanent teeth as a consequence of the frequent intake of acidic foods and drinks, or due to gastric acid which may reach the oral cavity following reflux or vomiting episodes. The presence of acids is a prerequisite for dental erosion, but the erosive wear is complex and depends on the interaction of biological, chemical and behavioral factors. Even though erosion may be defined or described as an isolated process, in clinical situations other wear phenomena are expected to occur concomitantly, such as abrasive wear (which occurs, e.g, due to tooth brushing or mastication). In order to control dental loss due to erosive wear it is crucial to take into account its multifactorial nature, which predisposes some individuals to the condition.

Objectives This triple-blind, 2 × 2 crossover in situ study, was undertaken to verify whether the wear resistance of enamel and root dentine would be affected by bleaching with a 10% carbamide peroxide agent and a placebo agent. Methods Thirty slabs of each substrate (2 mm × 3 mm × 2 mm) were selected for each phase, after flattening and polishing procedures and microhardness test. After a 7-day lead-in period, one specimen of each substrate was randomly bonded on the facial surface of each one of 30 subject's upper second premolars. The volunteers received instructions on how to perform toothbrushing and application of gel in the tray. Fifteen volunteers bleached their maxillary arch with a 10% carbamide peroxide bleaching agent for a 2-week period, while the remainders used a placebo agent. After a 1-week washout period, a new set of enamel and root dentine slabs were bonded to the premolars and volunteers were crossed over to the alternate agent for 14 days. The resistance of enamel and root dentine to wear following bleaching, toothbrushing and intraoral exposure was measured with a profilometer, using reference areas. Results For enamel, ANOVA did not demonstrate significant difference between wear provided by placebo and bleaching agent ( p = 0.3713), but higher wear depth was observed for bleached root dentine ( p = 0.0346). Conclusions While overnight bleaching caused no alteration in wear resistance of enamel, root dentine showed increased tissue loss.

Background and Objectives Er:YAG laser has been used for caries removal and cavity preparation, using ablative parameters. Its effect on the margins of restorations submitted to cariogenic challenge has not yet been sufficiently investigated. The aim of this study was to assess the enamel adjacent to restored Er:YAG laser-prepared cavities submitted to cariogenic challenge in situ, under polarized light microscopy. Study Design/Materials and Methods Ninety-one enamel slabs were randomly assigned to seven groups (n = 13): I, II, III—Er:YAG laser with 250 mJ, 62.5 J/cm2, combined with 2, 3, and 4 Hz, respectively; IV, V, VI—Er:YAG laser with 350 mJ, 87.5 J/cm2, combined with 2, 3, and 4 Hz, respectively; VII—High-speed handpiece (control). Cavities were restored and the restorations were polished. The slabs were fixed to intra-oral appliances, worn by 13 volunteers for 14 days. Sucrose solution was applied to each slab six times per day. Samples were removed, cleaned, sectioned and ground to polarized light microscopic analysis. Demineralized area and inhibition zone width were quantitatively assessed. Presence or absence of cracks was also analyzed. Scores for demineralization and inhibition zone were determined. Results No difference was found among the groups with regard to demineralized area, inhibition zone width, presence or absence of cracks, and demineralization score. Inhibition zone score showed difference among the groups. There was a correlation between the quantitative measures and the scores. Conclusion Er:YAG laser was similar to high-speed handpiece, with regard to alterations in enamel adjacent to restorations submitted to cariogenic challenge in situ. The inhibition zone score might suggest less demineralization at the restoration margin of the irradiated substrates. Correlation between the quantitative measures and scores indicates that score was, in this case, a suitable complementary method for assessment of caries lesion around restorations, under polarized light microscopy. The aim of this study was to evaluate the effect of industrialised foods and drinks on primary tooth enamel previously eroded with hydrochloric acid (HCl). The crowns of one hundred two specimens were subjected to an erosive challenge with HCl and randomly divided into six groups (n = 17): Chocolate Milk (Toddynho® - Pepsico) - negative control; Petit Suisse Yogurt (Danoninho® - Danone); Strawberry Yogurt (Vigor); Apple puree (Nestlé); Fermented Milk (Yakult® - Yakult); and Home Squeezed Style Orange Juice (del Valle) - positive control. The 28-day immersion cycles for the test products were performed twice daily and were interspersed with exposure of the test substrate to artificial saliva. Measurements of enamel surface microhardness (SMH) were performed initially, after immersion in HCl and at 7, 14, 21 and 28 days of experimentation. A two-way ANOVA, according to a split-plot design, followed by the sum of squares decomposition and Tukey’s test, revealed a significant effect for the interaction between Foods and Drinks and Length of Exposure (p < 0.00001). Orange juice resulted in greater mineral loss of enamel after 28 days. None of the test products was associated with recovery of tooth enamel microhardness.

Objective: This study sought to evaluate whether root dentine caries-like lesions could be remineralised by saliva substitutes. Methods: Root dentine slabs (3 × 3 × 2 mm) were cut from bovine incisors, ground flat, polished and pre-tested for Knoop microhardness (KHN) at five locations spaced 500 μm apart and 500 μm from the left edge of each sectioned piece. After 60 out of the 100 slabs had been selected based upon their KHN values, specimens were coated with wax except for their outer surface. Specimens were then cycled through a highly cariogenic challenge model to induce caries-like lesions, whose formation was confirmed by KHN measurements located 500 μm from the right edge of the specimen. According to a randomised complete block design, the experimental units (n = 15) were exposed to 1.5 ml of saliva substitutes, based on either mucin (MC) or carboxymethylcellulose (CM), to natural human saliva (HS) or to 100% relative humidity (RH) over 20 days. A critical testing aspect for a reliable two-body wear simulation may be the lubricity condition. The main hypothesis tested in this study was that, over the experiment duration, the presence or absence of lubricants would provide to a composite similar wear rates to those observed under human saliva lubrication.Seventy specimens, fabricated from a minifill composite, were tested in a programmable logic controlled wear simulator with human saliva (HS), mucin-based artificial saliva (MC), carboxymethylcellulose-containing saliva (CM), deionized water (DW) or with no lubrication (UN). Wear depth was quantified at the end of 1000, 5000, 10,000 and 50,000 cycles and at each 50,000 through 250,000 cycles.Over all the test period, UN led to the greatest wear. Initially, lubrication resulted in no significant differences among the wear rates. Subsequently, up to 50,000 cycles, depths of wear provided by HS and MC were still similar, being the lowest, whereas DW yielded greater wear than CM. Later, interposition of CM and HS resulted in equivalent wear. At 250,000 cycles, wear of the composite ranked as follows: MCHSCMDWUN. Regression analyses showed that for all lubricity conditions wear of the composite rose logarithmically with number of cycles.The role of the interposed lubricant was of importance for the wear testing. The present findings supported by studies on the lubricity ability of mucin-based artificial saliva, allowed to conclude that this preparation may be considered as a potential lubricant for in vitro wear test purposes.

This study evaluated in vitro the cariostatic effect of 6 restorative materials with and without fluoride release (Fuji II LC, F-2000, Degufil Mineral, Sure Fil and Z-250) associated with a fluoridated and a non-fluoridated dentifrice (Sensodyne Original Formula and Sensodyne Sodium Bicarbonate), on human enamel. Class V-like cavities were prepared on 240 enamel slabs, assigned to 12 groups (6 materials and 2 dentifrices). After cavity restoration, the slabs were submitted to a thermocycling regimen of 1000 thermal cycles and demineralization/remineralization cycles. During pH cycles, slurries of fluoridated and non-fluoridated dentifrices were applied for 5 min. Formation of artificial caries-like lesions was scored independently and blindly by 5 calibrated examiners according to an ordinal scale ranked 0 to 3 by visual examination. The results were analyzed statistically by the Kruskal-Wallis test and pair-wise comparisons (alpha=0.05). There were no significant differences (p>0.05) among the restorative materials associated with the fluoridated dentifrice. When used in association with the non-fluoridated dentifrice, Ketac-Fil showed the highest cariostatic effect followed by Fuji II LC and the other materials. Ketac-Fil was the only material that did not differ statistically when combined with either the fluoridated or the non-fluoridated dentifrice. In conclusion, under the tested experimental conditions, the association of restorative materials and fluoridated dentifrice yielded higher cariostatic effect, except for the conventional glass ionomer cement, whose cariostatic effect was not influenced by the type of dentifrice. O objetivo deste trabalho foi avaliar, in vitro, o efeito cariostático de 6 materiais restauradores contendo ou não flúor (Fuji II LC, F-2000, Degufil Mineral, Sure Fil and Z-250) associados à aplicação de dentifrícios (fluoretado e não-fluoretado; Sensodyne Fórmula Original and Sensodyne Sodium Bicarbonate) em esmalte dental humano. Cavidades classe V foram preparadas no esmalte de 240 fragmentos dentais, aleatoriamente divididos em 12 grupos (6 materiais e 2 dentifrícios). Após serem restauradas as cavidades, os fragmentos foram submetidos a ciclos térmicos e de desmineralização e remineralização, simulando um alto desafio cariogênico. Sobre os fragmentos restaurados, ainda, foram aplicados dentifrícios contendo ou não flúor, 5 min por dia. As diferenças no desenvolvimento de lesões experimentais de cárie adjacente às restaurações foram avaliadas por 5 examinadores calibrados, através de inspeção visual, atribuindo-se escores de 0 a 3. Os resultados foram avaliados pelo teste de Kruskal-Wallis seguido pelo teste de comparações múltiplas (alfa=0,05). Quando associados ao dentifrício fluoretado, os materiais restauradores não diferiram entre si em relação ao potencial cariostático (p>0,05). Quando os materiais foram utilizados em associação com o dentifrício não-fluoretado o Ketac-Fil apresentou o maior potencial cariostático, seguido pelo Fuji II LC e pelos demais materiais. O Ketac-Fil foi o único material que não diferiu significativamente quando associado com o dentifrício com ou sem flúor. Sob as condições experimentais do estudo, a associação dos materiais restauradores ao dentifrício fluoretado resultou em uma maior ação cariostática, exceto para o cimento de ionômero de vidro convencional, que não diferiu com a aplicação dos dentifrícios.

The purpose of this paper was to evaluate in situ the microhardness of a composite resin, two resin-modified glass-ionomers and two polyacid-modified composite resins at different post-irradiation times. The materials analysed were: Z 100/3M (Z1); Fuji II LC/GC Corporation (Fu); Vitremer/3M (Vi); Freedom/SDI (Fr); and Dyract/ Dentsply (Dy). Palatal intra-oral acrylic resin appliances were made for each of the 24 volunteers in the experiment. In each appliance, a specimen of each material was fixed. Microhardness tests were performed at 30 min, 1, 2, 4, 8, 24 and 48 h, 7, 10, 14, 17 and 21 days after initial light-curing. The analysis of variance followed by the Tukey test showed significant differences of microhardness among all materials (Fu < Vi < Fr < Dy < Z1) up to 48 h. From the seventh to the fourteenth day, statistical differences were not found between Freedom and Dyract (Fu < Vi < Fr = Dy < Z1). On the seventeenth and twenty-first days, Vitremer, Freedom and Dyract did not present statistical differences, but differed from Fuji II LC and Z 100 (Fu < Vi = Fr = Dy < Z1). The polynomial regression showed an increase of microhardness over time for the glass-ionomers/ composite resin hybrid materials, although there were differences of microhardness among these materials. There was no significant changes in microhardness levels for the composite resin over time.

Objective . The aim of this in vitro study was to evaluate the interaction between two sources of fluoride (restorative systems and dentifrices) in inhibiting artificial root caries development. Methods . One hundred and eighty tooth segments were embedded in polyester resin, and sanded flat. Cylindrical cavities 1.0 mm-deep and 1.5 mm-diameter were prepared in root dentin and randomly restored by fluoride-containing restorative systems: Ketac-fil/Espe (Ke), Fuji II LC/GC Corp (Fj), F2000/3M (F2), Surefil/Dentsply (Su) or a control: Filtek Z250/3M (Z2). Ten experimental groups were made to test the association among the five restorative systems and two dentifrices: with F − (Sensodyne Baking Soda) or without F − (Sensodyne Original) ( n =18). After surface polishing, a 1 mm-wide margin around the restorations was demarcated and initial dentin surface Knoop microhardness values (KHN i ) were obtained. The specimens were submitted to a pH-cycling model, and to applications of slurries of dentifrice. Afterwards the final dentin surface Knoop microhardness values (KHN f ) were measured. Results . The differences between KHN i and KHN f , and the covariate KHN i were considered by the ANCOVA and Tukey's test ( α =0.05). The interaction between restorative system and dentifrice was statistically significant ( p =0.0026). All restorative systems provided some protection against artificial caries challenge when associated with the fluoride-containing dentifrice treatment. The means (standard deviation) of reductions in Knoop hardness values for systems associated with the fluoride-containing dentifrice were: Ke: 40.0(1.02) a , Fj: 41.9(1.02) b , F2: 43.3(1.04) c , Su: 43.5(1.00) c , Z2: 44.0(1.02) c ; and with the non-fluoride-containing dentifrice were: Ke: 42.9(1.02) a , Fj: 44.7(1.01) b , F2: 45.2(1.09) bc , Su: 46.0(0.99) c , Z2: 46.6(0.99) c (statistical differences were expressed by different letters). Conclusion . The cariostatic effect shown by the fluoride-containing dentifrice could enhance that shown by Ketac-fil and Fuji II LC, and could mask that shown by F2000.

The aim of the study was to determine the reproducibility and validity of DIAGNOdent in detecting active and arrested caries lesions on free smooth surfaces. Volunteers were selected from state schools of Piracicaba, São Paulo, Brazil. Overall, 220 lesions were clinically examined. Two specially trained (‘calibrated’) examiners performed both clinical and laser evaluations independently, and after a 1-week interval, the examinations were repeated. The intra-examiner agreement for the laser evaluation was substantial (ĸex1 = 0.79, ĸex2 = 0.71). There was almost perfect agreement between the two examiners for the clinical examination (ĸex1 = 0.95, ĸex2 = 0.85). The inter-examiner agreement showed substantial reproducibility (ĸ = 0.77) for the laser examination and almost perfect agreement (ĸ = 0.85) for the clinical evaluation. The validation criterion was the clinical examination of white spots, recorded as active or arrested. The sensitivity was 0.72 and the specificity was 0.73, which indicates that the DIAGNOdent was a good auxiliary method for detecting incipient caries lesions on free smooth surfaces. The utilization of both methods can improve the efficacy of caries diagnosis.

Polytrauma continues to be one of the main causes of death in the population between 10-40 years of age, and causes severe discapability in surviving patients. The aim of this study is to perform an analysis of the quality of care of the polytrauma patient using an epidemiological study.Prospective registry of all polytrauma patients treated at our hospital over 16 years of age, admitted to the critical care area or dead before admission.From March 2006 to August 2014, we registered 1200 polytrauma patients. The majority were men (75%) with a median age of 45. The mean ISS was 20,9±15,8 and the most common mechanism of injury was blunt trauma (94% cases), The global mortality rate was 9.8% (117 cases), and neurological death was the most frequent cause (45.3%), followed by hypovolemic shock (29,1%). In 17 cases (14,5% of deaths) mortality was considered evitable or potentially evitable, A total of 327 patients (27.3%) needed emergency surgery and 106 patients (8,8%) needed emergency treatment using interventional radiology. 18,5% of patients (222) presented an inadverted injury, with a total of 318 inadverted injuries.Trauma care at our centre is adequate. A prospective registry of the global care of polytrauma patients is necessary to evaluate the quality of care and improve results.

Objective This in situ study evaluated the effect of fluoride varnish combined with CO 2 laser in controlling enamel demineralization caused by cariogenic challenges. Design In a crossover study conducted in 2 phases of 14 days each, 14 volunteers ( n = 14) wore palatal appliances with bovine enamel slabs treated with fluoride varnish + CO 2 laser (FV + CO 2 ), fluoride varnish (FV), nonfluoride placebo varnish (PV) and nonfluoride placebo varnish + CO 2 laser (PV + CO 2 ). Drops of sucrose solution were dripped onto enamel slabs allowing the accumulation of biofilm. At the first phase, half of the volunteers received 4 enamel slabs treated with FV while the remainders received slabs exposed to the PV with and without CO 2 laser. In the second phase, the vonlunteers were reversed treatments. The slabs were evaluated for cross-sectional microhardness (CSMH) and the concentration of loosely bound fluoride (CaF 2 ) and firmly bound fluoride (FAp). The concentration of fluoride in biofilm were also determined. Results Two-way ANOVA showed that the CSMH values were higher in laser-irradiated enamel, regardless of the fluoride varnish. Friedman test showed that FV group presented significantly larger amount of fluoride in biofilm ( P 2 , which was not different from FV ( P > 0.05). Conclusion The synergistic effect of fluoride varnish and CO 2 laser on enamel demineralization was not observed, however, CO 2 laser reduces enamel demineralization. Clinical significance CO 2 laser might reduce the demineralization of subsurface enamel, although its association with a high concentrated fluoride therapy may not result in a positive synergistic interaction.

This study evaluated, in vitro, the erosive potential of pediatric liquid medicines in primary tooth enamel, depending on the exposure time. Sixty deciduous incisors were randomly assigned to 4 groups (n=15), according to the immersion solutions: guaifenesin; ferrous sulfate; salbutamol sulfate and artificial saliva. The immersion cycles in the medicines were undertaken under a 1-min agitation, which wasperformed three times daily, during 28 days. Surface microhardness was measured at 7,14, 21 and 28 days. The titratable acidity and buffering capacity of the immersion media were determined. Data were analyzed by Analysis of Variance and Tukey's test (α=0.05). Salbutamol sulfate caused a gradual loss in enamel microhardness deciduous, observed at all times (p0.005) at any of the studied times. Scanning Electron Microscopy (SEM) images revealed that after 28 days the surfaces clearly exhibited structural loss, which was unlike those immersed in artificial saliva. Erosion of deciduous enamel was dependent on the type of medicine and exposure time. Este estudo avaliou, in vitro, o potencial erosivo de medicamentos líquidos pediátricos em esmalte de dentes decíduos, em função do tempo de exposição. Sessenta incisivos decíduos foram divididos aleatoriamente em 4 grupos (n=15), de acordo com a solução de imersão: guaifenesina, sulfato ferroso, sulfato de salbutamol e saliva artificial. Os ciclos de imersão nos medicamentos foram realizados sob agitação por 1 min, três vezes ao dia, durante 28 dias. As medidas de microdureza superficial foram realizadas após 7, 14, 21 e 28 dias. A acidez titulável e capacidade tampão dos meios de imersão foram determinadas. Os dados foram submetidos à Análise de Variância e teste de Tukey (α=0,05). O sulfato de salbutamol causou uma perda gradual na microdureza do esmalte decíduo, em todos os tempos verificados (p0,005) da microdureza em nenhum dos tempos estudados. As imagens de microscopia eletrônica de varredura (MEV) revelaram que após 28 dias, as superfícies expostas aos medicamentos apresentaram perda estrutural, diferindo dos que foram imersos em saliva artificial. A erosão do esmalte decíduo foi dependente do tipo de medicamento e do tempo de exposição.

This study evaluates the bond strength of dentin prepared with Er:YAG laser or bur, after rewetting with chlorhexidine on long-term artificial saliva storage and thermocycling. One hundred and twenty human third molars were sectioned in order to expose the dentin surface (n = 10). The specimens were randomly divided in 12 groups according to treatment and aging: Er:YAG laser rewetting with deionized water (LW) and 24 h storage in artificial saliva (WC); LW and 6 months of artificial saliva storage + 12.000 thermocycling (6M), LW and 12 months of artificial saliva storage + 24.000 thermocycling (12M), Er:YAG laser rewetting with 2% chlorhexidine (LC) and WC, LC and 6M, LC and 12M, bur on high-speed turbine rewetting with deionized water (TW) and WC, TW6M, TW12M, bur on high-speed turbine + 2% chlorhexidine (TC) and WC, TC and 6M, TC and 12M. The specimens were etched with 35% phosphoric acid, washed, and dried with air. Single Bond 2 adhesive was applied and the samples were restored with a composite. Each tooth was sectioned in order to obtain 4 sticks, which were submitted to microtensile bond strength test (µTBS). The two-way ANOVA, showed no significant differences for the interaction between the factors and for the aging factor. Tukey 5% showed that the LC group had the lowest µTBS. The rewetting with chlorhexidine negatively influenced the bond strength of the preparation with the Er:YAG laser. The artificial saliva aging and thermocycling did not interfere with dentin bond strength.

A B S T R A C T Background: In the international scientific literature, there are few studies that emphasize the presence or absence of hair in forensic facial reconstructions. There are neither Brazilian studies concerning digital facial reconstructions without hair, nor research comparing recognition tests between digital facial reconstructions with hair and without hair. The miscegenation of Brazilian people is considerable. Brazilian people, and, in particular, Brazilian women, even if considered as Caucasoid, may present the hair in very different ways: curly, wavy or straight, blonde, red, brown or black, long or short, etc. For this reason, it is difficult to find a correct type of hair for facial reconstruction (unless, in real cases, some hair is recovered with the skeletal remains). Aims and methods: This study focuses on the performance of three different digital forensic facial reconstructions, without hair, of a Brazilian female subject (based on one international database and two Brazilian databases for soft facial-tissue thickness) and evaluates the digital forensic facial reconstructions comparing them to photographs of the target individual and nine other subjects, employing the recognition method. A total of 22 assessors participated in the recognition process; all of them were familiar with the 10 individuals who composed the face pool. Results and conclusions: The target subject was correctly recognized by 41% of the 22 examiners in the International Pattern, by 32% in the Brazilian Magnetic Resonance Pattern and by 32% in the Brazilian Fresh Cadavers Pattern. The facial reconstructions without hair were correctly recognized using the three databases of facial soft-tissue thickness. The observed results were higher than the results obtained using facial reconstructions with hair, from the same skull, which can indicate that it is better to not use hair, at least when there is no information concerning its characteristics.

To evaluate the effect of a 1.23% acidulated phosphate fluoride (APF) gel combined with CO2 laser in protecting carious root dentin against further cariogenic challenges.After a 7-day lead-in period, 12 volunteers wore an intraoral palatal device containing four carious root dentin slabs, treated with APF and APF+CO2 or placebo and placebo+CO2. After a 14-day wash-out period, volunteers were crossed-over to the other treatment arm. During both intraoral phases, specimens were submitted to cariogenic challenges and then evaluated for cross-sectional Knoop microhardness.Two-way ANOVA demonstrated that there was significant effect for both main factors: CO2 laser irradiation (P0.0001) and gel treatment (P0.0001), and that there was no interaction between them (P = 0.4706). Protection of carious root dentin against further cariogenic challenges may be provided by APF fluoride gel and CO2 laser, but no additive benefit was found by combining such strategies.

Facial reconstruction is a method that seeks to recreate a person's facial appearance from his/her skull. This technique can be the last resource used in a forensic investigation, when identification techniques such as DNA analysis, dental records, fingerprints and radiographic comparison cannot be used to identify a body or skeletal remains. To perform facial reconstruction, the data of facial soft tissue thickness are necessary. Scientific literature has described differences in the thickness of facial soft tissue between ethnic groups. There are different databases of soft tissue thickness published in the scientific literature. There are no literature records of facial reconstruction works carried out with data of soft tissues obtained from samples of Brazilian subjects. There are also no reports of digital forensic facial reconstruction performed in Brazil. There are two databases of soft tissue thickness published for the Brazilian population: one obtained from measurements performed in fresh cadavers (fresh cadavers' pattern), and another from measurements using magnetic resonance imaging (Magnetic Resonance pattern). This study aims to perform three different characterized digital forensic facial reconstructions (with hair, eyelashes and eyebrows) of a Brazilian subject (based on an international pattern and two Brazilian patterns for soft facial tissue thickness), and evaluate the digital forensic facial reconstructions comparing them to photos of the individual and other nine subjects. The DICOM data of the Computed Tomography (CT) donated by a volunteer were converted into stereolitography (STL) files and used for the creation of the digital facial reconstructions. Once the three reconstructions were performed, they were compared to photographs of the subject who had the face reconstructed and nine other subjects. Thirty examiners participated in this recognition process. The target subject was recognized by 26.67% of the examiners in the reconstruction performed with the Brazilian Magnetic Resonance Pattern, 23.33% in the reconstruction performed with the Brazilian Fresh Cadavers Pattern and 20.00% in the reconstruction performed with the International Pattern, in which the target-subject was the most recognized subject in the first two patterns. The rate of correct recognitions of the target subject indicate that the digital forensic facial reconstruction, conducted with parameters used in this study, may be a useful tool.

To assess, by a crossover 2 x 2 in situ study, the speculated protective role of a sodium bicarbonate-containing toothpaste in controlling erosive lesions.Bovine enamel slabs were sterilized, and submitted to baseline Knoop microhardness measurements. After a 3-day lead-in period, 14 volunteers wore palatal acrylic appliances containing six enamel slabs (three on each side), for 4 consecutive days. On the first day, appliances with contained specimens were placed in the oral cavity to allow salivary pellicle formation. On the subsequent days, half of the enamel slabs were immersed extraorally in a lemonade-like soft drink for 90 seconds, twice daily. On both of these occasions, the appliance was dipped in toothpaste slurry of either a sodium bicarbonate-containing toothpaste or a regular counterpart for 60 seconds. Following a 3-day washout period, a new set of enamel slabs were mounted and the volunteers started the second period using the alternate dentifrice.ANOVA (alpha = 0.05) showed no statistically significant difference between enamel treated with regular and sodium bicarbonate-based dentifrices, regardless of whether specimens were eroded or not (P = 0.8430). Acid-challenged specimens revealed lower microhardness values than uneroded samples.

This study assessed the effect of the Er:YAG laser on enamel adjacent to restorations submitted to cariogenic challenge in situ, by microhardness analysis.The influence of Er:YAG laser energy on secondary caries formation has been investigated in vitro. However, no research involving demineralization around cavities prepared with the Er:YAG laser under intra-oral conditions has yet been described.Slabs of enamel were randomly assigned to seven groups (n = 12), according to the cavity preparation technique: groups I, II, and III--Er:YAG laser, 250 mJ, at 2, 3, and 4 Hz, respectively; groups IV, V, and VI--350 mJ, at 2, 3, and 4 Hz, respectively; group VII--high-speed handpiece (control). Cavities were restored and the specimens were fixed in intra-oral appliances and worn by 12 volunteers for 14 d. Sucrose solution was applied to each slab 6 times/d. Samples were removed, sectioned, and examined for microhardness at 100, 200, and 300 microm (factor distance), and 30, 60, and 90 microm (factor depth), from the restoration and enamel surface, respectively.Analysis of variance according to a split-plot model showed no difference among the cavity preparation techniques, among distances, or among depths, as well as no difference in the interaction between the factors of cavity preparation and distance. A significant difference was found in the interaction of cavity preparation and depth (p0.0001), as identified by contrast technique.The results revealed that the Er:YAG laser did not differ from conventional cavity preparation with regard to enamel microhardness.

To evaluate the micromorphology and surface roughness of sound and demineralized enamel and dentin treated with a 10% carbamide peroxide bleaching agent in situ for 3 weeks.A 10% carbamide peroxide bleaching agent, Opalescence (OPA) was evaluated and a placebo agent (PLA) not containing carbamide peroxide, was used as a control group. Forty dental slabs [10 sound enamel slabs (SE), 10 demineralized enamel slabs (DE), 10 sound dentin slabs (SD) and 10 demineralized dentin slabs (DD)] were randomly fixed on the facial surface of the first maxillary molars and second maxillary premolars of 10 volunteers. Demineralized enamel and dentin fragments were obtained by a dynamic model using demineralizing and remineralizing solutions. The volunteers were divided into two groups that received the bleaching or the placebo agent in different sequences and periods in a double blind 2 x 2 cross-over study with a wash-out period of 2 weeks. Roughness was performed on the slab surfaces followed by scanning electron microscope (SEM) evaluations. The images were evaluated by three calibrated examiners in duplicate in two different periods at x1500 (SE and DE) and x2000 original magnification (SD and DD).The Wilcoxon test showed significant differences in roughness for SE and DE treated with OPA or PLA, with rougher surfaces treated with OPA. No differences in roughness were observed for SD or DD treated with OPA or PLA. As the Kappa value for the inter-examiner agreement of SEM evaluations showed "moderate" reproducibility (0.41k0.60) in Period 1 and "substantial" (0.61k0.8) in Period 2, the latter was considered for the statistical analysis. The McNemar test showed no significant differences between SE, DE, SD or DD treated with OPA or PLA.

To evaluate enamel permeability after external bleaching procedures.The coronal portion of 75 human canines were covered with an acid resistant varnish, except for a 7.1 mm2 circular area on the labial surface and, according to a randomized complete block design, specimens were allocated to bleaching treatments using carbamide peroxide-containing agents (CP): 10%, 16% or 37%; or a 35% hydrogen peroxide product (HP). The control group was exposed to artificial saliva. The bleaching agents were applied following the manufacturer's recommendations over 21 days. In between bleaching exposures, specimens were stored in artificial saliva. Samples were then prepared according to a histochemical coloring method based on copper sulfate and dithiooxamide solutions. Digital images captured in an optical microscope were used to measure the percentage of copper ion penetration over the total enamel thickness on three sections taken from the exposed area.Analysis of Deviance showed a significant difference for the permeability values among groups (P = 0.046). Likelihood ratio test revealed that the use of the CP10% and HP35% agents caused significantly greater enamel permeability than did the control treatment. No significant differences in percentage of copper ion penetration were found among CP10%, CP16%, CP37% and HP35%. This in vitro study demonstrated that depending on the external bleaching procedure, enamel permeability may be increased.

The acquired dental pellicle helps prevent erosion, but the protection level is unknown. This in situ study tested whether a two-hour pellicle protects against different erosive challenges by orange juice. Subjects wore palatal appliances loaded with either enamel or dentin specimens. Pellicle was allowed to form, or not (control), on the surfaces of the specimens intra-orally for 2 hrs before the erosive challenges of 0 (control), 10, 20, and 30 minutes’ duration. Specimens were randomly removed from the appliances after each challenge. Percentage of surface microhardness change (%SMC) was determined for the enamel specimens, and that of mineral loss and lesion depth for the dentin specimens. Enamel specimens with the pellicle showed a significantly lower %SMC, only after the 10-minute challenge. No protection was found for dentin. It was concluded that the acquired pellicle reduced dental erosion, but that this effect was limited to the less severe erosive challenge on enamel surfaces.

To evaluate in vitro the abrasive potential of different dentifrices on enamel previously exposed to an acidic soft drink.150 bovine enamel slabs measuring 6 x 3 x 2 mm were grounded flat and polished. Surface roughness was measured to obtain Ra (microm) baseline values using a profilometer. The slabs' outer surface was covered with a tape except for a 3 x 4 mm window. According to a randomized complete block design, specimens were exposed to an acidic drink or distilled water (control) for 5 minutes, and then to artificial saliva for 1 minute. Next, the experimental units were submitted to 5,000 brushing strokes using one of four dentifrices - regular (RE), baking soda (BS), tartar control (TC) or whitening (WT) - or distilled water as control (CO) (n=15). At the end of five repetitions of this erosive/abrasive cycle, the tape was removed and final readings of surface roughness were carried out. Roughness gain consisted of the subtraction of the baseline values from the post-treatment measurements. Also, the enamel wear due to erosive/abrasive challenges was profilometrically determined.ANOVA did not show significant interaction between enamel condition (eroded and sound) and dentifrice (P0.05) for both roughness gain and wear. No significant effect was found for enamel condition and dentifrice in the wear analysis. The exposure of enamel to the acidic drink provided higher roughness gain than did distilled water. Tukey's test evidenced that TC yielded significantly greater increase in surface roughness than did both CO and WT, whereas BS and RE induced an intermediate roughness gain.

This study evaluated the effects of 10% carbamide peroxide, carbopol and glycerin and their associations on microhardness over time on enamel and dentin. Eight treatment agents were evaluated: a commercial bleaching agent containing 10% carbamide peroxide (Opalescence 10% Ultradent), 10% carbamide peroxide, carbopol, glycerin, 10% carbamide peroxide + carbopol, 10% carbamide peroxide + glycerin, carbopol + glycerin and 10% carbamide peroxide + carbopol + glycerin. Three hundred and twenty human dental fragments, 80 sound enamel fragments (SE), 80 demineralized enamel fragments (DE), 80 sound dentin fragments (SD) and 80 demineralized dentin (DD) fragments, were exposed to the treatment agents (n=10). These agents were applied onto the surface of the fragments eight hours a day for 42 days. After eight hours, they were washed from the dental fragment surfaces after five back-and-forth movements with a soft bristle toothbrush under distilled and deionized running water. During the remaining time (16 hours per day), the fragments were kept in individual vials in artificial saliva. After the 42-day treatment period, the specimens were kept individually in artificial saliva for 14 days. Knoop microhardness measurements were performed at baseline, after eight hours, and 7, 14, 21, 28, 35 and 42 days, and 7 and 14 days post-treatment (corresponding to 49 and 56 days after the initial treatment agent applications). The non-parametric Kruskal-Wallis analysis showed significant differences among the agents at each time interval, except at baseline for sound and demineralized enamel and dentin. For SE, SD and DD, there was a decrease in microhardness values during treatment with all agents. There was a tendency towards lower microhardness values after treatment with carbopol and its associations for sound tissues. DD showed low microhardness values during and after treatment with CP and its associations. For DE, there was an increase in microhardness values during treatment with all agents and in the post-treatment phase. The baseline microhardness values were not recovered during the 14-day post-treatment phase. Opalescence 10%, carbamide peroxide, carbopol, glycerin and their associations may change the microhardness of sound and demineralized dental tissues, even in the presence of artificial saliva.

This in situ study sought to evaluate the surface roughness evolution of resin composites finished and polished by different methods. A total of 108 rectangular-shaped specimens of a microfilled and a hybrid composite were cured against a Mylar matrix strip and left unpolished or instrumented with diamond burs, Al2O3-coated disks, Al2O3-impregnated UDMA disk, or with diamond burs followed by either one of the disks. After specimens had been profiled for the average surface roughness (Ra, microm), 18 volunteers wore a removable palatal appliance, which accommodated one specimen of each one of the 12 groups investigated. Surface roughness for up to 28 days of intraoral exposure was then measured at 1- or 7-day intervals. A split-plot ANOVA (alpha = 0.05) revealed a significant interaction between group and time. Tukey's test and regression analyses ascertained that initially finishing with burs only provided the roughest surface to both composites. Unpolished surfaces and those specimens polished with Al2O3-coated disks, regardless of previous use of diamond burs, attained smoother surface. The Al2O3-impregnated UDMA disk was capable of smoothening the surface of the hybrid material previously finished with diamond burs. The roughness achieved after finishing and polishing composites may be either smoothened or roughened after intraoral exposure. On the basis of the roughness range, it is advisable to use Al2O3-coated disks, regardless of whether diamond burs were previously used. Al2O3-impregnated UDMA disks (with or without previous application of diamond burs) may be also suitable for instrumenting hybrid restoratives.

The usefulness of fluoride-releasing restorations in secondary caries prevention may be questioned because of the presence of other common sources of fluoride and because of ageing of the restorations. This study tested the hypothesis that glass-ionomer cement restorations, either aged or unaged, do not prevent secondary root caries, when fluoride dentifrice is frequently used. Sixteen volunteers wore palatal appliances in two phases of 14 days, according to a 2 × 2 crossover design. In each phase the appliance was loaded with bovine root dentine slabs restored with either glass-ionomer or resin composite, either aged or unaged. Specimens were exposed to cariogenic challenge 4 times/day and to fluoridated dentifrice 3 times/day. The fluoride content in the biofilm (FB) formed on slabs and the mineral loss (ΔZ) around the restorations were analysed. No differences were found between restorative materials regarding the FB and the ΔZ, for either aged (p = 0.792 and p = 0.645, respectively) or unaged (p = 1.00 and p = 0.278, respectively) groups. Under the cariogenic and fluoride dentifrice exposure conditions of this study, the glass-ionomer restoration, either aged or unaged, did not provide additional protection against secondary root caries.

To evaluate the effect of six bleaching agents: Nite White [NW] 10% and 22% Excel 2Z (Discus Dental), Rembrandt (REM) 10% and 22% (DenMat), Opalescence (OPA) 10% and 20% (Ultradent) and a placebo agent on demineralized dentin microhardness at different time intervals.105 human dentin slabs (3 x 3 mm) were embedded, planed and submitted to cariogenic challenges, composed of de- and remineralization cycles. For 42 days, specimens were exposed to bleaching agents, consisting of applying them daily for 8 hours, removing and storing the specimens in artificial saliva for 16 hours. At the end of the bleaching treatment, specimens were kept in artificial saliva for 14 days. Knoop Microhardness tests were performed on specimens' surface before (baseline), during (8 hours, 7, 14, 21, 28, 35 and 42 days) and after bleaching procedures (7 and 14 days).The ANOVA for split-plot showed significant effect on the interaction between bleaching agent and time (alpha =0.05). Tukey's test revealed no significant differences on demineralized dentin microhardness exposed to bleaching agents until Day 7. Regression analyses demonstrated that NW 10% and 22% and OPA 10% and 20% increased dentin microhardness in different magnitudes, whereas REM 10% and 22% induced mineral loss during bleaching agent application, followed by microhardness recovery in the post-treatment period.

It has been reported that bond strength of resin to tooth structure can be reduced when the bonding procedure is carried out immediately after the bleaching treatment. This study evaluated the effect of bleaching of non-vital teeth bleaching on the shear bond strength (SBS) of composite resin/bovine dentin interface and the influence of delaying the bonding procedures for different time intervals following internal bleaching. According to a randomized block design, composite resin cylinders (Z100/Single bond - 3M) were bonded to the flattened dentin surface of two hundred and fifty-six teeth which had previously been subjected to four different treatments: SPH - sodium perborate + 30% hydrogen peroxide; SPW - sodium perborate + distilled water; CP - 37% carbamide peroxide; and CON - distilled water (control), each one followed by storage in artificial saliva for 0 (baseline), 7, 14, and 21 days after bleaching (n = 16). The bleaching agents in the pulp chambers were replaced every 7 days, over 4 weeks. The SBS test of the blocks was done using a universal testing machine. The ANOVA showed that there was no significant interaction between time and bleaching agents, and that the factor time was not statistically significant (p > 0.05). For the factor bleaching treatment, the Student's t-test showed that [CON = CP] > [SPW = SPH]. The bleaching of non-vital teeth affected the resin/dentin SBS values when sodium perborate mixed with 30% hydrogen peroxide or water was used, independently of the elapsed time following the bleaching treatment.

This in vitro study aimed to evaluate the effect of bleaching agents on dentin microhardness during and after bleaching.Specimens were randomly assigned to seven groups (n = 15): Nite White Excel 2 Z [NW] 10% and 22%; Rembrandt [REM] 10% and 22%; Opalescence [OPA] 10% and 20%; and a placebo agent. The 42-day whitening treatment consisted of daily application of the agents to the dentin surfaces for 8 hours, followed by immersion in artificial saliva for 16 hours. After the bleaching treatment, specimens were kept immersed in artificial saliva for 14 days. Microhardness was measured at baseline, 8 hours, and 7, 14, 21, 28, 35, and 42 days of bleaching and during the posttreatment period (7 and 14 days).The analysis of variance for split-plot showed a significant effect on the interaction between bleaching agent and time. Tukey's test and regression analyses revealed that during the bleaching period, the agents NW 10%, NW 22%, and OPA 20%, which did not differ from each other, did not alter dentin microhardness, showing constant microhardness values. There were no differences among REM 10%, REM 22%, and OPA 10%, which showed significant reductions in microhardness after day 14 compared to other agents. After bleaching procedures, there was an increase in dentin microhardness for all groups.Throughout the bleaching treatment, depending on the agent applied, dentin showed a transitory decrease in microhardness values. In the posttreatment period, artificial saliva presented a remineralizing effect on the bleached surfaces.

Dental bleaching represents an effective, conservative, and relatively low-cost method for improving the appearance of discolored pulpless teeth. Among the bleaching techniques, the walking bleach technique with sodium perborate associated with water or hydrogen peroxide stands out because of its esthetic results and safety. A modified walking bleach technique with the use of 37% carbamide peroxide as the bleaching agent is presented. Additionally, the adverse effects of dental bleaching in the following restorative procedures are discussed, showing the advantages with the use of 37% carbamide peroxide.

Background Different concentrations of carbamide peroxide (10 to 22 percent) have been used successfully as bleaching agents, but the adverse effects on enamel microhardness at different times are unknown. Methods The authors analyzed seven bleaching agents and a placebo. The agents were applied on the surface of human dental fragments for eight hours per day for 42 days and stored in vials containing artificial saliva. Microhardness measurements were performed at baseline, eight hours and seven, 14, 21, 28, 35 and 42 days, as well as at seven and 14 days after treatment had ended. Results Enamel treated with different bleaching agents or a placebo experienced a similar decrease in microhardness values over time, with the exception of fragments exposed to Opalescence PF 20 percent (Ultradent Products, South Jordan, Utah), or OPA20. Until the 49th day, the enamel exposed to OPA20 exhibited the lowest differences from baseline values. After 14 posttreatment days, enamel treated with placebo, Nite White 10 percent Excel (Discus Dental, Los Angeles), Nite White 16 percent Excel and Opalescence 10 percent exhibited the greatest differences from baseline values. An increase in enamel microhardness occurred at the end of the posttreatment period, although baseline values were not reached. Conclusions Different concentrations of carbamide peroxide agents result in decreases in enamel microhardness. A post-bleaching period in artificial saliva resulted in recovery of baseline microhardness values (one agent) or an increase in values, although baseline values were not reached for these products. Clinical Implications Higher concentrations of carbamide peroxide containing 0.11 percent ion fluoride can bleach teeth in a shorter period, with fewer hazardous effects on the enamel mineral content.

The effect of salivary exposure time on the abrasive wear of acid-eroded dentine was evaluated in situ. One-hundred and twenty bovine root dentine slabs were randomly assigned into six groups (A-F) and placed in intraoral palatal devices, which were worn by 10 volunteers for 4 d. On the first day, no erosive/abrasive procedures were carried out. On the following 3 d, erosive challenges were performed extraorally, two times per day, by immersing the device for 90 s in a soft drink. Subsequently, the group A specimens were immediately brushed (40 strokes), and the others were brushed after the following times: B, 20 min; C, 40 min; and D, 60 min. Group E specimens were only acid-eroded and those of group F were only brushed. Dentine wear was measured with a profilometer. anova and Dunnett's test showed that groups A-D did not differ statistically from the control group E but differed from the control group F. The lowest mean value was found for group F. Regression analysis was unable to show salivary effect on dentine wear reduction. The data suggest that the exposure time of saliva of up to 60 min has no effect on reducing the eroded dentine wear by toothbrushing.

This crossover study aimed to investigate abrasion of previously eroded hard dental tissues by a whitening dentifrice compared to a regular dentifrice. After a 3-day lead-in period, 14 volunteers were randomly assigned to use one of the toothpastes while wearing a removable appliance, containing 3 enamel and 3 root dentine slabs on each side. On the first day salivary pellicle was allowed to form. Twice daily for the following 3 days, one side of each appliance was immersed in an acidic carbonated drink ex vivo while the other side remained unexposed. Specimens were then brushed with the allocated dentifrice. After a 3-day washout period, new sets of enamel and dentine slabs were mounted in the appliances and the participants commenced period 2 using the alternative toothpaste. Acid-treated specimens always showed more wear than untreated specimens. The whitening dentifrice did not significantly increase the wear of softened enamel compared with the regular dentifrice. Brushing with the whitening toothpaste led to significantly greater wear of sound enamel and of both eroded and sound dentine than the regular dentifrice. The results suggest that whitening dentifrices may not increase the wear of acid-softened enamel but may have a more deleterious effect on dentine than regular toothpastes.

To evaluate the in vitro microhardness of enamel treated with a 10% carbamide peroxide agent and two desensitizing dentifrices at different bleaching times.A 10% carbamide peroxide bleaching agent was evaluated (Rembrandt 10%) (REM). A placebo agent was used as a control group (PLA). The bleaching and the placebo agents were applied to human enamel dental fragments for 8 hours per day, followed by immersion for 5 minutes in a slurry solution of desensitizing dentifrices: Sensodyne (S) or Sensodyne Fluor (SF). During the remaining time, the enamel fragments were individually stored in 13.5 ml of artificial saliva. Knoop microhardness measurements were performed at baseline, 8 hours, 7, 14, 21, 28, 35 and 42 days of treatment and at 7 and 14 days of a post-treatment period.Analysis of variance and Tukey's test showed no differences in enamel microhardness for REM + SF (P=0.069) and PLA + SF (P=0.93) within each time interval. The dental fragments treated with REM + S and PLA + S showed an increase in microhardness values within each time interval (P0.0001). There were significant differences among the treatment agents from the 28th to the 56th day. The use of 10% carbamide peroxide bleaching and a desensitizing dentifrice significantly increased the enamel microhardness values during the bleaching treatment and after 14 days after the completion of the treatment. After the post-treatment period, the enamel fragments treated with a placebo agent and with a 10% carbamide peroxide agent and with a desensitizing fluoride dentifrice maintained the baseline values.

Given the increased aesthetic demands of patients, along with improvements in the formulation of resin composites, the ability of these materials to bond to tooth structures, and concerns about dental amalgam fillings, the applicability of resin composites in dentistry has become increasingly widespread. As resistance to wear represents an important factor in determining the clinical success of resin composite restoratives, the aim of this article was to define what constitutes wear; the major underlying phenomena involved in this process-adhesion, abrasion, fatigue, and corrosion-being described. Discussions were also focused on factors that contribute both to the magnitude and minimization of resin composite wear. Finally, insights were included on both in vivo and laboratory studies used to determine wear resistance.

Since the use of bovine instead of human dentine to evaluate cariogenic and anticariogenic substances is not well established, this in situ study was conducted. Eleven volunteers wore palatal acrylic devices containing 4 dentine slabs (2 human and 2 bovine). Sucrose solution (20%) was dripped over all slabs 4 times a day, simulating a cariogenic challenge. Dentifrice slurries, fluoridated or not, were dripped over specified dentine slabs 3 times a day to evaluate caries reduction. After 14 days, the biofilm formed on the dentine slabs was collected for microbiological analysis. In dentine, mineral loss (ΔZ) and lesion depth (LD) were determined by cross-sectional microhardness and by polarized light microscopy, respectively. The total streptococci and mutans streptococci counts in the biofilm formed either on human or on bovine slabs, whether treated or not with fluoride dentifrice, were not statistically different. The ΔZ and the LD values of dentine treated with fluoride dentifrice were significantly lower than the values of dentine treated with non-fluoride dentifrice. The differences in the ΔZ and LD values between the human and bovine dentine were not statistically significant. The results suggest that bovine dentine can be used instead of human to evaluate caries development and inhibition.
