Parenting that is warm and supportive has been consistently linked to better emotion regulation in children, but less is known about this association in adolescents. Adolescence is thought to be an important period for emotion regulation development given that it coincides with the emergence of mental health issues. Respiratory sinus arrhythmia (RSA) is a measure of parasympathetic regulation linked to emotion and behavior regulation. Despite the well-documented links between parenting practices and emotion regulation, and between RSA and emotion regulation, few studies have focused on the association between positive parenting and adolescent RSA or included both mothers and fathers. The current study analyzed the influence of warm parenting throughout adolescence (ages 10-16) on basal RSA at age 17 in 229 Mexican-origin youths. Latent-growth curve models were used to analyze associations between maternal and paternal warmth and baseline RSA. Changes in maternal, but not paternal, warmth from age 10 to 16 were related to youths' basal RSA at age 17. Specifically, youths who perceived increasing (or less decreasing) maternal warmth across adolescence had higher basal RSA. This finding suggests that positive maternal parenting experiences during adolescence "get under the skin" to enhance parasympathetic functioning that supports youths' emotion regulation capacities.

Clinicians form initial impressions about a childs diagnosis based on behavioral features, but research has not yet identified specific behaviors to guide initial diagnostic impressions. Participants were toddlers (N = 55, mean age 22.9 months) from a multi-site early detection study, referred for concern for ASD due to screening or parent/provider concern. Within 5 min of meeting a child, clinicians noted ASD or non-ASD impression, confidence in impression, and behaviors that informed their impression. These clinicians also determined final diagnoses for each child. When a childs final diagnosis was ASD (n = 35), senior clinicians formed an initial impression of ASD in 22 cases (63%) but missed 13 cases (37%). When final diagnosis was non-ASD (n = 20), senior clinicians made an initial impression of non-ASD in all cases (100%). Results were similar among junior clinicians. Senior and junior clinicians used the same behaviors to form accurate impressions of ASD and non-ASD: social reciprocity, nonverbal communication, and eye contact. Senior clinicians additionally used focus of attention when forming accurate impressions of ASD and non-ASD; junior clinicians used this behavior only when forming accurate non-ASD impressions. Clinicians initial impressions of ASD are very likely to be consistent with final diagnoses, but initial impressions of non-ASD need follow-up. Toddlers who show all four atypical behaviors (social reciprocity, nonverbal communication, eye contact, and focus of attention) might receive expedited ASD diagnoses. However, presence of apparently typical behaviors should not rule out ASD; for some children a longer evaluation is necessary to allow for more opportunities to observe subtle social behavior.

Problems with memory, executive function, and language are a significant public health concern, especially when they begin during midlife. However, there is relatively little work on risk and protective factors for cognitive function in middle adulthood. Using data from 883 Mexican-origin adults assessed up to 6 times across 12 years (Mage at Time 1 = 38.2 years; range = 27-63 years), the present study examined whether developmental trajectories (levels and slopes) of Big Five personality domains and socioeconomic factors (per capita income, economic stress) were prospectively associated with cognitive function (memory, mental status, verbal fluency) at the final assessment. We found that individuals with higher levels of, and smaller decreases in, Neuroticism had worse cognitive function 12 years later. Further, individuals with higher initial Conscientiousness had better subsequent memory, mental status, and verbal fluency, and individuals with higher Openness and Extraversion had better subsequent verbal fluency (but not memory or mental status). The trajectories of per capita income and economic stress were robustly associated with cognitive function, such that higher initial levels and greater increases in socioeconomic resources had protective associations, whereas higher levels and greater increases in economic stress had deleterious associations with cognitive function. Higher education level was associated with better cognitive function 12 years later. These findings suggest that changes in personality and socioeconomic factors across adulthood are associated with cognitive function, which may be informative for interventions to support healthier cognitive aging starting at least as early as midlife. Lay abstractThis study examined a widely used autism screening tool, the Modified Checklist for Autism in Toddlers, Revised, with Follow-Up to identify differences in screening for autism between toddler males and females. Examining sex differences in screening for autism in toddlerhood is important as it determines who will be referred for evaluations and receive diagnoses, which is critical for access to autism-specific early intervention. This study found that females were less likely to screen positive and be invited for evaluations compared with males. Females at high likelihood for autism were less likely to be diagnosed with autism, which decreases confidence in the screener's results. Importantly, the Modified Checklist for Autism in Toddlers, Revised, with Follow-Up accurately identified both males and females with autism. Future research should examine ways to improve accuracy in screening results for females.

BACKGROUND: The Global Typhoid Genomics Consortium was established to bring together the typhoid research community to aggregate and analyse Salmonella enterica serovar Typhi (Typhi) genomic data to inform public health action. This analysis, which marks 22 years since the publication of the first Typhi genome, represents the largest Typhi genome sequence collection to date (n=13,000). METHODS: This is a meta-analysis of global genotype and antimicrobial resistance (AMR) determinants extracted from previously sequenced genome data and analysed using consistent methods implemented in open analysis platforms GenoTyphi and Pathogenwatch. RESULTS: Compared with previous global snapshots, the data highlight that genotype 4.3.1 (H58) has not spread beyond Asia and Eastern/Southern Africa; in other regions, distinct genotypes dominate and have independently evolved AMR. Data gaps remain in many parts of the world, and we show the potential of travel-associated sequences to provide informal sentinel surveillance for such locations. The data indicate that ciprofloxacin non-susceptibility (>1 resistance determinant) is widespread across geographies and genotypes, with high-level ciprofloxacin resistance (≥3 determinants) reaching 20% prevalence in South Asia. Extensively drug-resistant (XDR) typhoid has become dominant in Pakistan (70% in 2020) but has not yet become established elsewhere. Ceftriaxone resistance has emerged in eight non-XDR genotypes, including a ciprofloxacin-resistant lineage (4.3.1.2.1) in India. Azithromycin resistance mutations were detected at low prevalence in South Asia, including in two common ciprofloxacin-resistant genotypes. CONCLUSIONS: The consortiums aim is to encourage continued data sharing and collaboration to monitor the emergence and global spread of AMR Typhi, and to inform decision-making around the introduction of typhoid conjugate vaccines (TCVs) and other prevention and control strategies. The current study examined the Five Cs model of positive youth development (PYD; Lerner et al., 2005) in U.S. Mexican-origin youth (N = 674, 50% female) and tested the extent to which ethnic pride, familismo, and respeto, as an index of cultural orientation, predicted PYD across midadolescence. PYD was modeled using a bifactor structure, which defined global PYD and the Five Cs (Caring, Character, Competence, Confidence, and Connection) using theoretically similar measures matched to the conceptual definitions of the Cs. Tests of longitudinal invariance of the bifactor model at ages 14 and 16 established scalar invariance, providing support for the structure and stability of the Five Cs and global PYD using the theoretically similar measures across time. Adolescents cultural orientation (latent factor incorporating familismo, respeto, and ethnic pride) at age 14 was positively associated with the Five Cs within and across time. Greater cultural orientation at age 14 predicted increased global PYD across ages 14 and 16. The contribution of cultural orientation to the PYD across midadolescence did not differ by adolescent gender or nativity. These findings demonstrate the robust nature and stability of the Five Cs model of PYD and provide novel evidence that ethnic pride, familismo, and respeto promote greater PYD in Mexican-origin youth during midadolescence. Reducing the age of first autism diagnosis facilitates access to critical early intervention services. A current "waitlist crisis" for autism diagnostic evaluation thus demands that we consider novel use of available clinical resources. Previous work has found that expert autism clinicians can identify autism in young children with high specificity after only a brief observation; rapid identification by non-experts remains untested. In the current study, 252 children ages 12-53 months presented for a comprehensive autism diagnostic evaluation. We found that junior clinicians in training to become autism specialists (n = 29) accurately determined whether or not a young child would be diagnosed with autism in the first five minutes of the clinic visit in 75% of cases. Specificity of brief observations was high (0.92), suggesting that brief observations may be an effective tool for triaging young children toward autism-specific interventions. In contrast, the lower negative predictive value (0.71) of brief observations, suggest that they should not be used to rule out autism. When trainees expressed more confidence in their initial impression, their impression was more likely to match the final diagnosis. These findings add to a body of literature showing that clinical observations of suspected autism should be taken seriously, but lack of clinician concern should not be used to rule out autism or overrule other indicators of likely autism, such as parent concern or a positive screening result.

This paper used cross-lagged panel models to test the longitudinal interplay between maternal cultural socialization, peer ethnic-racial discrimination, and ethnic-racial pride across 5th to 11th grade among Mexican American youth (N = 674, Mage  = 10.86; 72% born in the United States; 50% girls; Wave 1 collected 2006-2008). Maternal cultural socialization predicted increases in subsequent youth ethnic-racial pride, and youth ethnic-racial pride prompted greater maternal cultural socialization. However, peer ethnic-racial discrimination was associated with subsequent decreases in ethnic-racial pride. The magnitude of these associations was consistent across 5th to 11th grades suggesting that maternal cultural socialization messages are necessary to maintain ethnic-racial pride across adolescence, thus families must continually support the development of ethnic-racial pride in their youth to counter the effects of discrimination.

There is increasing evidence supporting the effectiveness of the Early Start Denver Model (ESDM) for children on the autism spectrum. However, substantial variability in response to the ESDM has been reported across participants. We examined the plausible yet untested hypothesis that variations in the fidelity level of therapists delivering the intervention contribute to variability in childrens response to the ESDM. Videotaped sessions (n = 40) of toddlers on the autism spectrum who received the ESDM from trained therapists were coded to obtain measures of therapist fidelity and childrens learning in response to the therapists instruction. Variations in overall fidelity, along with variations in most items included in the ESDM fidelity checklist, contributed to the childrens learning response during the sessions.

Pediatric brain tumors are the leading cause of cancer-related death in children in the United States and contribute a disproportionate number of potential years of life lost compared to adult cancers. Moreover, survivors frequently suffer long-term side effects, including secondary cancers. The Children's Brain Tumor Network (CBTN) is a multi-institutional international clinical research consortium created to advance therapeutic development through the collection and rapid distribution of biospecimens and data via open-science research platforms for real-time access and use by the global research community. The CBTN's 32 member institutions utilize a shared regulatory governance architecture at the Children's Hospital of Philadelphia to accelerate and maximize the use of biospecimens and data. As of August 2022, CBTN has enrolled over 4700 subjects, over 1500 parents, and collected over 65,000 biospecimen aliquots for research. Additionally, over 80 preclinical models have been developed from collected tumors. Multi-omic data for over 1000 tumors and germline material are currently available with data generation for > 5000 samples underway. To our knowledge, CBTN provides the largest open-access pediatric brain tumor multi-omic dataset annotated with longitudinal clinical and outcome data, imaging, associated biospecimens, child-parent genomic pedigrees, and in vivo and in vitro preclinical models. Empowered by NIH-supported platforms such as the Kids First Data Resource and the Childhood Cancer Data Initiative, the CBTN continues to expand the resources needed for scientists to accelerate translational impact for improved outcomes and quality of life for children with brain and spinal cord tumors.

IntroductionPrevious studies examined the trajectory of self-esteem during critical developmental periods and over the life-span. However, little is known about how self-esteem changes during the school-to-work transition.MethodWe examined the effect of beginning a job for the first time on self-esteem development, using data from 368 adolescents assessed up to six times across a 14-year time span. Specifically, we analyzed the pattern of self-esteem change during the transition to work and whether the self-esteem trajectory varied as a function of several school- and job-related variables, while controlling for important covariates.ResultsResults revealed linear increases in self-esteem across the 14-year study period, with partial support that the rate of increase slowed slightly after the school-to-work transition. We found significantly greater variability in the slopes after the transition, supporting the idea that people differ in the way they cope with the developmental tasks associated with important life transitions. We also found evidence for an interaction between college graduation and educational expectations, such that the positive effect of college graduation on self-esteem change was stronger for those who graduated with low (vs. high) educational expectations.ConclusionSchool-to-work transition has an effect on self-esteem development. Developmental processes of findings were discussed.

This paper tested whether shift-&-persist coping, or coping involving the combination of cognitive reappraisal, acceptance, and optimism (Chen & Miller, Perspectives on Psychological Science, 2012, 7, 135), attenuates the risks presented by economic hardship and ethnic discrimination for change in depressive symptoms from 9th to 12th grade, in a sample of 674 Mexican American youth (Mage W1 = 10.86; 50% female; 72% US born) and whether this effect depends on ethnic pride. Structural equation modeling indicated that, when accounting for economic hardship, shift-&-persist was associated with fewer concurrent depression symptoms. Youth with lower ethnic pride who endorsed high levels of shift-&-persist were protected against the negative impacts of peer ethnic discrimination on depressive symptoms. Future research on ethnic discrimination should examine patterns of coping and identity that can mitigate risk.

Black and Latino adult cancer patients are underrepresented in cancer clinical trials, which limits generalizability of findings and amplifies disparities in healthcare access and outcomes. Community-level education programs designed to address barriers to participation could improve representation in cancer clinical trials. Through a community-campus partner framework, this study evaluated the Women United: Clinical Trials and the Fight Against Breast Cancer Program in Spanish and English. Participants were 422 women (141 Black, 140 Latina Spanish preference, 141 Latina English preference) who were randomized to view either the intervention (n = 215) or a control (n = 207) program. Assessments of clinical trials knowledge and barriers to clinical trials participation were taken before and after viewing. Results suggested that clinical trials knowledge increased and perceived barriers to participation decreased for those who viewed the educational program. More specifically, those in the intervention condition perceived fewer barriers related to personal benefits, mistrust, and familiarity of clinical trials. As expected, there were no differences in perceived barriers related to community support for either condition. Participants in both conditions were equally likely to join a subsequent study or a clinical trials community ambassador program. There were no differences in any of the outcomes across ethnicity or language, suggesting the program works equivalently across groups. This program is easy to administer and can be recommended for use among Black and Latina women to address factors related to clinical trials participation.

The experience of poverty embodies complex, multidimensional stressors that may adversely affect physiological and psychological domains of functioning. Compounded by racial/ethnic discrimination, the financial aspect of family poverty typically coincides with additional social and physical environmental risks such as pollution exposure, housing burden, elevated neighborhood unemployment, and lower neighborhood education levels. In this study, we investigated the associations of multidimensional social disadvantage throughout adolescence with autonomic nervous system (ANS) functioning at 17 years. Two hundred and twenty nine low-income Mexican-American adolescents (48.6% female) and their parents were assessed annually between the ages of 10 and 16. Participants' census tracts were matched with corresponding annual administrative data of neighborhood housing burden, education, unemployment, drinking water quality, and fine particulate matter. We combined measures of adolescents' electrodermal response and respiratory sinuses arrhythmia at rest and during a social exclusion challenge (Cyberball) to use as ANS indices of sympathetic and parasympathetic activity, respectively. Controlling for family income-to-needs, youth exposed to greater cumulative water and air pollution from ages 10-16 displayed altered patterns of autonomic functioning at rest and during the social challenge. Conversely, youth living in areas with higher housing burden displayed healthy patterns of autonomic functioning. Altogether, results suggest that toxin exposure in youths' physical environments disrupts the ANS, representing a plausible mechanism by which pollutants and social disadvantage influence later physical and mental health.

BackgroundSubstance use (SU) typically increases from middle to late adolescence. Anxiety is one factor associated with greater SU, although variability in who uses substances remains. Some models suggest that brain-based susceptibility markers could reveal which adolescents are at a higher risk for psychopathology, but it is unknown whether these individual differences attenuate or accentuate the association between anxiety and elevated SU even if normative. This study addressed this gap by testing whether neural response to social exclusion moderates the association between anxiety symptoms and increased SU from middle to late adolescence.MethodsParticipants were 181 Mexican-origin adolescents (48% female; 16-17 years old) who completed a social exclusion task during a functional magnetic resonance imaging scan and filled out questionnaires about their SU and anxiety symptoms. Analyses focused on neural response to social exclusion versus inclusion within 3 regions of interest and change in SU across 2 years.ResultsDorsal anterior cingulate cortex response to social exclusion, but not subgenual anterior cingulate cortex or anterior insula, moderated the relation between anxiety symptoms and SU, such that higher anxiety symptoms predicted a greater relative increase in SU only for those youth with a lower dorsal anterior cingulate cortex response to exclusion.ConclusionsBlunted dorsal anterior cingulate cortex response to social exclusion may serve as a neural susceptibility marker of altered conflict monitoring or emotion regulation in middle adolescence that, in combination with high levels of anxious feelings, elevates the risk for onset of and/or increased SU by late adolescence. These findings have implications for designing targeted interventions to mitigate SU among adolescents.

A large body of research has documented how personality develops across adulthood, yet very little longitudinal work has examined whether these findings generalize beyond predominantly middle-class, highly educated White American or Western European individuals. This preregistered study uses longitudinal data from 1,110 Mexican-origin adults who completed a well-validated personality measure, the Big Five Inventory, up to six times across 12 years. Individuals generally maintained their rank ordering on the Big Five over time (rs = .66-.80), and the relative ordering of the Big Five within persons was also highly stable (rs = .58-.66). All of the Big Five traits showed small, linear mean-level decreases across adulthood. These trajectories showed few associations with sociodemographic factors (sex, education level, and IQ) and cultural factors (generational status, age at immigration, Spanish/English language preference, Mexican cultural values, American cultural values, and ethnic discrimination). The statistically significant findings we did observe mostly concerned associations between cultural values and Extraversion, Agreeableness, and Openness. Acquiescence bias was also positively associated with Big Five personality trait scores at every wave. There was no evidence of mean-level change in the Big Five when including time-varying acquiescence scores as covariates in the models. Divergences between the present findings and previous research highlight the need to study personality development with diverse aging samples. Reappraisal (reconstruing emotional experiences to alter their impact) and suppression (inhibiting emotionally expressive behavior) are emotion-regulation strategies with important implications for depression. While reappraisal generally predicts lower depressive symptoms, suppression generally predicts higher depressive symptoms. Because cultural factors can influence the processes involved in these links and because adolescence-especially for ethnic minority youth-brings particular emotional challenges, it's critical to investigate these links among Mexican-origin adolescents. However, research examining emotion regulation among Mexican-origin individuals is scarce and generally limited to cross-sectional designs. Thus, we examined prospective associations between reappraisal and suppression (assessed at age 17) and 2 facets of depressive symptoms (anhedonia and general distress) over 3 years (assessed at ages 16, 18, and 19) among 228 Mexican-origin adolescents. Latent growth curve models indicated that reappraisal was associated with lower anhedonia at baseline (age 16) and lower anhedonia over time, whereas suppression predicted greater anhedonia at baseline but not change over time. Consistent with the Mexican cultural value of simpatía, which emphasizes expressing positive emotions and inhibiting negative emotions, suppression of positive emotions was associated with greater anhedonia over time whereas suppression of negative emotions was associated with lower anhedonia over time. However, neither associated with anhedonia at baseline. Reappraisal and suppression were not associated with distress symptoms, and no effects were moderated by familism, household income, gender, or child nativity. The anhedonia results suggest that the benefits of reappraisal extend to Mexican-origin adolescents, but the effects of suppression may depend upon emotional valence in this group. In this cross-sectional and longitudinal analysis of mapping the T-cell repertoire in kidney transplant recipients, we have investigated and validated T-cell clonality, immune repertoire chronology at rejection, and contemporaneous allograft biopsy quantitative tissue injury, to better understand the pathobiology of acute T-cell fraction, T-cell repertoire and antibody-mediated kidney transplant rejection. To follow the dynamic evolution of T-cell repertoire changes before and after engraftment and during biopsy-confirmed acute rejection, we sequenced 323 peripheral blood samples from 200 unique kidney transplant recipients, with (n=100) and without (n=100) biopsy-confirmed acute rejection. We report that patients who develop acute allograft rejection, have lower (p=0.01) T-cell fraction even before transplantation, followed by its rise after transplantation and at the time of acute rejection accompanied by high TCR repertoire turnover (p=0.004). Acute rejection episodes occurring after the first 6 months post-transplantation, and those with a component of antibody-mediated rejection, had the highest turnover; p=0.0016) of their T-cell repertoire. In conclusion, we validated that detecting repertoire changes in kidney transplantation correlates with post-transplant rejection episodes suggesting that T-cell receptor sequencing may provide recipient pre-transplant and post-transplant predictors of rejection risk.

Krueger et al. (2022) argue that our review (Orth & Robins, 2022) finds benefits of self-esteem primarily for subjective outcomes and largely fails to demonstrate any "objective" benefits. We disagree with this portrayal of the findings and highlight research that provides evidence for the benefits of self-esteem using objective measures. We also address Krueger et al.'s claim that positivity bias in self-reports can account for the effects of self-esteem on subjectively assessed life outcomes, and explain how the statistical analyses used to document these effects substantially control for this bias. We maintain that there is now a large body of evidence from meta-analyses and large-scale longitudinal studies demonstrating that high self-esteem has adaptive consequences for social relationships, school, work, mental health, physical health, and antisocial behavior. Brummelman (2022) presents a compelling theoretical framework that can guide the design of interventions to improve children's self-esteem. We agree with his concerns about the need for randomized controlled trials to evaluate the efficacy of self-esteem interventions and the importance of ensuring that children's self-esteem can be raised without causing them to become narcissistic. The research reviewed in our article indicates that high self-esteem is adaptive for children, adolescents, and adults, suggesting that well-designed and effective self-esteem interventions might be beneficial for individuals of all ages. Debates about the benefits of self-esteem have persisted for decades, both in the scientific literature and in the popular press. Although many researchers and lay people have argued that high self-esteem helps individuals adapt to and succeed in a variety of life domains, there is widespread skepticism about this claim. The present article takes a new look at the voluminous body of research (including several meta-analyses) examining the consequences of self-esteem for several important life domains: relationships, school, work, mental health, physical health, and antisocial behavior. Overall, the findings suggest that self-esteem is beneficial in all these domains, and that these benefits hold across age, gender, and race/ethnicity, and controlling for prior levels of the predicted outcomes and potential third variable confounds. The meta-analytic estimates of self-esteem effects (which average .10 across domains) are comparable in size to estimates for other hypothesized causal factors such as self-efficacy, positive emotionality, attachment security, and growth mindset, and larger than some generally accepted pharmaceutical interventions. Discussion focuses on several issues that are critical for evaluating the findings, including the strength of the evidence for making causal inferences, the magnitude of the effects, the importance of distinguishing between self-esteem and narcissism, and the generalizability of the results. In summary, the present findings support theoretical conceptions of self-esteem as an adaptive trait that has wide-ranging influences on healthy adjustment and adaptation, and suggest that interventions aimed at boosting self-esteem might, if properly designed and implemented, benefit individuals and society as a whole. Suicide among young people is an increasingly prevalent and devastating public health crisis around the world. To reduce the rate of suicide, it is important to identify factors that can help us better predict suicidal ideation and behaviors. Adolescent temperament (effortful control, negative emotionality, positive emotionality) may be a source of risk and resilience for the onset of suicidal ideation, plans, and attempts. The present study uses longitudinal data from a large, community sample of Mexican-origin youth (N = 674), assessed annually from age 12 to 21, to examine how temperament is associated with the onset of suicidal ideation and behaviors during adolescence and young adulthood. Results indicate that higher levels of effortful control (activation control, inhibitory control, attention) are associated with decreased probability of experiencing the onset of suicidal ideation, plans, and attempts, whereas higher levels of negative emotionality (particularly aggression, frustration, and depressed mood) are associated with increased probability of experiencing the onset of suicidal ideation and behaviors. Positive emotionality (surgency, affiliation) was not associated with the onset of suicidal ideation and behaviors. Supplemental analyses showed conceptually similar findings for the Big Five, with Conscientiousness associated with decreased risk, Neuroticism associated with increased risk, and the other three dimensions showing largely null results. The findings did not vary significantly for boys and girls or for youth born in the U.S. versus Mexico. Overall, these findings suggest that adolescent temperament serves as both a protective factor (via effortful control/Conscientiousness) and a risk factor (via negative emotionality/Neuroticism) for suicidal ideation and behaviors in Mexican-origin youth. The English and Spanish versions of the Multidimensional Health Locus of Control scales have not been psychometrically evaluated for use with Hispanic Americans. Hispanic American adults (N = 436) completed the English (n = 210) or Spanish (n = 226) Multidimensional Health Locus of Control scales. A multiple-group confirmatory factor analysis did not support equivalent four-factor structures for Spanish- and English-speaking Hispanic Americans. Follow-up exploratory factor analyses of the 24 items supported an 18-item, four-factor structure for English-speaking Hispanic Americans and a 22-item, three-factor structure for Spanish-speaking Hispanic Americans. These results suggest caution when using the Multidimensional Health Locus of Control scales with Hispanic Americans.

BackgroundAutism spectrum disorder (ASD) affects one in 54 children in the United States of America, and supporting people with ASD across the lifespan presents challenges that impact individuals, families, and communities and can be quite costly. The American Academy of Pediatrics has issued recommendations for routine ASD screening at 18 and 24 months, but some research suggests that few pediatricians perform high-fidelity, standardized screening universally. Furthermore, the United States Preventive Services Task Force (USPSTF) found insufficient evidence to recommend for or against universal ASD screening. The objective of this study is to test the hypothesis that children with ASD who have high fidelity; standardized screening will achieve superior outcomes at 5 years of age compared to children receiving usual care ASD detection strategies.MethodsThis is a cluster-randomized, controlled clinical trial in 3 sites in the USA. Pediatric practices will be randomized to implement universal, standardized, high-fidelity toddler screening or usual care, with randomization stratified by the practice size. The study will enroll 3450 children, approximately half in each group. From this sample, we anticipate 100 children to be diagnosed with ASD. Children in both groups receiving an ASD diagnosis will be administered the Early Start Denver Model, an evidence-based early intervention addressing social, communication, and cognitive functioning. Treatment will last for 1 year, with up to 20 h per week of therapy for children with ASD.ResultsPrimary outcomes measured at baseline, following treatment, and at 4 and 5 years of age include ASD symptom severity (Brief Observation of Social Communication Change (BOSCC)) and cognitive functioning (Mullen Scales of Early Learning (MSEL) and Differential Abilities Scale-II (DAS-II)). Secondary outcomes in children include measures of adaptive functioning, ASD symptoms, and kindergarten readiness; secondary analyses will also examine stress and empowerment among parents. Several novel exploratory measures will be included as well. The study will utilize a modified intention-to-treat analysis.ConclusionsThis trial will evaluate the impact of universal, standardized, high-fidelity screening for ASD among children at 18 months of age, with a goal of providing evidence to support this strategy to detect ASD in toddlers in order to start treatment as young as possible and maximize outcomes.Ethics and disseminationThis study was approved by the Institutional Review Board at Drexel University (IRB protocol: 1607004653). BackgroundNeurophysiological patterns may distinguish which youth are at risk for the well-documented increase in internalizing symptoms during adolescence. Adolescents with internalizing problems exhibit altered resting-state functional connectivity (RSFC) of brain regions involved in socio-affective processing. Whether connectivity-based biotypes differentiate adolescents' levels of internalizing problems remains unknown.MethodSixty-eight adolescents (37 females) reported on their internalizing problems at ages 14, 16, and 18 years. A resting-state functional neuroimaging scan was collected at age 16. Time-series data of 15 internalizing-relevant brain regions were entered into the Subgroup-Group Iterative Multi-Model Estimation program to identify subgroups based on RSFC maps. Associations between internalizing problems and connectivity-based biotypes were tested with regression analyses.ResultsTwo connectivity-based biotypes were found: a Diffusely-connected biotype (N = 46), with long-range fronto-parietal paths, and a Hyper-connected biotype (N = 22), with paths between subcortical and medial frontal areas (e.g. affective and default-mode network regions). Higher levels of past (age 14) internalizing problems predicted a greater likelihood of belonging to the Hyper-connected biotype at age 16. The Hyper-connected biotype showed higher levels of concurrent problems (age 16) and future (age 18) internalizing problems.ConclusionsDifferential patterns of RSFC among socio-affective brain regions were predicted by earlier internalizing problems and predicted future internalizing problems in adolescence. Measuring connectivity-based biotypes in adolescence may offer insight into which youth face an elevated risk for internalizing disorders during this critical developmental period.

The present study attempts to replicate and extend Snyder et al. (2015, JPSP). The original study examined the latent factor structure of the Early Adolescent Temperament Questionnaire-Revised (EATQ-R), a commonly used measure of adolescent temperament, and showed that the resulting latent factors (i.e., effortful control, negative emotionality, and positive emotionality) had theoretically meaningful concurrent associations with several measures of adolescent functioning (depression, anxiety, attention-deficit hyperactivity disorder [ADHD], relational aggression, and school performance and behavior). We performed these same analyses using data from a large sample of Mexican-origin youth (N = 674), and also examined prospective associations between the three EATQ-R factors and measures of adolescent functioning assessed two years later. We found some evidence supporting the bifactor models reported in the original study but poor replication of the correlations among latent factors. Additionally, model comparisons demonstrated that correlated factors models produced more interpretable factors than the bifactor models. In contrast, we replicated most of the concurrent correlations (and extended the findings to prospective associations) between the EATQ-R factors and measures of adolescent functioning, supporting the construct validity of the EATQ-R as a measure of adolescent temperament. Thus, these findings raise concerns about the generalizability of the factor structure identified by Snyder et al. (2015), but bolster claims about the generalizability of the concurrent and predictive validity of the EATQ-R. Overall, differences between the present findings and those of Snyder et al. (2015) highlight the importance of ongoing construct validation in youth temperament research, especially with participants from groups traditionally underrepresented in psychological research. Researchers often study constructs that are conceptually and/or empirically related, but distinct (i.e., "sibling constructs"). In social-personality psychology, as well as psychology more generally, there is little guidance for how to deal with sibling constructs, which can result in researchers ignoring or mishandling them. In this article, we start by situating sibling constructs in the literature on the jingle-jangle fallacies. Then, we outline 10 conceptual and empirical criteria for determining the degree to which, and in what ways, constructs may share a sibling relationship, using self-esteem and grandiose narcissism as a running example. Finally, we discuss strategies for handling sibling constructs in a systematic and transparent way. We hope that the procedures described here will help social-personality psychologists identify sibling constructs, understand when and why they pose problems for their research, and adopt strategies that ameliorate their adverse effects.

Poverty is a chronic stressor associated with disruptions in psychophysiological development during adolescence. This study examined associations of chronic poverty and income changes experienced from pre- to mid-adolescence with hypothalamic-pituitary-adrenal (HPA) axis stress responses in late adolescence. Participants (N = 229) were adolescents of Mexican-origin (48.7% female). Household income (converted to income-to-needs ratios) was assessed annually when children were 10-16 years old. At 17 years, adolescents completed Cyberball, a social exclusion simulation task while undergoing a functional magnetic resonance imaging scan. Saliva samples were collected prior to and five times over a 50-minute period following the scan, from which salivary cortisol was assayed. Results showed that differential trajectories of poverty from ages 10-16 predicted HPA axis activity at age 17. Relative to others, distinct HPA suppression (hyporeactivity) was demonstrated by youth who started adolescence in deep poverty and were still living in poverty at age 16 despite experiencing some income gains. Youth from more economically secure families evinced typical cortisol increases following the lab stressor. These results suggest that subsequent HPA functioning varies as a function of economic status throughout adolescence, and that efforts to increase family income may promote healthy HPA functioning for youths in the most impoverished circumstances.

Diagnosticians report that autism spectrum disorder (ASD) is immediately apparent in some, but not all, children ultimately diagnosed. Clinicians' initial diagnostic impressions have implications for ASD early detection, yet the literature raises questions about their accuracy. This study explores diagnostic impressions of ASD specialists made within the first 5 minutes of meeting a young child and investigates factors associated with the match between initial impressions and final diagnoses. Participants were children (n = 294, aged 12-53 months) referred for an ASD evaluation as part of multi-site ASD screening studies. After 5 minutes observing each child, clinicians with expertise diagnosing ASD recorded if they thought the child would meet criteria for ASD following a complete evaluation, and recorded their confidence in this impression. Clinicians' initial impressions matched the final diagnosis in 81% of cases. Ninety-two percent of cases initially thought to have ASD met criteria following a full evaluation; however, 24% of cases initially thought not to have ASD also met criteria, suggesting a high miss rate. Clinicians were generally confident in their initial impressions, reporting highest confidence for children initially thought correctly not to have ASD. ASD behavioral presentation, but not demographic characteristics or developmental level, were associated with matching initial impression and final diagnosis, and confidence. Brief observations indicating ASD should trigger referral to intervention services, but are likely to under-detect positive cases and should not be used to rule out ASD, highlighting the need to incorporate information beyond initial clinical impression. LAY SUMMARY: When children come in for an autism evaluation, clinicians often form early impressions-before doing any formal testing-about whether the child has autism. We studied how often these early impressions match the final diagnosis, and found that clinicians could not easily rule out autism (many children who initially appeared not to have autism were ultimately diagnosed), but were generally accurate ruling in autism (when a child appeared to have autism within 5 minutes, they were almost always so diagnosed).

Although Latinx parents' perceptions of the climate of their children's schools may play a role in their children's academic adjustment, research examining this idea is sparse. Every 2 years beginning when children were in fifth grade (Mage  = 10.86 years) until they were in 11th grade, Mexican-origin mothers (N = 674) reported on their perceptions of the climate of their children's schools; information on children's academic adjustment was collected from children and mothers. Multilevel modeling indicated that when mothers had more positive school climate perceptions, their children valued school more and performed better in school, but did not necessarily hold higher educational expectations. The findings suggest the importance of schools in creating welcoming environments for Mexican-origin parents.

The association between personality traits and motivational units, such as life goals, has been a long-standing interest of personality scientists. However, little research has investigated the longitudinal associations between traits and life goals beyond young adulthood. In the present study (N = 251), we examined the rank-order stability of, and mean-level changes in, the Big Five and major life goals (Aesthetic, Economic, Family/Relationship, Hedonistic, Political, Religious, Social) from college (age 18) to midlife (age 40), as well as their co-development. Findings showed that personality traits and major life goals were both moderately-to-highly stable over 20 years. On average, there were mean-level increases in the Big Five and mean-level decreases in life goals over time. Patterns of co-development suggest people formulate goals consistent with their personality traits, and conversely, investing in goal-relevant contexts is associated with trait change. We discuss the results in light of Social Investment Theory and the developmental regulation literature.

In virtually all areas of psychology, the question of whether a particular construct has a prospective effect on another is of fundamental importance. For decades, the cross-lagged panel model (CLPM) has been the model of choice for addressing this question. However, CLPMs have recently been critiqued, and numerous alternative models have been proposed. Using the association between low self-esteem and depression as a case study, we examined the behavior of seven competing longitudinal models in 10 samples, each with at least four waves of data and sample sizes ranging from 326 to 8,259. The models were compared in terms of convergence, fit statistics, and consistency of parameter estimates. The traditional CLPM and the random intercepts cross-lagged panel model (RI-CLPM) converged in every sample, whereas the other models frequently failed to converge or did not converge properly. The RI-CLPM exhibited better model fit than the CLPM, whereas the CLPM produced more consistent cross-lagged effects (both across and within samples) than the RI-CLPM. We discuss the models from a conceptual perspective, emphasizing that the models test conceptually distinct psychological and developmental processes, and we address the implications of the empirical findings with regard to model selection. Moreover, we provide practical recommendations for researchers interested in testing prospective associations between constructs and suggest using the CLPM when focused on between-person effects and the RI-CLPM when focused on within-person effects. We examined depression and anxiety symptom trajectories in Mexican-origin youth (N = 674) and tested longitudinal associations with acculturation dimensions. We used eight waves of data from the California Families Project, collected annually from 5th (Mage = 10.86, SD = 0.51) to 12th (Mage = 16.79, SD = 0.50) grade. Major depression disorder (MD) and generalized anxiety disorder (GAD) symptoms were assessed by structured psychiatric interview. Cultural measures, selected based on theory and empirical evidence, included English/Spanish use, familism, traditional gender role (TGR) attitudes, and ethnic pride. Symptom trajectories were modeled using latent growth analyses, and parallel process growth models examined covariation between internalizing and acculturation trajectories. Models adjusted for child sex, nativity, mother's education, and family income. MD symptoms decreased across adolescence on average, with steeper decreases among boys and children born in Mexico. GAD symptoms also decreased on average, with higher mean levels among girls. Age 10 Spanish use, familism, and ethnic pride were inversely related to age 10 MD symptoms. Steeper increases in Spanish use, familism, and ethnic pride predicted decreasing MD. Higher age 10 MD predicted increasing Spanish use and decreasing English use. Greater age 10 TGR attitudes predicted higher age 10 GAD but steeper declines in GAD and MD. Increasing ethnic pride slopes predicted decreasing GAD. Greater childhood TGR attitudes, and the maintenance of Spanish use, familism, and ethnic pride into adolescence, were associated with more optimal trajectories of MD and GAD symptoms. Interventions for Mexican-origin youth internalizing problems should encourage the retention of heritage culture strengths, including familism and ethnic pride.

Despite the importance of life satisfaction for health and well-being, there is a paucity of longitudinal studies tracking changes in life satisfaction in ethnic minority youth. In a sample of 674 Mexican-origin youth, the present research examined life satisfaction trajectories from middle (age 14) to late adolescence (age 17) and from late adolescence to young adulthood (age 21). On average, life satisfaction did not change significantly from age 14 to 17, and then decreased from age 17 to 21 (d = .30), perhaps reflecting difficulties transitioning into adult roles. Drawing on ecological systems theory, we examined both proximal (i.e., family) and distal (i.e., social-contextual) environmental factors (measured via self- and parent-reports) that may account for between-person variation in life satisfaction trajectories. Youth with more positive family environments in middle adolescence (age 14) had higher mean life satisfaction from middle adolescence to young adulthood (age 21). In contrast, youth with more negative family environments and who experienced greater economic hardship and more ethnic discrimination in middle adolescence (age 14) had lower life satisfaction during this period. Many of these factors also predicted change in life satisfaction from middle (age 14) to late adolescence (age 17), but not from late adolescence to young adulthood (age 21). This research extends the current understanding of life satisfaction during a critical developmental period in an understudied population. ObjectiveThis study investigates associations between Big Five personality trait change, organizational socialization, and organizational identification during a 3-year police officer training program (N = 416 police officer cadets).MethodParticipants completed a questionnaire measuring the Big Five personality traits when they entered the training academy, and then, completed the same personality questionnaire, along with measures of organizational socialization and identification, during their 2nd (n = 360) and 3rd (n = 397) year of training.ResultsResults corroborated the hypotheses that (a) the Big Five traits can show systematic changes even across a relatively short time period and (b) this change is functional, given that the latent difference scores of all Big Five traits significantly predicted increases in organizational socialization and identification.ConclusionThe Big five personality traits showed significant mean level changes across the 3-year training program. Although these changes were not fully consistent with theoretical expectations, they did predict two aspects of organizational adjustment (socialization and identification). The theoretical and practical implications of these findings were discussed.

The present study (a) tested whether a structure of common mental disorders within the hierarchical taxonomy of psychopathology was invariant from late childhood to adolescence in a sample of Mexican-origin youth, (b) examined the developmental course of psychopathology at different levels of the hierarchy, and (c) tested the degree to which changes in psychopathology were associated with changes in the Big Five personality domains. Results were consistent with the longitudinal hierarchical invariance of common mental disorders from age 12 to 17 (n = 674). Further, initial levels of conscientiousness, agreeableness, and emotional stability were positively associated with lower initial levels of a higher order factor of psychopathology, and increases in extraversion and decreases in neuroticism were associated with decreases in a higher order factor of psychopathology, which captured the general tendency for externalizing, internalizing, and attention-hyperactivity-related dimensions of psychopathology to correlate. Results of the present study indicate that a hierarchical model of common mental disorders extends to Mexican-origin youth and that developmental change in Big Five personality are related to developmental change in psychopathology. In this study, we examined the effect of family environment on self-esteem development from late childhood (age 10) through adolescence (age 16), using 4-wave longitudinal data from 674 Mexican-origin families living in the United States. To assess family environment, a multi-informant approach was used (i.e., mother, father, and child) to construct latent variables that minimize the influence of response biases. Using cross-lagged panel models (CLPMs) and random intercepts cross-lagged panel models (RI-CLPMs), we tested the prospective effects of parenting behaviors (warmth, hostility, monitoring, and involvement in child's education) and other characteristics of the family environment (quality of parental relationship, positive family values, maternal and paternal depression, economic conditions of the family, and presence of father). In the CLPMs, significant positive effects on children's self-esteem emerged for warmth, monitoring, low maternal depression, economic security (vs. hardship), and presence (vs. absence) of father. With regard to the reciprocal effects, children's self-esteem predicted positive family values (i.e., importance and centrality of the family) of mother and father. In the RI-CLPMs, the pattern of results was similar (in terms of point estimates of the effects); however, only the effects of maternal depression on child self-esteem, and the effect of child self-esteem on family values of father, were statistically significant. In all models, the effects did not differ significantly for boys and girls, or across ages 10 to 16. The findings suggest that multiple features of the family environment shape the development of self-esteem during late childhood and adolescence. The present study investigated the developmental precursors of effortful control, a temperament trait that involves the propensity to regulate one's impulses and behaviors, to motivate the self toward a goal when there are conflicting desires, and to focus and shift attention easily. Data came from the California Families Project, a multimethod longitudinal study of 674 Mexican-origin youth (and their parents), who were assessed at ages 10, 12, 14, 16, and 19. Effortful control (measured via self- and parent-reports) was moderately stable over time (r = .47 from age 10 to 19), and its developmental trajectory followed a u-shaped pattern (decreasing from age 10 to 14, before increasing from age 14 to 19). Findings from latent growth curve models showed that youth who experience more hostility from their parents, associate more with deviant peers, attend more violent schools, live in more violent neighborhoods, and experience more ethnic discrimination tend to exhibit an exacerbated dip in effortful control. In contrast, youth with parents who closely monitor their behavior and whereabouts exhibited a shallower dip in effortful control. Analyses of the facets of effortful control revealed important disparities in their trajectories; specifically inhibitory control showed linear increases, attention control showed linear decreases, and activation control showed the same u-shaped trajectory as overall effortful control. Moreover, most of the precursors of effortful control replicated for inhibitory control and attention control, but not for activation control. To date, there have been no long-term longitudinal studies of continuity and change in narcissism. This study investigated rank-order consistency and mean-level changes in overall narcissism and 3 of its facets (leadership, vanity, and entitlement) over a 23-year period spanning young adulthood (Mage = 18, N = 486) to midlife (Mage = 41, N = 237). We also investigated whether life experiences predicted changes in narcissism from young adulthood to midlife, and whether young adult narcissism predicted life experiences assessed in midlife. Narcissism and its facets showed strong rank-order consistency from age 18 to 41, with latent correlations ranging from .61 to .85. We found mean-level decreases in overall narcissism (d = -0.79) and all 3 facets, namely leadership (d = -0.67), vanity (d = -0.46), and entitlement (d = -0.82). Participants who were in supervisory positions showed smaller decreases in leadership, and participants who experienced more unstable relationships and who were physically healthier showed smaller decreases in vanity from young adulthood to middle age. Analyses of the long-term correlates of narcissism showed that young adults with higher narcissism and leadership levels were more likely to be in supervisory positions in middle age. Young adults with higher vanity levels had fewer children and were more likely to divorce by middle age. Together, the findings suggest that people tend to become less narcissistic from young adulthood to middle age, and the magnitude of this decline is related to the particular career and family pathways a person pursues during this stage of life. ObjectiveTo determine the effect of tirzepatide, a dual agonist of glucose-dependent insulinotropic polypeptide and glucagon-like peptide 1 receptors, on biomarkers of nonalcoholic steatohepatitis (NASH) and fibrosis in patients with type 2 diabetes mellitus (T2DM).Research design and methodsPatients with T2DM received either once weekly tirzepatide (1, 5, 10, or 15 mg), dulaglutide (1.5 mg), or placebo for 26 weeks. Changes from baseline in alanine aminotransferase (ALT), aspartate aminotransferase (AST), keratin-18 (K-18), procollagen III (Pro-C3), and adiponectin were analyzed in a modified intention-to-treat population.ResultsSignificant (P < 0.05) reductions from baseline in ALT (all groups), AST (all groups except tirzepatide 10 mg), K-18 (tirzepatide 5, 10, 15 mg), and Pro-C3 (tirzepatide 15 mg) were observed at 26 weeks. Decreases with tirzepatide were significant compared with placebo for K-18 (10 mg) and Pro-C3 (15 mg) and with dulaglutide for ALT (10, 15 mg). Adiponectin significantly increased from baseline with tirzepatide compared with placebo (10, 15 mg).ConclusionsIn post hoc analyses, higher tirzepatide doses significantly decreased NASH-related biomarkers and increased adiponectin in patients with T2DM.

Many adolescents have difficulty regulating their impulses and become prone to externalizing problems (e.g., attention-deficit/hyperactivity disorder [ADHD], oppositional defiant disorder [ODD], and conduct disorder [CD]) and other adverse consequences. Using multimethod data from a longitudinal study of Mexican-origin youth (N = 674), assessed annually from age 10 to 16, we examined the relations between effortful control and ADHD, ODD, and CD symptoms over time. Bivariate latent growth curve models showed negative correlations between the trajectories of effortful control and ADHD, ODD, and CD, indicating that steeper decreases in effortful control were related to steeper increases in ADHD, ODD, and CD symptoms. Using a novel statistical technique, the factor of curves model (FOCUS), we found that ADHD, ODD, and CD share a common "externalizing" trajectory during adolescence. Although effortful control was strongly associated with this common trajectory, it had few unique associations with the individual disorder trajectories, above and beyond their shared trajectory. When we extended the FOCUS model to include the effortful control trajectory as an indicator, we found that ADHD and ODD had strong loadings, whereas effortful control and CD had comparatively weak loadings on the shared developmental trajectory. Follow-up analyses showed that a two-factor solution, with externalizing symptom trajectories on one factor and the effortful control facet trajectories on a separate factor, was a better fit to the data than a one-factor solution. Finally, parent ASPD symptoms were related to increases in CD, but had no significant influence on effortful control, ADHD, or ODD. BackgroundOfranergene obadenovec (VB-111) is an anticancer viral therapy that demonstrated in a phase II study a survival benefit for patients with recurrent glioblastoma (rGBM) who were primed with VB-111 monotherapy that was continued after progression with concomitant bevacizumab.MethodsThis pivotal phase III randomized, controlled trial compared the efficacy and safety of upfront combination of VB-111 and bevacizumab versus bevacizumab monotherapy. Patients were randomized 1:1 to receive VB-111 1013 viral particles every 8 weeks in combination with bevacizumab 10 mg/kg every 2 weeks (combination arm) or bevacizumab monotherapy (control arm). The primary endpoint was overall survival (OS), and secondary endpoints were objective response rate (ORR) by Response Assessment in Neuro-Oncology (RANO) criteria and progression-free survival (PFS).ResultsEnrolled were 256 patients at 57 sites. Median exposure to VB-111 was 4 months. The study did not meet its primary or secondary goals. Median OS was 6.8 versus 7.9 months in the combination versus control arm (hazard ratio, 1.20; 95% CI: 0.91-1.59; P = 0.19) and ORR was 27.3% versus 21.9% (P = 0.26). A higher rate of grades 3-5 adverse events was reported in the combination arm (67% vs 40%), mainly attributed to a higher rate of CNS and flu-like/fever events. Trends for improved survival with combination treatment were seen in the subgroup of patients with smaller tumors and in patients who had a posttreatment febrile reaction.ConclusionsIn this study, upfront concomitant administration of VB-111 and bevacizumab failed to improve outcomes in rGBM. ObjectiveAltered activity within reward-related neural regions, including the ventral striatum (VS) and medial prefrontal cortex (mPFC), is associated with concurrent problematic substance use. The aims of the present study were (a) to identify patterns of reward-related neural activity that prospectively predicted changes in alcohol use 2 years after magnetic resonance imaging in a sample of adolescents, and (b) to examine whether these patterns differed by sex. We also tested whether depression symptoms or impulsivity mediated associations between neural activity and future alcohol use.MethodParticipants were 262 adolescents (129 male and 133 female) of Mexican origin who completed the Monetary Incentive Delay task during a functional magnetic resonance imaging scan at age 16. Participants reported on their alcohol use at ages 16 and 18.ResultsResults indicated that different patterns of reward-related neural activity predicted future increases in alcohol use for male and female adolescents. In boys, higher VS activity during reward anticipation and average ventral mPFC activity during reward feedback predicted increases in alcohol use from age 16 to 18 years; in girls, higher dorsal mPFC activity and blunted VS activity during reward anticipation predicted increases in alcohol use from age 16 to 18 years. Depression symptoms or impulsivity did not mediate these associations.ConclusionThe results suggest that different pathways of risk may lead to problematic alcohol use for adolescent boys and girls. These sex differences in neural risk pathways have important implications for prevention and intervention approaches targeting Mexican-origin youth.

PurposeSleep disturbance and cancer-related fatigue (CRF) are among the most commonly reported symptoms associated with breast cancer and its treatment. This study identified symptom cluster groups of breast cancer patients based on multidimensional assessment of sleep disturbance and CRF prior to and during chemotherapy.MethodsParticipants were 152 women with stage I-IIIA breast cancer. Data were collected before chemotherapy (T1) and during the final week of the fourth chemotherapy cycle (T2). Latent profile analysis was used to derive groups of patients at each timepoint who scored similarly on percent of the day/night asleep per actigraphy, the Pittsburgh Sleep Quality Index global score, and the five subscales of the Multidimensional Fatigue Symptom Inventory-Short Form. Bivariate logistic regression evaluated if sociodemographic/medical characteristics at T1 were associated with group membership at each timepoint.ResultsThree groups (Fatigued with sleep complaints, Average, Minimal symptoms) were identified at T1, and five groups (Severely fatigued with poor sleep, Emotionally fatigued with average sleep, Physically fatigued with average sleep, Average, Minimal symptoms) at T2. The majority of individuals in a group characterized by more severe symptoms at T1 were also in a more severe symptom group at T2. Sociodemographic/medical variables at T1 were significantly associated with group membership at T1 and T2.ConclusionsThis study identified groups of breast cancer patients with differentially severe sleep disturbance and CRF symptom profiles prior to and during chemotherapy. Identifying groups with different symptom management needs and distinguishing groups by baseline sociodemographic/medical variables can identify patients at risk for greater symptom burden.

ObjectiveImprovement in health-related quality of life (HRQoL) is a public health goal of Healthy People 2020. Hispanics living in the United States are at risk for poor HRQoL, but the causes and correlates of this risk are not well understood. Thus, the present study examined individual-level psychosocial and neighborhood-level built environment correlates of physical and mental HRQoL among Hispanic adults.MethodA community sample of Hispanic adults (N = 383) completed self-report health-related questionnaires, and census tract was used to collect data on neighborhood-level built environment variables. Multilevel modeling was used to examine individual-level psychosocial (language preference, religiosity, subjective social status, discrimination, and number of years lived in the United States) and neighborhood-level built-environment (the retail food environment, proximity to alcohol retailers, and tobacco retailer density) correlates of physical and mental HRQoL.ResultsHigher subjective social status was significantly associated with better HRQoL, and more experiences with discrimination were significantly associated with lower HRQoL. For physical HRQoL, these relationships were stronger in neighborhoods with a higher density of tobacco retail outlets.ConclusionsFindings from this study suggest that subjective social status and discrimination play important roles in HRQoL among Hispanics, in particular in neighborhoods with a higher density of tobacco retail outlets. This study highlights the importance of considering neighborhood context, and in particular neighborhood disadvantage, when examining the relationship between social status, discrimination and HRQoL among Hispanics. The contribution that parental educational expectations for youth and youth's perceptions of academic competence can have on youth's own educational expectations across early to late adolescence is not well-understood. In a sample of Mexican-origin families, the current study examined longitudinal (from early to late adolescence) associations among mothers, fathers, and youth's educational expectations, how youth's educational expectations were associated with perceived academic competence, and the potential mediating role of youth's perceived academic competence. Data from two-parent families which included one focal child (7th grade: N= 469; youth: Mage = 12.31, 50% female) at three waves (7th, 9th, and 11th grade) were utilized. Structural equation modeling and multi-group analysis were implemented to assess the study's goals. Results revealed significant associations among parents' 7th grade educational expectations and youth's 9th and 11th grade educational expectations. The findings also revealed three significant associations among youth's perceived academic competence and educational expectations between 7th and 11th grade. Specifically, youth's 7th grade perceived academic competence predicted youth's 9th grade educational expectations, youth's 7th grade educational expectations predicted youth's 9th grade perceived academic competence, and youth's 9th grade perceived academic competence predicted youth's 11th grade educational expectations. Multigroup analysis did not reveal gender differences for the associations tested. The findings highlight the long-term significance of parents' educational expectations on youth's educational expectations and underscore youth's academic competence, an individual level factor, as critical to consider for understanding educational expectations across adolescence for Mexican-origin youth.

Sexual debut, or first intercourse, predicts problem behaviors such as substance use. This association could reflect a direct effect of debut itself, general developmental trends, or the fact that some youth are more predisposed to a wide array of problem behaviors (e.g., risky sex, substance use). Understanding the association between sexual debut and substance use thus requires methods that can distinguish between these various accounts. In this study the association between sexual debut and substance use was investigated in a longitudinal sample of Mexican-origin youth (N = 674) assessed annually from 5th (Mage = 10.86 years, SD = 0.51) through 12th grade (Mage = 17.69 years, SD = 0.48). The longitudinal aspect of the data allowed the direct effect of sexual debut on substance use to be tested while accounting for long-term trends in substance use, and stable individual differences in those trends based on early risk and debut timing. Substance use increased over time, and early risk and debut were consistently associated with more substance use. Sexual debut also modestly predicted an increase in substance use after accounting for these effects, however. Taken together, results provide some evidence consistent with each of the potential explanations for the association between sexual debut and substance use across adolescence.

The current study examined the trajectory of gender role attitudes of 471 Mexican-origin adolescents (236 girls, 235 boys) from 5th grade (Mage = 10.86 years) to 11th grade (Mage = 16.75 years), investigating how situating identities (i.e., gender, nativity, SES), ethnic identity (i.e., ethnic pride), and familial context (i.e., parents' attitudes) contributed to adolescents' gender role attitudes across time. Participant interviews were conducted every other year, resulting in 4 waves of data. Most parents (96%) were Mexico natives, with an average immigration age of 18.16 years for fathers and 14.01 years for mothers. Results revealed linear and quadratic trends in gender attitude traditionality for all adolescents, characterized by a linear decline through age 16 years that leveled off through age 18 years. Although both girls and boys trended toward egalitarian gender role attitudes across adolescence, girls endorsed more egalitarian attitudes than did boys. Adolescents from higher-SES backgrounds endorsed more egalitarian attitudes than those from lower-SES backgrounds. Significant within-person effects of ethnic pride surfaced, such that children with higher levels of ethnic pride at any given time also reported more traditional gender role attitudes. Significant between-person effects of mothers' and fathers' attitudes were found, such that parents with more traditional gender role attitudes tended to have children with relatively more traditional gender role attitudes. Overall, these findings highlight the importance of studying gender development in conjunction with situating identities, cultural identities, and the broader context, particularly when children are embedded in multiple cultures with contrasting gender role expectations. Effortful control refers to the propensity to regulate one's impulses and behaviors, to focus and shift attention easily, and to motivate the self toward a goal when there are competing desires. Although it seems likely that these capacities are relevant to successful functioning in the school context, there has been surprisingly little longitudinal research examining whether youth with poor effortful control are more likely to act out in the classroom, get suspended, and skip school. Conversely, there is even less research on whether youth who exhibit these school behavioral problems are more likely to decline over time in effortful control. We used multimethod data from a longitudinal study of Mexican-origin youth (N = 674), assessed biennially from 5th to 11th grade, to examine the codevelopment of effortful control and school behavioral problems. Bivariate latent growth curve models revealed a negative association between the trajectories of effortful control and school behavioral problems, indicating that steeper decreases in effortful control were related to steeper increases in school behavioral problems. Furthermore, this codevelopmental pattern was bidirectional; cross-lagged regression analyses showed that low effortful control was associated with relative increases in school behavioral problems, and school behavioral problems were associated with relative decreases in effortful control. Gender, nativity status, Mexican cultural values, and school-level antisocial behavior had concurrent associations with effortful control and school behavioral problems, but they did not moderate the codevelopmental pathways. We discuss the theoretical implications for personality development, as well as the practical implications for reducing school behavioral problems during adolescence. Exposure to threat increases the risk for internalizing problems in adolescence. Deficits in integrating bodily cues into representations of emotion are thought to contribute to internalizing problems. Given the role of the medial prefrontal cortex in regulating bodily responses and integrating them into representations of emotional states, coordination between activity in the medial prefrontal cortex and autonomic nervous system responses may be influenced by past threat exposure with consequences for the emergence of internalizing problems. A sample of 179 Mexican-origin adolescents (88 female) reported on neighborhood and school crime, peer victimization, and discrimination when they were 10-16 years old. At age 17, participants underwent a functional neuroimaging scan during which they viewed pictures of emotional faces while respiratory sinus arrhythmia (RSA) and skin conductance responses were measured. Adolescents also reported symptoms of internalizing problems. Greater exposure to threats across adolescence was associated with more internalizing problems. Threat exposure was also associated with stronger negative coupling between the ventromedial prefrontal cortex and RSA. Stronger negative ventromedial prefrontal cortex-RSA coupling was associated with fewer internalizing problems. These results suggest the degree of coordinated activity between the brain and parasympathetic nervous system is both enhanced by threat experiences and decreased in adolescents with more internalizing problems.

Decisions about when to start or switch a therapy often depend on the frequency with which individuals are monitored or tested. For example, the optimal time to switch antiretroviral therapy depends on the frequency with which HIV-positive individuals have HIV RNA measured. This paper describes an approach to use observational data for the comparison of joint monitoring and treatment strategies and applies the method to a clinically relevant question in HIV research: when can monitoring frequency be decreased and when should individuals switch from a first-line treatment regimen to a new regimen? We outline the target trial that would compare the dynamic strategies of interest and then describe how to emulate it using data from HIV-positive individuals included in the HIV-CAUSAL Collaboration and the Centers for AIDS Research Network of Integrated Clinical Systems. When, as in our example, few individuals follow the dynamic strategies of interest over long periods of follow-up, we describe how to leverage an additional assumption: no direct effect of monitoring on the outcome of interest. We compare our results with and without the "no direct effect" assumption. We found little differences on survival and AIDS-free survival between strategies where monitoring frequency was decreased at a CD4 threshold of 350 cells/μl compared with 500 cells/μl and where treatment was switched at an HIV-RNA threshold of 1000 copies/ml compared with 200 copies/ml. The "no direct effect" assumption resulted in efficiency improvements for the risk difference estimates ranging from an 7- to 53-fold increase in the effective sample size.
