Objectives Prescription opioid overdoses are a leading cause of death in the United States. Emergency departments ( EDs) are potentially high-risk environments for doctor shopping and diversion. The hypothesis was that opioid prescribing rates from the ED have increased over time. Methods The authors analyzed data on ED discharges from the 2006 through 2010 NHAMCS, a probability sample of all U.S. EDs. The outcome was documentation of an opioid prescription on discharge. The primary independent predictor was time. Covariates included severity of pain, a pain-related discharge diagnosis, age, sex, race, payer, hospital ownership, and geographic location of hospital. Up to three discharge diagnoses were available in NHAMCS to identify 'pain-related' (e.g., back pain, fracture, dental/jaw pain, nephrolithiasis) ED visits. Multivariate logistic regression was performed to assess the independent associations between opioid prescribing and predictors. All analyses incorporated NHAMCS survey weights, and all results are presented as national estimates. Results Opioids were prescribed for 18.7% (95% confidence interval = 17.7% to 19.7%) of all ED discharges, representing 18.8 million prescriptions per year. There were no significant temporal trends in opioid prescribing overall (adjusted p = 0.93). Pain-related discharge diagnoses that received the top three highest proportion of opioids prescriptions included nephrolithiasis (62.1%), neck pain (51.6%), and dental/jaw pain (49.7%). A pain-related discharge diagnosis, non-Hispanic white race, older age, male sex, uninsured status, and Western region were positively associated with opioid prescribing (p < 0.05). Conclusions No temporal trend toward increased prescribing from 2006 to 2012 was found. Objectives Atrial fibrillation and flutter ( AFF) are the most common arrhythmias seen in the outpatient setting, and they affect more than 300,000 adult Canadians. The aims of this study were to examine temporal and geographic trends in emergency department ( ED) presentations made by adults (age ≥ 35 years) for AFF in Alberta, Canada, from 1999 to 2011. Statistical disease cluster detection techniques were used to identify geographic areas with higher numbers of individuals presenting with AFF and higher numbers of ED presentations for AFF than expected by chance alone. Geographic clusters of individuals with stroke or heart failure follow-up within 365 days of ED presentations for AFF were also identified. Methods All ED presentations for AFF made by individuals aged ≥35 years were extracted from Alberta's Ambulatory Care Classification System. The Alberta Health Care Insurance Plan provided population counts and demographics for the patients presenting (age, sex, year, geographic unit). The Physician Claims File provided non- ED physician claims data after a patient's ED presentation. Statistical analyses included numerical and graphical summaries, directly standardized rates, and statistical disease cluster detection tests. Results During 12 years, there were 63,395 ED presentations for AFF made by 32,101 individuals. Standardized rates remained relatively stable over time, at about two per 1,000 for individuals presenting to the ED for AFF and about three per 1,000 for ED presentations for AFF. The northern and southeastern parts of the province were identified as clusters of individuals presenting for AFF, and ED presentations for AFF, and several of the areas demonstrated clusters in multiple years. Further, several of the geographic clusters were also identified as potential clusters for stroke or heart failure within 365 days after the ED presentations for AFF. Conclusions This population-based study spanned 12 fiscal years and showed variations in the number of people presenting to EDs for AFF and the number of ED presentations for AFF over geography. The potential clusters identified may represent geographic areas with higher disease severity or a lower availability of non- ED health services. The clusters are not all likely to have occurred by chance, and further investigation and intervention could occur to reduce ED presentations for AFF. [ABSTRACT FROM AUTHOR]

Objectives The objectives were to describe the frequency of repeat enrollment within a specific exception from informed consent trial testing benzodiazepine treatment of prehospital status epilepticus and to estimate the effect of repeat enrollments on the analysis of the primary outcome. Methods This was a secondary analysis of data collected as part of the Rapid Anticonvulsant Medication Prior to Arrival Trial ( RAMPART), a study comparing intramuscular midazolam to intravenous lorazepam given by paramedics to patients with prehospital status epilepticus. Subjects in RAMPART achieved a successful primary outcome if they had cessation of seizures by the time of emergency department arrival. Data were collected on all subjects, but only the first enrollment for each individual was used in the primary analysis. The patterns of repeat enrollment are described, along with the demographics of these subjects. In addition, an intraclass correlation coefficient ( ICC) was estimated to assess the amount of within-subject correlation and its effect on the estimated treatment effect when all enrollments are included in the analysis. Results A total of 1,023 enrollments occurred in RAMPART among 893 unique individuals (range of repeat enrollment observed = two to 14). The ICC for seizure cessation within individual was low at 0.119; when excluding subjects with benzodiazepine crossover, the ICC was 0.094. Conclusions In clinical trials of emergency conditions with interval complete resolution, accounting for repeat enrollments is feasible. The RAMPART experience demonstrated that in this setting the within-subject correlation is low and can be accounted for at relatively low statistical cost. [ABSTRACT FROM AUTHOR]

Introduction: Oral anticoagulation (OAC) can reduce stroke and mortality risk in patients with atrial fibrillation (AF); however, clinical outcomes associated with OAC prescription after a US emergency department (ED) discharge are unknown. Objective: To determine the impact of early OAC prescribing on ischemic stroke and major bleeding after ED evaluation for AF. Methods: This retrospective study included Medicare, fee-for-service patients (age ≥65yrs) discharged from the ED in 2011-2012 with newly diagnosed actionable AF— high-stroke risk (by CHA2DS2-VASc) with low-bleeding risk (by HAS-BLED), and no OAC prescription filled 90 days prior. Patients were stratified as receipt of an early OAC prescription (by any provider) within 10 days of ED diagnosis and no early OAC. Ischemic strokes were identified via ICD-9 codes from inpatient claims. Major bleeding was identified using prior validated algorithms by ICD-9 diagnosis with requirement of admission. A blanking period of 10 days was used to exclude patients who were in the process of having an ischemic stroke at the index visit. Time to ischemic stroke and major bleeding in patients with actionable AF were compared using Kaplan Meier curves and a Cox proportional hazards model with propensity score weights and a correction for clustering on facility. Results: Of 3,983 with new actionable AF, 25.4% were prescribed an early OAC, who were similar in mean age (77.4 vs. 78.2 yrs) and co-morbidities (mean Charlson score 2.1 vs. 2.3) to no early OAC group, but less likely to be female (69.4% vs. 76.3%) (p<0.001). Patients were followed for a median of 330 days with a maximum of 640 days. Among patients with actionable AF, there were no differences in hazards of ischemic stroke [HR 0.67(0.33-1.40)] nor major bleed [HR 1.45 (0.91-2.30)] (p>0.05) between the early vs. no OAC groups. However, KM curves suggest a trend towards fewer events among patients with early OACs, especially early in follow-up after ED visit. Conclusion: In patients with a new ED diagnosis of actionable AF, early OAC did not statistically affect clinical outcomes. However, there may be a clinically meaningful difference among those prescribed. Large and prospective studies are needed to elucidate the impact of early OAC prescribing on clinical outcomes.
