Objective: to describe the perception of people with arterial hypertension of the aspects that facilitate and hinder treatment adherence. Method: descriptive qualitative study developed with 16 people met in Nursing consultations. Interviews were conducted with a semi-structured guide and the content analysis technique was used. Data were collected between January and August 2017. Results: having willpower, family and multidisciplinary support, knowledge about the pathology and forms of prevention and fear of death facilitated adherence; on the other hand, little knowledge, laziness, lack of urban infrastructure and climatic conditions, habit of consuming unhealthy foods, alcoholic beverages and tobacco, cost of treatment and forgetfulness of taking the medication hindered adherence. Conclusion: the treatment of hypertension causes changes in the dynamics of life, being essential to have willpower, family and professional support, in addition to overcoming unhealthy habits.

Introdução e objetivo: Hipertensão arterial e diabetes mellitus como problemas de saúde pública se evidenciam pela alta prevalência, impacto socioeconômico e dificuldades na adesão ao tratamento. O objetivo desta pesquisa foi investigar a adesão ao tratamento farmacológico e não farmacológico de usuários do Sistema Único de Saúde, internados devido a complicações decorrentes do insucesso no controle destas doenças. Materiais e métodos: É um estudo descritivo, de abordagem quantitativa. Utilizou-se a entrevista estruturada como instrumento junto a 11 pessoas. Resultados: Destas, 10 faziam uso de medicamentos em domicílio, 4 relataram esquecer de toma-los. Referente ao tratamento não farmacológico, 3 seguiam dieta com restrições alimentares, 10 se consideravam ativos fisicamente e 1 praticava exercícios físicos regularmente. Discussão e conclusão: Os resultados indicam a necessidade de se olhar o usuário em sua totalidade, identificando fatores que o levam a não adesão aos tratamentos disponíveis e fomentando estratégias para melhorar sua adesão aos mesmos. Adhesion level to medicinal treatment by adults diagnosed with hypertension is identified by a descriptive and transversal study involving 213 people diagnosed with hypertension and participating in arterial hypertension campaign in a town in the hinterland of the state of São Paulo, Brazil. Data were collected by half-structured interviews and by Morisky & Green questionnaire. Most participants (84%) failed to adhere to medicinal treatment, with females complying most with adhesion. Adhesion impairments comprised difficulties in changes in lifestyle, irregular visits to the doctor´s and medicine timetable. Adhesion to medicine treatment is a complex and multifactor process that needs special attention by the multiprofessional health team to increase adhesion rates and life quality of patients. Aim: to analyze the scientific literature on the main causes of adherence or not to the treatment of hypertension and type 2 diabetes mellitus. Method: an integrative literature review, which selected full articles published between 2005 and 2015, detected by the descriptors “medication adherence”, “patient compliance”,” type 2 diabetes mellitus”, “hypertension” and “nursing”. The searches were conducted in the Virtual Health Library (Nursing), LILACS and PubMed databases. Results: Twenty-six articles were selected, the factors associated with adherence were: young age, to have a healthy lifestyle and items related to therapy. On the other hand, among the factors that lead to poor adherence, there are lower income, little knowledge about the disease and complex treatment. Conclusions: the adherence factors to the treatment of hypertension and diabetes are related to the patient, to the type of treatment and health system, which constitute important issues to be approached by nurses

Objetivo: Avaliar as evidências disponíveis na literatura sobre os fatores relacionados à não adesão ao tratamento da hipertensão arterial sistêmica no período de 2010 a 2014. Método: Realizou-se uma revisão integrativa nas bases de dados MEDLINE, LILACS e BDENF, conduzida pela seguinte questão norteadora: qual a produção de conhecimento sobre os fatores associados a não adesão de pacientes à terapia anti-hipertensiva? Os descritores empregados na busca pelos estudos foram: hipertensão, cooperação do paciente e adesão à medicação. Após aplicação dos critérios de inclusão foram selecionados 10 estudos para análise e síntese. Resultados: Identificaram-se como principais fatores relacionados a não adesão ao tratamento o baixo nível de escolaridade, o esquecimento dos medicamentos, o baixo nível de conhecimento sobre a doença e regime terapêutico. Conclusão: Inúmeros fatores implicam no fenômeno da não adesão, assim, sugere-se o aprofundamento da temática e o uso de estratégias para aumentar a adesão ao tratamento.

Objetivo Analisar a prevalência e os fatores associados a cooperação do paciente portador de hipertensão arterial na atenção primária. Métodos Estudo transversal que incluiu com 458 hipertensos selecionados aleatoriamente entre os usuários na assistência primária. As variáveis de estudo foram socioeconômicas, demográficas e clínicas e os instrumentos de pesquisa utilizados eram validados. Foi utilizado o modelo de regressão logística para análise estatística multivariada. Resultados As prevalências encontradas para cooperação do paciente foram 26,6%, 16,6% e 85,6% para os testes de Morisky-Green, Batalla e Haynes-Sackett, respectivamente. Análise de regressão logística ajustada mostrou classe econômica e fumo como os preditores independentes para a cooperação do paciente portador de hipertensão arterial. Conclusão A prevalência de adesão ao tratamento variou de acordo com o instrumento de avaliação utilizado. Os fatores associados à adesão ao tratamento foram: classe socioeconômica elevada, morar com companheiro e ser eutrófico.

Objetivo: Descrever os fatores relacionados à adesão ao tratamento do usuário hipertenso. Métodos: Estudo transversal descritivo, com abordagem quantitativa, desenvolvido em Fortaleza-CE, Brasil. A amostra foi constituída por 400 usuários hipertensos. A coleta de dados ocorreu entre os meses de julho e outubro de 2010, por meio de entrevista. Foi utilizada a estatística descritiva e os dados representados em quadros. Resultados: A maioria dos entrevistados consistia em mulheres (269 - 67,2%), com faixa etária entre 40 e 79 anos (390 - 97,5%). De acordo com os entrevistados, destacaram-se como condições favoráveis à adesão ao tratamento: em relação ao próprio usuário - 280 (70%) usuários indicaram o aporte de recursos financeiros para aquisição de alimentos e medicamentos, e 172 (43%) afirmaram o investimento no autocuidado; à família - 98 (24,5%) usuários declararam a ajuda financeira para aquisição da medicação anti-hipertensiva, 78 (19,5%) citaram o encorajamento na adesão e 78 (19,5%) desejaram a redução da responsabilidade financeira com a família; à equipe de saúde - 189 (49,5%) usuários indicaram liberdade para verbalização, 168 (42%) citaram atendimento resolutivo e humano e 115 (28,7%) solicitaram escuta ativa; à instituição de saúde - 242 (60,5%) usuários optaram pela redução do tempo na sala de espera para consulta e realização de exames. Objetivo:Validar o conteúdo das definições operacionais do construto “não adesão ao tratamento da hipertensão arterial”. Método: Estudo metodológico de validação de conteúdo. Foi realizada uma revisão integrativa que demonstrou quatro dimensões da não adesão: pessoa, doença/tratamento, serviço de saúde e ambiente. Objetivo: O estudo teve por objetivo avaliar os índices de adesão às reuniões do grupo de hipertensos e diabéticos por meio do Sistema de Cadastramento e Acompanhamento de Hipertensos e Diabéticos (HIPERDIA), bem como o uso efetivo das pela retirada dos medicamentos da Unidade de Saúde da Família. Métodos: Durante o período de outubro de 2013 a março de 2014 foi realizado um estudo longitudinal e prospectivo, compreendendo uma coorte de pacientes de ambos os sexos, residentes na área de abrangência da Unidade de Saúde da Família São Francisco de Assis cadastrados no HIPERDIA. Os pacientes foram acompanhados durante as reuniões mensais nas comunidades, nas quais eram avaliadas as taxas de participação efetiva e a utilização das medicações por meio da checagem da saída mensal da medicação em estoque, vinculada ao paciente. Resultados: Dos 238 pacientes cadastrados no programa HIPERDIA, 226 deles (94%) aderiram ao tratamento, e 12 pacientes (6%) não utilizavam a medicação prescrita pelo médico da Unidade de Saúde. Apenas 34 pacientes (14,3%) participaram das reuniões e retiravam a medicação nos encontros do HIPERDIA. Outros 192 pacientes (80,7%) não participavam das reuniões e retiravam as medicações na Unidade de Saúde em algum momento durante o mês. Doze deles (5%) não participaram das reuniões nem retiraram a medicação junto à Unidade de Saúde. Conclusão: A adesão ao tratamento do diabetes e da hipertensão arterial sistêmica foi considerada satisfatória, apesar do baixo índice de participação dos grupos nas reuniões do HIPERDIA.

A proposta foi investigar os aspectos comportamentais referentes ao seguimento da terapêutica farmacológica e não farmacológica e o grau de adesão ao tratamento anti-hipertensivo de um grupo específico. Estudo observacional descritivo com análise quantitativa, realizado no Centro de Referência da Assistência Social em Fortaleza-CE, de agosto/2008 a maio/2009. Dados obtidos de 49 indivíduos por entrevista e exame físico. Mais de 50% dos participantes seguiam as terapêuticas não farmacológicas. Prevaleceram a terapia combinada (53%) e as classes medicamentosas de diuréticos (72%) e inibidores da enzima conversora de angiotensina (55%). Muitos participantes (49%) referiram reações adversas. Destas, as mais citadas foram poliúria e tontura (29%). Quanto ao grau de adesão, a média correspondeu ao conceito não adesão leve. A avaliação dos comportamentos de seguimento terapêutico e a caracterização clínico-epidemiológica são necessárias para o enfermeiro planejar estratégias educativas. Ambas possibilitam ajustes no planejamento das intervenções, contribuindo para a melhor adesão terapêutica dos indivíduos.El propósito ha sido investigar los aspectos de comportamiento en el seguimiento de la terapia farmacológica y no-farmacológica y el grado de adhesión del paciente al tratamiento anti-hipertensión en un grupo específico. Estudio observacional descriptivo, mediante análisis cuantitativo, desarrollado en el Centro de Referencia de Asistencia Social, en Fortaleza, Ceará, de agosto/2008 hasta mayo/2009. Los datos han sido recogidos por entrevista y examen físico en 49 individuos. Más de 50% de los participantes siguió el tratamiento no-farmacológico. La terapia de combinación prevaleció (53%) y las clases más expresivas de los medicamentos han sido los diuréticos (72%) y los inhibidores de la enzima de conversión de angiotensina (55%). Casi la mitad (49%) reportaron reacciones adversas, sobretodo poliuria y mareos (29%). El promedio del grado de adhesión al tratamiento correspondió a bajo. La evaluación de las características clínico-epidemiológica y la conducta sobre el seguimiento de la terapia son necesarias para validar las acciones educativas desarrolladas por enfermeras, lo que permite ajustes en la planificación de intervenciones y contribuye para una mejor adhesión al tratamiento.The purpose was to investigate the behavioral aspects on the follow-up of pharmacologic therapy and no-pharmacologic and the patient's compliance degree to treatment of Hypertension in a specific group. Observational descriptive study by quantitative analysis accomplished on the Reference Center of Social Assistant, Ceará, from August 2008 to May 2009. Data collected by the interview and physical examination, on 49 individuals. More than 50% of participants followed the non-pharmacological treatment, except the physical activity. It was prevailed the combination therapy (53%), the use of diuretics (72%) and angiotensin converting enzyme inhibitors (55%). Almost half (49%) reported adverse reactions, predominantly polyuria and dizziness (29%). The mean of patient's compliance degree was corresponding to light noncompliance. The evaluation of the behavior about therapeutic follow-up and clinical-epidemiological characteristics are necessary to validate educational actions developed by nurses, allowing adjustments to plan of interventions, contributing to a better compliance to the treatment.

The greatest challenge posed by Systemic Hypertension (SH) is related to patients' compliance with treatment. Thus, this study aimed to determine attendance of these patients to medical appointments and the percentage of adherence to medication and non-medication regimens, and also identify the main reasons hypertensive patients report for non-adherence. This is a descriptive study with 68 hypertensive patients (64.71% women with average age of 63.9 years) at a teaching outpatient clinic. The instruments used for data collection were: multiprofesional team care report form, the Morisky-Green test and telephone interview. The results show that 61.76% attended the medical consultations, 86.76% did not comply with the medication regimen and 85.29% did not comply with the non-medication regimen, reporting at least one non-healthy life habit. The emotional factor was the most reported (69.12%) among patients' reasons for non-adherence to treatment. The study can support interventions in care delivery to patients with systemic hypertension, with a view to improving their level of adherence and quality of life.El mayor desafío de la hipertensión arterial sistémica (HAS) es la adhesión de los pacientes a su tratamiento, así, este estudio tuvo como objetivos determinar la frecuencia a las consultas y el porcentaje de adhesión al tratamiento medicamentoso y no medicamentoso, además de identificar los principales motivos referidos por los pacientes hipertensos para la no adhesión. Se trata de estudio descriptivo, realizado con 68 hipertensos en un ambulatorio escuela, con 64,71% de mujeres (promedio de edad de 63,9 años). Los instrumentos utilizados para recolección de datos fueron: formulario de atención del equipo multiprofesional, la prueba de Morisky-Green y la búsqueda a través del teléfono. Los resultados: muestran que 61,76% eran asiduos a las consultas, 86,76% no presentaron adhesión al tratamiento medicamentoso y 85,29% al tratamiento no medicamentoso, refiriendo, por lo menos, un hábito de vida no saludable. Entre los motivos para la no adhesión, el factor emocional fue el más relatado (69,12%). This study was developed to identify determinant factors of non-adherence in the treatment of hypertensive individuals in a Brazilian municipality. This cross-sectional and descriptive study with a quantitative approach was conducted from November 2012 to April 2013. A total of 138 hypertensive patients participated, 65.9% were female, aged 60.5 years old (SD=11.32) on average, 71% had incomplete primary/middle school and an individual income of 1 to 2 times the minimum wage. In regard to occupation, 39.9% were retired and 21.7% were homemakers. Among the factors contributing to nonadherence to treatment, 70.3% reported personal reasons. Adherence was at 15.9% for nonpharmacological treatment and 21% for pharmacological treatment. We verified the need for new studies within the primary health care sphere conducted with hypertensive patients and also the need for multidisciplinary actions intended to encourage treatment adherence among hypertensive patients cared for in the municipality under study. O presente estudo avaliou as características sociodemográficas e de adesão terapêutica de 27 portadores de hipertensão arterial em tratamento ambulatorial que apresentaram crises de urgências ou emergências hipertensivas e haviam sido atendidos em uma unidade de leito-dia e em uma unidade de emergência da cidade de Fortaleza-Ceará, no período de outubro de 2002 a maio de 2003. A maioria era mulher, com idade de 50 a 60 anos, pouca escolaridade, tempo de tratamento inferior a cinco anos e tempo de diagnóstico entre cinco e dez anos. O uso dos remédios foi o tratamento mais referido, seguido pela redução do consumo de sal e comparecimento às consultas. No entanto, o fato de comparecerem às consultas e receberem orientação parece não modificar o comportamento, uma vez que a maioria dos entrevistados não praticava exercícios físicos e demonstrava deficiência no conhecimento sobre a doença, atribuindo a elevação da pressão arterial a fatores emocionais.El presente estudio evaluó las características sociodemográficas y de adhesión terapéutica de 27 portadores de hipertensión arterial en tratamiento ambulatorio que presentaron crisis de urgencias o emergencias hipertensivas y habían sido atendidos en una unidad de internamiento diurno y en una unidad de emergencia de la ciudad de Fortaleza - Ceará, en el período de octubre del 2002 a mayo del 2003. La mayoría fue del sexo femenino, con edad de 50 a 60 años, poca escolaridad, tiempo de tratamiento inferior a cinco años y tiempo de diagnóstico entre cinco y diez años. El uso de los remedios fue el tratamiento más referido, seguido por la reducción del consumo de sal y asistencia a las consultas. Entre tanto, el hecho de asistir a las consultas y recibir orientación parece no modificar su comportamiento, pues la mayoría de los entrevistados no praticaba ejercicios físicos y demostraba falta de conocimiento sobre la enfermedad, atribuyendo la elevación de la presión arterial a factores emocionales.This study assessed the socio-demographic characteristics and the characteristics of therapeutic adhesion of 27 bearers of arterial hypertension undergoing ambulatorial treatment who had hypertensive urgencies crises or emergencies in the city of Fortaleza in the period between October of 2002 and May of 2003. The majority were women, between 50 and 60-years old, with little formal education, treatment time shorter than 5 years and time of diagnosis varying from 5 to 10 years. The use of medicine was the treatment that was most mentioned, followed by the reduction of the consumption of salt and attendance to medical appointments. However, attending the appointments and receiving orientation did not seem to change their behavior, since most of the patients that were interviewed practiced no physical exercises and demonstrated little knowledge of the illness, for they attributed the rise of the arterial pressure to emotional factors.

Objectives: to evaluate factors associated with adherence to drug therapy in hypertensive patients in a Primary Care Unit. Methods: a descriptive, exploratory study, applying the assessment of adherence and Self-reported Medication Adherence Measure in 54 patients of the Adult Health Program. For categorical variables, the chi-square test or Fisher exact test were used; for the numeric, the Mann-Whitney or Kruskal- Wallis test were used; and for correlation the correlation coefficient of Spearman was used. Results: patients had a mean age of 61.4 (± 12.5) years, 51.8% women and 48.1% men, mean schooling was 4.3 (± 3.5) years, 59% were employed, 35.1% performed blood pressure control only for medical consultation and/or nursing, 61.1% were non-adherent to medication treatment despite showing high favourability behavior accession. Men and those who did not follow dietary recommendations were more careless when taking medications; those who worked out of their home forgot to take the medication more often; smokers and those who frequently ingest alcohol were behaviors less favorable to adhesion. Conclusions: factors associated with nonadherence were gender and healthy habits. Thus, the major challenge for health professionals is to implement measures that actually change the behavior of adhesion. The development of an intervention based on reflection, respect for the autonomy and individuality of the patient may help to improve adherence. [ABSTRACT FROM AUTHOR]

The greatest challenge posed by Systemic Hypertension (SH) is related to patients' compliance with treatment. Thus, this study aimed to determine attendance of these patients to medical appointments and the percentage of adherence to medication and non-medication regimens, and also identify the main reasons hypertensive patients report for non-adherence. This is a descriptive study with 68 hypertensive patients (64.71% women with average age of 63.9 years) at a teaching outpatient clinic. The instruments used for data collection were: multiprofesional team care report form, the Morisky-Green test and telephone interview. The results show that 61.76% attended the medical consultations, 86.76% did not comply with the medication regimen and 85.29% did not comply with the non-medication regimen, reporting at least one non-healthy life habit. The emotional factor was the most reported (69.12%) among patients' reasons for non-adherence to treatment. The study can support interventions in care delivery to patients with systemic hypertension, with a view to improving their level of adherence and quality of life. [ABSTRACT FROM AUTHOR]
