Malignant nodular hidradenoma (MNH) is a rare invasive tumour arising from eccrine sweat glands, with significant risk for local recurrence and distant metastases. Hidradenoma papilliferum is reported in pregnancy. To our knowledge this is the first MNH in a human immunodeficiency virus (HIV) infected pregnant woman. The patient presented with a painless perianal growth. Wide local excision was performed and the wound left to heal by secondary intension. The pathology report confirmed MNH. The patient has been followed for 14 months without evidence of recurrence. MNH should be considered in the differential diagnosis of similar lesions in HIV infected patients. Treatment for such HIV infected patients with MNH may be similar to that in immune competent patients; they may benefit from a longer follow-up period.Keywords: Malignant Nodular Hidradenoma, HIV infection, Pregnancy

An 8-year-old girl presented with a scalp swelling. The swelling was recurrent, reappearing everytime after local excision. She underwent surgery and the histopathologic diagnosis was malignant hidradenoma. This very rare and aggressive tumor is known to occur in elderly population and is histopathologically distinct from its commonly occuring benign counterpart. Malignant hidradenoma is resistant to chemotherapy and radiotherapy. We empahsize that being cognizant of the possibility of this rare tumor would assist in timely action in the form of wide resection, with possible reduction in morbidity and mortality.

We report a case of malignant nodular hidradenoma in a middle-aged man, who presented with a nodular swelling in the eyelid. The tumor was similar to its benign counterpart but had additional features such as surface ulceration, numerous mitiotic figures, and an infiltrative growth pattern. Malignant forms of hidradenomas are unusual and the possibility this variant should be considered in the differential diagnosis of eyelid tumors.
