We studied the changes in the heart rate variability of lame and nonlame dairy cows in response to transrectal examination (TRE) and parlor milking. We hypothesized that lame cows experience greater stress, manifested in heart rate variability parameters, that serves as an argument to promote more caution in the everyday handling of lame animals. In the study, we selected 55 lame (with lesions on at least one hoof, otherwise clinically healthy, with locomotion scores 4 and 5 of 5-point scores) and 55 nonlame (clinically healthy, with locomotion scores 1 and 2 of 5-point scores) cows. The heart rate, root mean square of successive beat-to-beat intervals (RMSSD), the normalized unit of the high-frequency (HF) component of the spectral analysis and Poincaré measures (SD2/SD1) were compared between lame and nonlame cows during 5 distinct stages of TRE and 7 stages of parlor milking. Heart rate, RMSSD, and SD2/SD1 showed similar patterns during TRE and milking, while HF remained fairly constant during the studied phases. No distinct RMSSD, HF, or SD2/SD1 changes were observed during the phases expected to elicit a stress response. Between-group differences were only observed in terms of HF. Baseline HF was higher in lame cows than in nonlame ones, and such a difference in direction was observed throughout the whole procedure. During milking and TRE, the heart rate, RMSSD, and SD2/SD1 values showed temporal changes in times of higher physical activity: moving to and waiting in the holding pen and moving into the milking stalls in the parlor for preparation in both lame and nonlame cows. The differences in baseline HF (normalized units) between lame and nonlame cows cannot be fully explained based on available information. The lack of a distinct, stress-related change in heart rate variability parameters in both groups can originate in methodological challenges inherent in animal heart rate variability measurements in field conditions. Background: We evaluated the influence of different partial carbon dioxide pressure (PaCO2) levels on organ perfusion in patients with respiratory failure receiving pressure-support ventilation with veno-venous extracorporeal membrane oxygenation (V-V ECMO). Methods: In this twelve patients prospective study, ECMO gas-flow was decreased from baseline (PaCO2 < 40 mmHg) until PaCO2 increased by 5–10 mmHg (High-CO2 phase). Resistance indices of gut, spleen, and snuffbox artery, the peripheral perfusion index (PPI), and heart rate variability were measured at baseline and High-CO2 phase. Results: When PaCO2 increased from 36 (36–37) mmHg at baseline to 42 (41–43) mmHg in the High-CO2 phase (p < 0.001), PPI decreased significantly (p = 0.026). The snuffbox artery (p = 0.022), superior mesenteric artery (p = 0.042), and spleen (p = 0.012) resistance indices increased significantly. The root mean square of successive differences (RMSSD) decreased from 19.5(18.1–22.7) to 15.9(14.4–18.6) ms (p = 0.034), and the ratio of low-frequency to high-frequency components(LF/HF) increased from 0.47 ± 0.23 to 0.70 ± 0.38 (p = 0.013). Conclusions: High PaCO2 might cause decreased peripheral tissue and visceral organ perfusion through autonomic nervous system in patients with respiratory failure undergoing PSV with V-V ECMO. Vagally‐mediated heart rate variability (vmHRV) is a physiological index reflecting parasympathetic activity that has been linked to emotion regulation (ER) capacity. However, very limited research has examined associations of physiological indices of regulation such as vmHRV with emotional functioning in daily life. The few studies that exist have small samples sizes and typically focus on only a narrow aspect of ER or emotional functioning. In this study, we examined associations between vmHRV assessed in the laboratory and emotional/mental health functioning in daily life using a 7‐day ecological momentary assessment design in 303 adult community participants. We hypothesized that higher resting vmHRV would be associated with higher positive affect (PA), lower negative affect (NA), less affective variability, greater well‐being, fewer dysphoria symptoms, greater use of engagement ER strategies, and less use of avoidance ER strategies, as assessed in daily life. Results revealed that higher resting vmHRV in the laboratory (as indexed by both high frequency heart rate variability, HF‐HRV, and the root mean of successive square deviations between heart beats, RMSSD) was significantly associated with less frequent use of avoidance ER strategies in daily life. Theoretical and clinical implications are discussed, including the association of vmHRV with negatively valenced, rather than positively valenced, daily life experiences. [ABSTRACT FROM AUTHOR]

Introduction: Huntington's disease (HD) is known as a neurodegenerative disease with movement disorder and cognitive impairment; autonomic involvement is also becoming common in some recent studies. The aim of this study is to demonstrate the presence of cardiac autonomic involvement in HD patients. Method: Time and frequency domain parameters obtained from the 24-h Holter ECG(hECG) were compared between 20 HD patients and 20 healthy control subjects. Results: Fourteen HD patients had tachycardia, bradycardia, and extra beats. Interval between two heartbeats, normal-to-normal (NN), standard deviation of all normal-to-normal (SDNN), square root of the mean of the sum of the squares of the differences between consecutive N-N intervals in ms (rMSSD), and the ratio of the number of consecutive pairs of N-N intervals that differ by more than 50 ms to the total number of N-N intervals (pNN50) were all significantly higher in the patient group than in the control group during 24-h hECG monitoring. However, hECG monitoring showed that the patient group had significantly higher values of the frequency-domain metrics high frequency (HF) than the control group did (P = 0.003). Very low frequency (VLF) was lower in the patient group (P = 0.009). There was no difference in low frequency (LF) in both groups. In comparison to the control group, LF/HF was much reduced in the patient group (P = 0.001). Conclusion: Cardiac disfunction increases, and autonomic functions change in HD, but more comprehensive studies are needed to distinguish sympathetic and parasympathetic involvement. Besides health monitoring, a regular check of dairy heifers' growth rate is desirable, but it is rarely done because procedures that require restraint and handling can be associated with substantial stress for both animals and farmers. Inexperienced heifers, especially if they are highly responsive to humans, may find restraint and handling potentially aversive. This study investigated whether training heifers of different age and responsiveness toward humans (RTH), through operant conditioning, could reduce stress in animals, ease close contact and handling, and be feasible in terms of farmers' effort. We assessed 60 Holstein heifers of 2 age classes (young, n = 29, 291 ± 39 d; old, n = 31, 346 ± 62 d) according to the avoidance distance test and classified them as confident (n = 20), neutral (n = 21), or nonconfident (n = 19). Half of the heifers of each age and RTH class were trained (n = 29), whereas the other half was not (n = 31). The trained heifers were subjected to target training for 8 sessions and positively reinforced with feed to allow being touched on the muzzle, rump, and perineum. If a heifer refused positive reinforcement, the trainer stepped back as negative reinforcement. In the last week of the experiment, the effect of training on the reaction to handling was assessed in all heifers. We measured heart rate, root mean square of successive interbeat interval differences (RMSSD), and fecal cortisol metabolites (FCMet). The presence of behavioral distress signs was recorded as well. The avoidance distance test was performed a second time 24 h after the measuring session. All of the trained heifers, regardless of RTH class, successfully accomplished the target training task in 6 sessions, each spending on average 25.3 s per session. All of the trained heifers allowed touches on the rump and perineum at the end of the fourth session. Training nonconfident heifers required more time compared with the others. Trained heifers showed higher RMSSD than nontrained heifers (14.2 vs. 16.9 ms, respectively), indicating a lower vagal tone, and thus, a slightly lower stress level than nontrained heifers. Training did not lead to differences in HR, FCMet, or presence of stress behavioral signs. Nonconfident heifers had the highest mean baseline FCMet values compared with neutral and confident heifers (38.4 vs. 30.3 vs. 29.1 ng/g, respectively). Nonconfident heifers also showed the lowest value of FCMet 12 h after the measuring session (36.7 vs. 44.6 vs. 49.7 ng/g), likely due to a decreased responsiveness of the adrenal gland to a stressor. The average avoidance distance decreased between the beginning and the end of the experiment, especially for neutral and nonconfident heifers, regardless of whether they were trained or not. These results show how using operant conditioning on some heifers not only decreased their vagal tone, but also reduced the responsiveness to humans of all the animals, trained and not trained; in the latter case, this reduction was through nonassociative learning, such as habituation. Background: Short-term (5 min) heart rate variability (HRV) analysis is widely used in assessing autonomic nervous system activity during exercise. While shortening the HRV measurement duration can help improve its application efficiency, its accuracy needs to be verified. This study investigated the agreement between ultra-short-term (UST) HRV (3 or 4 min) and standard 5 min HRV and explored the optimal recording duration under resting and post-exercise conditions. Methods: Fourteen participants exercised on a cycle ergometer at 60% of their maximum peak power. Data were collected during the rest condition (Pre-E) and three post-exercise conditions (Post-E1, Post-E2, and Post-E3), with indicators of the standard deviation (SDNN) of the ultra-short and short-term RR intervals and the root mean square (RMSSD) of the continuous difference between RR intervals. Repeated measures ANOVA, Cohen's d statistic, Bland–Altman analysis, and interclass correlation coefficients (ICC) assessed the agreement between UST-HRV and ST-HRV. Results: The consistency results of SDNN and RMSSD in resting and post-exercise were different. At the Pre-E, Post-E2, and Post-E3 phases, no statistical differences for SDNN and RMSSD were observed, with ICCs surpassing 0.9, indicating a high level of agreement. However, at Post-E2, there was a significant difference between 3 min RMSSD and 5 min RMSSD (p < 0.05), as well as between 3 min SDNN, 4 min SDNN, and 5 min SDNN (p < 0.05). Furthermore, the limits of agreement were observed to decrease as the time duration increased in Bland–Altman plots. Conclusions: UST-HRV analysis is a reliable substitute for standard 5 min HRV assessment, particularly during resting conditions. For post-exercise measurements, assessing the appropriateness of a 3- or 4 min duration based on the exercise's length is recommended to ensure accuracy and reliability. [ABSTRACT FROM AUTHOR]

Background: Heart rate variability (HRV) may provide objective information about cardiogenic autonomic balance in individuals with spinal cord injury (SCI). The aim of this study was to characterize the diurnal variation of HRV in individuals with SCI at lesion level T6 and above and lesion level below T6. Methods: This was a retrospective analysis of a prior cross-sectional study. Individuals with chronic SCI underwent 24 h recording of the time between consecutive R waves (RR interval) to derive parameters of HRV as follows: standard deviation of all normal-to-normal R–R intervals (SDNN) and square root of the mean of the squared differences between successive R–R intervals (RMSSD) (time domain); and high frequency power (HF), low-frequency power (LF), very low frequency power (VLF), ultra-low frequency power (ULF) and total power (TP) (frequency domain). Changes in the magnitude of HRV outcomes over the 24 h period were investigated using a novel multi-component cosinor model constrained to the form of a three-harmonic Fourier series. Results: Participants were grouped as lesion level T6 and above (n = 22) or below T6 (n = 36). Most of them were male (n = 40, 69%) and the median age (interquartile range) was 50.5 (28) years. Both groups exhibited similar diurnal patterns in most HRV metrics. The lowest values occurred in the late afternoon (4–6 pm) and gradually increased, peaking around midnight to early morning (1–6 am). Exceptions included RMSSD, which peaked before midnight, and ULF, which showed a double peak pattern that peaked from 11 am to 1 pm and 4–6 am in participants with lesion level at T6 and above. The HRV values in participants with lesion level T6 and above were generally lower than participants with lesion level below T6, except for peak values of RMSSD, HF and LF. Conclusion: This study demonstrated substantial diurnal variation of HRV in participants with SCI in both groups of participants. Although cardiac autonomic markers (CAMs) are commonly used to assess cardiac reinnervation in heart-transplant patients, their relationship to the degree of sympathetic and vagal cardiac reinnervation is not well understood yet. To study this relationship, we applied a mathematical model of the cardiovascular system and its autonomic control. By simulating varying levels of sympathetic and vagal efferent sinoatrial reinnervation, we analyzed the induced changes in CAMs including resting heart rate (HR), bradycardic and tachycardic HR response to Valsalva maneuver, root mean square of successive differences between normal heartbeats (RMSSD), low-frequency (LF), high-frequency (HF), and total spectral power (TSP). For assessment of vagal cardiac reinnervation levels >20%, resting HR (ρ = 0.99, p < 0.05), RMSSD (ρ = 0.97, p < 0.05), and TSP (ρ = 0.96, p < 0.05) may be equally suitable as HF-power (ρ = 0.97, p < 0.05). To assess sympathetic reinnervation, LF/HF ratio (ρ = 0.87, p < 0.05) and tachycardic response to Valsalva maneuver (ρ = 0.9, p < 0.05) may be more suitable than LF-power (ρ = 0.77, p < 0.05). Our model reports mechanistic relationships between CAMs and levels of efferent autonomic sinoatrial reinnervation. The results indicate differences in the suitability of these markers to assess vagal and sympathetic reinnervation. Although our analysis is purely conceptual, the developed model can help to gain important insights into the genesis of CAMs and their relationship to efferent sinoatrial reinnervation and, thus, provide indications for clinical study evaluation. The off-season for natural bodybuilders (BB) is characterized by increased training loads and fluctuations in caloric intake, which may lead to insufficient recovery. The autonomic nervous system (ANS) plays a pivotal role in recovery. The purpose of this study was to evaluate resting ANS activity and recovery following a maximal exercise bout in off-season BB and compare them to those of recreationally active individuals. Fifteen males participated; 7 recreationally active (RA) (24.6 ± 2.1 years, 81.1 ± 10.8 kg) and 8 BB (21.8 ± 2.9 years, 89.3 ± 13.0 kg). Each performed a graded exercise test. Heart rate variability (HRV) was measured at rest and during a 45 min recovery period. HRV was analyzed as: root mean square of successive differences (lnRMSSD), standard deviation of normal-to-normal sinus beats (lnSDNN), high frequency (lnHF), low frequency (lnLF), and the ratio of low frequency to high frequency (lnLF/lnHF). A one-way ANOVA showed no differences for any resting marker of HRV, HR, and HR recovery. A significant depression in all markers of HRV was observed in the BB group at the 15 min point, and no recovery was observed before 45 min when compared to RA. The results of this study demonstrated depressed HRV recovery following the graded exercise test in BB when compared to the RA group. [ABSTRACT FROM AUTHOR]

Passive translational acceleration (PTA) has been demonstrated to induce the stress response and regulation of autonomic balance in healthy individuals. Electrodermal activity (EDA) and heart rate variability (HRV) measurements are reliable indicators of the autonomic nervous system (ANS) and can be used to assess stress levels. The objective of this study was to investigate the potential of combining EDA and HRV measurements in assessing the physiological stress response induced by PTA. Fourteen healthy subjects were randomly assigned to two groups of equal size. The experimental group underwent five trials of elevator rides, while the control group received a sham treatment. EDA and HRV indices were obtained via ultra-short-term analysis and compared between the two groups to track changes in the ANS. In addition, the complexity of the EDA time series was compared between the 4 s before and the 2–6 s after the onset of PTA to assess changes in the subjects' stress levels in the experimental group. The results revealed a significant increase in the skin conductance response (SCR) frequency and a decrease in the root mean square of successive differences (RMSSD) and high frequency (HF) components of HRV. In terms of stress assessment, the results showed an increase in the complexity of the EDA time series 2–6 s after the onset of PTA. These results indicate an elevation in sympathetic tone when healthy subjects were exposed to a translational transport scenario. Autonomic dysreflexia (AD) is a common autonomic complication of spinal cord injury (SCI) characterized by a sudden increase is blood pressure triggered by peripheral stimulation, such as bladder distention. Iatrogenic AD events often occur during various medical procedures including urodynamic assessments (UDSs) used to evaluate lower urinary tract (LUT) function in individuals with SCI. To date, there are no established clinical practices that would allow early detection of the development of episodes of AD. Heart rate variability (HRV) is a reliable and non-invasive metric for evaluating autonomic regulation of the cardiovascular system, with demonstrated utility in people with SCI during UDSs. We aim to provide a comprehensive evaluation of cardiovascular function during UDS-induced AD using ultra-short-term HRV analysis and identify changes in cardiovascular dynamics to predict the onset of AD. We assessed cardiovascular data in a total of 24 participants with sensorimotor complete SCI above T6 (17 males, 7 females, median age = 43 [36–50] years) who experienced AD during UDS. We used continuous electrocardiographic recordings to evaluate HRV in 60 sec overlapping windows during filling cystometry. The mean of "normal-to-normal" heartbeats (meanNN), its standard deviation (SDNN), and the root mean square of successive differences (RMSSD) were calculated and used in all subsequent analyses. We found that SDNN and RMSSD diminished during the early phase of bladder filling and sharply increased during AD. Using the lowest point of statistical variability in heart rate (i.e., SDNN), we were able to predict AD events within 240 sec (percentile 25–percentile 75: 172–339 sec) before the first systolic blood pressure peak after AD onset (sensitivity = 0.667; specificity = 0.875). Our results indicated a temporary increase in sympathetic activity during the early phase of bladder filling, which is followed by an increase in parasympathetic outflow to the heart when AD occurs. These findings have significant clinical implications that extend beyond the context of UDS and demonstrate the importance of identifying early changes in HRV in order to accurately predict AD episodes in people living with SCI. Sufficiently cold-water temperatures (<7°C) are needed to elicit the sympathetic response to the cold pressor test using the hand. However, it is not known if stimulating the trigeminal nerve via face cooling, which increases both sympathetic and cardiac parasympathetic activity, also has a threshold temperature. We tested the hypothesis that peak autonomic activation during a progressive face cooling challenge would be achieved when the stimulus temperature is ≤7°C. Twelve healthy participants (age: 25 ± 3 yr, four women) completed our study. Six pliable bags, each containing water or an ice slurry (34°C, 28°C, 21°C, 14°C, 7°C, and 0°C) were applied sequentially to participants’ forehead, eyes, and cheeks for 5 min each. Mean arterial pressure (photoplethysmography; index of sympathetic activity) and heart rhythm (3-lead ECG) were averaged in 1-min increments at the end of baseline and throughout each temperature condition. Heart rate variability in the time [(root mean square of successive differences (RMSSD)] and frequency [high-frequency (HF) power] domains was used to estimate cardiac parasympathetic activity. Data are presented as the increase from baseline ± SD. Mean arterial pressure only increased from baseline in the 7°C (13.1 ± 10.3 mmHg; P = 0.018) and 0°C (25.2 ± 7.8 mmHg; P < 0.001) conditions. Only the 0°C condition increased RMSSD (160.6 ± 208.9 ms; P = 0.009) and HF power (11,450 ± 14,555 ms2; P = 0.014) from baseline. Our data indicate that peak increases in sympathetic activity during face cooling are initiated at a higher forehead skin temperature than peak increases in cardiac parasympathetic activity.NEW & NOTEWORTHY Using a progressive face cooling challenge, we demonstrated that peak sympathetic activation occurs at a higher forehead skin temperature (forehead temperature: 19°C; stimulus temperature: 7°C) than peak cardiac parasympathetic activation (forehead temperature: 11°C; stimulus temperature: 0°C). Therefore, we suggest using a sufficiently cold stimulus (∼0°C ) when using face cooling to interrogate autonomic activation to stimulate both the sympathetic and cardiac parasympathetic nervous systems. Although the mechanisms underlying the pathophysiology of long COVID condition are still debated, there is growing evidence that autonomic dysfunction may play a role in the long-term complications or persisting symptoms observed in a significant proportion of patients after SARS-CoV-2 infection. However, studies focused on autonomic dysfunction have primarily been conducted in adults, while autonomic function has not yet been investigated in pediatric subjects. In this study, for the first time, we assessed whether pediatric patients with long COVID present abnormalities in autonomic cardiac function. Fifty-six long COVID pediatric patients (mean age 10.3 ± 3.8 y) and 27 age-, sex-, and body surface area-matched healthy controls (mean age 10.4 ± 4.5y) underwent a standard 12-lead electrocardiography (ECG) and 24-h ECG Holter monitoring. Autonomic cardiac function was assessed by time-domain and frequency-domain heart rate variability parameters. A comprehensive echocardiographic study was also obtained by two-dimensional echocardiography and tissue Doppler imaging. Data analysis showed that pediatric patients with long COVID had significant changes in HRV variables compared to healthy controls: significantly lower r-MSSD (root mean square of successive RR interval differences, 47.4 ± 16.9 versus 60.4 ± 29.1, p = 0.02), significant higher values VLF (very low frequency, 2077.8 ± 1023.3 versus 494.3 ± 1015.5 ms, p = 0.000), LF (low frequency, 1340.3 ± 635.6 versus 354.6 ± 816.8 ms, p = 0.000), and HF (high frequency, 895.7 ± 575.8 versus 278.9 ± 616.7 ms, p = 0.000). No significant differences were observed between the two groups both in systolic and diastolic parameters by echocardiography. Conclusion: These findings suggest that pediatric patients with long COVID have an imbalance of cardiac autonomic function toward a relative predominance of parasympathetic tone, as already reported in adult patients with long COVID. Further studies are needed to clarify the clinical significance of this autonomic dysfunction and demonstrate its role as a pathophysiological mechanism of long COVID, paving the way for effective therapeutic and preventive strategies. Background: Although there is a very high comorbidity between tobacco dependence and other addictive disorders, there are only few studies examining the implementation and outcomes of a tobacco cessation program in patients with addictive diseases. Therefore, the aim of this study is to investigate to what extent a standardized tobacco cessation program leads to improvements regarding psychological/physical parameters in patients with addiction undergoing therapy and whether there is a reduction in tobacco consumption. Methods: The study took place in a therapeutic community specialized in addiction therapy. A total sample of 56 participants were non-randomly assigned to an intervention group (IG; n = 31) and a treatment as usual group (TAUG; n = 25). The IG participated in a 6-week tobacco cessation program, while the TAUG received no additional treatment. Both groups were assessed for changes in primary outcomes (tobacco dependence, smoked cigarettes per day (CPD), and general substance-related craving) and secondary outcomes (heart rate variability (HRV): root mean square of successive differences, self-efficacy, and comorbid psychiatric symptoms) at two measurement time points (pre- and post-treatment/6 weeks). Results: We observed significant improvements in self-efficacy (F(1,53) = 5.86; p <.05; ηp2 =.11) and decreased CPD in the IG (β = 1.16, ρ <.05), while no significant changes were observed in the TAUG. No significant interaction effects were observed in psychiatric symptoms, general substance-related craving, and HRV. Conclusions: The results highlight the potential benefit of an additional tobacco cessation program as part of a general addiction treatment. Although no improvements in the physiological domain were observed, there were significant improvements regarding self-efficacy and CPD in the IG compared to the TAUG. Objective: To investigate the circadian changes of the autonomic function in patients with zoster‐associated pain (ZAP). Methods: A total of 37 patients with ZAP from April 2022 to October 2022 were enrolled as the observation group, and 37 normal volunteers at the same time were selected as the control group. All participants were required to wear a 24‐h Holter, which was used to compare the heart rate variability (HRV) between the two groups. HRV analysis involved time‐ and frequency‐domain parameters. Results: There was no statistically significant difference in general information between two groups. Patients with ZAP had an increased mean heart rate and decreased the standard deviation of normal‐to‐normal (SDNN) R–R interval, the root mean square of the differences (RMSSD) in successive RR interval, low frequency (LF), and high frequency (HF) compared with control groups in all periods (p <.05). The ratio of LF/HF between two groups had no significant difference (p =.245). SDNN had no significant difference between day and night in the control group (p >.05), whereas SDNN of ZAP patients in night period was reduced than that in day period (p <.001). The level of RMSSD during the day was lower than those at night in the control group (p <.05), whereas no significant difference of RMSSD between two periods was observed in patients with ZAP (p >.05). Conclusion: The results of this study indicated that ZAP contributes to the decline of autonomic nervous system (ANS) function, especially parasympathetic components. The patients with ZAP lost parasympathetic advantage and had a worse ANS during the night. In this work, we conducted an observational study of Circadian Changes of Autonomic Function in Patients with Zoster‐associated Pain via HRV. To our best knowledge, our study is one of the rare and comprehensive attempts to explore circadian changes of autonomic function in patients with ZAP via 24‐hour heart rate variability analysis. The results of this study indicated that ZAP contributes to change in the ANS resulting in decreased the function of ANS and PNS in all periods. Background: Heart rate variability (HRV) is an established, non-invasive parameter for the assessment of cardiac autonomic nervous activity and the health status in general cardiology. However, there are few studies on HRV in adults with congenital heart defects (CHDs). The aim of the present study was to evaluate the use of long-term continuous HRV measurement for the assessment of global health status in adults with cyanotic CHD. Methods: This prospective study included 45 adults (40% female, mean age = 35.2 ± 9.2 [range: 19–58] years) after cardiac surgical repair. HRV parameters were calculated from continuous 24 h measurements using a Bittium Faros 180 sensor (Bittium Corp., Oulu, Finland). Results: Postoperative patients with transposition of the great arteries (TGA) (n = 18) achieved significantly higher values of standard deviation of NN intervals (SDNN) (175.4 ± 59.9 ms vs. 133.5 ± 40.6 ms; p = 0.013) compared with patients with other conotruncal anomalies (n = 22). Comparing patients with TGA after a Senning–Brom or Mustard operation (n = 13) with all other heart surgery patients (n = 32), significantly higher HRV parameters were found after atrial switch (root mean square of successive RR interval differences: 53.6 ± 20.7 ms vs. 38.4 ± 18.3 ms; p = 0.019; SDNN: 183.5 ± 58.4 ms vs. 136.3 ± 45.3 ms; p = 0.006). A higher SDNN was also measured after Senning–Brom or Mustard operations than after a Rastelli operations (n = 2) (SDNN: 183.5 ± 58.4 ms vs. 84.5 ± 5.2 ms; p = 0.037). When comparing atrial switch operations (n = 3) with Rastelli operations, the SDNN value was significantly shorter in the Rastelli group (p = 0.004). Conclusions: Our results suggest that continuous HRV monitoring may serve as a marker of cardiac autonomic dysfunction in adults with cyanotic CHD after surgical repair. Impaired cardiac autonomic nervous activity may be associated with an increased risk of adverse reactions in patients with repaired CHD. Therefore, a longitudinal assessment of HRV patterns and trends may provide a deeper insight into dynamic changes in their autonomic regulation and disease progression, lifestyle changes, or treatments. As each person has individual variability in heart rate, HRV may be useful in assessing intra-individual disease progression and may help to improve personalized medicine. Introduction: Acupuncture analgesia (AA) is widely used in clinical practice. The autonomic nervous system (ANS) may be an important pathway for acupuncture signal transduction. However, real-time changes in autonomic function during AA and the effect of "needle sensation" remain unclear. Methods: We established a human pain model in healthy adults and randomly assigned 128 participants to the model, sham acupuncture, and acupuncture groups in a 1:1:2 ratio. Heart rate variability (HRV), including total power (TP), low-frequency power (LF), high-frequency power (HF), ratio of LF to HF (LF/HF), standard deviation of the normal-normal intervals (SDNN), and root mean square of successive interval differences (RMSSD), were used to assess autonomic function. The visual analog scale (VAS) and efficiency were used to assess the analgesic effect of acupuncture. The Massachusetts General Hospital acupuncture sensation scale (MASS) was used to indicate the intensity of the needle sensation. Anxiety levels were also measured. Finally, the correlation of MASS with HRV, VAS, and anxiety levels was analyzed. Results: VAS decreased after 10 min of needling and 5 min after needle withdrawal in the acupuncture group compared with those in the model group (p = 0.038, p = 0.020). The efficacy rates were 82.0, 50.0, and 61.3% in the acupuncture, model, and sham groups, respectively. These represent significant differences between the acupuncture group and the model and sham acupuncture groups (p < 0.001 in each case). No differences were observed between the model and sham acupuncture groups. HF, TP, SDNN, and RMSSD were all increased in the acupuncture group compared with those in the model group (p = 0.045, p = 0.041, p = 0.002, p = 0.006, respectively). No differences were observed in the sham acupuncture group compared to the model group (p = 0.632, p = 0.542, p = 0.093, p = 0.222, respectively). The LF and LF/HF did not differ among all three groups. A positive correlation was observed between MASS and RMSSD2, LF2, RMSSD4, TP4, VAS5, and anxiety levels. Conclusion: AA was associated with enhanced vagal activity. The intensity of needle sensation was positively correlated with vagal and sympathetic nerve activities. Introduction: Concussion is a growing concern in worldwide sporting culture. Heart rate variability (HRV) is closely tied with autonomic nervous system (ANS) deficits that arise from a concussion. The objective of this review was to determine if a history of concussion (HOC) can impact HRV values in the time-domain in individuals at rest. This review works to add to the literature surrounding HRV testing and if it can be used to check for brain vulnerabilities beyond the recovery of concussion symptoms. Materials and methods: The systematic review was conducted using the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) method. A computer based systematic review scanned articles dating from 1996 to June 2023 through PubMed, Cochrane Library, Google Scholar, and EMBASE databases. A risk of bias assessment was conducted using the ROBINS-E tool. The average difference in time between heartbeats (MeanNN), the standard deviation of the differences (SDNN), and the root mean squared of the successive intervals (RMSSD) were measured. Results: Six total studies were found that fit the inclusion criteria including a total of 242 participants (133 without HOC, 109 with HOC). The average age of the control group was 23.3 ± 8.2, while the average age of the history of TBI group was 25.4 ± 9.7, with no significant difference between the groups (p = 0.202). Four of the studies reported no significant difference in any of the three measures, while two of the studies reported significant difference for all three measures. The meta-analysis was conducted and found that MeanNN (p = 0.03) and RMSSD (p = 0.04) reached statistical significance, while SDNN did not (p = 0.11). Conclusion: The results of this meta-analysis showed significant difference in two of the three HRV time-domain parameters evaluated. It demonstrates that there can be lowered HRV values that expand beyond the recovery of symptoms, reflecting an extensive period of ANS susceptibility after a concussion. This may be an important variable in determining an athlete's return to play (RTP). Lack of homogenous study populations and testing methods introduces potential for bias and confounding factors, such as gender or age. INTRODUCTION: Preselection of pilot candidates in the military is critical and determines the quality of subsequent selection. The Aero Medical Institute in Belgrade uses the following centrifuge endpoints: peripheral vision loss, heart rate above 180 bpm, cardiac arrhythmias, and G-induced loss of consciousness to assess relaxed +Gz tolerance. The aim of this study was to evaluate heart rate variability (HRV) associated with cardiovascular adaptability to different types of stress as a predictor of +Gz tolerance. METHODS: Thirty-six candidates were exposed to centrifuge runs, consisting of the following +Gz-acceleration phases: a 2-G plateau followed by an increase to 5.5 G, a decrease to 2 G, and ending with a plateau. Time-domain HRV indices were calculated for candidates, for a 60-s 2-G plateau, and for the entire test. The correlation was made between the groups that did (Group 1) and did not meet the criteria (Group 2). RESULTS: The results show significantly lower values in all time domain HRV indices, namely standard deviation of the normal-tonormal interval (SDNN) and root mean square of successive differences, in Group 2. Mean SDNN values were 45.98 ± 24.80 ms (2-G plateau) and 109.99 ± 39.64 ms (entire test) in Group 1, while the SDNN were 22.99 ± 12.04 ms and 69.70 ± 33.45 ms in Group 2. Root mean square of successive differences was higher in Group 1 for the 2-G plateau and for the entire test. Background: Heart rate variability (HRV) is a common means of monitoring responses to training, yet in professional cycling, one may question its usefulness, particularly during multi-day competitions such as Grand Tours. Objectives: This study aims to report and analyze HRV responses in a male professional cyclist over a season, including the Tour de France. Methods: A professional cyclist recorded resting and exercise inter-beat intervals during 5 months, comprising a training period with two altitude sojourns and two competition blocks, including the Tour de France. Resting recordings lasted 5 min in the supine position and were used for computation of mean heart rate (HR), root mean square of the successive differences (RMSSDs), and power in the low- and high-frequency bands (LF and HF, respectively). Training load quantification was based on recorded HR during exercise and expressed as training impulses (TRIMPSs). Results: LF (3,319 ± 2,819 vs. 1,097 ± 1,657 ms2), HF (3,590 ± 1858 vs. 1,267 ± 1,683 ms2), and RMSSD (96 ± 26 vs. 46 ± 30 ms) were higher and HR (47 ± 4 vs. 54 ± 2 bpm) was lower during the training period when compared to the two competition blocks. The coefficient of variation (CV) was significantly lower during the training period than during the two competition blocks for RMSSD (26 vs. 72%), LF (85 vs. 160%), and HF (58 vs. 141%). Discussion: The present study confirms that monitoring daily HRV responses during training periods is valuable in professional cycling, but questions its usefulness during the Tour de France. Respiratory sinus arrhythmia (RSA) is an index of parasympathetic nervous system activity reflecting respiratory influences on heart rate. This influence is typically measured as high frequency heart rate variability (HF‐HRV) or root mean square of successive differences (RMSSD) of adjacent inter‐beat intervals. Examining the long‐term stability of its measurement is important as levels of resting RSA have been conceptualized as a marker of individual differences; in particular, of an individual's autonomic regulation and affect‐related processes, including emotion regulation. At present, it is not known if resting RSA levels reflect stable differences over a long‐term period (i.e., >1 year). Even less is known about how RSA stability differs as a function of depression history and whether it relates to depression risk trajectories. In the present study, we examined the 1.5‐year test–retest reliability of resting RSA using the intraclass correlation coefficient (ICC) in 82 adults: n = 41 with a history of depression (ever‐depressed); n = 41 controls with no depression history (never‐depressed). HF‐HRV was fairly stable in both groups (ever‐depressed ICC = 0.55, never‐depressed ICC = 0.54). RMSSD was also fairly stable in ever‐depressed adults (ICC = 0.57) and never‐depressed controls (ICC = 0.40). ICC values for both indices did not differ between groups per overlapping 95% confidence intervals. Therefore, RSA stability as assessed by both frequency (HF‐HRV) and time domain (RMSSD) measures was not attenuated by a depression history. Implications and the need for future research are discussed. Researchers commonly utilize resting levels of parasympathetically‐mediated heart rate variability (e.g., HF‐HRV, RMSSD) to assess emotion regulation in healthy and clinical samples, particularly depression. However, little is known about its long‐term stability, and even less so in the presence of psychopathology. Our findings indicate that resting HF‐HRV and RMMSD are fairly stable indices of RSA over a period of 1.5 years, on average, among adults with and without a history of depression. To ensure both optimal health and performances, monitoring physiological and psychological states is of main importance for athletes. It is well known that monitoring heart rate variability and using validated questionnaires is useful for monitoring both the health and training status of athletes of different sports. Motorsports such as rally require high levels of physical and mental preparation thus information about psychophysiological status of rally athletes is fundamental. The aim of this study was to assess the autonomic regulation, stress, recovery conditions of one driver and one co-driver competing at the Italian National Rally Championship during their competition period. Heart rate variability parameters, acute recovery and stress states were assessed the day before, during the two days of race and the day following the races. Results showed that driver and co-driver had a sharp decrease of mean RR intervals, root mean square of successive differences between normal heartbeats, and standard deviation of the N-N interval during race days, while the stress index showed the inverse trend, and this behaviour was clearly visible in the Poincaré plots and power spectrum density graphs. The acute recovery and stress states questionnaire showed significant differences in recovery and stress scoring for the driver but not for the co-driver, although the trends were similar. This study describes the psychophysiological demands of a rally competition period suggesting that a daily evaluation of heart rate variability, recovery, stress states is useful for monitoring health status in rally athletes and could be implemented to make decision about training and recovery strategies. Background: Prior research suggests that cardiovascular autonomic dysfunction might be an early marker of cardiotoxicity induced by antitumor treatment and act as an early predictor of cardiovascular disease-related morbidity and mortality. The impact of thoracic radiotherapy on the parasympathetic and sympathetic nervous systems, however, remains unclear. Therefore, this study aimed to evaluate the short-term effects of thoracic radiotherapy on the autonomic nervous system, using deceleration capacity (DC), acceleration capacity (AC) of heart rate, and heart rate variability (HRV) as assessment tools. Methods: A 5 min electrocardiogram was collected from 58 thoracic cancer patients before and after thoracic radiotherapy for DC, AC, and HRV analysis. HRV parameters employed included the standard deviation of the normal-normal interval (SDNN), root mean square of successive interval differences (RMSSD), low frequency power (LF), high frequency power (HF), total power (TP), and the LF to HF ratio. Some patients also received systemic therapies alongside radiotherapy; thus, patients were subdivided into a radiotherapy-only group (28 cases) and a combined radiotherapy and systemic therapies group (30 cases) for additional subgroup analysis. Results: Thoracic radiotherapy resulted in a significant reduction in DC (8.5 [5.0, 14.2] vs. 5.3 [3.5, 9.4], p = 0.002) and HRV parameters SDNN (9.9 [7.03, 16.0] vs. 8.2 [6.0, 12.4], p = 0.003), RMSSD (9.9 [6.9, 17.5] vs. 7.7 [4.8, 14.3], p = 0.009), LF (29 [10, 135] vs. 24 [15, 50], p = 0.005), HF (35 [12, 101] vs. 16 [9, 46], p = 0.002), TP (74 [41, 273] vs. 50 [33, 118], p < 0.001), and a significant increase in AC (-8.2 [-14.8, -4.9] vs. -5.8 [-10.1, -3.3], p = 0.003) and mean heart rate (79.8 ± 12.6 vs. 83.9 ± 13.6, p = 0.010). Subgroup analysis indicated similar trends in mean heart rate, DC, AC, and HRV parameters (SDNN, RMSSD, LF, HF, TP) in both the radiotherapy group and the combined treatment group post-radiotherapy. No statistically significant difference was noted in the changes observed in DC, AC, and HRV between the two groups pre- and post-radiotherapy. Conclusion: Thoracic radiotherapy may induce cardiovascular autonomic dysfunction by reducing parasympathetic activity and enhancing sympathetic activity. Importantly, the study found that the concurrent use of systemic therapies did not significantly amplify or contribute to the alterations in autonomic function in the short-term following thoracic radiotherapy. The aim of this systematic review with meta-analysis was to determine differences in cardiovascular autonomic parameters between patients with myasthenia gravis (MG) and healthy controls (HCs). Two reviewers searched four electronic databases, namely PubMed, Web of Science, EMBASE, and SCOPUS, from database inception to 7 July 2023 for studies investigating cardiovascular autonomic parameters in MG vs. HCs. A random-effects meta-analysis was performed to compute Hedges' g ± 95% confidence intervals (CI). Out of a total of 2200 records, 8 observational studies with a sample size of 301 patients with MG and 454 HCs were included in the systematic review. Meta-analysis revealed lower values of expiration/inspiration ratio (g = −0.45, I2 = 74.7), baroreflex sensitivity (g = −0.56, 95%CI −0.80, −0.33; I2 = 0.3), percentage of adjacent NN intervals differing by more than 50 ms (g = −1.2, I2 = 82.8), square root of the mean of squared differences between successive beat intervals (g = −1.94, I2 = 95.1), mean of the standard deviations of all NN intervals (g = −0.83, 95%CI −1.37, −0.28; I2 = 55.5), and high frequency of HRV during tilt (g = −0.75, 95%CI −0.11, −0.39; I2 = 0). MG patients vs. HCs had higher systolic blood pressure (g = 0.39; I2 = 56.1), sympathovagal balance at rest/during tilt (LF/HF-RRIsupine, g = 0.44; I2 = 0; LF/HF-RRItilt, g = 0.86; I2 = 0; LF/HFtilt, g = 0.40; I2 = 0). Purpose: Heart rate variability (HRV) has gained acceptance as a key marker of cardiovascular health. We compared HRV responses after continuous moderate-intensity exercise (CMIE) and high-intensity interval exercise (HIIE) matched for intensity and duration in individuals with midspectrum chronic kidney disease (CKD). Methods: Twenty men and women (age 62.0 ± 10 yrs.) diagnosed with CKD stages G3a and G3b participated in a 2 (condition) x 4 (time point) repeated cross-over measures design study. HRV time-domain indices were based on the standard deviation of all NN intervals (SDNN) and the square root of the mean of the sum of the squares of differences between adjacent NN intervals (RMSSD) and frequency domain. High-frequency (HF), low-frequency (LF), total power (TP) were examined. CMIE consisted of treadmill walking for 30 minutes at a 2% incline and speed corresponding to 60%–65% of reserve volume of oxygen (VO2R). HIIE included five intervals of 3 minutes at 90% of VO2R and 2 minutes at 20% VO2R intervals. Conditions were designed to be of the same average intensity (60% to 65% of VO2R) and caloric expenditure (~144 kcal). Results: Immediately following exercise SDNN, RMSSD, HF, LF, and TP were significantly lower compared to before exercise (p <.05). HRV responses were not different between conditions and conditions X time (p >.05). Conclusions: Thirty minutes of either CMIE or HIIE decreased HRV indices, pointing to an autonomic imbalance favoring vagal mediation. HRV's responses regarding HIIE were no different from CMIE, therefore, from an autonomic function point of view this similarity may be useful for CKD exercise prescription and programming. The Polar Team Pro computer software program simultaneously analyzes heart rate variability (HRV) for multiple athletes and requires little to no expertise from the user. Still, there remains the need to examine the accuracy and reliability of this candidate software when compared to the recognized gold standard HRV computer software program (Kubios HRV). Twenty-one (n = 21) healthy female soccer players volunteered to be participants. An ultra-short recording period (60-s) of R–R intervals (ms) was collected in the supine position in a controlled laboratory setting over two data collection periods spaced one month apart. R–R intervals were exported into both the candidate and reference computer software for analysis. The root mean square of the successive differences (rMSSD) were calculated along with mean R–R interval length. rMSSD values derived from the candidate software were compared to both the raw and artefact corrected values from the reference software. After performing artefact correction, mean (± SD) rMSSD values were not statistically different between software (candidate = 61.2 ± 31.0 ms 'vs' reference = 63.1 ± 31.1 ms, p = 0.214). Mean (± SD) R–R intervals were significantly different (candidate = 893.4 ± 119.8 ms 'vs' reference = 882.3 ± 111.3 ms, p = 0.003). Excellent reliability in artefact corrected rMSSD (r = 0.95, p < 0.001) and mean R–R interval length (r = 0.99, p < 0.001) was observed. Conceptual models of psychosocial influences on short-term changes (i.e., reactivity) in vagally-mediated heart rate variability (vmHRV) emphasize self-regulatory effort and social threat versus comfort. However, these two general perspectives have been tested separately in nearly all cases, limiting conclusions about the relative importance or possible interactive effects of effortful self-regulation and social stress. The present study compared effects of effort to regulate emotional expression and social stress versus safety on vmHRV reactivity during an interpersonal interaction, in a 2 (self-regulate emotion vs. express emotion freely) × 3 (positive vs. neutral vs. negative interaction valence) × 2 (male vs. female) between-subjects randomized factorial design. A sample of 180 undergraduates (90 women; 69 % White) discussed a current events topic (i.e., human-caused climate change) with a prerecorded partner, presented as a live interaction over a computer. Self-reports of affective responses, self-regulation effort, and appraisals of the partner's behavior, as well as observer ratings of participants' behavior during the interaction, supported the effectiveness of self-regulation and interaction valence manipulations, although the former manipulation may have been somewhat weaker than the latter. Primary analyses of high-frequency heart rate variability (HF-HRV) and root mean square of successive differences (RMSSD) in heart beat intervals recorded at baseline and during the interaction revealed larger decreases in vmHRV during negative than neutral or positive interactions, but no effects of self-regulation instructions. Overall, results indicated more robust effects of social stress on vmHRV reactivity, relative to effects of self-regulatory effort. • Social safety and emotion regulation can evoke changes in heart rate variability. • These two influences on HRV change are typically examined separately. • In a simultaneous test, only negative social interactions altered HRV. • Social stress may exert a greater influence on brief HRV change than self-regulation. • Interpersonal concepts and methods may be useful in research on HRV. Introduction: Early detection and diagnosis of diabetic autonomic neuropathy, especially cardiac autonomic neuropathy (CAN), have gained attention recently because of their elevated cardiovascular mortality risk. Although the connection between type 2 diabetes mellitus and autonomic neuropathy is well established, evidence is emerging that the association might predate the stage of prediabetes. Objective: The present study was undertaken to compare the prevalence of CAN in prediabetes versus that in normoglycemic controls. Materials and Methods: The study population was selected by purposive sampling from individuals attending a tertiary care hospital from January 2018 to June 2019. Fifty individuals with prediabetes diagnosed by the American Diabetes Association's glycated haemoglobin criteria and 50 age- and gender-matched healthy controls were recruited. CAN was assessed by standard cardiovascular reflex tests, as described by Ewing and Clarke. Changes in R-R with deep breathing, Valsalva manoeuver, and changes in blood pressure (BP) in response to standing and sustained handgrip were evaluated. Three-time domains [standard deviation of normal-to-normal intervals (SDNN), root mean square of successive RR intervals (rMSSD) and percentage of successive normal to normal R-R (NN) intervals that differ by more than 50 ms (pNN50)] and four frequency domain indices [very low-frequency band (VLF), low-frequency band (LF), high-frequency band (HF), LF/HF ratio) of heart rate variability (HRV)] were examined. Results: The mean heart rate was 71.37 ± 7.94 and 65.59 ± 8.73 beats/min in patients with prediabetes and controls, respectively (P < 0.05). All three-time-domain indices of HRV were significantly lower in persons with prediabetes compared to controls. The peak frequency of LF, peak power of LF, normalised unit of LF, and LF/HF ratio was significantly lower in subjects with prediabetes than in controls. There was no difference in the traditional cardiovascular autonomic reflex testing. Conclusion: Our study demonstrates the presence of subclinical autonomic dysfunction in persons with prediabetes. The dramatic decrease in heart rate (HR) during entrance into hibernation is not a mere response to the lowering of core body temperature (T b) but a highly regulated fall, as the decrease in HR precedes the drop in T b. This regulated fall in HR is thought to be mediated by increased cardiac parasympathetic activity. Conversely, the sympathetic nervous system is thought to drive the increase of HR during arousal. Despite this general understanding, we lack temporal information on cardiac parasympathetic regulation throughout a complete hibernation bout. The goal of this study was to fill this gap in knowledge by using Arctic ground squirrels implanted with electrocardiogram/temperature telemetry transmitters. Short-term HR variability (root mean square of successive differences [RMSSD]), an indirect measure of cardiac parasympathetic regulation, was calculated in 11 Arctic ground squirrels. RMSSD, normalized as RMSSD/RR interval (RRI), increased fourfold during early entrance (from 0.2 ± 0.1 to 0.8 ± 0.2 , P < 0.05). RMSSD/RRI peaked after HR dropped by over 90% and T b fell by 70%. Late entrance was delineated by a decline in RMSSD/RRI while T b continued to decrease. During arousal, HR started to increase 2 h before T b, with a concurrent decrease in RMSSD/RRI to a new minimum. As T b increased to a maximum during interbout arousal, HR declined, and RMSSD/RRI increased. These data suggest that activation of the parasympathetic nervous system initiates and regulates the HR decrease during entrance into hibernation and that withdrawal of parasympathetic activation initiates arousal. We conclude that cardiac parasympathetic regulation persists throughout all phases of a hibernation bout—a feature of the autonomic nervous system's regulation of hibernation that was not appreciated previously. Objective: This study aimed to investigate the relationships of heart rate variability (HRV) with the presence, severity, and individual neuroimaging markers of cerebral small vessel disease (CSVD). Method: A total of 4676 participants from the Third China National Stroke Registry (CNSR‐III) study were included in this cross‐sectional analysis. CSVD and its markers, including white matter hyperintensity (WMH), lacunes, enlarged perivascular spaces (EPVS), cerebral microbleeds (CMBs), and brain atrophy (BA), were evaluated. Two common HRV parameters, including the square root of the mean of the sum of the squares of differences between adjacent N–N intervals (RMSSD) and the standard deviation of all N–N intervals (SDNN), were used to evaluate the function of the autonomic nervous system (ANS). Binary or ordinal logistic regression analyses were performed to investigate the association between HRV and CSVD. In addition, two‐sample mendelian randomization (MR) analyses were performed to investigate the causality of HRV with CSVD. Results: RMSSD was significantly associated with total burden of CSVD (Wardlaw's scale, common odds ratio [cOR] 0.80, 95% confidence interval [CI] 0.67–0.96, p = 0.02; Rothwell's scale, cOR 0.75, 95% CI 0.60–0.93, p = 0.008) and the presence of CSVD (Rothwell, OR 0.75, 95% CI 0.60–0.93, p = 0.008). However, no significant associations between SDNN and the presence or total burden of CSVD were observed. Moreover, RMSSD was related to WMH burden (OR 0.80, 95% CI 0.66–0.96, p = 0.02), modified WMH burden (cOR 0.82, 95% CI 0.69–0.97, p = 0.02), and Deep‐WMH (OR 0.75, 95% CI 0.62–0.91, p = 0.003), while SDNN was related to Deep‐WMH (OR 0.80, 95% CI 0.66–0.96, p = 0.02) and BA (cOR 0.80, 95% CI 0.68–0.95, p = 0.009). Furthermore, adding HRV to the conventional model based on vascualr risk factors enhanced the predictive performance for CSVD, as validated by the integrated discrimination index (p < 0.05). In addition, no causality between HRV and CSVD was observed in two‐sample MR analyses. Patients with severe traumatic brain injury (TBI) may have autonomic dysfunction, one manifestation of which is orthostatic intolerance. This potentially impairs physical rehabilitation. However, the exact mechanisms remain elusive. In 30 patients participating in a trial of early tilt training versus standard care and 15 healthy volunteers, 5‐min electrocardiography was recorded in the supine position and during 70° head‐up tilt. Heart rate variability was analyzed by the low‐ and high‐frequency (LF and HF) power, the LF–HF ratio, the total power, the ratio of the standard deviation of normal‐to‐normal intervals (SDNN), the root mean square of successive differences (RMSSD), the detrended fluctuations, and sample entropy. In patients in the upright compared to the supine position, SDNN (p < 0.001), RMSSD (p < 0.001), and total power (p = 0.004) all decreased, while the remaining variables were unchanged; no long‐term differences in heart rate variability in the supine position were found between early tilt training and standard care. In the healthy volunteers, all measures besides SDNN and total power changed significantly between supine and upright position. In patients with severe TBI compared to healthy volunteers, several measures of heart rate variability changed differentially during mobilization from the supine to the upright position. Internalizing psychopathologies (IPs) are characterized by disruptions in emotion regulation (ER). A potential target for ER modulation in individuals with IPs is the theta band. We hypothesized that offset theta-tACS (transcranial alternating current stimulation) would result in more enhanced ER, indexed by greater increase in heart rate variability (HRV), than transcranial direct current stimulation (tDCS) in participants with IPs. This pilot study utilized a double-blind, pseudo-counterbalanced design. Participants with internalizing psychopathologies (anxiety and depression) were randomly assigned to receive either offset theta-tACS (n = 14) or tDCS (n = 15) and underwent four sessions of stimulation (two sham). In both arms, there were alternating iterations of an emotion regulation task (ERT) during or immediately after stimulation and rest. Heart rate data were collected during each ERT and rest iteration, and analyses were completed using high-frequency (HF) and root mean square of successive differences (RMSSD) HRV metrics. tACS participants consistently displayed increases in both HRV metrics from Time 1 to Time 4. Participants receiving tDCS displayed few significant changes in HF-HRV and no significant changes in RMSSD-HRV. Due to the small sample size, analyses were limited. Additionally, the lack of a baseline ERT makes it difficult to determine overall ER improvement. tACS appears to increase ER capacity as reflected in increased HRV in individuals with internalizing psychopathologies, particularly after two sessions of stimulation. This study adds validity to the use of tACS as a neuromodulatory technique in cognitive and clinical research. Simple Summary: Obstructive sleep apnea (OSA) is associated with behavioral, cardiovascular, and metabolic dysfunction. Autonomic changes have also been described in patients with OSA. In this study, we measured the heart rate variability (HRV) on electrocardiogram (ECG) tracings of OSA patients as a measure of cardiac autonomic regulation and putative predictor of disturbed breathing during nighttime. Our findings confirm an altered cardiac autonomic regulation in OSA patients during nighttime and identify a possible role of nighttime HRV in predicting sleep breathing disorders. Obstructive sleep apnea (OSA) can have long-term cardiovascular and metabolic effects. The identification of OSA-related impairments would provide diagnostic and prognostic value. Heart rate variability (HRV) as a measure of cardiac autonomic regulation is a promising candidate marker of OSA and OSA-related conditions. We took advantage of the Physionet Apnea-ECG database for two purposes. First, we performed time- and frequency-domain analysis of nocturnal HRV on each recording of this database to evaluate the cardiac autonomic regulation in patients with nighttime sleep breathing disorders. Second, we conducted a logistic regression analysis (backward stepwise) to identify the HRV indices able to predict the apnea–hypopnea index (AHI) categories (i.e., "Severe OSA", AHI ≥ 30; "Moderate-Mild OSA", 5 ≥ AHI < 30; and "Normal", AHI < 5). Compared to the "Normal", the "Severe OSA" group showed lower high-frequency power in normalized units (HFnu) and higher low-frequency power in normalized units (LFnu). The standard deviation of normal R–R intervals (SDNN) and the root mean square of successive R–R interval differences (RMSSD) were independently associated with sleep-disordered breathing. Our findings suggest altered cardiac autonomic regulation with a reduced parasympathetic component in OSA patients and suggest a role of nighttime HRV in the characterization and identification of sleep breathing disorders. Deep breathing exercises are the second most used complementary health approach in the United States. Two heart rate variability (HRV) parameters, the root mean square of successive differences (RMSSD) and the respiratory sinus arrhythmia (RSA), are used to assess parasympathetic reactivity to deep breathing, but they are often not in agreement. Our purpose was to determine the cause of the disagreement. We investigated HRV parameters in 38 subjects during baseline, deep breathing, and recovery. Here we show that RMSSD as a measure of parasympathetic reactivity is unreliable; it does not reflect the increase in HRV during deep breathing as determined by RSA. We observed a decrease in RMSSD values despite a marked increase in HRV as determined by RSA and the standard deviation of normal heartbeat interval (SDNN) in healthy subjects and patients with functional bowel disorders. We show that RSA captures all aspects of HRV, whereas successive differences in heart rate intervals are only a small part of HRV, with decreasing variability during deep breathing in most subjects. We present a new measure of calculating RSA during deep breathing that may become an essential tool for researchers and clinicians. We also provide a unique visualization of the increased heart rate variability during deep breathing. Hence, RMSSD cannot be used to assess parasympathetic reactivity during deep breathing; using RSA is recommended. The use of RMSSD in previous influential studies may have led to erroneous conclusions about parasympathetic reactivity during deep breathing. Purpose: This systematic review aimed to evaluate the effect of transcutaneous auricular vagus nerve stimulation on heart rate variability and baroreflex sensitivity in healthy populations. Method: PubMed, Scopus, the Cochrane Library, Embase, and Web of Science were systematically searched for controlled trials that examined the effects of transcutaneous auricular vagus nerve stimulation on heart rate variability parameters and baroreflex sensitivity in apparently healthy individuals. Two independent researchers screened the search results, extracted the data, and evaluated the quality of the included studies. Results: From 2458 screened studies, 21 were included. Compared with baseline measures or the comparison group, significant changes in the standard deviation of NN intervals, the root mean square of successive RR intervals, the proportion of consecutive RR intervals that differ by more than 50 ms, high-frequency power, low-frequency to high-frequency ratio, and low-frequency power were found in 86%, 75%, 69%, 47%, 36%, and 25% of the studies evaluating the effects of transcutaneous auricular vagus nerve stimulation on these indices, respectively. Baroreflex sensitivity was evaluated in six studies, of which a significant change was detected in only one. Some studies have shown that the worse the basic autonomic function, the better the response to transcutaneous auricular vagus nerve stimulation. Conclusion: The results were mixed, which may be mainly attributable to the heterogeneity of the study designs and stimulation delivery dosages. Thus, future studies with comparable designs are required to determine the optimal stimulation parameters and clarify the significance of autonomic indices as a reliable marker of neuromodulation responsiveness. Objective. Most current algorithms for detecting atrial fibrillation (AF) rely on heart rate variability (HRV), and only a few studies analyse the variability of photopletysmography (PPG) waveform. This study aimed to compare morphological features of the PPG curve in patients with AF to those presenting a normal sinus rhythm (NSR) and evaluate their usefulness in AF detection. Approach. 10 min PPG signals were obtained from patients with persistent/paroxysmal AF and NSR. Nine morphological parameters (1/Δ T), Pulse Width [PW], augmentation index [AI], b/a, e/a, [b-e]/a, crest time [CT], inflection point area [IPA], Area and five HRV parameters (heart rate [HR], Shannon entropy [ShE], root mean square of the successive differences [RMSSD], number of pairs of consecutive systolic peaks [ R – R ] that differ by more than 50 ms [NN50], standard deviation of the R – R intervals [SDNN]) were calculated. Main results. Eighty subjects, including 33 with AF and 47 with NSR were recruited. In univariate analysis five morphological features (1/Δ T, p < 0.001; b/a, p < 0.001; [b-e]/a, p < 0.001; CT, p = 0.011 and Area, p < 0.001) and all HRV parameters (p = 0.01 for HR and p < 0.001 for others) were significantly different between the study groups. In the stepwise multivariate model (Area under the curve [AUC] = 0.988 [0.974–1.000]), three morphological parameters (PW, p < 0.001; e/a, p = 0.011; (b-e)/a, p < 0.001) and three of HRV parameters (ShE, p = 0.01; NN50, p < 0.001, HR, p = 0.01) were significant. Pulmonary vascular diseases (PVDs), defined as arterial or chronic thromboembolic pulmonary hypertension, are associated with autonomic cardiovascular dysregulation. Resting heart rate variability (HRV) is commonly used to assess autonomic function. Hypoxia is associated with sympathetic overactivation and patients with PVD might be particularly vulnerable to hypoxia-induced autonomic dysregulation. In a randomised crossover trial, 17 stable patients with PVD (resting PaO2 ≥ 7.3 kPa) were exposed to ambient air (FiO2 = 21%) and normobaric hypoxia (FiO2 = 15%) in random order. Indices of resting HRV were derived from two nonoverlapping 5–10-min three-lead electrocardiography segments. We found a significant increase in all time- and frequency-domain HRV measures in response to normobaric hypoxia. There was a significant increase in root mean squared sum difference of RR intervals (RMSSD; 33.49 (27.14) vs. 20.76 (25.19) ms; p < 0.01) and RR50 count divided by the total number of all RR intervals (pRR50; 2.75 (7.81) vs. 2.24 (3.39) ms; p = 0.03) values in normobaric hypoxia compared to ambient air. Both high-frequency (HF; 431.40 (661.56) vs. 183.70 (251.25) ms2; p < 0.01) and low-frequency (LF; 558.60 (746.10) vs. 203.90 (425.63) ms2; p = 0.02) values were significantly higher in normobaric hypoxia compared to normoxia. Heart rate variability (HRV) changes were investigated by several studies after resective epilepsy surgery/vagus nerve stimulation. We examined anterior thalamic nucleus (ANT)-deep brain stimulation (DBS) effects on HRV parameters. We retrospectively analyzed 30 drug-resistant epilepsy patients' medical record data and collected electrocardiographic epochs recorded during video- electroencephalography monitoring sessions while awake and during N1- or N2-stage sleep pre-DBS implantation surgery, post-surgery but pre-stimulation, and after stimulation began. The mean square root of the mean squared differences between successive RR intervals and RR interval standard deviation values differed significantly (p < 0.05) among time-points, showing increased HRV post-surgery. High (0.15–0.4 Hz) and very low frequency (<0.04 Hz) increased, while low frequency (0.04–0.15 Hz) and the LF/HF ratio while awake decreased, suggesting improved autonomic regulation post-surgery. Change of effect size was larger in patients where both activated contacts were located in the ANT than in those where only one or none of the contacts hit the ANT. In patients with drug-resistant epilepsy, ANT-DBS might positively influence autonomic regulation, as reflected by increased HRV. This article aimed to synthesize the various triggers of the diving response and to perform a meta‐analysis assessing their effects on cardiac vagal activity. The protocol was preregistered on PROSPERO (CRD42021231419; 01.07.2021). A systematic and meta‐analytic review of cardiac vagal activity was conducted, indexed with the root mean square of successive differences (RMSSD) in the context of the diving response. The search on MEDLINE (via PubMed), Web of Science, ProQuest and PsycNet was finalized on November 6th, 2021. Studies with human participants were considered, measuring RMSSD pre‐ and during and/or post‐exposure to at least one trigger of the diving response. Seventeen papers (n = 311) met inclusion criteria. Triggers examined include face immersion or cooling, SCUBA diving, and total body immersion into water. Compared to resting conditions, a significant moderate to large positive effect was found for RMSSD during exposure (Hedges' g = 0.59, 95% CI 0.36 to 0.82, p <.001), but not post‐exposure (g = 0.11, 95% CI −0.14 to 0.36, p =.34). Among the considered moderators, total body immersion had a significantly larger effect than forehead cooling (QM = 23.46, df = 1, p <.001). No further differences were detected. Limitations were the small number of studies included, heterogenous triggers, few participants and low quality of evidence. Further research is needed to investigate the role of cardiac sympathetic activity and of the moderators. In the first meta‐analysis on this topic, the diving response has been found to be moderately to largely effective in increasing cardiac vagal activity during exposure. Face immersion and total body immersion caused stronger effects than forehead cooling and should be further investigated as potential interventions in psychotherapy and stress management in the work place. Summary: Obstructive sleep apnea is a common sleep breathing disorder related to autonomic nervous function disturbances. Heart rate variability is an important non‐invasive indicator of autonomic nervous system function. The PubMed, Embase, Medline and Web of Science databases were systematically searched for English literature comparing patients with obstructive sleep apnea with controls up to May 2021. Heart rate variability outcomes, including integrated indices (parasympathetic function and total variability), time domain indices (the standard deviation of NN intervals and the root mean square of the successive differences between normal heartbeats) and frequency domain indices (high‐frequency, low‐frequency, very‐low‐frequency and the ratio of low‐frequency to high‐frequency) were derived from the studies. Twenty‐two studies that included 2565 patients with obstructive sleep apnea and 1089 healthy controls were included. Compared with controls, patients with obstructive sleep apnea exhibited significantly reduced parasympathetic function. For the obstructive sleep apnea severity subgroup meta‐analysis, patients with severe obstructive sleep apnea had significantly lower parasympathetic function, high‐frequency, root mean square of the successive differences between normal heartbeats and standard deviation of NN intervals, and higher low‐frequency and ratios of low‐frequency to high‐frequency. However, only the ratio of low‐frequency to high‐frequency was significantly higher in patients with moderate obstructive sleep apnea than in controls. Finally, for the collection time analysis, patients with obstructive sleep apnea had significantly higher low‐frequency and ratio of low‐frequency to high‐frequency at night, significantly lower parasympathetic function, high‐frequency, root mean square of the successive differences between normal heartbeats and standard deviation of NN intervals, and a higher ratio of low‐frequency to high‐frequency during the day than controls. Autonomic function impairment was more serious in patients with severe obstructive sleep apnea. During sleep, low‐frequency can well reflect the impairment of autonomic function in obstructive sleep apnea, and the ratio of low‐frequency to high‐frequency may play an important role in obstructive sleep apnea diagnosis. Summary: Heart rate variability (HRV) can be used to assess changes in output of the parasympathetic nervous system (PNS). Considering that patients with post‐traumatic stress disorder (PTSD) often experience disturbances in sleep, arousal, and autonomic functioning, we sought to explore the association of PNS activity during sleep with hyperarousal symptoms of PTSD. Because a broad literature supports the importance of rapid eye movement (REM) sleep in PTSD, REM‐sleep features were specifically examined as predictors of PTSD symptom severity. A total of 90 participants, primarily civilian and female, aged 18–40 years who had experienced a traumatic event in the last 2 years, underwent an ambulatory polysomnography (PSG) acclimation night followed by a second PSG night from which sleep physiological measures were computed. PTSD severity was measured using the PTSD Checklist for the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (PCL‐5). Dependent variables were total PCL‐5 score as well as its hyperarousal symptom subscore. Predictors included REM latency, percentage, density, segment length, and an index of parasympathetic tone (root mean square of the successive differences in the R–R interval or RMSSD). Hierarchical regression models were conducted to analyse the association of REM features with PCL‐5 total and hyperarousal subscales. Using hierarchical regression, REM‐sleep RMSSD accounted for a significant proportion of the variation in outcome variables, even when accounting for other REM‐sleep features. The present findings support hypothesised relationships between PTSD symptomatology and REM‐sleep physiology and, specifically, that lowered parasympathetic tone in REM may be an important associate of the hyperarousal symptom cluster in PTSD. Background and Aims: Single‐use electrocardiography (ECG) leads have been developed to reduce healthcare‐associated infection. This study compared the validity and reliability of short‐term heart rate variability (HRV) obtained from single‐use disposable ECG leads. Methods: Thirty healthy subjects (33 ± 10 years; 9 females) underwent 5‐min resting HRV assessments using disposable (single use) ECG cable and wire system (Kendall DL™ Cardinal Health) and a standard, reusable ECG leads (CardioExpress, Spacelabs Healthcare). Results: Intraclass correlation coefficient (ICC) with 95% confidence interval (CI) between disposable and reusable ECG leads was for the time domain [R‐R interval (ms); 0.99 (0.91, 1.00)], the root mean square of successive normal R‐R interval differences (RMSSD) (ms); 0.91 (0.76, 0.96), the SD of normal‐to‐normal R‐R intervals (SDNN) (ms); 0.91 (0.68, 0.97) and frequency domain [low‐frequency (LF) normalized units (nu); 0.90 (0.79, 0.95), high frequency (HF) nu; 0.91 (0.80, 0.96), LF power (ms2); 0.89 (0.62, 0.96), HF power (ms2); 0.90 (0.72, 0.96)] variables. The mean difference and upper and lower limits of agreement between disposable and reusable leads for time‐ and frequency‐domain variables were acceptable. Analysis of repeated measures using disposable leads demonstrated excellent reproducibility (ICC 95% CI) for R‐R interval (ms); 0.93 (0.85, 0.97), RMSSD (ms); 0.93 (0.85, 0.97), SDNN (ms); 0.88 (0.75, 0.95), LF power (ms2); 0.87 (0.72, 0.94), and HF power (ms2); 0.88 (0.73, 0.94) with coefficient of variation ranging from 2.2% to 5% (p > 0.37 for all variables). Quantifying cognitive workload, i.e., the level of mental effort put forth by an individual in response to a cognitive task, is relevant for healthcare, training and gaming applications. However, there is currently no technology available that can readily and reliably quantify the cognitive workload of an individual in a real-world environment at a seamless way and affordable price. In this work, we overcome these limitations and demonstrate the feasibility of a magnetocardiography (MCG) sensor to reliably classify high vs. low cognitive workload while being non-contact, fully passive and low-cost, with the potential to have a wearable form factor. The operating principle relies on measuring the naturally emanated magnetic fields from the heart and subsequently analyzing the heart rate variability (HRV) matrix in three time-domain parameters: standard deviation of RR intervals (SDRR); root mean square of successive differences between heartbeats (RMSSD); and mean values of adjacent R-peaks in the cardiac signals (MeanRR). A total of 13 participants were recruited, two of whom were excluded due to low signal quality. The results show that SDRR and RMSSD achieve a 100% success rate in classifying high vs. low cognitive workload, while MeanRR achieves a 91% success rate. Tests for the same individual yield an intra-subject classification accuracy of 100% for all three HRV parameters. Future studies should leverage machine learning and advanced digital signal processing to achieve automated classification of cognitive workload and reliable operation in a natural environment. This study examined the effect of maturation on parasympathetic nervous system (PNS) response from rest to light- to moderate-intensity exercise and recovery from maximal exercise in pre- (n = 10; maturity offset = −3.0 ± 1.2 years; age = 10.1 ± 1.9 years), mid- (n = 9; maturity offset = −0.1 ± 0.6 years; age = 13.7 ± 1.0 years), and postpubertal (n = 10; maturity offset = 1.9 ± 0.6 years; age = 15.6 ± 1.2 years) boys and men (n = 10; age = 24.1 ± 2.0 years). Participants completed seated rest, light-intensity exercise (50% HRmax), and moderate-intensity exercise (65% HRmax). Following moderate-intensity exercise, intensity was ramped to elicit maximal HR and followed by 25 min of seated recovery. Log transformed values for root mean square of successive differences (lnRMSSD), high-frequency power (lnHF) and normalized HF power (lnHFnu) assessed PNS modulation during 3 min of rest, light-intensity exercise, moderate-intensity exercise, and 3-min epochs throughout recovery. During light-intensity exercise, lnRMSSD and lnHF were greater in prepubertal (lnRMSSD = 3.4 ± 0.3 ms; lnHF = 5.4 ± 0.7 ms2) compared to men (lnRMSSD = 2.8 ± 0.5 ms; lnHF = 4.0 ± 0.9 ms2). Emerging evidence suggests that COVID-19 may affect cardiac autonomic function; however, the limited findings in young adults with COVID-19 have been equivocal. Notably, symptomology and time since diagnosis appear to influence vascular health following COVID-19, but this has not been explored in the context of cardiac autonomic regulation. Therefore, we hypothesized that young adults who had persistent symptoms following COVID-19 would have lower heart rate variability (HRV) and cardiac baroreflex sensitivity (BRS) compared with those who had COVID-19 but were asymptomatic at testing and controls who never had COVID-19. Furthermore, we hypothesized that there would be relationships between cardiac autonomic function measures and time since diagnosis. We studied 27 adults who had COVID-19 and were either asymptomatic (ASYM; n = 15, 6 females); 21 ± 4 yr; 8.4 ± 4.0 wk from diagnosis) or symptomatic (SYM; n = 12, 9 females); 24 ± 3 yr; 12.3 ± 6.2 wk from diagnosis) at testing, and 20 adults who reported never having COVID-19 (24 ± 4 yr, 11 females). Heart rate and beat-to-beat blood pressure were continuously recorded during 5 min of rest to assess HRV and cardiac BRS. HRV [root mean square of successive differences between normal heartbeats (RMSSD); control, 73 ± 50 ms; ASYM, 71 ± 47 ms; and SYM, 84 ± 45 ms; P = 0.774] and cardiac BRS (overall gain; control, 22.3 ± 10.1 ms/mmHg; ASYM, 22.7 ± 12.2 ms/mmHg; and SYM, 24.3 ± 10.8 ms/mmHg; P = 0.871) were not different between groups. However, we found correlations with time since diagnosis for HRV (e.g., RMSSD, r = 0.460, P = 0.016) and cardiac BRS (overall gain, r = 0.470, P = 0.014). These data suggest a transient impact of COVID-19 on cardiac autonomic function that appears mild and unrelated to persistent symptoms in young adults. Objectives: Pre-diabetes represents the initial stage of type 2 diabetic disease. This study aimed to highlight the importance of the 6-month integrated approach of yoga therapy (IAYT) on the time domain and frequency domain of heart rate variability (HRV) in pre-diabetic subjects. Method: The study was conducted on 250 pre-diabetic adults aged 30–50 years attending a tertiary care hospital. The patients were divided into two groups: The study group (n = 125) was subjected to the IAYT and the control group (n = 125) was not engaged in yoga therapy. Biochemical parameters such as blood glucose, glycated hemoglobin, and the time domain and frequency domain of HRV were recorded at baseline and after 6 months of yoga intervention. Results: Participants had a mean age of 45.4 ± 6.4 years, the post-yoga intervention resulted in a significant decline in blood glucose, glycated hemoglobin, and significantly decreased frequency domain parameters low frequency (LF), and LF/high-frequency ratio (LF/HF ratio), and significantly increased in high frequency (HF) and in timedomain parameter standard deviation of normal-normal (NN) interval, mean percentage of differences higher than 50 ms in RR intervals (pRR50), and root mean square of successive differences between normal heartbeat (RMSSD). The number of pairs of successive NN (R-R) intervals that differ by more than 50 ms (NN50) significantly increased. Conclusion: The study clearly indicates that after 6 months of yoga intervention, autonomic nervous system shifted toward parasympathetic dominance, which was assessed by time domain and frequency domain parameters of HRV. However, the recovery dynamics of RR intervals (RRI) and HRV after a maximal incremental exercise test have been studied less in non-athletic population and especially in females. The main aim of the study was to examine the recovery dynamics of RRI and HRV immediately after a maximal cardiopulmonary exercise test (CPET) in adult females.Twenty-eight healthy, novice recreational female runners (33.7 ± 4.2 years) were selected to participate in this study from a larger group of participants (Run RCT). RRIs were collected continuously during a controlled rest, a CPET on a treadmill and a 15-min controlled, passive recovery. Minute-by-minute recovery dynamics were analyzed.The dynamic phase of RRI and heart rate recovery (HRR) (from 192.2 ± 9.9 to 111.4 ± 2.4, P < 0.001) occurred during the first 4.5 min with HRR of 36 ± 10 beats during the first minute of the recovery. Depression in children and adolescents has gradually attracted social attention. Heart rate variability (HRV) has been found to be influenced by depression severity, but results have not been uniformed in children and adolescents. This study investigated the relationship between depression severity and heart rate variability in children and adolescents, aiming to provide additional evidence for an objective, effective, and convenient depression screening tool in this population. Literature searching was conducted in China National Knowledge Infrastructure (CNKI), Wanfang Data, Web of Science, PubMed, ScienceDirect, and EBSCO. Relevant studies investigating the relationship between depression severity and HRV in children and adolescents were selected for meta-analysis. 31 articles were included in this meta-analysis, involving 4534 participants. Depression severity in children and adolescents was significantly negatively correlated with high frequency (HF) and root mean square of successive differences (RMSSD) in HRV (HF: r = −0.10, 95% CI: −0.17 to −0.04, p = 0.001; RMSSD: r = −0.18, 95% CI: −0.30 to −0.05, p = 0.01). The relationship between HF and depression severity was moderated by age, higher among those aged >12 than among those aged <12 (r = −0.17, −0.02, Q = 7.32, p = 0.007). Rates of alcohol use disorder (AUD) are increasing among civilian and veteran populations of women in the United States, and stress pathophysiology (i.e., abnormal acute and long-term change in physiological responses to stress) is central to themaintenance of alcoholmisuse within this population. Heart rate variability (HRV) is onemeasure of stress regulation thatmay help to explain the association of stress with alcoholmisuse among women. In the current analysis of pilot data, 20 women veterans attended an in-person laboratory session and completed 35 daily assessments of their alcohol use and craving. During the lab session, the effects of a stress induction procedure on self-reported alcohol craving and HRV were assessed. HRV was continuously measured and indexed in the time domain, using the root mean square of successive differences between normal heartbeats (RMSSD). Alcohol craving and use during the longitudinal 35-day study period were measured via self-report questionnaires sent to participants' phones. Results indicated that resting HRV in the lab was positively associated with odds of daily craving. Moreover, HRV during the stressor, as measured in lab, was positively associated with (1) overall alcohol craving in the lab (i.e., with resting and post-stress craving), and (2) number of daily drinks during the 35-day study period. This pilot study suggests the potential role of HRV in response to stressors in predicting alcohol craving and use among female veterans.
