Magnetic resonance imaging (MRI) is a versatile modality that can generate high-resolution images with a variety of tissue contrasts. However, MRI is a slow technique and requires long acquisition times, which increase with higher temporal and spatial resolution and/or when multiple contrasts and large volumetric coverage is required. In order to speedup MR data acquisition, several approaches have been introduced in the literature. Most of these techniques acquire less data than required and exploit intrinsic redundancies in the MR images to recover the information that was not sampled. This article presents a review of MR acquisition and reconstruction methods that have exploited redundancies in the temporal, spatial, and contrast/parametric dimensions to accelerate image data acquisition, focusing on cardiac and abdominal MR imaging applications. The review describes how each of these dimensions has been separately exploited for speeding up MR acquisition to then discuss more advanced techniques where multiple dimensions are exploited together for further reducing scan times. Purpose: Develop a novel 2D cardiac MR fingerprinting (MRF) approach to enable simultaneous T1, T2, T2*, and fat fraction (FF) myocardial tissue characterization in a single breath-hold scan., Methods: Simultaneous, co-registered, multi-parametric mapping of T1, T2, and FF has been recently achieved with cardiac MRF. Here, we further incorporate T2* quantification within this approach, enabling simultaneous T1, T2, T2*, and FF myocardial tissue characterization in a single breath-hold scan. T2* quantification is achieved with an eight-echo readout that requires a long cardiac acquisition window. A novel low-rank motion-corrected (LRMC) reconstruction is exploited to correct for cardiac motion within the long acquisition window. The proposed T1/T2/T2*/FF cardiac MRF was evaluated in phantom and in 10 healthy subjects in comparison to conventional mapping techniques., Results: The proposed approach achieved high quality parametric mapping of T1, T2, T2*, and FF with corresponding normalized RMS error (RMSE) T1 = 5.9%, T2 = 9.6% (T2 values <100 ms), T2* = 3.3% (T2* values <100 ms), and FF = 0.8% observed in phantom scans. In vivo, the proposed approach produced higher left-ventricular myocardial T1 values than MOLLI (1148 vs 1056 ms), lower T2 values than T2-GraSE (42.8 vs 50.6 ms), lower T2* values than eight-echo gradient echo (GRE) (35.0 vs 39.4 ms), and higher FF values than six-echo GRE (0.8 vs 0.3 %) reference techniques. The proposed approach achieved considerable reduction in motion artifacts compared to cardiac MRF without motion correction, improved spatial uniformity, and statistically higher apparent precision relative to conventional mapping for all parameters., Conclusion: The proposed cardiac MRF approach enables simultaneous, co-registered mapping of T1, T2, T2*, and FF in a single breath-hold for comprehensive myocardial tissue characterization, achieving higher apparent precision than conventional methods., (© 2022 The Authors. Background: Cardiac magnetic resonance (MR) images are often collected with different imaging parameters, which may impact the calculated values of myocardial radiomic features., Purpose: To investigate the sensitivity of myocardial radiomic features to changes in imaging parameters in cardiac MR images., Study Type: Prospective., Population: A total of 11 healthy participants/five patients., Field Strength/ Sequence: A 3 T/cine balanced steady-state free-precession, T 1 -weighted spoiled gradient-echo, T 2 -weighted turbo spin-echo, and quantitative T 1 and T 2 mapping. For each sequence, the flip angle, in-plane resolution, slice thickness, and parallel imaging technique were varied to study the sensitivity of radiomic features to alterations in imaging parameters., Assessment: Myocardial contours were manually delineated by experienced readers, and a total of 1023 radiomic features were extracted using PyRadiomics with 11 image filters and six feature families., Statistical Tests: Sensitivity was defined as the standardized mean difference (D effect size), and the robust features were defined at sensitivity < 0.2. Sensitivity analysis was performed on predefined sets of reproducible features. The analysis was performed using the entire cohort of 16 subejcts., Results: 64% of radiomic features were robust (sensitivity < 0.2) to changes in any imaging parameter. In qualitative sequences, radiomic features were most sensitive to changes in in-plane spatial resolution (spatial resolution: 0.6 vs. flip angle: 0.19, parallel imaging: 0.18, slice thickness: 0.07; P < 0.01 for all); in quantitative sequences, radiomic features were least sensitive to changes in spatial resolution (spatial resolution: 0.07 vs. slice thickness: 0.16, flip angle: 0.24; P < 0.01 for all). Cardiac magnetic resonance (CMR) imaging is an effective method for noninvasively imaging the heart which in the last two decades impressively enhanced spatial and temporal resolution and imaging speed, broadening its spectrum of applications in cardiovascular disease. CMR imaging techniques are designed to noninvasively assess cardiovascular morphology, ventricular function, myocardial perfusion, tissue characterization, flow quantification and coronary artery disease. These intrinsic features yield CMR suitable for diagnosis, follow-up and longitudinal monitoring after treatment of cardiovascular diseases. The aim of this paper is to review the technical basis of CMR, from cardiac imaging planes to cardiac imaging sequences.

Objective: To test whether the method utilizing three-dimensional (3D) whole-heart MRI has an additional benefit over that utilizing conventional two-dimensional (2D) cine MRI in quantifying ventricular volumes., Materials and Methods: In 110 patients with congenital heart disease, a navigator-gated, 3D whole-heart MRI during end-systole (ES) and end-diastole (ED), 2D short-axis cine MRI, and phase contrast MRI of the great arteries were acquired. Ventricular volumes were measured by using a 3D threshold-based segmentation for 3D whole-heart MRI and by using a simplified contouring for 2D cine MRI. The cardiac trigger delays of 3D whole-heart MRI were compared with those of a 2D cine MRI. The stroke volumes calculated from the ventricular volumes were compared with the arterial flow volumes, measured by phase contrast MRI., Results: The ES and ED trigger delays of whole-heart MRI were significantly less than cine MRI for both the left ventricle (-16.8 ± 35.9 ms for ES, -59.0 ± 90.4 ms for ED; p < 0.001) and the right ventricle (-58.8 ± 30.6 ms for ES, -104.9 ± 92.7 ms for ED; p < 0.001). Compared with the arterial flow volumes, 2D cine MRI significantly overestimated the left ventricular stroke volumes (8.7 ± 8.9 mL, p < 0.001) and the 3D whole-heart MRI significantly underestimated the right ventricular stroke volumes (-22.7 ± 22.9 mL, p < 0.001)., Conclusion: Three-dimensional whole-heart MRI is often subject to early timing of the ED phase, potentially leading to the underestimation of the right ventricular stroke volumes.

Purpose: T 1 mapping is an emerging MRI research tool to assess diseased myocardial tissue. Recent research has been focusing on the image acquisition protocol and motion correction, yet little attention has been paid to the curve fitting algorithm., Methods: After nonrigid registration of the image series, a vectorized Levenberg-Marquardt (LM) technique is proposed to improve the robustness of the curve fitting algorithm by allowing spatial regularization of the parametric maps. In addition, a region-based initialization is proposed to improve the initial guess of the T 1 value. The algorithm was validated with cardiac T 1 mapping data from 16 volunteers acquired with saturation-recovery (SR) and inversion-recovery (IR) techniques at 3T, both pre- and post-injection of a contrast agent. Signal models of T 1 relaxation with 2 and 3 parameters were tested., Results: The vectorized LM fitting showed good agreement with its pixel-wise version but allowed reduced calculation time (60 s against 696 s on average in Matlab with 256 × 256 × 8(11) images). Increasing the spatial regularization parameter led to noise reduction and improved precision of T 1 values in SR sequences. The region-based initialization was particularly useful in IR data to reduce the variability of the blood T 1 ., Conclusions: We have proposed a vectorized curve fitting algorithm allowing spatial regularization, which could improve the robustness of the curve fitting, especially for myocardial T 1 mapping with SR sequences., (Copyright © 2018 Elsevier Ltd. Quantitative cardiovascular Magnetic Resonance Imaging techniques are gaining wide acceptance within the MR community due to their potential to diagnose non-localized disease, guide therapy and improve patient outcome. During the last decade, there has been an increasing interest for developing new techniques that allow for simultaneous quantification of both T1 and T2 maps of myocardium. Newer studies demonstrated that the incorporation of MRI simulations could yield similar results to conventional mapping techniques in the myocardium. However, these simulation-based quantitative MR techniques usually compare the in-vivo T1 estimates against less accurate T1 techniques, whereas they present inconsistencies between simulation studies, phantom and in-vivo measurements. Moreover, these studies do not investigate the effect of Magnetization Transfer on the myocardial T1 and T2 estimates but are usually validated on phantoms where the MT effect is small. The main aim of this study was to perform simultaneous mapping of the native T1 and T2 of the myocardium through the utilization of a modified MOLLI pulse sequence and the incorporation of advanced MR simulations through the SQUAREMR framework. A second aim of this study was to investigate the effect of MT on simulation-based quantitative MR techniques. A conventional MOLLI pulse sequence was modified so as to present combined high T2 sensitivity and low MT effect. The new technique was applied in healthy volunteers and demonstrated an improved T1 accuracy compared to the conventional MOLLI and a T2 accuracy similar to the one provided by the T2prep-bSSFP method. The effect of MT on T1 and T2 estimates was also investigated in the current study. Phantoms with an increasing MT effect as well as phantoms without an MT effect were included in this work whereas several variants of the modified-MOLLI that introduce different amounts of T2 modulation on the MR signal and induce different MT effects were applied on the phantoms. The proposed simulation-based quantitative MR technique for simultaneous T1 and T2 mapping of the myocardium does not require the incorporation of a complicated custom designed pulse sequence and does not require a complicated reconstruction workflow. Background: We aimed at developing and testing a nearly automated method for breath-hold artifacts compensation in short-axis (SA) cardiac magnetic resonance (CMR) images. The purpose was the reduction of potential misalignment between standard cine SA and two- and four-chamber long-axis (LA) CMR images to allow 3D reconstruction for segmentation or modeling purposes., Methods: The 3D position of each SA image was optimized on the basis of the pixel intensities at the intersections with the two- and four-chamber LA images. The algorithm accuracy was first tested on a dedicated virtual phantom dataset, derived from a high resolution computed tomography frame where known misalignments were applied. The method was then applied to SA and LA CMR end-diastolic and end-systolic frames datasets obtained in 20 consecutive patients. Assessment of the results was performed by two independent observers by visual comparison and by quantifying the residual distances between LA and SA left ventricle endocardial contours before and after correction., Results: Errors on the simulation dataset were quantified as residual distance from the ground truth position of SA planes and values were found of the order of the pixel resolution. On CMR datasets, a perceived improvement was reported in about 70% of the slices in need for correction and median residual error between manual SA and LA contours was reduced from 2.4mm to 1.8mm., Discussion: RESULTS found on virtual and clinical datasets proved feasibility and usefulness of the method as a necessary pre-processing step for volumetric analysis of CMR data in clinical setting., (Copyright © 2014 Elsevier Ltd. Purpose: To improve compressed sensing (CS) reconstruction of accelerated breath-hold (BH) radial cine magnetic resonance imaging (MRI) by exploiting auxiliary data acquired between different BHs., Materials and Methods: Cardiac function is usually assessed using segmented cine acquisitions over multiple BHs to cover the entire left ventricle (LV). Subjects are given a resting period between adjacent BHs, when conventionally no data are acquired and subjects rest in the scanner. In this study the resting periods between BHs were used to acquire additional free-breathing (FB) data, which are subsequently used to generate a sparsity constraint for each cardiac phase. Images reconstructed using the proposed sparsity constraint were compared with conventional CS using a composite image generated by averaging different cardiac phases. The efficacy of the proposed reconstruction was compared using indices of LV function and blood-myocardium sharpness., Results: The proposed method provided accurate LV ejection fraction measurements for 33% and 20% sampled datasets compared with fully sampled reference images, and showed 14% and 11% higher blood-myocardium border sharpness scores compared to the conventional CS., Conclusion: The FB data acquired during resting periods can be efficiently used to improve the image quality of the undersampled BH data without increasing the total scan time., (Copyright © 2013 Wiley Periodicals, Inc.)

Purpose: Our aim is to assess the potential of an MR system with ultrahigh performance gradients (200 mT/m maximum gradient strength) to address two interrelated challenges in cardiac DTI: low SNR and sensitivity to bulk motion. Methods: Imaging was performed in 20 healthy volunteers, two patients, and one swine post‐myocardial infarction. The impact of maximum gradient strength was assessed with spin echo cardiac DTI featuring second‐order motion compensation and varying maximum system gradient strengths (40, 80, 200 mT/m). Motion compensation requirements at 200 mT/m were assessed with sequences featuring zeroth‐, first‐, and second‐order motion compensation. SNR, mean diffusivity, fractional anisotropy, helix angle transmurality, and secondary eigenvector angle in the left ventricle were compared. Results: Increasing maximum system gradient strength from 40 and 80 mT/m to 200 mT/m increased SNR of b = 500 s/mm2 images by 150% and 40% due to reductions in TE. Observed improvements in DTI metrics included reduction in variance in mean diffusivity and helix angle transmurality across healthy volunteers, improved visualization of myocardial borders and delineation of suspected scar. Whereas second‐order motion compensation acquisitions were robust to motion‐induced signal dropout, zeroth‐ and first‐order motion compensation acquisitions suffered from severe signal loss and localized signal voids, respectively. Conclusion: Ultrahigh performance gradients (200 mT/m) enable high SNR DWIs of the heart and resultant improvements in diffusion tensor metrics. Background: Coronavirus 2019 (COVID‐19) causes morbidity and mortality in an increasing number of people worldwide. Although it mainly affects the respiratory system, it influences all organs, including the heart. It is associated with a broad spectrum of widespread cardiovascular problems ranging from mild myocardial injury to fulminant myocarditis. We aimed to evaluate the presence and prevalence of cardiac involvement in asymptomatic or symptomatic patients after they recovered from COVID 19 infection. Methods: A total of 100 consecutive patients with COVID‐19 proven by reverse transcription polymerase chain reaction (RT‐PCR), under 40 years of age and without any known additional chronic diseases were analyzed retrospectively for cardiac magnetic resonance (CMR) results and symptoms. Results: Cardiac involvement was detected in 49 out of 100 patients on CMR imaging. In the cardiac involvement group, the number of patients with chest pain and/or dyspnea was 41 (84%), which was statistically significant (p = 0.001). Twenty‐four patients (47%) in the without cardiac involvement group were asymptomatic and this was also statistically significant (p = 0.001). LV ejection fraction was statistically significantly lower in the group with cardiac involvement (61% vs 66%, p = 0.001). LV stroke volume and tricuspid annular plane systolic excursion (TAPSE) were statistically significantly lower in patients with cardiac involvement (p = 0.028 and p = 0.019, respectively). Conclusion: Based on single center experience, myocardial involvement is common in symptomatic patients after COVID‐19. Cardiovascular magnetic resonance (CMR) has become a widely adapted imaging modality in the diagnosis and management of patients with cardiovascular diseases. It provides unparalleled data of cardiac function and myocardial morphology. Majority of CMR imaging is currently being performed on 1.5 Tesla (T) MR systems. Over the last many years, the cardiac imaging protocols have been standardized and optimized in the 1.5T systems. 3T MR systems are now being used more and more in small and large institutions in our country due to their proven advantages in the field of neuro, body, and musculoskeletal imaging. Cardiac imaging on 3T system can be a double-edged sword. On one hand, it may provide nondiagnostic images due to significant artifacts, and on the other hand, it may complete the examination in quick time and provide excellent quality images. It is therefore important for the user to be aware of the potential pitfalls of CMR in 3T systems and also the necessary steps to avoid them. In this study, we discuss various challenges and advantages of performing CMR in a 3T system. We also present potential technical solutions to improve the image quality. [ABSTRACT FROM AUTHOR]

Background: Heart failure related to cardiac siderosis remains a major cause of death in transfusion dependent anaemias. Replacement fibrosis has been reported as causative of heart failure in siderotic cardiomyopathy in historical reports, but these findings do not accord with the reversible nature of siderotic heart failure achievable with intensive iron chelation. Methods: Ten whole human hearts (9 beta-thalassemia major, 1 sideroblastic anaemia) were examined for iron loading and fibrosis (replacement and interstitial). Five had died from heart failure, 4 had cardiac transplantation for heart failure, and 1 had no heart failure (death from a stroke). Heart samples iron content was measured using atomic emission spectroscopy. Interstitial fibrosis was quantified by computer using picrosirius red (PSR) staining and expressed as collagen volume fraction (CVF) with normal value for left ventricle <3%. Results: The 9 hearts affected by heart failure had severe iron loading with very low T2* of 5.0 ± 2.0 ms (iron concentration 8.5 ± 7.0 mg/g dw) and diffuse granular myocardial iron deposition. In none of the 10 hearts was significant macroscopic replacement fibrosis present. In only 2 hearts was interstitial fibrosis present, but with low CVF: in one patient with no cardiac siderosis (death by stroke, CVF 5.9%) and in a heart failure patient (CVF 2%). In the remaining 8 patients, no interstitial fibrosis was seen despite all having severe cardiac siderosis and heart failure (CVF 1.86% ±0.87%). Conclusion: Replacement cardiac fibrosis was not seen in the 9 post-mortem hearts from patients with severe cardiac siderosis and heart failure leading to death or transplantation, which contrasts markedly to historical reports. Minor interstitial fibrosis was also unusual and very limited in extent. These findings accord with the potential for reversibility of heart failure seen in iron overload cardiomyopathy. Furthermore, to study the correlation between cardiac morphology and other patient information within a large population, it is necessary to automatically generate cardiac mesh models of each subject within the population. In this study, we introduce MCSI-Net (Multi-Cue Shape Inference Network), where we embed a statistical shape model inside a convolutional neural network and leverage both phenotypic and demographic information from the cohort to infer subject-specific reconstructions of all four cardiac chambers in 3D. In this way, we leverage the ability of the network to learn the appearance of cardiac chambers in cine cardiac magnetic resonance (CMR) images, and generate plausible 3D cardiac shapes, by constraining the prediction using a shape prior, in the form of the statistical modes of shape variation learned a priori from a subset of the population. This, in turn, enables the network to generalise to samples across the entire population. To the best of our knowledge, this is the first work that uses such an approach for patient-specific cardiac shape generation. MCSI-Net is capable of producing accurate 3D shapes using just a fraction (about 23% to 46%) of the available image data, which is of significant importance to the community as it supports the acceleration of CMR scan acquisitions. Abstract: Objective: Delayed myocardial enhancement is caused by a variety of cardiovascular diseases. The extent of the enhanced area has been examined by the inversion recovery (IR) method, whereby at the inversion time (TI), normal myocardium shows a low signal intensity. In this sequence, as pericardial fat shows a very high intensity, a delayed enhancement just below the pericardium may be indistinct. To improve the accuracy of delayed myocardial enhancement, we employed the spectral presaturation of inversion recovery (SPIR) method. Materials and Methods: Thirty-five patients with symptoms of cardiovascular disease aged between 36 and 80 years old (mean age, 62 years old) were investigated. Thirty were men and five were women. Inversion recovery and SPIR images were obtained 25 min after initial administration of a gadolinium-based contrast material. Each TI, when the signal intensity of the normal myocardium was null, was determined by images obtained at serial different TIs. A radiologist and a cardiologist examined each image by a consensus reading. The extent of myocardial enhancement was described as none, subendocardial, transmural and a random pattern in each case. Images were ranked over three levels and were based on whether myocardial enhancement could be easily detected or whether the contour of the myocardium was visualized precisely. Student''s t-test was conducted to compare the quality of two sequences in all patients and in 22 patients who showed delayed myocardial enhancement. Results: The imaging quality in evaluating delayed myocardial enhancement in all patients was superior with IR compared with SPIR, although it was not statistically significant. The imaging quality in the patients with delayed myocardial enhancement was similar between SPIR and IR. SPIR was superior to the IR sequence in two of the four patients who exhibited transmural enhancement. Conclusion: SPIR exhibited equivalent image quality to IR in evaluating delayed myocardial enhancement. As it has the potential advantage in patients with rich adipose tissue surrounding the myocardium, it can be an alternative sequence to evaluate myocardial viability. [Copyright &y& Elsevier]

Our purpose was to assess image quality and interpretation problems of two-dimensional (2D) coronary MR angiograms. The coronary arteries of 27 subjects (12 normal volunteers and 15 patients) were evaluated with 2D coronary MR angiography (MRA). Coronary MRA was performed with a fat-suppressed electrocardiographically gated breath-hold gradient-echo sequence with k-space segmentation using a 1.5-T imager. Image quality throughout the study was occasionally degraded by: image ghosting (22%), ringing (19%), and/or blurring (22%) and incomplete fat-suppression (19%). Intermittent difficulties with breathholding were encountered in 44% of subjects. When limiting the analysis to those images with optimal image quality, interpretative difficulties were sometimes found: misregistration due to inconsistent breathholding (37%); difficulty in distinguishing veins from arteries (37%); obscured anatomy due to overlapping structures (26%); and poor visualization of portions of the left main coronary artery (59%). Two-dimensional coronary MRA studies have image quality and interpretive problems which need to be understood and addressed before routine clinical scanning is initiated. [ABSTRACT FROM AUTHOR]

Purpose: The objective of this study was to investigate whether three dimentional (3D)- Coronary CT angiography (CCTA)- feature tracking (FT) can measure global myocardial strain of the left ventricle (LV) in patients with heart failure using cardiac MR (CMR) as reference.Methods: Consecutive patients (n = 44) with variable degrees of heart failure who underwent an ECG-gated CCTA and CMR within 24 h were included. Both modalities were compared for 2D/3D LV global radial strain (2D/3D-GRS), circumferential strain (2D/3D-GCS), longitudinal strain (2D/3D-GLS) and conventional functional parameters.Results: Compared to CMR, CCTA-derived 3D-GLS and LVEF showed no significant difference (p > 0.05). Bland-Altman plots showed a small bias (0.3 %) between CCTA-derived 3D-GLS and CMR 3D-GLS. Close correlations were observed between the two modalities regarding LV global strain (3D-GRS, r = 0.89; 3D-GCS, r = 0.86; 3D-GLS, r = 0.79, respectively, p < 0.001 for all). However, CCTA-derived 3D-GRS and 3D-GCS were statistically different compared with CMR. CCTA-derived 3D-GLS had an inverse correlation with CCTA-LVEF(r=-0.75, p < 0.05). Intraobserver agreements for CCTA-derived 3D-global strain were good (ICC = 0.856 for 3D-GLS, ICC = 0.741 for 3D-GCS and ICC = 0.762 for 3D-GRS). 2D global strain showed statistical differences between the two modalities (p＜0.05 for all), but close correlations were observed regarding 2D LV global strain (2D-GRS, r = 0.80; 2D-GCS, r = 0.81; 2D-GLS, r = 0.81, respectively, p < 0.001 for all). The average radiation dose-long-product (DLP) of CCTA was 387.86 ± 89.3 mGy*cm.Conclusion: CCTA-derived 3D-GLS can provide both reliable and interchangeable results for quantitative assessment of myocardial mechanical changes in HF patients compared to CMR with good intra-observer agreement.
