Sounds we hear in our daily life contain changes in the acoustic features (e.g., frequency, intensity, and duration or "what" information) and/or changes in location ("where" information). The purpose of this study was to examine the cortical auditory evoked potentials (CAEPs) to the change within a stimulus, the acoustic change complex (ACC), in frequency (F) and location (L) of the sound in normal hearing listeners. Fifteen right-handed young normal hearing listeners participated in the electroencephalographic (EEG) recordings. The acoustic stimuli were pure tones (base frequency at 250 Hz) of 1 s, with a perceivable change either in location (L, 180°), frequency (F, 5% and 50%), or both location and frequency (L+F) in the middle of the tone. Additionally, the 250 Hz tone of 1 sec without any change was used as a reference. The participants were asked to listen passively to the stimuli and not to move their heads during the testing. Compared to the reference tone, by which only the onset-CAEP was elicited, the tones containing changes (L, F, or L+F) elicited both onset-CAEP and the ACC. The waveform analysis of ACCs from the vertex electrode (electrode Cz) showed that, larger sound changes evoked larger peak amplitudes [e.g., (L+50%F)- > L-change; (L+50%F)- > 5%F-change] and shorter the peak latencies ([(L+5%F)- < 5%F-change; 50%F- < 5%F-change; (L+50%F)- < 5%F-change] . The current density patterns for the ACC N1' peak displayed some differences between L-change vs. F-change, supporting different cortical processing for "where" and "what" information of the sound; regardless of the nature of the sound change, larger changes evoked a stronger activation than smaller changes [e.g., L- > 5%F-change; (L+5%F)- > 5%F-change; 50%F- > 5%F-change] in frontal lobe regions including the cingulate gyrus, medial frontal gyrus (MFG), superior frontal gyrus (SFG), the limbic lobe cingulate gyrus, and the parietal lobe postcentral gyrus. The results suggested that sound change-detection involves memory-based acoustic comparison (the neural encoding for the sound change vs. neural encoding for the pre-change stimulus stored in memory) and involuntary attention switch., (Copyright © 2020 University of Cincinnati. In bimodal listening, cochlear implant (CI) users combine electric hearing (EH) in one ear and acoustic hearing (AH) in the other ear. In electric-acoustic stimulation (EAS), CI users combine EH and AH in the same ear. In quiet, integration of EH and AH has been shown to be better with EAS, but with greater sensitivity to tonotopic mismatch in EH. The goal of the present study was to evaluate how external noise might affect integration of AH and EH within or across ears. Recognition of monosyllabic words was measured for normal-hearing subjects listening to simulations of unimodal (AH or EH alone), EAS, and bimodal listening in quiet and in speech-shaped steady noise (10 dB, 0 dB signal-to-noise ratio). The input/output frequency range for AH was 0.1-0.6 kHz. EH was simulated using an 8-channel noise vocoder. The output frequency range was 1.2-8.0 kHz to simulate a shallow insertion depth. The input frequency range was either matched (1.2-8.0 kHz) or mismatched (0.6-8.0 kHz) to the output frequency range; the mismatched input range maximized the amount of speech information, while the matched input resulted in some speech information loss. In quiet, tonotopic mismatch differently affected EAS and bimodal performance. In noise, EAS and bimodal performance was similarly affected by tonotopic mismatch. Differences in current spread and/or spread of excitation may differently affect integration of acoustic and electric hearing within (electric-acoustic stimulation, or EAS) or across ears (bimodal). In this study, vowel recognition was measured in normal-hearing subjects listening to simulations of cochlear implant (CI), EAS, and bimodal listening. Residual acoustic hearing was limited between 0.1 and 0.6 kHz. The carrier bandwidth in the CI simulations was varied across carriers: broad-band noise, narrow-band noise, and sine waves. Results showed that reducing the bandwidth (and the inherent noise fluctuations) in the CI simulations significantly affected CI-only and bimodal performance, but not EAS performance.

Advances in cochlear implant (CI) technology allow for acoustic and electric hearing to be combined within the same ear (electric-acoustic stimulation, or EAS) and/or across ears (bimodal listening). Integration efficiency (IE; the ratio between observed and predicted performance for acoustic-electric hearing) can be used to estimate how well acoustic and electric hearing are combined. The goal of this study was to evaluate factors that affect IE in EAS and bimodal listening. Vowel recognition was measured in normal-hearing subjects listening to simulations of unimodal, EAS, and bimodal listening. The input/output frequency range for acoustic hearing was 0.1-0.6 kHz. For CI simulations, the output frequency range was 1.2-8.0 kHz to simulate a shallow insertion depth and the input frequency range was varied to provide increasing amounts of speech information and tonotopic mismatch. Performance was best when acoustic and electric hearing was combined in the same ear. IE was significantly better for EAS than for bimodal listening; IE was sensitive to tonotopic mismatch for EAS, but not for bimodal listening. These simulation results suggest acoustic and electric hearing may be more effectively and efficiently combined within rather than across ears, and that tonotopic mismatch should be minimized to maximize the benefit of acoustic-electric hearing, especially for EAS.

Objective: To evaluate the use of monosyllabic word recognition versus sentence recognition to determine candidacy and long-term benefit for cochlear implantation., Study Design: Prospective multi-center single-subject design., Methods: A total of 21 adults aged 18 years and older with bilateral moderate to profound sensorineural hearing loss and low monosyllabic word scores received unilateral cochlear implantation. The consonant-nucleus-consonant (CNC) word test was the central measure of pre- and postoperative performance. Additional speech understanding tests included the Hearing in Noise Test sentences in quiet and AzBio sentences in +5 dB signal-to-noise ratio (SNR). Quality of life (QoL) was measured using the Abbreviated Profile of Hearing Aid Benefit and Health Utilities Index., Results: Performance on sentence recognition reached the ceiling of the test after only 3 months of implant use. In contrast, none of the participants in this study reached a score of 80% on CNC word recognition, even at the 12-month postoperative test interval. Measures of QoL related to hearing were also significantly improved following implantation., Conclusion: Results of this study demonstrate that monosyllabic words are appropriate for determining preoperative candidate and measuring long-term postoperative speech recognition performance., Level of Evidence: 2c. Objectives: Speech materials validated with normal-hearing listeners may not be appropriate for clinical assessment of cochlear implant (CI) users. The aim of this study was to validate list equivalency of the Mandarin Speech Perception (MSP) sentences, disyllables, and monosyllables in Mandarin-speaking CI patients., Design: Recognition of MSP sentences, disyllables, and monosyllables each were measured for all 10 lists., Study Sample: 67 adult and 32 pediatric Mandarin-speaking CI users., Results: There was no significant difference between adult and pediatric subject groups for all test materials. Significant differences were observed among lists within each test. After removing one or two lists within each test, no significant differences were observed among the remaining lists. While there was equal variance among lists within a given test, the variance was larger for children than for adults, and increased from monosyllables to disyllables to sentences., Conclusions: Some adjustment to test lists previously validated with CI simulations was needed to create perceptually equivalent lists for real CI users, suggesting that test materials should be validated in the targeted population. Differences in mean scores and variance across test materials suggest that CI users may differ in their ability to make use of contextual cues available in sentences and disyllables.

Objectives: Cochlear implant listeners are able to at least partially adapt to the spectral mismatch associated with the implant device and speech processor via daily exposure and/or explicit training. The overall goal of this study was to investigate interactions between short-term unsupervised learning (i.e., passive adaptation) and the degree of spectral mismatch in normal-hearing listeners' adaptation to spectrally shifted vowels., Design: Normal-hearing subjects were tested while listening to acoustic cochlear implant simulations. Unsupervised learning was measured by testing vowel recognition repeatedly over a 5 day period; no feedback or explicit training was provided. In experiment 1, subjects listened to 8-channel, sine-wave vocoded speech. The spectral envelope was compressed to simulate a 16 mm cochlear implant electrode array. The analysis bands were fixed and the compressed spectral envelope was linearly shifted toward the base by 3.6, 6, or 8.3 mm to simulate different insertion depths of the electrode array, resulting in a slight, moderate, or severe spectral shift. In experiment 2, half the subjects were exclusively exposed to a severe shift with 8 or 16 channels (exclusive groups), and half the subjects were exposed to 8-channel severely shifted speech, 16-channel severely shifted speech, and 8-channel moderately shifted speech, alternately presented within each test session (mixed group). The region of stimulation in the cochlea was fixed (16 mm in extent and 15 mm from the apex) and the analysis bands were manipulated to create the spectral shift conditions. To determine whether increased spectral resolution would improve adaptation, subjects were exposed to 8- or 16-channel severely shifted speech., Results: In experiment 1, at the end of the adaptation period, there was no significant difference between 8-channel speech that was spectrally matched and that shifted by 3.6 mm. There was a significant, but less-complete, adaptation to the 6 mm shift and no adaptation to the 8.3 mm shift. In experiment 2, for the mixed exposure group, there was significant adaptation to severely shifted speech with 8 channels and even greater adaptation with 16 channels. For the exclusive exposure group, there was no significant adaptation to severely shifted speech with either 8 or 16 channels., Conclusions: These findings suggest that listeners are able to passively adapt to spectral shifts up to 6 mm. For spectral shifts beyond 6 mm, some passive adaptation was observed with mixed exposure to a smaller spectral shift, even at the expense of some low frequency information. Mixed exposure to the smaller shift may have enhanced listeners' access to spectral envelope details that were not accessible when listening exclusively to severely shifted speech. The results suggest that the range of spectral mismatch that can support passive adaptation may be larger than previously reported. Some amount of passive adaptation may be possible with severely shifted speech by exposing listeners to a relatively small mismatch in conjunction with the severe mismatch.

To better represent fine structure cues in cochlear implants (CIs), recent research has proposed varying the stimulation rate based on slowly varying frequency modulation (FM) information. The present study investigated the abilities of CI users to detect FM with simultaneous amplitude modulation (AM). FM detection thresholds (FMDTs) for 10-Hz sinusoidal FM and upward frequency sweeps were measured as a function of standard frequency (75-1000 Hz). Three AM conditions were tested, including (1) No AM, (2) 20-Hz Sinusoidal AM (SAM) with modulation depths of 10%, 20%, or 30%, and (3) Noise AM (NAM), in which the amplitude was randomly and uniformly varied over a range of 1, 2, or 3 dB, relative to the reference amplitude. Results showed that FMDTs worsened with increasing standard frequencies, and were lower for sinusoidal FM than for upward frequency sweeps. Simultaneous AM significantly interfered with FM detection; FMDTs were significantly poorer with simultaneous NAM than with SAM. Besides, sinusoidal FMDTs significantly worsened when the starting phase of simultaneous SAM was randomized. These results suggest that FM and AM in CI partly share a common loudness-based coding mechanism and the feasibility of "FM+AM" strategies for CI speech processing may be limited.

This experiment examined the effects of spectral resolution and fine spectral structure on recognition of spectrally asynchronous sentences by normal-hearing and cochlear implant listeners. Sentence recognition was measured in six normal-hearing subjects listening to either full-spectrum or noise-band processors and five Nucleus-22 cochlear implant listeners fitted with 4-channel continuous interleaved sampling (CIS) processors. For the full-spectrum processor, the speech signals were divided into either 4 or 16 channels. For the noise-band processor, after band-pass filtering into 4 or 16 channels, the envelope of each channel was extracted and used to modulate noise of the same bandwidth as the analysis band, thus eliminating the fine spectral structure available in the full-spectrum processor. For the 4-channel CIS processor, the amplitude envelopes extracted from four bands were transformed to electric currents by a power function and the resulting electric currents were used to modulate pulse trains delivered to four electrode pairs. For all processors, the output of each channel was time-shifted relative to other channels, varying the channel delay across channels from 0 to 240 ms (in 40-ms steps). Within each delay condition, all channels were desynchronized such that the cross-channel delays between adjacent channels were maximized, thereby avoiding local pockets of channel synchrony. Results show no significant difference between the 4- and 16-channel full-spectrum speech processor for normal-hearing listeners. Recognition scores dropped significantly only when the maximum delay reached 200 ms for the 4-channel processor and 240 ms for the 16-channel processor. When fine spectral structures were removed in the noise-band processor, sentence recognition dropped significantly when the maximum delay was 160 ms for the 16-channel noise-band processor and 40 ms for the 4-channel noise-band processor. There was no significant difference between implant listeners using the 4-channel CIS processor and normal-hearing listeners using the 4-channel noise-band processor. The results imply that when fine spectral structures are not available, as in the implant listener's case, increased spectral resolution is important for overcoming cross-channel asynchrony in speech signals.

Noise was traditionally regarded as a nuisance, which should be minimized if possible. However, recent research has shown that addition of an appropriate amount of noise can actually improve signal detection in a nonlinear system, an effect called stochastic resonance. While stochastic resonance has been described in a variety of physical and biological systems, its functional significance in human sensory systems remains mostly unexplored. Here we report psychophysical data showing that signal detection and discrimination can be enhanced by noise in human subjects whose hearing is evoked by either normal acoustic stimulation or electric stimulation of the auditory nerve or the brainstem. Our results suggest that noise is an integral part of the normal sensory process and should be added to auditory prostheses.

Current multichannel cochlear implant devices provide high levels of speech performance in quiet. However, performance deteriorates rapidly with increasing levels of background noise. The goal of this study was to investigate whether the noise susceptibility of cochlear implant users is primarily due to the loss of fine spectral information. Recognition of vowels and consonants was measured as a function of signal-to-noise ratio in four normal-hearing listeners in conditions simulating cochlear implants with both CIS and SPEAK-like strategies. Six conditions were evaluated: 3-, 4-, 8-, and 16-band processors (CIS-like), a 6/20 band processor (SPEAK-like), and unprocessed speech. Recognition scores for vowels and consonants decreased as the S/N level worsened in all conditions, as expected. Phoneme recognition threshold (PRT) was defined as the S/N at which the recognition score fell to 50% of its level in quiet. The unprocessed speech had the best PRT, which worsened as the number of bands decreased. Recognition of vowels and consonants was further measured in three Nucleus-22 cochlear implant users using either their normal SPEAK speech processor or a custom processor with a four-channel CIS strategy. The best cochlear implant user showed similar performance with the CIS strategy in quiet and in noise to that of normal-hearing listeners when listening to correspondingly spectrally degraded speech. These findings suggest that the noise susceptibility of cochlear implant users is at least partly due to the loss of spectral resolution. Efforts to improve the effective number of spectral information channels should improve implant performance in noise.

It is widely assumed that the proper transformation of acoustic amplitude to electric amplitude is a critical factor affecting speech recognition in cochlear implant users and normal-hearing listeners. A four-channel noise-band speech processor was implemented, reducing spectral information to four bands. A power-law transformation was applied to the amplitude mapping stage in the speech processor design, and the exponent of the power function varied from a strongly compressive (p = 0.05) to a weakly compressive (p = 0.75) for implant listeners and from 0.3 to 3.0 for acoustic listeners. Results for implants showed that the best performance was achieved with an exponent of about 0.2, and performance gradually deteriorated when either more compressive or less compressive exponents were applied. The loudness growth functions of the four activated electrodes in each subject were measured and those data were well fit by a power function with a mean exponent of 2.72. The results indicated that the best performance was achieved when the normal loudness growth was restored. For acoustic listeners, results were similar to those observed with cochlear implant listeners, except that best performance was achieved with no amplitude nonlinearity (p = 1.0). The similarity of results in both acoustic and electric stimulation indicated that the performance deterioration observed for extreme nonlinearity was due to similar perceptual effects. The function relating amplitude mapping exponent and performance was relatively flat, indicating that phoneme recognition was only mildly affected by amplitude nonlinearity.
