Cells of our body are constantly exposed to events that can create lesions in the DNA. If unremoved by the DNA repair mechanisms, the lesion may lead to a mutational event. Some of these events might drive the transformation of a healthy cell to a malignant one. This thesis focuses on understanding the generation of mutations and their positive selection in somatic tissues. First, we study the coding and noncoding driver landscape of more than 2500 patients. We also attempt to distinguish driver and passenger mutations in driver genes, and link them to mutational processes. We then study the mutations related to chemotherapies in both tumoral and healthy cells, as well as the effects of treatment in clonal hematopoiesis (CH). We also perform a systematic discovery of CH driver genes. Lastly, we study the interplay of DNA damage and repair in nucleosome-bound DNA, uncovering a 10bp periodic mutation rate which might have influenced the genome composition in eukarya. Les cèl·lules del nostre cos estan constantment exposades a esdeveniments que poden lesionar l’ADN. Si aquestes lesions no són eliminades pels mecanismes de reparació, poden causar mutacions. Alguns d’aquests podrien ajudar a transformar una cèl·lula sana en una maligna. Aquesta tesi doctoral té com objectiu estudiar la generació de les mutacions i com són seleccionades positivament als teixits somàtics. Primer, estudiem el panorama de drivers en zones codificant i no codificant en més de 2500 pacients. També intentem distingir quines mutacions són driver de les passenger, i les relacionem amb els processos mutacionals. Després estudiem les mutacions relacionades amb la quimioteràpia en cèl·lules sanes i tumorals, i també l’efecte que tenen a la hematopoesi clonal. D’aquest últim fenomen en descobrim els gens drivers. Per últim, estudiem la relació entre el dany a l’ADN i els mecanismes de reparació quan la molècula s’empaqueta en nucleosomes. Aquesta relació deixa una periodicitat de 10 parells de bases que pot haver influenciat la composició del genoma dels eucariotes.

The rate at which mutations accumulate along the genome is not uniform but influenced by factors such as chromatin compactness, replication time or transcription. Most of these factors create mutational biases that encompass large areas of the genomes, including several megabases. In recent years, though, local mutational asymmetries spanning just a few base pairs have also been identified. This thesis focuses on the study of two of these local mutational asymmetries. First, we describe a reduction in the number of exonic somatic mutations caused by DNA polymerase mismatches, which we attribute to a higher efficacy of the mismatch repair mechanism in these locations. Second we study the UV induced DNA damage formation and repair at transcription factor binding sites and assess the relative contribution of these two factors to the unexpected number of mutations of these areas across transcription factors families. The presence of these local mutation rate variations illustrates the difficulty of properly modeling the mutation rate, an important procedure in many cancer genomics and evolutionary studies. La velocitat a la que les mutacions s’acumulen al llarg del genoma no és uniforme sinó que depèn de diversos factors. Alguns dels més coneguts són l’empaquetament de la cromatina, el moment de replicació o la transcripció. La majoria d’aquests factors creen variacions mutacionals que abarquen grans àrees del genoma, incloent varies megabases. En els últims anys, però, també s’ha identificat variabilitat en el ritme en que s’acumulen les mutacions a escala molt més petita, en regions de poques bases. Aquesta tesi es centra en l’estudi de dos d’aquestes variacions locals en el ritme en que les mutacions tenen lloc. Primer, hem descrit una reducció en el número de mutacions somàtiques en els exons causades per errors de la AND polimerasa, que hem atribuït a una major eficàcia del mecanisme encarregat aquest tipus d’errors en els exons. En segon lloc, hem estudiat com les lesions en el DNA causades per la llum ultraviolada es generen i són reparades als llocs d’unió dels factors de transcripció i hem determinat fins a quin punt cada un d’aquests processos permeten explicar l’inesperat número de mutacions en aquestes regions. La presència d’aquestes variacions locals la velocitat a la que les mutacions s’acumulen al llarg del genoma posen de manifest la dificultat de modelar correctament aquest procés, un procediment central en molts estudis evolutius i de genòmica del càncer.

Cancer is a disease of the genome. The study of tumor genomic alterations is used to guide several precision medicine strategies, some approved and a large number under clinical development. On the other hand, the study of tumor immunity is recently becoming the key for the success of other personalized strategies, named immunotherapies. Along this thesis I have made several contributions towards the advance of cancer precision medicine, based on the study of tumor “omics” data. First, I evinced the landscape of genomic-guided anti-cancer therapies. Second, I developed OncoPaD, a tool for the rational design of cost-effective cancer gene panels. Third, I contributed to the development of Cancer Genome Interpreter, a tool for the biological and therapeutic interpretation of variants found in newly sequenced tumors. Forth, I identified tumor intrinsic molecular mechanisms involved in tumor immune evasion. El càncer és una malaltia del genoma. L'estudi de les alteracions genòmiques dels tumors s’utilitza com a guia en varies estratègies de medicina de precisió, algunes d'elles aprovades i d'altres en assajos clínics. D'altra banda, l’estudi de la immunitat tumoral està esdevenint una peça clau per l’èxit d’altres estratègies terapèutiques, anomenades immunoteràpies. Al llarg d'aquesta tesi, mitjançant l'estudi de les dades “òmiques” tumorals, he contribuït de varies maneres cap a l'avenç de la medicina de precisió pel càncer. Primer, he identificat el panorama de les teràpies anticanceroses guiades per alteracions genòmiques. Segon, he desenvolupat OncoPaD, una eina pel disseny cost-efectiu i racional de panells de seqüenciació per càncer. A més, he contribuït al desenvolupament del Cancer Genome Interpreter, una eina que ajuda a la interpretació biològica i terapèutica de les variants presents a tumors novament seqüenciats. Per últim, he identificat diversos mecanismes mitjançant els quals els tumors són capaços d'evadir l’atac del sistema immunològic.

Given the complexity and heterogeneity of cancer, the development of new high-throughput wide-genome technologies has open new possibilities for its study. Several projects around the globe are exploiting these technologies for generating unprecedented amount of data for cancer genomes. Its analysis, integration and exploration are still a key challenge in the field. In this dissertation, we first present Gitools, a tool for accessing databases in biology, analysing high-throughput data, and visualising multi-dimensional results with interactive heatmaps. Then, we show IntOGen, the methodology employed for collection and organization of the data, the methods used for its analysis, and how the results and analysis were made available to other researchers. Finally, we compare several methods for impact prediction of non-synonymous mutations, showing that new tools specifically designed for cancer outperform those traditionally used for general diseases, and also the need for using other sources of information for better prediction of cancer mutations., Davant de la complexitat i heterogeneitat del cancer, el desenvolupament de noves tecnologies per l'estudi de genomes, ha obert noves posibilitats. Diversos projectes al voltant del mon les fan servir per generar quantitats de dades de genomes de cancer mai vistes abans. En aquest treball, primer presentem Gitools, una eina que permet obtenir dades de bases de dades en biologia, anal itzar dades genomiques, i visual itzar els resul tats multidimensionals mitjançant mapes de calor interactius. Després mostrem IntOGen, les metodologies per obtenir i organitzar les dades, els metodes per el seu analisi, i com es van possar a disposició d'altres investigadors. Finalment, comparem diversos metods de predicció de l'impacte de les mutacions no sinonimes, que ens mostra com nou metods desenvolupats per cancer funcionen millor que els utilitzats tradicionalment per enfermetats generals, aixis com la necesitat de recorrer a altres fonts d'informació per tenir millor prediccions per mutacions de cancer.

Cancer is a complex disease caused by somatic alterations of the genome and epigenome in tumor cells. Increased investments and cheaper access to various technologies have built momentum for the generation of cancer genomics data. The availability of such large datasets offers many new possibilities to gain insight into cancer molecular properties. Within this scope I present two methods that exploit the broad availability of cancer genomic data: OncodriveROLE, an approach to classify mutational cancer driver genes into activating and loss of function mode of actions and MutEx, a statistical measure to assess the trend of the somatic alterations in a set of genes to be mutually exclusive across tumor samples. Nevertheless, the unprecedented dimension of the available data raises new complications for its accessibility and exploration which we try to solve with new visualization solutions: i) Gitools interactive heatmaps with prepared large scale cancer genomics datasets ready to be explored, ii) jHeatmap, an interactive heatmap browser for the web capable of displaying multidimensional cancer genomics data and designed for its inclusion into web portals, and iii) SVGMap, a web server to project data onto customized SVG figures useful for mapping experimental measurements onto the model., El cancer és una malaltia complexa causada per alteracions somàtiques del genoma i epigenoma de les cèl•lules tumorals. Un augment d’inversions i l'accés a tecnologies de baix cost ha provocat un increment important en la generació de dades genòmiques de càncer. La disponibilitat d’aquestes dades ofereix noves possibilitats per entendre millor les propietats moleculars del càncer. En aquest àmbit, presento dos mètodes que aprofiten aquesta gran disponibilitat de dades genòmiques de càncer: OncodriveROLE, un procediment per a classificar gens “drivers” del càncer segons si el seu mode d’acció ésl'activació o la pèrdua de funció del producte gènic; i MutEx, un estadístic per a mesurar la tendència de les mutacions somàtiques a l’exclusió mútua. Tanmateix, la manca de precedents d’aquesta gran dimensió de dades fa sorgir nous problemes en quant a la seva accessibilitat i exploració, els quals intentem solventar amb noves eines de visualització: i) Heatmaps interactius de Gitools amb dades genòmiques de càncer a gran escala, a punt per ser explorades, ii) jHeatmap, un heatmap interactiu per la web capaç de mostrar dades genòmiques de cancer multidimensionals i dissenyat per la seva inclusió a portals web; i iii) SVGMap, un servidor web per traslladar dades en figures SVG customitzades, útil per a la transl•lació de mesures experimentals en un model visual.

Recently, we created IntOGen, a resource to integrate a large amount of cancer genomic data. IntOGen aims at facilitating the detection of the most recurrent alterations that drive tumorigenesis. It collates, annotates and analyzes high-throughput data about transcriptional, genomic and mutational changes taking place in tumors from different studies annotated with specific cancer types. Currently, it contains 118 studies for mRNA expression profiling and 188 studies for genomic alterations covering in total 64 different tumor topographies. In this article, we describe the Biomart portal for IntOGen. The portal provides easy access to different types of data and facilitates the bulk download of all the analysis results. Here, we describe the general features of IntOGen and give example queries to demonstrate its use. Database URL: www.intogen.org.
