Author analyses Fichte’s understanding of knowledge as image that related to his late period of creativity, precisely Doctrine of Science 1813. The appeal to the late period of Fichte’s work is necessary because these texts are little researched. They are also pre-mortal, so they can safely be called a mature work of Fichte’s philosophical project. The study of the late period allows us to destroy most of the stereotypes and deformations that overgrown Fichte’s philosophy until the second half of the twentieth century through dominant Hegelian interpretation. Explaining nature of knowledge, how it is in itself, Fichte tries to dissociate from philosophy of Spinoza at the same time, developing doctrine of the image as a manifestation of the Absolute. The image for him is not a copy of being, but possesses some autonomy, and at the same time, possesses the formative and creative capacity. The Absolute (God), who manifest himself through the image, but is not reduced to him and Fichte has difficulties with explanation how the Absolute is present in his manifestation. The article also investigates the idea of “I” as image of God. Philosopher understands limitation of the explanation of consciousness from the point of the same subject, therefore he appeals to rich metaphorical language, which has an aims to look on consciousness and reflection in different way. In this attempt to explain the nature of knowledge, the last word is not always told by the subject himself. Theological perspective over which the philosopher builds his doctrine of image broadens the understanding of knowledge like image of God., У статті проаналізовано розуміння знання як образу в пізньому періоді творчості Й. Ґ. Фіхте (1762–1814), а саме у його «Вченні про науку» 1813 року. Дослідження зрілого доробку філософського проєкту Фіхте, що є маловивченим, дає змогу зруйнувати більшість стереотипів і викривлень, якими обросла філософія Фіхте аж до другої половини ХХ ст. здебільшого через геґелівську інтерпретацію. Пояснюючи природу знання, а саме яким воно є в собі, Фіхте намагається відмежуватись від філософії Б. Спінози, при цьому розробивши вчення про образ як маніфестацію Абсолюту. Образ для нього не є копією буття, він має певну автономність і, водночас, здатність до формування, творення. Сам Абсолют (Бог) проявляється через образ, але не зводиться до нього. Фіхте має труднощі, коли намагається пояснити, наскільки Абсолют є присутній у своєму прояві. У статті досліджено також Фіхтеву ідею «я» як образу Бога. Філософ розуміє обмеженість тлумачення свідомості з позиції самого суб’єкта, тому вдається до багатої метафоричної мови, яка має на меті в дещо інший спосіб поглянути на свідомість і рефлексію. У цій спробі пояснити природу знання останнє слово не завжди є за самим суб’єктом. Теологічна перспектива, над якою філософ надбудовує своє вчення про образ, розширюється до розуміння знання як образу Бога.

Purpose: To summarize COVID-19 research endeavors by ophthalmologists/researchers in terms of publication numbers, journals and author countries, and to detail key findings. Methods: The LitCovid database was systematically reviewed for ophthalmology-focused COVID-19 articles. The quality of the evidence was assessed for articles investigating conjunctivitis in COVID-19 patients. Results: There were 21,364 articles in LitCovid on June 12, 2020, of which 215 (1%) were ophthalmology-focused. Of articles on COVID-19 transmission, 3.3% were ophthalmology-focused. Ophthalmology-focused articles were published in 68 journals and originated from 25 countries. The top five countries publishing ophthalmology-focused articles (China, India, United States of America, Italy, and United Kingdom) produced 145/215 (67%) articles. A total of 16 case reports/series from eight countries reported that conjunctivitis can be the initial or the only symptom of COVID-19 infection. Conjunctivitis may occur in the middle phase of COVID-19 illness. A total of 10 hospital-based cross-sectional studies reported that between 0% and 31.6% of COVID-19 patients have conjunctivitis or other ocular conditions, with a pooled prevalence of 5.5% reported in a meta-analysis. Viral RNA was detected in conjunctival swabs of patients with and without ocular manifestations, after resolution of conjunctivitis, after nasopharyngeal swabs turned negative and in retina of deceased COVID-19 patients. Conclusion: Within 3 months of declaring the COVID-19 pandemic, 215 ophthalmology-focused articles were published in PubMed, concentrating on disease manifestations and transmission. The reported presence of conjunctivitis or other ocular conditions in COVID-19 patients is varied. Clinicians should be alert for ocular involvement in COVID-19 infections and possible ocular transmission even in patients without ocular symptoms.

The International Carnival of Vrnjci is a manifestation that has been organized for 15 years in Vrnjačka Banja. As a manifestation that gathers a large number of tourists and visitors, besides its positive impacts, attention should be paid to the level of satisfaction of the local population in Vrnjačka Banja with this manifestation, which is the aim of this paper. In addition, the paper examines the existence of a statistically significant difference in the level of local population's satisfaction with this manifestation depending on their different socio-demographic characteristics. A survey method was applied to a sample of 300 respondents. Descriptive statistical analysis, independent sample t-test and one-factor analysis of variance (ANOVA) were used to process the collected responses. The results of the research showed that there is a statistically significant difference in the level of satisfaction with the Carnival of Vrnjci between respondents of different gender, as well as that the level of satisfaction of the respondents influenced by professional status, length of residence in Vrnjacka Banja and the connection between the respondent's job and tourism.

A 53-year-old woman presented with bone pain and was diagnosed with osteomalacia because of hypophosphatemia, hyperphosphatasemia, bone pain, and radiographic findings. Because her intact-fibroblast growth factor 23 (FGF23) levels were high and contrast-enhanced computed tomography revealed a mass in the anterior ethmoid sinus, FGF23-related osteomalacia was diagnosed. The tumor was resected, but she developed hypercalcemia and elevated blood parathyroid hormone (PTH) levels. Primary hyperparathyroidism (PHPT) was diagnosed, and surgical resection was performed. To our knowledge, this is the first case of a FGF23-producing tumor complicated by PHPT. Because PHPT manifested after resecting the FGF23-producing tumor, FGF23 is thus considered to suppress PTH secretion in humans.

Since spring 2016, union organizations, as much as police forces charged with the control and surveillance of the masses of protesters, are being confronted regularly, whether it is in Paris and in other cities around the country, to a congregation of actors which, beyond its social and political heterogeneity, forms in front of the protest what as been designated and popularized by its founders under the name of the “head of the cortège” (“cortège de tête”). The stakes of this article are to make intelligible this wrongly identified object which intrigues, but also to realize the way union officials position themselves in front of this invasive and unruly collective actor, which deeply disturbs the protest order.

Ulcerative colitis, a chronic and recurrent inflammatory disease, is localized to the colonic mucosa but can affect other organs and lead to various complications. Gastroduodenitis associated with ulcerative colitis has been reported. However, little is known about esophageal ulcers. We herein report two rare cases of esophageal ulcers associated with ulcerative colitis. Furthermore, the clinical and histological characteristics of 18 previously reported cases are summarized. This case series and literature review will encourage the accurate diagnosis and treatment of esophageal ulcers associated with ulcerative colitis.

Clown nose-like lesion refers to the manifestation of a reddish or skin-colored bulge on the tip of the nose or the manifestation of bulbous tip of the nose. More and more clinical cases show that clown nose-like lesion can also be the indication of some genetic syndromes, not just the manifestation of metastatic visceral tumor as it initially proposed. However, the clinical features of clown nose-like lesion indicated by metastatic malignancies, genetic cancer predisposition syndromes or primary diseases involving the nasal tip are lacking. In this study, patients with clown nose-like lesion in our clinical practices and from published literatures were collected and reviewed. We found that clown nose-like lesions caused by metastatic malignancies including lung cancer are often solitary and more common in male (24/31) older individuals (average age 62.3, ranging 40–78 years old). In addition, they usually appear for a short time, and are prone to be misdiagnosed as primary nasal diseases, leading to a poor prognosis (all patients with data available died within 4 months). Clown nose-like lesions associated with genetic cancer predisposition syndromes usually develop at a young age (mean age 15.3) with female preference (9/10). They are accompanied by multiple-systemic involvements, including low hair volume, developmental delay, cancer predisposition or neurological diseases. They show slow development and often positive family history (6/10). These two kinds of clown nose-like lesions are often asymptomatic, which delays the diagnosis and treatment of underlying malignancies or syndromes. In brief, the term of clown nose-like lesion is underrecognized, and should be updated. Clown nose-like lesions can serve as indicators to at least three categories of clinical issues: metastatic visceral tumors, genetic syndromes, and primary diseases involving the nasal tip. Increased awareness of clinical features of updated clown nose-like lesions can alert physicians to these underlying malignancies or syndromes, render earlier detection of associated medical issues, and allow for genetic counseling of family members.

Alpha-1-antitrypsin (AAT) and fibrinogen are secretory acute phase reactant proteins. Circulating AAT and fibrinogen are synthesized exclusively in the liver. Mutations in the encoding genes result in conformational abnormalities of the two molecules that aggregate within the rough endoplasmic reticulum (RER) instead of being regularly exported. That results in AAT-deficiency (AATD) and in hereditary hypofibrinogenemia with hepatic storage (HHHS). The association of plasma deficiency and liver storage identifies a new group of pathologies: endoplasmic reticulum storage disease (ERSD).

At the end of December 2019, a new coronavirus denominated Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) was identified in Wuhan, Hubei province, China. Less than three months later, the World Health Organization (WHO) declared coronavirus disease-19 (COVID-19) to be a global pandemic. Growing numbers of clinical, histopathological, and molecular findings were subsequently reported, among which a particular interest in skin manifestations during the course of the disease was evinced. Today, about one year after the development of the first major infectious foci in Italy, various large case series of patients with COVID-19-related skin manifestations have focused on skin specimens. However, few are supported by histopathological, immunohistochemical, and polymerase chain reaction (PCR) data on skin specimens. Here, we present nine cases of COVID-positive patients, confirmed by histological, immunophenotypical, and PCR findings, who underwent skin biopsy. A review of the literature in Italian cases with COVID-related skin manifestations is then provided.

Background: Pandemics are direct antecedent of distinctive physical, psychological, social and financial impacts. A large number of researches are being conducted regarding previous epidemics and pandemics and lot more is currently in progress vis-a-vis COVID-19. The current research is an attempt to explore psychological impacts of COVID-19 specifically to find out the existence, intensity and dynamics of COVID-19 fear in non-clinical educated population. Subjects and methods: A cross sectional online study was conducted with non-clinical educated Pakistani citizens. Self-structured questionnaire comprising close and open ended questions was used for data collection from different cities of Pakistan. N=317 participants (men=121, women=196) were the sample for this study. Demographic information was also sought. The age range of sample was 18 to 50+ years. Most of the participants fall in the category of age group 23-28 of sample. All the participants were educated from Intermediate till PhD but majority of participants had 16 years of education. SPSS 22 was used for quantitative data analysis. Qualitative data were analyzed using thematic analysis and content analysis. Results: Results yield significant age wise and profession wise difference in existence of COVID fear. Nine major themes were extracted regarding nature of fear i.e. Corona Fear, Loss, fear of isolation or quarantine, religion related fear, death, consequences of COVID-19, Under developed country, Psychological component of fear and empathy. Those who denied fear were asked the reasons and six major themes were extracted here i.e Religion, Inevitability of death, Precautions, Belief in self, Myths or misinterpretation of disease and Avoidant approach. Conclusions: Age and profession significantly influenced fear of COVID-19. Gender-wise exploration of themes yields interesting insights. Dysgeusia is the first recognized oral symptom of novel coronavirus disease (COVID‐19). In this review article, we described oral lesions of COVID‐19 patients. We searched PubMed library and Google Scholar for published literature since December 2019 until September 2020. Finally, we selected 35 articles including case reports, case series and letters to editor. Oral manifestations included ulcer, erosion, bulla, vesicle, pustule, fissured or depapillated tongue, macule, papule, plaque, pigmentation, halitosis, whitish areas, hemorrhagic crust, necrosis, petechiae, swelling, erythema, and spontaneous bleeding. The most common sites of involvement in descending order were tongue (38%), labial mucosa (26%), and palate (22%). Suggested diagnoses of the lesions were aphthous stomatitis, herpetiform lesions, candidiasis, vasculitis, Kawasaki‐like, EM‐like, mucositis, drug eruption, necrotizing periodontal disease, angina bullosa‐like, angular cheilitis, atypical Sweet syndrome, and Melkerson‐Rosenthal syndrome. Oral lesions were symptomatic in 68% of the cases. Oral lesions were nearly equal in both genders (49% female and 51% male). Patients with older age and higher severity of COVID‐19 disease had more widespread and sever oral lesions. Lack of oral hygiene, opportunistic infections, stress, immunosuppression, vasculitis, and hyper‐inflammatory response secondary to COVID‐19 are the most important predisposing factors for onset of oral lesions in COVID‐19 patients.

The coronavirus disease 2019 (COVID-19) pandemic, caused by the novel severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), is still underway. An understanding of the virus's mode of transmission and infection is required for its effective containment. Besides the respiratory and digestive tracts, the ocular surface presents an additional mucosal surface that is exposed to infectious droplets and direct/indirect contact. The relationship between SARS-CoV-2 infection and the eye remains controversial. This review examines up-to-date information on ocular manifestation, laboratory testing, transmission, and prevention of COVID-19. Based on clinical observations, the risk of conjunctivitis in COVID-19 is low. Despite the low incidence, positive SARS-CoV-2 results in eye specimens suggest that the ocular surface may harbor SARS-CoV-2, which may increase the infection and transmission risk. We conclude that the ocular surface remains a potential transmission route for the virus that should not be ignored. In addition, the intraocular findings have also been described in COVID-19. The measures for eye and face protection should be widely adopted to stem the tide of the pandemic.

Background 47,XYY is a chromosomal abnormality syndrome that is typically observed in patients with a male phenotype. Few patients with XYY syndrome will have infertility. We here report a case of 46,XY/47,XYY syndrome diagnosed in a patient with a female phenotype. Case presentation A 15-year-old patient with a female phenotype visited our hospital owing to a chief complaint of short stature as of the age of 6 years. She was diagnosed with dwarf syndrome at the age of 10, but no change was noted after 2 months of growth hormone treatment. The patient’s height was 136 cm and the weight was 29 kg, both of which were below the third percentile for her age/gender. In addition to short stature, the 4th and 5th metacarpals were short and there was no significant sex development. Karyotype analysis showed 47,XYY, and chromosomal microarray examination showed a chimera of 46,XY/47,XYY. Conclusion This is an extremely rare case of 47,XYY abnormality in a patient with a female phenotype, with only one such known case reported previously. Since the cause is unknown, and symptoms of this syndrome are highly atypical and variable in childhood, clinicians should be aware of this possibility to avoid misdiagnosis and offer counseling and hormone therapy as needed to patients and their parents to improve their quality of life.

AIM: This study aims to assess the level of awareness about the effect of sickle cell disease (SCD) on the eye and vision and factors influencing SCD awareness. SUBJECTS AND METHODS: The study design was cross-sectional and was carried out in 2018 among the general population in the Eastern province of Saudi Arabia. It was conducted using an online, validated questionnaire, after obtaining consent from the participants. The outcome variable was 557. The level of awareness was correlated to demographic information. RESULTS: The study population was 557 Saudi adults. 84 were male and 473 female. Their mean age was 22 ± 23 years. Majority of the participants (57.3%) were not aware that SCD could affect the eye and vision. There was no difference in the knowledge regarding ocular complications of SCD among different ages and sexes. CONCLUSIONS: The result of this study indicates the need for raising the knowledge regarding the disease, its ocular complications, screening methods, and management. Health educations campaigns would be an effective tool in increasing SCD awareness.

Background Globally, the burden of diabetes mellitus has increased to epidemic proportions. Estimates from the International Diabetes Federation predict that the greatest future increase in the prevalence of diabetes mellitus will occur in Africa. Methods This article reviews literature on the manifestation of diabetes in adult patients in sub-Saharan Africa highlighting the distinct phenotypes, plausible explanations for this unique manifestation and the clinical significance of comprehensively defining and understanding the African diabetes phenotype. Results There are few studies on the manifestation or phenotype of diabetes in Africa. The limited data available suggests that, compared to the Western world, the majority of patients with diabetes in Africa are young and relatively lean in body size. In addition, hyperglycaemia in most cases is characterised by a significantly blunted acute first phase of insulin secretion in response to an oral or intravenous glucose load and pancreatic beta cell secretory dysfunction, rather than peripheral insulin resistance predominates. Genetic and environmental factors like chronic infections/inflammation, early life malnutrition and epigenetic modifications are thought to contribute to these distinct differences in manifestation. Conclusions While published data is limited, there appears to be distinct phenotypes of diabetes in sub-Saharan Africa. Large and more detailed studies are needed especially among newly diagnosed patients to fully characterize diabetes in this region. This will further improve the understanding of manifestation of diabetes and guide the formulation of optimal therapeutic approaches and preventive strategies of the condition on the continent.

In Montreal, homeless people and protesters receive a disproportionate number of statements of offence. These statements of offence are issued by the Montreal police department for the homeless and protesters’ occupation of public spaces (such as sleeping in a park at night or participating in an unlawful assembly). However, those offences are rarely tried and most of the statements of offence are instead withdrawn through prosecutorial discretion, but not before several court hearings. This process seems to indicate that the police court network is departing from the adjudicative ideal to enter a managerial justice paradigm. Punishment seems to happen both at the moment of the arrest and during the pre-trial procedures. In times of managerial justice, the police are punishing certain illegalities and the court is managing marginalized populations. We report a term newborn who presented on day 3 of life with unilateral, tender scrotal swelling with skin discolouration, which was subsequently diagnosed as haemophilia A at about 6 months of age. He received intramuscular vitamin K and hepatitis B vaccine at birth uneventfully. The scrotal swelling was treated as an infected hydrocoele, considering the absence of a family history of bleeding disorder and other risks of bleeding tendency, as well as the ultrasonographic features. He also had congenital pneumonia requiring venepuncture, non-invasive oxygen supplementation and intravenous antibiotics, without any complication. The swelling slowly improved over 2 months. He later developed post-trial vaccine injection haematoma at 4 months of age, and multiple non-traumatic bruises when he was 6 months old, leading to the diagnosis. This case demonstrates an uncommon, but a possible, early manifestation of haemophilia A at birth with a unilateral scrotal haematoma.

Background and Aims: Nephrotoxicity is a rare idiosyncratic reaction to 5-aminosalicylate (5-ASA) therapies. The aims of this study were to describe the clinical features of this complication and identify clinically useful genetic markers so that these drugs can be avoided or so that monitoring can be intensified in high-risk patients.Methods: Inflammatory bowel disease patients were recruited from 89 sites around the world. Inclusion criteria included normal renal function prior to commencing 5-ASA, >= 50% rise in creatinine any time after starting 5-ASA, and physician opinion implicating 5-ASA strong enough to justify drug withdrawal. An adjudication panel identified definite and probable cases from structured case report forms. A genome-wide association study was then undertaken with these cases and 4109 disease controls.Results: After adjudication, 151 cases of 5-ASA-induced nephrotoxicity were identified. Sixty-eight percent of cases were males, with nephrotoxicity occurring at a median age of 39.4 years (range 6-79 years). The median time for development of renal injury after commencing 5-ASA was 3.0 years (95% confidence interval [CI] 2.3-3.7). Only 30% of cases recovered completely after drug withdrawal, with 15 patients requiring permanent renal replacement therapy. A genome-wide association study identified a suggestive association in the HLA region (p = 1 x 10(-7)) with 5-ASA-induced nephrotoxicity. A sub-group analysis of patients who had a renal biopsy demonstrating interstitial nephritis (n = 55) significantly strengthened this association (p = 4 x 10(-9), odds ratio 3.1).Conclusions: This is the largest and most detailed study of 5-ASA-induced nephrotoxicity to date. It highlights the morbidity associated with this condition and identifies for the first time a significant genetic predisposition to drug-induced renal injury.

Coronavirus disease 2019 (COVID-19) is a new viral disease that has become a global public health concern in 2020. This disease is caused by a novel coronavirus firstly detected in the People’s Republic of China. This viral infection can cause febrile illness and respiratory problems. There are other uncommon clinical presentations of COVID-19 such as afebrile illness, diarrhea, non-coughing and confusion. These atypical presentations can result in difficulty in diagnosing the disease. In this short review, the authors specifically discuss the important uncommon atypical presentations of COVID-19.

The understanding about the creation of our universe is explored in many philosophies, natural sciences, religions, ideologies, traditions and many other disciplines. Currently, natural science cannot answer this question at the most fundamental level. In this work, based on the ancient Chinese Tao wisdom about creation, we propose the Law of Tao Yin–Yang Creation. This law states that everything is created from emptiness through yin–yang interaction. Yin and yang are the two basic elements that make up everything. Yin and yang are opposite, relative, co-created, inseparable and co-dependent. The Law of Tao Yin–Yang Creation gives us a deeper insight about space and time. We propose that space and time are two basic measurements we conduct. Time relates to the measurement of movement and change. Space relates to the measurement of stillness and solidity. Space and time are a yin–yang pair. Interaction of two fundamental yin–yang pairs, the space and time yin–yang pair and the inclusion and exclusion duality pair, create our universe. We demonstrate that from this insight, one can derive string theory, superstring or M-theory and the universal wave function interpretation of string theory. We suggest that the Law of Tao Yin–Yang Creation presents the exact process how “it from bit” and it could be the fundamental principle leading to the grand unification theory and the theory of everything.

It is not unusual that narcotics-dependent patients fulfill their medical requirements in the emergency department (ED). The behavior of these patients varies, and their manifestations and predictors are still not fully studied. We performed this retrospective study by prospectively collecting data on patients with suspected drug dependence who were undiagnosed at first and then treated for some kind of reported pain at the ED. Patients who were confirmed to have narcotics dependence were compared with control patients in a ratio of 1:3 matching for age, gender, disease, and clinical diagnoses. From January 2006 to October 2009, 26 of 223 patients treated for pain were found to be drug dependent (12 males and 14 females). The aim of this study was to investigate the clinical presentation, distribution, and drug susceptibility characteristics in liver recipients with A. baumannii infection.Methods: We retrospectively investigated 17 liver recipients who developed A. baumannii infection between January 1, 2007 and December 31, 2014. The distribution of A. baumannii and drug susceptibility characteristics were reviewed.Results: Infectious complications due to A. baumannii appeared in 17 liver recipients, with a total of 24 episodes. Approximately 63% (15/24) of A. baumannii infections occurred within 2 weeks after transplantation. The most common source of infection was multiple culture-positive sites (35.3%, n=6), followed by the intra-abdominal/biliary tract (23.5%, n=4) and lung (23.5%, n=4). Eight patients (47.1%) had a body temperature of 38°C or higher at the onset of A. baumannii infection. IgG4-related sclerosing cholangitis (IgG4-SC) is a rare biliary manifestation in which many other organs might be affected. The purpose of our study was to investigate the different clinical characteristics and initial steroid response between IgG4-SC patients with and without other organs affected. A series of patients with IgG4-SC in the period from January 2006 to December 2015 at our hospital were included. The pancreas and major salivary glands were screened, and the initial corticosteroid therapy was given. Clinical information was collected and analyzed including demographics, clinical presentation, IgG4 serology, imaging features, and treatment outcomes. The study identified 72 IgG4-SC patients, including 60 males and 12 females. The mean age was 59.8 years old. Among these IgG4-SC patients, 10 patients had only bile duct involved, 42 patients had 2 organs involved and 20 patients had multiple organs involved. In patients with multiple organs involved, more complaints were given (mean 2.9 kinds), higher serum IgG4 levels were found (23458 ± 19402.7 mg/L), and more stricture lesions of biliary tract were shown. All 72 patients exhibited a disease response within 4 to 6 weeks of starting steroids. The remission rate in the multiple lesions group was lower (60%), and the recurrence rate is higher (83.3%). Case definitions for European Lyme disease have been published. However, multiple erythema migrans may pose a diagnostic challenge. Therefore, we retrospectively reviewed the clinical and serological findings and response to therapy in a cohort of consecutive 54 patients with PCR-confirmed erythema migrans, referred to a university dermatology clinic. The proportion of patients with multiple erythema migrans lesions (usually 2 or 3) was almost equal (46%) to the proportion of patients with single erythema migrans lesions (54%). All patients, except for 2 multiple erythema migrans patients with a concomitant autoimmune disease, completely responded to treatment. In conclusion, multiple erythema migrans may be more common than anticipated, and since only 50% of the patients were seropositive when seeking medi-cal help, PCR testing of skin lesions is helpful to confirm the diagnosis in clinically atypical cases.

The aim of this paper is to review the literature and identify orofacial manifestations of hematological diseases, with particular reference to anemias and disorders of hemostasis. A computerized literature search using MEDLINE was conducted for published articles on orofacial manifestations of hematological diseases, with emphasis on anemia. Mesh phrases used in the search were: oral diseases AND anaemia; orofacial diseases AND anaemia; orofacial lesions AND anaemia; orofacial manifestations AND disorders of haemostasis. The Boolean operator "AND" was used to combine and narrow the searches. Anemic disorders associated with orofacial signs and symptoms include iron deficiency anemia, Plummer-Vinson syndrome, megaloblastic anemia, sickle cell anemia, thalassaemia and aplastic anemia. The manifestations include conjunctiva and facial pallor, atrophic glossitis, angular stomatitis, dysphagia, magenta tongue, midfacial overgrowth, osteoclerosis, osteomyelitis and paraesthesia/anesthesia of the mental nerve. Orofacial petechiae, conjunctivae hemorrhage, nose-bleeding, spontaneous and post-traumatic gingival hemorrhage and prolonged post-extraction bleeding are common orofacial manifestations of inherited hemostatic disorders such as von Willebrand's disease and hemophilia. A wide array of anemic and hemostatic disorders encountered in internal medicine has manifestations in the oral cavity and the facial region. Most of these manifestations are non-specific, but should alert the hematologist and the dental surgeon to the possibilities of a concurrent disease of hemopoiesis or hemostasis or a latent one that may subsequently manifest itself.

Migrainous headache is reported by patients with photosensitive epilepsy, whereas their relatives complain more often about headache than the relatives of patients with other types of epilepsy. We therefore investigated whether headache itself could be an epileptic symptom related to photosensitivity Four probands with headache and photosensitive epilepsy were selected. Their first-degree family members were studied using video-EEG with extensive intermittent photic stimulation and pattern stimulation. Nine of the 12 subjects (10 female and two male, mean age 30 years, range 14–46 years) proved to be photosensitive with either focal ( n = 5) or generalized ( n = 4) epileptiform discharges. In two subjects an ictal recording of headache occurred after visual stimulation. We found evidence that, in specific patients, headache could be an ictal sign of epilepsy. Photic stimulation during EEG recording can contribute to correct diagnosis and lead to the best care and management of the patient.

Le centenaire du CLG est une belle occasion pour revenir aux sources du principe d’immanence en linguistique et en sémiotique, et pour en suivre le cheminement dans la relecture de Saussure par Hjelmslev, et dans celle de Saussure et Hjelmslev par Greimas.On rappelle notamment pourquoi, chez Saussure et Hjelmslev, l’immanence est indissociable de la définition et de la délimitation d’un objet de connaissance pour une discipline qui s’efforce de circonscrire sa place parmi les disciplines existantes. The centenary of the CGL is a great opportunity to return to the sources of the principle of immanence in linguistics and semiotics, and to follow its progress in Saussure, then in Hjelmslev’s interpretation of Saussure, and finally in Greimas’s interpretation of Saussure and Hjelmslev.We have to remind ourselves in particular why, for Saussure and Hjelmslev, immanence is inseparable from the definition and delimitation of a cognitive object belonging to a discipline that seeks to define its place among the existing disciplines. And we aim primarily to show how, in Saussure, immanence accommodates a permanent invention of semiosis within the sign, and then, in Hjelmslev and especially in Greimas, how it becomes even a creative constraint in the postulated and observed gap between manifestation and immanence, and between generated and attested manifestation.The principle of immanence becomes a scientific strategy that allows Saussure to articulate formality and sociality, while for Hjelmslev it allows for the articulation of generativity and creativity, and for Greimas that of veridiction and invention of semiosis.
