It is important to remember that knee pain can also occur as a projection of the obturator nerve conducting signals from the hip joint. During physical examination the hip joint should be also carefully examined. Diagnostic imaging such as radiographs are helpful in evaluating bone parts, whereas ultrasonography and MRI can reveal pathology in periarticular soft tissue. Laboratory tests can help to rule out a septic process and reveal other associated conditions. The aim The aim of this work wasto analyze the patient’s case of transient synovitis of the hip manifested by knee pain and discuss other hip joint causes of knee pain in the pediatric population. Case report An individual case report of the five-year-old male patient suffered from non-traumatic knee pain and inability to walk with analysis of undertaken diagnostic and treatment management. Results The patient was correctly diagnosed with transient synovitis of the hip joint and successfully treated due to a thorough physical examination and right diagnostic tests carried out by the emergency physician. Conclusions In every case of non-traumatic knee pain, hip pathology should be always ruled out. Carefully performed physical examination, basic laboratory tests and imagining allow practitioners to make the right diagnosis. 2. Dietary Therapies in Pediatric Crohn’s Disease

Abstract: Introduction and Purpose: Crons’ disease is a chronic illness of the digestive system. Its etiology is multifactorial and one of the factors is nutrition. That is why the researchers are trying to labour the valuable types of dietary therapy, which could be used in treatment of pediatric CD. The purpose of our review is to point out the impact of dietary therapy on achieving remission in pediatric Crohn’s disease. Materials and methods: We have reviewed the literature from the PubMed database searching for clinical trials, meta analysis and randomized controlled trials from the past years. The keywords we agreed on offered us the most informative articles and made us hope for the further development of our article. Brief Description Of The State Of Knowledge: Steroid therapy is the principle of CD treatment, but it has a number of side effects that influence the quality of pediatric patients’ life. It has been shown that the use of dietary therapies (EEN, CDED+PEN, SCD) enables the achievement of clinical and biochemical remission, mucosal healing and regulation of dysbiosis. There are many hypotheses explaining this effect, probably the reason of this is the exclusion from the diet of food products that provoke inflammatory processes. Summary: Despite its effectiveness, dietary therapies require motivation and full commitment from patients in order to achieve results. That is why it is so important to conduct further research in this area so that the selected therapy is as acceptable and tolerated by the patient as possible. In addition, education of children and parents, psychological and dietary support are also crucial in the treatment process.

According to the definition of JIA, the disease begins before the age of 16 and lasts more than 6 weeks. The International League of Associations for Rheumatology (ILAR) has divided juvenile idiopathic arthritis into seven categories: systemic, oligoarticular, polyarticular RF (-), polyarticular RF (+), psoriatic, enthesitis-related and undifferentiated arthritis. Currently, the following groups of drugs are used in therapy: nonsteroidal anti–inflammatory drugs, glucocorticosteroids, disease-modifying anti-rheumatic drugs, biologic drugs.Treatment should be based on teamwork, with the participation of a pediatric rheumatologist, orthopedist, psychologist, and physiotherapist. Quick diagnosis and initiation of treatment can prevent the development of irreversible joint damage. Purpose The aim of this review is to present the classification, etiopathogenesis, diagnosis, treatment and complications of juvenile idiopathic arthritis. Each of the chapters has been described in accordance with the latest scientific reports. Methods Literature searches in PubMed, Google Scholarship, and open source books were used to gather information. Results Complex interactions between cells of the immune system (macrophages, lymphocytes, neutrophils, monocytes) are responsible for the pathophysiology of JIA. These interactions indicate the need to divide the disease into clinical subtypes, the heterogeneity of which requires different therapeutic measures. There are many groups of drugs with different mechanisms of action used in the treatment of JIA, including: T lymphocyte inhibitors, anti-TNFα, JAK inhibitors, IL-1 and IL-6 blockers. Despite the great progress and the commitment of scientists, there is still no treatment strategy to completely stop the development of the disease. Conclusions Scientific research conducted around the world has led to the recognition of numerous pathways leading to the formation of the inflammatory process and the symptoms of JIA. Knowledge of these mechanisms allows scientists to conduct research on further drugs, the aim of which is to find a treatment strategy that prevents permanent joint damage, improves treatment results, and enables sustainable remission. It is necessary to expand knowledge about the pathways responsible for the formation of the inflammatory process, the interruption of which would allow complete inhibition of the development of the disease. Keywords:Juvenile idiopathic arthritis, arthritis, pediatric, biological treatment ABSTRAKT Wstęp Młodzieńcze idiopatyczne zapalenie stawów (MIZS) jest najczęstszą przewlekłą chorobą reumatyczną o nieznanej etiologii, która dotyka dzieci. Zgodnie z definicją MIZS choroba zaczyna się przed 16 rokiem życia i trwa dłużej niż 6 tygodni. Międzynarodowa Liga Stowarzyszeń Reumatologicznych (ILAR) podzieliła młodzieńcze idiopatyczne zapalenie stawów na siedem kategorii: układowe, skąpostawowę, wielostawowe RF (-), wielostawowe RF (+), łuszczycowe, związane z zapaleniem przyczepów ścięgnistych i niezróżnicowane. Obecnie w terapii stosuje się następujące grupy leków: niesteroidowe leki przeciwzapalne, glikokortykosteroidy, leki przeciwreumatyczne modyfikujące przebieg choroby, leki biologiczne. Leczenie powinno opierać się na pracy zespołowej, przy udziale reumatologa dziecięcego, ortopedy, psychologa i fizjoterapeuty. Szybka diagnoza i rozpoczęcie leczenia mogą zapobiec rozwojowi nieodwracalnych uszkodzeń stawów. Cel Celem tej pracy przeglądowej jest prezentacja podziału, etiopatogenezy, diagnostyki, leczenia oraz powikłań młodzieńczego idiopatycznego zapalenia stawów. Każdy z rozdziałów został opisany zgodnie z najnowszymi doniesieniami naukowymi. Metody W celu zgromadzenia informacji przeszukano literaturę w bazie PubMed, Googlescholarship oraz ogólnodostępne książki. Wyniki Złożone interakcje między komórkami układu odpornościowego (makrofagi, limfocyty, neutrofile, monocyty) odpowiadają za patofizjologię MIZS. Wspomniane interakcje wskazują na konieczność podziału choroby na podtypy kliniczne, których heterogenność wymaga różnych działań terapeutycznych. Wyróżnia się wiele grup leków o odrębnych mechanizmach działania wykorzystywanych w leczeniu MIZS m.in.: inhibitory limfocytów T, anty-TNFα, inhibitory JAK, blokery IL-1 oraz Il-6. Mimo dużego postępu i zaangażowania naukowców nie istnieje wciąż strategia leczenia pozwalająca całkowicie zahamować rozwój choroby. Wnioski Badania naukowe prowadzone na całym świecie doprowadziły do poznania licznych szlaków prowadzących do powstania procesu zapalnego oraz objawów MIZS. Znajomość wspomnianych mechanizów umożliwia naukowcom prowadzenie badań nad kolejnymi lekami, których celem jest znalezienie strategii leczenia zapobiegającej trwałym uszkodzeniom stawów, poprawiającej wyniki leczenia, umożliwiającej trwałą remisję. Konieczne jest poszerzanie wiedzy na temat szlaków odpowiedzialnych za powstawanie procesu zapalnego, których przerwanie umożliwiałoby całkowite zahamowanie rozwoju choroby. Many methods of treating obesity are available, but they are often not effective enough. Due to the strong connection between psychological factors and the development of obesity, it can be useful to include mindfulness training in treatment. The aim of this study:Summarize current knowledge on the effectiveness of mindfulness training in the treatment of obesity and evaluate the effect of mindfulness on weight loss. Material and method: The review includes publications published in 2012-2022. The search of publications in the Pubmed database was carried out using the following keywords: mindful eating, mindfulness, weight loss, obesity. Description of the stage of the knowledge: The use of mindfulness techniques in the treatment of eating disorders is becoming more popular. A growing number of studies evaluating the effects of mindfulness on weight loss are emerging, but the results are inconclusive. Mindfulness training is effective in improving eating habits and promoting lifestyle changes. Mindfulness training combined with behavioral treatments for obesity has positive effects on weight loss. Summary: Research shows that mindfulness training may be a promising intervention in obesity treatment because it increases self-awareness and promotes lifestyle changes. However, more research is needed to assess the long-term effectiveness of mindfulness for weight control. Keywords: mindful eating, mindfulness, weight loss, obesity.

Szpitalna 16 37-200 Przeworsk, ORCID : 0000-0002-8915-4959, E-mail: dominika.kojder9@gmail.com ABSTRACT According to the World Health Organization (WHO), the rates of caesarean section delivery, which is the most common surgical procedure in gynecology and obstetrics, are increasing globally and amount to 21%. In Poland percentage of caesarean sections (CS) is one of the highest in Europe and amount to 42.2%. The reason for this is the extension of medical indications, but also an increase in the number of CS at the request of pregnant women. Although CS can be a lifesaving procedure, it can also cause many health complications for both - the woman and the child. Numerous studies indicate that caesarean delivery is associated with childhood asthma. Several hypotheses of the pathogenesis of this relationship have been presented. One of them, which is based on the hygiene hypothesis indicates that mode of delivery can cause different bacteria colonization in infants. Lack of contact of fetus with the mother’s vaginal flora during CS labor may cause improper immune system maturation. Another hypothesis is that reduced exposure to stress hormones and mechanical forces during CS labor can indicate infant respiratory complications such as respiratory distress syndrome (RDS) or transient tachypnea of the newborn (TTN). It is suggested that these abnormalities have an impact on asthma development in later life. On the other hand, there are studies which do not confirm that mode of delivery has an influence on the induction of asthma. Due to the significant heterogeneity of studies and unclear risk factors and pathomechanism of the childhood asthma it is impossible to strong confirm the association between caesarean section and asthma developing. 6. Polycystic Ovary Syndrome - increased risk of depression development. Links and risk factors

Abstract: PCOS is a common endocrine disorder affecting up to 6-10% of women in reproductive age. Patients suffer from many manifestations of this disease including symptoms of hyperandrogenism (hirsutism, acne, androgenetic alopecia), ovulation disorders, infertility, overweight and obesity, glucose-insulin homeostasis disorders (insulin resistance, type 2 diabetes). Many studies emphasize the relationship between the presence of PCOS in patients and the appearance of depression. The median incidence of depression in women with this syndrome was 36.6%, while in the group of women without PCOS it was 14.2%. The exact mechanism of this relationship is still unknown, but many factors may play an important role in it, e.g. increased BMI, infertility, high cortisol levels, body image, vitamin D deficiency or elevated inflammation markers.

The disease presents antiphospholipid antibodies such as anticardiolipin antibodies (aCL), lupus anticoagulant (LA) and antiβ2-glycoprotein 1 antibodies (β2GPI). The most common symptoms include thrombosis in veins and arteries and obstretical complications such as early miscarriage, intrauterine fetal death, intrauterine growth restriction (IUGR), placental insufficiency, premature labor and eclampsia. To diagnose a patient with APS certain criteria have been chosen, where at least one clinical and one laboratory criterion must be present. In many cases it takes a lot of time before a proper diagnosis has been made, when a female patient presents obstretical complications. Adequate pharmacological treatment increases the odds of live birth rate from 20-30% to 70-80%. Scientific research shows correlation between antiphospholipid syndrome, infertility and premature ovarian insufficiency. Treatment mostly consists of heparin and low-dose aspirin, in certain cases hydrochloroquine is prescribed. Aside from pharamcological therapy, it is very important to minimize the effects of modifiable risk factors. The following article focuses on complications, diagnosing and therapy in pregnant women suffering from Antiphospholipid syndrome. All sources can be found in Pubmed’s website database. However, only some of them develop psychiatric disorders. The aim of the study is to present the importance of Premenstrual Syndrome and Premenstrual Dysphoric Disorder as important diseases at the edge of gynecology and psychiatry. Many symptoms classified as premenstrual have been described in the literature. Mood swings turn out to be the most important psychological symptom of the disorders whereas bloating is a common somatic manifestation. Despite some difficulties in the establishment of the exact cause, diagnostic criteria for both have been developed and are the basis on which the clinician makes a final diagnosis. The paper also describes the current state of knowledge about the treatment of Premenstrual Syndrome and Premenstrual Dysphoric Disorder. The continuous increase in the importance of non-pharmacological methods such as lifestyle changes and cognitive-behavioral therapy was emphasized. Manners of pharmacological treatment were also described, including SSRIs and hormonal drugs, which should be selected individually, depending on the characteristics of the clinical picture. It is estimated that 15% to 20% of the working population works in shift work, particularly in the healthcare, energy, communication systems, public safety, and hospitality industries. Research indicates that shift work, particularly night work, can lead to health problems such as fatigue, exposure to harmful work environments, increased risk of workplace accidents, and sleep disorders. Night work is also associated with increased risk of hypertension, nervous system dysfunction, cardiovascular dysfunction. Those working at night are also more prone to hormonal disorders, digestive disorders, lowered immunity, and cancer. Sleep-wake cycle disorders, such as excessive sleepiness during waking hours and insomnia at night, are symptoms of shift work intolerance syndrome, which can occur after several months or years of shift work. It is estimated that over 20% of shift workers suffer from sleep-wake cycle disorders, and various factors such as chronotype, age, gender, family and social obligations, medications, medical and psychiatric conditions, and shift work experience can impact tolerance of shift work. The treatment of sleep and wake disorders related to shift work includes planning for main sleep and supplementary naps, appropriate exposure to light, treatment with melatonin, taking sleeping and psychostimulant medications. BCC includes both superficial and nodular lesions with a good prognosis as well as foci that are difficult to treat and require a multidisciplinary approach. There are two basic methods of treating BCC, surgery to remove the tumor and cryosurgery. The paper presents a case of an 82-year-old female patient suffering from BCC in a non-advanced stage of the disease with the presentation of subsequent stages of treatment. Aim of the study An attempt was made to confront an alternative method of BCC treatment by cryosurgery with the first-choice procedure - surgical excision of the tumor with a margin of healthy tissues. Description of the case An individual clinical case study including the patient's medical records. Conclusions The work emphasizes the importance of comprehensive care for a patient diagnosed with BCC. One of the most asked for treatments is a lip filler using hyaluronic acid. The popularity and frequency of the said treatment are directly connected to the rising number of complications in this area. The right diagnosis of lip filler complications allows to implement correct management and therefore therapeutic success for the patient. The aim The aim of the work was to analyze and discuss patient’s complications and implemented medical management after lip filler treatment using hyaluronic acid. Case report An individual case study that included clinical examination, medical and photographic documentation analysis. Results The patient was diagnosed with one of the most common complications after lip filler which is swelling. The swelling was a result of an allergic reaction and was classified as severe. Medical management using corticosteroid and antihistamines was implemented with a good therapeutic outcome. Conclusions Implemented management using corticosteroids and antihistamines for severe swelling after lip filler is effective. Due to the rising number of treatments using soft tissue fillers, it is expected that more patients will present with complications after aesthetic medicine procedures. Keywords: aesthetic medicine; hyaluronic acid; lip filler treatment; allergic reaction ABSTRAKT Wprowadzenie Zabiegi medycyny estetycznej w XXI wieku zyskują coraz większą popularność ze względu na zmieniające się trendy w zakresie piękna oraz chęć spowolnienia procesu starzenia. Jednym z najczęściej wybieranych zabiegów przez pacjentów jest zabieg powiększania ust z użyciem preparatu na bazie kwasu hialuronowego. Popularność i częstotliwość wykonywania tego zabiegu przekłada się na rosnący odsetek powikłań w tym zakresie. Umiejętne rozpoznanie reakcji niepożądanych pozwala na zastosowanie odpowiedniego leczenia i osiągnięcie sukcesu terapeutycznego. Cel pracy Celem pracy było przeanalizowanie i omówienie powikłań oraz zastosowanego leczenia u pacjentki po zabiegu powiększania ust z użyciem preparatu na bazie kwasu hialuronowego. Opis przypadku Studium indywidualnego przypadku klinicznego z uwzględnieniem badania przedmiotowego i podmiotowego, interpretacji dokumentacji medycznej oraz fotograficznej. Wyniki U pacjentki zdiagnozowano jedno z najczęstszych powikłań po zabiegu powiększania ust czyli obrzęk, który następnie zakwalifikowano jako ciężki w przebiegu reakcji alergicznej. Wdrożono odpowiednie leczenie z użyciem glikokortykosteroidu systemowego oraz leków przeciwhistaminowych uzyskując dobry efekt terapeutyczny. Wnioski Wdrożenie leczenia z zastosowaniem glikokortykosteroidu systemowego oraz leków przeciwhistaminowym w ciężkim obrzęku ust jest skuteczne. W związku z rosnącą liczbą zabiegów z użyciem wypełniaczy skórnych z dużym prawdopodobieństwem należy spodziewać się w przyszłości coraz większej ilości pacjentów zgłaszających objawy związane z powikłaniami po zabiegach z zakresu medycyny estetycznej. Abstract: BACKGROUND: Among patients with stable coronary disease and moderate or severe ischemia, whether clinical outcomes are better in those who receive an invasive intervention plus medical therapy than in those who receive medical therapy alone is uncertain. METHODS: We randomly assigned 5179 patients with moderate or severe ischemia to an initial invasive strategy (angiography and revascularization when feasible) and medical therapy or to an initial conservative strategy of medical therapy alone and angiography if medical therapy failed. The primary outcome was a composite of death from cardiovascular causes, myocardial infarction, or hospitalization for unstable angina, heart failure, or resuscitated cardiac arrest. A key secondary outcome was death from cardiovascular causes or myocardial infarction. RESULTS: Over a median of 3.2 years, 318 primary outcome events occurred in the invasive-strategy group and 352 occurred in the conservative-strategy group. At 6 months, the cumulative event rate was 5.3% in the invasive-strategy group and 3.4% in the conservative-strategy group (difference, 1.9 percentage points; 95% confidence interval [CI], 0.8 to 3.0); at 5 years, the cumulative event rate was 16.4% and 18.2%, respectively (difference, -1.8 percentage points; 95% CI, -4.7 to 1.0). Results were similar with respect to the key secondary outcome. The incidence of the primary outcome was sensitive to the definition of myocardial infarction; a secondary analysis yielded more procedural myocardial infarctions of uncertain clinical importance. There were 145 deaths in the invasive-strategy group and 144 deaths in the conservative-strategy group (hazard ratio, 1.05; 95% CI, 0.83 to 1.32). CONCLUSIONS: Among patients with stable coronary disease and moderate or severe ischemia, we did not find evidence that an initial invasive strategy, as compared with an initial conservative strategy, reduced the risk of ischemic cardiovascular events or death from any cause over a median of 3.2 years. The trial findings were sensitive to the definition of myocardial infarction that was used. Abstract: Mitochondrial dysfunction caused by cigarette smoke is involved in the oxidative stress-induced pathology of airway diseases. Reducing the levels of harmful and potentially harmful constituents by heating rather than combusting tobacco may reduce mitochondrial changes that contribute to oxidative stress and cell damage. We evaluated mitochondrial function and oxidative stress in human bronchial epithelial cells (BEAS 2B) following 1- and 12-week exposures to total particulate matter (TPM) from the aerosol of a candidate modified-risk tobacco product, the Tobacco Heating System 2.2 (THS2.2), in comparison with TPM from the 3R4F reference cigarette. After 1-week exposure, 3R4F TPM had a strong inhibitory effect on mitochondrial basal and maximal oxygen consumption rates compared to TPM from THS2.2. Alterations in oxidative phosphorylation were accompanied by increased mitochondrial superoxide levels and increased levels of oxidatively damaged proteins in cells exposed to 7.5 μg/mL of 3R4F TPM or 150 μg/mL of THS2.2 TPM, while cytosolic levels of reactive oxygen species were not affected. In contrast, the 12-week exposure indicated adaptation of BEAS-2B cells to long-term stress. Together, the findings indicate that 3R4F TPM had a stronger effect on oxidative phosphorylation, gene expression and proteins involved in oxidative stress than TPM from the candidate modified-risk tobacco product THS2.2.

Abstract: The methylation of histone lysine residues modifies chromatin conformation and regulates the expression of genes implicated in cell metabolism. Lysine-specific demethylase 1 (LSD1) is a flavin-dependent monoamine oxidase that can demethylate mono- and dimethylated histone lysines 4 and 9 (H3K4 and H3K9). The removal of methyl groups from the lysine residues of histone and non-histone proteins was found to be an important regulatory factor of cell proliferation. However, its role has not been fully elucidated. In this study, we assessed LSD1-mediated cell cycle progression using a human endothelial cell model. The short hairpin RNA knockdown of LSD1 inhibits the G2/M phase of cell cycle progression by checkpoint kinase 1 (Chk1) phosphorylation (S137). We observed elevated DNA damage, which was consistent with the increased detection of double-strand breaks as well as purines and pyrimidines oxidation, which accompanied the activation of ATR/ATRIP signaling by H2AXS139 phosphorylation. The irreversible pharmacological inhibition of LSD1 by 2-phenylcyclopropylamine (2-PCPA) inactivated its enzymatic activity, causing significant changes in heterochromatin and euchromatin conformation assessed by chromatin assembly factor 1 subunit A (CAF1A) and heterochromatin protein 1 isoform &alpha, and &gamma, (HP1&alpha, /&gamma, ) immunofluorescence analysis. We conclude that the knockdown of LSD1 in endothelial cells leads to increased HP1-positive chromatin, the stimulation of DNA repair processes, and the dysregulation of proliferation machinery.

Abstract: Rather than being a passive barrier between circulating blood and smooth muscle cells and the underlying tissues, the endothelium is a fundamental functional component of the vasculature, and could be viewed as the largest human endocrine gland/organ, secreting multiple pro-/antiangiogenic factors, cytokines and low-molecular-weight mediators controlling the vascular tone. The location of endothelium, at the interface between the circulation and the tissues, makes this epithelial layer particularly exposed to physical and chemical cues coming from the bloodstream. In response to such stimuli, the endothelium modulates its morphology and functions to maintain vascular homeostasis. Dietary components significantly affect the proper functioning of the endothelium. High-calories and high-fat western diets, in the long term, cause endothelial dysfunction, which is a major contributor to the development of the metabolic syndrome and its pathological consequences, including atherosclerosis, diabetes, and hypertension. On the contrary, plant-derived antioxidant molecules and polyphenols have been shown to exert beneficial effects on endothelial function. Extensive research in the last decade has clearly shown the close relationship between food intake, dietary habits, and gene expression, which is driven by the action of macro- and micronutrients on chromatin regulation. Nutrient-induced chromatin epigenetic modifications via DNA methylation and histone post-translational modifications, especially in the context of the western diet, significantly contribute to the dysregulation of endothelial functioning. Here, we review the current understanding on how dietary components (macronutrients, antioxidants), acting on epigenetic mechanisms, regulate endothelial physiology, and physiopathology. © 2016 BioFactors, 2016

Abstract: The adaptor protein p66Shc modulates cellular redox status integrating oxidative stress with mitochondrial stress responses. Upon oxidative stress, p66Shc is translocated to mitochondria or mitochondria-associated membranes in a multi-step process, resulting in locally increased reactive oxygen species production. This signaling pathway is believed to be important in the context of drug-induced organ toxicity. The use of anthracyclines as anticancer agents is limited due to a dose-dependent and cumulative toxicity resulting in cardiomyopathy. Treatment with the anthracycline doxorubicin (DOX) results in a dose-dependent and cumulative cardiotoxicity which is mediated, at least in part, by increased oxidative stress. In the present study, we investigated for the first time whether p66Shc signaling is activated during DOX treatment of the rat cardiomyoblast H9c2 cell line. We further tested whether the transcriptional factor FoxO3a, which activates target genes responsible for apoptosis and cell cycle arrest, is also involved in p66Shc-dependent redox signaling pathway. Our results suggest that DOX treatment induces p66Shc protein up-regulation specifically in nuclear fractions. Increased nuclear expression of FoxO3a was also detected in H9c2 cells after DOX treatment. Treatment with the antioxidant and protein kinase C (PKC-β) inhibitor hispidin decreased DOX-induced activation of caspase 9 and p66Shc alterations. Taking together, we hypothesize that p66Shc signaling is involved in the activation of stress/toxicity responses elicited by the treatment of H9c2 cells with DOX. Hence, the selective inhibition of this redox pathway may be a promising therapeutic approach to circumvent DOX cardiotoxicity.

Abstract: Background Patients with diabetes are at a high risk of developing both micro- and macrovascular disease. Hyperglycaemia seems to be the main factor in the pathogenesis of diabetic cardiomyopathy, often based on increased oxidative stress. Carvedilol, a β-adrenergic blocker, has intrinsic antioxidant properties and was previously described to be effective in the protection of cardiac mitochondria against oxidative stress. The objective of this study was to evaluate the effect of carvedilol on hyperglycaemia-induced oxidative damage and mitochondrial abnormalities in cardiac and skeletal muscle in streptozotocin-treated rats. Materials and methods Body mass, blood glucose, the level of protein carbonylation, caspase-9- and caspase-3-like activities, mitochondrial proteins, the status of antioxidant defence system and stress-related proteins were evaluated in streptozotocin vs streptozotocin + carvedilol (1 mg/kg/day)-treated rats. Results The results showed that carvedilol decreased blood glucose in streptozotocin-treated animals. Content of catalase in the heart and SOD2, SOD1 and catalase in skeletal muscle were increased by carvedilol treatment in streptozotocin-treated animals. At this particular time point, streptozotocin-induced hyperglycaemia did not cause caspase activation or increase in protein carbonylation status. The data showed that carvedilol increased the level of antioxidant enzymes, what may contribute to preserve cell redox balance during hyperglycaemia. We also showed here for the first time that carvedilol effects on streptozotocin-treated rats are tissue dependent, with a more predominant effect on skeletal muscle. Conclusions Based on data showing modulation of the antioxidant network in the heart, carvedilol may be beneficial in diabetic patients without advanced disease complications, delaying their progression.

Abstract: The aims of this study were to determine the proportion of rheumatoid arthritis (RA) patients attending hospital in whom amyloid deposits were present in abdominal fat aspiration (AFA) samples, and to assess possible risk factors for amyloid development in RA. One -hundred and twenty-one patients (16 males, 105 females) with RA referred to the Department of Rheumatology in Wroclaw between 1996 and 2001 were studied regardless of RA duration or laboratory findings. Abdominal subcutaneous fine-needle aspiration was performed, and samples of adipose tissue stained with alkaline Congo red then examined by polarized light microscopy. The presence or absence of amyloid fat deposits (AFD) was determined according to whether typical apple-green birefringence was observed. Amyloid deposits were found in 35 (29%) patients. Amyloidosis was significantly more common in males and in patients with longer disease duration. Abstract: Article-at-a-Glance Background Cleveland Health Quality Choice is a regional initiative to assess hospital performance which was implemented in 1989. The project developed and validated CHOICE, a severity adjustment system that includes diagnosis-specific models for medical, surgical, and obstetrical patients which are based on clinical data abstracted from patients' medical records. Methodology Since 1992 Cleveland Health Quality Choice has disseminated semi-annual reports that profile hospital mortality rates, lengths of stay, and cesarean section rates using the CHOICE severity adjustment models. Hospitals receive tabular and graphical representations of hospital outcomes and electronic patient-level data files that can be used to further examine outcomes in clinical subgroups. Results Four case studies illustrate how outcomes data derived from the CHOICE models led to the development of successful hospital programs to decrease lengths of stay, cesarean section rates, and hospital mortality rates. Although each case study reflected a unique approach to process improvement, several common characteristics were observed: (1) establishment of interdisciplinary process improvement teams with senior physician and nursing leadership; (2) detailed review of the process of care to identify modifiable clinical practices likely to affect outcomes; (3) development of practice guidelines based on group consensus or published recommendations that were designed to affect modifiable practices; and (4) aggressive sharing of serial data with individual practitioners. Conclusions Although outcomes data can provide powerful insight on where to target quality improvement efforts, hospitals must identify influential and modifiable clinical practices. Such efforts are most likely to be successful if driven by interdisciplinary work groups, supported by senior clinicians and administrators, and based on locally accepted practice standards.

Abstract: Objective. —To assess the safety and efficacy of optic nerve decompression surgery compared with careful follow-up alone in patients with nonarteritic anterior ischemic optic neuropathy (NAION). Design. —The Ischemic Optic Neuropathy Decompression Trial (IONDT) is a randomized, single-masked, multicenter trial. Setting. —Twenty-five US clinical centers. Participants. —The IONDT ceased recruitment on October 20, 1994, on the recommendation of its Data and Safety Monitoring Committee. The preliminary results presented herein are based on data as of September 8,1994, from 244 patients with NAION and visual acuity of 20/64 or worse. One hundred twenty-five patients had been randomized to careful follow-up, and 119 had been randomized to surgery, with 91 and 95, respectively, having completed 6 months of follow-up. Intervention. —Patients in the surgery group received optic nerve decompression surgery and follow-up ophthalmologic examinations; those in the careful follow-up group received ophthalmologic examinations at the same times as the surgery group. Main Outcome Measures. —Gain or loss of three or more lines of visual acuity on the New York Lighthouse chart at 6 months after randomization, as measured by a technician masked to treatment assignment. Results. —Patients assigned to surgery did no better when compared with patients assigned to careful follow-up regarding improved visual acuity of three or more lines at 6 months: 32.6% of the surgery group improved compared with 42.7% of the careful follow-up group. The odds ratio (OR) for three or more lines better, adjusted for baseline visual acuity and diabetes, was 0.74 (95% confidence interval [CI], 0.39 to 1.38). Patients receiving surgery had a significantly greater risk of losing three or more lines of vision at 6 months: 23.9% in the surgery group worsened compared with 12.4% in the careful follow-up group. The 6-month adjusted OR for three or more lines worse was 1.96 (95% CI, 0.87 to 4.41). No difference in treatment effect was observed between patients with progressive NAION and all others. Conclusion. —Results from the IONDT indicate that optic nerve decompression surgery for NAION is not effective, may be harmful, and should be abandoned. The spontaneous improvement rate is better than previously reported. ( JAMA . 1995;273:625-632)
