1. Diagnostic Accuracy of Frequency-Doubling Technology and the Moorfields Motion Displacement Test for Glaucoma

Abstract

Portable perimetric testing could be useful for community-based glaucoma screening programs. Frequency doubling technology (FDT) and the Moorfields motion displacement test (MDT) are portable perimeters which have shown promise as potential screening tools for glaucoma. This study's goal was to determine the diagnostic accuracy of FDT and MDT for visual field defects and glaucoma.Prospective, cross-sectional, diagnostic accuracy study.A consecutive series of patients aged ≥50 years who presented to a glaucoma clinic in South India and had never undergone Humphrey field analyzer (HFA) visual field testing in the past.Participants underwent 24-2 SITA Standard HFA perimetry, FDT perimetry, MDT perimetry, and iPad perimetry using visualFields Easy in random order. Ophthalmologist grades of HFA and optic nerve head photographs were used as the reference standards for glaucoma and field defect presence. Receiver operating characteristic curves were constructed to assess diagnostic accuracy of various parameters for each test.Sensitivity, specificity, area under the receiver operating characteristic curve (AUROC) RESULTS: 292 eyes from 173 participants were included, with 112 eyes classified as moderate or worse glaucoma. For moderate or worse glaucoma detection, the best parameter on FDT was mean deviation (MD) (AUROC 0.84, 95% CI 0.79 to 0.89), and the best parameter on MDT was global probability of true damage (GPTD) (AUROC 0.87, 95% CI 0.82 to 0.91). When specificity was set to 90%, the sensitivity for detection of moderate or worse glaucoma was 55% (95% CI 39 to 68%) for FDT MD and 62% (95% CI 52 to 71%) for MDT GPTD.FDT and MDT perimetry had fair diagnostic accuracy for glaucoma detection when administered to naïve test-takers in this South Indian population. Although not appropriate for use as a sole glaucoma screening test, these perimetric tests may be useful as ancillary tests.

Abstract

Pandemi Covid-19 di Indonesia telah memaksa pemerintah untuk menerapkan sejumlah kebijakan, salah satunya adalah work from home (WFH). WFH bagi pekerja di Indonesia masih menjadi sesuatu yang asing, sehingga sulit untuk menempatkan urusan pekerjaan dan keluarga sesuai dengan kebutuhan. Penelitian ini membahas mengenai pengaruh bekerja dari rumah terhadap work-family conflict dan juga akan melihat bagaimana pengaruh work-family culture terhadap work-family conflict dengan menggunakan metode kuantitatif melalui survei. Hasil dari penelitian tersebut menunjukkan bahwa bekerja dari rumah menyebabkan konflik pekerjaan-keluarga, dan budaya kerja-keluarga mempengaruhi konflik pekerjaan-keluarga. Karena adanya pengaruh tersebut, maka organisasi disarankan untuk menerapkan budaya kerja-keluarga yang mendukung pekerjaan karyawannya.

Abstract

This study aims to determine the impact of inflation in Indonesia on stock returns at PT. Unilever 2015 – 2019. In determining the impact of inflation on stock returns, data that is relevant to the title of the study is needed. This research method is a quantitative research method, the data collection method is a secondary data collection method derived from journals, final assignments, and books that are relevant to this research. Furthermore, for the data analysis method used quantitative descriptive analysis method with stock return data used which means the difference between stock prices in the current period and stock prices in the previous period (capital gain loss). The results of this study indicate that inflation has no effect on the value of stock returns at PT. Unilever in 2015 – 2019.

Abstract

Aims: This study will explore the incidence rate of childhood vaccination refusal in Malaysia as well as the reasons for this refusal over a four-year period (2016–2019). Given the complexities and dynamic nature of vaccine refusal, this research is essential for defining the target group, identifying the vaccine types they commonly refuse, and comprehending their vaccination concerns. Study Design: This is a retrospective review study. Methodology: This study used a nationally representative secondary data set from the Malaysian Ministry of Health's Family Health Division for the years 2016 to 2019. We included vaccination refusals involving children younger than 24 months of age. Data was extracted from the mothers’ immunization refusal form includes information of the ethnicity, citizenship, religion, occupation, the type of vaccine they refused, and the reason for their refusal. The annual incidence rates of childhood vaccination refusal, the types of vaccines and the refusal reason were summarized by percentage, with 95% confidence intervals. Abstract

This observation-based research aims to investigate the impact of the implementation of green housekeeping at the Holiday Resort Lombok, particularly on the operational cost efficiency. The data were collected in three months through observation, interview, and documentation technique, involving as many as 4 (four) informants. The data were analyzed using descriptive quantitative analysis technique and the discussion was focused on the influence of Internal and External factors on the efficiency of operational cost. The research result shows that the implementation of green housekeeping has a positive impact in decreasing the operational cost and increasing the amount of savings yearly. In 2018, before the implementation of green housekeeping, the total savings of operational cost was Rp 2.045.750.506, while in 2019 there was a greater amount, indicating a significant result by 18.43% more frugal due to the green housekeeping implementation. Studying the global trend of green hotel, Holiday Resort wants to point out and emphasize the advantages offered by the green hotels. This also suggests that Holiday Resort can become a more competitive destination considering its potential and resources. However, it is recommended that Holiday Resorts Lombok allocate more funds for composting and vermi composting, biomethanation, and biosanitizing processes.

COVID-19 telah ditetapkan sebagai pandemi dunia, sehingga menyebabkan pelayanan kesehatan anak tidak dapat berjalan secara optimal, termasuk kegiatan pemantauan di posyandu. Untuk itu, peneliti tertarik untuk merancang sebuah aplikasi yang dapat dijadikan sebagai alternatif pemantauan pertumbuhan dan perkembangan balita bagi ibu yang tidak bisa hadir di posyandu, baik akibat pandemi COVID-19 maupun alasan lainnya. Penelitian ini bertujuan untuk mengetahui kepuasan penggunaan Aplikasi e-Posyandu Kesehatan (ePoK) berbasis android sebagai alternatif posyandu di era new normal. Penelitian ini menggunakan quasi eksperimen pada 152 ibu balita yang diambil secara proportional stratified random sampling. Ibu diberikan aplikasi dan menggunakannya selama 1 bulan. Data yang diperoleh dianalisis secara univariat dan bivariat dengan menggunakan uji chi kuadrat. Berdasarkan hasil peneltian sebagian besar responden berusia 20-40 tahun (98%), pendidikan mayoritas tinggi (59,9%), sebagian besar merupakan ibu rumah tangga (65,8%), jumlah anak pada umumnya lebih dari satu (77,6%), dan rata-rata masih berusia kurang dari 2 tahun (84,2%). Kategori kepuasan responden terhadap aplikasi ePoK sebagian besar berada pada kategori sangat puas, yaitu 88,2%. Abstract

Sukatani Village, Compreng Sub-District, Subang District in carrying out village administration services still uses Microsoft Word in making village correspondence then the letter number is recorded in the village ledger and the Sukatani village population data is still stored in excel form. The problem is that typos often occur, it takes a long time to search for correspondence files and cannot present population data accurately. This problem can be overcome by using a village administration management system that will be used in Sukatani village. The system was developed using the OpenSID framework which is licensed and free to use and developed for village purposes. In order for the village apparatus to easily use the village administration management system, training and assistance is needed. The training and mentoring is carried out by dividing into two groups and each group is accompanied by a trainer so that the training for village officials can run smoothly and well. The result of this service activity is that the village administration management application and village apparatus can use the application well so that the application can be used continuously to improve village administration services to Sukatani villagers.

Abstract

Background The effects of empagliflozin in patients with chronic kidney disease who are at risk for disease progression are not well understood. The EMPA-KIDNEY trial was designed to assess the effects of treatment with empagliflozin in a broad range of such patients. Methods We enrolled patients with chronic kidney disease who had an estimated glomerular filtration rate (eGFR) of at least 20 but less than 45 ml per minute per 1.73 m2 of body-surface area, or who had an eGFR of at least 45 but less than 90 ml per minute per 1.73 m2 with a urinary albumin-to-creatinine ratio (with albumin measured in milligrams and creatinine measured in grams) of at least 200. Patients were randomly assigned to receive empagliflozin (10 mg once daily) or matching placebo. The primary outcome was a composite of progression of kidney disease (defined as end-stage kidney disease, a sustained decrease in eGFR to Results A total of 6609 patients underwent randomization. Abstract

Penelitian ini bertujuan untuk mengetahui pengaruh Kepuasan Kerja, Komitmen Organisasi dan Stres Kerja baik secara parsial maupun simultan terhadap Intention to Leave pada PT. Rajawali Nusindo Medan. Penelitian ini merupakan penelitian asosiatif kausal dan kuantitatif. Sampel penelitian ini berjumlah 92 orang. Penentuan sampel dilakukan dengan metode sampling jenuh. Pengumpulan data menggunakan kuesioner dan wawancara, sedangkan analisis data dilakukan dengan menggunakan analisis regresi linier berganda. Hasil penelitian pada taraf signifikansi 5% menunjukkan bahwa: 1) Hasil uji t pada variabel kepuasan kerja adalah -2.162, dengan nilai signifikansi 0.033, yang berarti bahwa kepuasan kerja memiliki pengaruh negatif dan signifikan terhadap intention to leave. 2) Hasil uji t pada variabel komitmen organisasi adalah -2.333, dengan nilai signifikansi 0.022, yang berarti bahwa komitmen organisasi berpengaruh negatif dan signifikan terhadap intention to leave. 3) Hasil uji t pada variabel stres kerja adalah 2.241, dengan nilai signifikansi 0.021, yang berarti bahwa stres kerja berpengaruh positif dan signifikan terhadap intention to leave. 4) Uji koefisien determinasi (R2) untuk variabel X1 ,X2, dan X3 adalah 0.234, yang berarti bahwa intention to leave dapat dipengaruhi oleh kepuasan kerja, komitmen organisasi dan stres kerja sebesar 23,4%, sedangkan sisanya sebesar 76,6% dipengaruhi oleh faktor-faktor lain yang tidak diteliti dalam penelitian ini.

Abstract

Forest to oil palm plantations conversion has changed soil's physical properties significantly. It caused significant changes in the hydrological cycle and affects the water resources around the research site. One of the strategies that could minimize the negative impact of monoculture oil palm plantations is better plantation management by implementing a mixed cropping system (agroforestry). This study aims to analyze the infiltration, surface runoff, and surface runoff coefficient in agroforestry and monoculture oil palm. This observation was conducted in the oil palm plantation area of PT Humusindo Makmur Sejati, Bungku Village, Bajubang District, Batanghari Regency. The infiltration rate was calculated according to the Horton method, and the amount of runoff in agroforestry and oil palm monoculture used the soil conservation service-curve number method. The results showed differences in infiltration, surface runoff, and runoff coefficients between the agroforestry land and the monoculture oil palm. The highest infiltration is in A48, which is 42,00 cm/hour in the dry season and 18,00 cm/hour in the rainy season. The lowest runoff is in A23, which is 70.14 mm with a runoff coefficient of 0.04. The highest runoff occurred in S48 of 339.88 mm, with a runoff coefficient of 0.19. The differences in infiltration, surface runoff, and runoff coefficient in this study are affected by the vegetation density and the amount of litter. 11. Burnout Among Caretakers of Paediatric Patient with Chronic Kidney Disease

Abstract

Introduction: Caring for children who have chronic kidney disease (CKD) is challenging. The goal of this study was to find out the prevalence of burnout and what factors contribute to it. We also want to know the correlation between caretaker’s strain and burnout. Methods: This was a cross-sectional study that involved caretakers of children with CKD. They were recruited from a university hospital in Kelantan, Malaysia. Self-administered questionnaire in Malay language comprised of Demographic Information Form, Copenhagen Burnout Inventory (CBI-M) and Caregiver Strain Index (CSI-M) was used. Results: Eighty-eight caretakers were involved in this study. The mean age of caretakers was 42 years old, majority were female (72.4%), Malay (99.0%) and Muslim (99.0%). The mean age of children with CKD was 11 years old, most of them were on medication (69.3%) and mean duration of illness was 4.6 years. The prevalence of burnout in all domains (personal, work-related, client-related) among the caretakers was 5.7%. Eight caretakers had personal burnout (9.1%) while five (5.7%) had client-related and work-related burnout respectively. Multiple linear regression showed positive association of duration since the initial diagnosis and total ward admission within six months with total burnout score. Pearson correlation revealed a positive and fair correlation between strain and burnout. Conclusion: Prevalence of burnout among caretakers was generally low, but personal burnout outnumbered other domains. Children with CKD with longer duration of illness and more frequent ward admission are the important factors leading to burnout among caretakers. A high number of caretakers having significant strain indicated that there were possibilities of developing burnout later on.

Abstract

Background: Neutrophil plays a defense role against bacteria and fungi specifically by the neutrophil phagoburst activity. The nitroblue tetrazolium (NBT) test is the gold standard method to measure this but the flow cytometry assays also have been developed. The aim of the study was to evaluate the performance of flow cytometry using Dihydrorhodamine (DHR) 123 and to compare with the conventional NBT test. Methods: This study was carried out to determine the phagoburst activity at Universiti Kebangsaan Malaysia Medical Centre on 26 normal healthy donors and 5 chronic myeloid leukemia (CML) patients using both methods. Results: In flow cytometry, higher fluorescence intensity was observed in Phorbol myristate acetate (PMA) stimulated neutrophils. Comparing the mean fluorescent intensity (MFI) between CML and healthy donor showed CML patients have significant lower MFI (312.15 vs 738.22; p95% of stimulated cells suggest the higher sensitivity. All samples with negative NBT showed negative DHR, reflecting high degree of agreement between these two methods and eliminating possible false negative result in flow cytometry. Conclusion: This study showed that flow cytometry assay has superior technical advantages compared to NBT test as it is easy to perform, quick, more sensitive and requires only a small amount of blood sample. Abstract

La pandemia por COVID-19 ha supuesto un reto para los países y sus profesionales sanitarios. La entrada viral en el hospedador a través del receptor ACE-2 y una activación excesiva del sistema inmunológico son claves para comprender tanto la incidencia como la gravedad de la enfermedad. La enfermedad inflamatoria intestinal (EII) representa una condición especial asociada con una respuesta descontrolada del sistema inmunológico a agentes externos. Los tratamientos para la EII se han asociado con un mayor riesgo de infecciones bacterianas y virales. Esto ha planteado la cuestión de una posible mayor incidencia y gravedad de la infección por COVID-19 en pacientes con EII. A lo largo de este año se han publicado varios artículos que tratan de responder esa cuestión. La vacunación contra la COVID-19 ofrece una gran promesa para controlar la infección en pacientes con EII. Según la evidencia actual, los pacientes con EII no tienen mayor incidencia de COVID-19 ni peor evolución de la enfermedad en comparación con la población general. La edad avanzada y la presencia de un mayor número de comorbilidades se han asociado con peores resultados. Los corticosteroides están asociados con un mayor riesgo de infección por COVID-19, una mayor tasa de hospitalizaciones y un mayor riesgo de COVID-19 grave. La mesalazina/sulfasalazina y las tiopurinas presentan un posible aumento del riesgo de COVID-19 grave, aunque se requieren más estudios para demostrar esta asociación. Dentro de los fármacos biológicos, los anti-TNF pueden tener un posible efecto protector. Los anti-IL-12/23, anti-integrinas y tofacitinib presentan resultados comparables con anti-TNF. Se recomienda mantener el tratamiento con agentes biológicos. En base a la eficacia, las recomendaciones de los expertos y la ausencia de otra evidencia, se recomienda la vacunación de pacientes con EII.

Abstract

Purpose To determine the diagnostic accuracy of potential screening tests for moderate to advanced glaucoma. Design Prospective diagnostic test accuracy study. Participants The study enrolled a consecutive series of patients aged ≥50 years who presented to a glaucoma clinic in South India without ever having received automated visual field testing. Methods All participants underwent 8 index tests: OCT of the peripapillary retinal nerve fiber layer, optic disc photography, Moorfield’s Motion Displacement Test (MDT), frequency doubling technique perimetry, noncontact tonometry, pneumatonometry, presenting visual acuity, and best-corrected visual acuity. Participants also underwent stereoscopic photographs and Humphrey visual fields, which were used by 2 ophthalmologists to arrive at the reference standard diagnosis of moderate to advanced glaucoma. Main Outcome Measures Sensitivity, specificity, positive likelihood ratio, and negative likelihood ratio. Results A total of 217 people were enrolled; 321 eyes from 180 participants had all tests performed. Of these, 127 eyes (40%) were classified as having moderate to advanced glaucoma. Among the 8 tests, OCT best optimized sensitivity (84%, 95% confidence interval [CI], 76–90) and specificity (75%, 95% CI, 68–81). Moorfield’s Motion Displacement Test was the best perimetric test, with a sensitivity of 91% (95% CI, 85–96) and specificity of 53% (95% CI, 44–61). Pressure and vision tests were not sensitive (e.g., sensitivity of 16%, 95% CI, 9–23 for noncontact tonometry and 23%, 95% CI, 15–31 for best-corrected visual acuity). Moorfield’s Motion Displacement Test identified 16 of 127 eyes (13%) with glaucoma that were not captured by OCT, but also had false-positive results in 65 of 194 eyes (34%) without glaucoma that OCT correctly classified as negative. Conclusions OCT had moderate sensitivity and fair specificity for diagnosing moderate to advanced glaucoma and should be prioritized during an initial assessment for glaucoma.

This study was conducted to determine and analyze the financial performance of banking companies listed in the LQ 45 index for the 2014-2020 period. The factors analyzed are credit risk, liquidity risk and operational risk. The method used is descriptive. The data comes from the financial statements of banking companies incorporated in the LQ 45 index for the 2014-2020 period, namely Bank Central Asia (BCA), Bank Negara Indonesia (BNI), Bank Rakyat Indonesia (BRI), Bank Tabungan Negara (BTN) and Bank Mandiri. Data processing software using SPSS. This study partially obtains the results of credit risk and liquidity risk not having a significant effect on financial performance while operational risk has a significant effect on financial performance. So that credit risk, liquidity risk and operational risk simultaneously have a significant effect on financial performance.

Abstract

The purpose of this study was to find out how big the Influence of Inflation Risk, Credit Risk (NPL) and Liquidity Risk (LDR) on Profitability (ROA) During the Covid-19 Pandemic Period in State-Owned Banks for the 2018-2020 Period. The method used was descriptive method with a quantitative approach assisted by the SPSS program, namely through classical assumption test analysis, multiple linear regression analysis and hypothesis testing using simultaneous test (F test), partial test (t test), and coefficient of determination test (R2). . The data used in this study was secondary data where the data collection was indirectly so that it uses existing data information. This study uses the population at state-owned banks for the 2018-2020 period using the Saturated Sampling technique because the sampling technique takes all the population to be used as samples. Based on the results of the t-test partially Credit Risk (NPL) has a significant effect on Profitability (ROA), while Inflation Risk and Liquidity Risk (LDR) have no significant effect on Profitability (ROA). From the results of Adjusted R Aquare shows that simultaneously Profitability (ROA) was influenced by Inflation, Credit Risk (NPL), and Liquidity Risk (LDR) by 62%, while the remaining 38% is influenced by other factors.

Abstract

This study aims to determine the effect of credit risk, liquidity risk and operational risk either partially or simultaneously on the profitability of state-owned commercial banks listed on the IDX for the period 2013-2020. The type of research method is associative. The samples used were 4 banking companies including PT Bank Rakyat Indonesia (Persero) Tbk, PT Bank Mandiri (Persero) Tbk, PT Bank Negara Indonesia (Persero) Tbk and PT Bank Tabungan Negara (Persero) Tbk. In this study, a sampling technique was used in the form of saturated sampling with secondary data in the form of the annual financial statements of state-owned commercial banks for the period 2013-2020 which were obtained through the official website of the Indonesia Stock Exchange www.idx.co.id. The analysis technique was carried out through multiple linear regression analysis and with SPPS 16 software as data processing. This study obtained the results that credit risk and liquidity risk partially have no significant effect on profitability, while operational risk partially has a negative and significant effect on profitability. Simultaneous results, namely credit risk, liquidity risk and operational risk have a significant effect on profitability.

Abstract

The purposive of this study, namely to find out how much and what are the sources of income for traditional fishermen's families in Belang District, and the expenses of traditional fishermen's families in Belang District is, te last step is to find out how the standard of living of traditional fishermen's families in Belang District is. The Method used in this research is a survey method. The population in this study are traditional fishermen in Belang District. The data collection method used is Sampling . The sampling method is using a quataed Multi-Stages Purposive Sampling, with the number of sample villages being 15 villages and the sample fishermen taken are fishermen with fishing rods and nets as much as 10% of the total population in each sample village. The data collected consists of primary data and secondary data. Primary data was collected through observation and interviews guided by questionnaires. Secondary data were obtained from the Belang District of office and related to this research. The data analysis used is descriptive qualitative and quantitative analysis. The standard of living of traditional fishing families is analyzed using the Engel index. Based on the results of the analysis, it is known that the income of traditional fishermen's families in Belang District comes from their main job as fishermen, side jobs outside the field of fisheries. The total incomes of traditional fishermen's families in Belang District on average for one year is Rp.53,394,000. There are 2 types of expenditures for traditional fishermen's families in Belang District, namely expenditures for food and expenditures for non-food such as clothing, housing, health, education, electricity, transportation and others. The total average expenditure for one year is Rp. 53,394,000, which is divided into expenditures for food needs of Rp. 32,284,500 and expenditures for non-food needs which are Rp. 21,109,500. The results of the analysis of the Engel index are 60 0.46% , meaning that the total income of traditional fishermen in Belang District is 60.46% used to meet food needs only. The proportion for food, which is 39,54 % . This is indicates that the standard of living of traditional fishermen in Belang District is still relatively low because more than half of their income is only sufficient to meet food needs.

Home Industry (IRT) is an activity carried out in the home, although in the end the Home Industry will be marketed outside the home. If the activity goes on for a long time, there will be an impact on the home business, namely it is not registered and does not have an official permit from the Government. One of the Home Industries (IRT) is in Petumbukan, Deli Serdang Regency. This Home Industry is a food and snack business or is called a snack. This Home Industry (IRT) has the name RJ, which stands for the name of the owner, Raju. This Raju Home Industry has five workers. This Raju Home Industry Business starts from morning until evening. Besides producing food (light snacks) consisting of sweet potato chips and banana chips. Currently the Home Industry (IRT) is very rapidly developing in Indonesia in general and in the Deli Serdang Regency area in particular. With the proliferation of Home Industries in City Districts, they should understand their obligation to regulate Business Place Permits (SITU) because if the Home Industry has taken up its home business, it will benefit a lot, one of which is getting a loan / capital credit from Government bank. Abstract

Aedes albopictus poses a public health risk in tropical countries and temperate countries in recent decades due to its capability to transmit various human arboviruses including dengue, yellow fever, and chikungunya. Vector control is the key for preventing transmission of these pathogenic viruses. Improving the effectiveness of currently utilized collection methods, such as ovitraps, is important for best species abundance monitoring, assessment of the threat of arbovirus transmission, and optimizing control activities. Therefore, this study aimed to assess the potential use of lactic acid bacteria (LAB) waste as an infusion-baited ovitrap for Aedes collection. The performance of overnight tap water, grass hay infusion and LAB waste infusion were compared for their ability in attracting gravid female Ae. albopictus. In this study, the LAB waste infusion was substantially more alluring to Ae. albopictus mosquitoes than the two controls grass hay infusion and tap water.

Abstract

Teratomatous tumours of the head are rather uncommon. We report a 3-month-old child who presented with acute signs of raised intracranial pressure, needing craniectomy and subtotal tumour removal. The patient was diagnosed as intracranial immature teratoma grade 3, from the pathological study and elevated alpha-fetoprotein (AFP). Managing brain teratoma posed a challenge to the managing team due to the location of the tumour, the unavailability of standardized chemotherapy protocol and the dilemma of commencing adjuvant chemotherapy in a very young infant. The tumour was partially removed due to its critical location. And chemotherapy was delayed until the patient achieved 7 months of age. After four rounds of chemotherapy, the patient remained in remission for more than three years follow up.

Abstract

BACKGROUND AND OBJECTIVES The present study was planned to assess the clinical utility of reticulocyte haemoglobin content (CHr) and immature reticulocyte fraction (IRF) in the early detection of latent iron deficiency in blood donors. MATERIALS AND METHODS The prospective longitudinal observational study was conducted using the purposive sampling method. Written informed consent was obtained and donors were allocated into the first-time (FTD) and regular donor (RD) group. The enrolled blood donors (n = 205 in each group) were followed up for two subsequent whole blood donations. Haemoglobin (Hb), CHr, IRF and serum ferritin values were recorded at enrolment and two follow-ups. RESULTS The sensitivity of CHr in detecting iron-deficient erythropoiesis (serum ferritin values ≤ 26 μg/dl) was 45% and 56.7%, specificity 96.7%, positive predictive value (PPV) 85.6% and 90.8% and negative predictive value (NPV) 80.1% and 78.7%, respectively in FTD and RD cohorts. The sensitivity of IRF was 45.1% and 44.8%, specificity 93.4% and 97.1%, PPV 74.8% and 90.4% and NPV 79.6% and 74.5%, respectively in both the cohorts. The sensitivity of CHr in detecting absent iron stores (serum ferritin values ≤ 15 μg/dl) was 66.2% and 74.4%, specificity 92% and 90.6%, PPV 56.7% and 68.7% and NPV 94.5% and 92.8% among FTD and RD cohort, respectively. The sensitivity of IRF was 72.7% and 65.3%, specificity 90.3% and 94.3%, PPV 54.4% and 76% and NPV 95.4% and 90.8%, respectively in both the cohorts. CONCLUSION Reticulocyte hemoglobin content and IRF can be used along with complete blood count for early detection of iron deficiency in blood donors using the same blood sample at no extra cost.

Abstract

Daerah Aliran Sungai (DAS) Cidurian merupakan salah satu DAS di Pulau Jawa dengan kategori sebagai DAS yang dipulihkan. Kinerja DAS Cidurian secara umum dari hulu sampai dengan hilir termasuk dalam klasifikasi kategori buruk. Penelitian ini bertujuan untuk 1) mengkaji karakteristik hidrologi DAS Cidurian menggunakan model hidrologi SWAT; 2) mengkaji kinerja hidrologi berdasarkan skenario simulasi yang disusun; dan 3) menyusun strategi keberhasilan Rehabilitasi Hutan dan Lahan pada DAS Cidurian berdasarkan skenario terbaik. Berbagai skenario penggunaan lahan dan pengelolaannya diujicobakan menggunakan model SWAT (Soil and Water Assessment Tool) untuk menduga dampak penerapannya terhadap karakteristik hidrologi pada DAS Cidurian. Skenario yang diaplikasikan adalah sebagai berikut: 1) Implementasi RTRW (Rencana Tata Ruang Wilayah) Provinsi Banten dan Jawa Barat; 2) Implementasi Rehabilitasi Hutan dan Lahan dengan penanaman penanaman vegetative dan; 3). Implementasi Rehabilitasi Hutan dan Lahan penanaman vegetatif dan Konservasi Tanah dan Air secara vegetatif dan sipil teknis. Kalibrasi dan validasi dari model menunjukkan kategori memuaskan dengan nilai NSE (Nash-Sutcliffe Efficiency) 0,53 dan 0,50. Semua skenario menunjukkan peningkatan respon hidrologi dibandingkan dengan kondisi eksisting tahun 2020. Ketiganya memberikan respon limpasan permukaaan dan hasil sedimen yang menurun serta aliran lateral dan aliran dasar yang meningkat. Semua skenario ini menurunkan nilai Koefisien Aliran Tahunan (KAT) dan Koefisien Regim Aliran (KRA). Skenario penerapan kombinasi Rehabilitasi Hutan dan Lahan penanaman vegetatif dan Konservasi Tanah dan Air menjadi skenario terbaik dengan nilai KAT 0,27 (kategori rendah) dan KRA 22,40 (kategori rendah).

31. Derivation of a minimal functional XIST by combining human and mouse interaction domains

Abstract

X-inactive specific transcript (XIST) is a 17–19 kb long non-coding ribonucleic acid (RNA) critical for X-chromosome inactivation. Tandem repeats within the RNA serve as functional domains involved in the cis-limited recruitment of heterochromatic changes and silencing. To explore the sufficiency of these domains while generating a functional mini-XIST for targeted silencing approaches, we tested inducible constructs integrated into 8p in a male cell line. Previous results suggested silencing could be accomplished with a transgene comprised of the repeat A, which is highly conserved and critical for silencing; the repeat F that overlaps regulatory elements and the repeat E that contributes to XIST localization by binding proteins such as CIZ1 (AFE). As polycomb-repressive complex 1 (PRC1) is recruited through HNRNPK binding of repeats B–C–D, we included a second ‘mini-XIST’ comprising AFE with the mouse Polycomb Interaction Domain (PID), a 660-nucleotide region known to recruit PRC1. Silencing of an adjacent gene was possible with and without PID; however, silencing more distally required the addition of PID. The recruitment of heterochromatic marks, evaluated by immunofluorescence combined with RNA fluorescence in situ hybridization, revealed that the AFE domains were sufficient only for CIZ1 recruitment. However, mini-XIST transgene recruited all marks, albeit not to full XIST levels. The ability of the PID domain to facilitate silencing and heterochromatic mark recruitment was unexpected, and inhibition of PRC1 suggested that many of these are PRC1 independent. These results suggest that the addition of this small region allowed the partial recruitment of all the features induced by a full XIST, demonstrating the feasibility of finding a minimal functional XIST.

Surface runoff is a primary driving factor for water regulation services on oil palm plantations as it determines the hydrological components and other biogeochemical process. Therefore, understanding on their interaction and contribution within the watershed system is important to support decision-making system. Here, we applied Soil and Water Assessment Tools (SWAT) model to simulate water regulation services for an intermittent micro-catchment dominated by oil palm plantation in Harapan Landscapes, Batanghari Regency, Jambi Province. In this study, we used two different runoff curve number (CN) approaches in the SWAT model, namely the soil moisture curve number (CN-SM) and the plant evaporation curve number (CN-ET), to evaluate their applicability and uncertainty for assessing water regulation services. SWAT was automatically calibrated and validated against daily observed streamflow data. The results showed that the model performed well as indicated by hydrograph visual interpretation and statistical indicators. The performance was good for calibration and validation for both approaches with high R2 and Nash-Sutcliffe Efficiency (NSE). Also, the uncertainty was acceptable with P-factor >70% and R-factor

Tanaman kelapa sawit (Elaeis guinensis) memiliki arti yang sangat penting didalam kehidupan masyarakat. Suatu tanaman dapat dikatakan normal jika tanaman tersebut dapat menjalankan fungsi-fungsi fisiologis dengan baik. Proses budidaya tanaman kelapa sawit dipengaruhi oleh berbagai faktor, salah satunya adalah faktor hama dan penyakit. Pada umumnya, petani membedakan antara hama dan penyakit, hal ini karena sebagian besar petani kekurangan informasi dan masih mengandalkan pengalaman petani lain untuk mengatasi masalah hama dan penyakit yang ada dikebun mereka. Berikut ini di rangkum beberapa jenis penyakit yang menyerang pada tanaman kelapa sawit, yaitu Kumbang Tanduk (Oryctes), Penyakit Tajuk Tanaman (Crown Desease), Genoderma, Ulat Pemakan Daun. Penyakit Bercak Daun, Penyakit Busuk Daun. Pada penanganan hama dan penyakit, dibutuhkan konsultan pertanian yang mampu mendiagnosa hama dan penyakit pada tanaman kelapa sawit. Meskipun sudah banyak pakar spesialis penyakit tanaman kelapa sawit tetapi pakar pun mempunyai waktu keterbatasan jam kerja sehingga masih menyulitkan para petani untuk bertanya langsung pada pakarnya, jadi dengan keterbatasan waktu tersebut peneliti dapat mengembangankan suatu teknologi informasi yang dapat meringankan para petani dan pakar dengan cara mengembangan aplikasi berbasis WEB.

Diare masih menjadi permasalahan kesehatan di Kota Bandung dan salah satu penyakit penyebab kematian pada bayi dan balita. Meningkatnya kejadian diare seiring dengan penurunan ASI Eksklusif. Tujuan penelitian ini adalah menganalisis pengaruh riwayat ASI eksklusif dan perilaku cuci tangan pakai sabun terhadap kejadian diare pada bayi. Penelitian ini menggunakan desain kasus kontrol. Populasi kasus dalam penelitian ini yaitu ibu yanng memiliki bayi usia 7-12 bulan yang mengalami diare, sedangkan populasi kontrol bayi yang tidak menderita diare. Sampel diambil secara random sampling sebanyak 120 responden. Instrumen yang digunakan dalam penelitian ini adalah kuesioner. Uji statistik yang digunakan adalah chi square. Hasil penelitian didapatkan sebagian besar balita tidak memiliki riwayat berat badan lahir rendah (70,8%), memiliki riwayat mendapatkan ASI Eksklusif (75,8%), memiliki riwayat imunisasi dasar lengkap (67,5%), dan ibu yang melakukan perilaku cuci tangan pakai sabun (67,5%). Faktor penyebab yang berhubungan dengan kejadian diare pada balita adalah Riwayat ASI eksklusif (p=0,033) dan Perilaku cuci tangan pakai sabun (p=0,019). Bayi lebih berisiko mengalami diare jika tidak diberikan ASI eksklusif dan ibu tidak melakukan perilaku cuci tangan pakai sabun. Tenaga kesehatan bagian promosi kesehatan diharapkan bisa bekerja sama dengan instansi lainnya seperti Kecamatan untuk melakukan sosialisasi mengenai pentingnya cuci tangan pakai sabun dan pemberian ASI eksklusif.

Abstract

This study aims to present the state of the academic literature on the sustainability of non-profit organizations (NPOs) and how previous researchers have approached NPO sustainability. The review has resulted in the final 30 articles for review and analysis. The research characteristics of the articles are identified by examining the publication outlet. It is observed that research in the 1990s on NPO sustainability is scarce, with a focus on understanding and identifying models of sustainability. Research in the first decade of the 21st Century has focused on the financial and non-financial approaches to address the sustainability of NPO. Meanwhile, research in the 21st Century's second decade acknowledged the need to integrate the diverse perspective of NPO sustainability into a comprehensive framework. This study found various approaches that influence NPO sustainability: financial or economic approach; organizational characteristics and strategy; capacity building; partnership or collaboration; and value-based perspective. Theoretically, this study provides an overall view of how academic researchers have attempted to address NPOs' sustainability. In terms of practical contribution, this study serves as a guide for the NPOs and relevant regulators on how to address the organization’s sustainability from various perspectives. We further set out suggestions for future research as well as the limitations and contributions of this study.

Introduction:Spindle cell carcinomas (SpCC) are a rare variant of squamous cell carcinoma which is rarely encountered in the head and neck. It is also known as“pseudocarcinoma”, “sarcomatoidcarcinoma,” and “carcinosarcoma”.It has an aggressive nature and has poor prognosis despite aggressive treatment. Case report: A 48-year-old lady presented to us with a right painless upper gum swelling which was progressively enlarging for 3 months after tooth extraction. Examination of the oral a fungating mass over the right upper alveolus extending to the right upper gingivolabial sulcus. She was subjected to a debulking palliative to reduce the tumour bulk. The intraoperative samples sent for histopathological examination was reported as SpCC, AJCC (8th edition) stage pT2pN3b. She recovered well after the surgery with quality of life after the surgery. In light of recent studies on the changing views on masculinity, this quantitative study aimed to determine the attitudes of South African undergraduate students toward the display of the tenets associated with orthodox and inclusive masculinities in contact sport, and to determine to what extent these attitudes were predicted by students' gender, race, religiosity, and attitudes toward the importance of primary gender-role sport socialization and homosexuality. Findings, which originated from the data collected through the use of 200 structured interviews, indicated that male and female students endorsed displays of a 'softening' in masculinities in sport, and were slightly opposed toward displays of 'harder' masculinities, a trend that was strongest among women. The more importance these students' ascribed to primary gender-role sports socialization, the more likely they were to support the display of orthodox masculinities in sport, and the less likely they were to endorse inclusive masculinities, with the converse being true for those who were more accepting of homosexuality. The study contributes to current theorization in a twofold way: Firstly, by problematizing a simplistic differentiation between orthodox and inclusive masculine typologies in favor of ascribing to theorization that indicates how the attitudes among students of the said South African university campus arguably attest to the concurrent existence of 'multiple dominant masculinities.' In-keeping with this, the findings arguably echo the Andersonian emphasis on the co-existence of declining levels of homohysteria among younger persons and the continuing prevalence of homophobia among some students on university campuses.

Abstract

PurposeWe aim to develop a multi-task three-dimensional (3D) deep learning (DL) model to detect glaucomatous optic neuropathy (GON) and myopic features (MF) simultaneously from spectral-domain optical coherence tomography (SDOCT) volumetric scans.MethodsEach volumetric scan was labelled as GON according to the criteria of retinal nerve fibre layer (RNFL) thinning, with a structural defect that correlated in position with the visual field defect (i.e., reference standard). MF were graded by the SDOCT en face images, defined as presence of peripapillary atrophy (PPA), optic disc tilting, or fundus tessellation. The multi-task DL model was developed by ResNet with output of Yes/No GON and Yes/No MF. SDOCT scans were collected in a tertiary eye hospital (Hong Kong SAR, China) for training (80%), tuning (10%), and internal validation (10%). External testing was performed on five independent datasets from eye centres in Hong Kong, the United States, and Singapore, respectively. For GON detection, we compared the model to the average RNFL thickness measurement generated from the SDOCT device. To investigate whether MF can affect the model’s performance on GON detection, we conducted subgroup analyses in groups stratified by Yes/No MF. The area under the receiver operating characteristic curve (AUROC), sensitivity, specificity, and accuracy were reported.ResultsA total of 8,151 SDOCT volumetric scans from 3,609 eyes were collected. For detecting GON, in the internal validation, the proposed 3D model had significantly higher AUROC (0.949 vs. 0.913, p < 0.001) than average RNFL thickness in discriminating GON from normal. In the external testing, the two approaches had comparable performance. In the subgroup analysis, the multi-task DL model performed significantly better in the group of “no MF” (0.883 vs. 0.965, p-value < 0.001) in one external testing dataset, but no significant difference in internal validation and other external testing datasets. The multi-task DL model’s performance to detect MF was also generalizable in all datasets, with the AUROC values ranging from 0.855 to 0.896.ConclusionThe proposed multi-task 3D DL model demonstrated high generalizability in all the datasets and the presence of MF did not affect the accuracy of GON detection generally.

Berdasarkan observasi kegiatan Program pengelolaan penyakit kronis (Prolanis) di Puskesmas Singaparna diketahui terdapat banyak pasien dengan rawat jalan dengan diagnosis diabetes mellitus dengan komplikasi pada pasien tersebut. Menurut Koordinator kader Pobindu Penyakit Tidak Menular (PTM) menyatakan bahwa pelatihan tentang screening diabetes mellitus belum pernah dilakukan sehingga belum menunjang dalam kegiatan Posbindu PTM dalam pencarian kasus diabetes mellitus. Pengabdian kepada masyarakat bertujuan meningkatkan pengetahuan dan keterampilan kader Posbindu PTM dalam melakukan screening Diabetes Mellitus (DM) sebagai upaya pencegahan deteksi dini. Kegiatan pengabdian berupa pelatihan melakukan screening dengan metode demonstrasi secara langsung. Kader Posbindu mendapatkan materi tentang cara melakukan tes kadar gula darah untuk menentukan status DM. Pelatihan diikuti oleh 16 kader Posbindu PTM di Desa Cikunir pada tanggal 12 September 2019. Rangkaian kegiatan diawali dengan pre-test, pelatihan screening diabetes mellitus, dan ditutup dengan post-test. Hasil kegiatan menunjukkan peningkatan pengetahuan dan keterampilan kader posbindu PTM dalam melakukan screening diabetes mellitus. Nilai rata-rata pre-test sebesar 60,0 dan meningkat menjadi 88,0 saat post-test. Pelatihan merupakan kegiatan yang penting sebagai upaya dalam five level of prevention PTM yang diantaranya berupa early diagnosis diabetes mellitus. Kegiatan diharapkan berdampak pada penurunan prevalensi DM dan pencegahan komplikasi DM karena secara dini masyarakat mengetahui status kesehatannya dengan screening DM.Based on observations of Prolanis activities at the Singaparna Health Center, it is known that there are many patients with outpatient care with a diagnosis of diabetes mellitus with complications in these patients. According to the Coordinator of Non-Communicable Diseases Pobindu cadres, he stated that training on diabetes mellitus screening had never been carried out, so it did not support PTM Posbindu activities in searching for cases of diabetes mellitus. This current community service purpose was to increase the knowledge and skills of Posbindu PTM cadres in screening DM to help prevent the disease. The method used in this community service was a demonstration to perform the DM screening. The Posbindu cadres were given training on how to measure blood sugar levels. This training was attended by 16 Posbindu PTM cadres from Cikunir Village on 12th September 2019. The training was conducted following this stage; the pre-test, the diabetes mellitus screening demonstration and the post-test. The training increased the knowledge and skills of the cadres in performing diabetes mellitus screening. This can be seen from the average score of the post-test (88.0), which is higher than the pre-test (60.0). This community service indicates the importance of the training as part of the non-communicable disease (NCD) five levels of prevention to reduce DM incidence. It is hoped that DM complications could also be resolved as people take care of their health by screening DM.

Abstract

The comfort and productivity of workers may be affected differently by the indoor air quality (IAQ) and related risk factors at the office and at home. Sick Building Syndrome (SBS) is one of the health issues usually faced by workers. SBS is generally associated with the time spent in a building, IAQ, and other related risk factors. The study reviewed papers published in journal articles and conferences regarding IAQ, environmental risk factors and SBS in the last ten years. The review employed the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) 2020 items from two significant databases, Scopus and Web of Science. The review steps involved identification, screening, eligibility, data extraction and analysis. The study found that air quality in a building significantly influences work productivity and may contribute to SBS. Findings show that SBS symptoms are linked to various personal characteristics, sociodemographic, working environment and IAQ factors. The physical contaminants, chemical contaminants and ventilation rate have established relations with SBS symptoms. These findings can help to form interventions aiming to improve IAQ and the productivity of occupants.

This study evaluated the performance, efficiency, and effectiveness of the Kota Kandis Irrigation Area (KKIA) secondary irrigation network and the KKIA potential for agriculture, education, and ecotourism (Agri-Edu-Ecotourism) area. The data were obtained from field investigation and interviews of farmers for primary data and supported with secondary data gained by data collected from reports and related statistical data. The average inflow and outflow discharges were 367.31 and 304,90 liters/second, respectively, and creating water loss rate reached 18.53%. The irrigation distribution efficiency and effectiveness are 81.47% and 54.19%, respectively. It indicated that much water had not been properly distributed. The evaluation results of the technical and social aspects are still categorized as unsatisfactory. According to the PUPR Ministerial Regulations (2017), the Irrigation Performance Index (IPI) of KKIA is 69.5, categorized as poor. The damage level of irrigation infrastructure ranges from 20 to 40%. It is necessary to improve socialization, guiding, coaching, and extension activities to improve operations management, routine, and periodic maintenance and repair. The KKIA also has excellent potential to be developed for an Agri-Edu-ecotourism area.

Variabel independen dalam penelitian ini adalah perputaran kas dan perputaran piutang. Untuk variabel dependen dalam penelitian ini adalah profitabilitas (ROA). Populasi yang diambil dalam penelitian ini adalah perusahaan kosmetik dan keperluan rumah tangga yang terdaftar di Bursa Efek Indonesia selama periode 2011 - 2016. Teknik pengambilan sampel yang digunakan adalah teknik purposive sampling sejumlah 5 perusahaan. jenis data yang digunakan adalah data sekunder yang diambil dengan metode dokumentasi dengan cara mengakses beberapa situs dan studi pustaka. Teknik analisis data yang digunakan adalah analisis regresi linier berganda, uji asumsi klasik dan uji hipotesis menggunakan uji t. Hasil penelitian uji hipotesis secara parsial menunjukkan bahwa terdapat pengaruh positif yang signifikan antara perputaran kas terhadap profitabilitas (ROA) dan terdapat pengaruh positif yang signifikan antara perputaran piutang terhadap profitabilitas (ROA). Abstract

Objective To compare cardiac index (CI) between Proaqt/PulsioFlex® and oesophageal Doppler (OD) and the ability of the PulsioFlex® to track CI changes induced by fluid challenge and secondly to assess the impact of the time interval between two auto-calibrations of PulsioFlex® on the accuracy of the measured CI. Methods In a single hospital, 49 intermediate-risk oncologic abdominal surgery patients were included in an observational study. We measured the cardiac Index (CI) provided by OD and by the Proaqt/PulsioFlex® before and after internal calibration, which were performed randomly at specific intervals after the initial one (30, 60, 90 and 120 min). The ability to track fluid responsiveness was evaluated by measuring stroke volume variation, pulse pressure variation (PPV) and CI before and after a 250 ml fluid challenge and assessed by a receiver operating characteristic curve analysis. Results The percentages of error before calibration were 51, 58, 82, 81% for 30, 60, 90 and 120 min, they were 39, 57, 65, and 54% after calibration. Trending ability is assumed by a 93% concordance rate after applying a 15% exclusion zone. The trend interchangeability rate was 13.75%. The area under the curve for fluid responsiveness measured by PPV and SVV PulsioFlex were respectively 0.67 [0.57–0.77], P Conclusions The Proaqt/Pulsioflex® system is not equivalent to OD for haemodynamic monitoring during non-vascular abdominal surgery in intermediate-risk patients. More studies are required to define the effect of the auto-calibration on the system.

Abstract

Introduction:endometrial carcinoma (EC) is the seventh most common cancer in females in Malaysia, of which the majority is composed of lower grade type I EC. Although less prevalent, type II EC which is of higher grade has poorer outcome and prognosis. Human epidermal growth factor receptor 2 (HER2) is one of the possible prognostic molecular markers which can be a target for immunotherapy. This study aimed to assess the expression of HER2 in common type of EC in the local population and to determine its correlation with the clinicopathological features. Methods:a total of 53 cases of endometrioid type of EC were selected within a six-year period comprising of 22 cases of grade 1, 25 cases of grade 2 and six cases of grade 3 carcinoma. The selected whole tumour tissue sections were immune-stained with HER2 antibody. The scoring was semi-quantitatively analyzed based on 2013 American Society of Clinical Oncology (ASCO)/College of American Pathologists (CAPs) guidelines for the scoring of HER2 in breast cancer. Results:all cases regardless of grades of endometrioid carcinoma showed negative expression of HER2 (score 0). Conclusion:there was no significant HER2 expression in endometrioid carcinoma. However, a follow-up study with a larger number of samples from different type of endometrial carcinoma is needed. Testing of several tumour tissue blocks to assess possible tumour heterogeneity, as well as correlation with HER2 gene amplification status by in-situ-hybridisation, are also recommended.

Abstract

Background and objectives During the worldwide pandemic of coronavirus disease 2019 (COVID-19), oncological procedures considered to be urgent could not be delayed, and a specific procedure was required to continue surgical activity. The objective was to assess the efficacy of our preoperative screening algorithm. Methods This observational retrospective study was performed between the 25th of March and the 12th of May 2020 in a comprehensive cancer center in France. Patients undergoing elective oncologic surgery were tested by preoperative nasopharyngeal reverse-transcription polymerase chain reaction (RT-PCR) that could be associated with a chest computerized tomography (CT) scan. Results Of the 510 screening tests (in 477 patients), only 5% (15/477) were positive for COVID-19 in 24 patients (18 RT-PCR+ and 7 CT scan+/RT-PCR-). Four patients were ultimately false positives based on the CT scan. In total, only 4.2% (20/477) of the patients were COVID-19+. The positivity rate decreased with time after the containment measures were implemented (from 7.4% to 0.8%). In the COVID-19+ group, 20% of the patients had postoperative pulmonary complications, whereas this was the case for 5% of the patients in the COVID-19 group. Conclusions Maintaining secure surgical activity is achievable and paramount in oncology care, even during the COVID-19 pandemic, with appropriate screening based on preoperative RT-PCR.

Abstract

Sağır, Özlem (Balikesir Author), Introduction More than 300 million surgical procedures are performed each year. Acute kidney injury (AKI) is a common complication after major surgery and is associated with adverse short-term and long-term outcomes. However, there is a large variation in the incidence of reported AKI rates. The establishment of an accurate epidemiology of surgery-associated AKI is important for healthcare policy, quality initiatives, clinical trials, as well as for improving guidelines. The objective of the Epidemiology of Surgery-associated Acute Kidney Injury (EPIS-AKI) trial is to prospectively evaluate the epidemiology of AKI after major surgery using the latest Kidney Disease: Improving Global Outcomes (KDIGO) consensus definition of AKI. Methods and analysis EPIS-AKI is an international prospective, observational, multicentre cohort study including 10 000 patients undergoing major surgery who are subsequently admitted to the ICU or a similar high dependency unit. The primary endpoint is the incidence of AKI within 72 hours after surgery according to the KDIGO criteria. Secondary endpoints include use of renal replacement therapy (RRT), mortality during ICU and hospital stay, length of ICU and hospital stay and major adverse kidney events (combined endpoint consisting of persistent renal dysfunction, RRT and mortality) at day 90. Further, we will evaluate preoperative and intraoperative risk factors affecting the incidence of postoperative AKI. In an add-on analysis, we will assess urinary biomarkers for early detection of AKI. Ethics and dissemination EPIS-AKI has been approved by the leading Ethics Committee of the Medical Council North Rhine-Westphalia, of the Westphalian Wilhelms-University Münster and the corresponding Ethics Committee at each participating site. Results will be disseminated widely and published in peer-reviewed journals, presented at conferences and used to design further AKI-related trials. Trial registration number NCT04165369. ©

Abstract

Objective To evaluate the use of a Cannabis sativa oil in the management of patients diagnosed with primary burning mouth syndrome (BMS). Design Prospective, open-label, single-arm pilot study. Setting University hospital. Subjects Seventeen patients with diagnosed BMS were included. Methods Subjects were treated for 4 weeks with a full cannabis plant extract, which was prepared from standardized plant material (cannabis flos) in specialized pharmacies by means of Romano-Hazekamp extraction and was diluted in oil (1 g of cannabis in 10 g of olive oil). The primary outcome was the change in pain intensity (assessed by the visual analog scale, Present Pain Intensity scale, McGill Pain Questionnaire, and Oral Health Impact Profiles) at the end of the protocol and during the succeeding 24 weeks; the neuropathic pain was also investigated with a specific interview questionnaire (DN4-interview [Douleur Neuropathique en 4 Questions]). Levels of anxiety and depression were considered as secondary outcomes, together with reported adverse events due to the specified treatment. Results Subjects showed a statistically significant improvement over time in terms of a clinical remission of the oral symptoms. Levels of anxiety and depression also changed statistically, displaying a favorable improvement. No serious reactions were detailed. None of the patients had to stop the treatment due to adverse events. Conclusions In this pilot evaluation, the C. sativa oil provided was effective and well tolerated in patients with primary BMS. Further bigger and properly defined randomized controlled trials, with different therapeutic approaches or placebo control, are needed, however.

Abstract

Ceruminous adenoma is described as a glandular neoplasm of ceruminous glands. It is seen for less than one percent of all external ear tumours. Ceruminous adenoma cases were reported to have recurrence and residual tumour, however there was no malignant transformation known in ceruminous adenoma up to this date. Here, we report a young adult woman with two years history of progressive reduced hearing and tinnitus of the right ear. She was proven to have right moderate conductive hearing loss with pure tone audiometry. There was a cystic mass with serous content arising from the posterior wall of the right ear canal. We proceeded with wide local excision of the mass via trans-canal approach. Ceruminous adenoma was confirmed with histopathology and immunohistochemistry of CK7 staining. The hearing impairment was resolved completely post excision and there was no recurrence of the tumour on one year follow-up. We concluded wide local excision with appropriate margin of the mass is adequate to prevent recurrence in ceruminous adenoma cases.Serüminöz adenom, serüminöz bezlerdeki tümör olarak tanımlanır. Tüm dış kulak tümörlerinin yüzde birinden azını oluşturmaktadır. Serüminöz adenom olgularında tekrarlama ve artık tümör kalma durumları bildirilmiştir ancak bu tarihe kadar serüminöz adenomda bilinen kötücül transformasyon saptanmamıştır. Bu çalışmada, sağ kulakta iki yıllık ilerleyici işitme azalması ve kulak çınlaması öyküsü olan genç bir yetişkin kadını sunuyoruz. Saf ton odyometresi ile orta derecede iletim tipi işitme kaybına sahip olduğu kanıtlanmıştır. Sağ kulak kanalı arka duvarından kaynaklanan seröz içerikli kistik kitle bulunmuştur. Trans-kanal yaklaşımla kitlenin geniş lokal eksizyonunu gerçekleştirilmiştir. Serüminöz adenom, CK7 boyama histopatolojisi ve immünohistokimyası ile doğrulanmıştır. İşitme bozukluğu eksizyon sonrası tamamen düzeldi ve bir yıllık takipte tümör tekrarı saptanmamıştır. Serüminöz adenom olgularında tekrarları önlemek için sağlıklı kitle marjına sahip geniş lokal eksizyonun yeterli olduğu sonucuna vardık.

Abstract

Liver failure (LF) is a severe entity, which represents a diagnostic challenge for clinicians. Knowing the etiology of liver aggression is key for its management. We present a rare case of cholestatic LF secondary to primary AL (kappa-light chain) amyloidosis as a presentation associated to multiple myeloma.

Abstract

The availability of clean water is one of the main problems for the people in Kendari City. Overutilization of groundwater can cause seawater intrusion to groundwater. This study aims to identify and map seawater intrusion distribution to shallow groundwater based on water quality data. Sampling was obtained from 100 dug wells of the inhabitant (n=100) in March-July 2019 with a variation of the distance from the beach 0.04-5.52 km and the elevation of Groundwater (MAT) between 0.43-30.01 MASL. The distribution of shallow groundwater is mapped on the criteria to the value of Electrical Conductivity (DHL) at the standard temperature of 25 °C. The analysis showed that the average groundwater temperature was 29.28 °C, DHL was 393.52 µmhos, Total Dissolved Solid (TDS) was 196.98 ppm, and pH was 6.31. Shallow groundwater, which has been varied intruded with high intruded (26.26%), rather high intruded (14.14%), medium intruded (32.32%), low intruded (6.06%) and not intruded (21.21%). The correlation between distance from the coast to the value of DHL was not significantly correlated (R2 = -0.49), while groundwater level elevation has a significant correlation (R2 = 0.71). The geological factors of alluvial deposits, which generally have low permeability and slope to flat topography, cause seawater intrusion to shallow groundwater. Sumber air bersih masyarakat diperoleh dari airtanah. Pemanfaatan airtanah yang berlebihan dapat menyebabkan terjadinya intrusi air laut terhadap airtanah. Penelitian ini bertujuan untuk mengidentifikasi dan memetakan sebaran intrusi air laut terhadap airtanah dangkal berdasarkan data kualitas air. Pengambilan sampel diperoleh dari sumur gali penduduk sebanyak 100 sampel (n=100) pada periode Maret-Juli 2019 dengan variasi jarak dari pantai 0,04-5,52 km dan elevasi muka airtanah (MAT) antara 0,43-30,01 mdpl. Hasil analisis menunjukan bahwa rerata suhu airtanah sebesar 29,28 °C, DHL sebesar 393,52 µmhos, Total Dissolved Solid (TDS) sebesar 196,98 ppm dan pH sebesar 6,31. Airtanah dangkal secara umum telah terintrusi dengan variasi meliputi terintruksi tinggi (26,26%), terintrusi agak tinggi (14,14%), terintrusi sedang (32,32%), terintrusi sedikit (6,06%) dan tidak terintrusi (21,21%). Korelasi antara jarak dari pantai terhadap nilai DHL tidak signifikan (R 2 = -0,49), sedangkan MAT memiliki korelasi signifikan (R 2 = 0,71). Faktor geologi berupa endapan alluvial yang memiliki permeabilitas umumnya rendah dan berada pada topografi landai hingga datar menyebabkan terjadinya intrusi air laut terhadap airtanah dangkal.
