WOS: 000411484400007, Objective: The incidence of colorectal cancer in developed countries has been found to increase with age. Early diagnosis and screening decrease the mortality rates in colorectal cancer. This study aimed to use inflammatory markers neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and red blood cell distribution width (RDW)as new biomarkers for early diagnosis and screening in patients with colorectal cancer. Materials and Methods: A total of 59 patients with colorectal cancer and 59 age-and sex-matched healthy participants were included in the study. Localization, tumor node metastasis (TNM) stage, and preoperative hemoglobin levels, neutrophil counts, lymphocyte counts, platelet counts, and RDW values were obtained from medical records. Using the receiver operating characteristic (ROC) curve, the optimal cutoff levels of the biomarkers were determined. Results: NLR, PLR, and RDW were significantly higher in patients with colorectal cancer than in healthy participants (p< 0.001). According to ROC analysis, the cutoff value for NLR was 2.05 [area under the curve (AUC): 0.740, sensitivity: 78%, specificity: 66%]; the cutoff value for PLR was 130 (AUC: 0.702, sensitivity: 65%, specificity: 72%); and the cutoff value for RDW was 14 (AUC: 0.774, sensitivity: 68%, specificity: 73%). Conclusions: NLR, PLR, and RDW were found to be significantly higher in patients with colorectal cancer than in healthy participants. Therefore, it is recommended that these additional biomarkers can be used for early diagnosis and screening of colorectal cancer.

WOS: 000372329600005, Objective: Factor V (FV) (1691 G-A) and methylenetetrahydrofolate reductase (MTHFR) (677 C-T) mutations have been identified as potential risk factors for cardiovascular disease. In this study, we determined vitamin B-12, folate, and total homocysteine t(Hcy) concentrations in heterozygote MTHFR (677 C-T) and FV (1691 G-A) mutation-carrying individuals. Materials and Methods: The study included a total of 74 individuals with MTHFR (677 C-T) or FV (1691 G-A) mutations and 70 controls. All subjects had the history of thromboembolic disease. t(Hcy), folate, and vitamin B-12 concentrations were compared between the groups. Results: A significant difference was found in vitamin B-12 and folate concentrations between patients and controls in the MTHFR (677 C-T) group (p=0.041, p=0.049, respectively). Further, t(Hcy) concentrations were found to be higher in patients than in controls in the FV (1691 G-A) mutation-carrying group (p=0.002). No significant difference was found between the groups in relation with gender in both mutations. Conclusion: t(Hcy) concentrations should be assessed to decrease the risk of future venous thromboembolism in the presence of heterozygote FV (1691 G-A) mutation.

Today, abuse of volatile substances is an increasingly common public health problem in Turkey, as throughout the world. These substances are attractive to childen and adolescents as products containing inhalants are relatively inexpensive, legally and easily accessible, have a rapid effect and unwanted symptoms disappear quickly. Case No 1: A 16-year old male collapsed in the street who was intubated by paramedics and taken to the Emergency Department. A cigarette lighter and gas refill can were found in his pockets. Case No 2: The dead body of a 17-year old male who lived with his parents was found in the kitchen of their house. There was a court record of substance abuse inhalation. In the autopsies of both cases, the brain and cerebellum were observed to be hyperemic and oedematous. Hyperemia was also observed in tracheal and gastric mucosa. Toxicological and histopathological analyses were applied after the autopsies. Butane, as the active ingredient of lighter gas, was confirmed in samples of blood, bile and internal organs in the toxicology analysis. Histopathological examination of the brain, cerebellum, lung, liver and kidneys determined congestion. In this study, it was aimed to emphasize the importance of the causes of the increasing prevalence of inhalant abuse in children and young adults and the significance of forensic autopsy in the determination of the cause of death. To prevent inhalant abuse of substances which can be obtained cheaply and easily in our country, visual-auditory education-training and appropriate legal regulations are required. WOS: 000432307500007, Objective: This study aimed to determine the frequency of HBV reactivation and the potential predictive factors in resolved hepatitis B virus (HBV) patients [hepatitis B surface antigen (HBsAg)-negative and antibody to hepatitis B core antigen (antiHBc)-positive patients] who received rituximab-containing chemotherapy for B-cell lymphoma. Materials and Methods: A retrospective examination was performed for HBV-related markers in 106 patients before and after receiving rituximab-containing chemotherapy for CD20-positive B-cell lymphoma. Results: Of the 106 patients with CD20-positive B-cell lymphoma who received rituximab-containing chemotherapy, 98 were HBsAg negative and 8 were HBsAg positive; among the 98 patients, 64 (65.7%) were anti-HBc negative and 34 (34.7%) were anti-HBc positive. Of the 34 CD20-positive B-cell lymphoma patients with resolved HBV infection who received rituximab-containing chemotherapy, 26 (76.5%) were anti-HBsAg antibody (anti-HBs) positive and 8 (23.5%) were anti-HBs negative. Of these 8 anti-HBs-negative patients, HBV reactivation occurred in 4 (50%) patients; no HBV reactivation was observed in any of the 26 anti-HBs-positive patients. Amaç: Mortalite ve morbidite oranı oldukça yüksek olan intraserebral hemorajinin(İSH) prognozunda inflamatuar süreç oldukça önemli bir yer tutar. İnflamatuar bir gösterge olarak kabul edilen platelet/lenfosit oranının(PLO); son yapılan çalışmalarla özellikle akut koroner sendrom, kalp kapak hastalıkları, çeşitli onkolojik hastalıklar ve serebral infarktüste prognozla ilişkili olduğu ortaya konmuştur. Ancak akut dönem İSH’li hastaların prognozunu göstermedeki yeri henüz bilinmemektedir. Gereç ve Yöntemler: Çalışmamıza retrospekif olarak Ocak 2008- Haziran 2016 tarihleri arasında kliniğimize şikayetlerinin başladığı ilk 24 saat içinde başvuran, İSH tanısı kesinleşmiş 296 hasta ile benzer yaş ve cinsiyete sahip 180 kontrol dahil edildi. 296 hastanın 120 tanesi 30 günlük hastane yatışı sırasında hayatını kaybetmişti. Hastalarla kontrollerin ve ölenlerle sağ kalan hastaların nötrofil/lenfosit oranları(NLO) ile PLO değerleri karşılaştırılarak 30 günlük mortaliteyi saptamada NLO ve PLO’nun rolü değerlendirildi. Bulgular: Hasta grubu kontrollerle, ölen hastalar sağ kalan hastalarla kıyaslandığında nötrofil ve platelet sayıları ile NLO ve PLO değerleri hasta grubunda ve özellikle de ölen hastalarda daha yüksekti. Yapılan tek ve çok değişkenli lojistik regresyon analizlerinde, NLO ve PLO değerlerindeki yüksekliğin akut dönem İSH’li hastalardaki kısa dönem mortaliteyi saptamada anlamlı bağımsız değişkenler olduğu saptandı(p= 0.014,p=0.024).Ek olarak yapılan ROC analizi ile PLO için 261.7, NLO için ise 9.5 üzerindeki değerlerin İSH’ye bağlı 30 günlük mortaliteyi saptamada kullanılabileceği gösterildi. Sonuç: Bu sonuçlarla akut dönem İSH’lı hastalarda NLO ve PLO değerlerinin arttığı ve mekanizması tam olarak bilinmese de NLO ve PLO değerlerindeki yüksekliğin İSH’li hastalarda kısa dönem mortalite ile yakından ilişkili olduğu söylenebilir., Aim: Inflammation plays an important role in the prognosis of intracerebral hemorrhage(ICH), which has high mortality and morbidity rates. Platelet to lymphocyte ratio(PLR), which was accepted as an indicator of inflammation, have been increasingly demonstrated a prognostic factor, particularly in acute coronary syndrome, various oncological diseases, and in cerebral infarction in recent studies. Nonetheless, its significance regarding the determination of prognosis in patients with ICH has not yet been clarified. Materials and Methods: This is a retrospective study including 296 patients who presented to our clinic within 24 hours of the onset of symptoms and who were diagnosed with ICH between January 2008 and June 2016, along with 180 age and sex-matched controls. During their 30 day-hospitalization, 120 of these 296 patients died. Patients and controls were compared in terms of neutrophil to lymphocyte ratio(NLR) and PLR values; these were also compared between died and surviving patients. The significance of NLR and PLR in predicting mortality was also evaluated. Results: Patients had significantly higher neutrophil and platelet counts and NLR and PLR values than controls; these results were similar between died and surviving patients. Univariate and multivariate logistic regression analyses indicated that NLR and PLR were independent predictors of short term mortality in ICH (p= 0.014, p=0.024). In addition, ROC analysis showed that cut-off values of 261.7 for PLR and 9.5 for NLR may be used to determine 30-day mortality due to ICH. Conclusion: These results indicate that NLR and PLR can increase in patients with ICH, and that although the mechanism is unclear, the increase of NLR and PLR is closely associated with short term mortality in patients with ICH.

WOS: 000436137800011, Objective: The objective of this study was to evaluate the antimicrobial activity of different power settings of potassium-titanylphosphate (KTP) laser in experimentally infected root canals. Methods: A total of 119 freshly extracted human single-rooted teeth with mature apices were selected for study. After preparation and sterilization, the specimens were inoculated with 15 mu L Enterococcus faecalis for 24 hours at 37 degrees C. The contaminated roots were divided into five experimental groups (1W, 1.5W, 2W, 3W, and 4W KTP laser), one negative control [sodium hypoclorite (NaOCl)] group, and one positive control [sterile saline (SF)] group of 17 teeth each. Before and after applications in the groups, samples retrieved with sterile paper points from the root canal were transferred to tubes containing 5 mL of the brain heart infusion broth. Next, 10 mu L of these suspensions were placed at two different sides of the blood agar medium. Bacterial reduction was counted according to the colony-forming units and data were statistically analyzed using the Kruskal-Wallis and Tukey's tests. Results: The maximum decrease occurred in the 2.5% NaOCl (100%+/- 0.00%) group. This decrease was followed by decreases in the following KTP laser groups: 4W (94.26%), 3W (87.98%), 2W (77.43%), SF (68.80%), 1.5W (65.89%), and 1W (52.08%). WOS: 000432307500004, Objective: We aimed to compare the efficacies of laminaria, vaginal misoprostol, and mechanical dilator applications before surgical intervention with regard to the optimal cervical dilation and severity of pain in first-trimester pregnant women with missed abortion. Materials and Methods: The prospective study included a total of 103 patients with a diagnosis of missed abortion in the sixth-tenth gestational week randomly divided into 3 groups of 400 pg vaginal misoprostol. luminaria. or mechanical dilator applications for the dilation of the cervix before surgical intervention. The effects of laminaria and vaginal misoprostol on the cervical ripening for surgical intervention were evaluated using the Hegar test. Visual analog scale (VAS) values were evaluated during the first application, throughout the period of their applications, and after the surgical intervention. Results: The cervical dilation was found to be similar in the laminaria and vaginal application groups (p=0.64). During the first application. laminaria caused more pain than misoprostol but the difference was not statistically significant (p=0.28). Throughout the period of application and after the surgical intervention. although there was less pain with respect to VAS values after laminaria application. this was not statistically significant (p=0.11). The VAS values after the surgical intervention was determined to be statistically higher after mechanical dilatation compared to other procedures (p=0.001). Conclusion: Laminaria provides cervical preparedness similar to intracervical misoprostol without increasing the side effects in the management of first-trimester pregnant women with missed abortion.

Bu çalışmada asendan aortta anevrizmal dilatasyonla eş zamanlı bulgulanan splenik arter anevrizma olgusunu sunmayı amaçladık., Due to the advanced diagnostic tools, aneurysms of splenic artery as the 3rd most common cause of intraabdominal aneurysms, could be detected more commonly although 80 % are asymptomatic. Rupture is its most significant complication. If the diameter of the aneurysm exceeds 20 mm surgical therapy is the safest and most effective option. It is manifested by erosions and blisters commonly seen in head and neck area and traumatized areas. In literature, practice of coronary revascularization in bullous pemphigoid cases is very rarely seen. Therefore we aimed to present our strategy in such a case.

Multiple sklerozlu bireylerde vücut sıcaklığı düşürülerek sinir iletisinin hızlandırılması ve semptomların giderilmesi amacıyla çeşitli soğutma tedavisi yöntemleri kullanılmakta ve yorgunluk düzeyinde azalmalar olduğu bildirilmektedir. MS hastalarında soğutma tedavisi; soğuk su içme, soğuk duş alma, deriye soğuk ped uygulama veya soğutucu giysi gibi yapay soğutma sistemlerinin kullanımı ile sağlanmaktadır. Bu derleme, MS hastalarında yorgunluk yönetiminde soğutucu giysi uygulaması ile ilgili literatür bilgisini paylaşmak amacıyla yazılmıştır., Fatigue is one of the most common symptoms in individuals with multiple sclerosis. In the studies evaluating the fatigue associated with multiple sclerosis, it was determined that fatigue increased with the elevation of body and environmental temperature in most of the cases. Various cooling methods are being used to accelerate nerve conduction and eliminate symptoms by reducing body temperature in the individuals with multiple sclerosis and it is reported that level of fatigue decreased with cooling therapies. In MS patients cooling treatment is provided by use of artificial cooling systems, such as drinking cold water, taking a cold shower, applying cold pad or cooling suits. This review aims to share knowledge about the application of cooling clothing in fatigue management in MS patients.

Amaç: Omurilik hasarı inflamatuar yanıta ve oksidatif strese yol açarak çeşitli organ sistemlerinde zararlı etkiler oluşturur. Riboflavin insan ve hayvanda sağlığın sürdürülmesinde önemli role sahip olan ve kolayca absorbe edilen mikrobesindir. Bu çalışma omurilik yaralanmasına bağlı omurilik ve böbrek dokusunda riboflavinin koruyucu etkilerini araştırmak üzere planlandı.Yöntemler: Omurilik hasarı oluşturmak için anestezi altındaki sıçanlara T10 seviyesinde 100 g/cm şiddetinde ağırlık düşürme metodu uygulandı. Hasarlı hayvanlara riboflavin 25 mg/kg dozunda ya da taşıyıcı çözelti tedavisi hasardan 15 dakika sonra verildi ve 7 gün süreyle devam edildi. Hasardan sonra 7.günde nörolojik testin arkasından hayvanlar dekapite edilerek spinal ve böbrek dokuları alındı. Dokularda histolojik tayinler yapıldı ve malondialdehit (MDA), glutatyon (GSH), 8-hidroksi-2-deoksiguanozin (8-OHdG) düzeyleri ile myeloperoksidaz (MPO), süperoksid dismutaz (SOD) ve katalaz aktiviteleri tayin edildi.Bulgular: Omurilik hasarı dokularda GSH düzeylerinde ve SOD aktivitesinde azalamaya, MDA düzeyinde ve MPO ve kaspaz aktivitelerinde artışa neden oldu. Riboflavin tedavisi bu parametreleri geri çevirdi ve histolojik bulgularda düzelme gösterdi.Sonuç: Çalışmamızda Omurilik hasarı, dokuya nötrofil göçüne ve oksidan strese yol açarken, antiapoptotik ve nöroprotektif özellikleri ile riboflavin lipid peroksidasyonunu ve nötrofillerin dokuya infiltrasyonunu inhibe etti. Ayrıca, çalışmamız riboflavinin antiapoptotik ve antioksidan etkisinin sadece omurilikte değil omurilik hasarında ikincil olarak ortaya çıkan böbrek hasarında da önemli faydaları olduğunu gösterdi., Objective: Spinal cord injury (SCI) leads to an inflammatory response and results in oxidative stress, which has deleterious effects on several organ systems. Riboflavin is an easily absorbed micronutrient that plays an important role in maintaining health in humans and animals. The present study was designed to investigate the putative protective effect of riboflavin against SCI-induced spinal cord and kidney damage.Methods: In order to induce SCI, the standard weight-drop method was used to induce a moderately severe injury (100 g/cm force) at the T10 vertebral level. Injured animals were given either 25 mg/kg riboflavin or carboxymethyl cellulose 15 min after injury, and this regimen was repeated twice daily for 7 days. On the 7th post-injury day, a neurological examination was performed and rats were sacrificed. Spinal cord and kidney samples were harvested and prepared for a histological examination. Tissue levels of malondialdehyde (MDA), glutathione (GSH), and 8-hydroxy-2′-deoxyguanosine (8-OHdG) and activities of myeloperoxidase (MPO), superoxide dismutase (SOD), and caspase-3 were determined.Results: SCI caused a significant decrease in tissue GSH levels and SOD activities, which were accompanied by significant increases in MDA and 8-OHdG levels and MPO and caspase-3 activities. However, riboflavin treatment reversed these parameters and improved histological findings.Conclusion: SCI caused tissue injury through oxidative stress and neutrophil infiltration into tissues. Riboflavin inhibited tissue injury through its neuroprotective and antiapoptotic effects. Moreover, our study demonstrated that riboflavin not only exerts antioxidant and antiapoptotic effects on the spinal cord but also has a significant impact on preventing kidney damage secondary to SCI.

Amaç: Araştırma klimakterik dönemdeki kadınların cinsel yaşam kalitelerinin belirlenmesi ve cinsel yaşam kalitesinin menopozal tutumlar ve yakınmalarla ilişkisinin incelenmesi amacı ile yapılmıştır.Çalışma Planı: Araştırmanın örneklemini, en az ilkokul mezunu ve menopozal dönemde olan 1 Eylül - 1 Aralık 2014 tarihleri arasında Sivas Devlet Hastanesi ve Cumhuriyet Üniversitesi Sağlık Hizmetleri Araştırma ve Uygulama Hastanesi Kadın Hastalıkları ve Doğum Polikliniğine gelen 359 kadın oluşturmuştur. Araştırmada 23 soruluk 'Kişisel Bilgi Formu', 20 soruluk 'Menopoza İlişkin Tutum Ölçeği', 15 soruluk 'Menopozal Yakınmaları Tarama Listesi', 18 soruluk 'Cinsel Yaşam Kalitesi Ölçeği' kullanılmıştır. Verilerin değerlendirilmesinde ortalama, standart sapma, yüzde, bağımsız iki örnek t testi, regresyon analizi kullanılmış olup istatistiksel anlamlılık 0,05 önem düzeyinde incelenmiş, SPSS 14 paket programı kullanılmıştır.Bulgular: Araştırmada elde edilen bulgulara göre, menopozal tutum ölçeği puanlarındaki artışlar cinsel yaşam kalitesi ölçek puanı üzerinde artışlara neden olmaktadır. ICD lead'i benzeri intrakardiyak yabancı cisimlerdeki mikrobiyal infeksiyon tanımını da içeren infektif endokardit tanı ve tedavideki büyük ilerlemelere rağmen halen sağkalımı düşük yaşamı tehdit eden bir patolojidir. Bu çalışmada hipertrofik non-obstrüktif kardiyomyopatili olguda gelişen infektif endokardite bağlı ICD lead'i ile üzerindeki vejetasyonların inflow oklüzyon tekniğiyle başarılı ekstirpasyon yaklaşımımızı sunmayı amaçladık. Yüksek sağkalım oranı açısından kültürantibiyograma dayalı tüm duyarlı antibiyoterapiye rağmen lead endokarditinde cerrahi ekstirpasyon birincil seçenek olmayı sürdürmektedir. Özellikle inflow oklüzyon tekniğiyle, kardiyopulmoner by pass'ın da majör yan etkilerinden kaçınılarak uygulanacak açık cerrahi ekstirpasyonun güvenilirliği ve etkinliği bu olgu aracılığıyla kanıtlanmaktadır, Infective endocarditis, including the microbial infections on ICD lead-like intracardiac foreign bodies, is a life-threatening pathology with poor survival in spite of all advances in diagnosis and treatment. In this study we aimed to present our successful extirpatıon of ICD lead and its vegetations with inflow occlusion technique developing in a hypertrophic non-obstructive cardiomyopathy case secondary to infective endocarditis. Surgical extirpation of the infected lead is still treatment of choice for better survival in lead endocarditis in spite of all susceptible antibiotherapy based on antibiogram. Particularly with inflow occlusion technique, one may avoid major side effects of cardiopulmonary bypass. Open surgical extirpation using this technique is a safe and effective method as proven with this case

Amaç: Elektromanyetik alanın (EMA) farklı ağrı türlerini azalttığı bilinmektedir. Bununla birlikte, manyetik alanın analjezik etki mekanizması tam olarak anlaşılamamıştır. Bu çalışmanın amacı, nitrik oksit sentaz (NOS) inhibitörlerinin sıçanlarda çok düşük frekanslı EMA maruziyeti ile oluşan analjezi üzerine etkilerini araştırmaktır.Gereç ve Yöntem: Bu çalışmada 72 yetişkin erkek Wistar albino sıçan (yaklaşık 230 ± 12 g ağırlığında) kullanıldı. Sıçanlar, 22 ± 2 °C oda sıcaklığında, 12 saat aydınlık/karanlık siklusun sağlandığı ve ses yalıtımı olan ortamda tutuldu. Amaç: Bebeklerin her hangi bir nedenle annelerinin sütü ile beslenemediği durumlarda kullanılabilecek en ideal kaynaklardan birisi banka sütü olup, anne sütü ile beslenmenin sürdürülmesinde sağlık personelinin bilgi ve görüşleri önemlidir. Bu nedenle çalışma, Doğum-Kadın Sağlığı ve Çocuk Sağlığı alanlarında çalışan hemşire, ebe ve hekimlerin anne sütü bankalarına ilişkin bilgi ve görüşlerinin belirlenmesi amaçlanmaktadır. Gereç ve Yöntem: Tanımlayıcı olarak Ankara ve Malatya’da yapılmıştır. Örneklem, uygulamanın yapıldığı birimlerde görevli, çalışma hakkında aydınlatılmış onamları alınmış toplam 344 hemşire, ebe ve hekiminden oluşmuştur. Methods: The descriptive study conducted in Ankara and Malatya. The sample consists of 344 nurses, midwives and physicians who worked in clinical practice and obtained informed consent about the study. Giriş ve Amaç: Bakım kalitesinin değerlendirilmesinde en önemli göstergelerden biri hastanın verilen bakım hizmetine yönelik memnuniyetidir. Araştırma, ülkemizde hemşirelik bakımının hastalar tarafından nasıl algılandığının belirlenmesi amacı ile yapılmıştır.Yöntem: Tanımlayıcı türde olan araştırma yedi coğrafi bölge kapsamında her bölgeden ikişer ilden seçilen 15 hastanede yatarak tedavi gören 2272 hastadan oluşan örneklem grubu ile çok merkezli olarak yürütülmüştür. Veriler, Hasta Tanıtım Formu ve Hemşirelik Bakımı Algılayış Ölçeği ile elde edilmiştir.Bulgular: Araştırmada hastaların yaş ortalaması 50,27±18,26 ve Hemşirelik Bakımını Algılayışları Ölçeği puan ortalaması 60,44±9,41 bulunmuştur. This research was conducted with the aim of determining how nursing care is perceived by patients in our country.Methods: This descriptive multi-centeredstudy was carried out with a sample group of 2272 patients receiving inpatient treatment in 15 outpatient hospitals within seven geographic regions with two in each region. Data was obtained with the Patient Presentation Form and the Nursing Care Perception Scale.Results: The mean age of patients was 50.27±18.26 and the mean score of Nursing Care Perceptions Scale was 60.44±9.41. The highest rated items were "I felt I was well taken care of by the nurses" and "I'm sure they would be there when I needed them". The lowest rated item was "They informed me about things I do not know about the hospital". It was found that a significantly higher total score of Nursing Care Perceptions Scale was measured for patients having chronic illnesses, for those who were three or more times hospitalized, or more likely to prefer the hospital again, who had education lower than upper secondary level, who stayed longer than 31 days in the hospital, those living in rural areas, and for patients who stay in single rooms.Conclusion: According to the findings of the research, nursing care perception regarding patients' confidence in nursing care was positive. The high patient/nurse ratio and multi-person patient rooms, that effect the perception of care adversely, should be improved.

Kardiyak tutulum bulgulanan Sjögren sendromlu olgular genelde asemptomatiktir. Bu çalışmada 17 yıldır primer Sjögren sendromu tanısıyla tedavi altındaki senil olguda başarılı koroner revaskülarizasyon stratejimizi sunmayı amaçladık. Amaç: Bu araştırmanın amacı, üniversite öğrencilerinin cinsellik ve cinsel sağlıkla ilişkili bilgi düzeylerini değerlendirmeyi sağlayacak bir ölçme aracı geliştirmek, geçerlik ve güvenirliğini belirlemektir. Yöntem: Cinsel Sağlık Bilgi Testi'nin geçerlilik ve güvenilirliğini belirlemek amacıyla yapılan metodolojik çalışmada, katılımcıların cinsellikle ilgili konularda bilgi düzeylerini ölçmek amacıyla 59 sorudan oluşan çoktan seçmeli test oluşturulmuştur. Uzman görüşü yoluyla testin kapsam geçerliliği değerlendirilmiştir. Uzmanların önerileri çerçevesinde testte yer alan sorularda düzeltmeler yapılmıştır. Test, bir devlet üniversitesinde okuyan 113 öğrenciye uygulanmıştır. Verilerin analizi SPSS 14 programında yapılmıştır. İstatistiksel analizde madde güçlük indeski, madde ayırt edicilik indeksi ve Cronbach alfa güvenilirlik katsayısı hesaplanmıştır. Bulgular: Yapılan analizler doğrultusunda güvenilirlik katsayısı 0.88 olan 40 sorudan oluşan testin son hali elde edilmiştir. Sonuç: Cinsel Sağlık Bilgi Testi'nin, üniversite öğrencilerinin cinsellik ve cinsel sağlıkla ilişkili bilgilerini ölçmede geçerli ve güvenilir bir ölçme aracı olduğunu belirlenmiştir., Aim: The aim of this study was order to an measuring tool develop, validity and reliability that it will allow individuals to determine their level of sexuality and sexual health-related issues information. Method: In this methodological study a questionnaire composed of 59 multiple choice questions, regarding issues related to individuals' sexuality, was composed. Validity of question content was reviewed by an expert. Recommended corrections were made and questions determined to be unsuitable were excluded according to the expert's opinion resulting in 40 approved questions. The questionnaire was distributed to students (n=113) at Cumhuriyet University. Data was evaluated with the SPSS 14 package program. Correlation of item difficulty and item discrimination values were calculated in the statistical analysis. Cronbach alpha reliability was calculated in order to determine the internal consistency of questions in the questionnaire. Results: The Cronbach alpha reliability coefficient was found to be 0.88. Conclusion: Results obtained from this study showed that the Sexual Health Questionnaire was an effective measurement tool with a high degree of reliability and validity in determining individual sexuality, and sexual health-related, issues.

Amaç: Sağlık kurumlarındaki şiddet hasta, hasta yakınları ya da diğer herhangi bir bireyden gelen, sağlık çalışanı için risk oluşturan tehdit davranışı, sözel tehdit, fiziksel saldırı ve cinsel saldırıdan oluşan durum olarak tanımlanmıştır. Tüm meslek grupları arasında, zor durumdaki bireylerle doğrudan teması gerektiren sağlık hizmet sektöründe çalışanların, iş yeri şiddetinin en önemli hedefi olduğu giderek daha fazla kabul görmektedir. Bu çalışmada Sivas il merkezindeki sağlık çalışanlarının şiddete maruziyet sıklıkları araştırılmıştır. Materyal ve Metot: Sivas il merkezinde 2.927 sağlık çalışanı görev yapmaktadır. Kistik ekinokokkozis (KE) zoonotik bir hastalık olup, ülkemiz gibi hayvancılıkla uğraşılan bölgelerde endemik görülen bir sağlık problemidir. Ço- cuklarda en sık akciğer tutulumu görülür. Akciğerdeki kistler asemptomatik olabileceği gibi bazen de rüptüre olarak semptomatik hale gelirler. Rüptüre akciğer hidatik kistleri sıklıkla klinik ve radyolojik olarak tüberküloz (Tbc) ile karışabilmektedir. Bu yazıda 2 haftadan uzun süredir devam eden öksürük, ateş ve balgam şikayetiyle başvuran, akciğer grafisi ve toraks bilgisayarlı tomografisi (BT) öncelikle Tbc lehine yorumlanan fakat izleminde rüptüre akciğer KE tanısı ile opere edilen 8 yaşındaki kız hastayı sunuyoruz. Bu olgu ile solunum sistemi bulgularıyla başvuran ve hayvan ile temas öyküsü olan çocuklarda ayırıcı tanıda KE ve Tbc’nin mutlaka düşünülmesini ve tanının diğer mikrobiyolojik-serolojik testlerle desteklenmesi gerektiğini vurgulamak istedik., Cystic echinococcosis (CE) is a zoonotic disease; in places such as Turkey where livestock is common, it is an endemic health concern. The most commonly involved organ is the lungs in children. Pulmonary cysts can be asymptomatic; in some cases, they may rupture and become symp- tomatic. Ruptured lung hydatid cysts may often be confused with tuberculosis (Tbc) radiologically and clinically. . In this report, we present an 8-year-old female patient admitted with cough, fever, and sputum persisting since 2 weeks; her chest radiography and computed tomography (CT) findings initially indicated Tbc, but the follow-up surgery led to a diagnosis of ruptured lung CE. We want to emphasize that in children be- longing to places where livestock is common, if respiratory symptoms are observed, CE and tuberculosis must be considered in the differential diagnosis, and the final diagnosis should be supported by other microbiological-serological tests.

Amaç: Hipofiz adenomları lokal bası etkisi ve hormon salgıları nedeniyle ciddi morbidite ile seyretmektedirler. Tedavideki amaç ise bu bası etkisinin ortadan kaldırılması, hormon hipersekresyonunun normale getirilmesi, normal ön ve arka hipofiz fonksiyonlarının korunması ve olası rekürrens ihtimalinin ortadan kaldırılmasıdır. Gereç ve Yöntemler: Çalışmamızda hipofiz lojunda yer kaplayan lezyonu olan ve transsfenoidal mikrocerrahi yapılan 70 hastanın preoperatif, postoperatif erken dönem ve üçüncü ayda alınan hormon değerlerini, endokrinolojik remisyonları açısından incelemeyi amaçladık. Bulgular: Olguların en sık başvuru şikayeti baş ağrısı, galaktore, görme bozukluğu ve akromegalik semptomlardı. Tümör boyutları 70 olgunun 45'inde (64.28%) makroadenom, 25'inde (35.7%) mikroadenom olarak saptandı. Olguların 11 tanesi endokrin inaktif adenoma , 25 tanesi prolaktinoma, 17 tanesi GH hormon sekrete eden adenoma, 10 tanesi ACTH sekrete eden adenom ,6 tanesi GH ve prolaktinsekrete eden adenom , 1 olgu ACTH ve prolaktinoma sekrete eden adenom olarak değerlendirildi. TSC yöntemiyle opere edilen tüm hastalardaki endokrin kür oranları: akromegali olgularında % 76.4, akromegali ve prolaktinomalı olgularda %66.6, cushing olgularında %90 cushing ve prolaktinomalı olgularda %100, prolaktinomalı hastalarda %80 oranında bulunmuştur.Sonuç: Sonuç olarak transsfenoidal mikrocerrahi daha düşük mobidite ve mortalite oranı ile tümör çıkarılırken normal pitüiter bezin korunmasında güvenli ve etkin bir yöntem olarak bulunmuştur., Objective: Hypophyseal adenomas cause very high morbidity because of local mass effect and hypophyseal hormones secretion. Treatment goals are hormone secretion normalization, removing local mass effect, protection of both anterior and posterior hypophyseal functions and prevent recurrence. Meterial and Methods: In our study, we examined pre-operative, post-operative early days and three months after surgery hormonal values in 70 patients operated via transsphenoidal surgery to evaluate endocrinologic remission. Patients complaints were mostly headache, galactorrhea, vision problems and acromegaly.Results: Hypophyseal lesion size were macroadenoma in 45 (%64.28) patients, microadenoma in 25 (%35.7) patients. We evaluated 11 cases have endocrine inactive adenomas, 25 cases have prolactinomas, 17 cases have growth hormone secreting adenomas, 10 cases have ACTH secreting adenomas, 6 cases have GH and prolactin secreting adenomas and 1 case has ACTH and prolactin secreting adenoma. Endocrinologic remission rates in patients operated by transsphenoidal surgery were %76.4 in acromegaly cases, %66.6 in acromegaly and prolactinoma cases, %90 in Cushing cases, %100 in Cushing and prolactinoma cases and %80 in prolactinoma cases.Conclusion: At the end, transsphenoidal hypophyseal microsurgery was effective and reliable technique because of low morbidity and mortality rates and protection the normal pituitary function after surgery.

Discrimination between antemortem and postmortem lesions inflicted by animals from other traumas, and contribution of these injuries to the cause of death which is often encountered in forensic autopsies may cause some difficulties. Detailed identification, and, evaluation of bite marks which has the depth and shape of the teeth and jaw of the animal interfering to the cadaver, tissue losses, paw and tooth marks nd vital signs must be performed during autopsy. Current autopsy reports filed in the Morgue Department of Council of Forensic Medicine between the years of 1996 and 2001 had been retrospectively screened and various cases inflicted by animals during antemortem, perimortem and postmortem periods had been included in the study in order to determine emerging problems in autopsy cases with signs of animal attack and develop recommendations for points to be considered in these cases. Location of animal attack, severity of injury, typical wound characteristics, differential diagnosis among other lesions, the cause of death and effects on identification of the victim and consequences in terms of forensic medicine were evaluated based on autopsy reports of 130 cases. Postmortem (n=129 cases) and antemortem (n=1) signs of animal attack were detected. Lesions were commonly caused by marine animals, and observed on the head region. The identity of the victims, and cause of death couldn't be determined in 22, and 48 % of the cases, respectively. When we consider the problems encountered during phases of determination of the cause and type of death and identification of the signs of postmortem animal scavening, the importance of forensic medicine approach in crime scene investigation on antemortem and/or postmortem animal attacks, identifying the profile of the offending animal and the challenging autopsy procedure becomes much more clear. Amaç: Sivas Cumhuriyet Üniversitesi Hastanesi’nde ampütasyon ve debridman operasyonları yapılan hastaların özelliklerinin tanımlanması ve bu özelliklerden yaş ve cinsiyetin operasyona neden olma riskinin tahminidir. Yöntem: Çalışma için Ocak 2007-Mart 2012 tarihleri arasında Cumhuriyet Üniversitesi Tıp Fakültesi Hastanesi ”Ortopedi ve Travmatoloji” ve ”Plastik ve Rekonstrüktif Cerrahi” Anabilim Dalları’nda diyabetik ayak sonucu yapılan tüm ampütasyon ve debridman operasyonları retrospektif olarak tarandı, toplam olgu sayısı 144 idi. Diyabette yaş ve cinsiyetin diyabetik ayağa bağlı operasyonlara yol açma riskini hesaplayabilmek için 2008’de yapılmış Sivas diyabet prevalansı toplum taramasının diyabetikleri kontrol grubu (n=237) olarak seçildi. Diyabetiklerde yaş ve cinsiyetin diyabetik ayak operasyonlarıyla ilişkisini araştırmak üzere çoklu lojistik regresyon analizi tekniğini kullandık. Methods: The medical records of all patients with diabetes who underwent surgical debridement or lower extremity amputation at the Plastic and Reconstructive Surgery or Orthopedics Clinics of Cumhuriyet University Hospital in Sivas (Turkey) during the period between January 2007 and March 2012 were reached by medical record screening. Of all cases (n=144), 122 underwent amputation, 22 surgical debridement. In order to estimate the effect size of age and gender on amputation or surgical debridement we used a control group (n=237) that included all diabetics identifed in a population diabetes screening study conducted in Sivas in 2008. Multiple logistic regression analysis was used to estimate the adjusted odds ratios, and the model used included age and gender as independent variables. Amaç: Bu çalışmanın amacı baş boyun kanserinde prognostik faktör olarak komorbidite, performans durumu, hemoglobin düzeyi, kilo kaybı, radyoterapiye ara verme ve mukozitin öneminin araştırılmasıdır. Yöntem: Üniversitesimizin Onkoloji Merkezine 2006 ve 2012 tarihleri arasında, baş boyun kanseri tanısı ile tedavi edilen 210 hastanın verileri incelendi. Hastaların performans durumu, hasta başvurusunda Eastern Cooperative Oncology Group (ECOG) skorlama sistemine göre değerlendirildi. Hastalık evresi 2010’da revize edilen TNM evrelemesine göre yapıldı. Bulgular: Hastalar, medyan 24 (dağılım: 1-134) ay takip edildi. Medyan 11 (dağılım: 3-96) ayda hastaların %24'ünde nüks gelişirken, medyan 15 (dağılım: 1-134) ayda %13'ünde uzak metastaz gelişti. Amaç: Bu çalışmada ilköğretim çağındaki çocuklarda baş biti görülmesini etkileyen faktörlerin belirlenmesi amaçlanmıştır. Yöntemler: Bu amaçla yatılı ilköğretim bölge okullarında (Merkez YİBO ve Pamukpınar YİBO) öğrenim gören, yaşları 6-15 arasında değişen 249’u kız ve 93’ü erkek olmak üzere toplam 342 çocuk baş biti açısından değerlendirilmiştir. 23 sorulu bir anket çocuklara dağıtılmıştır. Amaç: Bu çalışmadaki amacımız C-reaktif protein’nin (CRP) Akut eksezervasyonda kronik obstrüktif akciğer hastalığı (AEKOAH) ve stabil kronik obstrüktif akciğer hastalığında( SKOAH) artıp artmadığını tespit etmek, CRP deki artışın diğer inflamatuar markırlarla olan korelasyonunu belirlemektir. Gereç ve Yöntem: Anthonisen’in akut eksezervasyon kriterlerini taşıyan ve KOAH akut eksezervasyonu (AEKOAH) tanısı nedeniyle hastaneye yatan hastaların dosyaları retrospektif olarak tarandı. Tüm hastaların göğüs radyolografileri değerlendirildi. Göğüs radyografisinde yeni infiltrasyon, konsolidayon, hava bronkografi vb varsa pnömoni (PKOAH) olarak değerlendirildi. İki ay boyunca eksezervasyonu olmayan hastalar SKOAH olarak değerlendirildi. Aynı yaş ve cinsiyetten oluşan sağlıklı kontrol grubu oluşturuldu. Bulgular: Ortalama CRP seviyesi akut ataktaki KOAH hastalarda kontrol grubuna ve SKOAH göre anlamlı derecede yüksekti (p, Aim: In this study, we aimed to find out if there is an increase in the C-reactive protein (CRP) levels in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) and in stable COPD (SCOPD) and to determine the correlation of a possible increase with other markers of inflammation. Material and Method: All patients with AECOPD were also categorized according to radiologic findings. Chest X-rays were evaluated and COPD patients who had new infiltrates, consolidation, air bronkogram were considered to have pneumonia (PCOPD). SCOPD patients without a history of exacerbation for the previous 2 months were recruited prospectively from our outpatient clinic. The pulmonary function test results and the values of arterial blood gas analyses, leukocytes, neutrophils, and CRP measured within 24 hour of admission were obtained from the records of the patients with COPD,. Results: The mean serum CRP level during AECOPD was significantly higher compared to those in the control group and SCOPD patients. The mean CRP level was higher in the SCOPD patients compared to the control group as well. The serum CRP levels were positively correlated with the PaCO2 levels that showed the severity of the attack and negatively correlated with the pH, FEV1 and FEV1/FVC values. Positive correlation between leukocyte count and CRP was detected in all groups except the control group. Discussion: We indicated that there was an association between serum CRP levels and severity of COPD. In COPD exacerbations, leukocyte count and serum CRP levels had a significant positive correlation.

Magnetic resonance (MR) imaging showed multiple superficial and deep white-matter hyperintensity in T2-weighted and proton den- sity images. Echo-planar “trace” diffusion MR imaging revealed high signal intensity changes at the lesion sites on b=1000 s/mm2 images, initially suggesting restricted diffusion. On corresponding apparent diffusion coefficient (ADC) maps, the lesions had low signal intensity and low ADC values (0.32 to 0.43 x 10ˉ3 mm2/s), in comparison with the normal frontal white matter (0.80 to 1.03 x 10ˉ3 mm2/s). This was consistent with the presence of decreased diffusion, hence cytotoxic edema.

Ranunculus (buttercup or mayflower) is an important plant species causing chemical irritation. It is one of the traditional treatment methods usually used for rheumatic disorders in Central Anatolia particularly in spring and summer months. Here we report three cases of burn injury that developed secondary to mayflower use. Amaç: Çalışma, yenidoğan tarama testlerine ilişkin annelerin görüşlerini belirlemek ve tarama programlarına katılma durumlarını saptamak amacıyla planlandı. Yöntem: Araştırma, tanımlayıcı araştırma türündedir. Örneklemi Kasım 2015 - Mart 2016 tarihleri arasında bir üniversite hastanesinin postpartum kliniklerinde yatan 398 kadın oluşturdu. Kadınlara araştırmanın amacı açıklandı, sözel onam alındı, onam veren kadınlara veri toplama araçları uygulandı. Kadınlar ile taburculuk sonrası tekrar görüşüldü ve taburculuk sonrası yenidoğan tarama testlerini yaptırmak üzere yeniden sağlık kuruluşuna başvurup başvurmadıkları belirlendi. Veriler SPSS 22.0 paket programı kullanarak değerlendirildi. Bulgular: Annelerin %52'sinin 26-35 yaşlar arasında olduğu, %48,7'sinin ilkokul ve altında eğitim düzeyine sahip olduğu, %11,6'sının birinci dereceden akraba evliliği yaptığı belirlendi. Annelerin % 54,8'inin bazı hastalıkların anne ya da baba aracılığı ile bebeğe geçebileceğini, %56'sının kalıtsal geçişli hastalıkların ortaya çıkmasında akraba evliliğinin etkili olduğunu, %51,3'ünün bu hastalıkların bebekte geri dönüşsüz beyin hasarına %42'sinin fiziksel büyüme ve gelişme geriliğine yol açtığını, %48'inin bebeğin topuğundan alınacak iki damla kan ile tanı konulabildiğini bildiği saptandı. Annelerin %46,7'si topuk kanı ile beslenme arasındaki ilişkiye, %48,2'si topuk kanı alınması için en uygun zamana, %58,5'i erken tanı ve tedavinin hastalığın ilerlemesini engelleyip engellemediğine ilişkin kararsız olduğunu belirtti. Postpartum taburculuk sonrası annelerin 219'una ulaşılabildi. Annelerin %48,4'ü postpartum 1 haftadan sonra sağlık kuruluşuna başvurduklarını, %65,8'i yenidoğan tarama testinin ebeler tarafından gerçekleştirildiğini, %77,6'sı işleme ilişkin bilgilendirildiğini belirtti. Bakteriyel ve viral enfeksiyonlarda yeni belirteçlerin klinik uygulamada kullanımını onların maliyetleri sınırlandırmaktadır. Bu çalışmada kırım kongo kanamalı ateşi (KKKA) hastalığında nötrofil lenfosit oranının (NLO) değerlendirilmesi amaçlandı. In this study we assessed the potential use of the neutrophil to lymphocyte count ratio (NLCR) in crimean congo hemorraghic fever (CCHF). NLCR values were measured in total 104 patients with CCHF with 77 patients in whom bleeding was observed and 27 patients without bleeding observed and in 61 healthy adults. Amaç: Bu çalışma son sınıf hemşirelik ve ebelik öğrencilerinin gebelikte kadına yönelik şiddetle ilgili görüşlerini belirlemek amacıyla tanımlayıcı olarak yapılmıştır. Yöntem: Çalışmanın örneklemini 124 son sınıf öğrenci (88 hemşirelik, 36 ebelik öğrencisi) oluşturmuş ve veriler araştırmacılar tarafından literatür doğrultusunda hazırlanan bir soru formu ile toplanmıştır. Verilerin istatistiksel analizi, yüzde dağılımları ve ki-kare testi kullanılarak yapılmış ve yorumlanmıştır. Bulgular: Öğrencilerin %50.8'i aile içi şiddet (AİŞ) uygulayan kişiye yönelik yasal yaptırımlar, %49.2'si AİŞ mağdurunun yasal hakları, %31.5'i AİŞ'ye müdahale ve %29.0'ı gebelikte şiddet sıklığı ve önemi konularında eğitimleri süresince aldıkları bilgiyi yetersiz bulmuşlardır. Diğer taraftan öğrencilerin %38.7'si gebelikte şiddet sıklığı ve önemi, %53.2'si gebelikte şiddetin anne ve fetal sağlığa etkileri, %51.6'sı gebelikte şiddetin doğum sonu anne ve bebek sağlığına etkileri konularında eğitimleri süresince aldıkları bilgiyi yeterli bulmuşlardır. Ancak öğrencilerin eğitimleri sırasında genelde AİŞ ve özelde gebelikte şiddet konusunda yeterli bilgi almadıkları söylenebilir., Objective: To define the view of final year nursing and midwifery students towards domestic violence against women during pregnancy.Method: 124 students (88 nursing, 36 midwifery) completed a questionnaire developed by researchers. Data were presented as frequency distribution, and chi-square test was used for statistical analysis.Results: Students found the training they receive in the following topics inadequate: people who apply legal sanctions against domestic violence (50.8% of students surveyed), the legal rights of the victims of domestic violence (49.2%), domestic violence intervention (31.5%), and frequency and importance of the issue of violence in pregnancy (29.0%). On the other hand 38.7% of the students found their training to be adequate with regard to violence frequency and importance in pregnancy, violence during pregnancy and maternal and fetal health effects (53.2%), and postpartum violence during pregnancy in mothers and their effects on infant health (51.6%). 98.4% of the students agreed that "forcing women to bear a child unwillingly is a form of violence", 83.1% agreed that "the sex of the unborn baby can expose pregnant women to violence", 90.3% agreed that "determining the risk of violence to pregnant women during prenatal care is an important duty of health personnel", 87.9% agreed that "more time should be given to the issue of violence during pregnancy in the training of health personnel ", and 48.4% agreed that "the severity of the problem of violence during pregnancy is ignored".Conclusion: The majority of students agreed that during their training, importance should be given to potential situations that could lead to a pregnant woman's exposure to violence, and how health personnel can easily diagnose these situations. However, during the training of the students said that they receive inadequate information about usually domestic violence, and in particular violence during pregnancy.

Klinik örneklerden Korinebakteriler izole edildiğinde klinik önemine karar vermek güç ola-- bilmektedir. EUCAST (The European Committee on Antimicrobial Susceptibility Testing) 2014 yılında Ko-- rinebakteri izolatları için disk difüzyon yöntemi hakkında klinik sınır değerleri bildirmiştir. Çalışmamızda çeşitli klinik örneklerden izole edilen Korinebakteriler için bu sınır değerler kullanılarak antimikrobiyal du-- yarlılık sonuçları belirlenmeye çalışılmıştır. Gereç ve Yöntemler. Çalışmamızda Cumhuriyet Üniversitesi Tıp Fakültesi Hastanesi Klinik Mikrobiyoloji Laboratuvarına gönderilen klinik örneklerden izole edilen 107 Korinebakteri'nin duyarlılık incelenmiştir. İzole edilen bakteriler Microflex LT MALDI--TOF MS (Bru-- ker Daltonics, Bremen, Germany) cihazı ile tanımlanmış, Mueller Hinton Fastidious Agar'da (% at kanı ve 20 mg/l fs--NAD) EUCAST önerilerine göre disk difüzyon yöntemi ile çalışılmıştır. Bulgular: Çalışmamız sonucunda çeşitli klinik örneklerden izole edilen 107 izolatın %49,5'i Corynebacterium striatum olarak ta-- nımlanmıştır. C. amyloticum, C. propinquum, C. afermentans, C. jeı'keum, C. minitissı'muın, C. urealytı'cum izolatları sırası ile %17,8, %10,3, %8,4, %65, %4,7 ve %2,8 oranlarında tanımlanmışlardır. Korinebakteri türlerinin tamamı vankomisin ve linezolide karşı duyarlı bulunmuştur. Korinebakteri türlerinde diğer an-- tibiyotiklere karşı %90 ile %100 arasında direnç oranları belirlenmiştir. Korinebakteri türlerine göre direnç oranlarında anlamlı bir farklılık görülmemiştir. Sonuç: Son yıllarda difteri dışı Korinebakteriler, immun-- supresif, uzun süreli invaziv kateterlere maruz kalan hastalarda kan dolaşım enfeksiyonları, endokardit ve pnömoni gibi enfeksiyonlarda etken olarak karşımıza çıkabilmektedir. Monosit Sayısı Dekompanse Kalp Yetersizliğinde Belirteç midir? Giriş: Çalışmanın amacı akut dekompanse kalp yetersizliği (ADKY) olan hastalarda kardiyovasküler mortalite ile monosit sayısı arasındaki ilişkinin araştırılmasıdır. Gereç ve Yöntem: Çalışma kardiyolojiye ardışık başvurmuş 237 hasta içeren retrospektif kohort çalışmasıdır. Klinik takipler telefon görüşmesi veya hastanenin medikal kayıtlarından yapıldı. Bulgular: Hastalar iki gruba ayrıldı: 1. Grup takipte veya hastanede yatışı sırasında ölenlerden, 2. grup yaşayan hastalardan oluşturuldu. İki grup arasında monosit yüzdesi ve sayısı arasında anlamlı farklılık vardı (p=0.003; p=0.006, sırasıyla). ROC eğrisi ile değerlendirildi (AUC=0.618, %95 GA:0.544-0.693, p=0.003). Sonuç: Monosit ile ilişkili sitokinler ADKY de önemli rol oynarlar. Material and Method: The study is a retrospective cohort study including 237 consecutive patients with ADHF admitted to cardiology. Clinical follow-up was performed by telephone interviews with the patient and/or relatives and by review of hospital medical records. Results: These patients were divided into two groups: the deceased group (group 1), who died during hospitalization or the follow-up period, and the survival group (group 2). There was a significant difference in the mean percentage of monocytes and monocyte count between the two groups (p=0.003; p=0.006, respectively). ROC curve was drawn (AUC=0.618, 95% confidence interval: 0.544-0.693, p=0.003). Conclusion: In summary monocyte-related cytokines play an important role in ADHF. Our study shows that monocytes can be used in patients with ADHF as a independent predictor of mortality; moreover it is inexpensive and easy to perform. Nonetheless, further larger-scale and multicenter studies needed to confirm these findings.

Atrial fibrillation is the most common rhythm disturbance in general population. We present a 19-year-old man who was admitted to emergency department with progressive weakness, intermittent nausea, vomiting, and palpitations for 1-2 hours. He stated that he had palpitations for the first time. His past history was notable for venlafaxine use for panic disorder. He was diagnosed with AF with rapid ventricular rate and admitted to coronary care unit. Medical cardioversion with amiodarone was attempted and normal sinus rhythm was restored. He had no additional problems during hospital stay and was discharged on anticoagulant therapy (warfarin sodium). Atrial fibrillation should be born in mind in cases presenting to emergency department with palpitations, chest pain, syncope, or weakness in order to provide early diagnosis and treatment for this important disorder causing many complications. Midgut volvulus, ince barsak mezenter kökünün süperior mezenterik arter etrafında rotasyonu sonucu oluşan nadir görülen bir malrotasyondur. İnfantlarda sık, erişkinlerde ise nadir olarak görülmektedir. Ondokuz yaşında erkek hasta altı saat önce başlayan şiddetli santral karın ağrısı şikâyeti ile acil servise başvurdu. Özgeçmişinden yaklaşık iki ay önce bıçakla yaralanma sonrası eksploratif laparotomi yapıldığı öğrenildi. Fizik muayanede batında yaygın hassasiyet ve defans mevcut iken; laboratuar testlerinde ise beyaz küre sayısının 12 bin olması dışında anormal bir değere rastlanmadı. Çekilen akciğer grafisinde solda diafragma altında şüpheli serbest hava görüntüsü izlendi. Batın ultrasonografisinde herhangi bir patoloji izlenmeyen hastaya, intravenöz kontrast madde verilerek tüm batın bilgisayarlı tomografi çekildi ve pankreas inferior kesimden başlayıp iliak bifurkasyo düzeyine kadar devam eden saat yönünde yaklaşık 300 derecelik rotasyon yapan mezenterik volvulus ile uyumlu görüntü saptandı. Genel cerrahi ile konsülte edilen hasta opere edilerek rotasyone mezenter dokusu detorsiyone edildi. Operasyon sonrası 3. günde hasta şifa ile taburcu edildi. Sonuç olarak, acil servise ani başlangıçlı karın ağrısı ve intestinal obstrüksiyon bulguları ile başvuran; özgeçmişinde geçirilmiş abdominal cerrahi öyküsü olan hastalarda midgut volvulustan şüphelenilmelidir. Bu hastalarda erken tanı ve hızlı cerrahi müdahale, prognozu iyileştirici anahtar faktörlerdir, Midgut volvulus is a rare malrotation caused by a rotation of the small intestine mesentery root around the superios mesenteric artery. It is seen mostly in infants and rarely in adults. In this case report we experienced aa 19-year old man who had central abdominal pain for 6 hours. He had a history of explorative laparatomy because of stabbing injury about two months ago. Physical examination showed common defence and tenderness. The laboratory tests were in normal range except increased white blood cells which was 12,000. Chest radiography showed suspected abnormal presence of air in the left diaphragm. Since the abdominal ultrasonography was normal, the patient went through the abdominal computed tomography which showed mesenteric volvulus of 300 degree abnormal rotation between inferior pancreas and iliac bifurcation. The patient was taken to the operation and malrotated mesentery was detortioned after the evaluation of general surgery department, The patient was externed three days after the operation. As a result; midgut volvulus should be considered in patients with acute abdominal pain and intestinal obstruction who had a history of abdominal surgery. Early diagnosis and surgical intervention are key factors for good prognosis in those patients

Üst ekstremite periferik arter yalancı anevrizmaları, alt ekstremitede görülenlere göre daha nadir bulgulanır. Ancak üst ekstremite veya parmak kayıpları gibi önemli fonksiyonel eksikliklere yol açabildikleri için tanılandırımları ve cerrahi yaklaşımla tedavi edilmeleri önem taşımaktadır. Bu çalışmada koroner kateterizasyon girişimi sonrası 1. yılda olgunlaşan dev ekspansif brakiyal arter yalancı anevrizmasına gerçekleştirdiğimiz tanılandırım ve başarılı cerrahi tedavi protokollerimizi literatür ışığında sunmayı amaçladık, Iatrogenic arterial pseudoaneurysms of the extremities are more commonly seen due to increasing number of centers doing invasive interventions for diagnostic and therapeutic purposes. The pseudoaneurysms of upper extremity arteries are more rarely seen than those of lower extremity. Nevertheless, they may cause loss of digits or even the whole upper extremity and therefore it is important to diagnose and treat them surgically. In this study, we aimed to present our diagnostic and therapeutic protocols of a giant expanding brachial arterial pseudoaneurysm developing one year after the coronary catheterization under the light of current literature data.

Giriş ve Amaç: Dünyada tüm kültürler, emzirme konusunda kendi mitlerine (doğru bilinen yanlışlar) sahiptirler. Emzirme mitleri, anne ve bebeğin başarılı emzirme deneyimine engel oldukları için zararlı uygulamalar arasında görülebilir. Anne sütü ve emzirme süreci hakkında ebe öğrencilerin mitlerinin bilinmesi, annelerin anne sütü ve emzirme üzerine olan olumlu veya olumsuz etkilerinin değerlendirilmesinin, olumsuz etkilere neden olanların ortadan kaldırılmasının bebek beslenmesine ve sağlığına katkı sağlayacaktır. Araştırma, mezuniyet sonrası, gebelik ve doğum sonu dönemde kadınlara ve topluma emzirme danışmanlığı yapacak olan ebelik öğrencilerinin, anne sütü ve emzirmeye ilişkin mitler konusunda düşüncelerini saptamak amacıyla planlandı.Gereç ve Yöntem: Araştırma, Cumhuriyet Üniversitesi Sağlık Bilimleri Fakültesi Ebelik Bölümü'nde tanımlayıcı bir çalışma olarak yapılmıştır. Araştırmanın evrenini, 2013-2014 eğitim ve öğretim yılında ebelik bölümünde okuyan 256 öğrenci oluşturmaktadır. Araştırmada örneklem seçimi yapılmamış olup evrenin tümüne ulaşılmaya çalışılmıştır. Çalışmaya toplam 235 öğrenci katılmıştır. Anket formları araştırmacılar tarafından yüz yüze görüşme yöntemi kullanılarak 20 Şubat-7 Mart 2004 tarihleri arasında uygulanmıştır. Araştırma verileri SPSS for Windows 14.0 paket programında yüzdelik dağılım kullanılarak değerlendirilmiştir. Bulgular: Öğrencilerin yaş ortalaması 21±1.5'tur. Öğrencilerin %97.4'ü bekar ve %62.1'i il merkezinde yaşamaktadır. Öğrencilerin %93.2'si yetersiz süt üretimi yorgunluk, yetersiz sıvı ve gıda alımı nedeniyledir; %81.3'ü anne bebeğini emzirmeden önce her seferinde meme uçlarını yıkamalıdır gibi mitlere inanmaktadırlar.Sonuç: Ebelik bölümlerinde okuyan öğrencilerin eğitim süresi içerisinde emzirme sürecine ilişkin kendi mitlerinin farkına varmaları, profesyonel meslek yaşantıları içerisinde bu mitlerden arınarak sağlık bakım hizmetleri vermeleri halinde anne, bebek, aile ve toplum sağlığının korunması, sürdürülmesi ve geliştirilmesine olumlu katkılar sağlanacaktır, Introduction and Objective: All cultures in the world have their own myths about breastfeeding (false beliefs known as true). Breastfeeding myths can be seen as harmful practices because they interfere with successful breastfeeding experience of mother and infant. Knowing midwifery students'myths about breast milk and breastfeeding process, evaluation of positive or negative effects of mothers'on breast milk and breastfeeding, elimination of the ones that cause adverse effects will contribute to infant nutrition and health. The study was planned to determine midwifery students'thoughts about breast milk and breastfeeding myths as they will make breastfeeding counseling to women who are in pregnancy and postpartum periods and to community after graduation.Materials and Methods: Research was conducted as a descriptive study in Cumhuriyet University Faculty of Health Science Midwifery Department. The population of the study constitutes 256 students studying in midwifery department in 2013-2014 academic year. Sample selection was not done in research to be able to reach out to entire universe. 235 students participated in the research. Questionnaires were made using face to face interview method by researchers between February 20 and March 7 in 2004. The research data was evaluated in SPSS for Windows 14.0 software package using the percentage distribution method.Findings: The average age of students is 21±1.5. 97.4% of students are single and 62.1% of students live in city center. 93.2% of students believe in such myths that insufficient milk production is because of fatigue, inadequate fluid and food intake. 81.3% of students believe in such myths that mothers should wash their nipples each time before breastfeeding.Results: If midwifery students realize their own myths regarding breast-feeding process in their education period and if they provide health care services free from these myths in their professional work lives, they will contribute positively to protection, maintenance and development of mother, infant, family and community health

Tümör rekürrensini önleyici en değerli yaklaşımın olgumuzda da uyguladığımız üzere maksimal ekstirpasyon olduğu bilinmektedir, Thymoma, which is combined neoplasm of thymic epithelial cells and lymphocytes, are benign lesions of anterior mediastinum. Evidence of thymoma is adequate indication for surgical extirpation. In this study we aimed to present our thymoma case representing as a giant solid mediastinal mass. Giriş: Bu çalışmada, femoral arterde akut arteriyel tı-kanıklık sebebiyle acil femoral embolektomi yapılan 70 yaş ve üstü hastalarımızın sonuçlarını literatür ışı-ğında irdelemeyi amaçladık.Yöntem: Ocak 2007-Aralık 2015 tarihleri arasında hastanemiz acil servisine ve polikliniğine başvuran 70 yaş ve üstü 45 hasta bacaklarında kısmi fonksiyon kaybı, soğukluk, ağrı ve renk değişikliği gibi yakınma-larıyla femoral arterde akut periferik arteriyel tıka-nıklık tanısı almış ve Kalp Damar Cerrahisi Kliniği ta-rafından aynı ekipçe acil olarak cerrahi embolektomi uygulanmıştır. Olguların yaş ortalaması 78,53±5,85 yıl; hastaların 31' i erkek (%68,9) iken, 14'ü kadın (%31,1) idi. Hastalar; yaş, cinsiyet, etiyolojik faktör-leri-eşlik eden hastalıkları, semptomların başlan-gıcından operasyona kadar geçen süre, hastaların kabulü esnasındaki iskemi derecelendirmesi, kabul sırasındaki biyokimyasal ve metabolik parametreler, embolektomi sonrası biyokimyasal parametrelerdeki değişmeler, yapılan ampütasyonlar ve mortalite açı-sından retrospektif olarak incelenmiştir. Bulgular: Etiyolojik faktörler incelendiğinde 11 has-tada (%24,4) atriyal fibrilasyon saptandı. Olgular atriyal fibrilasyon açısından değerlendirildiğinde ka-dın cinsiyette daha sık bulgulandı(p:0.020). Otuz iki hastada(%71,1) koroner arter hastalığı mevcut iken hiçbir hastada yeni geçirilmiş miyokard enfarktüsü hikayesi yoktu. Daha öncesinde periferik arter hasta-lığı tanısı almış ve medikal tedavisi düzenlenip takip edilen hasta sayısı 24 (%54,3) olgu olup erkeklerde kadınlara oranla bu sayının belirgin derecede yüksek bulgulandığı, bunun da istatistiksel olarak anlamlı ol-duğu belirlendi (p:0,025). Kronik obstrüktif akciğer hastalığının varlığı (p: 0,037), sigara kullanım hikaye-sinin olması (p:0,001) ve periferik damar hastalığının varlığı(p:0,025) erkelerde kadınlara oranla istatistik-sel olarak anlamlı yüksekti. Kadınlarda ise diyabet (p:0,001) ve atriyal fibrilasyon (p:0,020) erkeklere oranla daha fazla bulgulandı. Akut periferal arteriyel oklüzyonun lokalizasyonuna bakıldığında olguların %37,7'ında (17 hasta) ana femoral arter, %46,7'sin-de (21 hasta) yüzeyel femoral arter, %15,6'ünde (7hasta) hem yüzeyel hemde derin femoral arter etki-lenmiş durumda idi. Hastalarımızın başvuru anındaki alt ekstremitedeki iskemi derecelendirilmesinin da-ğılımına bakılırsa; 8 hasta (%17,8)Grade I, 19 has-ta(%42,2) Grade II ve 18 hasta(%40) Grade III olarak saptandı. Başvuru sırasında metabolik asidoz bulu-nan hastaların hasta sayısı 11(%24,4) olup, şikayetle-rin başlaması ile hastaneye başvurma süresi 48 saat ve üzerinde olan olguların tamamında (%100) meta-bolik asidoz bulgulandı. Hastaneye başvurma süresi geciktikçe mortalite oranı artmaktadır(p:0,031). İlk 12 saat içerisinde başvuranlarda mortalite yok iken, 12-48 saat arasında başvuranlarda 2 olgu (%16,7), 48 saat ve üzernde başvuranlarda 7 olgu (%36,8) kaybe-dilmiştir. Hastaların hastaneye kabul edildiği anda ve sonrasındaki takiplerinde alınan biyokimya-sal analizlerinde böbrek fonksiyonları(üre ve kreati-nin), karaciğer enzimleri (AST ve ALT) ve potasyum (K+) değerlerinin hastaların iskemi derecelendirmesi, başvuruesnasında metabolik asidozun durumu ve mortaliteye göre,anlamlı değişimleri değerlendirildi. Sonuç: Özellikle ileri yaş grubundaki hastalarda bir çok yandaş hastalığın birlikteliği ve periferik damar hastalığı bulunma oranı genç hasta grubuna göre daha yüksektir. Bu hasta grubunun hastaneye kabulü ile beraber mevcut metabolik durum hızla değerlen-dirilip, tedavinin hızlı bir şekilde başlanması önem arz etmektedir. Bunun yanısıra zeminde periferik damar hastalığı bulunma ihtimalinin yüksek olduğu unutul-mamalı, buna yönelik periferik anjio tekniklerinden herhangi biri kullanılarak vasküler yapı değerlendiril-mesinin gerektiğini düşünüyoruz., Introduction: In this study, we aimed to investigate our results on elder patients who are 70 years old or over and treated with emergent arterial femoral em-bolectomy for acute arterial obstruction with pers-pective of up-to-date literature.Methods: Between January 2007 and December 2015, 45 patientswho are 70 years old or over ad-mitted to our emergency department or out-patient clinic with partial functional loss, coldness, pain and discoloration. After confirmation of acute femoral artery obstruction, all of these patients were tre-ated with emergent femoral embolectomy by the same cardiovascular surgery team of our Cardiovas-cular surgery department. Open surgical access was the method of choice. Mean age of patients were 78.53±5.85. Between these patients 31 of them were male (68.9%) and 14 of them were female (31.1%). All of the patients were evaluated retrospectively with age, gender, etiologic factors-comorbidities, time between onset of symptoms to operation, grade of ischemia at admittance, biochemical and metabolic parameters at admittance, alterations at biochemical and metabolic parameters after embolectomy, am-putations, and mortality.Results: Atrial fibrillation was evident in 11 patients (24.4%). Atrial fibrillation was more common in fe-male gender (p:0.020). Thirty-two patients (71.1%) had coronary artery disease but none of the patients had medical history of recent myocardial infarction. Twenty-four patients (54.3%) previously had diag-nosis and treatment of peripheral arterial disease. Male gender was significantly higher in this group (p:0.025). Chronic obstructive pulmonary disease (p:0.037), smoking (p:0.001)and peripheral arterial disease(p:0.025) were significantly higher in male gender. Diabetes (p:0.001) and atrial fibrillation (p:0.020) were significantly higher in female gender. Pathology was located at common femoral artery in 37,7% (n:17), at superficial femoral artery in 46.7% (n:21), and both superficial and deep femoral arte-ries were effected in 15,6% (n:7) of the patients. Isc-hemia levels at the time of admittance were distri-buted as follows: 8 patients (17.8%)were Grade I, 19 patients (42.2%) were Grade II, and 18 patients (40%) were Grade III. Eleven patients (24.4%) had metabo-lic acidosis at time of admittance. All of the patients (100%) with time interval of 48 hours or more betwe-en onset of symptoms and admitting to hospital had metabolic acidosis. Mortality rates significantly inc-reased with delay of admitting to hospital (p:0.031). There was not any mortality in patients who admit-ted to hospital in first 12 hours. Mortality was 16.7% (n:2) in patients with interval of 12 to 48 hours and mortality was 36.8% (n:7) in patients admitted after 48 hours. Alterations of biochemical laboratory va-lues were evaluated at time of admittance and post-operatively. These tests included renal functions (urea and creatinine), liver enzymes (AST and ALT), potassium (K+). These alterations were also evalu-ated in terms of ischemia grade, metabolic acidotic state and mortality. Conclusion: Multiple comorbidities and peripheral vascular diseases are more likely to be found in elder patients. Metabolic state must be assessed quicklyat admittance to hospital.It is vital to startoptimal treat-ment as fast as possible. All patients must be presu-med to have peripheral vascular disease. We believe that, examination of vasculature with one of the app-ropriate peripheral angiography techniques is especi-ally important.

Material and Method: 61 patients who had been diagnosed with secondary amyloidosis by renal and rectal biopsy between 2007 and 2013 in the nephrology clinic of Cumhuriyet University. Faculty of Medicine, were included in the study. Demographic characteristics, causes of secondary amyloidosis, MEFV gene mutations, end-stage renal failure (ESRF), renal transplantation, and mortality rates were examined retrospectively. Results: In etiological terms, Familial Mediterranean Fever (FMF) occurrence was 62.2% (38), bronchiectasis and emphysema 9.8% (6), tuberculosis 4.9% (3), coexistence of FMF and ankylosing spondylitis 3.2% (2), coexistence of FMF and rheumatoid arthritis 1.60,6 (1), coexistence of FMF and systemic lupus erythematosus (SLE) 1.6% (1), osteomyelitis 1.6% (1). septic arthritis 1.6% (1), Crohn's disease 1.6% (1), colon cancer 1.6% (1), coexistence of bronchiect-axis and tuberculosis 1.6% (1), rheumatoid arthritis 1.6% (1), and idiopathic cases 6.5% (4). Proteinuria was determined at nephrotic level among 68% (32) of 47 patients who had secondary amyloidosis. MEFV gene mutation of 45 patients with secondary amyloidosis was assessed. Most patients had M694V gene mutation. Surprisingly, we detected heterozygous 61480 mutation in 3 cases. 12 cases died; of these, 9 had ESRF. Five cases with ESRF underwent renal transplantation. Discussion: We found FMF as the most common cause for secondary AA amyloidosis in this study. Further studies should be done with larger or multicenter cohorts.

Amaç: Bu çalışmamızda, Ocak 2013-Aralık 2015 tarihleri arasında Hastanemiz acil servisine ekstremite damar yaralanması ile başvuran hastaların tanı ve tedavi sürecini retrospektif olarak inceleyip, tanı yöntemleri, tedavi seçenekleri, mortalite ve morbidite üzerine etki eden faktörleri irdeleyip, literatür eşliğinde sunmayı amaçladık.Yöntem: Hastanemiz acil servisinde Ocak 2013-Aralık 2015 tarihleri arasında periferik damar yaralanması tanısı alıp, Kalp ve Damar Cerrahisi Kliniğince aynı ekip tarafından tedavi edilen 34 olguyu retrospektif olarak değerlendirdik. Olgularımızın 29'u (%85,3) erkek, 5'i (%14,7) kadın olup, yaş ortalaması 32,00±14,67 yıl (10-68 yıl) idi. Travmanın durumuna göre kalp ve damar cerrahisinin yanısıra ortopedi ve plastik cerrahi uzmanlarından oluşan mutlidispiliner bir yaklaşımla hastalar değerlendirildi. Fizik muayene ve sonografik el doppler aleti ile tüm hastalar değerlendirildi. Yara yeri ve genel durumu uygun olan olguların tamamına Doppler Ultrasonografi çekilerek, hem arteriyel hem de venöz sistem detaylı olarak incelendi. Künt travma hikayesi olanların tamamına böbrek fonksiyon test sonuçları uygun olduğu için Bilgisayarlı Tomografik Anjiografi çekildi. Ortopedik müdahaleler dahil diğer cerrahi branşların operasyonları da, hayatı tehdit eden bir durum yoksa, revaskülarizasyon veya hasarlı damarın tamirinden sonra gerçekleştirildi. Bundaki amaç iskemi süresini kısaltmaktır. Ameliyat esnasında kardiyovasküler ekip tarafından damar yaralanmasının tipi belirlenerek operasyonun planı tayin edildi. Ameliyatı takiben hastalar yoğun bakım koşullarında vasküler açıdan yakın takip edildi. Ameliyat sonrası Doppler Ultrasonografi ile vasküler yapılar kontrol edildi. Gerek görülmesi halinde kontrol BT Anjio çekildi. Bulgular: Yirmibir olguda(%61,8) kesici-delici aletle, 6'sında(%17,6) kemiğin kırık segmentinin deplasmanına sekonder gelişen kesi ile, 4'ünde (%11,8) künt travma ile, 3'ünde (%8,8) ateşli silahla yaralanma meydana gelmiştir. Altı hasta (%17,6) hemorajik şok tablosu ile acil servise başvurmuştu. Üst ekstremitede %20,6 (7 olgu) oranı ile en fazla brakiyal arter yaralanması saptanmış olup, alt ekstremitede %29,4 (10 olgu) oranında femoral arterde yaralanma daha fazladır. Hasta popülasyonumuza bakıldığında %73,5oranında transseksiyon (25 olgu), %8,8 oranında (3 olgu) intimal ayrılma ve %17,6 oranında (6 olgu) lateral ayrılma saptandı. Hastaların 13'ünde (%38,2) safen ven greft olarak kullanılarak interpozisyon tekniği ile tamir yapıldı. Üç hastaya medikal tedavi uygulandı. Venöz yaralanma 14 (%41,1) olguda saptandı. Bu olgulardan 5 tanesine(%14,7) karşı bacaktan safen ven alınarak tamir veya bypass yapıldı. Safen ven yama olarak 3 hastada kullanılırken, 2 hastada ise interpozisyon (popliteal vende) amaçlı greft olarak kullanıldı. Geri kalan 9 hasta (%26,5) ise venöz segment tamir veya bypassa imkan vermeyecek şekilde tahrip olduğu ve kalibrasyonları çok ince belirlendiği için bağlandı. Operasyon sonrası takiplere bakıldığında 3 hastada (%8,8) kompartman sendromu gelişti. Bunlardan 2 olguda popliteal arter ve 1 olguda brakiyal arter yaralanması mevcuttu. Bu hastaların tamamına fasiyatomi açıldı. Brakiyal arter yaralanması olan hastanın kolu kurtarılamayarak ön kol ampütasyonu, popliteal arter yaralanması olan hastalardan bir tanesinde de diz altı ampütasyon gerçekleştirildi. Kalan 32 hastada ampütasyona sebep olacak bir komplikasyonla karşılaşılmadı. Tartışma: Periferik damar yaralanmalarında erken tanı konulup, gerekli tedaviye erken dönemde başlanması; ekstremite kaybını ve mortalite oranını azaltmaktadır. Bu hastalarda, yaralanmanın durumuna göre kalp ve damar cerrahisinin yanısıra ortopedi ve plastik cerrahi uzmanlarından oluşan mutlidispiliner bir yaklaşımın daha etkili olacağı inancındayız, Objective: We retrospectively investigateddiagnosis and treatment procedures on patients admitted to Emergency Department at our hospital with vascular injury of extremity between dates January 2013 and December 2015. Diagnostic techniques, treatment options and factors effecting mortality and morbidity were examined and assessed with up-to-date litera-ture.Method: Between dates January 2013 and Decem-ber 2015, thirty-four patients who were treated surgically for peripheral vascular injury by the same surgical team were investigated retrospectively. Twenty-nine patients were male (85.3%) and 5 pa-tients were female (14.7%). Mean age of patients were 32.00±14.67 (between 10 to 68 years). Accor-ding to trauma type, Orthopedic and/or plastic sur-gery departments also examined the patients in a multidisciplinary approach. All patients were asses-sed by physical examination and continuous wave Doppler. Any patient suitable for further examination were assessed by arterial and venous Duplex ultraso-nography. All patients with blunt trauma had Compu-terized Tomography Angiography because all of the patients' renal tests were normal. All operations, inc-luding orthopedic and other surgical interventions, were performed after surgical revascularization or repair of the vessel unless there was a life threate-ning condition. This approach was reasonable for re-ducing the ischemia period. Vascular injury and ope-rative strategy was intraoperatively considered by cardiovascular team. In the postoperative period, pa-tients were closely followed up for eventual vascular problems in intensive care unit. Duplex ultrasonog-raphy was performed to check vascular structures in the postoperative period. If required, CT angiography was performed.Results: Twenty-one cases (61.8%) were injured by penetrating trauma. Injury was caused secondary displacement of bone fracture in six (17.6%) pati-ents. Injuries due to blunt trauma (n:4, 11.8%) and firearms (n:3, 8.8) were evident. Six patients (17.6%) admitted to emergency department with hemorrha-gic shock.Brachial artery was the most common site (n:7,20.6%)of injury at upper extremity. Femoral ar-tery was most commonly injured (n:10, 29.4%) in the lower extremity. Transection was evident in 73.5% of the patients (25 cases). Intimal separation was obser-ved in 8.8% (3 cases) of the patients and lateral tear was evident in 17.6% (6 cases) of the patients. Thir-teen patients (38.2%) were treated with saphenous vein graft interposition. Three patient had medical treatment. Venous injury was evident in fourteen patients (41.1%). Saphenous vein of the opposite leg was used for repair in five (14.7%) of these patients. Saphenous vein was preferred as patch for three and was preferred for graft interpositioning (popliteal vein) for two. Venous segment was heavily damaged and left no chance to repair or bypass and/or vessel calibers were too thin in the remaining nine (26.5%) patients. Vessels were tied-up. When we check pos-toperative data; compartment syndrome developed in three (8.8%) patients. Two of these cases had popli-teal artery and the remaining one had brachial artery injury. Fasciotomy was performed to these patients. Arm of the patient with brachial artery injury could not been saved and forearm amputation was perfor-med. One of the popliteal artery injury patients had below-knee amputation. None of the remaining 32 patients had a complication causing amputation. Conclusion: In peripheral vascular injuries, early di-agnosis and urgent initiation of treatment reduces extremity loss and mortality significantly. We beli-eve that, related to type of injury, multidisciplinary approach including orthopedic and plastic surgery departments additional to cardiovascular surgery de-partment, will be more efficient.

Talidomid antiinflamatuar, antiangiogenetik ve immünomodulator etkilerinden dolayı günümüzde çeşitli hastalıklarda artan oranda kullanılmaktadır . Talidomidin sinüzal bradikardi yapabildiği ile ilgili çalışmalar mevcuttur. Sadece bir vakada ise talidomidle ilişkili atrioventriküler(AV) tam blok bildirilmiştir. Ancak şu ana kadar talidomid ile ilgili 2. Derece AV blok olgusu bildirilmemiştir. Burada, 50 yaşında, multipl myleom nedeniyle talidomid kullanmakta olan, acil servise senkop ile başvuran ve 2:1 AV blok saptanan bir hastayı sunmaktayız. Hastanın son 5 aydır 100 mg /gün talidomid kullandığı ve ilaç başlanmadan önceki elektrokardiyografilerinde anormal bir bulgu olmadığı görüldü. Başvuru anında hastanın kalp hızı 45-75/dk arasında değişmekteydi. Hastanın hipotansiyona eşlik eden senkobu olması nedeniyle geçici kalp pili implante edildi. Talidomid tedavisi sonlandırıldıktan 5 gün sonra hastanın kalp hızının 80/dk olduğu izlendi ve ertesi gün geçici pacemaker çıkarıldı. Geçici pacemaker çıkarıldıktan sonra yapılan ritm holterde anormal bulgu izlenmedi. Talidomid kullanan hastalarda yakın kardiyak ve elektrokardiyografik takip yapılmalıdır., Due to its anti-inflammatory, anti-angiogenic, and immunomodulatory effects, thalidomide is increasingly used in the treatment of various diseases. Previous studies have demonstrated that thalidomide may cause sinusal bradycardia. Thalidomide-associated complete atrioventricular block has been reported in only a single case, but there is no case of second-degree atrioventricular block associated with thalidomide in the literature. Here, we present the case of a 50-year-old patient who had been using thalidomide for multiple myeloma. The patient was admitted to the emergency service with syncope, and had second-degree (2:1) atrioventricular block. The patient was using thalidomide (100 mg/day) for the previous 5 months and had not had any abnormal findings in previous electrocardiographies. His heart rate on presentation was variable, from 45 to 75 bpm. He was treated with a temporary pacemaker because of hypotension and syncope. Five days after the completion of the thalidomide treatment, the patient's heart rate was 80 bpm. The following day, the pacemaker was removed and a Holter ECG did not show any abnormalities. Patients undergoing thalidomide treatment should be followed up for cardiac and electrocardiographic parameters.

Hiatus hernisi, frenoözefageal ligamentin zayıflamasıyla diyafragmanın özefageal hiatustaki anatomik zayıflık veya defekt nedeniyle intraabdominal organların özellikle barsak anslarının toraks içine deplase olması şeklinde tanımlanmaktadır. Hastanın durumu, semptomları ve gelişebilecek olası komplikayonları göz önüne alınarak seçilecek tıbbi ya da cerrahi tedavisi öncesinde yandaş öncelikli ciddi koroner arter hastalığı varlığında multidisipliner yaklaşımla olgunun tedavisi tamamlanabilir. Bu çalışmada, tesadüfen hiatal herni bulgulanan ve çoklu morbidite faktörüne sahip yüksek cerrahi riskli olguda koroner cerrahi revaskülarizasyon stratejimizi aktarmayı amaçladık, Hiatal hernia is defined as displacement of intraabdominal organs -particularly intestinal loops- into thoracic cavity due to anatomical weakness or defect of diaphragmatic esophageal hiatus secondary to weakening of phrenoesophageal ligament. Taking the functional status of the patient, symptoms and possible complications into account; the case may be treated by multidisciplinary approach before medical or surgical strategy. Gulhane Medical School, University of Health Sciences, The aim of the present study is to determine the association of neutrophillymphocyte ratio (NLR) as an objective marker for detecting compliance to diet in celiac disease (CD). Thirty-seven patients with CD and 37 healthy volunteers were enrolled to the study. At the end of the first year, the patients were divided into 2 groups considering their compliance with diet. Seven patients, who are not compliant to gluten free diet (GFD), formed the first group (group 1). Thirty patients, who are compliant to GFD, formed the second group (group 2), and 37 healthy volunteers served as the control group (group 3). NLR was significantly higher in the patient group than the controls (p < 0.0001). There was a significant difference between the NLR values at the time of initial diagnosis and after a year of treatment in group 2 patients (p < 0.0001). However, we obtained no difference in terms of NLR between the initial and the first year of treatment in group 1 patients (P > 0.05). At the end of the first year, there were significant differences between group 1 and group 2 (p = 0.007) and between group 1 and group 3 in terms of NLR (p = 0.005). Receiver-operating characteristic curve analysis suggested the optimum NLR cutoff value for patients with GFD incompatible as 2.51, with a sensitivity and specificity of 85 % and 94 %, respectively (area under curve: 0.819, 95 % confidence interval = 0.589-1.000, p = 0.009).
