Thanks to shared circulation in monochorionic twins, single intrauterine fetal death (IUD) may lead to acute fetofetal transfusion (aFFTR). AFFTR was present in 49/78 (62.8%) of placentas with arterio-arterial (AA) anastomosis. The aim of the study was to illustrate pregnancy outcomes of monochorionic diamniotic twin pregnancies in our institute - a tertiary fetal medicine centre with fetal therapy program since 2012. 4.5% of pregnancies were delivered at 24-26 weeks of gestation, 4.8% at 26-28 weeks, 18.8% at 28-32 weeks and 51.0% at 32-36 weeks. The essential risk factor for F-F TRF was the presence of superficial low-resistance AA anastomose (p < 0,001) and close umbilical cord insertions. In all cases with umbilical cord insertions less than 4cm acute F-F TRF occurred which also correlates with the diameter of the anastomosis above 1mm (p < 0,005).
