We designed a pediatric proximal femoral nail (PPFN) to overcome fixation method-related complications when performing femoral derotation osteotomy in cerebral palsy patients. Preliminary results of cerebral palsy patients who underwent femoral derotation osteotomy fixed using PPFN to treat in-toeing were evaluated. Sixteen patients with a mean age of 10 years were included. Mean follow-up duration was 36 months. There was no significant difference in the follow-up neck-shaft angle and articulotrochanteric distance values (P = 0.2 and 0.3). PPFN provides stable fixation, early weight-bearing, reduces soft-tissue disruption while limiting the complications due to fixation technique.

Background: Pediatric proximal femoral locking plates (PFLPs) are widely used when performing proximal femoral osteotomy in children with cerebral palsy (CP). The purpose of this study is to report the difficulties and risk factors of titanium PFLPs removal in CP., Methods: PFLP removal was performed in 58 hips of 33 patients (17 males, 16 females). The mean age at the time of surgery (plate removal) was 10.9 (range, 5.7 to 19.2) years. The patients were divided into 2 groups as group 1 and 2, if any difficulty was observed during surgery or not., Results: Difficulty was not detected in 42 (72.4%) hips (group 1). Difficulties were encountered in 16 (27.6%) hips (group 2). A total of 364 screws were used (259 in group 1, 105 in group 2). The mean plate screw density ratios were 0.88 in group 1 and 0.94 in group 2. The difference between group 1 and 2 was statistically significant. The mean duration between the insertion and removal of the PFLP was 14.9 months (11.9 mo in group 1, 22.7 mo in group 2). The difference between group 1 and 2 was statistically significant. The screw heads were cut and the shafts were left in the bone in 4 hips (4 screws); 3 of these 4 screws were calcar screws. Therefore, calcar screw application can be accepted as a handicap for screw removal., Conclusions: As a conclusion, this study suggested that difficulty in titanium PFLP removal in CP is common and PFLP removal is not a harmless procedure. A longer time from internal fixation to removal, increased plate screw density ratio, and calcar screw application are risk factors for difficulties in titanium PFLP removal in CP., Level of Evidence: Level III.

The lateral exposure of the supracondylar femur includes the risk of damaging the neurovascular structures or tightening of the neurological structures within the popliteal fossa may occur as a complication of the osteotomy. Although different pathways of common peroneal nerve (CPN) have been reported throughout the literature, division of deep and superficial branches above the supracondylar femur level has not been reported. A 15-year-old boy with bilateral knee flexion contracture and spastic diplegic cerebral palsy underwent bilateral femoral distal extension osteotomy. The authors found an unusual higher division of CPN above the supracondylar femur level. This report is aimed at warning surgeons about the division of the CPN at a higher level and highlighting a need for a high-powered cadaveric research.

Background: Knee flexion contracture in children with cerebral palsy (CP) is very common and functional impairment leads to a crouch gait. Correction of the knee flexion contracture and improvement of the gait pattern by supracondylar femoral extension osteotomy seems to be a more effective method than distal hamstring procedures in long-standing severe crouch. Only a small number of publications reported the neurological complications of this surgical technique. This study was planned to identify the risk factors leading to neurological complications after supracondylar femoral extension osteotomy in patients with CP., Methods: Supracondylar femoral osteotomies performed for a primary diagnosis of CP with rigid knee flexion deformity of 10 to 30 degrees were included in the study. Supracondylar femoral extension osteotomy was performed in 28 patients (total: 48 cases). Neurological complication was not detected in 43 cases (group 1) and detected in 5 cases (group 2). Previous surgical operation, concomitant operations on the same extremity, application of a brace or long leg cast after operation, preoperative and postoperative popliteal angle, amount of correction, radiologic correction, tourniquet time, level of malnutrition, and emotional state were reviewed., Results: There were 18 boys and 10 girls. The mean age was 12±4 years in group 1 and 13±1 years in group 2. Except 6 patients, all patients had concomitant operations (38 cases in group 1 and 4 cases in group 2). Postoperatively, long leg cast was used in 38 cases and brace in 10 cases. In group 1 mean correction was 23±3.8 degrees and in group 2 it was 19±5.7 degrees., Conclusions: Correlation was not found between the incidence of neurological complications and amount of correction and deformity. After supracondylar femoral extension osteotomy, all patients must be suspected of neurological complication, and measures taken to alleviate the stretch at once if nerve palsy is diagnosed., Level of Evidence: Level IV.

Knee anomalies are common in patients with congenital short femurs who require lengthening to correct limb-length discrepancies. We retrospectively reviewed the incidence of knee arthritis and the factors influencing its occurrence after femoral lengthening using the Wagner method. Twenty-three patients with congenital short femurs treated with the Wagner method were followed up until skeletal maturity (minimum, 5 years postoperatively). The mean age of the patients at lengthening was 10.8 years (range, 8.4-14.5 years). The mean leg-length discrepancy at the time of surgery was 9.7 cm (femur, 7.6 +/- 3.7 cm; tibia, 2.1 +/- 1.8 cm). Femoral lengthening (mean, 7.9 cm) was performed in 17 patients. Femoral lengthening and tibial lengthening were performed simultaneously in six patents (mean, 11.8 cm). The mean age of the patients at the last followup was 16.8 years (range, 14-20.3 years). Eighteen patients had arthritis at followup. Nine patients had severe arthritis develop, seven of whom had knee instability preoperatively and temporary subluxation during the lengthening procedure. Seventy-eight percent of patients had arthritis develop in the knee after lower-limb lengthening using the Wagner method for congenital short femurs. Patients who had an unstable knee before surgery had temporary knee subluxation develop during the lengthening procedure, and patients who had simultaneous lengthening of the femur and tibia had a high association with degenerative arthritis changes in the knee.

The purpose of this study is to evaluate if bilateral peri-ilial pelvic osteotomies (PIPO) combined with proximal femoral varus derotation osteotomies (VDRO) influenced postoperative complications in children with spastic quadriplegia. Bilateral and unilateral hip subluxation or dislocation secondary to spasticity was present in 29 and 61 patients, respectively. The children were divided into 2 groups: group 1 were to be performed a surgery for unilateral PIPO associated with bilateral VDRO, and group 2 were to be performed a surgery for bilateral PIPO and VDRO. The average age of patients at initiation of surgery was 11.7 years (range, 5-20 years) for group 1 and 10.7 years (range, 5-19 years) for group 2. The hospital stay was similar for group 1 and 2 (P = 0.797), which was 9 days. In group 1, 11 (18%) of 61 children needed an average of 2 days (range, 1-7 days) in the intensive care unit postoperatively, and 4 of these 11 patients were reintubated because of respiratory problems. In group 2, 6 (20%) of 29 children were transferred to the intensive care unit postoperatively for an average of 2 days (range, 1-4 days) and 3 of these 6 children were reintubated because of respiratory problems. In conclusion, respiratory problems and anemia were the most common early postoperative complications, which occur with a similar rate in children with spastic quadriplegia who underwent bilateral or unilateral peri-ilial pelvic osteotomy(ies) combined with proximal femoral osteotomies. We believe that bilateral hip procedures including PIPO, proximal femoral osteotomies, and soft-tissue release can be performed safely in 1 stage and, based on this data, the staged procedure would probably have the same risk as the first procedure.

Twenty percutaneous rotational osteotomies, stabilized with interlock nails, were performed in the lower limbs of 15 skeletally mature adolescents with cerebral palsy to correct rotational deformities. The medical records and radiographs of those patients were retrospectively reviewed. Nineteen osteotomies (95%) in 15 patients healed without major complications. One patient had one tibia (5%) pseudarthrosis, which was successfully treated with additional fibular osteotomy and exchanging the nail. Excluding this case, the average healing time for the femoral and tibial osteotomies was 8 weeks, ranging from 7 to 9 and from 6 to 10 weeks, respectively. Casting was not required to add stability. Percutaneous rotational osteotomy with intramedullary nail fixation is a reliable and effective treatment option to correct rotational malalignment of the lower limb in skeletally mature patients with cerebral palsy.

Radii of cortical and medullary curvatures of femurs were calculated using these images. The values obtained were compared to the radius of curvatures of ten different intramedullary nails., Results: Medullary bowing was between 114 and 1,389 mm (mean: 722 mm, SD: 230 mm) and the cortical bowing was between 109 and 1,666 mm (mean: 770 mm, SD: 267 mm). For males, these values were 114-1,389 mm (mean: 722 mm, SD: 230 mm) and 109-1,666 mm (mean: 770 mm, SD: 267 mm), respectively. For females, they were 114-1,389 mm (mean: 722 mm, SD: 230 mm) and 109-1,666 mm (mean: 770 mm, SD: 267 mm), respectively. The differences between genders were not significant. Cortical and medullar bowing was strongly correlated with age (r=-0.269, p<0.006 and r=-0.234, p<0.017, respectively). These significances were produced by females only. Radii of curvatures of intramedullary nails ranged between 150 and 300 cm and were higher than the mean cortical (77 cm) and medullary (72.2 cm) bowings., Conclusion: The difference between the curves of femur and the contemporary femoral nails implicates the inadequacy of the design of such nails for the Caucasian race living in Anatolia. Therefore, such nails should be revised accordingly to prevent the above-mentioned complications.

A new technique using a hinged external fixator to stabilize an open reduction with pelvic and femoral osteotomies has been developed for treating high-dislocated hips in older children with developmental dislocated hip (DDH). This technique was performed in 11 patients (12 hips) at a mean age of 8.2 years. At follow up, radiographic results showed no redislocation/subluxation and clinical results demonstrated 11 hips as excellent/good and only one hip as poor from persistent stiffness. In conclusion, this new technique produces acceptable results in the treatment of older children with high dislocation of the hip from DDH.

Unlabelled: Pelvic support osteotomies for chronically dislocated hips improve stability but result in limb-length discrepancy and valgus deformity of the knee. We prospectively evaluated and followed up 16 patients (14 females, two males) to ascertain whether a monolateral external fixator could achieve a pelvic support osteotomy and eliminate the limb-length discrepancy without disturbing knee motion. The patients had a mean age of 25.3 years at the time of surgery. The Harris hip score, knee range of motion, and Trendelenburg sign were evaluated preoperatively and at followup. The average time from fixator application until removal was 7 months (range, 5.2-9.5 months). The followup averaged 52.5 months (range, 26-84 months). The mean Harris hip score increased from 50 points (range, 32-73 points) preoperatively to 87.6 points (range, 67-98 points) at last followup. Four patients retained a positive Trendelenburg sign. At the time of fixator removal, 88% of preoperative knee range of motion was retained, and preoperative knee motion was achieved at last followup in all patients. The monolateral external fixator was suitable for a pelvic support osteotomy, equalizing limb-length discrepancy, and obtaining lower extremity alignment. Unlabelled: Eleven patients with femoral diaphyseal nonunions after intramedullary nailing were treated with cyclic compression and distraction with an external fixator over the nail. We evaluated the limitations of this technique and whether patients having this closed procedure could achieve union without additional operative procedures. Patients with hypertrophic nonunions (n = 4) were treated with gradual compression of the nonunion site. Cyclic compression and distraction was done in patients with oligotrophic (n = 2) or atrophic nonunions (n = 5) to stimulate consolidation. The average age of the patients was 32.9 years (range, 21-48 years), and the average followup was 40.5 months (range, 24-64 months). Union was achieved in an average of 5.8 months in all patients after one operation and without additional surgical intervention. However, pain necessitating strong analgesic agents and pin-related complications consisting of osteomyelitis, septic arthritis, and pin breakage in the atrophic nonunion group were a major limitation of this technique. Based on our study, the cyclic compression and distraction technique can be used in hypertrophic and oligotrophic nonunions that have failed one or more prior exchange nailings. Objectives: Using a synthetic bone model, we investigated changes in the muscle length of the gluteus medius and gluteus minimus following trochanteric osteotomy and pelvic support osteotomy (PSO) and compared the results with those of traditional PSO., Methods: On two pelvises and four femurs, the lengths of the gluteus medius and gluteus minimus were measured in the following circumstances, with the hips in neutral position and in 45 degrees of flexion: (i) alignment of the hip joint with normal congruency; (ii) dislocated hip joint; (iii) following an osteotomy 2.5 cm below the lesser trochanter and stabilization with an angulation of 45 degrees of abduction; (iv) the insertion point was then moved 2 cm distally and 1 cm laterally, simulating a translation osteotomy of the greater trochanter., Results: The muscle lengths increased with PSO compared to those of the dislocated hips (p<0.0001). Following distal and lateral translation osteotomy, the lengths significantly exceeded those obtained with traditional osteotomy (p<0.002), but were significantly less than those in the neutral position (p<0.001). Measurements in 45 degrees of flexion yielded similar results. Normal lengths could not be obtained in any of the procedures., Conclusion: Distal and lateral translation osteotomy following traditional PSO seems to increase the length of the abductor moment arm more than that obtained by traditional PSO alone.
