5. [Clinical value of emergency endovascular embolization in the interventional treatment for oral hemorrhage caused by carcinoma]

To evaluate the clinical value of emergency endovascular embolization in the interventional treatment for oral hemorrhage caused by carcinoma, 32 patients with oral hemorrhage caused by carcinoma, who received emergency endovascular embolization due to unsatisfactory hemostatic effect of conventional conservative treatment in the First Affiliated Hospital of Zhengzhou University from January 2014 to December 2019, were included in this study and their clinical data, laboratory data and imaging information were retrospectively analyzed. There were 16 males and 16 females, aged (60.6±13.6) years (34-88 years). Technical successful rate of emergency endovascular embolization, immediate successful rate of controlling hemorrhage, blood pressure before and after operation, hemoglobin before and after operation, postoperative complications and recurrence rate of oral hemorrhage were statistically analyzed. Results showed that technical successful rate of operation and immediate successful rate of controlling oral hemorrhage are both 100% (32/32). Recurrent oral hemorrhage occurred in 4 patients (13%). The hemorrhagic shock symptoms of all patients were significantly improved after interventional therapy. After operation, local swelling happened in 34% (11/32) patients and intermittent local pain happened in 22% (7/32) within 24 hours; the swelling and the pain gradually disappeared from 2nd to 5th days. Mild complications of transient fever happened in 9% (3/32) patients and disappeared spontaneously in the short term. No serious complications such as blindness, cerebrovascular accident or central nervous system disturbance occurred in all patients after operations. During the whole follow-up period (1 to 12 months), a total of 8 patients died. The causes of death were progression and metastasis of carcinoma (为分析恶性肿瘤致口腔出血的急诊血管内栓塞介入治疗价值,本研究纳入2014年1月至2019年12月期间郑州大学第一附属医院介入科收治的32例恶性肿瘤致口腔出血患者,均因常规保守治疗止血效果不理想而接受急诊血管内栓塞介入治疗,其中男性和女性各16例,年龄(60.6±13.6)岁(34~88岁)。回顾性分析患者的临床资料、实验室数据与影像学检查,计算急诊血管内栓塞介入治疗恶性肿瘤致口腔出血的技术成功率、即时口腔出血成功控制率、血压、血红蛋白、术后并发症及口腔出血复发率。结果显示,患者急诊血管内栓塞技术成功率及即时口腔出血控制成功率均为100%(32/32);13%(4/32)患者复发口腔出血。术后所有患者的失血性休克症状均较术前显著改善。术后24 h,34%(11/32)患者出现局部肿胀感,22%(7/32)的患者间断性局部疼痛,术后2~5 d肿胀、疼痛逐渐减轻至消失。术后1周,9%(3/32)患者出现一过性发热,对症处理后体温恢复正常。所有患者术后均未出现失明、脑血管异位栓塞等严重并发症。术后随访1~12个月,8例患者死亡,死因分别为恶性肿瘤进展与转移(4例)、心功能衰竭(2例)、重度肺炎(1例)和口腔出血复发导致的呼吸衰竭(1例)。急诊血管内栓塞介入治疗的短期疗效显著、可重复操作、出血复发率低、并发症发生率低,可用于恶性肿瘤致口腔出血的临床治疗。.

6. Associations of IL-1β and IL-6 gene polymorphisms with Parkinson's disease

The aim of this study was to explore the associations of interleukin-1β (IL-1β) and IL-6 gene polymorphisms with the pathogenesis of Parkinson's disease.A total of 200 patients with Parkinson's disease in our hospital were collected as the disease group. Meanwhile, 200 healthy subjects were taken as the control group. Peripheral blood samples were drawn from all research subjects. The polymorphic regions of IL-1β and IL-6 were amplified via polymerase chain reaction (PCR). Moreover, the polymorphisms were detected and analyzed, followed by further analysis based on the changes in gene expressions and Hoehn-Yahr grade of patients.The allele distributions at IL-1β rs571556428 (p=0.015) and IL-6 rs543214973 (p=0.012) were statistically different between control group and disease group. In disease group, the G allele frequency at IL-1β rs571556428 and T allele frequency at IL-6 rs543214973 were significantly higher (p0.05). Genotype distributions at IL-1β rs572292175 (p=0.017) and rs571556428 (p=0.000), and IL-6 rs543214973 (p=0.002) in disease group were also different from those in control group. In addition, the frequencies of CT genotype at IL-1β rs572292175, AA genotype at IL-1β rs571556428 and AA genotype at IL-6 rs543214973 in disease group were significantly lower (p0.05). After modeling and analysis, it was found that the distribution of recessive model at IL-1β rs571556428 (p=0.012) and IL-6 rs543214973 (p=0.014) in disease group exhibited significant differences from those in control group. The frequencies of TA haplotype at IL-1β rs572292175 and rs571556428 (p=0.038) and GA haplotype at IL-6 rs1474348 and rs543214973 (p=0.047) in disease group were lower than those in control group (p0.05). The polymorphisms at IL-1β rs571556428 and IL-6 rs1474348 were significantly associated with gene expression (p0.05). Moreover, the expressions of IL-1β and IL-6 rose significantly in patients with GG genotype at rs571556428 and CG genotype at rs1474348, respectively (p0.05). Furthermore, the polymorphism at IL-1β rs571556428 was significantly correlated with the grade of Parkinson's disease (p=0.000). Parkinson's disease was in a higher grade (grade 4-5) in patients with AA genotype, whereas in a lower grade (grade 1-2) in patients with GG and AG genotypes.IL-1β and IL-6 gene polymorphisms are significantly associated with the pathogenesis of Parkinson's disease.

目的 探讨利妥昔单抗联合短疗程、高强度方案治疗成人Burkitt白血病患者的疗效和安全性。 方法 收集2006年1月30日至2018年9月12日中国医学科学院血液病医院收治的11例Burkitt白血病患者病例资料,分析统计患者的临床特征、完全缓解(CR)率、总生存率、无复发生存率及不良事件。 结果 11例患者中位年龄34(15~54)岁,其中男6例,女5例。发病时中位WBC 12.28(2.21~48.46)×109/L,HGB 113(74~147)g/L,PLT 35(13~172)×109/L,乳酸脱氢酶2 721(803~17 370)U/L,外周血中位原始细胞比例0.40(0.03~0.76),骨髓中位原始细胞比例0.840(0.295~0.945)。10例患者接受利妥昔单抗联合短疗程、高强度化疗,其中2例患者巩固化疗后行自体造血干细胞移植。所有治疗患者1个疗程CR率为100%,4年总生存率为90%,4年无复发生存率为90%。所有治疗患者中,只有1例患者在诱导化疗中出现肿瘤溶解综合征,经血液透析等治疗后肾功能恢复。无治疗相关性死亡病例。 结论 利妥昔单抗联合短疗程、高强度方案治疗成人Burkitt白血病疗效及安全性均较为理想。

目的 探讨急性早幼粒细胞白血病(APL)患者继发治疗相关性髓系肿瘤(t-MN)的临床特点、诊断、治疗及预后。 方法 回顾性分析中国医学科学院血液病医院2012年10月至2019年1月收治的4例APL继发t-MN患者的临床资料,并进行相关文献复习。 结果 4例APL继发t-MN患者均为女性,中位年龄42(40~53)岁,3例接受了以维甲酸(ATRA)+亚砷酸(ATO)为基础联合蒽环/蒽醌类药物±阿糖胞苷的前期诱导缓解及巩固治疗方案,1例采用了ATRA联合蒽环/蒽醌类药物±阿糖胞苷的治疗方案,均没有使用烷化剂。在APL获得完全缓解(CR)后40~43个月出现t-MN,其中治疗相关性骨髓增生异常综合征(t-MDS)1例,治疗相关性急性髓系白血病(t-AML)3例,出现t-MN时PML-RARα融合基因均为阴性。3例t-AML患者接受了2~4个疗程再诱导治疗,其中有1例t-AML患者在获得CR后行异基因造血干细胞移植(allo-HSCT),1例t-MDS患者接受了去甲基化治疗。中位随访54.5(48~62)个月,2例t-AML患者死亡,出现t-MN后中位生存期为12(5~18)个月。1989至2018年文献共报道63例APL继发t-MN病例,与本次报道的4例汇总分析,67例患者中男27例,女40例;中位年龄为52.5(15~76)岁;中位潜伏期39(12~168)个月,确诊t-MN后中位生存时间为10(1~39)个月。 结论 APL继发t-MN较为少见,目前缺乏有效的防治措施,预后不佳,在随访过程中(尤其是获得CR后39个月左右)若出现病情变化,应警惕t-MN的发生,对此类患者应尽快明确疾病的变化,给予合理的治疗。

Chronic obstructive pulmonary disease (COPD) is an incomplete, reversible disease with progressive inflammation obstruction in airways. This study aims to explore the regulatory mechanism of hypoxia-inducible factor-1α (HIF-1α) in inflammatory response and progression of COPD.71 bronchoalveolar lavage fluid (BALF) samples were collected, including 59 samples from COPD patients (COPD group) and 12 from patients with normal pulmonary function (control group). The mRNA and protein levels of HIF-1α and epidermal growth factor receptor (EGFR) in BALF were detected by quantitative Real Time-Polymerase Chain Reaction (qRT-PCR) and Western blot, respectively. Serum levels of interleukin-13 (IL-13), IL-9, IL-1, and tumor necrosis factor-α (TNF-α) were detected by enzyme-linked immunosorbent assay (ELISA). Hypoxia cell model was constructed by COCl2 induction in human embryonic lung cells. Expression levels of HIF-1α, EGFR and p-AKT in NCI-H1563 cells treated with 740Y-P, the phosphoinositide 3-kinase (PI3K) agonist were detected. Finally, we detected proliferation and apoptosis in NCI-H1563 cells with HIF-1α overexpression by cell counting kit-8 (CCK-8) assay and flow cytometry, respectively.The mRNA and protein levels of HIF-1α and EGFR were higher in COPD groups compared with those of control group. Serum levels of IL-13, IL-9, IL-1, and TNF-α in COPD patients were elevated. CoCl2 induction in NCI-H1563 cells led to upregulated levels of IL-13, IL-9, IL-1, and TNF-α. 740Y-P treatment remarkably activated EGFR/PI3K/AKT pathway. Overexpressed HIF-1α inhibited proliferation but induced apoptosis of NCI-H1563 cells.HIF-1α was overexpressed in COPD, which upregulated expressions of inflammatory factors via activating the EGFR/PI3K/AKT pathway. The activated EGFR/PI3K/AKT pathway induced by pulmonary inflammation further upregulated HIF-1α expression in a feedback loop, thus aggravating COPD pathological changes.

12. Screening of parathyroid gland by high frequency ultrasound and the relationship between recurrent urinary calculi and primary hyperparathyroidism

To understand the value of high frequency ultrasound in the clinical screening of parathyroid gland, and to summarize the intrinsic relationship between primary hyperparathyroidism and recurrent urinary calculi.98 cases of urinary calculi were randomly selected, and the patients were admitted to our hospital from March 2014 to August 2017. A total of 100 healthy subjects were selected as group B in the same period. High frequency color Doppler ultrasonography scan recorded the results.Among the subjects in group A, 67 (68.37%) showed parathyroid gland, 14 cases (14.29%) had tumor mass in the parathyroid system, 40 cases more than those in group B (40.00%) and 2 cases (2.00%), (p0.05). There were 10 cases (10.20%) of primary hyperparathyroidism in group A and no cases of primary hyperparathyroidism in group B (p0.05). The occurrence of primary hyperparathyroidism was 26.92% (7/26) in the number of cases, with 3 and more cases of urinary calculi, which was higher than that in the first recurrent cases (3/72), (p0.005).One of the key causes of recurrent episodes of urinary calculi is primary hyperparathyroidism, which can be applied to high frequency ultrasonography to develop professional screening of parathyroid gland in cases of urinary calculi.

In this study, we aimed to investigate the downstream effector of GLI2 in gastric cancer (GC) and their regulative effect on cancer stem cell (CSC) properties of GC.Bioinformatic data mining was performed in TCGA-Stomach Adenocarcinoma (STAD), as well as in Kaplan-Meier plotter. Moderate-differentiated GC cell line SGC-7901 and poor-differentiated GC cell line MKN-45 were used as in-vitro model to investigate the regulative effect of GLI2 on PDGFRB expression. MKN-45 cells were further used to explore the effect of GLI2 shRNA or PDGFRB shRNA on CSC properties of the cells.Bioinformatic results showed that GLI2 is usually upregulated in GC tissues than in normal tissues, and high GLI2 expression is associated with unfavorable first progression free survival (PFS) and also worse overall survival (OS) in patients with GC. PDGFRB is co-upregulated with GLI2 in GC and its promoter region contains a putative GLI2 binding site. The results of dual luciferase assay confirmed this binding site. Enforced GLI2 expression elevated PDGFRB expression at both mRNA and protein level. GLI2 or PDGFRB knockdown showed similar effect on reducing spheroid colony formation and on reducing the expression of CSC related genes, including CD44, Nanog, and Oct4 in MKN-45 cells.High GLI2 or PDGFRB expression is associated with unfavorable survival in GC patients. GLI2 can induce PDGFRB expression in GC cells via directly binding to its promoter. In addition, the GLI2-PDGFRB axis might be an important signaling pathway modulating CSC properties of GC cells.

M344 is a novel histone deacetylase inhibitor. There is no report on the effect of M344 treatment on the development of pig embryos after somatic cell nuclear transfer (SCNT). In the present study, we investigated the effect of M344 on the blastocyst formation rate in cloned embryos, acetylation level of histone H4 lysine 12 (AcH4K12), and the expression of pluripotency-related genes , , and . Our results indicated that treatment with 5 µ M344 for 6 h improved the development of porcine embryos, in comparison with the untreated group (25.1% ± 5.0 vs. 10.9% ± 2.4;0.05). Moreover, M344-treated embryos had increased average fluorescence intensity of AcH4K12 at the pseudo-pronuclear stage (0.05). However, no differences exist in Oct4, NANOG, and SOX2 expression in M344-treated and untreated SCNT blastocysts. In evaluating the effect of M344 on in vivo development, 845 M344-treated embryos were transferred into 3 surrogates, 1 of whom became pregnant and developed 3 fetuses. These findings suggested that M344 elevated the level of histone acetylation, facilitated the nuclear programming, and subsequently improved the developmental competence of pig SCNT embryos.

In the present study, the exon 2 and 3' end sequence of intron 1 of BoLA-DRB3 gene of 211 Beijing Holstein cows was amplified and a uniform fragment of 284 bp was obtained. The genetic polymorphism was investigated by PCR-RFLP using two restriction endonucleases HaeIII and BstYI. Seven genotypes were detected by digesting the PCR products with HaeIII. The frequency of AA, AB, AC, AD, BB, BC and BF genotypes was 0.4638, 0.0193, 0.0193, 0.3478, 0.0290, 0.0386 and 0.0822, respectively. Three genotypes were found by digesting the PCR products with BstYI. The frequency of AA, AB and BB genotypes was 0.0569, 0.2844 and 0.6587, respectively. The relationship between the polymorphisms in exon 2 of BoLA-DRB3 gene and somatic cell score (SCS) in Beijing Holstein cows was analyzed by least squares linear model. No significant difference was detected among least squares means of SCS for seven HaeIII-RFLP genotypes (P0.05). As for BstYI-RFLP analysis, least squares mean of SCS for AA was significantly lower than that for AB (P0.05) or BB (P0.05), and no significant difference was detected between AB and BB genotypes (P0.05). BstYI AA was the most favorable genotype and BstYI BB was the most unfavorable genotype for mastitis resistance. The information found in the present study is very important for improving mastitis resistance in dairy cattle by marker assisted selection.

Computer assisted surgery (CAS) for tumor resection can assist the surgeon in locating the tumor margin accurately via some form of guidance method. A wide array of guidance methods can be considered, including model-based visual representations, symbolic graphical interfaces, and those based on other sensory cues such as sound. Given the variety of these guidance methods, it becomes increasingly important to test and analyze guidance methods for CAS in a quantitative and context-dependent manner to determine which is most suitable for a given surgical task. In this paper, we present a novel experimental methodology and analysis framework to test candidate guidance methods for CAS. Different viewpoints and stereographic, symbolic and auditory cues were tested in isolation or in combination in a set of virtual surgery experiments. A total of 28 participants were asked to circumscribe a virtual tumor with a magnetically tracked scalpel while measuring the surgical trajectory. This allowed measurement of surgical accuracy, speed, and the frequency with which the tumor margin was intersected, and enabled a quantitative comparison of guidance approaches. This study demonstrated that adding sound to pictorial guidance methods consistently improved accuracy, speed and margin intersection of the virtual surgery. However, the use of stereovision showed less benefit than expected. While guidance based on a combination of symbolic and pictorial cues enhanced accuracy, we found that speed could be substantially impaired. These studies demonstrate that optimal guidance combinations exist which would not be apparent by studying individual guidance methods in isolation. Our findings suggest that care is needed when using expensive and sometimes cumbersome virtual visualization technologies for CAS, and that simpler, non-stereo presentation may be sufficient for specific surgical tasks.

To explore the diagnostic value of serum procalcitonin (PCT), interleukin-10 (IL-10), C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) in community acquired pneumonia (CAP) and tuberculosis (PTB).113 CAP cases patients and 78 PTB cases were enrolled from May 2011 to March 2012. Routine blood test, serum PCT, CRP, IL-10 and ESR of patients within 24 hours were analyzed retrospectively.The serum concentrations of PCT, IL-10, CRP and ESR in CAP patients with CAP were 0.35±0.017 mg/mL, 0.095±0.004 mg/L, 59.80±5.12 mg/L and 35.00±4.81 mm/1h, respectively, significantly higher than patients with PTB (p0.01); According to the result of ROC curve analysis in CAP and PTB, the PTC area under ROC curve is 0.715 (95% CI 0.647-0.782), the sensitivity and specific degree of serum PTC were significant better than CRP and IL10 (p0.05). In tuberculosis sputum culture, the serum concentrations of IL-10 and ESR in TB positive group were 0.045±0.013 mg/L and 62.50±8.69 mm/1h, significantly higher than that of TB negative group (p0.05); whereas, the concentrations of serum PCT and CRP in TB positive and negative groups had no significant difference (p0.05).The levels of serum PCT, IL-10, CRP and ESR in CAP patients are higher than that in PTB patients. Therefore, the serum PCT, IL10, CRP and ESR level is benefit to distinguish between CAP and PTB. This could provide a comprehensible evidence for both diagnosis and prognosis.

Thyroid stimulating hormone beta chain (TSHB) is mainly expressed in pituitary and its expression is closely related to photoperiodic control of seasonal reproduction in animals. In the present study, ten primer pairs have been used to clone the DNA sequence and to detect genetic mutations of goat TSHB gene. Two DNA fragments of goat TSHB gene were obtained, which were 2,614 and 1,031 bp in length, respectively. They comprised about 2.5 kb 5′ regulatory region, all of the three exons and two introns. Goat TSHB gene has a coding region of 417 bp, encoding 138 amino acids which was predicted to be a secretory protein with a signal peptide of 16 amino acids. The sequence of TSHB gene is highly conserved among mammals. In addition, five mutations (C53A, 3 bp Indel at the 287–289 locus, 34 bp Indel at the 584–617 locus, A1819C and E2_72TA) were found in goat TSHB gene and they were shown to be in strong linkage disequilibrium. Interestingly, the genotype distributions for both single locus and haplotype have shown to be significant different between seasonal and nonseasonal goat breeds. And haplotype H2 and diplotype H2/H4 may be related to year-round estrus. We preliminarily presumed that the five closely linked mutations of goat TSHB gene may be part of the causal sources for the diversities of reproductive seasonality in goats. Our study may provide a possible efficient genetic way to decrease seasonality in goats.

Hirschsprung's disease (HD) is a common congenital disease of colorectum. Although it was described more than one century ago, the effective treatment was established only half a century later. The initially treatment consisted of preoperative diverting colostomy, followed by definite pull-through and closure of colostomy on later date. A variety of procedures like Swenson, Duhamel, Rehbein and Soave were evolved with time. With the better understanding of pathogenesis and improvement on surgical technique, now the operation has become less extensive. In recent days, the classical 3 staged procedure is replaced by single staged procedure. All the procedure can be completed transanally. Since De la Torre in 1998, first reported total transanal endorectal pull-through, it became the most popular technique for the treatment of HD.

Computer assistance for breast conserving surgery requires a guidance method to assist a surgeon in locating tumor margin accurately. A wide array of guidance methods can be considered ranging from various pictorial representations, symbolic graphical interfaces as well as those based on other sensory cues such as sound. In this study, we present an experimental framework for testing candidate guidance methods in isolation or in combination. A total of 22 guidance approaches, based on stereographic, non-stereographic, symbolic and auditory cues were tested in a simulation of breast conserving surgery. Observers were asked to circumscribe a virtual tumor with a magnetically tracked scalpel while measuring the spatial accuracy, time and the frequency with which the tumor margin was intersected. A total of 110 studies were performed with 5 volunteers. Based on these findings, we demonstrated that a single view of the tumor with a stereo presentation in conjunction with an auditory guidance cue provided the best balance of accuracy, speed and surgical integrity. This study demonstrates a practical and helpful framework for testing guidance methods in a context dependent manner.

Cancer incidence(1950-1995) among 27,011 medical X-ray workers in comparison with 25,782 non X-ray medical specialists between 1950 and 1980 in China was investigated. The average cumulative dose received by the X-ray workers also reconstructed by retrospective dosimetry methods. Significant cancer risk was seen among medical X-ray workers(RR = 1.2). Significantly elevated risks were found in leukemia, cancer of skin, female breast, lung, liver, bladder and esophagus, the RRs were 2.2, 4.1, 1.3, 1.2, 1.2, 1.8 and 2.7 respectively. The patterns of cancer risk were associated with years since beginning of X-ray work, age and calendar year of initial employment and cumulative dose suggest that the risks of leukemia, skin cancer and female breast cancer, possibly thyroid cancer were related to occupational exposure to X-rays. A significant cancer risk could be induced by prolonged exposure to low dose ionizing radiation when the cumulative dose reached a certain level.

To investigate the specific brain drug targeting of 5-fluoro-2'-deoxyuridine (FUdR) by synthesis of 3', 5'-dioctanoyl-5-fluoro-2'-deoxyuridine (DO-FUdR) and incorporation into DO-FUdR pharmacosomes (DO-FUdR-PS).DO-FUdR-PS was prepared by thin-layer ultrasonication technique. In vitro drug release was studied in pH 7.4 phosphate-buffered saline containing 0.3% pancreatic enzyme at 37 degrees C using bulk-equilirium reverse dialysis bag technique. The concentration of FUdR in various organs were determined by reversed phase HPLC after i.v. administration of DO-FUdR-PS and FUdR.The mean particle size of DO-FUdR-PS was 76 nm with drug loading of 29.02% and entrapment efficiency of 96.62%. The in vitro drug release kinetics could be characterized by bioexponential equation. Compared with FUdR injection, DO-FUdR-PS showed high concentration in tested organs. The brain AUC ratio of DO-FUdR-PS to FUdR was 10.97.DO-FUdR-PS showed a good targeting efficiency in vivo. PS can improve the ability of drug to cross blood-brain barrier and is a promising drug targeting system for the treatment of central nervous system disorders.

The effect of 8-(N, N-diethylamino)-n-octyl-3,4,5-trimethoxybenzoate (TMB-8) on the elevation of [Ca2+]i induced by 2, 5-di (tert-butyl)-1, 4-benzohydroquinone (BHQ), norepinephrine (NE), KCl in cultured single smooth muscle cells of the calf basilar artery was studied by a system of measurement of AR-CM-MIC, using Fura-2/AM as a fluoresent indicator. In the presence of extracellular Ca2+ 1.3 mmol.L-1, the resting [Ca2+]i was not changed by TMB-8 (10, 30 and 100 mumol.L-1), but the elevation of [Ca2+]i induced by BHQ, NE and KCl were reduced by TMB-8 (30 mumol.L-1) significantly. In Ca2+ free Hank's solution containing EGTA 0.1 mmol.L-1, the resting [Ca2+]i was markedly reduced by TMB-8 (10, 30 and 100 mumol.L-1), and the increase of [Ca2+]i evoked by BHQ and NE was blocked completely by TMB-8 (30 mumol.L-1). The result suggested that TMB-8 inhibited the Ca2+ release from intracellular stores or increased the up-take of Ca2+ into sarcoplasmic reticulum and the inhibition of Ca(2+)-influx from extracellular site may be an indirect machanism.

To investigate the antitumor effects of cytosine deaminase (CD) gene in combination with prodrug flucytosine (Flu, 5-fluorocytosine) on human hepatocellular carcinoma.CD gene was transduced into human hepatocellular carcinoma cell line SMMC7721 with retroviral method and the cytotoxicity of Flu on the tumor cells was assayed in vitro with clonogenic techniques. The xenograft tumor model in nude mice was used to study in vivo therapeutic effects of CD gene/Flu system against human hepatocellular carcinoma.CD gene/Flu system had significant antitumor activities on human hepatocellular carcinoma cells in vitro and in nude mice. The antitumor activities of Flu 500 mg.kg-1 on hepatocellular carcinoma xenografts in nude mice were more potent than those of 5-fluouracil 10 mg.kg-1. CD gene/Flu system possessed bystander killing effects on hepatocellular carcinoma xenografts in nude mice.The experiment demonstrates the potential value of the CD gene/Flu system in the treatment of human hepatocellular carcinoma.

To study the effects of quercetin (Que) on myocardial excitation-contraction coupling and cardiac remodeling.Left ventricles and femoral arteries of rats were cannulated for hemodynamic recording. Mouse cardiac hypertrophy was induced by abdominal aortic coarctation (AAC). Cultured myocardial cells in neonatal rats were loaded with Fura 2-AM. The intracellular calcium ([Ca2+]i) and spontaneous [Ca2+]i oscillations ([Ca2+]i-SO) were tested by AR-CM-MIC cation measurement system.Que 3 or 25 mg.kg-1 i.v. in rats decreased heart rate from (420 +/- 19) to (390 +/- 15) and (314 +/- 18) beat.min-1, respectively, companied with very modest changes in both left ventricular pressures (LVP) and its differential dpLV/dtmax. Que 10, 50, 250 mumol.L-1 concentration-dependently slowed the frequency of [Ca2+]i-SO in cultured myocardial cells from (26 +/- 4) to (25 +/- 3), (18 +/- 4), and (12 +/- 3) time.min-1, respectively, but did not change their resting [Ca2+]i or amplitudes of [Ca2+]i-SO. Similarly, the increases in frequency of [Ca2+]i-SO caused by either isoproterenol (Iso) or ouabain (Oua) were prevented by Que 100 mumol.L-1, while the simultaneous increases in amplitude of [Ca2+]i-SO remained. Besides, [Ca2+]i rises excited by angiotensin II (Ang II) but not high [K+]o were prevented by Que 100 mumol.L-1. Daily administration of Que 120 mg.kg-1 i.g. for 5 d markedly prevented the cardiac hypertrophy in AAC mice, without effects on the ventricular mass to body weight ratio (VM/BW) in sham-operated mice.Que decreased myocardial [Ca2+]i-oscillation frequency and prevented cardiac remodeling, but had no direct effect on cardiac excitation-contraction coupling.

To study whether prolonged or repeated low-dose ionizing radiation could induce female breast cancer, we analyzed the incidence and risk factors of breast cancer among female medical diagnostic X-ray workers in China by cohort study and case-control study nested in the cohort. The risk of breast cancer enhanced significantly than the control group. It occurred in those who engaged in X-ray work before 1960, those who have been worked more than 25 years and those who are exposed before age 30, however, the age of onset cancer did not advance. The significant risk factors are accumulative radiation dose of the breast, obesity and family history of breast cancer. In addition, interaction could exists between obesity, non-lactation history and occupational X-ray exposure.

The effect and mechanism of EGF and progesterone on the meiotic maturation resumption were studied. The results showed that progesterone regulated the action of EGF. The inhibitor of 3 beta-HSD, the key enzyme of progesterone synthesis, Epostane could not only prevent the effect of EGF, but inhibit the progesterone production promoted by EGF in cultured ovary granulosa cells as well. On the other hand, heparin could lessen the function of EGF and progesterone, which implies that calcium participated the effect of EGF and progesterone. Fura-2 was used to indicate the changes of intracellular calcium level in single cumulus cell. It is found that a calcium wave appeared in cumulus cell when EGF or progesterone acted. But Ca2+ level returned to normal immediately after progesterone was added, and Ca2+ in the cumulus cell still remained higher level after EGF acted. This results indicated that calcium in cumulus cell adjusted the action of EGF and progesterone.

To examine the relationship between various hormonal and metabolic variables in a large group of women with unequivocal evidence of polycystic ovarian syndrome (PCOS) to dissect out the metabolic heterogeneity of this condition.Cross-sectional observational study of PCOS (n = 122) and non-PCOS (n = 26) subjects.Reproductive medicine unit in a tertiary teaching hospital.Subjects with presumed PCOS were recruited from the Reproductive Medicine and Gynaecological Clinics and later confirmed as PCOS with recognized criteria. Several other subjects were identified through recruiting reference subjects. The PCOS population consisted of 122 patients. Reference subjects were recruited from partners of male factor infertility patients in the clinics and from the general population (n = 27).A 75 g 2-hour oral glucose tolerance test was performed on all subjects in their midluteal phase. Blood was taken at fasting and at 30, 60, 90, and 120 minutes.Age, body mass index (BMI), waist to hip ratio, levels of integrated glucose and insulin, concentrations of maximum insulin, sex hormone-binding globulin, T, triglyceride, apolipoproteins (Apo A1, B), high-density lipoprotein cholesterol, and low-density lipoprotein cholesterol (LDLC).Five clusters could be identified. They are characterized as a nonobese group, a moderately obese group, and three very obese groups. The nonobese group (n = 41, BMI = 24.1) exhibited the lowest level of integrated insulin (236.4 mIU/L or microU/mL) and concentration of serum T (5.5 nmol/L). The moderately obese group had the second lowest level of integrated insulin (497.1 mIU/L) whereas the three very obese groups (n = 15, 13, and 5, respectively) had significantly higher but different levels of integrated insulin (group 3: 850.8 mIU/L; group 4: 1,131.5 mIU/L; and group 5: 1,531.9 mIU/L), triglyceride (group 3: 1.39 mmol/L; group 4: 1.76 mmol/L; and group 5: 2.78 mmol/L [1 mmol/L = 88mg/mL]), Apo B (group 3: 1.18 g/L; group 4: 1.08 g/L; and group 5: 1.55 g/L) and LDLC (group 3: 3.81 mmol/L; group 4: 3.05 mmol/L; and group 5: 5.06 mmol/L [1 mmol/L = 38.6 mg/100 mL]).The metabolic heterogeneity of the PCOS population is reflected at least partly in patients' levels of insulin, lipids, and lipoproteins, dependent and independent of BMI.

A foveal cone densitometer was built for measuring the kinetics of foveal cone pigments in human eyes. The light from a 500-W xenon lamp was divided into reference, measuring, and bleaching beams, and projected through an optical fiber leading to a modified fundus camera. After the light was projected into the ocular fundus, the reflected light (central 1 degree) was measured by a photomultiplier and the regeneration time and the two-way density were abstracted from the records. The two-way density, which is the decadic logarithm of the measuring-reference beam ratio under bleached conditions divided by the measuring-reference beam ratio under dark conditions, eliminated the artifacts caused by eye movement of blink. Reliable curves were obtained in 45 eyes out of 53 healthy Japanese subjects (age range, 9 to 82 years). The mean +/- standard deviation SD of the two-way density (log) and the time constant (sec) were 0.34 +/- 0.09 and 139.9 +/- 79.3, respectively. The time constant significantly increased with aging.

A follow-up study regarding the occurrence of segmental collapse of femoral head has been done in 53 patients of united femoral neck fracture including a periodic examination by 99mTc-MDP scintimetry. The results showed that the sequential variations in the nuclide uptake ratio of the femoral heads could reflect the histological repairing process and metabolic level in the femoral head. A sustainedly high nuclide uptake in 12 months after operation often denotes that there are defects in the repairing process of the ischemic femoral head, and that the segmental collapse of the femoral head will ensue.

Two cases of critical nosocomial pneumonia caused by flavobacterium meningosepticum (FM) were reported and both of them were successfully cured. There were 2 other cases of FM pneumonia reported in Chinese literature previously, but none of them survived. It has been found that the treatment for FM respiratory infection was very difficult because of its resistance to majority of antibiotics, including the third generation cephalosporins. The symptoms of FM pneumonia are similar to those of other gram-negative bacillus pneumonias, such as Klebsiella pneumoniae pneumonia. Definite diagnosis depends principally on etiological examination and clinical manifestations of pneumonia. Cefoperazone, Cefsulodin, Astreonam and Ciprofloxacin are valuable drugs in saving the lives of patients with FM Pneumonia.

Necrosis of the head of the femur with segmentary collapse after transcervical fractures occurs in 30 to 50% of the cases. Diagnosis of avascular necrosis should be made early before the head collapses. In a clinical and experimental study, we attempted to perform early screening for this complication. We studied 41 fractures of the neck of the femur with technitium scintigraphy with intravenous injections and compared uptake in pathological and normal heads. In the first group of 20 cases of fresh fractures, scintigraphy was performed before operation then repeated every three months to the 36th months later. High uptake we seen in 12 of the 20 cases then returned to normal 12 to 18 months later. Consolidation occurred in all 12 without necrosis. In 8 cases uptake was low initially then rose greatly and remained higher than normal. Necrosis occurred in 6 of these 8 cases. In a second group of 21 patients with a history of necrosis of the head of the femur, uptake was high. Pathologic examination of the surgical specimen after resection for prosthesis showed cell growth around the zone of necrosis. In conclusion, this technique, when possible, would appear to be highly useful to screen for necrosis early.

The multipin peptide synthesis technique, a method for simultaneous multiple peptide synthesis, was developed for large-scale screening of oligopeptides [Geysen et al. (1984) Proc. Natl. Acad. Sci. USA, 81, 3998-4002]. A modification of the technique allows the peptides assembled on polyethylene pins to be cleaved in their native amide form and reconstituted into physiologically compatible solutions. In this study, the suitability of these peptides for quantitative receptor binding assay was evaluated. Substance P and 18 analogs, including a set of N-terminal truncated substance P and a set of naturally occurring substance P analogs, were synthesized by the multipin methods. An average yield of 20 +/- 3 nmol of peptide per pin was obtained. The purity of the peptides was estimated to be ca. 90%. The binding activities of these peptides were determined in a competition assay against 125I-BHSP binding to a rat brain synaptosome preparation. The rank order of the affinities of these peptides depicted a typical pharmacological profile of central NK1 receptor. The IC50 values obtained were also in good agreement with data reported by other groups using similar experimental conditions, except that bulk synthesized peptides were used. This study demonstrates that the peptides synthesized with the multipin technique are suitable for quantitative receptor studies, particularly for a high-volume screening of bioactive peptides.

35. [A biomechanical study in the repairing process of avascular necrosis of the femoral head in dogs]

We report the result of an animal experiment studying the biomechanical, radiological and histological changes during the repairing process of avascular necrosis of the femoral head. At 1/2 month, histology showed complete necrotic changes involving both marrow content and bone trabecula. However, biomechanical tests showed there was no significant difference in material properties between experimental and control sides. A2 2 and 4 months, reparative changes were observed by histology and radiography. The decrease in mean compressive strength in the subchondral bone was 17.5% at 2 months and 35.5% at 4 months. Subchondral bone mean modulus of elasticity decreased by 25.1% at 2 months, 43.0% at 4 months. Greater decrease in mean strain energy in the cancellous bone occurred at 2 months (43.2%) and 4 months (42.9%). In the 6 months group, all necrotic femoral heads showed varying degrees of collapse. The compressive strength and the modulus in the subchondral bone decreased by 73.3% and 76.9% respectively. The strain energy in the cancellous bone decreased by 37.9%. The present study indicates that the biological resorptive cativity of the advancing reparative processes may lead to significant decrease in the mechanical properties. This is probably the most direct cause of late femoral head collapse.

In recent 5 years, 33 cases of tracheobronchial benign lesions have been treated with the method of Nd-YAG laser via bronchoscope. The follow-up survey has shown that the curative ratio reached 66.7 percent and total effective rate came up to 78.8 percent. There was significant relationship between the therapeutic effectiveness and types of lesions (P0.01). It had higher cure rate in benign tumour, tuberculous granuloma and foreign body's granuloma than that of inflammatory granuloma and amyloidosis. This method is a new and effective one for treating benign pathologic changes in air passage. The indication and prevention of complications have been discussed.

Cell surface expression of the T-cell receptor (TCR)/CD3 complex on the cells from 11 acute type adult T-cell leukemia (ATL) and 4 lymphoma type ATL patients was examined by flow cytometry. Cells from 10 of 11 acute ATL patients were TCR alpha beta+ and CD3+, and their mean fluorescence intensities were low (TCR alpha beta, 25.3-84.6; CD3, 22.8-87.8). Cells from two of four lymphoma type ATL did not express this complex, and the other two were CD3+, TCR alpha beta-. In contrast, the mean fluorescence intensity of the TCR/CD3 complex in cells from a patient with T4 chronic lymphocytic leukemia was not low (TCR alpha beta, 129.9; CD3, 117.1). mRNA expressions of the TCR alpha, beta, and CD3 gamma, delta, epsilon, and zeta chains were examined by Northern blots. ATL cells from two acute and two lymphoma types expressed amounts of this complex equal to or greater than those expressed by T4 chronic lymphocytic leukemia. CD3 delta and TCR beta mRNA in ATL and T4 chronic lymphocytic leukemia cells were equally stable to actinomycin D treatment. The synthesis of CD3 zeta protein by ATL cells was detected by Western blotting assay. On the basis of these findings, we discuss the possible involvement of the TCR/CD3 complex in activation of ATL cells.

A prognostic model was developed by examination of the prospectively recorded presentation characteristics of patients presenting with low grade non-Hodgkin's lymphomas (LGL). A geographically distinct group (Edinburgh and Borders) was excluded in order to test the validity of the model based on the rest of the Scotland and Newcastle Lymphoma Group (SNLG) population. Between 1979 and 1987 506 patients presented with low grade lymphoma according to Working Formulation definition within one of each of the three main pathology categories. The median available follow-up was 47 months. In a multivariate analysis performance status, age, stage, gender and haemoglobin all contributed separately. 25% of all patients had relatively good prognosis with a median survival not reached and 84% alive at 5 years. Conversely for the worst prognostic group of 25% of patients the median survival is 18 months with only 26% surviving at 5 years. Finally for the intermediate group of 50% of all patients, median survival is 69 months with 58% alive at 5 years. These striking prognostic separations are shown also to be valid in sub-group analysis. Thus young patients within any pathology subgroup have been identified with bad prognostic lymphoma for whom novel strategies in therapy should be devised.

Using simultaneous multiple peptide synthesis by the multipin approach, the feasibility of systematic large-scale pharmacological screening of peptide ligands was investigated. The method involves the assembly of small quantities of peptides (ca. 50 nmol) on plastic pins derivatized with an ester linker based on glycolate and 4-(hydroxymethyl)benzoate. These esters are stable under peptide synthesis and side-chain deprotection conditions but cleave under relatively mild basic conditions to generate peptides having C-terminal acid, amide and methylamide. A two-step approach to side-chain deprotection and cleavage from the solid support allows potentially toxic reagents to be removed (washed) from the peptide prior to cleavage. Consequently, the resulting peptide solutions can be used in bioassays with minimal processing. A series of angiotensin II and substance P analogs were synthesized and evaluated in an in vivo rat model and in vitro radioreceptor assay, respectively. Structure-activity studies on analogs of these bioactive peptides are well documented. The data obtained were consistent with that reported in the literature.

For study of tumor metastatic mechanism, a mouse uterine cervix cancer cell line (MUCC), as derived from a transplantable mouse uterine cervix cancer No. 27 (U 27) with metastatic capacities through lymphatics and blood, was established in vitro. It has been successfully maintained for more than 24 months through more than 100 passages. MUCC cells are epithelioid with definite malignant morphological features. The doubling time of the cell population is 27.8 hours. The average mitotic index is 30.6%, with a maximum of 58.9%. Chromosomes show aberration in number and structure. The homotransplant rate is 75% (9/12). The solid tumor, produced by homotransplant, is similar to the tumor U 27. Metastases are present in the lymph nodes and lungs in these 9 animal models. It is evident that this cell line retains the capacity of inducing daughter tumor lines with spontaneous lymphatic and blood metastases. MUCC cell line might be very useful in isolating subpopulations which possess the different metastatic abilities and the study of metastatic mechanism.

Hearing determination of 51 cases had been made to observe and explore the TCM pathogenic relation between the collateral channels of Shen and ears. Model 51A-T72N pure tone auditional determinator was used. For each case, routine auditional determination was made after rest in bed quietly for an hour after breakfast. The determinating results were divided into normal (average hearing loss less than 15dB), low frequency, high frequency and plain-typing hearing loss.Among the 51 cases, 8 cases were normal hearing (15.7%), the hearing loss cases were 43 (84.3%); 30 cases were double-ear hearing loss (58.8%); 13 cases were single ear hearing loss (25.5%). Cases of renal diseases frequently occurred among the young (in this group, average age was 29.5 years). Such high incidence was obviously non-physiological, but related to the renal disease itself. More serious hearing damage were seen and were in high frequency. Whether or not it was caused by hypoalbuminaemia, oedema and deprivation in blood vessel leading to disorder of circulation of sound sensitizen, or accumulation of toxin owing to renal dysfunction, or changing composition of endolymph influencing the hearing bristle cells, further research should be done in the near future.
