Purpose: The last article on causes of sight impairment (SI) in England and Wales was for April 2007-March 2008. This report updates these figures for April 2012-March 2013., Methods: In England and Wales, registration for SI is initiated by completion of a certificate of vision impairment (CVI). The main cause of visual impairment was ascertained for certificates completed April 2012-March 2013. A proportional comparison against April 2007-March 2008 was made., Results: We received 24 009 CVIs of which 10 410 were for severe sight impairment (SSI) and 13 129 were for SI. These numbers were slightly higher than those observed in April 2007-March 2008 (9823 SSI; 12 607 SI). The ratio SI:SSI has remained static with 55% of all certifications being SI. The proportion of certificates without a single main cause has fallen slightly (16.6 to 14%). The proportion of certificates with a main cause of degeneration of the macula and posterior pole (mostly age-related macular degeneration (AMD)) decreased from 58.6 to 50% SSI and from 57.2 to 52.5% SI. Glaucoma remains the second most common cause (11% SSI; 7.6% SI) but hereditary retinal disorders overtook diabetes as third leading cause of SSI., Conclusion: AMD is still by far the leading cause of certifications for sight impairment in England and Wales (both SI and SSI). Proportionate changes have been observed since 2008, but it is important to note that a proportionate increase in one condition will impact on others.

Objectives: The Sight Loss and Vision Priority Setting Partnership aimed to identify research priorities relating to sight loss and vision through consultation with patients, carers and clinicians. These priorities can be used to inform funding bodies' decisions and enhance the case for additional research funding., Design: Prospective survey with support from the James Lind Alliance., Setting: UK-wide National Health Service (NHS) and non-NHS., Participants: Patients, carers and eye health professionals. Academic researchers were excluded solely from the prioritisation process. The survey was disseminated by patient groups, professional bodies, at conferences and through the media, and was available for completion online, by phone, by post and by alternative formats (Braille and audio)., Outcome Measure: People were asked to submit the questions about prevention, diagnosis and treatment of sight loss and eye conditions that they most wanted to see answered by research. Returned survey questions were reviewed by a data assessment group. Priorities were established across eye disease categories at final workshops., Results: 2220 people responded generating 4461 submissions. Sixty-five per cent of respondents had sight loss and/or an eye condition. Following initial data analysis, 686 submissions remained which were circulated for interim prioritisation (excluding cataract and ocular cancer questions) to 446 patients/carers and 218 professionals. The remaining 346 questions were discussed at final prioritisation workshops to reach agreement of top questions per category., Conclusions: The exercise engaged a diverse community of stakeholders generating a wide range of conditions and research questions. Background: Asians from the Indian Subcontinent form the largest ethnic minority in the United Kingdom. Data on the prevalence of visually-impairing eye conditions in this population are vital for planning eye health care services., Materials and Methods: This survey was based in the two London boroughs with the largest Asian populations. Subjects originating from the Indian Subcontinent were identified from GP practice records. All subjects were asked about demographic details and were given a full ophthalmological examination. The severity of cataract, glaucoma, diabetic retinopathy, and age-related maculopathy was recorded. Blindness was defined as logMAR visual acuity of 0.99 (Snellen equivalence 20/200 in the better eye) or worse, 'low vision' was defined as Snellen equivalence of 20/63 or worse (logMAR 0.5 or higher), and visual impairment was defined as visual acuity worse than 20/40., Results: The median age was 56 years. Two hundred and eighty four subjects did not attend for eye examination. Of the 922 examined, 128 subjects (13.9%) were 'visually impaired,' 39 (4.2%) had 'low vision,' and 6 (0.7%) were bilaterally blind. The overall prevalence of cataract, open-angle glaucoma, age-related macular degeneration, and diabetic retinopathy were 77%, 1.0%, 8.7%, and 8.8%, respectively., Conclusion: Visual impairment rates amongst Asians seem to be similar to Caucasian populations in the UK. The prevalence of cataract and diabetic retinopathy is higher, while the risk of ARMD and OAG are comparable. In view of the high cataract prevalence, a more detailed assessment of the visual profile and factors limiting healthcare accessibility in this community are needed.

We investigated the relationship between visual acuity, early growth and eye disease in a retrospective cohort study of 700 individuals in North Hertfordshire. Records of birth weight and weight at one year were used to determine early growth. We measured visual acuity and age-related eye diseases using standard instruments in those same individuals (now aged 63 to 73). Visual acuity below the legal threshold for driving in the UK (6/11 or poorer) was present in 13% of subjects. There was no clear association between birth weight or weight at one year and visual acuity. Vision impairment was found to be associated with refractive error, cataract, age-related maculopathy, and elevated macular threshold. After controlling for the effects of eye disease, increasing age remained a significant predictor of poorer visual acuity.

A pilot study was carried out to determine the prevalence of ophthalmic disease in the Indian community of Southall and to ascertain the best methods applicable for a larger formal study. Three sites were chosen for the study, a Sikh gurdwara, a mosque and a Hindu temple. The subjects were volunteers aged 30 years and over who had visited the appropriate place of worship at least twice in the previous month. A total of 184 subjects were examined. The prevalence of blindness was 2.7% by the World Health Organization (WHO) criteria, while 9.8% had uniocular blindness. The prevalence of glaucoma and ocular hypertension was 2.7% and 7%, respectively. Of the 184 subjects examined, 58% had cataract and 3.8% had age-related maculopathy. The prevalence of visually disabling trachomatous eye disease was 9.7%. The prevalence of diabetes mellitus was 17.9%, and that of hypertension 22.8%. This small study suggests that people with origins from the Indian subcontinent have a higher prevalence of ophthalmic disease than the Caucasian population.

A randomly selected sample of subjects over 75 years old or housebound in three London inner city general practices were screened for eye disease by an ophthalmologist and an ophthalmic trained nurse. All subjects were examined at specialist outreach clinics run at the surgery of their general practitioner (GP), except for the housebound who were assessed by domiciliary visit. Patients presenting to their GP with an eye problem during the study were also seen at the outreach clinic at the GP's request. Over the 3-month period of the study, 126 over-75s, 62 housebound and 35 GP referrals were seen. This pilot study found high prevalence rates of treatable eye disease in the elderly and housebound subjects and these are compared with the findings of other epidemiological surveys. The needs for health care provision to this sector of the community and the feasibility of providing it through outreach clinics are also discussed.

Objective: To determine the prevalence of visual disability and common eye disease among elderly people in inner London., Design: Cross sectional random sample survey., Setting: Inner London health centre., Subjects: Random sample of people aged 65 and over taken from practice's computerised age-sex register., Main Outcome Measures: Presenting binocular Snellen 6 m distance acuity and best monocular 3 m Sonksen-Silver acuity to classify prevalence of blindness by World Health Organisation criteria (less than 3/60 in better eye) and American criteria for legal blindness (better eye equal to 6/60 or less) and of low vision by WHO criteria (best acuity 6/18) and visual impairment by American criteria (less than 6/12 or 20/40 but greater than 6/60 or 20/200 in better eye). Principal cause of visual loss by diagnosis, referral indication by cause to hospital eye service, and proportion of cases known to primary care., Results: 207 of 288 (72%) eligible people were examined. 17 (8%) housebound subjects were examined at home. The prevalence of blindness was 1% by WHO criteria and 3.9% by American criteria. The prevalence of low vision (WHO criteria) was 7.7%. The prevalence of visual impairment (American criteria) was 10.6%. Cataract accounted for 75% of cases of low vision. Only eight out of 16 patients with low vision were known by their general practitioner to have an eye problem. 56 subjects (27%) would probably have benefited from refraction. Comparisons with studies in the United States and Finland suggested higher rates in this sample, mainly due to the prevalence of disabling cataract., Conclusion: There seems to be a considerable amount of undetected ocular disease in elderly people in the community.

Aims: To study trends in the prevalence of being treated for glaucoma and ocular hypertension from 1994 to 2003, and to examine factors determining treatment in 2002. Methods: Computerised data (the DIN-LINK database) from 131 general practices across the United Kingdom, in which half a million patients aged 40 years or more were registered annually, were used. On average 10 000 patients were treated for glaucoma and ocular hypertension annually. Results: Prevalence of being treated for glaucoma and ocular hypertension increased from 1.7% in 1994 to 2.3% in 2003. Those aged 85 years or more were 13 times (95% Cl 12.2 to 13.8) more likely to be treated than those aged 40–64 years. Men were more likely to be treated than women (OR 1.24, 95% Cl 1.19 to 1.28). Subjects ‘hard pressed’ were less likely to be treated than ‘wealthy achievers’ (OR 0.92, 95% CI 0.86 to 0.99). While use of topical 13 blocker only medications has declined since 1995, use of topical prostaglandins and combination therapies has increased. In 2003, use of prostaglandins overtook 13 blocker only medications. Conclusion: Prevalence of being treated for glaucoma has increased over time, and rises with age. Differences in treatment by sex and social status could be explained by use of or access to health care or by underlying prevalence of disease. Aims: To assess the pattern of use of antimetabolites in trobeculectomy surgery by all consultant ophthalmologists in the United Kingdom. Methods: A postal questionnaire of 12 questions regarding antimetabolite use in trabeculectomy surgery was sent to all 749 consultant ophthalmologists in the United Kingdom. The consultants were asked to estimate the number of trabeculectomies they performed per year, how often they used 5- Huorouracil (5-FU) in primary and redo surgery, their usual method of administration of 5-FU, how often they used mitomycin (MMC) in primary and redo surgery, and their usual dosage regimen of MMC. Factors that influenced the decision to use or not use ontimetabolites were also assessed. Results: The response rate of consultants returning the questionnaire was 82% (615 out of 749); 87% (533) of these consultants perform trabeculectomy surgery. Of these 533 consultants, 98 (18%) never use an antimetabolite. Most consultants (82%) use antimetabolites, but use them infrequently (only 9% using antimetabolites in more than half their cases). The preferred antimetabolite is 5-FU rather than MMC. Of the 435 consultants performing trabeculectomy surgery and using antimetabolites, 402 (93%) use 5-FU and 179 (41%) use MMC. Various factors influenced the decision to use or not use an antimetabolite, but experience of complications associated with their use was a factor for 34% of consultants. Conclusion: The use of antimetabolites, particularly MMC, in the United Kingdom is much less than in America or Japan, where trabeculectomy with MMC is the surgical procedure preferred by glaucoma specialists. Purpose The Drusen Laser Study (DLS) of high-risk age-related maculopathy (ARM) is a randomised, controlled clinical trial designed to answer two questions: (1) Do drusen resolve after macular laser photocoagulation (2) Does macular laser photocoagulation prevent choroidal neovascularisation (CNV) in high-risk eyes? In this report, we present the results of the interim, pooled analysis of CNV prophylaxis for patients in the Unilateral Group of the DLS. Methods The DLS is a randomised controlled clinical trial of prophylactic macular photocoagulation for high-risk ARM. Patients in the Unilateral Group had a neovascular complication in the first eye; their fellow eye (Study Eye) had visual acuity of 6/12 or better and drusen. Following informed consent, patients were randomised to the Treatment Group or the No Treatment Group. Patients randomised to treatment received 12 light spots of argon laser photocoagulation to their Study Eye: four burns were placed 750 lm from the centre of the fovea at 12, 3, 6, and 9 o'clock, and the eight remaining burns were placed 1500 lm from the centre of the fovea at 12, 1:30, 3, 4:30, 6, 7:30, 9, and 10:30 o'clock. Drusen were treated directly only if they were present at the protocol treatment locations. All patients were followed in an identical fashion at regular intervals. Best-corrected visual acuity was measured and recorded by a masked observer. Fluorescein angiography was performed at baseline and yearly review, as well as nonprotocol visits if symptoms suggested CNV. Five clinical centres utilised and conformed to a common DLS protocol. Patient care and data collection methodologies were deemed sufficiently similar to permit a pooled data analysis. Results There were 156 patients included in the interim analysis, and timed information was available on 153. The author reflects on the National Health Service (NHS) Bill, which deals with free health and dental care. The author says that optometric services must be included to the bill specifically in hospitals that contain eye departments and clinics. He argues that the inclusion of eye-health services in the bill will result to high costs. He mentions that integration of eye-care was neglected in the NHS Act of 1951, thus, NHS Diabetic Eye Screening Program was implemented.

The authors reflect on the involvement of patients of eye disease or their so-called "consumers" in eye health research. The authors say that many governments as well as the World Health Organization (WHO) endorse patient and public involvement. Moreover, the authors relate that they give their patients significant information on the benefits and dangers of treatment, for the patients to have evidence-based choices.
