Studies suggest that acute pesticide poisonings (APP) may be linked with long-term neurological effects. To examine long-term neurological and psychological distress symptoms associated with having experienced an APP, we conducted a cross-sectional study of 300 conventional and organic smallholder farmers from Zarcero County, Costa Rica, May–August 2016. We collected self-reported data on sociodemographic characteristics, occupational history, pesticide exposure, APPs, neurological and psychological distress symptoms (using the Brief Symptom Inventory (BSI)). Adjusted logistic regression models were fit. A total of 14% of the farmers (98% male) reported experiencing at least one APP during their lifetime. Self-reported APP was associated with neurological symptoms during the 12 months prior to interview (e.g., fainting (Odds Ratio: 7.48, 95% Confidence Interval: 1.83, 30.74), shaking hands (3.50, 1.60, 7.60), numbness/tingling in hands or feet (3.23, 1.66, 6.32), insomnia (2.53, 1.34, 4.79), accelerated heartrate (2.42, 1.03, 5.47), dizziness (2.38, 1.19, 4.72), increased irritability/anger (2.37, 1.23, 4.55), low energy (2.33, 1.23, 4.46), and difficulty concentrating (2.01, 1.05, 3.85)). Farmers who reported an APP in the ten years prior to interview experienced increased odds of abnormal BSI scores for hostility (4.51, 1.16, 17.70) and paranoid ideation (3.76, 0.99, 18.18). Having experienced an APP may be associated with long-term neurological and psychological distress symptoms.

Studies from India and several eastern African countries found that the impact of dairy animal ownership on household nutrition varied greatly, depending on the socio-geographic context. The purpose of this study was to examine the association between livestock ownership and household dietary quality in rural Kolar district, India. We collected data from a household survey in four study villages (n = all 195 households of the four villages) of Kolar district, applying a cross-sectional design. Kendall’s rank correlation coefficient was employed to determine the correlation between milk consumption and other dietary variables. Multivariable logistic regression was used to describe the relationship between dairy animal ownership and household milk consumption. Households owning dairy animals more often had access to irrigation (58.3% vs. 25.2%) and were less often woman-headed (2.4% vs. 22.5%). Household milk consumption was significantly correlated with consumption of vegetable variety, egg, and meat (all p-values &lt, 0.05). After adjusting for multiple confounders, the odds ratio of milk consumption between dairy animal-owning households as compared to other households was 2.11 (95% confidence interval 0.85, 5.45). While dairy animal ownership was found to be associated with improved dietary quality, larger households were in a better position to adopt dairy animals, which, in turn, might contribute to better household nutrition.

The application of a health impact assessment (HIA) for a large-scale copper mining project in rural Zambia triggered the long-term monitoring and evaluation of determinants of health and health outcomes in communities living in proximity to the mine. Three consecutive cross-sectional surveys were conducted at intervals of four years, thus, at baseline (2011), four (2015) and eight (2019) years into the project&rsquo, s development. Using the same field and laboratory procedures, the surveys allowed for determining changes in health indicators at the household level, in young children (&lt, 5 years), school attendees (9&ndash, 14 years) and women (15&ndash, 49 years). Results were compared between communities considered impacted by the project and communities outside the project area (comparison communities). The prevalence of Plasmodium falciparum infection increased in both the impacted and comparison communities between 2011 and 2019 but remained consistently lower in the impacted communities. Stunting in children &lt, 5 years and the prevalence of intestinal parasite infections in children aged 9&ndash, 14 years mostly decreased. In women of reproductive age, selected health indicators (i.e., anaemia, syphilis, underweight and place of delivery) either remained stable or improved. Impacted communities generally showed better health outcomes than comparison communities, suggesting that the health interventions implemented by the project as a consequence of the HIA have mitigated potential negative effects and enhanced positive effects. Caution is indicated to avoid promotion of health inequalities within and beyond the project area.

Background Natural resource extraction projects can have positive but also negative effects on the health of affected communities, governed by demographic, economic, environmental, physical and social changes. Negative effects often prevail and these might widen existing health inequities. Health impact assessment (HIA) is a decision-support tool that aims at maximizing benefits and minimizing negative impacts on people's health. A core value of HIA is equity; yet, little is known about health equity in the frame of HIA, particularly in sub-Saharan Africa. Methodology We conducted a scoping review to determine whether and to what extent HIA in sub-Saharan Africa addresses health equity. We included peer-reviewed publications and guidelines pertaining to HIA, environmental impact assessment (EIA) and social impact assessment (SIA). Health equity was investigated by identifying (i) how health considerations were addressed and (ii) whether health was stratified by subgroups of the community. Results Out of 1′640 raw hits, we identified 62 articles (16 HIA, 36 EIA, one SIA and nine integrated assessments), 32 of which specifically addressed health. While 20 articles focused on a specific health topic, 12 articles used a more comprehensive approach to address health. In 15 articles there were specific subgroup analyses (e.g. mothers, children or marginalized groups) as a measure of health equity. Another 12 papers referred to the community in a more general way (e.g. affected). Without exception, health was an integral part of the nine included guidelines. HIA guidelines addressed health systematically through environmental health areas, risk assessment matrix or key performance indicators. Conclusions We found evidence that previously conducted HIA in sub-Saharan Africa and current guidelines address health equity. However, there is a need to stratify community subgroups more systematically in order to determine health differentials better. In rural settings of Côte d’Ivoire, access to water, sanitation, and hygiene (WASH) at schools is often lacking. The purpose of this study was to assess the availability, quality, and use of WASH infrastructure in schools in the south-central part of Côte d’Ivoire, and to determine the hygiene practices of schoolchildren. A cross-sectional study was conducted in 20 primary schools with (n = 10) or without (n = 10) direct access to drinking water. The survey was comprised of a questionnaire directed at schoolchildren aged 8–17 years, an assessment of the WASH infrastructure, and the testing of drinking water samples for Escherichia coli and total coliforms. Overall, 771 schoolchildren were enrolled in the study. One out of four children (24.9%) reported that they used available toilets. Among those children not using toilets, more than half (57.5%) reported that they practised open defecation. Drinking water infrastructure was limited in most schools because of poor storage tanks, the low flow of water, or broken wells. All drinking water samples (n = 18) tested positive for total coliforms and 15 (83.3%) tested positive for E. coli. The lack of WASH infrastructures in primary schools in the south-central part of Côte d’Ivoire, in combination with poor hygiene practices, might govern disease transmission and absenteeism at school, especially among females.

Background Poor housing conditions, such as poor building materials and weak structures as well as high levels of indoor air pollution, are important risk factors for a broad range of diseases, including acute respiratory infections (ARI). In mining areas, research on the determinants of respiratory health predominantly focuses on exposures to outdoor air pollutants deriving from mining operations. However, mining projects also influence the socioeconomic status of households, which, in turn, affect housing quality and individual behaviors and, thus, housing quality and levels of indoor air pollution. In this study, we aimed to determine how proximity to an industrial mining project impacts housing quality, sources of indoor air pollution, and prevalence of ARI. Methods We merged data from 131 Demographic and Health Surveys (DHS) with georeferenced data on mining projects in sub-Saharan Africa (SSA) to determine associations between housing quality, indoor air pollution sources, and child respiratory health. Spatial differences in selected indicators were explored using descriptive cross-sectional analyses. Furthermore, we applied a quasi-experimental difference-in-differences (DiD) approach using generalized linear mixed-effects models to compare temporal changes in household and child health indicators at different operational phases of mining projects and as a function of distance to mines. Results For cross-sectional analyses, data of 183,466 households and 141,384 children from 27 countries in SSA were used, while 41,648 households and 34,406 children from 23 SSA countries were included in the DiD analyses. The increase in the share of houses being built from finished building materials after mine opening was more than 4-fold higher (odds ratio (OR): 4.32, 95% confidence interval (CI): 2.98–6.24) in close proximity to mining sites (i.e., ≤ 10 km) compared to areas further away (i.e., 10–50 km). However, these benefits were not equally distributed across socioeconomic strata, with considerably weaker effects observed among poorer households. Increases in indoor tobacco smoking rates in close proximity to operating mines were twice as high as in comparison areas (OR: 2.06, 95% CI: 1.15–3.68). The cross-sectional analyses revealed that traditional cooking fuels (e.g., charcoal, dung, and wood) were less frequently used (OR: 0.27, 95% CI: 0.23–0.31) in areas located in close proximity to mines than in comparison areas. Overall, no statistically significant association between mining operations and the prevalence of symptoms related to ARI in children under the age of 5 years was observed (OR: 0.78, 95% CI: 0.29–2.07). Conclusions Mines impact known risk factors for ARI through diverse pathways. The absence of significant changes in ARI symptoms among children is likely the result of counteracting effects between improvements in housing infrastructure and increased exposures to air pollutants from outdoor sources and tobacco smoking. For mining projects to unfold their full potential for community development, we recommend that impact assessments move beyond the mere appraisal of mining-related pollution emissions and try to include a more comprehensive set of pathways through which mines can affect ARI in exposed communities.

BACKGROUND: Pesticide use is increasing in low- and middle-income countries (LMICs) including Costa Rica. This increase poses health risks to farm owners, farm workers, and communities living near agricultural farms. OBJECTIVE: We aimed to examine the health effects associated with occupational pesticide exposure in farm owners and workers from conventional and organic smallholder farms in Costa Rica. METHODS: We conducted a cross-sectional study involving 300 owners and workers from organic and conventional horticultural smallholder farms in Zarcero County, Costa Rica. During the baseline study visit, we administered a structured, tablet-based questionnaire to collect data on sociodemographic characteristics, pesticide exposure, and health conditions (eg, respiratory and allergic outcomes and acute pesticide intoxication symptoms) and administered a neurobehavioral test battery (eg, Finger Tapping Test and Purdue Pegboard); we measured blood pressure, anthropometry (height, weight, and waist circumference), and erythrocytic acetylcholinesterase activity and also collected urine samples. In addition, a functional neuroimaging assessment using near-infrared spectroscopy was conducted with a subset of 50 study participants. During the follow-up study visit (~2-4 weeks after the baseline), we administered participants a short questionnaire on recent pesticide exposure and farming practices and collected hair, toenail, and urine samples. Urine samples will be analyzed for various pesticide metabolites, whereas toenails and hair will be analyzed for manganese (Mn), a biomarker of exposure to Mn-containing fungicides. Self-reported pesticide exposure data will be used to develop exposure intensity scores using an exposure algorithm. Furthermore, exposure-outcome associations will be examined using linear and logistic mixed-effects regression models. RESULTS: Fieldwork for our study was conducted between May 2016 and August 2016. In total, 113 farm owners and 187 workers from 9 organic and 83 conventional horticultural smallholder farms were enrolled. Data analyses are ongoing and expected to be published between 2019 and 2020. CONCLUSIONS: This study is one of the first to examine differences in health effects due to pesticide exposure between farm owners and workers from organic and conventional smallholder farms in an LMIC. We expect that this study will provide critical data on farming practices, exposure pathways, and how occupational exposure to pesticides may affect farm owners and workers' health. Finally, we hope that this study will allow us to identify strategies to reduce pesticide exposure in farm owners and workers and will potentially lay the groundwork for a future longitudinal study of health outcomes in farm owners and workers exposed to pesticides. The artisanal and small-scale gold mining (ASGM) sector in Burkina Faso increasingly employs cyanide leaching to exploit the natural resource gold. The long-term consequences of chronic cyanide exposure in humans have yet to be fully elucidated. Meanwhile, the local health and development actors are facing immediate, unfamiliar challenges. The aim of this study was to deepen the understanding of cyanide exposure pathways, potential human exposure and associated health effects in ASGM communities in order to tailor health promotion and sustainable development recommendations for local actors. In April and May 2017, a cross-sectional epidemiological study was carried out in three ASGM sites in Burkina Faso. Three exposure groups were included: (i) miners using cyanide; (ii) miners who do not use cyanide; and (iii) ASGM community members not directly involved in mining. A questionnaire was administered to all study participants, followed by a finger prick blood sample to determine blood lactate level as a proxy indicator for cyanide exposure. Overall, 279 individuals participated (99 cyanide-using miners, 90 non-cyanide using miners and 90 community members not involved in mining activities). Mean blood lactate levels were significantly higher in miners using cyanide (4.7 mmol/L, 95% confidence interval (CI) 3.8–5.6 mmol/L), compared to non-cyanide using miners (3.4 mmol/L, 95% CI 2.9–3.7 mmol/L) and other community members not involved in mining activities (2.8 mmol/L, 95% CI 2.4–3.2 mmol/L). Multiple linear regression found loss of short-term memory reported by participants associated with higher blood lactate level. In view of our findings, we conclude that the use of cyanide in Burkina Faso’s ASGM sector is associated with potential negative health effects. Hence, specific public health interventions are warranted, such as raising awareness among miners of the risks of cyanide use, promotion of protective measures and strengthening of health sector capacities to recognize and manage mining-related health issues and law enforcement.

Background Large projects in the extractive industry sector can affect people’s health and wellbeing. In low- and middle-income countries (LMICs), women’s health is of particular concern in such contexts due to potential educational and economic disadvantages, vulnerability to transactional sex and unsafe sex practices. At the same time, community health interventions and development initiatives present opportunities for women's and maternal health. Methods Within the frame of the health impact assessment (HIA) of the Trident copper mining project in Zambia, two health surveys were conducted (baseline in 2011 and follow-up in 2015) in order to monitor health and health-related indicators. Emphasis was placed on women residing in the mining area and, for comparison, in settings not impacted by the project. Results All measured indicators improved over time, regardless of whether communities were affected by the project or not. Additionally, the percentage of mothers giving birth in a health facility, the percentage of women who acknowledge that HIV cannot be transmitted by witchcraft or other supernatural means and the percentage of women having ever tested for HIV showed a significant increase in the impacted sites but not in the comparison communities. In 2015, better health, behavioural and knowledge outcomes in women were associated with employment by the project (or a sub-contractor thereof), migration background, increased wealth and higher educational attainment. Conclusions Our study reveals that natural resource development projects can positively impact women’s health, particularly if health risks are adequately anticipated and managed. Hence, the conduct of a comprehensive HIA should be a requirement at the feasibility stage of any large infrastructure project, particularly in LMICs. Continued monitoring of health outcomes and wider determinants of health after the initial assessment is crucial to judge the project’s influence on health and for reducing inequalities over time. Large private sector investments in low- and middle-income countries are often critically evaluated with regards to their environmental, social, human rights, and health impacts. A health impact assessment, including a baseline health survey, was commissioned by the Addax Bioenergy Sierra Leone project in 2010. As part of the monitoring, a follow-up survey was conducted three years later. A set of health indicators was assessed at six impacted and two control sites. Most of these indices improved, particularly at the impacted sites. The prevalences of stunting, wasting, and Plasmodium falciparum in children under five years of age decreased significantly at impacted sites (all p &lt, 0.05) and non-significantly at control sites. Anemia in children and in women of reproductive age (15–49 years) decreased significantly at impacted and control sites (p &lt, 0.05 and p &lt, 0.001, respectively). Health facility-based deliveries increased significantly at the impacted sites (p &lt, 0.05). The prevalences of helminth infections in children aged 10–15 years remained approximately at the same levels, although focal increases at the impacted sites were noted. Access to improved sanitation decreased significantly (p &lt, 0.05) at control and non-significantly at impacted sites. Water quality remained poor without significant changes. The epidemiologic monitoring of a bioenergy project provides a useful contribution for evidence-based decision-making.

BACKGROUND: There is a paucity of epidemiological data pertaining to schistosomiasis and soil-transmitted helminth (STH) infections in the Democratic Republic of the Congo (DRC). METHODS: A cross-sectional survey was carried out in the north-eastern part of DRC enrolling 400 schoolchildren aged 9-14 years. Stool and urine samples were subjected to standard diagnostic methods and examined under a microscope for helminth eggs. RESULTS: Four out of five children were infected with at least one helminth species. Schistosoma mansoni was the predominant species (57.8%). Urine examinations were all negative for S. haematobium. CONCLUSIONS: S. mansoni and STH infections are highly endemic in the surveyed part of the DRC, calling for interventions in school-aged children.

We assessed the association between childhood diarrhoeal incidence and climatic factors in rural and urban settings in the health district of Mbour in western Senegal. We used monthly diarrhoeal case records among children under five years registered in 24 health facilities over a four-year period (2011–2014). Climatic data (i.e., daily temperature, night temperature and rainfall) for the same four-year period were obtained. We performed a negative binomial regression model to establish the relationship between monthly diarrhoeal incidence and climatic factors of the same and the previous month. There were two annual peaks in diarrhoeal incidence: one during the cold dry season and one during the rainy season. We observed a positive association between diarrhoeal incidence and high average temperature of 36 °C and above and high cumulative monthly rainfall at 57 mm and above. The association between diarrhoeal incidence and temperature was stronger in rural compared to urban settings, while higher rainfall was associated with higher diarrhoeal incidence in the urban settings. Concluding, this study identified significant health–climate interactions and calls for effective preventive measures in the health district of Mbour. Particular attention should be paid to urban settings where diarrhoea was most common in order to reduce the high incidence in the context of climatic variability, which is expected to increase in urban areas in the face of global warming.

Background To avoid or mitigate potential project-related adverse health effects, the Trident copper project in Kalumbila, northwestern Zambia, commissioned a health impact assessment. HIV was identified a priority health issue based on the local vulnerability to HIV transmission and experience from other mining projects in Africa. Hence, an HIV/AIDS management plan was developed, including community and workplace interventions, with HIV testing and counselling (HTC) being one of the key components. We present trends in HTC data over a 4-year period. Methods In 13 communities affected by the Trident project, HTC was implemented from 2012 onwards, using rapid diagnostic tests, accompanied by pre- and post-test counselling through trained personnel. In addition, HTC was initiated in the project workforce in 2013, coinciding with the launch of the mine development. HTC uptake and HIV positivity rates were assessed in the study population and linked to demographic factors using regression analysis. Results In total, 11,638 community members and 5564 workers have taken up HTC with an increase over time. The HIV positivity rate in the community was 3.0% in 2012 and 3.4% in 2015, while positivity rate in the workforce was 5.2% in 2013 and 4.3% in 2015. Females showed a significantly higher odds of having a positive test result than males (odds ratio (OR) = 1.96, 95% confidence interval (CI): 1.55–2.50 among women in the community and OR = 2.90, 95% CI: 1.74–4.84 among women in the workforce). HTC users in the 35–49 years age group were most affected by HIV, with an average positivity rate of 6.6% in the community sample and 7.9% in the workforce sample. These study groups had 4.50 and 4.95 higher odds of being positive, respectively, compared to their younger counterparts (15–24 years). Conclusions While HTC uptake increased five-fold in the community and almost three-fold in the workplace, the HIV positivity rates were insignificantly higher in 2015 compared to 2012. Our data can be used alongside other surveillance data to track HIV transmission in this specific context. Guided by the health impact assessment, the HIV prevention and control programme was readily adapted to the current setting through the identification of socioeconomic and environmental determinants of health. Background Diarrhoeal diseases remain an important cause of mortality and morbidity among children, particularly in low- and middle-income countries. In Senegal, diarrhoea is responsible for 15% of all deaths in children under the age of five and is the third leading cause of childhood deaths. For targeted planning and implementation of prevention strategies, a context-specific understanding of the determinants of diarrhoeal diseases is needed. The aim of this study was to identify risk factors of diarrhoeal diseases in children under the age of five in Mbour, Senegal. Methods Between February and March 2014, a cross-sectional survey was conducted in four zones of Mbour to estimate the burden of diarrhoeal diseases (i.e. diarrhoea episodes in the 2 weeks preceding the survey) and associated risk factors. The zones covered urban central, peri-central, north peripheral and south peripheral areas. Overall, 596 households were surveyed by a questionnaire, yielding information on sociodemographic, environmental and hygiene behavioural factors. Univariable and multivariable logistic regression analyses were used to identify risk factors associated with the occurrence of diarrhoea. Results The reported prevalence of diarrhoea among children under the age of five during the 2 weeks preceding the survey was 26%. Without adjustment, the highest diarrhoea prevalence rates were observed in the peri-central (44.8%) and urban central zones (36.3%). Multivariable regression revealed significant associations between diarrhoeal diseases and unemployment of mothers (adjusted odds ratio [aOR] = 1.62, 95% confidence interval [CI]: 1.18–2.23), use of open bags for storing household waste (aOR = 1.75, 95% CI: 1.00–3.02), evacuation of household waste in public streets (aOR = 2.07, 95% CI: 1.20–3.55), no treatment of stored drinking water (aOR = 1.69, 95% CI: 1.11–2.56) and use of shared toilets (aOR = 1.69, 95% CI: 1.11–2.56). Conclusion We found a high prevalence of diarrhoea in children under the age of five in Mbour, with the highest prevalence occurring in the central and peri-central areas. These findings underscore the need for public health interventions to alleviate the burden of diarrhoea among vulnerable groups. Promotion of solid waste disposal and reduction of wastewater exposure should be implemented without delay. Summary: The widespread application of functional neuroimaging within the field of environmental epidemiology has the potential to greatly enhance our understanding of how environmental toxicants affect brain function. Because many epidemiological studies take place in remote and frequently changing environments, it is necessary that the primary neuroimaging approach adopted by the epidemiology community be robust to many environments, easy to use, and, preferably, mobile. Here, we outline our use of functional near-infrared spectroscopy (fNIRS) to collect functional brain imaging data from Costa Rican farm workers enrolled in an epidemiological study on the health effects of chronic pesticide exposure. While couched in this perspective, we focus on the methodological considerations that are necessary to conduct a mobile fNIRS study in a diverse range of environments. Thus, this guide is intended to be generalizable to all research scenarios and projects in which fNIRS may be used to collect functional brain imaging data in epidemiological field surveys. https://doi.org/10.1289/EHP2049

The epidemiology of malaria, anaemia and malnutrition in children is potentially altered in mining development areas. In a copper extraction project in northwestern Zambia, a health impact assessment (HIA) was commissioned to predict, manage and monitor health impacts. Two cross-sectional surveys were conducted: at baseline prior to project development (2011) and at four years into development (2015). Prevalence of Plasmodium falciparum, anaemia and stunting were assessed in under-five-year-old children, while hookworm infection was assessed in children aged 9-14 years in communities impacted and comparison communities not impacted by the project. P. falciparum prevalence was significantly higher in 2015 compared to 2011 in both impacted and comparison communities (odds ratio (OR) = 2.51 and OR = 6.97, respectively). Stunting was significantly lower in 2015 in impacted communities only (OR = 0.63). Anaemia was slightly lower in 2015 compared to baseline in both impacted and comparison communities. Resettlement due to the project and migration background (i.e., moving into the area within the past five years) were generally associated with better health outcomes in 2015. We conclude that repeated cross-sectional surveys to monitor health in communities impacted by projects should become an integral part of HIA to deepen the understanding of changing patterns of health and support implementation of setting-specific public health measures.

As biofuel projects may be associated with positive and negative effects on people's health and wellbeing, a health impact assessment was performed for the Addax Bioenergy Sierra Leone (ABSL) project. We present data from the baseline health survey, which will provide a point of departure for future monitoring and evaluation activities.; In December 2010, a cross-sectional survey was carried out in eight potentially affected communities. A broad set of clinical and parasitological indicators were assessed using standardised, quality-controlled procedures, including anthropometry and prevalence of anaemia, Plasmodium falciparum and helminth infections.; Complete datasets were obtained from 1221 individuals of 194 households and eight schools. Of children aged >5 years (n=586), 41.8% were stunted, 23.2% were underweight and 4.8% were wasted. Very high prevalences of anaemia and P. falciparum were found in children aged 6-59 months (n=571; 86.1% and 74.0%, respectively). Overall, 73.7% of women of reproductive age (n=395) were anaemic. In school-aged children (n=240), 27.9% had light- to moderate-intensity hookworm infections, whereas Ascaris lumbricoides, Trichuris trichiura and Schistosoma mansoni were rare (>3% each).; The detailed description of the baseline health conditions, in combination with future health surveys, will deepen the understanding of how a biofuel project impacts on community health in a rural setting in sub-Saharan Africa.

In wastewater systems in Kampala, Uganda, microbial contamination has increased over the past two decades. Those people who live or work along the Nakivubo channel and wetland and those who use the recreational areas along the shores of Lake Victoria are at an elevated risk of gastrointestinal infections. A quantitative microbial risk assessment (QMRA) was applied for five population groups, characterised by different levels of exposure to wastewater in the Nakivubo area, namely: (i) slum dwellers at risk of flooding; (ii) children living in these slum settlements; (iii) workers maintaining the drainage system or managing faecal sludge (sanitation workers); (iv) urban farmers; and (v) swimmers in Lake Victoria. The QMRA was based on measured concentrations of Escherichia coli, Salmonella spp. and Ascaris spp. eggs in wastewater samples. Published ratios between measured organism and pathogenic strains of norovirus, rotavirus, Campylobacter spp., pathogenic E. coli, pathogenic Salmonella spp., Cryptosporidium spp. and Ascaris lumbricoides were used to estimate annual incidence of gastrointestinal illness and the resulting disease burden. The QMRA estimated a total of 59,493 disease episodes per year across all 18,204 exposed people and an annual disease burden of 304.3 disability-adjusted life years (DALYs). Incidence estimates of gastrointestinal disease episodes per year were highest for urban farmers (10.9) and children living in slum communities (8.3), whilst other exposed groups showed lower incidence (, Microbial Risk Analysis, 4, ISSN:2352-3522

Background: There is growing interest in predictive risk mapping of neglected tropical diseases (NTDs) particularly in the context of scaling up preventive chemotherapy surveillance and elimination efforts. Although soil transmitted helminths (Ascaris lumbricoides hookworm and Trichuris trichiura) are the most widespread infections among the NTDs attempts for district national or regional geostatistical analyses are scarce. The purpose of this study was to predict the spatial and temporal distribution of soil transmitted helminth infections including the number of infected people and treatment needs across sub Saharan Africa. Methods: We systematically searched PubMed Web of Knowledge and African Journal Online from inception to 31 December 2013 with no language restrictions to identify georeferenced surveys pertaining to soil transmitted helminth infections in 43 countries of sub Saharan Africa. We compiled data from readily available household surveys to extract information on sanitation drinking water sources and education of adult females. Bayesian geostatistical models were used to align the data in space and to obtain soil transmitted helminth infection risk estimates over a grid of around one million pixels at a spatial resolution of 5 × 5 km. Findings: We identified 459 sources reporting surveys on soil transmitted helminth infections. We georeferenced 6040 unique locations and entered them into the open access Global Neglected Tropical Diseases database. We estimate that the prevalence of hookworm A. lumbricoides and T. trichiura among school aged children from 2000 onwards was 16.5 6.7 and 4.4 respectively. Compared to surveys conducted before 2000 those estimates are between 52 and 74 lower. Interestingly in the latter time period prevalence levels among school aged children. are similar to those in entire communities. Following World Health Organization guidelines treatment requirements are estimated at 126 million doses per year. Interpretation: Patterns of soil transmitted helminth infection in sub Saharan Africa have changed and the prevalence of infection has declined considerably in the new millennium most likely due to socioeconomic development and large scale deworming. Because prevalence levels in school aged children and entire communities are now similar the global control strategy should be reassessed with greater emphasis placed on adults to consolidate morbidity control and move towards local elimination.

Summary Background Schistosomiasis affects more than 200 million individuals, mostly in sub-Saharan Africa, but empirical estimates of the disease burden in this region are unavailable. We used geostatistical modelling to produce high-resolution risk estimates of infection with Schistosoma spp and of the number of doses of praziquantel treatment needed to prevent morbidity at different administrative levels in 44 countries. Methods We did a systematic review to identify surveys including schistosomiasis prevalence data in sub-Saharan Africa via PubMed, ISI Web of Science, and African Journals Online, from inception to May 2, 2014, with no restriction of language, survey date, or study design. We used Bayesian geostatistical meta-analysis and rigorous variable selection to predict infection risk over a grid of 1 155 818 pixels at 5 × 5 km, on the basis of environmental and socioeconomic predictors and to calculate the number of doses of praziquantel needed for prevention of morbidity. Findings The literature search identified Schistosoma haematobium and Schistosoma mansoni surveys done in, respectively, 9318 and 9140 unique locations. Infection risk decreased from 2000 onwards, yet estimates suggest that 163 million (95% Bayesian credible interval [CrI] 155 million to 172 million; 18·5%, 17·6–19·5) of the sub-Saharan African population was infected in 2012. Mozambique had the highest prevalence of schistosomiasis in school-aged children (52·8%, 95% CrI 48·7–57·8). Low-risk countries (prevalence among school-aged children lower than 10%) included Burundi, Equatorial Guinea, Eritrea, and Rwanda. The numbers of doses of praziquantel needed per year were estimated to be 123 million (95% CrI 121 million to 125 million) for school-aged children and 247 million (239 million to 256 million) for the entire population. Interpretation Our results will inform policy makers about the number of treatments needed at different levels and will guide the spatial targeting of schistosomiasis control interventions. Background The epidemiology of malaria and anaemia is characterized by small-scale spatial and temporal heterogeneity, which might be influenced by human activities, such as mining and related disturbance of the environment. Private sector involvement holds promise to foster public health, including the prevention and control of malaria and anaemia. Here, results from a cross-sectional epidemiological survey, conducted in communities that might potentially be affected by the Bonikro Gold Mine (BGM) in Côte d’Ivoire, are reported. Methods In December 2012, a cross-sectional survey was carried out in seven communities situated within a 20-km radius of the BGM in central Côte d’Ivoire. Capillary blood samples were obtained from children aged six to 59 months. Samples were subjected to a rapid diagnostic test (RDT) for Plasmodium falciparum detection, whilst haemoglobin (Hb) was measured to determine anaemia. Additionally, mothers were interviewed with a malaria-related knowledge, attitudes and practices questionnaire. Results A total of 339 children and 235 mothers participated in the surveys. Schistosomiasis control efforts mainly target school-aged children. We studied the epidemiology of schis- tosomiasis in two high-risk communities in south Cote d'Ivoire, placing particular emphasis on pre-school-aged children. We used a suite of diagnostic techniques, including Kato-Katz, urine filtration, reagent strips, and urine circulating cathodic antigen cassettes. Risk factors for schistosomiasis were determined by focus group discussions and a structured question- naire. The prevalence of Schistosoma mansoni in the two study villages among the pre-school-aged children (age < 6 years) was 20.9% and 25.0%, whereas several-fold higher prevalences were found in school-aged children (58.7-68.4%) and adolescents/adults (59.5-61.7%). The prevalence of S. haematobium in the three age groups was 5.9-17.3%, 10.9-18.4%, and 3.8-21.3%, respectively. Most participants had light-intensity infections. Mothers' occupations and older siblings play important roles in the epidemiology of schistosomiasis in pre-schoolers. In the current epidemiologic settings, more attention is warranted on pre-school-aged children and adolescents/adults for successful schistosomiasis control.

Background There are health risks associated with wastewater and fecal sludge management and use, but little is known about the magnitude, particularly in rapidly growing urban settings of low- and middle-income countries. We assessed the point-prevalence and risk factors of intestinal parasite infections in people with different exposures to wastewater and fecal sludge in Kampala, Uganda. Methodology A cross-sectional survey was carried out in September and October 2013, enrolling 915 adults from five distinct population groups: workers maintaining wastewater facilities; workers managing fecal sludge; urban farmers; slum dwellers at risk of flooding; and slum dwellers without risk of flooding. Stool samples were subjected to the Kato-Katz method and a formalin-ether concentration technique for the diagnosis of helminth and intestinal protozoa infections. A questionnaire was administered to determine self-reported signs and symptoms, and risk factors for intestinal parasite infections. Univariate and multivariate analyses, adjusted for sex, age, education, socioeconomic status, water, sanitation, and hygiene behaviors, were conducted to estimate the risk of infection with intestinal parasites and self-reported health outcomes, stratified by population group. Principal Findings The highest point-prevalence of intestinal parasite infections was found in urban farmers (75.9%), whereas lowest point-prevalence was found in workers managing fecal sludge (35.8%). Hookworm was the predominant helminth species (27.8%). In urban farmers, the prevalence of Trichuris trichiura, Schistosoma mansoni, Ascaris lumbricoides, and Entamoeba histolytica/E. dispar was 15% and above. For all investigated parasites, we found significantly higher odds of infection among urban farmers compared to the other groups (adjusted odds ratios ranging between 1.6 and 12.9). In general, female participants had significantly lower odds of infection with soil-transmitted helminths and S. mansoni compared to males. Higher educational attainment was negatively associated with the risk of intestinal protozoa infections, while socioeconomic status did not emerge as a significant risk factor for any tested health outcome. Conclusions/Significance Urban farmers are particularly vulnerable to infections with soil-transmitted helminths, S. mansoni, and intestinal protozoa. Hence, our findings call for public health protection measures for urban farmers and marginalized communities, going hand-in-hand with integrated sanitation safety planning at city level., Author Summary Urban wastewater and fecal sludge use is of growing importance all over the world. However, unsafe management and inappropriate use might exacerbate the transmission of infectious diseases, including those caused by intestinal protozoa (e.g., amebiasis and giardiasis) and parasitic worms (e.g., soil-transmitted helminthiasis and schistosomiasis). People living and working in densely populated and rapidly transforming cities in Africa and Asia are especially vulnerable. We conducted a cross-sectional survey and assessed people’s risk of intestinal parasitic infections due to exposure to wastewater and fecal sludge management and use in Kampala, the capital of Uganda. We collected data on the prevalence, intensity, and risk factors of infections with parasitic worms and intestinal protozoa among slum dwellers, urban farmers, and workers maintaining the sanitation system. We found high infection prevalence of Schistosoma mansoni and soil-transmitted helminths in urban farmers and slum dwellers after adjusting for age, sex, and educational attainment. Our data suggest that urban farmers are especially vulnerable to infections with intestinal parasites, which may play an important role in the transmission through contamination of their living and working environments. In view of our results, the control of schistosomiasis and soil-transmitted helminthiasis should be accelerated in Kampala.

Background The epidemiology of malaria in the Senegal River Gorgol valley, southern Mauritania, requires particular attention in the face of ongoing and predicted environmental and climate changes. While “malaria cases” are reported in health facilities throughout the year, past and current climatic and ecological conditions do not favour transmission in the dry season (lack of rainfall and very high temperatures). Moreover, entomological investigations in neighbouring regions point to an absence of malaria transmission in mosquito vectors in the dry season. Because the clinical signs of malaria are non-specific and overlap with those of other diseases (e.g. acute respiratory infections and diarrhoea), new research is needed to better understand malaria transmission patterns in this region to improve adaptive, preventive and curative measures. Methods We conducted a multipurpose cross-sectional survey in the city of Kaédi in April 2011 (dry season), assessing three major disease patterns, including malaria. Plasmodium spp. parasite rates were tested among children aged 6–59 months who were recruited from a random selection of households using a rapid diagnostic test and microscopic examination of Giemsa-stained thick and thin blood films. Acute respiratory infection and diarrhoea were the two other diseases investigated, administering a parental questionnaire to determine the reported prevalence among participating children. Findings No Plasmodium infection was found in any of the 371 surveyed preschool-aged children using two different diagnostic methods. Acute respiratory infections and diarrhoea were reported in 43.4% and 35.0% of the participants, respectively. About two thirds of the children with acute respiratory infections and diarrhoea required medical follow-up by a health worker. Conclusions Malaria was absent in the present dry season survey in the capital of the Gorgol valley of Mauritania, while acute respiratory infections and diarrhea were highly prevalent. Surveys should be repeated towards the end of rainy season, which will enhance our understanding of the potential changes in malaria transmission in a region known as ‘hot spot’ of predicted climate change.
