Background: The arteriovenous fistula (AVF) is the preferred vascular access for hemodialysis. Four different cannulation techniques can be used: Rope Ladder (RL), Buttonhole (BH), Multiple Single cannulation Technique (MuST), or Area Puncture (AP). Objective: To build and validate a decision-making instrument for the optimal AVF cannulation technique in hemodialysis. Methodology: This two-stage methodological study using the Delphi method was conducted between October 2021 and February 2022 with 27 experts. The decision-making instrument for choosing the cannulation technique was divided into four blocks: physical assessment, ultrasound assessment, vascular access graphical representation, and observations. The content validity index ≥ 0.90 was used in the analysis. Results: The instrument's structure obtained the unanimous agreement of the experts and an overall content validity index of 0.94. The hemodialysis sector is a complex environment that requires actions to ensure the safety of patients undergoing renal replacement therapy. Thus, this article aims to report the interventions performed by nursing students on aspects of patient safety in hemodialysis. It is a descriptive study, with a qualitative approach of the experience report type. Nursing interventions were carried out by two nursing students in hemodialysis clinics. The interventions were carried out through permanent education on the nursing records and health education on the care with vascular accesses. Permanent educations were carried out with nursing technicians on the importance and the correct note of nursing. While health education was developed for patients with the aid of educational material and individualized guidance. It is concluded that these two interventions generated reflections on patient safety on dialysis, as well as improved this practice through simple actions that positively impacted both professionals and patients. Introducción: El estado de inmunosupresión de los pacientes con enfermedad renal crónica (ERC) aumenta su riesgo de obtener resultados clínicos desfavorables si llegaran a contraer COVID-19. Objetivo: Identificar la evidencia científica acerca de las repercusiones que tiene el COVID-19 en los pacientes en hemodiálisis. Materiales y Métodos: Se realizó una revisión sistemática en este estudio. Se hizo una búsqueda en las bases de datos Cochrane Library, Web of Science, Science Direct, PubMed y Virtual Health Library para identificar estudios relevantes. La calidad metodológica de los estudios se evaluó mediante la lista de chequeo adaptada por Downs y Black. La revisión siguió los lineamientos de la declaración PRISMA. Resultados:Tras el proceso de selección se incluyeron un total de 16 artículos en la revisión. Todos los artículos obtuvieron una calidad metodológica superior a 66,8%. Las repercusiones más comunes del COVID-19 en los pacientes en hemodiálisis fueron el aumento de la tasa de mortalidad (75%), el desarrollo de signos y síntomas típicos de la enfermedad como fiebre, tos, disnea y fatiga (68,75%), linfopenia (68,75%), progresión a un síndrome respiratorio agudo grave (56,25%), necesidad de ventilación mecánica (50%) e ingreso a cuidados intensivos (50%). Conclusiones: Los pacientes en hemodiálisis son más susceptibles a contraer COVID-19 y, cuando contraen el SARS-CoV-2, tienen resultados clínicos más adversos, enfermedades más graves, mayor mortalidad y peor pronóstico que la población general. Introdution: The immunosuppressive state of patients with CKD increases their risk of developing poor clinical outcomes if they acquire COVID-19 infection. Objective: To identify the scientific evidence about the repercussions of COVID-19 in hemodialysis patients. Materials and Methods: A systematic review was conducted in this study. The databases Cochrane Library, Web of Science, Science Direct, PubMed, and Virtual Health Library were searched to identify relevant studies. The methodological quality of the studies was assessed using the adapted Downs and Black checklist. The review adhered to the PRISMA guidelines. Results: A total of 16 articles were included after the screening process. All articles had a methodological quality higher than 66.8%. The most common repercussions of COVID-19 in hemodialysis patients were the increased mortality rate (75%), development of typical signs and symptoms of the disease such as fever, cough, dyspnea, and fatigue (68.75%), lymphopenia (68.75%), progression to severe acute respiratory syndrome (56.25%), need for mechanical ventilation (50%), and admission to intensive (50%). Conclusions: The hemodialysis patients are more susceptible to COVID-19 infection and, when infected by SARS-CoV-2, these patients have more adverse clinical outcomes, more serious diseases, higher mortality, and worse prognosis than the general population. The repercussions of COVID-19 in hemodialysis patients reveal a need for preventive nursing care in hemodialysis clinics. [ABSTRACT FROM AUTHOR]

Introducción: el síntoma de la fatiga se considera incapacitante y afecta a la calidad de vida de las personas con enfermedad renal crónica en tratamiento de hemodiálisis. Objetivo: evaluar la prevalencia, intensidad y gravedad del síntoma fatiga en personas con enfermedad renal crónica en tratamiento de hemodiálisis. Materiales y métodos: Estudio descriptivo cuantitativo, en el que participaron 335 personas en tratamiento de hemodiálisis. Para la recogida de los datos se utilizó la Visual Analog Scale" de 100mm, escala de gravedad de la fatiga y un cuestionario sociodemográfico. Se analizaron las variables dependientes de las escalas de fatiga, con las independientes de caracterización sociodemográfica y clínica. Resultados: 58% eran hombres, 52,2% tenían entre 20-59 años, 68,2% eran predominantemente blancos, 68,6% tenían bajo nivel de estudios, 55,7% tenían la jubilación como ingreso principal, 42,4% tenían ingresos entre R$ 800,01 y R$ 1760,00, la prevalencia de la fatiga fue de 63,0%, fatiga severa en viudos (35,2%) y mujeres (27,6%), y la principal comorbilidad fue la hipertensión arterial sistémica (40,9%). La intensidad de la fatiga tuvo un valor medio de 42,3 (DP=32,2) y la gravedad de la fatiga tuvo un valor medio de 35,8 (DP=17,8). Conclusiones: se evidenció que el factor socioeconómico y las comorbilidades influyen en el síntoma de fatiga en mujeres, viudos y ancianos. El uso de instrumentos de evaluación puede contribuir a mejorar el abordaje de la enfermedad y, en consecuencia, el bienestar y la calidad de vida de las personas. Introduction: The symptom of fatigue is considered incapacitating and affects the quality of life of people with chronic kidney disease, undergoing hemodialysis treatment. Objective: to assess the prevalence, intensity, and severity of the symptoms of fatigue in people with chronic kidney disease in hemodialysis treatment. Materials and Methods: Descriptive quantitative study, in which 335 in hemodialysis treatment participated. For the data collection, were used the 100mm Visual Analog Scale", the scale of severity of the fatigue, and a sociodemographic questionnaire. Were analyzed variable dependent on the fatigue scale, with the independent variables of sociodemographic and clinical characterization. Results: 58% were identified as male, aged between 20-59 years old in 52.2%, predominance of White color in 68.2%, 68.6% with low schooling, retirement as main income in 55.7%, yields between R$ 800.01 and R$ 1760.00 in 42.4%, with a prevalence of fatigue were 63.0%, severe fatigue in viúvos (35.2%) and women (27.6%), being that the main comorbidities of Systemic Arterial Hypertension (40.9%). At the intensity of fatigue, it has a mean value of 42.3 (DP = 32.2) and at the severity of fatigue, it has a mean value of 35.8 (DP = 17.8). Conclusions: We evidenced that the socioeconomic factor in comorbidities influences the symptoms of fatigue in women, widowers, and the elderly. The use of assessment instruments can contribute to help us address the sickness, and consequently, or improve the quality of life of these people. [ABSTRACT FROM AUTHOR]

12. Niveles de adherencia en una intervención de ejercicio guiada por enfermería en pacientes en diálisis.

Introduction: There are still several barriers against establishing exercise routines in regular dialysis treatments despite their well-known benefits. Developing cost-effective strategies is necessary to overcome these barriers and include exercise as a complement to dialysis treatment. Nurses play a central role in these units; therefore, examining adherence may be interesting to make exercise implementation easier. Methods: 71 subjects were randomized either into an intradialytic or a home exercise program. They did exercise for 16 weeks. Nurses ran the intradialytic exercise program and the home exercise program was autonomous. The total number of sessions carried out was recorded in order to obtain the levels of adherence to the exercise. Results: In the intradialytic exercise group, 25% of participants showed low adherence; 50% had medium adherence, and 25%, high adherence. In the home group, 49% showed low adherence; 9% exhibited medium adherence, and 42%, high adherence. Conclusions: There are higher levels of adherence in the intradialytic group when compared to the home group, but the levels of high adherence were more significant in the home group, showing a greater impact in those with low adherence. Objective: To know the nurse's routine in the hemodialysis sector of a public hospital in Northeast Brazil. Methods: Field study of exploratory and descriptive nature, qualitative approach. Developed in the hemodialysis sector of a public hospital located in Campina Grande, Paraíba, with all nurses working in the sector. Data collection took place in September 2017, through semi-structured interviews, and analyzed following the theory proposed by Bardin. Results: Nurses were interviewed with an average age of 41 years, predominantly female and with an average time of eight years working in the hemodialysis sector. The following categories emerged from the content analysis: Nursing assignments in the hemodialysis sector; Nursing management in the hemodialysis service; Systematization of nursing care in the hemodialysis service; Adversities and complications found in the hemodialysis sector; and Actions by the nursing team to minimize risks and possible complications. Conclusão: The multiplicity of duties and activities in the routine of nurses in the hemodialysis sector was evident in our study, which showed the essential role and role of nurses within such a specific and highly complex sector. Objective: To analyze the result of the current state in chronic kidney patients on hemodialysis. Method: quantitative study performed with 25 patients undergoing dialysis therapy in a tertiary hospital through the accomplishment of semi-structured interviews, physical examination and analysis of laboratory test results. In order to analyze the diagnostic inferences, we used the clinical reasoning of Alfaro-LeFevre and then applied the Outcome-Present State Test Model. Results: We inferred 12 nursing diagnoses found in 70% of the sample, where anxiety was the priority diagnosis. In order to minimize changes in the health of the patient, the interventions chosen were: teaching: procedure/treatment; exercise promotion; progressive muscle relaxation; distraction/inattention; emotional support; nutrition control; and improved socialization. Conclusion: the clinical reasoning technique used by this model can contribute to the agility and execution of the nursing process. [ABSTRACT FROM AUTHOR]

Resumo Objetivo: Conhecer as estratégias de enfrentamento utilizadas por pacientes renais crônicos submetidos ao tratamento hemodialítico, frente às dificuldades inerentes à doença. Métodos: Estudo descritivo, qualitativo, realizado com 30 pessoas que viviam com condições renais crônicas, em uma unidade de hemodiálise no Nordeste do Brasil. Os dados foram coletados de janeiro a março de 2013 por meio de um instrumento de entrevista semiestruturada e analisados segundo a análise de conteúdo, modalidade temática. Utilizou-se como referencial a teoria do Enfrentamento ou coping. Resultados: As categorias que emergiram dos discursos dos entrevistados foram: apoio familiar; apego à religião/crença; negação e esquiva; e resiliência. Conclusão: A forma para manejar as dificuldades inerentes à doença revelou-se por estratégias de enfrentamento tanto baseadas na emoção, como no problema. Assim, foram desenvolvidos esforços para administrar ou alterar os problemas iniciais, bem como tentativas de substituir ou regular o impacto emocional da doença.

22. Construcción y validación de un modelo de decisión para la canulación de fístulas arteriovenosas en hemodiálisis

Background: The arteriovenous fistula (AVF) is the preferred vascular access for hemodialysis. Four different cannulation techniques can be used: Rope Ladder (RL), Buttonhole (BH), Multiple Single cannulation Technique (MuST), or Area Puncture (AP). Objective: To build and validate a decision-making instrument for the optimal AVF cannulation technique in hemodialysis. Methodology: This two-stage methodological study using the Delphi method was conducted between October 2021 and February 2022 with 27 experts. The decision-making instrument for choosing the cannulation technique was divided into four blocks: physical assessment, ultrasound assessment, vascular access graphical representation, and observations. The content validity index = 0.90 was used in the analysis. Results: The instrument’s structure obtained the unanimous agreement of the experts and an overall content validity index of 0.94. Conclusion: The instrument under analysis proved to have the face and content validity for nursing consultations., Marco contextual: La fístula arteriovenosa se considera el acceso vascular de preferencia para la hemo diálisis y puede realizarse mediante cuatro técnicas de canulación diferentes, escalera, ojal, MuST o área. Objetivo: Construir y validar una herramienta de apoyo a la decisión para la técnica ideal de canulación de la fístula arteriovenosa en hemodiálisis. Metodología: Estudio metodológico en dos etapas mediante la técnica Delphi entre octubre de 2021 y febrero de 2022 con 27 jueces expertos. El instrumento de apoyo al modelo de decisión para la canulación se segmentó en cuatro bloques: evaluación física, evaluación ecográfica, esquema fotográfico con dermopigmentación y observaciones. Para el análisis, se utilizó el índice de validez de contenido = 0,90. Resultados: El consenso final de los jueces se obtuvo mediante un acuerdo unánime sobre la estructura del instrumento y un índice de validez de contenido global de 0,94. Conclusión: El instrumento estudiado demostró ser válido en apariencia y contenido para aplicarlo en la consulta de enfermería., Enquadramento: A fístula arteriovenosa é considerada como o acesso vascular de eleição para hemodiálise, a sua utilização pode ser realizada através de quatro técnicas de canulação distintas: Escada, Botoeira, MuST ou Área. Objetivo: Construir e validar um instrumento de apoio à decisão para a técnica de canulação ideal da fistula arteriovenosa em hemodiálise. Metodologia: Estudo metodológico em duas etapas mediante a realização da técnica de Delphi entre outubro 2021 e fevereiro de 2022 através de 27 juízes peritos. O instrumento de apoio ao modelode decisão para a canulação foi segmentado em quatro blocos: avaliação física, avaliação ecográfica esquema fotográfico com dermopigmentação e observações. O setor de hemodiálise é um ambiente complexo que requer ações que garantam a segurança do paciente que realiza o tratamento de terapia renal substitutiva. Desta forma, este artigo objetiva relatar as intervenções realizadas por acadêmicas de enfermagem sobre aspectos da segurança do paciente em hemodiálise. Trata-se de um estudo descritivo, com abordagem qualitativa do tipo relato de experiência. Realizaram-se intervenções de enfermagem por duas acadêmicas de enfermagem, em clínicas de hemodiálise. As intervenções se deram por meio de educação permanente sobre os registros de enfermagem e educação em saúde sobre o cuidado com acessos vasculares. As educações permanentes foram realizadas com os técnicos de enfermagem sobre a importância e a correta anotação de enfermagem. Enquanto que as educações em saúde foram desenvolvidas para os pacientes com o auxilio de um material educativo e orientações individualizadas. Conclui-se que essas duas intervenções geraram reflexões sobre a segurança do paciente em diálise, bem como aperfeiçoou essa prática por meio de ações simples que impactaram positivamente tanto os profissionais como os pacientes. El sector de la hemodiálisis es un entorno complejo que requiere acciones para garantizar la seguridad de los pacientes sometidos a terapia renal sustitutiva. Así, este artículo tiene como objetivo reportar las intervenciones realizadas por estudiantes de enfermería sobre aspectos de seguridad del paciente en hemodiálisis. Es un estudio descriptivo, con un abordaje cualitativo del tipo relato de experiencia. Las intervenciones de enfermería fueron realizadas por dos estudiantes de enfermería en clínicas de hemodiálisis. Las intervenciones se realizaron a través de la educación permanente sobre los registros de enfermería y la educación para la salud sobre el cuidado con accesos vasculares. Se realizaron capacitaciones permanentes con técnicos de enfermería sobre la importancia y la correcta nota de la enfermería. Mientras que la educación para la salud se desarrolló para los pacientes con la ayuda de material educativo y orientación individualizada. Se concluye que estas dos intervenciones generaron reflexiones sobre la seguridad del paciente en diálisis, además de mejorar esta práctica a través de acciones sencillas que impactaron positivamente tanto a los profesionales como a los pacientes. The hemodialysis sector is a complex environment that requires actions to ensure the safety of patients undergoing renal replacement therapy. Thus, this article aims to report the interventions performed by nursing students on aspects of patient safety in hemodialysis. It is a descriptive study, with a qualitative approach of the experience report type. Nursing interventions were carried out by two nursing students in hemodialysis clinics. The interventions were carried out through permanent education on the nursing records and health education on the care with vascular accesses. Permanent educations were carried out with nursing technicians on the importance and the correct note of nursing. While health education was developed for patients with the aid of educational material and individualized guidance. It is concluded that these two interventions generated reflections on patient safety on dialysis, as well as improved this practice through simple actions that positively impacted both professionals and patients.

Objetivo: Identificar las complicaciones clínicas y evaluar la percepción de salud general de los pacientes renales crónicos en hemodiálisis. Metodología: Estudio transversal, con enfoque cuantitativo-descriptivo, realizado en 77 pacientes en hemodiálisis en una Unidad Nefrológica del noroeste de Rio Grande do Sul. La recolección de datos se realizó durante los meses de mayo, junio y julio de 2010 con un formulario de características sociodemográficas/clínicas, también denominado Kidney Disease and Quality of Live-Short Form (kdqol-sftm). Los datos se analizaron mediante estadística descriptiva. El proyecto de investigación fue aprobado por el Comité de Ética en el Consejo nº 02780243000-09. Resultados: La mayoría de los pacientes eran varones (70,1%), adultos mayores (45,4%), casados (59,7%). Los eventos que se produjeron con mayor frecuencia durante la hemodiálisis fueron: debilidad, calambres e hipotensión. En cuanto a la evaluación de la salud general en comparación con la de hace un año, el 39% lo calificó como mucho mejor ahora y el 33,8% como un poco mejor ahora. Conclusiones: A pesar de las complicaciones, actualmente los pacientes valoran mejor su salud.

OBJECTIVE To measure the pleasure and suffering indicators at work and relate them to the socio-demographic and employment characteristics of the nursing staff in a hemodialysis center in southern Brazil. METHOD Quantitative research, with 46 workers. We used a self-completed form with demographic and labor data and the Pleasure and Suffering Indicators at Work Scale (PSIWS). We conducted a bivariate and correlation descriptive analysis with significance levels of 5% using the Epi-Info® and PredictiveAnalytics Software programs. RESULTS Freedom of Speech was considered critical; other factors were evaluated as satisfactory. The results revealed a possible association between sociodemographic characteristics and work, and pleasure and suffering indicators. There was a correlation between the factors evaluated. CONCLUSION Despite the satisfactory evaluation, suffering is present in the studied context, expressed mainly by a lack of Freedom of Speech, with the need for interventions to prevent injury to the health of workers.

Objective. To identify similarities between NANDA International nursing diagnoses and Roy’s adaptation model among chronic renal patients undergoing hemodialysis. Methodology. Cross-sectional and descriptive study with 178 individuals selected, through consecutive convenience sampling, in a dialysis center located in the Northeast of Brazil. The study was conducted between October 2011 and February 2012. Data collection instruments included an interview form and a physical assessment. Results. Similarity was found between 20 nursing diagnoses and 22 adaptation problems. Roy’s adaptation modes that presented these relationships were: physiological, self-conception and role function. Conclusion. There are similarities between the two typologies. Furthermore, the use of the nursing process from the perspective of a theory inherent to the field supports care delivery and strengthens scientific knowledge in the profession.

Aim: to evaluate the hope manifested by people undergoing hemodialysis and its relation with resilience. Method: cross-sectional study, in two hemodialysis units of the metropolitan region of Florianópolis. Sixty people participated in the study, from January to March 2017. Herth's Hope Scale, adapted and validated for the portuguese language and Connor - Davidson Resilience Scale were used as characterization instruments. Spearman's descriptive analysis and correlation coefficient. Results: moderate and positive correlation between hope and resilience (r: 0.470; p <0.01). Females with higher mean values for hope and resilience (M: 44;72; SD: 3,23) and (M: 84,44, SD: 10,66). Those on the waiting list for transplantation obtained lower scores for hope (M: 42,75; SD: 4,76) and for resilience (M: 79,33; SD: 10,54). Conclusion: people on hemodialysis have a disposition to hope and resilience, which suggests a better adaptation to treatment. [ABSTRACT FROM AUTHOR]

Aim: to identify factors that influence adherence of elderly people to hemodialysis treatment. Methodology: descriptive research with a quantitative approach, with 81 elderly patients in a hemodialysis clinic in the Northeast of Brazil. Data were collected from January to March, 2014 and analyzed through descriptive statistics. Results: women accounted for 50.6% of the elderly studied. The majority had between two to three years on hemodialysis (51.9%). Interdialytic weight gain varied from 1 to 6 kilos, most of the subjects (86.4) were assiduous at the hemodialysis sessions in the last six months. Regarding the aspects that negatively influence adherence to hemodialysis treatment, difficulty to travel, followed by puncture pain and bruising in the arteriovenous fistula member were referred by the subjects. Conclusion: the identification of factors that influence the adherence of elderly people to hemodialysis should support the development of educational nursing actions involving the patients and their families. The objective was to share successful experiences of nursing students when experiencing the care of the person undergoing dialysis. METHOD: This is a descriptive study of the experience report, shared by nursing students who make up the extension project experiencing nursing care in a dialysis unit, performed at a nephrology clinic in the Northeast of Brazil, from September to November 2016. RESULTS: It was possible to perceive the relevance of the nursing practices to the hemodialysis patient, as well as the competencies and autonomy of the professionals. CONCLUSION: The experiences lived by the students gave the students a greater dimension on the ethical, scientific responsibility of actions and nursing interventions to the patient in the dialysis unit. Objetivo: Caracterizar os homens em tratamento hemodialítico com vírus da imunodeficiência humana e da hepatite B e C. Método: Estudo descritivo, realizado com aplicação de questionário junto a 30 pacientes do Real Hospital Português, entre novembro de 2011 a fevereiro de 2012. Resultados: Do total, 36,7% dos pacientes possuem idade superior a 60 anos, 43,3% são brancos, 40% com desnutrição. Cerca de 70% têm o vírus da hepatite C, 63,3% não receberam orientações sexuais sobre fatores de risco, 73,3% possuíram mais de cinco parceiros sexuais e 66,7% já se submeteram a algum procedimento invasivo. Conclusão: A detecção oportuna das infecções virais crônicas entre pacientes em hemodiálise é necessária para o devido cumprimento da terapia, bem como para tomar medidas preventivas para a proteção de outros pacientes e funcionários.

OBJECTIVES: to identify adaptation problems under Roy's Model in patients undergoing hemodialysis and to correlate them with the socioeconomic and clinical aspects.METHOD: a transversal study, undertaken using a questionnaire. The sample was made up of 178 individuals. The Chi-squared and Mann-Whitney U tests were undertaken.RESULTS: the adaptation problems and the socioeconomic and clinical aspects which presented statistical associations were: Hyperkalemia and age; Edema and income; Impairment of a primary sense: touch and income; Role failure and age; Sexual dysfunction and marital status and sex; Impairment of a primary sense: vision and years of education; Intolerance to activity and years of education; Chronic pain and sex and years of education; Impaired skin integrity and age: Hypocalcemia and access; Potential for injury and age and years of education; Nutrition below the organism's requirements and age; Impairment of a primary sense: hearing and sex and kinetic evaluation of urea; Mobility in gait and/or coordination restricted, and months of hemodialysis; and, Loss of ability for self-care, and months of hemodialysis and months of illness.CONCLUSION: adaptation problems in the clientele undergoing hemodialysis can be influenced by socioeconomic/clinical data. These findings contribute to the development of the profession, fostering the nurse's reflection regarding the care.

O objetivo deste estudo foi descrever atitudes e práticas de enfermeiros de hemodiálise na trajetória percorrida na especialidade. Métodos: Estudo de abordagem qualitativa do tipo Teoria Fundamentada nos Dados, realizado no serviço de nefrologia de um hospital universitário do Rio de Janeiro. Participaram do estudo nove enfermeiros de hemodiálise. Foi adotada a entrevista semiestruturada e a observação participante. As informações foram analisadas considerando-se os procedimentos básicos pertinentes a Teoria Fundamentada nos Dados: codificação aberta, codificação axial e codificação seletiva. Resultados: Da análise emergiu o fenômeno "Tornando-se proficiente: o saber/fazer do enfermeiro de hemodiálise",que se forma a partir da junção das categorias: "(Re) Estabelecendo seu modo de fazer em hemodiálise" e "Dominando o cenário tecnológico". Conclusão: O conhecimento adquirido, aprofundado e atualizado, conduz o enfermeiro ao agir consciente,considerando a complexidade envolvida no processo de aprender na prática.

OBJECTIVE: to evaluate the conformity of the assistential practice in the maintenance of the temporary double-lumen catheter for hemodialysis, by means of the use of the process indicator, in the University Hospital of the University of São Paulo. METHOD: a quantitative, exploratory-descriptive and observational study. The sample was made up of 155 observations of persons with temporary double-lumen catheters, in the period March - November 2011, using the Indicator of the Maintenance of the Temporary Double Lumen Catheter for Hemodialysis. RESULTS: the rate of general conformity of the assistential practice corresponded to 65.8%. Of the practice's 13 components, 9 (69.2%) attained 100% conformity. The hygienization of hands by the professionals and the use of a mask by the patients during the disconnection from the hemodialysis had the worst rates (83.9%). CONCLUSION: although the actions evaluated are implemented in the unit, it is necessary to propose and apply educational strategies with the health team, as well as to institute periodical assessments, so as to raise the conformity rates, ensuring the quality of the hemodialysis services.

The objective of this study was to identify the nursing diagnoses of the safety/protection domain in the NANDA-I Taxonomy II and propose nursing interventions and activities based on the Nursing interventions Classification (NIC) for chronic renal patients undergoing hemodialysis. Diagnoses present in at least 75% of the sample were analyzed, corresponding to 25 patients hospitalized in Fortaleza- Ceará-Brazil in 2014/2015. The study was approved by the Research Ethics Committees of the State University of Ceará and the General Hospital of Fortaleza under Opinion nº 392.488 and CAAE: 19640613.2.0000.5534. Five high-risk nursing diagnoses present in 100% of the simple were selected. Some of the interventions and activities proposed for the diagnosis of risk of infections were infection control and adequate cleaning of the environment after the its use for each patient. The identified diagnoses are all related to risks. This calls attention to the role of nurses in preventive actions with patients and professionals, as they are responsible for the success of the treatment. Also, the knowledge of these professionals and their interventions and activities provide a scientific basis for evidence-based discussions. Finally, the research is relevant for its contributions to nursing/health care actions to patients/users undergoing hemodialysis treatment. [ABSTRACT FROM AUTHOR]

- Abstract
Objective: verify the association between socioeconomic and clinical characteristics and the factors associated with a diagnosis of fluid volume deficit risk in patients who are on hemodialysis. Materials and methods: This is a cross-sectional study done at the dialysis unit of a university hospital in northeastern Brazil. Featuring 50 patients, it was conducted through interviews and physical examinations. The Mann-Whitney U-test was used to analyze the data. Results: A number of significant associations were verified; namely, knowledge deficit and hemodialysis time (p = 0.037); weight extremes and family income (p = 0.040); weight extremes and ultrafiltration volume (p = 0.010), and fluid loss through abnormal pathways and ultrafiltration volume (p = 0.000). Conclusions: Associations were identified between the factors associated with a nursing diagnosis of fluid volume deficit risk and the socioeconomic and clinical characteristics of patients who are undergoing hemodialysis. The factors associated with that diagnosis can be influenced by socioeconomic characteristics and clinical aspects. [ABSTRACT FROM AUTHOR]

O objetivo deste estudo foi identificar os cuidados dos clientes em tratamento hemodialítico com o local da punção para a preservação da fístula arteriovenosa. Estudo qualitativo, descritivo-exploratório, que incluiu 17 clientes do Hospital Universitário (RJ) com doença renal crônica de qualquer etiologia, maiores de 18 anos de idade, de ambos os sexos, tendo como acesso a fístula arteriovenosa há, no mínimo, seis meses. Como resultados, obteve-se que 100% dos entrevistados conhecem a necessidade de cuidar e manter a via de punção pérvia; 42% já tiveram algum tipo de intercorrência na fístula, destacando-se a dor no membro e a perda do frêmito; 29% realizam compressa de gelo em casa, após a hemodiálise; 17% afirmaram que, a qualquer acontecimento diferente ou anormal, conversam com a equipe médica e de enfermagem. Concluiu-se que a enfermeira que orienta o cliente para a preservação do acesso necessita perceber indícios não verbais que apontem fragilidades no desempenho de papéis para o autocuidado.identificar cuáles son los cuidados que reciben los clientes en tratamiento de hemodiálisis con local de punción para la preservación de la fistula arteriovenosa. Conclusión: la enfermería que orienta el cliente para la preservación del acceso necesita enterarse de indicios no verbales que indiquen fragilidades en el desempeño de los papeles para el autocuidado.Identify the customer care on haemodialysis treatment with the puncture site for the preservation of arteriovenous fistula. Method: A qualitative study, descriptive-exploratory which included 17 clients of the University Hospital (RJ) with chronic kidney disease of any etiology, greater than 18 years old, both sexes, having as access an arteriovenous fistula for at least six months. Results: 100% of the interviewees know the necessity of caring and maintain the via puncture pervious, 42% had some type of intercadent in the fistula, stand out the pain in the limb and loss in the thrill, 29% carry ice pack at home after haemodialysis. And 17% assert that any different or abnormal event they talk to the medical staff and nursing. Conclusion: A nurse who directs the client to the preservation of access needs to realize nonverbal clues that point to weaknesses in the performance roles for self-care.

Objective: to analyze the accuracy of the defining characteristics of hypothermia in patients on hemodialysis. Methods: a diagnostic accuracy study was assembled within a cross-sectional study with 124 patients from two dialysis centers. A latent class model was used for data analysis. Results: the nursing diagnosis hypothermia was present in 13 (10.48%) study participants. The most prevalent defining characteristics were hypoxia (100%), decrease in blood glucose level (83.1%), hypertension (65.3%), piloerection (45.2%), and skin cool to touch (41.1%). The defining characteristics acrocyanosis (99.96%) and cyanotic nail beds (99.98%) had a high sensitivity. Acrocyanosis (91.8%), skin cool to touch (64.8%), and peripheral vasoconstriction (91.8%) had high specificity. Conclusion: specific and sensitive indicators of hypothermia work as good clinical indicators for confirming this diagnosis in patients on hemodialysis. The study findings can assist nurses in their clinical reasoning for a correct inference of hypothermia. RESUMEN Objetivo: analizar la exactitud de las características definitorias de la hipotermia en pacientes en hemodiálisis. Métodos: estudio de exactitud diagnóstica, con diseño transversal, realizado con 124 pacientes de dos centros de diálisis. Se utilizó un modelo de clases latentes para el análisis de datos. Resultados: el diagnóstico de enfermería hipotermia estuvo presente en 13 (10,48%) participantes del estudio. Las características definitorias más prevalentes fueron hipoxia (100%), reducción de la glucosa en sangre (83,1%), hipertensión (65,3%), piloerección (45,2%) y piel fría al tacto (41,1%). Las características definitorias de acrocianosis (99,96%) y lechos ungueales cianóticos (99,98%) mostraron alta sensibilidad. La acrocianosis (91,8%), la piel fría al tacto (64,8%) y la vasoconstricción periférica (91,8%) mostraran alta especificidad. Conclusión: los indicadores específicos y sensibles de hipotermia son buenos indicadores clínicos para confirmar este diagnóstico en pacientes en hemodiálisis. Los resultados del estudio pueden ayudar a las enfermeras a inferir correctamente la hipotermia. Métodos: estudo de acurácia diagnóstica, com delineamento transversal, realizado com 124 pacientes de dois centros de diálise. Um modelo de classe latente foi utilizado para a análise de dados. Resultados: o diagnóstico de enfermagem hipotermia esteve presente em 13 (10,48%) participantes do estudo. As características definidoras mais prevalentes foram hipóxia (100%), redução da glicemia sanguínea (83,1%), hipertensão (65,3%), piloereção (45,2%) e pele fria ao toque (41,1%). As características definidoras acrocianose (99,96%) e leitos ungueais cianóticos (99,98%) apresentaram alta sensibilidade. Acrocianose (91,8%), pele fria ao toque (64,8%) e vasoconstrição periférica (91,8%) apresentaram alta especificidade. Conclusão: os indicadores específicos e sensíveis de hipotermia são bons indicadores clínicos para confirmar esse diagnóstico em pacientes em hemodiálise. Os resultados deste estudo podem auxiliar os enfermeiros em seu raciocínio clínico para uma inferência correta de hipotermia.
