2. Emergency Department Neurosurgical Consultations in a Tertiary Care Hospital.

Objective: To reveal the characteristics of patients in need of neurosurgery by examining neurosurgery consultations within the emergency department (ED) admissions of a tertiary academic hospital. Methods: This is a retrospective, single-centre observational study. Patients admitted to the ED between 01.01.2022 - 31.12.2022 and consulted to the neurosurgery department were identified through the hospital computer system and included in the study. The demographic information of the patients, mode of admission to the ED, the reasons for admission, the time of admission, the number of brain computed tomography (CT) scans, whether they underwent surgery or not, and the mortality results were recorded. Results: A total of 441 neurosurgery consultations were examined. Fall 35.6% (n=157) and traffic accident 16.6% (n=73) were the most common reasons for consultation. It was observed that 92.5% (n=408) of the patients had a brain CT scan, and 19.5% (n=86) had two or more brain CTs. It was determined that 12.7% (n=56) of the patients consulted to neurosurgery were operated on, and 4.1% (n=18) of the patients' hospital admissions resulted in death. Only 53.7% (n=237) of the patients who underwent neurosurgery consultation were discharged from the ED. It was determined that significantly more Neurosurgery consultations were requested during working hours (p = 0.013). Conclusions: Most consultation calls from the ED to neurosurgery are for trauma patients. Brain CT examination is frequently used in neurosurgical patient evaluation. As a result of the consultations, almost half of the patients are hospitalized. Emergency physicians can select patients who need neurosurgery well. [ABSTRACT FROM AUTHOR]

Aim: Nowadays, with the rapid development and change of knowledge in the scientific field, the number of specialty branches in medicine has increased. Therefore, the rate of consultation requests in different specialties during the patient evaluation process gradually escalates. This study aims to evaluate the characteristics of the diagnostic and therapeutic consultations requested from the Otorhinolaryngology (ENT) branch in a university hospital. Methods: In this cross-sectional study, the data of patients consulted to the ENT clinic of Kutahya University of Health Sciences Evliya Celebi Training and Research Hospital between January 1st, 2017 and December 31st, 2019 were retrospectively scanned. Apart from demographic findings such as age and gender, departments requesting the consultation, post-consultation diagnoses and surgical treatments performed by an ENT physician were noted. Results: The total number of patients with an ENT consultation was 2340. The mean age of the patients was 45.95 (25.09) years. Most consultation requests were made from the emergency department (n=877, 37.5%), which was followed by 278 requests (11.9%) by the neurology and neurology ICU departments, and 193 (8.2%) requests by the anesthesiology and intensive care unit (ICU) departments. Although varying among age groups, epistaxis (10%) and vertigo (dizziness) (10%) were the most common diagnoses. A total of 146 patients received surgical treatment. The most common surgical operations were tracheotomy and nasal bone fracture reduction. Conclusion: Determining the characteristics of patients for whom ENT consultation is required will be beneficial in shaping the training programs for other clinics, reducing unnecessary consultation requests, thus reducing the workload of ENT physicians.

5. Characteristic Profile of the Patients Consulted from Emergency Department to Otolaryngology Clinic

Objective: Currently, consultation requests are frequently made from emergency departments to other clinics. The aim of this study is to perform a detailed analysis of the consultation requests from the emergency department to the otolaryngology clinic and to evaluate the case results and the need for consultation. Material and Methods: The files of 743 cases consulted from the emergency room to the otolaryngology clinic between January 2016 and December 2019 were reviewed retrospectively. Reasons for requesting consultation, symptoms and diagnoses of the cases, and procedures performed as a result of the consultation were analyzed in detail. Results: The ages of the patients ranged between 1 and 93 years (mean: 49.7 ± 9.38 years), and there were 354 (47.6%) female and 389 (52.4%) male patients. The most common reason for consultation request was epistaxis (24.5%). As a result of the consultation, 1.6% of all cases underwent emergency surgical operation and 4.9% underwent various surgical procedures under local anesthesia, and 4.2% were hospitalized for medical treatment. Outpatient clinic follow-up was recommended for 381 cases (51.3%), while elective operation was recommended for 125 cases (16.8%). As a result of 119 consultations (16.1%), no otolaryngologic pathology was found and these consultations were considered unnecessary. Conclusion: By having a sufficient number of specialist physicians in emergency departments, evaluating the problems associated with consultations with regular meetings between clinics, and organizing training programs in institutions that train specialist doctors, unnecessary requests for consultations can be reduced., Amaç: Günümüzde acil servislerden oldukça sık bir şekilde diğer kliniklere konsültasyon istemi yapılmaktadır. Bu çalışmanın amacı acil servisten kulak burun boğaz kliniğine istenen konsültasyonların detaylı analizini yaparak vaka sonuçlarını ve konsültasyon gerekliliğini değerlendirmektir. Gereç ve Yöntemler: Ocak 2016 ve Aralık 2019 tarihleri arasında acil servisten kulak burun boğaz kliniğine konsültasyon istenmiş olan 743 vakanın dosyaları retrospektif olarak incelendi. Konsültasyon istenme nedenleri, vakaların semptomları, tanıları ve konsültasyon sonucunda yapılan işlemler ayrıntılı analiz edildi. Bulgular: Hastaların yaşları 1 ile 93 yıl arasında (ortalama 49,7±9,38) değişmekte idi ve 354 (%47,6) kadın ve 389 (%52,4) erkek vardı. En sık konsültasyon istenme nedeni epistaksis (%24,5) olarak saptandı. Konsültasyon sonucunda tüm vakaların %1.6’sına acil cerrahi operasyon ve %4,9’una ise lokal anestezi ile çeşitli cerrahi işlemler yapıldığı, %4,2’sinin ise medikal tedavi için hastaneye yatırıldığı izlendi. 381 vakaya (%51,3) poliklinik kontrolü önerilirken 125 vakaya (%16,8) ise elektif operasyon önerildiği belirlendi. 119 konsültasyon (%16,1) sonucunda herhangi bir kulak burun boğaz patolojisine rastlanmadı ve bu konsültasyonlar gereksiz olarak değerlendirildi. Sonuç: Acil servislerde yeterli sayıda uzman hekim bulundurulması, konsültasyonlarla ilişkili sorunların klinikler arası düzenli toplantılarla değerlendirilmesi ve uzman hekim yetiştiren kurumlarda eğitim programlarının düzenlenmesi ile gereksiz konsültasyon istemleri azaltılabilecektir.

6. Examination of Emergency Ophthalmologic Consultations in Terms of Urgency, Ophthalmic Pathology, and the Weekend Effect

Objective: To evaluate the urgency of ophthalmology consultations in the emergency department (ED) and the presence of ophthalmic pathology. This study also aimed to determine whether the weekend phenomenon affected emergency ophthalmologic consultation requests. Materials and Methods: Ophthalmology consultations requested by the emergency physician for patients who visited the ED of a tertiary care hospital in Turkey from January to December 2019 were retrospectively investigated. Consultation requests were grouped as possible emergent, unlikely to be emergent, and undetermined. Results: A total of 256 eligible patients were included. The top three reasons for ophthalmologic consultations were blunt trauma (29.7%), foreign body (24.2%), and conjunctivitis (13.3%). 70.3% of the consultations were categorized as a possible emergent, 18.4% as unlikely to be emergent, and 11.3% undetermined. Most of the possible emergent consultations had ophthalmic pathology (p=0.001). Conclusion: Traumatic injuries are the most common cause of ophthalmology consultation in the ED. Approximately one-fifth of patients are unlikely to be emergent. Early ophthalmology evaluation is required in possible emergent category patients. The weekend effect does not influence ophthalmology consultations. Establishing eye emergency services or having an available ophthalmologist is crucial in the emergency diagnosis and treatment of patients who require special practice skills. Amaç: Acil serviste oftalmoloji konsültasyonlarının aciliyetini ve oftalmik patolojinin varlığını değerlendirmek. Bu çalışma aynı zamanda hafta sonu fenomeninin, acil oftalmolojik konsültasyon taleplerini etkileyip etkilemediğini belirlemeyi amaçladı. Materyal ve Metot: Türkiye'de bir üçüncü basamak hastanenin acil servisini Ocak-Aralık 2019 tarihleri arasında ziyaret eden hastalar için acil hekimi tarafından talep edilen oftalmoloji konsültasyonları geriye dönük olarak incelendi. Konsültasyon talepleri olası acil, acil olması muhtemel olmayan ve belirsiz olarak gruplandırıldı. Bulgular: Çalışmaya uygun toplam 256 hasta dahil edildi. Acil oftalmolojik konsültasyonların ilk üç nedeni künt travma (%29,7), yabancı cisim (%24,2) ve konjonktivit (%13,3) idi. Konsültasyonların %70,3'ü olası acil, % 18,4'ü acil olması muhtemel olmayan ve %11,3'ü belirsiz olarak kategorize edildi. Olası acil kategorideki konsültasyonların çoğunda oftalmik patoloji vardı (p=0.001). Sonuç: Travmatik yaralanmalar acil serviste oftalmoloji konsültasyonunun en sık nedenidir. 7. Reasons for Overcrowding in the Emergency Department: Experiences and Suggestions of an Education and Research Hospital.

Objectives In this study, we aimed to determine the causes of overcrowding in the Emergency Department (ED) and make recommendations to help reduce length of stay (LOS) of patients in the ED. Methods We analyzed the medical data of patients admitted to our ER in a one-year period. Demographic characteristics, LOS, revisit frequency, and consultation status of the patients were determined. Results A total of 163,951 patients were admitted to our ED between January 1, 2013, and December 31, 2013. In this period 1,210 patients revisited the ED within 24 hours. A total of 38,579 patients had their treatment in the observation room (OR) of the ED and mean LOS was found to be 164.1 minutes. Cardiology was the most frequently consulted specialty. Mean arrival time of the consultants in ED was 64 minutes. Conclusions Similar to EDs in other parts of the world, prolonged length of stay in the ED, delayed laboratory and imaging tests, delay of consultants, and lack of sufficient inpatient beds are the most important causes of overcrowding in the ED. Some drastic measures must be taken to minimize errors and increase satisfaction ratio. [ABSTRACT FROM AUTHOR]

Objective: We aimed to evaluate the demographic and clinical features of patients who attended the emergency department of Uludag University Hospital, our emergency practices and the deficiencies. Materials and Methods: All adults and trauma patients under 18 years were included in this prospective study. The evaluations were done with the study forms including patient data recorded by the doctors and nurses. Results: The mean age was 42.00±20.8 years, most (37,5%) comprised young adults (20-39). Only 10.1% of patient transfer to hospital was by ambulance. Most attendences were between 12: 00-16: 00 hours. 19.6% of the patients were categorized as Triage 1, signifying the very urgent group. were Evaluation by the physician of 76.4% of the patients was made in the first 5 minutes of attendance. Consultations were required in 39.1% of the patients, mostly from internal medicine, cardiology and orthopedics. Of the patients,12.2% were hospitalized and 4.5% were referred to another hospital. Patients were most frequently admitted to internal medicine, plastic surgery and cardiology clinics. Besides radiography, radiologic investigations were needed ın 12.6% of patients and Cranial Computed Tomography was the most frequently required. The average length of stay was 143.1 minutes for all patients, but this was 585.1 minutes for patients needing hospitalization. Conclusion: It was determined that the lengıh of stay of patients admitted to the hospital is longer than expected. Although there are deficiencies which must be ameliorated, it was concluded that the emergency department of Uludag University Hospital is adequate for a 3rd step emergency. [ABSTRACT FROM AUTHOR]

Çalışmamızda Acil Servise başvuran ve herhangi bir bölüme konsültasyonu yapılan hastaların epidemiyolojik özelliklerinin belirlenmesi ve konsültasyon sürecinin incelenmesi amaçlanmıştır. Çalışmaya 2013 hasta dahil edildi. Hastaların yaş ve cinsiyet özellikleri, ön tanı/tanıları, hastaneye yatış veya sevk ve mortalite oranları gibi epidemiyolojik özellikleri ile konsülte edildiği bölümler, acil serviste kalış süreleri ve konsültasyon süreçleri araştırıldı. Hastaların 521 (%25.9)’i 16:00-19:59 saatleri arasında başvurmuş olup, en sık başvuru saatlerinin de bu saatler arasında olduğu saptandı. Acil serviste en sık konsültasyon isteme nedenleri; basit travma(%33.5) ve kardiyo-vasküler hastalıklardı (%9.5). Hastaların %58.6’sı acil servisten taburcu edilirken, %27.9’u değişik kliniklere yatırıldı ve %9.1’i başka bir sağlık kuruluşuna sevk edildi. Acil serviste 8 saatten uzun süre kalan hasta sayısı 407(%20.21) idi. Konsültasyon süresini uzatan nedenler arasında; en sık herhangi bir bölüm tarafınca uygulanan ek tedavi veya tanısal girişimlerin (%2.8) olduğu görüldü. ÖZET: Amaç: Bu çalışmada, acil serviste aşırı yoğunluğun nedenlerini belirlemeyi ve hastaların acil serviste kalış sürelerini azaltmaya yönelik önerilerimizi sunmayı hedefledik. Gereç ve Yöntem: Bir yıllık sürede acil servise başvuran hastaların tıbbi bilgileri incelendi. Hastaların demografik özellikleri, kalış süreleri, tekrar başvuru sayıları ve konsültasyon durumları belirlendi. Bulgular: 1 Ocak 2013 ile 31 Aralık 2013 tarihleri arasında toplam 163951 hasta acil servise başvurdu. Bu süre içinde, 1210 hasta 24 saat içerisinde tekrar acile başvurdu. Toplam 38579 hasta tedavisini acil servisin gözlem odasında aldı ve ortalama kalış süresi 164.1 dakikaydı. En fazla konsültasyon istenen bölüm kardiyoloji idi. Konsültanların acil servise varış süresi ortalama 64 dakikaydı. Sonuç: Dünyanın diğer bölgelerindeki acil servislere benzer şekilde, acilde aşırı yoğunluğun en önemli nedenleri acil serviste uzun kalış süresi, gecikmiş laboratuvar ve görüntüleme testleri, konsültanların gecikmesi ve yeterli hastane yatağı olmamasıdır. Hataları en aza indirmek ve memnuniyet oranını artırmak için, ilgili farklı birimlerle temas halinde, bazı sert önlemler alınmalıdır. SUMMARY: Objectives: The purpose of this study is to determine the impact of the expected increase in the volume of patient visits in the emergency department during holiday periods on physicians' tendencies regarding test and consultation requests as well as on the length of time patients stay in the emergency department. Methods: The study groups included all of the patients who visited the emergency department during the nine-day public holiday (Eid al-Adha, a religious festival of sacrifice) celebrations and a nine-day non-holiday “normal” period. The patients' demographic information, reasons for their visits, comorbid diseases, whether or not they had undergone laboratory and screening tests, consultations, length of stay, and the way their visits ended were compared statistically. Results: Of the 6353 patients enrolled in the study, 3523 (55.5%) were seen in the emergency department during the holiday period, while 2830 (45.5%) were seen during the non-holiday period (p≤0.001). During the holiday period, there was a 1.9% decrease in laboratory test requests (p=0.108), a 7.7% increase in radiology examination requests (p≤0.001), and a 1.2% increase in consultation requests (p=0.063). The patients' length of stay during the holiday period was 55.9±75.3 minutes and was 56.3±71.9 minutes during the non-holiday period (p=0.819). The length of time for the patients who underwent tests or consultations was 88.6±92.8 minutes during the holiday period and 92.6±87.5 minutes during the non-holiday period (p=0.224). Conclusions: As expected, the number of patient visits to the emergency department increased during the holiday period, but this increase did not lead to a similar increase in test and consultation requests by the physicians, except for radiology examination requests. In addition, the length of time that patients stayed in the emergency department was not affected by the increase in the volume of patient visits during the holiday period. ÖZET: Amaç: Çalışmamızda uzun tatillerde acil servis başvurularında artış beklentisinin, hekimlerin inceleme ya da konsültasyon isteme yönelimine ve acil serviste hastaların kalış süresine etkisi olup olmadığını ortaya çıkarmayı amaçladık. Gereç ve Yöntem: Çalışmamızda tatil dönemi olarak dokuz günlük Kurban Bayramı tatili kabul edilirken, karşılaştırma grubu ise dokuz günlük tatil dışı bir dönemde acil servise başvuran tüm hastalardan oluşturuldu. Her iki dönemde acil servise başvuran hastalar demografik bilgileri, başvuru nedenleri, komorbid hastalıkları, laboratuvar ve görüntüleme incelemesi yapılıp yapılmadığı, konsültasyon istemleri, acil serviste kalış süreleri ve sonuçlanma şekilleri bakımından karşılaştırıldı. Bulgular: Tatil döneminde 3523 (%55.5), tatil dışı dönemde 2830 (%45.5) olmak üzere toplam 6353 acil servis başvurusu kaydedildi (p≤0.001). Tatil dışı döneme göre tatil döneminde laboratuvar inceleme istemlerinde %1.9 oranında azalma (p=0.108), radyolojik inceleme kullanımında %7.7 oranında artış (p≤0.001), konsültasyon istemlerinde %1.2 oranında artış (p=0.063) saptadık. Acil serviste ortalama kalış süreleri tatil döneminde 55.9±75.3 dakika, tatil dışı dönemde 56.3±71.9 dakika olarak bulundu (p=0.819). İnceleme veya konsültasyon yapılan hastaların ortalama kalış süresini tatil döneminde (88.6±92.8 dakika) tatil dışı döneme (92.6±87.5 dakika) göre daha kısa saptadık (p=0.224). Sonuç: Uzun süreli tatillerde acil servislerde beklendiği gibi hasta yoğunluğu artmaktadır. Bu yoğunluk, hekimlerin laboratuvar incelemesi ve konsültasyon istemlerinde artışa yol açmazken, radyolojik inceleme istemlerinde artış gözlenmiştir. Ayrıca acil serviste ortalama kalış süresi başvuru sayısındaki artıştan etkilenmemektedir.
