The influence of auditory and/or visual information on the neuromuscular control of chewing a crispy food was investigated. Participants chewed biscuits of three different levels of crispness under four experimental conditions: no masking, auditory masking, visual masking, and auditory plus visual masking. The order of the four masking condition blocks was randomized. The sound of chewing was masked by loud sounds on a headphone, and visual masking of the food was achieved by closing the eyes. Mechanical tests were performed on the biscuits to determine their characteristics, yield force, and sound production. Skull vibration, jaw-muscle activity, and jaw movement were measured while the subjects chewed and swallowed the food. Auditory and/or visual masking did not have a significant effect on skull vibration, muscle activity, and number of chewing cycles until swallowing. However, auditory and/or visual masking significantly increased the chewing cycle duration, but only for the participants who started the experiments with auditory and/or visual masking. The other participants were not influenced by masking. The memory of the unmodified stimuli helped these subjects to maintain their habitual chewing rate in later trials.

This paper describes a new protocol that addresses the question of whether, in human experiments, modulatory effects of remote nociceptive conditioning stimuli on reflex responses are mediated by the stress induced by the conditioning stimuli. The protocol has been illustrated by a study into the effect of a remote nociceptive conditioning stimulus on an inhibitory jaw reflex. Electromyograms were recorded from an active masseter muscle and inhibitory reflexes were evoked by applying electrical stimuli to the upper lip. This protocol utilised the application of discrete electrical conditioning stimuli applied to the sural nerve prior to the test stimulus. A preliminary experiment determined that the optimal interval between the conditioning and test stimuli, which produced modulatory effects was 100 ms. In the definitive study, computer software was used to deliver control and conditioned sweeps in a double-blind randomised sequence. This resulted in a "stress-equal" protocol in which the level of stress would be the same for both control and conditioned sweeps. Therefore any observed modulatory effects on the reflexes could not have been wholly secondary to stress. This protocol could be adapted to the study of the modulation of other reflexes or evoked sensations by nociceptive conditioning stimuli.

The purpose of the study was to investigate whether jaw reflexes evoked by selective stimulation of periodontal ligament me canoreceptors are susceptible to modulation by remote noxious stimulation. Experiments were performed on 10 volunteer subjects. Skin surface recordings were made from the jaw-closing masseter muscle. The subjects activated the muscle to approximately 10% of maximum by biting on a rubber impression of their molar teeth while they received visual feedback of the electromyogram (EMG) of the muscle. Reflexes were produced by the application of gentle mechanical stimuli to an upper central incisor tooth. The stimuli were in the form of 'ramp and hold' forces with a 5 ms rise-time and a 1.5 N plateau which lasted 350 ma. The resulting reflexes were recorded both under control conditions and while the subjects received a remote noxious stimulus (immersion of a hand in water at 3 degrees C). In all 10 subjects, the stimuli produced a single period of inhibition of masseteric activity (latency, 12.8 t 04 ms; duration, 18.1+/-1.3 ms; means +/- S.E.M.), which was usually followed by a period of increased masseteric activity. The period of inhibition constituted a downward wave in full-wave rectified, averaged signals. The integrals of such waves were significantly smaller (by 17+/- 6.5 %; P = 0.027; Student's t test) when the reflex was evoked during remote noxious stimulation rather than under control conditions. As such reflexes are beLieved to play a modulatory role during normal oral function this finding maybe relevant to disorders of mastication associated with pain.

Mastication was evaluated in patients with bulbar myasthenia gravis and compared with that of patients with ocular myasthenia gravis, patients in remission who previously suffered from bulbar symptoms, and healthy controls. Bulbar myasthenia gravis may impair mastication due to weakness of the masticatory muscles. The aim of the study was to objectively evaluate the influence of myasthenia gravis on mastication. The subjects chewed a piece of breakfast cake and chewed 1 min on a piece of chewing gum. Surface EMG of the masseter muscle, temporalis muscle and jaw opener muscles was recorded. Statistical analysis revealed that bulbar patients produced significantly less EMG activity in the closing phase of a chewing cycle in both experiments. The EMG of the masseter muscle expressed as percentage of the maximum EMG during maximal clenching showed significantly higher values in the bulbar group than in the other groups. This was not found for the temporalis muscle. It was suggested that bulbar patients use a strategy of limited effort to produce a bolus that can be swallowed. The ocular patients and the patients in remission showed no subclinical impairments in muscle function during chewing.

Successive inhibitory, excitatory, inhibitory and excitatory reflexes (the Q, R, S and T waves of the post-stimulus electromyographic complex (PSEC)), evoked by applying non-painful taps to an incisor tooth, were recorded from the jaw-closing muscles of 15 subjects. The effects on these reflexes of the subjects undertaking mental exercises (MEx) in the form of arithmetic calculations were compared with those of remote noxious stimulation (RNS; application of 3 degrees C to a hand). This was done to investigate whether the previously established effects of RNS were likely to be related to a change in the subject's mental state and/or to direct nociceptive mechanisms. Both MEx and RNS caused increases in EMG activity around the Q-R and S-T transitions of the PSEC, which resulted principally from shortenings of the inhibitory Q and S waves. Reducing the intensity of the tap stimuli, which mimicked condition-induced disinhibition, caused shortenings of the inhibitory waves at latencies similar to the shortenings induced by MEx or RNS. Masticatory muscle strength was quantified in patients with bulbar myasthenia gravis and compared with that of patients with ocular myasthenia gravis, patients in clinical remission (whether or not pharmacological) who previously suffered from bulbar myasthenia gravis, and healthy subjects. Maximal bite force and maximal activity of the masseter and temporalis muscles and of the submental muscle complex were measured. Bite force was decreased in the patients with bulbar myasthenia gravis, but was normal in the patients in the clinical remission group and in the ocular group. These findings were consistent with the results of electromyographic data. Although subjective reports of masticatory muscle weakness provide valuable information, quantitative measurements provide more information about the degree of muscle weakness of individual muscles. This is especially important for longitudinal evaluation of therapy in individual patients and for pharmacotherapeutic research.

In man, the principal exteroceptive reflexes evoked by intra-oral stimulation involve the jaw-closing muscles and include inhibitory and excitatory responses [H.W. van der Glas, A. De Laat, D. van Steenberghe, Oral pressure receptors mediate a series of inhibitory and excitatory periods in the masseteric post-stimulus EMG complex following tapping of a tooth in man, Brain Res. 337 (1985) 117-125.]. These reflexes can be observed in electromyograms (EMGs) recorded with bipolar surface electrodes. The likelihood that these reflexes play important roles in the integrative actions of the jaw has led to interest in the physiological control mechanisms by which they may be modulated. It has been reported recently that the complex series of jaw reflexes evoked by non-painful tapping on human teeth can be modulated by the application of noxious stimulation to the hand [S.W. Cadden, H.W. van der Glas, F. Lobbezoo, A. van der Bilt, Effects of remote noxious stimulation on exteroceptive reflexes in human jaw closing muscles, Brain Res. 726 (1996) 189-197.] or by exercises which produce a change in mental state [S.W. Cadden, H.W. van der Glas, F. Lobbezoo, A. van der Bilt, The influence of attentional factors on short- and long-latency jaw reflexes in man, Arch. Oral Biol. 41 (1996) 995-998.]. The effects of remote noxious stimuli and mental exercises usually involved transient increases in electromyographic (EMG) activity around the interfaces between the successive inhibitory and excitatory reflexes. As the mechanisms underlying the tap-induced inhibitory and excitatory reflexes may show some temporal overlap [H.W. van der Glas, A. De Laat, D. van Steenberghe, Oral pressure receptors mediate a series of inhibitory and excitatory periods in the masseteric post-stimulus EMG complex following tapping of a tooth in man, Brain Res. 337 (1985) 117-125.], these condition-induced increases in EMG activity could, in the simplest hypothesis, have been due to either (i) a condition-induced inhibition of the tap-induced inhibitory influences on the motoneurones (i.e., disinhibition) and/or (ii) a condition-induced facilitation of the tap-induced excitatory influences underlying the subsequent excitatory reflexes. In the present protocol, we describe how it is possible to differentiate between these different underlying mechanisms. The method includes a regression analysis of the relationship between condition-induced changes in amplitude of a reflex and the reflex amplitude under control conditions after taking account of the effect of chance. The analysis is applied on reflex data pooled from various subjects. Although this method of data analysis is illustrated with trigeminal reflexes, it is potentially of use for other complex extracellular recordings including those in other fields of motor control (e.g., EMGs from muscles other than jaw ones).

Skinfold measurements by means of calipers and the use of linear regression functions, representing relationships between maximal EMG activity and skinfold thickness, provide a simple and non-invasive procedure to diminish the influence of the thickness of the subcutaneous tissues overlying jaw-elevator muscles on electromyographic data of the maximal voluntary contraction (MVC) level. This method, which allows an improved comparison between different subject or muscle groups in terms of neuromuscular capacity, has been applied to MVC values from the masseter and the anterior temporal muscles (surface EMG) of 21 healthy males, 14 healthy females, and 14 females suffering from myogenous temporomandibular disorders (TMD). Non-corrected MVC values from both muscle groups were larger for the male controls than for the female controls. As the skinfold thickness was smaller for males than for females, these MVC levels did not differ significantly after correction for skinfold thickness. As the skinfold thickness was very similar for the female TMD patients and the controls, the correction method did not appreciably change the lower MVC values of the TMD patients.

The aims of this study were to establish a safe technique for selective stimulation of nerves in human tooth-pulp during long experiments and to validate its use even with stimuli of high intensities.A custom-made veneer containing 2 silver wire-conductive cream electrodes was attached with cement to the labial surface of an upper central incisor tooth. A variety of stimulus intensities were applied, and sensory and reflex responses from jaw-closing muscles were recorded.In 15 participants, the stimuli evoked predominantly sharp or painful sensations and reflex inhibitions of activity in the jaw muscles. Stimulation of 3 non-vital teeth evoked no sensations or reflexes, even at intensities that evoked maximal reflexes in vital teeth. The electrodes had reasonably stable resistances throughout experiments lasting up to 90 min.The method described enables responses to low- or high-intensity stimulation of human pulpal nerves to be investigated in long experiments.

Maximum voluntary bite force has often been studied as an indicator of the functional state of the masticatory system. Bilateral, as well as unilateral, methods have been used to determine bite force. Only a few studies have compared the outcomes of both methods. The aim of this study was to measure bite force and jaw-muscle activity during bilateral as well as unilateral maximum clenching in a large number of healthy subjects, so that the results could be compared. In a group of 81 dentate subjects we observed an average bilateral bite force of 569 N. The average unilateral bite force was significantly lower, being 430 N (right) and 429 N (left). Masseter and anterior temporal muscle activities were also significantly lower during unilateral clenching as compared with bilateral clenching. The masseter muscles showed no difference in activity between the ipsilateral side and the contralateral side during unilateral clenching. In contrast, the activity of the anterior temporal muscle on the ipsilateral side was significantly higher than on the contralateral side. Thus, the change in the forces acting on the jaw during unilateral clenching compared with bilateral clenching leads to a different response in the temporal muscles than in the masseter muscles.

In human beings, inhibitory jaw reflexes can be depressed by painful stimulation of remote parts of the body. Since similar effects can be produced by the stress of anticipating pain, we wished to investigate whether the effects of remote painful stimuli are dependent on stress. EMG recordings were made from a masseter muscle while subjects maintained activity in the muscle at approximately 12.5% of maximum using visual feedback. The protocols involved three sequences: (1) "standard controls" in which reflexes were evoked by electrical test stimuli applied to the upper lip; (2) "standard conditioning" in which painful electrical conditioning stimuli were applied over the sural nerve 100 ms before the test stimuli; (3) "random sequences" in which test-only and conditioning-test combinations were employed in a double-blind, random, order. Data are presented as means +/- SEMs. In the standard controls, the stimuli evoked clear inhibitory reflexes (latency 37 +/- 1.3 ms, duration 62 +/- 5.6 ms; n = 10) in all the subjects. During standard conditioning, the reflex magnitude was reduced significantly (by 50.0 +/- 8.5%, P = 0.0002, one-sample t-test). When the test-only and conditioning-test responses were extracted from the random sequences, there was also a significant reduction in the reflex magnitude following conditioning (by 34.6 +/- 5.5%, P = 0.0002, one-sample t-test) albeit less so than between the standard sequences (P = 0.03, paired t-test). A second series of experiments suggested that these lesser effects during the random sequences were not substantially due to any loss of temporal summation of the conditioning mechanisms. The evidence for this was that application of pairs of conditioning stimuli did not produce a significantly greater effect than single conditioning stimuli within a random sequence (39.9 +/- 9.6% as opposed to 32.7 +/- 9.1% reductions in the reflex, P = 0.117, paired t-test). Therefore since any stress in the random sequences would not have been "tied" to the conditioned responses alone, the effects of remote painful stimuli on this inhibitory jaw reflex cannot be entirely secondary to stress.

Because loading during chewing is not totally predictable and jaw-closing muscles are strong and act over short distances, feedback from oral receptors is important in the control of mastication. Information on such feedback can be obtained by studying reflexes in jaw muscle EMGs. This review will deal with the contribution of reflex mechanisms to modifying motor neuron activity during chewing, and the dependency of reflex sensitivity on motor task, phase of movement, and site of stimulation.

Purpose Mandibular retrognathia is a dentofacial deformity that can be surgically corrected. The purpose of this study was to evaluate the influence of orthognathic surgery on masticatory function in a sample of retrognathic patients and to compare these findings with those of controls. Patients and methods Eleven retrognathic patients were tested before and 1 to 1.5 years after mandibular advancement surgery and compared with 12 controls. The median particle size after chewing a silicon rubber test food, the maximum bite force, and the electromyographic activity (EMG) of the anterior temporalis and the masseter muscles during isometric clenching and during chewing were determined. Patients, before and after treatment, and controls were statistically compared by analysis of variance. Results Surgical correction of mandibular retrognathia did not change chewing efficiency, maximum bite force, EMG during maximal clenching, EMG during chewing, or the EMG/bite-force relationship. Compared with controls, the chewing efficiency, maximum bite force, EMG during maximal clenching, and EMG during chewing values were lower. No difference for the EMG/bite-force ratio at maximal clenching was found, indicating similar muscle efficiency for patients and controls. However, in the range of 10% to 40% of the maximum bite force, the slope of the EMG/bite-force regression line was steeper for the patients than for the controls, indicating decreased muscle efficiency for patients. Conclusions The results of this study suggest that in retrognathic patients, function of the masticatory system is impaired. Oral function was not influenced by mandibular advancement surgery.

Poststimulus EMG complexes (PSECs), consisting of a series of inhibitory and excitatory waves in full-wave rectified and averaged electromyogram (EMG), were elicited in the masseter muscles of 7 subjects following controlled tapping of a tooth, at a controlled clenching level. Applying local anaesthesia to this tooth decreased the total surface of the waves, on average by 89%. The excitatory and the inhibitory waves were similarly affected, indicating that mainly pressure receptors in the periodontium mediate the entire PSEC. In 4 subjects, who were exposed to acoustic noise to exclude a contribution of acoustic receptors, the recovery of the PSEC waves from local anaesthesia was tracked. In 3 subjects, one wave (the first inhibitory or the first excitatory one, respectively) recovered differently from the other waves, indicating that they are not necessarily mediated by one type of afferent axons. The evidence, nevertheless, suggests that the different PSEC waves in man reflect the projection of the periodontal afferents upon several brain structures, involved in the control of the activity of the masseteric motorneurones, as: (1) inhibitory and excitatory control requires different groups of interneurones; and (2) a mediation of the first inhibitory wave by slower conducting axons than the second inhibitory wave, or a mediation of both waves by axons of similar type, is not compatible with common interneurones.

Post-stimulus electromyogram (EMG) complexes (PSECs) were studied in the full-wave rectified and averaged EMGs of the masseter muscles in 15 subjects, who clenched at a controlled level. The PSECs, a series of downward- and upward-going waves reflecting inhibitory and excitatory influences upon the masseteric motoneurones, were elicited by mechanical stimulation of a tooth. The stumuli selectively activated mechanoreceptors in the periodontium and, by bone-conduction, acoustic receptors. Application of acoustic masking during the periods of stimulation revealed a series of inhibitory and excitatory acoustic influences in the PSEC, which were absent after local electrical stimulation of receptors in the periodontium or their afferents. By applying local anaesthesia to the periodontium of a mechanically stimulated tooth, the durations of the acoustic influences were on the average reduced by 76%. In subjects whose PSECs consistently included a second inhibitory period, the duration of the acoustic influences with respect to that of the PSEC (30%) was larger than otherwise (13%), suggesting a central gating of periodontal pathways which can block both periodontal and acoustic influences. The acoustic influences, of which the appearance in the PSEC largely depends upon activated periodontal pathways, represent a new finding of audio-motor reflexes.
