Case Description: In Latvia in 2014, acquired idiopathic megaesophagus (AIME) was observed in increased numbers of dogs that consumed varieties of 1 brand of dog food. Within 2 years, 253 dogs were affected. In Australia in November 2017, 6 working dogs that consumed 1 diet of another brand of dog food developed AIME. In total, 145 Australian dogs were affected., Clinical Findings: AIME was diagnosed predominantly in large-breed male dogs (> 25 kg [55 lb]). Regurgitation, weight loss, and occasionally signs consistent with aspiration pneumonia (coughing, dyspnea, or fever) were noted. Most Latvian dogs had mild to severe peripheral polyneuropathies as evidenced by laryngeal paralysis, dysphonia, weakness, and histopathologic findings consistent with distal axonopathy. In Australian dogs, peripheral polyneuropathies were not identified, and histopathologic findings suggested that the innervation of the esophagus and pharynx was disrupted locally, although limited samples were available., Treatment and Outcome: Investigations in both countries included clinical, epidemiological, neuropathologic, and case-control studies. Strong associations between the dog foods and the presence of AIME were confirmed; however, toxicological analyses did not identify a root cause. In Latvia, the implicated dietary ingredients and formulations were unknown, whereas in Australia, extensive investigations were conducted into the food, its ingredients, the supply chain, and the manufacturing facilities, but a cause was not identified., Clinical Relevance: A panel of international multidisciplinary experts concluded that the cause of AIME in both outbreaks was likely multifactorial, with the possibility of individualized sensitivities. Without a sentinel group, the outbreak in Australia may not have been recognized for months to years, as happened in Latvia. A better surveillance system for early identification of pet illnesses, including those associated with pet foods, is needed.

Objective: To describe the clinical course and novel biochemical findings in 3 dogs with amitraz toxicosis., Case Series Summary: Three Labrador Retrievers developed acute onset obtundation to stupor after being in a rice field. On admittance to the hospital, they all displayed bradycardia, hyperglycemia, hyperlactatemia, respiratory acidosis, and metabolic alkalosis. All clinical signs resolved in 18-48 hours with supportive care. One dog represented with similar clinical signs and biochemical abnormalities 3 days after discharge following spending time in a different rice field owned by the same owner. Toxicological analysis of serum from all 3 dogs and vomitus from 1 dog returned positive for amitraz and one of its metabolites., New or Unique Information Provided: This is the first case series of dogs with confirmed amitraz toxicosis following an environmental exposure. Novel biochemical findings of hyperlactatemia, respiratory acidosis, and metabolic alkalosis were documented in all 3 dogs. Clinicians should be concerned for amitraz toxicosis when presented with an animal with the constellation of signs including decreased mental status, bradycardia, and hyperglycemia, particularly if relevant acid-base abnormalities are also detected., (© Veterinary Emergency and Critical Care Society 2021.)

A female spayed dachshund/mixed-breed dog was evaluated following ingestion of lamotrigine tablets with subsequent rapid onset of vomiting, diarrhea, and generalized tremoring. On initial examination, the dog was moderately obtunded and nonambulatory with intermittent myoclonus and hyperesthesia. Electrocardiogram revealed sinus tachycardia with prolongation of the QT interval. Intravenous lipid emulsion (ILE) infusion was initiated, with reduction in tremoring and improved patient mentation being noted after ∼20 min of therapy. An elevated cardiac troponin I value measured at 1.02 ng/mL the day after presentation. Serum toxicological assay revealed marked reduction in serum lamotrigine levels following ILE and continued reduction during hospitalization. The dog's clinical signs resolved, corrected QT interval returned to normal, and the patient was discharged 38 hr after presentation. Individual cases of lamotrigine toxicosis have not been fully reported in veterinary literature. This case report documents the rapid onset of clinical signs including neurologic dysfunction, cardiac arrhythmias, and transient corrected QT prolongation. Serial serum concentrations of lamotrigine showed a rapid reduction with ILE therapy and corresponded with clinical recovery, suggesting efficacy of ILE treatment in this case.

In early 2007, American pet food ingredients leading to nephrotoxic renal failure of dogs and cats raised serious concerns about the safety of pet foods. Major pet food companies recalled more than 1,000 commercial pet foods in consideration of pet safety. A similar pet food-associated outbreak of nephrotoxic renal failure occurred in Asia, in late 2003 and 2004, resulting in a similar extensive pet food recall. At that time, contamination of ingredients with a nephrotoxin-producing fungus at a pet food production facility was suspected. However, toxicologic evidence to substantiate a mycotoxicosis was lacking. Moreover, the renal lesions were not typical of those reported with fungal nephrotoxins. During 2003 and 2004, 14 dogs were presented to the Veterinary Medical Teaching Hospital of Konkuk University, Seoul, Korea, with renal failure and distinctive renal pathologic findings. Grossly, the kidneys were greenish in color with greenish uroliths in the renal pelvis or bladder. Histologically, characteristic crystals with pinwheel radiating striations were present in distal tubular segments. Toxicologic analysis identified melamine, cyanuric acid, and ammelide in deparaffinized formalin-fixed kidney samples.

Background: Canine splenic hemangiosarcoma (HSA) is a fatal malignancy, and most affected dogs die within a few months of diagnosis. Most dogs present with signs from tumor rupture, resulting in hemoabdomen and intra-abdominal dissemination. The abdomen is also the main site of disease recurrence., Hypothesis: Intraperitoneal (IP) administration of doxorubicin will delay or prevent intra-abdominal tumor recurrence and prolong survival in dogs with HSA., Animals: Fourteen dogs with splenic HSA., Methods: A prospective, unmasked, uncontrolled clinical trial. After staging of disease status and splenectomy, pegylated liposomal encapsulated doxorubicin was administered intraperitoneally (1 mg/kg body weight) every 3 weeks for 4 cycles. All dogs were monitored for recurrence of HSA. Samples of plasma and abdominal fluid were collected for measurement of doxorubicin concentration and pharmacokinetic analysis. Nonlinear mixed-effect modeling was used to describe the pharmacokinetics of liposomal doxorubicin administered IP., Results: All 14 dogs died, 12 because of HSA and 2 from other causes. Postmortem examination was performed on 12 dogs. All 12 dogs died because of HSA-related causes and had hepatic metastases and hemoabdomen. The IP-treated dogs had fewer serosal, mesenteric, and omental metastases than historical controls treated with systemic doxorubicin. Results of the postmortem examination and pharmacokinetic analysis confirmed that IP delivery of doxorubicin resulted in an effective drug concentration with a clearance comparable with that after i.v. delivery., Conclusions and Clinical Importance: IP pegylated liposomal encapsulated doxorubicin administration did not prevent intraabdominal recurrence of HSA in dogs.

A 5-month-old 22-kg (48.4-lb) sexually intact male Collie was examined after ingesting a moxidectin-containing deworming medication. The dog was comatose and had respiratory arrest after progressively worsening lethargy, ataxia, and seizures. Exposure was confirmed by isolation of moxidectin from a biopsy specimen of adipose tissue, using liquid chromatography-mass spectroscopy methods. Treatment included use of intermittent positive-pressure ventilation, activated charcoal and cathartic administered enterally, nutrients administered via nasogastric tube, and intensive supportive care. The dog was weaned from a ventilator on day 6 after ingestion and was discharged on day 10. The dog was considered clinically normal during examination 24 days after ingestion. On the basis of the dog reported here and toxicologic data provided by the manufacturer of the deworming product, some Collies may have increased susceptibility to products containing high doses of moxidectin.

A 3-year-old German Short-haired Pointer was examined because of extreme agitation, hyperactivity, and vomiting that began within 24 hours after ingestion of approximately 750 mg of pemoline, a CNS stimulant. On physical examination, the dog was agitated, tachycardic, hyper-responsive, pyrectic, disoriented, and had mydriasis. These signs were consistent with excessive stimulation of the CNS and sympathomimetic effects resulting from pemoline toxicosis. Serial blood and urine samples were obtained, and toxicologic analyses were performed. Extrapolation of the plasma pemoline concentration 32 hours after ingestion provided an estimated peak plasma concentration of 368 micrograms/ml, dramatically higher than a therapeutic concentration of 1.7 to 7.0 micrograms/ml reported for children. Several sedatives were administered intravenously to alleviate clinical signs and to allow administration of activated charcoal (PO) and fluids (IV). Clinical signs resolved approximately 72 hours after ingestion of pemoline.

A dog was treated for fleas with the application of pennyroyal oil obtained by the owner at a health food store. Vomiting ensued within 2 hours, and despite emergency treatment, the dog died within 48 hours. At necropsy, pennyroyal oil was determined to be the cause of death.

A litter of 5-week-old Doberman Pinschers with pustular dermatitis was treated dermally with a hexachlorophene-containing emulsion. Shortly after a second treatment, all of the puppies developed neurologic signs consisting of muscle tremors, ataxia, and apparent muscle weakness. The clinical history and signs, histologic lesions within the central nervous system, and measurement of hexachlorophene in liver and kidney tissue confirmed a diagnosis of hexachlorophene toxicosis.
