Objective: To assess the agreement between measurements of total protein (TP) concentrations in canine serum samples between a commercially available veterinary digital refractometer (DR), an analog handheld refractometer (AR), and a laboratory-based chemistry analyzer (LAB). An additional objective was to assess the effects of various potential interferents (ie, hyperbilirubinemia, increased BUN, hyperglycemia, hemolysis, and lipemia) on DR measurements., Sample: 108 canine serum samples., Procedures: Serum samples were measured in duplicate on the DR, which reported TP concentration, assessed via optical reflectance and critical angle measurement. These serum samples were also assessed on the AR and LAB for comparison. Serum samples with grossly visible lipemia, hemolysis, and icterus were noted. Medical records were retrospectively assessed to determine concentrations of BUN, glucose, and bilirubin., Results: Method comparisons among the various data generated by the analyzers were completed using linear regression, Bland Altman, and calculation of intraclass coefficients. Mean bias between DRTP and LABTP in samples without potential interferents was 0.54 g/dL with 95% limits of agreement of -0.17 to 1.27 g/dL. One-third of DRTP samples without potential interferents had > 10% difference from their LABTP comparison. Interferents, particularly marked hyperglycemia, can result in inaccurate measurements on the DR., Clinical Relevance: There was a statistically significant difference between DRTP and LABTP measurements. TP measurements in samples with any potential interferent, particularly hyperglycemia, should be assessed cautiously on DR and AR.

Background: Measurement of pancreatic lipase in serum is being used for the diagnosis of pancreatitis., Objectives: The aims of this study were to measure serum lipase activity by the v-LIP-P slide and serum canine pancreatic lipase immunoreactivity (cPLI) concentration by Spec cPL in dogs with exocrine pancreatic insufficiency (EPI) and in serum samples that were hemolyzed, lipemic, or icteric., Methods: Serum samples from 50 dogs with EPI, 8 samples spiked with canine hemoglobin, Intralipid, or ditaurobilirubin, and 8 samples from dogs with severe hypertriglyceridemia were evaluated using v-LIP-P and Spec cPL assays., Results: Serum lipase activity (v-LIP-P) was in the lower 20% of the RI in 58% of EPI dogs, while serum cPLI (as measured by Spec cPL) was in the lower 20% of the RI in 98% of EPI dogs. The mean (±SD) observed-to-expected ratios for serum samples spiked with canine hemoglobin, Intralipid, or ditaurobilirubin were 63.0 ± 25.1%, 489.2 ± 469.7%, and 80.2 ± 11.6% for the v-LIP-P slide, respectively, and 99.3 ± 6.8%, 96.9 ± 9.4%, and 98.7 ± 11.0% for Spec cPL, respectively. However, naturally occurring hypertriglyceridemia did not appear to have a significant effect on serum lipase activity as measured by the v-LIP-P slide., Conclusions: These results show that Spec cPL is specific for pancreatic lipase, while the v-LIP-P slide is not. In addition, hemolysis and lipemia had no effect on Spec cPL, while severe icterus resulted in a slight decrease in Spec cPL. Hemolysis and icterus had a significant effect on the v-LIP-P slide. A 10-year-old spayed female Miniature Poodle was presented to the University of Georgia veterinary teaching hospital for evaluation of lethargy, vomiting and anorexia of 4 days' duration. Physical examination, history and a minimum database led to a diagnosis of immune-mediated hemolytic anemia accompanied by marked hyperbilirubinemia. Refractometric protein determination was within the reference interval, whereas the biuret method indicated hypoproteinemia. This discrepancy was attributed to interference of bilirubin and biliverdin with the spectrophotometric read-out of the biuret total protein assay. Medical records were reviewed of 24 dogs and four cats that underwent cholecystoduodenostomy to relieve extrahepatic biliary obstruction. These procedures had been performed using a 30-mm endoscopic gastrointestinal anastomosis stapler. At presentation, most animals had clinical signs of vomiting and anorexia, and total bilirubin was elevated in both dogs (n=21) and cats (n=4). Pancreatitis (n=13), cholangiohepatitis (n=7), and neoplasia (n=6) were the most common underlying conditions. Sixteen dogs and two cats survived to their 2-week reevaluation. The most common cause of death was euthanasia (n=9) secondary to neoplasia (n=4), peritonitis (n=3), or respiratory arrest (n=2).

A dog developed icterus, vomiting, and anorexia 2 wk after orthopedic surgery and treatment with meloxicam for approximately 1 y. Exploratory laparotomy revealed a single perforated duodenal ulcer. The most likely cause of the hyperbilirubinemia was intrahepatic cholestasis resulting from peritonitis associated with the perforation.

The medical records of 41 dogs and four cats with either surgical or postmortem confirmation of extrahepatic biliary tract obstruction were reviewed. Clinical signs of icterus, vomiting, anorexia, or a combination of these signs were exhibited by 100% of cases. Elevated serum bilirubin was documented in 37 of 41 dogs and two of four cats (86.6%). The predominant, underlying, pathological process was extraluminal obstruction of the biliary tract due to pancreatitis (19 of 45 cases; 42.2%) or neoplasia (11 of 45 cases; 24.4%). Biliary tract surgery was performed in 29 cases, of which 12 (41.3%) recovered. Animals diagnosed with extrahepatic biliary tract obstruction had a relatively good long-term prognosis, provided they were not compromised substantially due to severe necrotizing pancreatitis or neoplasia.

Alloantibodies to high-frequency red cell antigens, defined as inherited traits occurring in 92% to 99% or more of the general population, are recognized as a cause of hemolytic transfusion reactions in humans. Here we describe a dog (dog erythrocyte antigen [DEA] 1.2- and DEA 4-positive) sensitized by prior blood transfusion, for which a compatible blood donor could not be found; transfusion of DEA 1.1-negative blood resulted in hemolytic transfusion reactions. Patient serum from days 1 (before first transfusion) and 16 was available for further testing; using 4 dogs with different blood types as potential donors, the major crossmatches were compatible using serum from day 1. However the crossmatches were all incompatible with serum from day 16, indicating that the patient was sensitized to an antigen after the first transfusion. The presence of an alloantibody against DEA 1.1 was not ruled out in this patient, but the incompatibility reactions of patient serum with red cells from donors negative for DEA 1.1 indicated that an alloantibody against a red cell antigen other than DEA 1.1 or any other known DEA for which typing reagents were available (DEA 3, 5, and 7) was present. Subsequently, red cells from 1 of the patient's siblings (DEA 1.2-, 4-, and 7-positive) were found not to agglutinate when incubated with patient's serum from day 16, ruling out the presence of an anti-DEA 7 antibody, and suggesting that an alloantibody against a common red cell antigen missing in the patient and sibling was responsible for the blood incompatibility reactions.(ABSTRACT TRUNCATED AT 250 WORDS)

Pheyntoin (Epanutin: Parke, Davis) is widely used for the control of chronic convulsive disorders in the dog because it exerts an anti-epileptic effect without causing general depression of the central nervous system. To our knowledge fatal cases of phenytoin sensitivity in the dog have not been reported. A case is described in which hepatitis, jaundice and death followed the administration of the drug.

Various techniques utilized for the clinical investigation of small animal patients with signs of liver disease are discussed. The author reviews the evaluation of survey radiographs and selected radiographic contrast techniques, such as cholecystography. The author also outlines the basic principles of hepatic ultrasonography and includes examples of normal and abnormal hepatic ultrasonograms to demonstrate the application of this new imaging modality to the diagnosis of liver diseases.
