Abstract Background Primary bladder neck obstruction (PBNO) is one of the causes of bladder outlet obstruction (BOO) and rarely results in renal failure. We are presenting the clinical characteristics of young male patients with PBNO and renal failure. Methods Medical records of patients between 18 and 40 years old with PBNO and renal failure were retrospectively reviewed (2014–2020). Patients with anatomical cause of BOO, and any urological or systemic disease or previous history of any surgical procedure associated with renal failure and/or lower urinary tract dysfunction were excluded. Serum creatinine measurement, ultrasonography, uroflowmetry, cystoscopy, and videourodynamics were performed. Results Seven male patients were identified, and the mean age of the patients was 28.8 years. Symptom duration was > 5 years in all patients. Two patients presented with difficult voiding, and five patients presented with both storage and voiding lower urinary tract symptoms (LUTS). Three patients were previously misdiagnosed as overactive bladder. At presentation, serum creatinine levels were between 1.7 and 2.4 mg/dl. One patient was under hemodialysis treatment and waiting for renal transplantation. Mean detrusor pressure at maximum measured flow rate, mean maximum flow rate (Q max), and mean average flow rate (Q ave) was 67.6 cm H2O, 9.5 ml/s, and 5.5 ml/s, respectively. With α-blocker treatment, serum creatinine levels were stable or decreased after 12 months follow-up and mean Q max and Q ave were increased to 14.8 ml/s and 10.1 ml/s, respectively. Conclusions PBNO is a common disease in young men presenting with a long history of LUTS. Videourodynamics is mandatory for accurate diagnosis, but having a high clinical suspicion for PBNO is key to ensure the diagnosis. Clinicians should pay more attention to PBNO in young male patients with a long history of LUTS to prevent misdiagnosis, incorrect treatment, and possible decrease in renal function by years.

Purpose Diabetes mellitus (DM) is a chronic metabolic disorder that often leads to complications. We aimed to correlate two complications of DM, polyneuropathy and hyperactive bladder syndrome, using noninvasive measures, such as screening tests. Methods We included 80 female and 40 male type 2 diabetic patients in this prospective study. Diabetic polyneuropathy evaluations were conducted using the Douleur Neuropathique 4 Questions (DN4), and overactive bladder (OAB) evaluations were performed using the Overactive Bladder Questionnaire (OAB-V8). The patients were also evaluated for retinopathy and nephropathy. The diabetic male and female patients with or without OAB were chosen and compared for microvascular complications (polyneuropathy, retinopathy, and nephropathy). Results There were no significant correlations between OAB and retinopathy as well as between OAB and nephropathy among diabetic patients (female patients, P>0.05; male patients, P>0.05). Penile tourniquet syndrome (PTS), a rare urologic emergency, may lead to undesirable results including necrosis and amputation of penis, if not diagnosed and treated appropriately. Sometimes these injuries may be accepted as a forensic case. Miscellaneous objects used for strangulation can be metallic or nonmetallic. Of all ages, the most vulnerable period is infancy. Telogen effluvium is the most common cause of PTS in infants who are 0–6 years old. In the literature, telogen effluvium as a reason of PTS was not found except for this age group. Therefore, we aimed to present a boy who is 8 years old diagnosed as PTS because of his mother’s hair coil.

PURPOSE: Evaluate the rabbit augmented bladder with PelvicolÒ. MATERIALS AND METHODS: Twenty New Zealand rabbits were divided into 4 groups. Bladder augmentation was performed using a 10 x 10 mm sized porcine acellular collagen matrix. The material was placed on the dome of the bladder wall as a patch with 5-0 polyglycolic sutures. The bladder was resected on the 7th, 14th day, 30th and 90th days, and processed for histological analysis. RESULTS: No stone formation was found in the first, second and fourth weeks. In the first week, there was inflammatory appearance and roughness in the reconstructed area when compared to other sites on the bladder wall. The material could not be seen in some bladders because of acute inflammatory reaction. The normal bladder epithelium was found on the part of the bladder wall that follows the surface of the eroded material. In the second week, edema was observed through the bladder wall. Perivesical fat tissue increased and it was not easy to distinguish it from the surrounding area. In the fourth week, the bladder wall was thickened and there was a sensation of hardness present. The inner and outer surface of the material was darker than in the other bladders. In the third month, there was no inflammatory reaction; however, there was micro calcification and irregular detrusor regeneration. CONCLUSIONS: PelvicolÒ cannot be suitable material for bladder augmentation because of the resultant micro calcification, thickening of the bladder wall and irregular development of detrusor regeneration.

Objective:We evaluated the success rate and complications of sacral neuromodulation (SNM) in patients with non-neurogenic urological disorders.Materials and Methods:We retrospectively evaluated patients with an overactive bladder (OAB), bladder pain syndrome/interstitial cystitis (BPS/IC), and idiopathic non-obstructive urinary retention (IUR), who underwent SNM between 2015 and 2020. SNM was recommended for patients with OAB and BPS/IC who previously had unsuccessful conservative and medical therapies and botulinum toxin injections. Success was defined as more than 50% improvement in clinical symptoms or voiding diary parameters in patients with OAB; more than 50% improvement in storage symptoms or subjective pain improvement or improvement after pain medications in patients with BPS/IC; more than 50% reduction in urethral catheterization rate in patients with IUR. We reviewed the success rates and complications.Results:Twenty-four patients underwent the first stage of SNM and 16 patients (66.6%) received permanent implantation. Ten patients were female (62.5%) and six were male (37.5%). The mean age was 36.9 years. Seven patients (43.7%) had OAB, three patients (18.7%) had BPS/IC, and six patients (37.5%) had IUR. After a mean follow-up of 42.3 months, the overall success rate was 87.5% for all indications. The success rate was 100%, 100%, and 66.7% for OAB, BPS/IC, and IUR, respectively. Four patients underwent surgical reintervention: two had their devices removed due to failure (50%), one had their implantable pulse generator (IPG) repositioned due to serious pain (25%), and one changed IPG due to malfunction (25%).Conclusion:SNM is a safe and effective minimally invasive therapy for patients with non-neurogenic urological disorders.

Objective:This study aimed to evaluate the sleep quality and quantity of patients with benign prostate hyperplasia (BPH) and compare them with that of a control group using the Medical Outcomes Study-sleep scale (MOS-SS).Materials and Methods:The study included 114 consecutive men who were recruited between 2014 and 2018. Voiding patterns of patients with BPH were evaluated by free uroflowmetry, and symptom scores were evaluated using the International Prostate Symptom score (IPSS). Sleep quality and quantity of all patients were evaluated using the MOS-SS questionnaire. The participants were divided into two groups: 57 BPH patients (group 1) and 57 healthy individuals (group 2). They were compared statistically in terms of MOS-SS subdomains. The relationship between MOS-SS subdomains and IPSS, free uroflowmetry parameters, and post-voiding residual urine volume (PVR) was evaluated in BPH patients. Objective:Every day, many patients visit hospital due to hematuria. Ultrasonography and/or cystoscopy are performed in the initial evaluation and management. In this study, we compared ultrasonography and cystoscopy in the evaluation of microscopic or macroscopic hematuria.Materials and Methods:A total of 55 patients, who presented to our clinic with the complaint of hematuria between July 2016 and October 2017, were enrolled in this study. After obtaining informed consent, the patients were directed to urinary ultrasonography and cystoscopy for the evaluation of hematuria.Results:Ultrasonography showed 45 (81.8%) normal bladder and 10 (18.2%) masses, and cystoscopy detected 39 (70.9%) normal bladder and 16 (29.1%) masses in the bladder (p=0.001). Ultrasonography was able to report only 8 (50%) of 16 masses detected via cystoscopy. Two (20%) of 10 masses reported by ultrasonography were not confirmed through cystoscopy. The sensitivity and specificity of ultrasonography in detecting and excluding masses in the bladder were calculated to be 50% and 94.9%, respectively. Ultrasonography failed to detect lesions at the posterior, dome and right side and bladder neck. The cut-off value for blood cell count in urine to refer the patient to a cystoscopy procedure was detected to be 15 with 60% sensitivity and 50% specificity.Conclusion:With low sensitivity, ultrasonography could not offer enough knowledge about the bladder masses as sufficient as cystoscopy.
