Background: Abnormal peak plantar pressure in neuropathic diabetic foot during walking activities is well managed through the use of appropriate design and material selection for the fabrication of custom made insoles (CMI). The redistribution of plantar pressure is possible by selecting an appropriate material for the fabrication of CMI. The walking activities may alter the plantar pressure distribution; which may differ while using CMI with different materials., Objective: The objective of the study was to evaluate the effectiveness of CMI's materials on plantar pressure distribution during different walking activities, in diabetic feet with neuropathy., Methods: The study was conducted on sixteen diabetic neuropathic subjects. The subjects were provided with two types of CMI; CMI-A (Plastazote® and microcellular rubber) and CMI-B (Multifoam, Plastazote® and microcellular rubber). Maximum peak plantar pressure and plantar pressure distribution were determined by Pedar-X® sensor insole during level walking, ramp walking and stair walking., Results: The CMI-B lessened the maximum peak plantar pressure from the forefoot throughout the walking activities compared to CMI-A. The contact area was observed as lower using CMI-A compared to CMI-B, while performing walking activities., Conclusion: CMI-B, with multifoam as an additional top layer, provided more effective peak plantar pressure reduction at forefoot and it had better plantar pressure distribution compared to CMI-A during level walking and ramp ascending in diabetic foot with neuropathy., (Copyright © 2019 Elsevier B.V. Background: Using a total contact orthosis (TCO) is an effective method to offload in diabetic patients with foot neuropathy. However, the redistribution of peak plantar pressure is mostly observed during level walking, which may differ from other walking activities. The aim of this study was to investigate the plantar pressure from 4 regions of the foot during different walking activities (level walking, ramp ascending, ramp descending, stair ascending, and stair descending) in neuropathic diabetic patients with and without a TCO., Methods: Sixteen neuropathic diabetic patients aged 40 to 60 years with calluses and hallux valgus were included in this study and were provided with TCOs made up of multifoam, Plastazote, and microcellular rubber. The plantar pressure and contact area with the TCO and without the TCO were recorded using the Pedar X system during different walking activities., Results: A significant reduction of plantar pressure during different walking activities at the toes and forefoot regions was observed while walking with the TCO compared with walking without the TCO (control condition). Plantar pressure increased at the midfoot region when walking with the TCO, and no significant difference was observed at the hindfoot region between the control and TCO conditions. Furthermore, maximum contact area was observed during level walking with the TCO compared with other walking activities., Conclusion: The TCO significantly reduced and redistributed the peak plantar pressure from the sites where the ulceration rate is higher at the toes and forefoot compared with the other regions of the foot., Level of Evidence: Therapeutic level II, lesser quality randomized controlled trial.

Custom made insole (CMI) effectively reduces the incidence of foot ulceration and re-ulceration in the diabetic foot with neuropathy. Several factors influence the effectiveness of CMI, which can be studied using the finite element analysis (FEA). This study investigated the effect of varying CMI thickness and length on the peak plantar pressure using FEA. A subjectspecific 3D foot model was reconstructed with the CMI. The top surface of tibia and fibula were fixed and a displacement of 3 mm was exerted from the ground along with the upward Achilles tendon force. The peak contact pressure was reduced by 17.52% and 9.46% with an overall thickness of CMI of 9mm and 6mm compared to 3mm, respectively. Full-length CMI redistributed contact pressure at the midfoot better than half-length CMI. In conclusion, FEA provides more insight for the intervention of CMI on plantar pressure in the diabetic foot with neuropathy. [ABSTRACT FROM AUTHOR]

Featured Application: Finite element analysis to assess the effect of materials used for a custom-made insole to provide information for the suitable material selection in the conventional practice of insole fabrication for the diabetic foot with neuropathy. Offloading plantar pressure in a diabetic foot with neuropathy is challenging in conventional clinical practice. Custom-made insole (CMI) materials play an important role in plantar pressure reduction, but the assessment is costly and time-consuming. Finite element analysis (FEA) can provide an efficient evaluation of different insoles on the plantar pressure distribution. This study investigated the effect of CMI materials and their combinations on plantar pressure reduction for the diabetic foot with neuropathy using FEA. The study was conducted by constructing a three-dimensional foot model along with CMI to study the peak contact pressure between the foot and CMI. The softer material (E = 5 MPa) resulted in a better reduction of peak contact pressure compared with the stiffer material (E = 11 MPa). The plantar pressure was well redistributed with softer material compared with the stiffer material and its combination. In addition, the single softer material resulted in reduced frictional stress under the first metatarsal head compared with the stiffer material and the combination of materials. The softer material and its combination have a beneficial effect on plantar pressure reduction and redistribution for a diabetic foot with neuropathy. To reduce the trial and error in a real clinical scenario, the finite element analysis (FEA) can be effectively used to simulate various effective pad designs and a material selection to reduce and redistribute peak plantar pressure in a diabetic foot with neuropathy. The aim of this study was to investigate the effect of pad design and material stiffness on the reduction in plantar pressure in a diabetic foot with neuropathy using FEA. Three-dimensional foot models with a customized insole (CMI) were created to study the peak contact pressure. Ethylene vinyl acetate, Nora® Lunalastike, and thermoplastic polyurethane were assigned to the top, middle, and base layers of the CMI, respectively. Two types of pads were proposed: a heel pad and a heel–forefoot pad. Four different materials with different stiffnesses were assigned as pad materials including a void pad. The FEA revealed that pads with soft materials reduced peak plantar pressure more effectively than stiffer pads. The use of a softer heel–forefoot pad reduced the peak plantar pressure at the midfoot and forefoot compared with other pads. The findings suggest that the material and design selection for the fabrication of CMIs with pads are important factors in reducing plantar pressure and may be useful in the management of a neuropathic diabetic foot.
