Context: Recently, it has been shown in the general population that an allele in the adiponutrin (PNPLA3) gene was strongly associated with increased liver fat content (LFC), independently of visceral adiposity and insulin resistance., Objective: In this study, we set out to determine whether LFC, evaluated using (1)H-MR spectroscopy, was associated with PNPLA3 rs738409 polymorphism in people with type 2 diabetes. We also evaluated the influence of this polymorphism on the relationship between LFC and either visceral adiposity or carotid intima media thickness (CIMT)., Design, Settings, and Participants: A total of 218 type 2 diabetic patients were included in this study., Main Outcome Measures: LFC, area of visceral fat, and CIMT were measured., Results: A total of 139 (63.7%) patients had steatosis. The rs738409 minor G allele was associated with LFC. The number of patients with steatosis was significantly higher among minor G allele carriers in comparison to C allele homozygote carriers (70.3 vs. 57.2%; P=0.04) In the subgroup of C allele homozygote carriers, LFC correlated with body mass index (r=0.27; P=0.003) and visceral fat area (r=0.30; P=0.002), but not with CIMT. In the subgroup of minor G allele carriers, LFC correlated inversely with CIMT (r=-0.23; P=0.03), but not with body mass index or with visceral fat area. In multivariate logistic regression, the relationship between the highest quartile of CIMT and steatosis was different according to adiponutrin polymorphism., Conclusions: This study confirms that in people with type 2 diabetes, LFC is related to rs738409 polymorphism. The lack of a relationship with visceral obesity and the inverse correlation with CIMT suggest that fatty liver associated with the minor G allele of the PNPLA3 rs738409 polymorphism may not be linked to metabolic disorders.

Objective: This study was designed to assess the prognostic value of myocardial tomoscintigraphy perfusion imaging after percutaneous coronary intervention (PCI) in asymptomatic diabetic patients., Methods: One hundred and fourteen diabetic patients were followed up during 27+/-16 (mean+/-SD) months after the myocardial tomoscintigraphy. PCI-related events were studied after myocardial tomoscintigraphy stress testing and included major cardiac events (MACE) (cardiovascular death, myocardial infarction) and revascularization (bypass surgery or new PCI). Stress myocardial tomoscintigraphy imaging was performed 5+/-5 months after PCI and ischemia was considered as present if at least 2 contiguous segments were showing reversible defects., Results: Persistent silent ischemia was found in 49/114 (43%) patients. No difference was observed between the two groups for MACE: four among the 65 (6%) non ischemic patients versus 2 among the 49 (4%) ischemic patients (NS). In contrast, 15 (31%) among the ischemic patients and 4 (6%) among the non ischemic patients underwent iterative revascularization (p<0.01). The relative risk of revascularization for patients with significant ischemia was 5.5 versus non ischemic patients (p<0.001)., Conclusion: After PCI, in asymptomatic diabetic patients followed by myocardial tomoscintigraphy a high frequency of persistent silent ischemia was found and associated with a high risk for repeat interventional procedure, although no increase in major cardiac events was observed.

An increased prevalence of hepatitis C virus (HCV) infection in patients with diabetes mellitus has suggested a link between these two conditions and the possibility of patient-to-patient HCV transmission during hospital admissions in diabetes units. We investigated the prevalence of HCV antibodies in 259 patients with diabetes mellitus consecutively admitted to our diabetic unit in 1998. The control group was composed of 14,100 volunteer blood donors. We divided the diabetic patients into two groups according to their HCV antibody status and also analysed patients for the following variables: age, disease duration, diabetes treatment, previous hospital admissions in a diabetes unit and use of finger stick devices. Anti-HCV antibodies were detected in 8 diabetic patients and 6 blood donors (3.09% vs 0.04%, p < 0.001). No differences were observed between anti-HCV-positive and anti-HCV-negative diabetic patients in terms of mode of treatment, previous hospital admissions in a diabetic unit and use of finger stick devices for capillary blood sampling. Our findings indicate that these medical practices play no role in nosocomial transmission of HCV in diabetic patients.

Aim A link between chronic hepatitis C virus (HCV) infection, Type 2 diabetes mellitus and insulin resistance has been suggested by several studies. However, HCV infection appears to be associated with insulin resistance but not with the metabolic syndrome. The aim of this study was to determine whether chronic HCV infection had an impact on the clinical characteristics of Type 2 diabetes. Methods We studied retrospectively a group of patients with diabetes mellitus associated with HCV infection (HCV-DM) and compared them with patients with conventional Type 2 diabetes (DM). Results The HCV-DM patients had a lower body mass index ( P = 0.001) and systolic blood pressure ( P = 0.04) compared with patients with DM diabetes. Ten patients (27.0%) in the HCV-DM group and 35 (47.3%) in the DM group had microalbuminuria ( P = 0.04). DM patients had significantly higher serum creatinine levels than HCV-DM patients [87 (72–108) vs. 77 (64–86) µmol/l, P = 0.02; median (interquartile range)] but creatinine clearance (Cockroft Gault calculation) was similar. One HCV-DM patient (2.7%) and 44 DM patients (59.4%) were treated with hypolipidaemic therapy ( P = 0.0001). Even although nearly two-thirds of the overall DM group were prescribed cholesterol-lowering drugs, DM patients had significantly higher total cholesterol, high-density lipoprotein cholesterol and triglyceride levels than HCV-DM patients. Conclusion Our study provides further evidence that HCV-DM patients have specific clinical characteristics in comparison with classical DM patients. These data suggest an association between HCV virus infection and the development of insulin resistance or diabetes mellitus without the typical features of the metabolic syndrome. Introduction: La prévalence de la stéatose est élevée chez les patients diabétiques type 2 et les obèses. Dans le diabète type 1 (DT1), une étude italienne retrouve une forte prévalence (44 %) et un lien avec les complications micro et macro-vasculaires. Une autre étude trouve, au contraire, une faible prévalence. C’est pourquoi, nous avons évalué la prévalence de la stéatose chez des patients DT1 avec la technique non-invasive de référence. Patients et méthodes: Cent-huit patients DT1 ont bénéficié d’une spectroscopie-RMN. Un contenu hépatique en graisses (CHG) supérieur à 5,56 % définissait la stéatose. Résultats: Les caractéristiques de la population sont : âge = 40,6 ans±15,4 ; IMC = 25,1±3,8, durée de diabète = 21 ans±14. Moins de 6 % des patients DT1 sont stéatosiques (n=6/108). Parmi les 44 patients en surpoids (IMC>25), 13,6 % ont une stéatose. La stéatose est corrélée positivement à : l’IMC (p=0,007), à la dose d’insuline (p<0,001) et aux triglycérides (p=0,033). Les complications du diabète ne sont pas associées à une augmentation du CHG. En analyse multivariée, la stéatose est corrélée à la dose d’insuline (p=0,023) indépendamment de l’age, de l’IMC. Discussion: La prévalence de la stéatose est faible dans notre cohorte (< 6 %), très inférieure à celle décrite par échographie dans l’étude italienne des DT1 (44 %) et même à celle décrite dans la population générale (34–46 %). La lipogenèse hépatique insuffisamment stimulée du fait de l’insulinopénie portale dans le DT1 pourrait expliquer nos résultats. L’appariement de notre cohorte à une population témoin est indispensable pour confirmer ces données préliminaires. Introduction: Le profil comportemental de type A associant ű rapidité Ƈ, ű esprit de compétition Ƈ et ű forte implication au travail Ƈ est classiquement connu comme associé à un risque cardiovasculaire augmenté, mais pourrait aussi constituer un état ű positif Ƈ de combativité dans les maladies chroniques. Nous avons souhaité savoir si le profil psychologique de type A pouvait influencer l’équilibre glycémique et la survenue de complications au cours du diabète. Matériels et méthodes: Dans une étude prospective observationnelle, menée chez 400 patients diabétiques de type 1 et 2, nous avons étudié l’influence du profil comportemental de type A et de type B, défini selon l’échelle de Bortner, sur l’équilibre glycémique, sur les complications micro-vasculaires (rétinopathie, néphropathie) et sur les paramètres biologiques de l’inflammation (CRP). Résultats: L’HbA1c n’était significativement pas différente entre les profils psychologiques de type A ou B de même que l’ancienneté du diabète. En revanche, il était noté, chez les profils de type A, comparés aux patients de type B, une prévalence plus faible de la rétinopathie (16 % vs. 31 %, p=0,03), de l’artériopathie des membres inférieurs (2 % vs 11 %, p=0,03) et une tendance à moins de neuropathie (23 % vs. 37 %, p=0,06). La prévalence de la cardiopathie ischémique n’était pas différente entre les 2 groupes. Parallèlement le taux de CRP était significativement plus faible chez les patients de type A (4,9±4,3 vs. 7,8±7,6, p=0,013). Conclusion: Le profil comportemental de type A apparaît, indépendamment de l’équilibre glycémique, associé à un moindre degré d’inflammation et à une prévalence réduite de certaines complications (rétinopathie, par exemple). Une réduction du stress oxydatif, associé au profil comportemental de type A pourrait constituer un mécanisme sous-jacent pouvant rendre compte de ces résultats. [ABSTRACT FROM AUTHOR]

Introduction: La stéatose hépatique est fréquemment associée au syndrome métabolique et au diabète de type 2. Son association au risque coronarien semble bien démontrée dans la population générale. Par contre, dans la population des sujets diabétiques de type 2 (DT2) nous ne disposons pas d’étude confirmant cette relation. De plus la mesure de la stéatose est souvent réalisée avec des moyens non spécifiques comme la biologie (augmentation des transaminases), ou l’échographie. Notre objectif est d’évaluer la stéatose des patients diabétiques de type 2 par la technique non invasive de référence, la spectrométrie IRM, et de rechercher une association au risque coronarien évalué par la mesure de l’épaisseur de l’intima média carotidienne (EIM). Matériels et méthodes: Quatre-vingt dix-neuf sujets diabétiques de type 2 ont été inclus dans l’étude. Tous ont bénéficié d’un examen par spectrométrie IRM hépatique avec mesure de la stéatose de façon quantitative. La quantité de graisse viscérale a été mesurée par IRM. La mesure de l’EIM est la moyenne des mesures pour les 2 carotides. Résultats: Soixante et un (61,6 %) patients présentent une stéatose avec un pourcentage de graisse > 5 % à la spectrométrie IRM. La quantité de stéatose est corrélée au taux des transaminases (ASAT r=0,27, p =0,009 ; ALAT r=0,28, p =0,007), des triglycérides (r=0,27, p =0,01), des gamma-GT (r=0,20, p =0,05), mais pas au taux de HDL-cholestérol, de LDL-cholestérol ou à la quantité de graisse viscérale mesurée par IRM. Soixante-treize pourcent des patients avec une stéatose > 5 % et 57.8 % de ceux avec une stéatose sévère > à 15 % ont des taux de transaminases dans les normes. Concernant le risque vasculaire, l’EIM carotidienne n’est pas corrélée à la quantité de stéatose (r=0,05 ; p =0,68). Conclusion: Ce travail montre que chez les sujets diabétiques de type 2, la stéatose ne semble pas être associée à une augmentation du risque vasculaire évalué par la mesure de l’EIM carotidienne. La prévalence de la stéatose chez les sujets diabétiques est très élevée avec environ 2/3 de patients atteints de surcharge hépatique en graisse. Par contre, la mesure des transaminases ne semble pas être un bon outil pour le dépistage de la stéatose du patient diabétique. [Copyright &y& Elsevier]
