Objectives: Working Group 2 was convened to address topics relevant to prosthodontics and dental implants. Systematic reviews were developed according to focused questions addressing (a) the number of implants required to support fixed full-arch restorations, (b) the influence of intentionally tilted implants compared to axial positioned implants when supporting fixed dental prostheses (FDPs), (c) implant placement and loading protocols, (d) zirconia dental implants, (e) zirconia and metal ceramic implant supported single crowns and (f) zirconia and metal ceramic implant supported FDPs., Materials and Methods: Group 2 considered and discussed information gathered in six systematic reviews. Group participants discussed statements developed by the authors and developed consensus. The group developed and found consensus for clinical recommendations based on both the statements and the experience of the group. The consensus statements and clinical recommendations were presented to the plenary (gathering of all conference attendees) and discussed. Final versions were developed after consensus was reached., Results: A total of 27 consensus statements were developed from the systematic reviews. Additionally, the group developed 24 clinical recommendations based on the combined expertise of the participants and the developed consensus statements., Conclusions: The literature supports the use of various implant numbers to support full-arch fixed prostheses. The use of intentionally tilted dental implants is indicated when appropriate conditions exist. Implant placement and loading protocols should be considered together when planning and treating patients. One-piece zirconia dental implants can be recommended when appropriate clinical conditions exist although two-piece zirconia implants should be used with caution as a result of insufficient data. Clinical performance of zirconia and metal ceramic single implant supported crowns is similar and each demonstrates significant, though different, complications. Zirconia ceramic FDPs are less reliable than metal ceramic. Implant supported monolithic zirconia prostheses may be a future option with more supporting evidence., (© 2018 The Authors. Statement of Problem: Many clinicians use hand screwdrivers to tighten prosthetic and abutment screws. The impact of the screwdriver type, the location of the implant, and the sex of the practitioner on the generated torque is not well understood., Purpose: The purpose of this in vitro study was to compare the torque generated by different hand screwdrivers when used in the anterior and posterior regions and to evaluate the correlation between the applied torque and the sex and body mass index (BMI) of the practitioner., Material and Methods: A mandibular typodont with anterior and posterior implants was mounted in a mannequin head. Fifty clinicians (6 prosthodontists, 8 graduate prosthodontic students, and 36 dental students) used their maximum force to tighten the abutment screws in the anterior and posterior regions, using 3 different hand screwdrivers, with handles of different shapes and sizes (small, medium, and large). The torque values generated were measured using torque meters connected to the implants. The generated torque, sex, age, height, and weight were also recorded for each clinician. A generalized linear model was used to find correlations between the different factors and the generated torque (α=.05)., Results: Significant differences were observed among the applied torque values when different screwdrivers were used (P<.001). Higher torque values were generated in the anterior region than in the posterior region (P<.001). The interaction of the sex and BMI of the participants significantly affected the generated torque values (P=.044)., Conclusions: The type of screwdriver and location of the implant affected the generated torque. Torque values generated by the large screwdriver were higher than those of small and medium screwdrivers. Background: Failure to restore missing teeth in time can easily lead to the mesial tilting of the distal abutment teeth. However, a fixed partial denture (FPD) can improve stress conduction and distribution and prevent periodontal injuries. In these more complex cases, it is necessary to consider various factors comprehensively to improve conventional treatment planning and achieve better results. Methods: We selected a patient with a missing first molar and a mesial inclination of the second molar, leaving inadequate space or bone mass for implant denture restoration, necessitating an FPD for restoration. Three-dimensional finite element analysis (3D-FEA) combined with photoelastic analysis were used to explore how the inclination angle (0 ‒ 30°) and different dental restoration materials (zirconia, lithium disilicate, polymer-infiltrated ceramic network, and resin composite) affect the biomechanical behaviour of FPD‒abutments‒periodontal tissue complex. Results: The stress was easily concentrated in the FPD connectors, enamel shoulder collar, periapical area, and root bifurcation. The stress on FPD and the periodontal ligament (PDL) of the second premolar increased with an increase in the elastic modulus of FPD, with an opposite trend in the abutments, the alveolar bone, and the PDL of the second molar. The stress on the FPD and alveolar bone increased with increased inclination angle of the distal abutment. The stress on two abutments and their PDL were positively correlated with the inclination angle in two stages; however, when the inclination angle > 12°, the second premolar and its PDL showed a negative correlation. Conclusions: FPDs can be used for restoration within 24° of distal abutment inclination, but protecting the abutments (< 12° especially) and the periodontal tissue (> 12° especially) must be taken seriously. Virtual reality is considered to be useful in improving procedural skills in dental education, but systems using wearable devices such as head-mounted displays (HMDs) have many problems in terms of long-term use and hygiene, and the accuracy of stereoscopic viewing at close ranges is inadequate. We developed an abutment tooth formation simulator that utilizes a display (spatial reality display—SRD) to precisely reproduce 3D space with naked-eye stereoscopic viewing at close range. The purpose of this was to develop and validate the usefulness of an abutment tooth formation simulator using an SRD. A 3D-CG (three-dimensional computer graphics) dental model that can be cut in real time was output to the SRD, and an automatic quantitative scoring function was also implemented by comparing the cutting results with exemplars. Dentists in the department of fixed prosthodontics performed cutting operations on both a 2D display-based simulator and an SRD-based simulator and conducted a 5-point rating feedback survey. Compared to the simulator that used a 2D display, the measurements of the simulator using an SRD were significantly more accurate. The SRD-based abutment tooth formation simulator received a positive technical evaluation and high dentist satisfaction (4.37), suggesting its usefulness and raising expectations regarding its future application in dental education. Background: Angulated screw channel (ASC) abutment allows off-axis dental implants to be used in dental restorations without the need for cementation. As this is a relatively new system, research on its clinical performance is limited. Objectives: To summarize the available in-vitro and in-vivo studies on the mechanical and technical issues associated with the ASC system and compare its clinical performance with that of conventional implant-supported abutments. Methods: A comprehensive literature search in PubMed, Web of Science, and ScienceDirect databases was performed, focusing on articles about angulated (angled) screw channel (ASC) systems published in English between January 2015 and November 2023. Only in-vitro and in-vivo studies were included. Results: After analyzing the recorded articles, 26 studies (11 in vivo and 15 in vitro) were included in the final discussion and review. Conclusion: Although the ASC system is still relatively new, and is presently outperformed by conventional abutment systems in terms of technical and mechanical properties, in short- and medium-term in-vivo studies, it was shown reliable for retaining single or multiple-unit implant restorations in both posterior and anterior zones. Still, further long-term clinical research is needed to fully elucidate the risk factors associated with ASC failures.

Background: Mechanical complications affect the stability of implant restorations and are a key concern for clinicians, especially with the frequent introduction of new implant designs featuring various structures and materials. This study evaluated the effect of different prosthetic index structure types and implant materials on the stress distribution of implant restorations using both in silico and in vitro methods. Methods: Four finite element analysis (FEA) models of implant restorations were created, incorporating two prosthetic index structures (cross-fit (CF) and torc-fit (TF)) and two implant materials (titanium and titanium-zirconium). A static load was applied to each group. An in vitro study using digital image correlation (DIC) with a research scenario identical to that of the FEA was conducted for validation. The primary strain, sensitivity index, and equivalent von Mises stress were used to evaluate the outcomes. Results: Changing the implant material from titanium to titanium-zirconium did not significantly affect the stress distribution or maximum stress value of other components, except for the implant itself. In the CF group, implants with a lower elastic modulus increased the stress on the screw. The TF group showed better stress distribution on the abutment and a lower stress value on the screw. The TF group demonstrated similar sensitivity for all components. DIC analysis revealed significant differences between TF-TiZr and CF-Ti in terms of the maximum (P < 0.001) and minimum principal strains (P < 0.05) on the implants and the minimum principal strains on the investment materials in both groups (P < 0.001). Abstract Background While the concept of angled screw channels has gained popularity, there remains a scarcity of research concerning the torque loss and fracture strength of monolithic zirconia restorations with various screw channel angulations when exposed to thermomechanical cycling. This in-vitro study aimed to evaluate the reverse torque value and fracture resistance of one-piece screw-retained hybrid monolithic zirconia restorations with angulated screw channels after thermomechanical cycling. Methods One-piece monolithic zirconia restorations, with angulated screw channels set at 0°, 15°, and 25° (n = 6 per angulation) were fabricated and bonded to titanium inserts using a dual-cure adhesive resin cement. These assemblies were then screwed to implant fixtures embedded in acrylic resin using an omnigrip screwdriver, and reverse torque values were recorded before and after thermomechanical cycles. Additionally, fracture modes were assessed subsequent to the application of compressive load. One-way ANOVA and Bonferroni post hoc test were used to compare the groups (α = 0.05). Results The study groups were significantly different regarding the fracture resistance (P = 0.0015), but only insignificantly different in the mean percentage torque loss (P = 0.4400). Specifically, the fracture resistance of the 15° group was insignificantly higher compared to the 0° group (P = 0.9037), but significantly higher compared to the 25° group (P = 0.0051). Furthermore, the fracture resistance of the 0° group was significantly higher than that of the 25° group (P = 0.0114). Conclusions One-piece hybrid monolithic zirconia restorations with angulated screw channels can be considered an acceptable choice for angulated implants in esthetic areas, providing satisfactory fracture strength and torque loss.

The arrangement of a stress breaker in the pier abutment of a fixed-mobile bridge affects the deformations that may occur in its supports. This article aims to study the most appropriate position and arrangement of a stress breaker. To verify the established objectives, an experimental "in vitro" study was carried out with five different bridge designs: one with rigid connectors and four with non-rigid connectors (interlocks), placed on the distal and mesial surface of the intermediate abutment, and combining the retention stop at cervical and occlusal positions. The strain level at the support ground of each bridge was measured with strain gauges. The statistical analysis was performed with the help of the Kruskal–Wallis test and a linear regression model. Initial results show that the model with the highest average value of maximum strain is the non-rigid bridge with the interlock on the distal surface of the pier abutment and the occlusal retention stop. In any case, the presence of an intermediate abutment, with or without interlock, influences the maximum tension supported by the abutment, especially in relation to the intermediate abutment. Abstract Objectives The stability of the abutment screw is pivotal for successful implant‐supported restorations, yet screw loosening remains a common complication, leading to compromised function and potential implant failure. This study aims to evaluate the effect of different implant‐abutment types and heights on screw loosening in cases with increased crown height space (CHS). Materials and Methods In this in vitro study, a total of 64 abutments in eight distinct groups based on their type and height were evaluated. These groups included stock, cast, and milled abutments with heights of 4 mm (groups S4, C4, and M4), 7 mm (groups S7, C7, and M7), and 10 mm (groups C10 and M10). Purpose: To compare the retention forces of implant overdenture patrices (ball, bar, and TiSi.snap) to conventional (O-ring, metal housing, and clip) and polyvinyl siloxane (PVS)-based silicone (retention.sil 200, 400, and 600; Mucopren Soft; and GC Reline Soft) matrix materials.Materials and Methods: Two implant analogs, in which the patrices were to be placed, were embedded parallel to each other in polyethylene blocks, and the matrices were placed in heatpolymerized acrylic blocks that were fabricated as overdentures. Ten samples were prepared for each attachment type, and 180 samples were obtained from 18 groups. All samples were placed in a chewing simulator for occlusal force application and for insertion and removal of the pieces. Retention measurements were performed with a universal testing device at the initial (10 cycles), simulated first-year (1,825 cycles), and simulated second-year (3,650 cycles) periods, assuming that the patients would insert and remove their overdenture five times daily.Results: Loss of retention occurred in all the attachment systems at the end of 3,650 cycles (P < .05). The PVS matrix materials showed less retention than the O-ring and metal matrices when a ball patrix was used, while they had higher retention than bar clips when the Hader bar patrix was used (P < .05). Among the PVS matrix materials, retention.sil 200 produced the lowest retention values, whereas retention.sil 600 generated the highest.Conclusion: PVS matrix materials show higher retention than that achieved by yellow plastic bar matrices. This case control study measured early crestal bone changes around subcrestally placed platform-switched implants surrounded by thin soft tissue and compared them with regular, matching-platform implants placed in a supracrestal position and surrounded by thick soft tissue. Sixty-six patients received two-piece internal hex dental implants. Control group patients (n = 33) received implants that had a horizontally matching implant-abutment connection and were placed approximately 0.5 to 1 mm supracrestally. Test group patients (n = 33) received platform-switched implants that were placed about 1.5 mm subcrestally. Clinical examinations were conducted, intraoral radiographs were taken, and statistical analysis was performed. After 2 months, the mean bone loss was 0.2 mm (SD: 0.22 mm; range: 0.1 to 1.2 mm) in the control group and -0.69 mm (SD: 0.65 mm; range: 0 to 2.6 mm) in the test group; this difference was found to be statistically significant (P < .05). After 1 year, mean bone loss was 0.28 mm (SD: 0.36 mm; range: 0.1 to 1.63 mm) in the control group and -0.6 mm (SD: 0.55 mm; range: 0.05 to 1.8 mm) in the test group. Platform-switched implants placed in a subcrestal position in vertically thin soft tissues showed statistically significantly more bone loss than non--platformswitched implants placed supracrestally with vertically thick tissues. Purpose: To evaluate the effect of cementation protocols on the bonding interface stability and pull-out forces of temporary implant-supported crowns bonded on a titanium base abutment (TiB) or on a temporary titanium abutment (TiA).Materials and Methods: A total of 60 implants were restored with PMMA-based CAD/CAM crowns. Five groups (n = 12) were created: Group 1 = TiB/SRc: crown conditioned with MMA-based liquid (SR Connect, Ivoclar Vivadent); Group 2 = TiB/50Al-MB: crown airborne particle-abraded with 50-μm Al2O3 and silanized (Monobond Plus, Ivoclar Vivadent); Group 3 = TiB/30SiOAl-SRc: crown airborne particle-abraded with 30-μm silica-coated Al2O3 (CoJet, 3M ESPE) and conditioned with MMA-based liquid (SR Connect); Group 4 = TiB/30SiOAl-MB: crown airborne particle-abraded with 30- μm silica-coated Al2O3 (CoJet) and silanized (Monobond Plus); and Group 5 = TiA/TA-PMMA: crown manually enlarged, activated, and rebased with PMMA resin (Telio Lab, Ivoclar Vivadent). Specimens in the TiB groups were cemented using a resin cement (Multilink Hybrid Abutment, Ivoclar Vivadent). After aging (120,000 cycles, 49 N, 1.67 Hz, 5°C to 55°C, 120 seconds), bonding interface failure was analyzed (50x). Pull-out forces (N) (0.5 mm/minute) and modes of failure were registered. Chi-square and Kruskal-Wallis tests were used to analyze the data (α = .05).Results: Bonding failure after aging varied from 0% (Group 5) to 100% (Groups 1, 2, and 4) (P < .001). Mean pull-out force ranged between 53.1 N (Group 1) and 1,146.5 N (Group 5). The pull-off forces were significantly greater for Group 5 (P < .05), followed by Group 3 (P < .05), whereas the differences among the remaining groups were not significant (P > .05).Conclusion: The cementation protocol had an effect on the bonding interface stability and pull-out forces of PMMA-based crowns bonded on a titanium base. Airborne particle abrasion of the crown internal surface and conditioning it with an MMA-based liquid may be recommended to improve retention of titanium base temporary restorations. Objectives: The purpose of this pilot study is to compare gene expression in mucosa around dental implants with zirconia abutment to titanium and investigate presence of particles in mucosa samples and on implant heads. Material and Methods: Ten patients with a single implant supported prosthesis connected to zirconia or titanium abutments were invited at the five-year control. A clinical examination and a survey on experience of function and appearance were conducted. A mucosa biopsy taken in close vicinity to the implant were analysed by real-time polymerase chain reaction (qPCR) and presence of particles in a scanning electron microscope/energy-dispersive X-ray spectroscope (SEM/EDX). Cytological smear samples were collected and analysed through inductively coupled plasma mass spectrometry (ICP-MS) to investigate presence of particles on implant heads. Results: In total, 9 patients participated in the study, five with titanium abutments and four with zirconia abutments. All patients were satisfied with function and aesthetics. Titanium and iron particles were detected in mucosa biopsies. The ICP - MS analysis demonstrated presence of zirconia and titanium. Several proinflammatory genes were upregulated in the zirconia abutment group. Conclusions: Around zirconia abutments a slight increase in proinflammatory response and amount of wear particles was seen as compared to titanium. Wear particles of titanium were present in all soft tissue samples, however zirconia particles only in the samples from implants heads/mucosa with zirconia abutments.

Background: The aim of this study was to assess the impact of abutments with angled screw access channel on the retention of zirconia crowns. Methods: Seven implant replicas were inserted in epoxy resin blocks. Fourteen zirconia crowns for central incisor tooth were digitally fabricated and cemented to titanium bases (Ti-bases) with resin cement. Titanium bases were categorized into 2 groups (n = 7). Control group (Group STA) included straight screw access channel abutments. Study group (Group ASC) included angled screw access channel abutments. Following aging (5 °C-55 °C, 60 s; 250,000 cycles, 100 N, 1.67 Hz), the pull-off forces (N) were recorded by using retention test (1 mm/min). Types of failure were defined as (Type 1; Adhesive failure when luting agent predominantly remained on the Ti-base surface (> 90%); Type 2; Cohesive failure when luting agent remained on both Ti-base and crown surfaces; and Type 3; Adhesive failure when luting agent predominantly remained on the crown (> 90%). Statistical analysis was conducted by using (IBM SPSS version 28). Normality was checked by using Shapiro Wilk test and Q-Q plots. Independent t-test was then used to analogize the groups. Results: Mean ± standard deviation of retention force records ranged from 1731.57 (63.68) N (group STA) to 1032.29 (89.82) N (group ASC), and there was a statistically significant variation between the 2 groups (P <.05). Failure modes were Type 2 for group STA and Type 3 for group ASC. In contemporary dental practice, patients' interests are taken into account in terms of function and appearance. The importance in everyday circumstances, current advertising, and the media in general highlight their effect on an attractive appearance. Many academics in the field of dental aesthetics have stated equivocally that when examining the impact of tooth appearance necessitates a multidisciplinary approach. Laser technology in cosmetic dentistry is in its early stages of development and has a lot of potential in the future. Also, with expanding use of lasers in clinical dentistry, treatment planning and prognosis will see a significant improvement in the future. The introduction of this technology has turned dentistry into a painless, bloodless field with increased predictability and instant outcomes. Furthermore, these techniques can help a cosmetic dentist regulate gingiva and osseous outlines with greater creativity in the pursuit of aesthetic dental principles with more efficient use of patient time. Even from the patient standpoint, the reduced requirement for suturing and faster healing times improves case acceptance and enables the increased demand for aesthetic dentistry. Objective: To detail a technique for bonding periodontal splint precisely in a digital workflow. Clinical Considerations: Periodontal splinting can be considered to stabilize the mobile teeth, especially for mandibular anterior teeth. Reliable bonding of periodontal splints is a prerequisite for successful clinical performance. However, when bonding the indirect splint to place or making direct splint intraorally, there is a significant risk of mobile teeth drifting away from the splint. To guide accurate insertion of periodontal splint with no risk of displacement of mobile teeth, a guide device fabricated by digital workflow is introduced in this article. Conclusions: Periodontal compromised teeth can be provisionally fixed during splinting, with the help of the guided device, and precise bonding of the splint is readily accomplished by using such digital workflow. This technique is not only applicable to the lingual splints, but also suitable for the labial ones. Clinical Significance: The use of a guided device, after being digitally designed and fabricated, enables to stabilize the mobile teeth, in case of any displacement during splinting. Purpose: This study evaluated the micro shear bond strength of resin cement to an yttria-stabilized zirconia ceramic and the survival probability of zirconia abutments and crowns after different surface treatments through a fatigue test. Materials and Methods: The study was divided into two parts. For part 1, 95 zirconia disks were divided into five groups (n = 19): control, untreated, airborne particle abrasion with Al2O3 particles before sintering, airborne particle abrasion with Al2O3 particles after sintering, silicatization before sintering, and silicatization after sintering. Three samples of each group were used for evaluation of surface roughness by confocal laser scanning microscopy and afterward were prepared for surface microstructural analysis by scanning electron microscopy. Ten samples of each group were subjected to micro shear bond strength testing, and the interfaces of the remaining six were examined by scanning electron microscopy. In part 2, 70 external hex zirconia abutments and copings were made by computer-aided design/computeraided manufacturing (n = 14). Marginal fit of abutment/coping was measured in a confocal laser scanning microscope. Afterward, a fatigue test was carried out with progressive load of 80 up to 320 N (40 N steps), 5 Hz frequency, and 20,000 cycles at each step. Thermal cycling was simultaneously performed (5°C to 55°C). Results: The group treated after sintering with SiO achieved statistically higher micro shear bond strength (P < .01). Higher failure loads were associated with a combined failure. The surface changes in the group treated with SiO before sintering suggest silica deposition, and there was a lack of homogeneity, which was more evident on the surface of the groups treated before sintering. The marginal gap was higher for the group treated before sintering with SiO (P < .01), and the survival probability of the sets was similar for all tested groups (P = .57). Conclusion: The micro shear bond strength to zirconia was improved after silicatization after sintering, but the survival probability of crown/abutment/implant sets was not affected by different surface treatments. Objectives: The purpose of this study was to evaluate the effects of different resin-based and ceramic superstructure materials and two different abutment types on the stress distribution of the abutment screw using the three-dimensional finite element stress analysis method. Materials and Methods: A three dimensional implant, abutment (zirconia and titanium), abutment screw, crown (zirconia reinforced lithium silicate, lithium disilicate, polymer-infiltrated resin ceramic, and PEEK), and alveolar bone were designed using Rhinoceros 3D modeling software and VRMesh Studio software and combined to form 8 simulations. On the models prepared, loading was made on the lingual tubercule of the maxillary right first premolar crown at an angle of 30° with 150 N force obliquely in the buccolingual direction. The von Mises values obtained from the abutment screw were compared according to the types of abutment and crown materials. Results: The von Mises stress values in the abutment screw were higher in the models using a titanium abutment abutment (on average 1336.24 MPa), and the lower stress values were obtained in the models using a zirconia abutment (on average 964.26 MPa). When the prosthetic material used was changed, no apparent difference was observed in the stress values on the abutment screw. Conclusions: Considering that the abutment screw is the weakest link of the implant system, zirconia abutments can be used reliably in the maxillary first premolar region where aesthetic expectations are high.

Purpose: To evaluate the biomechanical behavior of implants with different neck designs.Materials and Methods: Finite element models of implants with two different neck designs (OsseoSpeed EV and OsseoSpeed Profile EV) were generated. Implants and abutments were subjected to buccally and lingually directed 30-degree oblique loads of 200 N.Results: Under buccally angulated load, the Profile implant model showed lower minimum principal stress when compared to the standard implant model (-63.4 vs -91.9 MPa), whereas the von Mises stress value of the Profile implant model was higher (342 MPa).Conclusion: This preliminary study showed that Profile implants placed in alveolar ridges with buccolingual bone height discrepancy showed similar stress distribution and stress value outcomes when compared to standard implants. This article describes a simplified technique for relining provisional prostheses on natural abutments that can be applied to this specific type of tooth preparation with feather-edge finish line. Starting from a diagnostic wax-up, a provisional fixed restoration is constructed, containing all the correct structural information. This includes the controlled depth of the prosthetic margin into the gingival sulcus, the emergence profile, and the area from the emergence profile to the gingival third. Chair time is saved during the clinical procedures because the finishing and polishing steps are shortened, and the resulting provisional restoration is precise and highly biocompatible. Titanium dental implants have had new competitors in recent years, such as fixtures made of zirconia, which promise better aesthetics. The purpose of this study is to evaluate their mechanical performance in silico (Finite Element Analysis). The investigation was performed on a single tooth Patent™ Dental Implant (Zircon Medical®, Altendorf, Switzerland) in two configurations: without offset (Test I) and with offset (Test II, 1.5 mm within the cortical bone). The Patent Implant system consists of two components: the implant with integrated abutment and the fibreglass post. The components of the dental implants were tested using a compression load of 400 N along the implant axis. The results showed that the chewing load generates stress distribution on the bone, therefore, the offset configuration should be avoided. [ABSTRACT FROM AUTHOR]

With the evolution of CAD/CAM technology, custom titanium and/or zirconia abutments are increasingly being used, leading to several comparisons in the literature, both mechanical and aesthetic, to evaluate performance differences between these two types of abutments. Therefore, the aim of this comprehensive review is to present the most recent data on the latest comparisons between CAD/CAM and stock abutment applications. The PICO model was used to perform this review, through a literature search of the PubMed (MEDLINE) and Scopus electronic databases. CAD/CAM abutments allow individualization of abutment parameters with respect to soft tissue, allow increased fracture toughness, predict the failure mode, show no change in the fracture toughness over time, reduce the prosthetic steps, and reduce the functional implant prosthesis score and pain perceived by patients in the early stages. The advantages associated with the use of stock abutments mainly concern the risk of corrosion, time spent, cost, and fit, evaluated in vitro, in the implant–abutment connection. Equal conditions are present regarding the mechanical characteristics during dynamic cycles, screw loss, radiographic fit, and degree of micromotion. Further randomized controlled clinical trials should be conducted to evaluate the advantages reported to date, following in vitro studies about titanium and/or zirconia stock abutments. Purpose: To compare the three-dimensional (3D) accuracy of conventional impressions (CIs) with digital scans (DSs) using an intraoral scanner (IOS) with intraoral scan bodies (SBs) and varying buccolingual interimplant angulations. A secondary aim was to measure the SB machining tolerance and height with and without application of torque. Materials and Methods: Three master models (MMs) with two implants simulating an implant-supported three-unit fixed partial denture for bone-level implants were used. The implants had buccolingual interimplant angulations of 0, 10, and 20 degrees. Test models for the CI test groups were made with impression copings and polyether impressions. SBs were attached to the MMs, tightened to 15-Ncm torque, and scanned by an IOS for the DS test groups (six test groups, n = 5). A coordinate measuring machine measured linear distortions (dx, dy, dz), 3D distortions (dR), angular distortions (dϴx, dϴy), and absolute angular distortions (Absdϴx, Absdϴy) of the physical CI test models and STL files of the DS virtual models relative to the MMs. Metrology software allowed both physical and virtual measurement of geometric targets that were comparable and allowed computation of relative displacements of implant centroids and axes. Results: Mean dR ranged from 31 ± 14.2 to 45 ± 3.4 μm for DS and 18 ± 8.4 to 36 ± 6.5 μm for the CI test groups. Mean Absdϴx ranged from 0.041 ± 0.0318 to 0.794 ± 0.2739 degrees for DS and 0.073 ± 0.0618 to 0.545 ± 0.0615 degrees for the CI test groups. Mean Absdϴy ranged from 0.075 ± 0.0615 to 0.111 ± 0.0639 degrees for DS and 0.106 ± 0.0773 to 0.195 ± 0.1317 degrees for the CI test groups. Two-way analysis of variance showed that the impression technique (P = .012) and implant angulations (P = .007) had a significant effect on dR. Distortions were mostly in the negative direction for DS test groups. Perfect coaxiality of the SB with the implant was never achieved. For SB to implant machining tolerances, the mean absolute horizontal displacement ranged from 4 ± 1.2 to 7 ± 2.3 μm. The SB dz was –5 ± 3.2 μm, which increased in the negative direction to –11 ± 4.9 μm with torque application (P = .002). Conclusion: Distortions were found for both DS and CI test groups. The best accuracy was obtained with CIs for parallel implants. With angulated implants, conventional and DSs were not significantly different. Excessive torque application that causes negative dz for SB fit would position the virtual implant at a deeper location compared with reality, resulting in possible framework misfit. The aim of this work is to describe a case of severe disparallelism between two implants supporting a cement-retained bridge, placed in the maxilla, employing two custom-angled abutments. Fractured abutment screw and fractured abutment involved two implant restorations in the maxilla. Once prosthetic components have been removed, a new prosthetic rehabilitation has been planned. The divergence between the implants was measured, obtaining an angle of divergence of 39°. Then, two custom-angled titanium abutments and two metal-ceramic splinted crowns were realized.

Background: The retention of cement-retained implant-supported restorations can be affected by surface treatments such as anodizing. This study aimed to assess the effect of the anodization of titanium abutments on their tensile bond strength to implant-supported lithium disilicate (LDS) all-ceramic crowns. Materials and Methods: This in vitro, experimental study was conducted on 26 straight abutments in two groups of anodization and control. In the anodization group, seven flat 9 V batteries connected in series were used to generate 64 V energy. A glass container was filled with 250 mL of distilled water, and 1 g of trisodium phosphate was added to it to create an electrolyte solution. The anode was then disconnected and the abutment was rinsed with acetone and deionized water. The surface roughness of abutments was measured by a profilometer. The abutments were scanned by a laboratory scanner, and maxillary central incisor monolithic crowns were fabricated by inLab SW18 software. The crowns were seated on the abutments and temporarily cemented with TempBond. They were then incubated in artificial saliva and subjected to 5000 thermal cycles. The tensile bond strength of crowns was then measured. Data were analyzed by the Student's t-test and Mann–Whitney U-tests (α =0.05). Results: The mean bond strength was significantly higher in anodized abutments (P = 0.003). The surface roughness of anodized abutments was slightly, but not significantly, higher than that of the control group (P > 0.05). The frequency of adhesive failure was almost twice higher in anodized abutments. Conclusion: Anodization of titanium abutments significantly improved their tensile bond strength to implant-supported LDS all-ceramic crowns.

The purpose of this study is to calculate microbiological composition of aligners after a day of wearing them. To date, the dental market for orthodontists offers many ways to correct bites. Aligners are transparent and almost invisible from the teeth. They are used for everyday wear to correct the incorrect position of the teeth, which was once considered the prerogative of braces. Scientists worldwide have repeatedly considered questions regarding the interaction between aligners and the oral cavity’s microflora; however, the emphasis has mainly shifted toward species composition and antibiotic resistance. The various properties of these microorganisms, including biofilm formation, adhesion to various cells, and the ability to phagocytize, have not been studied so widely. In addition, these characteristics, as well as the microorganisms themselves, have properties that change over time, location, and in certain conditions. In this regard, the problem of biofilm formation in dental practice is always relevant. It requires constant monitoring since high contamination of orthodontic materials can reduce the effectiveness of local anti-inflammatory therapy and cause relapses in caries and inflammatory diseases of the oral cavity. Adhesive properties, one of the key factors in forming the architectonics of biofilms, provide the virulence factors of microorganisms and are characterized by an increase in optical density, determining the duration and retrospectivity of diagnostic studies. This paper focuses on the isolation of clinical microbial isolates during aligner therapy and their ability to form biofilms. In the future, we plan to use the obtained strains of microorganisms to create an effective and safe biofilm-destroying agent. We aimed to study morphometric and densitometric indicators of biofilms of microorganisms persisting on aligners.

Purpose: The aim of this randomized clinical study was to compare the 3-year clinical outcome of metal-ceramic fixed dental prostheses (MC-FDPs) and zirconia all-ceramic fixed dental prostheses (AC-FDPs) replacing a posterior tooth.Materials and Methods: A sample of 34 patients with a missing posterior tooth were randomly chosen to receive either a MC-FDP (n = 17) or an AC-FDP (n = 17). The FDPs were evaluated at baseline and yearly until 3 years after cementation. They were assessed using the California Dental Association assessment system. Periodontal parameters were measured at the abutment teeth, and the contralateral teeth served as control. The statistical unit was the FDP/patient.Results: The survival rates for MC-FDPs and AC-FDPs were 100%. The success rate was 76% and 71% for MC-FDPs and AC-FDPs, respectively. Three technical complications were observed in the MC-FDP group and five in the AC-FDP group, all chipping fractures of the ceramic veneer. Furthermore, one biologic complication in the MC-FDP group (an apical lesion) was observed. No framework fractures occurred. All patients had optimal oral hygiene and showed no bleeding on periodontal probing at any of the recalls. Only minor changes in the periodontal parameters were observed during the 3 years of observation.Conclusions: Three-unit posterior MC-FDPs and AC-FDPs showed similar high survival rates and acceptable success rates after 3 years of function, and ceramic veneer chipping fracture was the most frequent complication for both types of restorations. Purpose: This study aimed to evaluate the possible effect of sealing materials on superstructure attachment (ie, tightening/loosening torque and implant-abutment gap) in two different implant systems.Materials and Methods: A silicone, a chlorhexidine gel, and an industrial lubricant were tested. A 3D microscope was used for assessment of the implant-abutment gap, and the abutment screw was tightened and loosened with a digital torque screwdriver. A total of 20 implants per test group (10 BEGO Semados RI and 10 Nobel Biocare Replace Select Straight) were evaluated.Results: The tested sealing materials did not influence the gap between implant and abutment, but the force necessary for loosening the abutment screws decreased significantly.Conclusion: Sealing materials may be useful against bacteria, but probably influence torque negatively.
