Objectives: To characterize all hospitalizations held in mainland Portugal (2010-2015) with dementia-related agitation based on International Classification of Diseases, Ninth Revision, Clinical Modification (ICD-9-CM) coding, and to investigate whether there is a relationship between agitation and hospitalization outcomes., Methods: A retrospective observational study was conducted using an administrative dataset containing data from all mainland Portuguese public hospitals. Only hospitalization episodes for patients aged over 65 years who have received a dementia diagnosis ascertained by an ICD-9-CM code of dementia with behavioral disturbance (294.11 and 294.21) and dementia without behavioral disturbance (294.10 and 294.20) were selected. Episodes were further grouped according to the presence of an agitation code. For each episode, demographic data and hospitalization outcomes, including length of stay (LoS), in-hospital mortality, discharge destination and all-cause hospital readmissions, were sourced from the dataset. Comparative analyses were performed and multivariable logistic methods were used to estimate the adjusted associations between agitation (exposure) and outcomes., Results: Overall, 53,156 episodes were selected, of which 6,586 had an agitation code. These were mostly related to male, younger inpatients (mean 81.19 vs. 83.29 years, p <  0.001), had a higher comorbidity burden, stayed longer at the hospital (median 9.00 vs. 8.00 days, p <  0.001) and frequently ended being transferred to another facility with inpatient care. Agitation was shown to independently increase LoS (aOR = 1.385; 95%CI:1.314-1.461), but not the risk of a fatal outcome (aOR = 0.648; 95%CI:0.600-0.700)., Conclusion: These results support the importance of detecting and managing agitation early on admission, since its prompt management may prevent lengthy disruptive hospitalizations.

The prevalence of neurodegenerative diseases has been significantly increasing in the last decades, and it is expected to continue to grow. These health disorders can impair patients' decision-making capacity in healthcare. The capacity to make healthcare decisions is a fundamental pillar of informed consent, therefore, it should be carefully assessed. Clinicians' assessment, when not supported by a standardized tool, has revealed to be unreliable, so the recourse to an instrument of capacity assessment is crucial. The present paper aims to identify and summarize published instruments of healthcare decision-making capacity. To do so, a search of peer-reviewed articles in English, Portuguese and Spanish was conducted. A total of eighteen articles, detailing seventeen assessment instruments were selected. Instruments differ on format, structure, assessed abilities and psychometric properties. Likewise, instruments' targeted population also varies, with a few being specifically developed for patients with dementia. Although a high number of instruments were found, there is still no gold standard for healthcare decision-making capacity assessment. The lack of a gold standard highlights the need for more research in this field, as well as an effort to develop guidelines and normative data, in order to improve clinical practices., (© 2021. Objective: The present study aims to analyze the psychometric characteristics of the TeLPI (Irregular Words Reading Test), a Portuguese premorbid intelligence test, using the Rasch model for dichotomous items., Results: The results reveal an overall adequacy and a good fit of values regarding both items and persons. A high variability of cognitive performance level and a good quality of the measurements were also found. The TeLPI has proved to be a unidimensional measure with reduced DIF effects., Conclusions: The present findings contribute to overcome an important gap in the psychometric validity of this instrument and provide good evidence of the overall psychometric validity of TeLPI results.

The validity of the Cambridge Neuropsychological Automated Tests has been widely studied, but their reliability has not. This study aimed to estimate the test-retest reliability of these tests in a sample of 34 older adults, aged 69 to 90 years old, without neuropsychiatric diagnoses and living in retirement homes in the district of Lisbon, Portugal. The battery was administered twice, with a 4-week interval between sessions. The Paired Associates Learning (PAL), Spatial Working Memory (SWM), Rapid Visual Information Processing, and Reaction Time tests revealed measures with high-to-adequate test-retest correlations (.71-.89), although several PAL and SWM measures showed susceptibility to practice effects. Two estimated standardized regression-based methods were found to be more efficient at correcting for practice effects than a method of fixed correction. We also found weak test-retest correlations (.56-.68) for several measures. These results suggest that some, but not all, measures are suitable for cognitive assessment and monitoring in this population.

In the present study we analyzed the psychometric characteristics of the MoCA (Portuguese version) using the Rasch model for dichotomous items. The total sample comprised of 897 participants distributed between two main subgroups: (I) healthy group that was comprised of 650 cognitively healthy community dwellers and (II) clinical group that was comprised of 90 patients with Mild Cognitive Impairment, 90 patients with Alzheimer's disease, 33 patients with frontotemporal dementia, and 34 patients with vascular dementia recruited at a reference dementia clinic. All patients were investigated through a comprehensive neuropsychological assessment, laboratory tests essential to exclude a reversible form of dementia, imaging studies (CT or MRI and SPECT or FDG-PET), Apolipoprotein E allele genotyping and CSF biomarker (Aβ42,Tau, and P-tau) analyses. The clinical diagnosis was established through the consensus of a multidisciplinary team, based on international criteria. The results demonstrated an overall good fit of both items and the person's values, a high variability on cognitive performance level, and a good quality of the measurements. The MoCA scores also demonstrated adequate discriminant validity, with high diagnostic value. DIF analyses indicated the generalized validity of the MoCA scores. In conclusion, the results of this study show the overall psychometric adequacy of the MoCA and verify the discriminant and generalized validity of the obtained results.

The Montreal Cognitive Assessment (MoCA) is a brief cognitive screening instrument with good psychometric features and an excellent sensitivity in the early detection of mild cognitive decline. The MoCA was applied to a community-based sample of cognitively healthy adults (n = 650), stratified according to sociodemographic variables (age, gender, educational level, geographic region, geographic localization, and residence area), with a distribution similar to that observed in the Portuguese population. The normative data were determined according to age and education as these were the sociodemographic variables that most significantly contributed to the prediction of the MoCA scores, explaining 49% of their variance.

Objective: Among cognitive reserve markers, educational attainment is the most widely studied, with several studies establishing a strong association with risk of dementia. However, it has not yet been fully examined in delirium. This study aims to analyse the relationship between educational attainment and delirium. Methods: The study included elderly hospitalised patients admitted (=48 h) into an intermediate care unit (IMCU) of Intensive Care Medicine Service. Exclusion criteria were as follows: Glasgow Coma Scale (total= 11), blindness/deafness, inability to communicate or to speak Portuguese. The European Portuguese Version of the Confusion Assessment Method (CAM) was used for delirium assessment. Results: The final sample (n = 157) had a mean age of 78.8 (SD = 7.6) the majority being female (52.2%), married (51.5%) and with low educational level (49%). According to CAM, 21% of the patients had delirium. The delirium group presented the fewest years of education (median 1 vs. 4), with statistical significance (p = 0.003). Delirium was more frequent among male patients [odds ratio (OR) 0.32; 95% confidence interval (CI) 0.12-0.86; p = 0.023], as well as those patients with lower education (OR 0.76; 95% CI 0.62-0.95; p = 0.016), and with respiratory disease (OR 3.35; 95% CI 1.20-9.33; p = 0.020), after controlling for age and medication. Conclusion: Similar to previous studies, these findings point to a negative correlation between education and delirium. This study appears as an attempt to contribute to the knowledge about the role of cognitive reserve in risk of delirium, particularly because is the first one that has been carried out in an IMCU, with lower educated elderly patients. The Selective Reminding Test (SRT) and the Free and Cued Selective Reminding Test (FCSRT) are multitrial memory tests that use a common “selective reminding” paradigm that aims to facilitate learning by presenting only the missing words from the previous recall trial. While in the FCSRT semantic cues are provided to elicit recall, in the SRT, participants are merely reminded of the missing items by repeating them. These tests have been used to assess age-related memory changes and to predict dementia. The performance of healthy elders on these tests has been compared before, and results have shown that twice as many words were retrieved from long-term memory in the FCSRT compared with the SRT. In this study, we compared the tests’ properties and their accuracy in discriminating amnestic mild cognitive impairment (aMCI;n = 20) from Alzheimer disease (AD;n = 18). Patients with AD performed significantly worse than patients with aMCI on both tests. The percentage of items recalled during the learning trials was significantly higher for the FCSRT in both groups, and a higher number of items were later retrieved, showing the benefit of category cueing. Our key finding was that the FCSRT showed higher accuracy in discriminating patients with aMCI from those with AD. [ABSTRACT FROM PUBLISHER]

The International Working Group on Alzheimer's disease ( AD) suggested the free and cued selective reminding test ( FCSRT) to assess memory, as it showed high sensitivity and specificity in the differentiation of AD from healthy controls and other dementias. The FCSRT involves the use of selective reminding with semantic cueing in memory assessment. This study aims to validate the FCSRT for mild cognitive impairment ( MCI) and AD through the analysis of the diagnostic accuracy and the suggestion of cut-off scores. Patients were classified into two groups according to standard criteria: MCI ( n = 100) and AD ( n = 70). A matched control group ( n = 101) of cognitively healthy subjects was included. The reliability and the validity of the FCSRT were analysed on the immediate ( IR) and delayed ( DR) recalls. The Cronbach's alpha was 0.915 for the IR and 0.879 for the DR. The total recall measures revealed good areas under the curve for MCI ( IR: .818; DR: .828) and excellent for AD ( IR: .987; DR: .991). Furthermore, the MCI group was subdivided with respect to a non-similar/similar AD pattern of impairment, with almost half of the subjects showing an AD-like decline. This analysis represents a novel contribution regarding the properties of the FCSRT in illustrating the heterogeneity of MCI at baseline. The psychometric properties of the Montreal Cognitive Assessment (MoCA) were examined by using the Partial Credit Model. The study sample included 897 participants who were distributed into two main subgroups: (I) the clinical group (90 patients with Mild Cognitive Impairment, 90 patients with Alzheimer's disease, 33 patients with Frontotemporal Dementia, and 34 patients with Vascular dementia, whose diagnoses were previously established according to a consensus that was reached by a multidisciplinary team, based on the international criteria) and (II) the healthy group (composed of 650 cognitively healthy community dwellers). The results show(i) an overall good fit for both the items and the persons' values, (ii) high variability for the cognitive performance level of the cognitive domains (ranging between 1.90 and23.35, where "Short-term Memory" was the most difficult item and "Spatial Orientation" wasthe easiest item) and between the subjects on the scale, (iii) high reliability for the estimation of the persons' values, (iv) good discriminant validity and high diagnostic utility, and (v) a minimal differential item functioning effect related to of pathology, gender, age, and educational level. The Montreal Cognitive Assessment (MoCA) is a brief cognitive instrument for screening milder forms of cognitive impairment. The present study aimed to analyze the influence of sociodemographic (age, gender, educational level, marital and employment status, geographic region, geographic localization, and residence area) and health variables (subjective memory complaints of the participant and evaluated by the informant, depressive symptoms, and family history of dementia) on the participants' performance on the MoCA. The investigation was carried out in a Portuguese community-based sample of 650 cognitively healthy adults, who were representative of the distribution observed in the Portuguese population. Educational level and age significantly contributed to the prediction of the MoCA scores, explaining 49% of the variance. Regarding health variables, only the subjective memory complaints of the participant showed a small contribution (9%) to the variance on the MoCA scores.
