Zadnjih desetak godina došlo je do značajnih promjena u zdravstvenom zbrinjavanju bolesnika s incidentalomima nadbubrežne žlijezde koje su rezultirale novim preporukama za dijagnostiku i liječenje ovih bolesnika napravljenim u suradnji Europskoga endokrinološkog društva i Europske mreže za istraživanje tumora nadbubrežne žlijezde. Na temelju navedenih preporuka i višegodišnjega vlastitog iskustva Referentni centar Ministarstva zdravstva Republike Hrvatske za bolesti nadbubrežne žlijezde predlaže izmjene i dopune preporuka za dijagnostiku i liječenje ovih bolesnika objavljenih u Liječničkom vjesniku 2010. godine., In the last decade, the clinical care of patients with adrenal incidentaloma has been significantly changed which prompted the European Society of Endocrinology and European Network for the Study of Adrenal Tumors to develop an updated clinical practice guideline for the management of these patients. In accordance with the aforementioned recommendations and based on its own experience, the Croatian Referral Center for adrenal gland disorders provides an update of the previous guideline for the clinical management of patients with adrenal incidentaloma published in Liječnički vjesnik in 2010.

The aim of the study was to compare bone properties of two groups of students which strongly differ in the level of their everyday physical activity ; the School of Medicine (SM) students and the Faculty of Physical Education (FPE) students, University of Zagreb. Quantitative ultrasound parameters – broadband ultrasound attenuation (BUA) and speed of sound (SOS) were measured. Quantitative ultrasound index (QUI) and estimated bone mineral density (BMD) were calculated by the device software. The final study sample consisted of 165 students from SM (94 males and 71 females) and 215 students from the FPE (164 males, 51 females). Dijabetes je jedna od vodećih kroničnih nezaraznih bolesti u svijetu od koje boluje više od 425 milijuna ljudi, od čega ih je u Hrvatskoj preko 400 000. Kao takav predstavlja ne samo zdravstveni, nego i veliki ekonomski problem. Godišnje je u svijetu dijabetes odgovoran za trošak od 1,3 bilijuna američkih dolara, gotovo 2% ukupnog svjetskog BDP-a. Većina oboljelih je ili na terapiji inzulinom, ili će ga početi uzimati kasnije u tijeku bolesti, a cijene inzulinskih pripravaka rastu iz godine u godinu. Posljednjih godina na tržištu su se pojavili biosimilari inzulina, koji predstavljaju veliki potencijal za uštedu. U ovom radu uspoređeni su trendovi primjene biosimilara inzulina u Hrvatskoj s trendovima propisivanja u ostatku Europske unije. Izračunata je potrošnja svih inzulina u Hrvatskoj za 2018. izražena u DDD-u te su podatci o potrošnji inzulinskih biosimilara uspoređeni s podatcima o potrošnji u ostalim europskih zemljama, dobivenim iz izvještaja tvrtke IQVIA. U rezultatima je vidljivo da je po udjelu propisanih biosimilara inzulina Hrvatska na razini prosjeka za Europsku uniju. Međutim, gledajući udio propisanih biosimilara u odnosu na referentni lijek, Hrvatska je među vodećim zemljama u Europi, zaostajući samo za Slovačkom i Poljskom. S trenutnim razinama primjene biosličnog inzulina u Hrvatskoj postoji mnogo prostora za dodatnu uštedu. Dodatna ispitivanja po pitanju sigurnosti zamjene referentnih lijekova biosimilarima mogla bi biti presudna u povećanju njihova propisivanja i ostvarivanju značajnih ušteda., Diabetes, one of the world's leading chronic noncommunicable diseases, affects more than 425 million people worldwide, over 400,000 of which are in Croatia. As such, diabetes represents not only a serious health issue, but a giant economic problem as well. Each year, diabetes related costs amount to upwards of 1.3 trillion USD, almost 2% of global GDP. Most patients' therapies either include or will include insulin at some point in the future and prices of insulin preparations continue rising. Few years ago, biosimilar insulins have appeared on the market, offering great money saving potential. In this paper, biosimilar insulin usage in Croatia is compared with pharmacotherapy trends in other European Union countries. Total usage of all insulin preparations in Croatia for 2018 has been calculated and expressed in DDD. This data was then compared with information on biosimilar insulin usages in other European countries, which were obtained from IQVIA’s report. The results show that biosimilar market share in Croatia’s total insulin market is on par with their market share in the EU. However, when comparing the biosimilar insulin market shares in their referenced product markets, Croatia ranks as one of Europe’s leading countries, trailing only behind Slovakia and Poland. With current levels of biosimilar insulin proscribing in Croatia, there is still much room for additional savings. To achieve that, further studies are needed regarding safety of interchangeability between biosimilar insulins and their original counterparts.
