Since the outbreak of the first SARS-CoV-2 epidemic in China, pharmacists have rapidly engaged and developed strategies for pharmaceutical care and supply. According to the guidelines of the International Pharmaceutical Federation (FIP), clinical pharmacists/hospital pharmacists, as members of care teams, play one of the most important roles in the pharmaceutical care of patients with COVID-19. During this pandemic, many immuno-enhancing adjuvant agents have become critical in addition to antivirals and vaccines in order to overcome the disease more easily. The liquid extract obtained from the Pelargonium sidoides plant is used for many indications such as colds, coughs, upper respiratory tract infections, sore throat, and acute bronchitis. The extract obtained from the roots of the plant has been observed to have antiviral and immunomodulatory activity. In addition to its anti-inflammatory and antioxidant effects, melatonin plays a role in suppressing the cytokine storm that can develop during COVID-19 infection. Knowing that the severity and duration of COVID-19 symptoms vary within 24 hours and/or in different time periods indicates that COVID-19 requires a chronotherapeutic approach. Our goal in the management of acute and long COVID is to synchronize the medication regimen with the patient's biological rhythm. With the ongoing pandemic declared by the World Health Organization, the number of hospitalizations and deaths caused by COVID-19 is increasing dramatically. As new variants of SARS-CoV-2 emerge, new combination therapies are needed to reduce the risk of COVID-19spreadduring this time of increased transmission risk. In this case, it is vitalto strengthen the immune system against highly inflammatory conditions such as the cytokine storm caused by COVID-19. This brief review highlights the benefits of taking melatonin and the new antiviral drug molnupiravir together in the treatment of COVID-19. The number of hospitalizations and fatalities brought on by COVID-19 has considerably grown since the World Health Organization proclaimed the disease as a global pandemic. New combination therapy is required to lessen the risk of COVID-19 progression during this time when the danger of transmission rises as new Omicron sub-variants arise. In this situation, it is critical to boost the immune system to combat highly inflammatory conditions like the cytokine storm brought on by COVID-19. Furthermore, if administered early in COVID-19 illness, monoclonal antibodies (mAbs) that neutralize SARS-CoV-2 can minimize the chance of hospitalization. LY-CoV1404 (bebtelovimab) is one of these mAbs that has recently gained prominence due to its ability to effectively neutralize the SARS-CoV-2 virus and protect binding to spike proteins of several variants including B.1.1.529 (Omicron) and its sub-variants (BA.1, BA.1.1, and BA.2) with various essential receptor binding domain (RBD) mutations. This brief review emphasizes the advantages of combining melatonin with bebtelovimab, which has been demonstrated to be the most effective SARS-CoV-2 neutralizing monoclonal antibody against the Omicron variant in the management of COVID-19. This study suggests that the combination therapy for Omicron sub-variants is beneficial and could be regarded as adjuvant therapy for COVID-19 disease. [ABSTRACT FROM AUTHOR]
