Dit rapport bevat een erratum d.d. 23-03-2018 op de laatste pagina Veel (jonge) seksueel actieve vrouwen en mannen zijn geïnfecteerd met het humaan papillomavirus (HPV), dat baarmoederhalskanker veroorzaakt. Sinds 2010 worden 12-jarige meisjes via het Rijksvaccinatieprogramma gevaccineerd om baarmoederhalskanker te voorkomen. Inmiddels weten we meer over deze vaccinatie; zij beschermt niet alleen tegen kanker in de baarmoederhals, maar ook tegen kanker aan penis, anus, vagina en vulva. Ook zijn er aanwijzingen dat het tegen kanker van de mond- en keelholte beschermt. Naar aanleiding van deze nieuwe inzichten heeft de minister van VWS de Gezondheidsraad gevraagd te bekijken of het advies uit 2008 om meisjes te vaccineren moet worden aangepast. Om de Gezondheidsraad hierin te ondersteunen heeft het RIVM de relevante informatie uit binnen-en buitenland verzameld en gestructureerd. Het gaat bijvoorbeeld om informatie over de mate waarin HPV-infecties en HPV-gerelateerde ziekten voorkomen bij meisjes/vrouwen en jongens/mannen en over de effectiviteit en veiligheid van de vaccins. De laatste jaren hebben meer mannen een vorm van kanker gekregen die door het HPV-virus veroorzaakt kan worden. Er zijn drie vaccins tegen HPV op de markt die zowel voor vrouwen als mannen geschikt zijn. In Nederland wordt het vaccin gegeven dat tegen twee typen van het HPV-virus beschermd (HPV-typen 16 en 18). De andere twee vaccins beschermen tegen meerdere typen van het HPV-virus. 61 procent van de meisjes heeft zich laten vaccineren. Door het huidige vaccinatieprogramma zullen per jaar naar schatting 350 minder vrouwen baarmoederhalskanker krijgen en 100 vrouwen minder aan deze vorm van kanker overlijden.

Meningococcal disease is a very serious infectious disease caused by a bacterium, the meningococcus. There are different types of meningococcus; people become ill mainly from the B, C, W and Y serogroups. Since 2002, vaccination against serogroup C meningococcal disease has been included in the National Immunisation Programme for children of 14 months. As a result, serogroup C meningococcal disease has virtually disappeared. Vaccines against serogroup B have recently become available. In addition, since 2015, there has been a rapid increase in serogroup W meningococcal disease. Multi-component vaccines are available against A, C, W and Y serogroups. Based on these developments, among others, the Health Council will advise the Minister for Health, Welfare and Sport on whether and how the current immunisation programme against meningococcal disease should be adapted. To this end, RIVM has collected background information and recent data on meningococcal disease in the Netherlands. It includes the number of people in the Netherlands who become ill each year, the efficacy and safety of the vaccines, and what the public thinks about vaccination against invasive meningococcal disease. The infection causes a severe medical condition such as meningitis or blood poisoning, which can rapidly develop into shock, frequently causing death. The disease often begins with flu-like symptoms and fever which subsequently worsen very rapidly. The infection is relatively rare in the Netherlands; there are currently 100 to 150 patients a year. Five to ten percent of these patients die despite antibiotics and intensive care. Thirty percent of the patients are left with lifelong impairments such as hearing loss, limb amputation or epilepsy. Bij kinderen kan de vaccinatie een dubbel effect hebben: zij zijn zelf beschermd tegen de griep en de vaccinatie kan de kans verkleinen dat mensen in hun omgeving de griep krijgen. Er bestaan veel verschillende vaccins tegen de griep. De vaccins die nu in Nederland worden gebruikt, beschermen matig. Ze voorkomen een derde tot de helft van de infecties. Ook geldt: hoe ouder mensen zijn op het moment dat ze zich laten vaccineren, hoe minder het vaccin hen beschermt. Recente onderzoeken laten zien dat nieuwe vaccins oudere proefpersonen beter beschermen. Deze vaccins worden nog niet gebruikt in Nederland. Vanaf 2019-2020 zal een vaccin tegen vier typen griepvirus worden gebruikt in plaats van het huidige vaccin tegen drie typen.

Herpes zoster (also known as shingles) is caused by infection with the varicella zoster virus which can also cause varicella (also known as chickenpox). After primary infection (varicella), the virus remains inactive in the recipient's body. If, at a later stage, the virus becomes active, it can cause herpes zoster. In the Netherlands, the Minister of Health, Welfare and Sport (VWS) determines which vaccinations are offered nationally. The minister takes this decision based on advice from the Health Council. The Health Council is currently preparing a new advice regarding herpes zoster vaccination. In 2016, herpes zoster vaccination with the then available vaccine Zostavax® did not qualify for a national programme as the vaccine did not provide sufficient protection. According to the Health Council, the vaccination could be reconsidered as soon as a new vaccine became available. This was the case at the beginning of 2018 (Shingrix®). To support the Health Council's advisory report, the RIVM collected background information about vaccination for herpes zoster and the extent to which it occurs in the Netherlands. These overviews are created when the Health Council prepares an advisory report about a possible new vaccination. This document includes information about the number of people in the Netherlands who become ill every year, the efficacy and safety of vaccines, and the opinion of the public about vaccination for herpes zoster. Herpes zoster usually starts with itching, tingling or severe, burning or stabbing pain. After a few days, groups of vesicles appear on the body, usually around the abdomen or waist. After 10 to 14 days, the vesicles dry to crusts. Sometimes herpes zoster can cause serious complications, such as nerve pain (postherpetic neuralgia or PHN) or inflammation of the facial nerve. Nerve pain can persist after the vesicles have disappeared, sometimes for a long time. People rarely die from herpes zoster. The chance that someone gets herpes zoster is 23 to 30 percent; it is most common in adults aged over 50.

Het rotavirus kan een maagdarminfectie veroorzaken die veel voorkomt bij jonge kinderen en soms ernstig kan verlopen. Er zijn twee vaccins beschikbaar die beide in druppelvorm via de mond worden toegediend. De Gezondheidsraad gaat de minister van VWS adviseren op welke manier vaccinatie van kinderen tegen het rotavirus oegankelijk wordt. De minister neemt op basis van dit advies een besluit. Om de Gezondheidsraad te ondersteunen heeft het RIVM achtergrondinformatie over het rotavirus bijeengebracht. De informatie betreft onder andere het aantal personen dat er jaarlijks in Nederland ziek door wordt, de effectiviteit en veiligheid van de vaccins, en hoe het publiek denkt over deze vaccinatie. Een door het rotavirus veroorzaakte maagdarminfectie komt veel in de wintermaanden voor, vooral bij kinderen tussen de 6 maanden en 2 jaar. De ziekte gaat gepaard met koorts, braken en hevige, waterdunne diarree. Doorgaans verloopt de ziekte zonder problemen, maar het komt voor dat de ziekte ernstig verloopt. Dit gebeurt vaker bij jonge kinderen, te vroeg geboren kinderen, kinderen met een laag geboortegewicht, of kinderen met aangeboren afwijkingen. De ziekte kan dan uitdrogingsgevaar veroorzaken. In deze gevallen moet een kind in het ziekenhuis worden opgenomen. De uitdroging wordt dan behandeld door via de mond of een infuus vocht toe te dienen. In zeldzame gevallen overlijdt een kind.

Rotavirus can cause a gastrointestinal infection and is common in young children. There are two vaccines available; both have to be administered via the mouth. The Dutch Health Council will advise the Ministry of Health, Welfare and Sport on how childhood vaccination against rotavirus will be made available. The Minister makes a decision on the basis of this advice. To support the Health Council, the RIVM has put together background information on rotavirus disease. The information includes the number of people in the Netherlands that become ill from rotavirus every year, the effectiveness and safety of rotavirus vaccines, and how the public thinks about rotavirus vaccination. A gastrointestinal infection caused by rotavirus is common during the winter months, particularly in children between six months and two years old. The disease is characterized by fever, vomiting and diarrhoea. Usually, rotavirus disease resolves by itself without problems, but can be severe resulting in dehydration. This happens more often in young children, premature children, children with low birth weight and children with congenital problems. These severe cases may need to be admitted to the hospital. Treatment for dehydration consists of oral or intravenous rehydration. In rare cases, a child dies.
