Backgrounds and Aims: Diabetes mellitus is established risk factor for higher morbidity and mortality from myocardial infarction. The sclerosis of coronary arteries is the main cause of myocardial ischemia. The peculiarities of coronary arteries damage in patients with diabetes mellitus are not fully characterized. Therefore, the aim of this study was to examine the prevalence, degree and location of coronary arteries stenosis in patients with diabetes mellitus and clinical signs of ischemic heart disease undergoing coronary angiography compared to age-matched subjects with ischemic heart disease without diabetes mellitus. Materials and Methods: We examined 229 patients with ischemic heart diseases with diabetes mellitus (mean age 61.2+3.5 years) and 226 subjects without diabetes (mean age 58.2+4.1 years). We analyzed the coronarograms of patients studied. The comparison was performed using Fisher''s test. The results were considered statistically different if p<0.05. Results: We found that the prevalence of three-vessels coronary arteries stenosis was significantly higher in patients with diabetes mellitus compared to non-diabetic subjects – 37.9% and 23.8%, respectively, p<0.05. The prevalence of two-vessels coronary arteries stenosis was not different between the 2 groups of patients studied – 35.4% and 39.4%, respectively, p>0.05. The damage of the single coronary artery was more prevalent in those without diabetes – 19.2% and 32.7% in diabetic and non-diabetic subjects, respectively, p<0.05. Patients with diabetes had higher prevalence of the stenosis in the distal part of the circumflex branch of the left coronary artery – 30.1% and 17.3%, p<0.05. However, there was no statistically significant difference in the prevalence of coronary stenosis of other locations between subjects with and without diabetes mellitus. Conclusion: The revealed characteristics of coronary arteries stenosis could underlie the higher prevalence and worse outcome of myocardial infarction in patients with diabetes mellitus.
