Purpose: To compare clinical outcomes of three different techniques of combined cataract and Presbia Flexivue Microlens refractive corneal inlay (Presbia Coöperatief U.A., Amsterdam, Netherlands) implantation surgery for presbyopia compensation over a 12-month follow-up., Methods: In this comparative pilot study, 15 patients with bilateral cataract were allocated to one of three groups with a different combination of surgical steps (cataract surgery, intrastromal pocket creation, and inlay implantation). In the three-step group, the intracorneal pocket was created in the non-dominant eye, bilateral cataract surgery was performed 3 months later, and the intracorneal inlay was inserted 3 months after that. In the two-step at 3 days group, bilateral cataract surgery was performed 3 days after pocket creation and inlay implantation in the non-dominant eye. In the two-step at 3 months group, the pocket creation and the inlay implantation were performed in the non-dominant eye 3 months after bilateral cataract surgery. Visual, refractive, and contrast sensitivity outcomes were evaluated and compared between the three groups., Results: Twelve months after the inlay implantation, mean monocular uncorrected distance visual acuity was 20/32 in the three-step group, 20/32 in the two-step at 3 days group, and 20/25 in the two-step at 3 months group. Achieved mean monocular uncorrected near visual acuity was similar in the three groups (20/25). The visual and refractive outcomes did not show significant differences between groups. Contrast sensitivity was similar between groups under mesopic and photopic conditions. No intraoperative or postoperative complications were observed., Conclusions: Clinical outcomes of three different techniques of combined cataract surgery and refractive corneal inlay implantation had no apparent differences between them. The corneal inlay provided excellent near vision acuity, with high patient satisfaction and a high spectacle independence rate after cataract surgery. Purpose: The aim of this study was to present the long-term results of corneal collagen cross-linking (CXL) in patients with keratoconus., Methods: In this prospective, interventional case series, patients with progressive keratoconus underwent CXL treatment according to the Dresden protocol. Visual, refractive, and topographic outcomes along with endothelial cell density were evaluated preoperatively and at 1, 2, 3, 4, and 5 years postoperatively., Results: Twenty-one patients (25 eyes) were enrolled. The mean follow-up was 43.7 ± 12.2 (range, 24-60) months. Logarithm of the minimum angle of resolution (logMAR) mean uncorrected visual acuity and the mean best spectacle-corrected visual acuity improved significantly from 0.92 ± 0.54 and 0.29 ± 0.21 preoperatively to 0.63 ± 0.41 (P = 0.010) and 0.18 ± 0.18 (P = 0.011), respectively, at the last follow-up. Mean steep and mean flat keratometry readings reduced significantly from 52.53 ± 6.95 diopters (D) and 48.11 ± 5.98 D preoperatively to 49.10 ± 4.50 D (P < 0.001) and 45.58 ± 3.81 D (P = 0.001), respectively, at the last follow-up. The mean endothelial cell density was 2708 ± 302 cells per square millimeter preoperatively and did not change significantly (P > 0.05) at any postoperative interval (2593 ± 258 cells/mm at the last follow-up; P = 0.149)., Conclusions: CXL seems to be effective and safe in halting progression of keratoconus over a long-term follow-up period up to 5 years postoperatively.

Purpose: To present the long-term results of combined transepithelial phototherapeutic keratectomy (PTK) and corneal collagen crosslinking (CXL) for keratoconus., Setting: Vardinoyiannion Eye Institute of Crete, University of Crete, Heraklion, Crete, Greece., Design: Prospective case series., Methods: Patients with progressive keratoconus had combined transepithelial PTK and CXL (Cretan protocol). Visual and refractive outcomes and the endothelial cell density (ECD) were evaluated preoperatively and postoperatively., Results: Twenty patients (23 eyes) were enrolled; postoperatively 23 eyes were evaluated at 1 and 2 years, 11 at 3 years, and 7 at 4 years. The mean follow-up was 33.83 months±10.82 (SD) (range 24 to 56 months). No intraoperative or postoperative complications occurred. The mean uncorrected distance visual acuity improved significantly from 0.99±0.57 logMAR preoperatively to 0.61±0.36 logMAR at the last follow-up (P<.001) and the mean corrected distance visual acuity, from 0.27±0.24 logMAR to 0.17±0.14 logMAR (P=.018), respectively. The mean steep and mean flat keratometry readings decreased significantly from 53.39±7.14 diopters (D) and 47.17±4.87 D, respectively, preoperatively to 49.99±4.36 D (P<.001) and 45.47±2.95 D (P=.002), respectively, at the last follow-up. The mean corneal astigmatism improved significantly from -6.27±4.19 D preoperatively to -4.52±2.80 D (P<.001) at the last follow-up. Unlabelled: We present the case of a patient who had refractive corneal inlay implantation for near vision improvement after bilateral cataract surgery. The patient had a history of bilateral cataract, and a 2-step procedure was suggested to improve her near and distance visual acuities. The first step was bilateral cataract extraction with a power target of plano intraocular lens implantation. Six months later, a refractive corneal inlay, Presbia Microlens, was implanted in the nondominant eye in the intracorneal pocket made with the femtosecond laser. No intraoperative or postoperative complications occurred. The bilateral uncorrected near visual acuity improved from less than Jaeger (J)6 to J1 and remained stable during the 2-year follow-up. Purpose: To describe a new minimally invasive surgical technique for the symptomatic management of bullous keratopathy in blind eyes., Methods: Four patients with severe corneal edema due to endothelial decompensation and no visual function in the affected eye presented for the relief of their ocular symptoms (pain and tearing). Femtosecond laser technology was used to create a deep corneal pocket into which silicone oil was inserted., Results: After the procedure, all patients demonstrated immediate relief of their symptoms, along with restoration of a normal corneal surface 7 days after the procedure (no bullae and no epithelial defects). All patients remained free of symptoms during the entire follow-up period (from 24 to 31 months). Anterior to the inserted implant, the corneal lamellae remained compact, transparent, and without bullae; whereas the posterior corneal stroma under the implant was edematous. No intraoperative or postoperative complications were noted., Conclusions: Intracorneal insertion of silicone oil is a feasible new technique for the symptomatic treatment of bullous keratopathy in blind eyes.

Purpose: To use anterior segment optical coherence tomography (AS-OCT) to compare corneal stroma demarcation line depth after corneal collagen crosslinking (CXL) with 2 treatment protocols., Setting: Vardinoyiannion Eye Institute of Crete, Faculty of Medicine, University of Crete, Heraklion, Crete, Greece., Design: Prospective comparative interventional case series., Methods: Corneal collagen crosslinking was performed in all eyes using the same ultraviolet-A (UVA) irradiation device (CCL-365). Eyes were treated for 30 minutes with 3 mW/cm(2) according to the standard Dresden protocol (Group 1) or for 10 minutes with 9 mW/cm(2) of UVA irradiation intensity (Group 2). One month postoperatively, 2 independent observers measured the corneal stroma demarcation line using AS-OCT., Results: Sixteen patients (21 eyes) were enrolled. Group 1 comprised 7 patients (9 eyes) and Group 2, 9 patients (12 eyes). The mean corneal stroma demarcation line depth was 350.78 μm ± 49.34 (SD) (range 256.5 to 410 μm) in Group 1 and 288.46 ± 42.37 μm (range 238.5 to 353.5 μm) in Group 2; the corneal stroma demarcation line was statistically significantly deeper in Group 1 than in Group 2 (P=.0058, t test for unpaired data)., Conclusion: The corneal stroma demarcation line was significantly deeper after a 30-minute CXL treatment than after a 10-minute CXL procedure with high-intensity UVA irradiation., (Copyright © 2014 ASCRS and ESCRS. Purpose: To present the results after simultaneous conventional photorefractive keratectomy combined with corneal collagen cross-linking for pellucid marginal corneal degeneration., Methods: In this prospective, interventional case series, 6 patients (8 eyes) with pellucid marginal corneal degeneration were enrolled. All patients underwent simultaneous conventional photorefractive keratectomy combined with corneal collagen cross-linking; corneal epithelium was removed by transepithelial phototherapeutic keratectomy during treatment (Cretan protocol plus conventional photorefractive keratectomy). Visual and refractive outcomes were evaluated along with endothelial cell density preoperatively and at 1, 3, 6, and 12 months postoperatively., Results: No intraoperative or postoperative complications were observed in any of the patients. LogMAR mean uncorrected distance visual acuity improved significantly from 1.05 ± 0.33 preoperatively to 0.41 ± 0.27 (P = .018) at 12 months postoperatively. Mean corrected distance visual acuity did not change significantly (P > .05) postoperatively. Mean spherical equivalent improved significantly from -3.52 ± 2.29 diopters preoperatively to -1.57 ± 1.76 diopters (P = .028) at last follow-up. Mean corneal astigmatism was significantly reduced from -6.83 ± 2.33 diopters preoperatively to -4.71 ± 1.89 diopters (P = .018) at the last follow-up. No endothelial cell density alterations were observed throughout the follow-up period (P > .05)., Conclusions: Simultaneous conventional photorefractive keratectomy combined with corneal collagen cross-linking seems to be an effective, safe, and promising treatment for the management of pellucid marginal corneal degeneration., (Copyright 2014, SLACK Incorporated.)

Background/aim: To compare stromal bed smoothness in LASIK flaps created with two different femtosecond lasers (IntraLase FS150 and WaveLight FS200) and a mechanical microkeratome (MMK) (Carriazo-Pendular microkeratome)., Methods: Sixty freshly enucleated porcine eyes were used for the study. Twenty flaps were created and constitute each of the following three groups: IntraLase FS150 (IFS group), WaveLight FS200 (WFS group) and MMK (MMK group). In each of the three groups, 10 flaps were created with intended thickness of 110 μm and another 10 flaps with 130 μm. Images were assessed with light microscopy and scanning electron microscopy. Qualitative surface roughness grading of the images was performed by five masked observers and statistical comparisons of scores were made between groups., Results: The results of qualitative surface roughness grading demonstrated that there was no significant difference between the two femtosecond (FS) lasers when comparing the subgroups with the same flap thickness (p>0.05 in both comparisons, Mann-Whitney U test). In addition, there was no statistically significant difference (p>0.05) in each flap maker group between different thickness subgroups. However, the scores of the FS laser subgroups were significantly better than the scores of the MMK subgroups with corresponding flap thicknesses (p<0.05, Mann-Whitney U test)., Conclusions: Our study demonstrates that the IntraLase FS150 and the WaveLight FS200 had a similar performance and provided flaps with smooth surfaces. In comparison to the MMK, the studied femtosecond laser systems had a superior performance in terms of smoothness.

Purpose: To evaluate the results of thin-flap laser in situ keratomileusis (LASIK) using a femtosecond-laser platform for flap creation., Setting: Institute of Vision and Optics, Faculty of Medicine, University of Crete, Heraklion, Greece., Design: Prospective interventional case series., Methods: Patients had LASIK with the FS200 femtosecond laser for flap creation and the Allegretto Wave Eye-Q excimer laser. Flap thickness was set at 105 μm. All eyes were examined 1 month postoperatively. Flap thickness was assessed with anterior segment optical coherence tomography using the manual flap tool at 5 locations on a horizontal B scan., Results: This study comprised 50 eyes of 25 patients (mean age 28 years ±5.72 [SD]); 42 eyes completed 6 months of follow-up. Preoperatively, the mean sphere was -3.61 ± 1.87 diopters (D) and the mean cylinder, -1.08 ± 1.23 D. Six months postoperatively, no eye lost lines of corrected distance visual acuity (CDVA), 29% gained 1 line, and 7% gained 2 lines. The mean spherical equivalent was -0.03 ± 0.42 D (range -0.88 to +0.88 D); 86% had an uncorrected distance visual acuity of 20/20 or better. The mean central flap thickness at 1 month was 102.98 ± 6.33 μm (range 91 to 114 μm). There were no intraoperative or postoperative complications., Conclusions: No significant complications occurred after treatment with this new femtosecond-laser platform in thin-flap LASIK. Background: The purpose of this study is to evaluate in-vitro the immediate effect of corneal collagen cross-linking (CXL) on corneal hydration and stiffness., Methods: Forty-two corneal buttons from freshly enucleated porcine eyes were immersed in riboflavin 0.1% in dextran 20% dilution for 3 h in order for their hydration to reach equilibrium. Corneal buttons where divided into two groups; the first group was stored in dark conditions while the other group was irradiated with UV radiation (370 nm) for 30 min to simulate CXL according to the clinically applied protocol. After irradiation, all corneas were immersed in dextran 20% solution for 3 additional hours. Subsequently, each button underwent weighing, thickness measurement, and was mounted in a special device for the measurement of force versus deformation by compression. Finally, all corneal buttons were dehydrated for 48 h in a desiccating oven set at 62 °C and weighed again to obtain their dry mass. Hydration (%) of each button was calculated., Results: Mean corneal hydration in the irradiated and the non-irradiated group of corneas was 69.8 and 72.2%, respectively (p < 0.001). Differences in thickness and compressibility were not statistically significant. Thickness and hydration were positively correlated (Pearson's r = 0.714, p < 0.001)., Conclusions: CXL causes corneal dehydration that can be detected immediately after the procedure. This phenomenon may contribute to increased mechanical stiffness of the cornea. A change in stiffness by means of compressibility could not be detected in porcine corneas.

Purpose: To evaluate the outcomes and safety of a refractive inlay (Flexivue Micro-Lens, Presbia Coöperatief U.A.) for the corneal compensation of presbyopia., Methods: This prospective, interventional clinical study comprised 47 emmetropic presbyopes with a mean age of 52±4 years (range: 45 to 60 years). The inlay was inserted, centered on the line of sight, inside a corneal pocket created in the patient's nondominant eye, using a femtosecond laser. Follow-up was 12 months. Visual acuity, corneal topography, wavefront aberrometry, contrast sensitivity, structural corneal alterations, and questionnaires were evaluated., Results: Twelve months after surgery, uncorrected near visual acuity was 20/32 or better in 75% of operated eyes, whereas mean uncorrected distance visual acuity (UDVA) of operated eyes was statistically significantly decreased from 0.06±0.09 logMAR (20/20) (range: -0.08 to 0.26) preoperatively to 0.38±0.15 logMAR (20/50) (range: 0.12 to 0.8) (P<.001), and mean binocular UDVA was not significantly altered (P=.516). Seventeen patients lost one line of corrected distance visual acuity in the operated eye. No patient lost 2 lines in CDVA in the operated eye. Overall, higher order aberrations increased and contrast sensitivity decreased in the operated eye. No tissue alterations were found using corneal confocal microscopy. No intra- or postoperative complications occurred., Conclusions: Twelve months after implantation, the Flexivue Micro-Lens intracorneal refractive inlay seems to be an effective method for the corneal compensation of presbyopia in emmetropic presbyopes aged between 45 and 60 years old., (Copyright 2013, SLACK Incorporated.)

Purpose: To investigate the outcomes and safety of an intrastromal refractive inlay (Invue Lens, Biovision AG) for the corneal compensation of presbyopia., Methods: This prospective, noncomparative, interventional clinical study comprised 45 emmetropic presbyopes with a mean age of 52.3 ± 3.3 years (range: 47 to 58 years). The refractive inlay was inserted in the non-dominant eye within a corneal pocket that was created using a mechanical microkeratome. Last follow-up was 12 months in all patients and examinations were scheduled at 1 week and 1, 3, 6, and 12 months after surgery. Visual, wavefront, contrast sensitivity, and topographic outcomes as well as structural corneal alteration were evaluated., Results: Twelve months after surgery, uncorrected near visual acuity was 20/32 or better in 98% of operated eyes and binocularly, whereas uncorrected distance visual acuity was 20/40 or better in 93% of operated eyes and 20/25 or better in all patients binocularly. Three patients lost one line of corrected distance visual acuity in the operated eye. Overall, higher order aberrations were increased and contrast sensitivity was decreased in the operated eye. No tissue alterations were found using corneal confocal microscopy. No intra- or postoperative complications occurred., Conclusions: The intracorneal refractive inlay (Invue Lens) seems to be an effective surgical method for the corneal compensation of presbyopia in emmetropes aged between 45 and 60 years., (Copyright 2012, SLACK Incorporated.)

Purpose: To describe the technique of femtosecond laser-assisted intracorneal pocket creation, using a mask for the implantation of a corneal inlay in the non-dominant eye of a 56-year-old, emmetropic, presbyopic woman for the correction of presbyopia., Methods: Using the iFlap treatment-type software of the femtosecond laser (IntraLase 150, Abbott Medical Optics), a full lamellar cut was created at 280-μm depth. A keyhole-shaped mask was placed at the internal part of the glass of the applanation cone. A separator was used to separate the stroma, and an inserter was used to implant the inlay at the center of the line of sight. To determine the line of sight, the microscope and centration system of the excimer laser (Allegretto Wave 400 Hz, WaveLight Laser Technologie AG) were used. The Flexivue Microlens inlay (Presbia) was implanted., Results: No intra- or postoperative complications occurred during follow-up. Uncorrected near visual acuity improved from 20/50 to 20/20 at the first postoperative week., Conclusions: Femtosecond-assisted intracorneal pocket creation using a mask is a simple, safe, and efficient approach for the implantation of corneal inlays., (Copyright 2011, SLACK Incorporated.)

A 29-year-old woman with keratoconus underwent posterior chamber toric implantable Collamer lens (Visian ICL; STAAR Surgical, Monrovia, CA) implantation 12 months after corneal collagen cross-linking with riboflavin and ultraviolet radiation-A. No intraoperative or postoperative complications were observed. Three months postoperatively, uncorrected distance visual acuity improved from counting fingers to 20/40 and corrected distance visual acuity improved from 20/100 to 20/30. Combined cross-linking treatment and posterior chamber toric phakic Collamer lens implantation in a two-step procedure seems to be an effective method for correcting keratoconus in patients with high myopia and astigmatism. Purpose: To report complications after the implantation of intrastromal corneal ring segments (Keraring; Mediphacos, Belo Horizonte, Brazil) in keratoconic eyes using a femtosecond laser (IntraLase; Advanced Medical Optics, Santa Ana, California, USA) for channel creation., Methods: Retrospective chart review of 531 patients (850 eyes) who underwent Keraring insertion using a femtosecond laser for channel creation. Intraoperative and postoperative complications were recorded., Results: Intraoperatively, there were 22 (2.7%) cases of incomplete channel formation. The rest of the intraoperative complications were galvo lag error (system malfunction) [five eyes (0.6%)], endothelial perforation [five eyes (0.6%)] and incorrect entry of the channel [two eyes (0.2%)]. Postoperatively, there were 11 (1.3%) cases of segment migration, two (0.2%) cases of corneal melting and one (0.1%) case of mild infection. The overall complication rate was 5.7% (49 cases out of 850 eyes)., Conclusion: In this study, intracorneal ring segment implantation using a femtosecond laser for channel creation was related to a number of complications. In this case series, 2 patients with post-laser in situ keratomileusis corneal ectasia were included. Patients were treated with new intrastromal corneal ring segments, INTACS SK (severe keratoconus or steep "K") (Addition Technology, Inc, Des Plaines, IL) implantation. Two segments were inserted without any intraoperative or postoperative complications. Three months after the procedure, uncorrected visual acuity was improved in both patients from counting fingers preoperatively to 20/40 and 20/50, respectively. Best spectacle-corrected visual acuity improved from 20/40 to 20/32 and from 20/50 to 20/40. Manifest refraction improved from -8.75/-5.00 x 125 and -14.25/-6.50 x 33 preoperatively to -5.50/-2.25 x 125 and -8.25/-1.50 x 15 with an improvement in topographic findings. One year postoperatively, uncorrected visual acuity, best spectacle-corrected visual acuity, and topographic findings remained improved. In conclusion, new INTACS SK might be able to improve visual acuity in patients with post-laser in situ keratomileusis corneal ectasia.

Background: We describe a modified method for deepitheliazation prior to corneal cross linking (CXL). The technique may overcome the current corneal pachymetric limitation parameter (over 400 microns) that is necessary for the safety of the procedure without affecting the overall benefits., Methods: In a series of two patients, with inferior topographic steepening and regional thinning (less than 400 microns corresponding to the area of corneal steepening), CXL after customized epithelial removal was performed., Results: There were no intra- or postoperative adverse events seen by the nine month follow up examination. Stabilization of the corneal ectasia was observed up to nine months post-costumized pachymetric-guided epithelial removal., Conclusion: The technique of customized pachymetric-guided epithelial removal is easy to perform and may overcome the limitations of the preoperative corneal pachymetry expanding the application of the procedure in patients with regional corneal thinning.

Purpose: To study the outcomes of LASIK after intended thin flap creation using the SCHWIND Carriazo-Pendular microkeratome with the 90-microm head., Methods: Forty-seven eyes of 26 patients (mean age 28.78+/-6.98 years) underwent LASIK with a superior hinge flap using the 90-microm head of the SCHWIND Carriazo-Pendular microkeratome. Evaluation included flap parameters (thickness, diameter, hinge size), complications, and visual outcome., Results: No intraoperative or early postoperative complications were observed. The mean flap thickness was 79.88+/-6.94 microm (range: 70 to 93 microm). Mean flap diameter was 9.25+/-0.45 mm (range: 8.5 to 11 mm) whereas mean hinge size was 4.63+/-0.66 mm (range: 3 to 6.5 mm). No eye lost lines of best spectacle-corrected visual acuity, and all eyes were emmetropic within one diopter postoperatively., Conclusions: The SCHWIND Carriazo-Pendular microkeratome with the 90-microm head seems to have increased accuracy in intended creation of thin flaps.

Purpose: To compare the early postoperative course and the 1-year clinical results of off-flap Epi-LASIK and Epi-LASIK for the treatment of low and moderate myopia., Design: Pilot double-masked, randomized, comparative study., Participants: Fifty-six patients (112 myopic eyes)., Methods: Epithelium was separated in all eyes with the use of Centurion SES epikeratome (Norwood Abbey EyeCare, Vic, Australia). The first eye treated and surgical method in the first eye were randomized. One eye of each patient underwent standard Epi-LASIK, whereas in the contralateral eye, the epithelial sheet was not retained on the photoablated stroma (off-flap Epi-LASIK eyes). Mean preoperative spherical equivalent was -3.50+/-1.22 diopters (D; range, -1.75 to -6.37 D) in Epi-LASIK eyes and -3.61+/-1.22 D (range, -1.50 to -6.50 D) in off-flap Epi-LASIK eyes (P>0.05, paired Student t test). Excimer laser corneal ablation was performed using the Allegretto 200Hz (Wavelight Laser Technologie AG, Erlangen, Germany). Patients were followed up daily until the epithelial healing was complete and at 1, 3, 6, and 12 months., Main Outcome Measures: Epithelial healing time, subjective pain score, and uncorrected visual acuity (UCVA) were evaluated during the first postoperative days. Uncorrected visual acuity, spherical equivalent refraction, best spectacle-corrected visual acuity, haze scores, and wavefront aberrations were recorded at all subsequent intervals., Results: Time of epithelial healing did not differ significantly in Epi-LASIK and off-flap Epi-LASIK eyes (4.76+/-0.84 days in Epi-LASIK eyes vs. 4.54+/-0.93 days in off-flap Epi-LASIK eyes). No significant difference in UCVA was found after the 2 techniques during the first postoperative days. Subjective pain score was lower in off-flap Epi-LASIK eyes at 2 postoperative hours, whereas no significant difference in pain scores was noted between the 2 techniques at the other intervals. There was no significant difference in spherical equivalent, line gain or loss, haze scores, and higher-order aberrations between Epi-LASIK and off-flap Epi-LASIK eyes at any interval. Uncorrected visual acuity was significantly better in Epi-LASIK eyes only at 6 months (-0.05+/-0.08 in Epi-LASIK eyes vs. 0.00+/-0.07 in off-flap Epi-LASIK eyes). Purpose: To report the results of intrastromal corneal ring segment [ICRS] (KeraRing; Mediphacos, Belo Horizonte, Brazil) implantation using a femtosecond laser (IntraLase Corp, Irvine, California, USA) in keratoconic patients., Design: Retrospective, noncomparative, interventional study., Methods: Thirty-two keratoconic patients (50 eyes) who underwent ICRS insertion using a femtosecond laser for channel creation and completed at least one year of follow-up were included in this study. Uncorrected visual acuity (UCVA), best spectacle-corrected visual acuity (BSCVA), refraction, topographic findings, and adverse events were assessed., Results: No intraoperative complications were demonstrated in this series of patients. At the first postoperative day, segment migration to the incision site was seen in three eyes (6%; early postoperative complication). To avoid melting, we repositioned the migrated segment away from the incision site. Serious second migration was not seen and we did not need to reposition any segment again. At the last postoperative examination, there was a statistically significant reduction in the spherical equivalent refractive error compared with that observed at the examination before implantation (mean +/- standard deviation, -5.62 +/- 4.15 diopters [D; range, -23.62 to 0.50 D] to -2.49 +/- 2.68 D [range, -11.12 to 3.5 D]; P < .001). The UCVA before implantation was 20/40 or worse in 47 eyes (94%; range, counting fingers to 20/30), whereas at the last follow-up examination, 14 (28%) of 50 eyes had a UCVA of 20/40 or better (range, counting fingers to 20/25). Nine eyes (18%) maintained the preimplantation BSCVA, whereas 39 eyes (68%) experienced a BSCVA gain of one to four lines at the last follow-up examination. Only in two eyes (4%; two patients) with advanced keratoconus (stage III) was there a decrease of up to two lines. Despite this deterioration in BSCVA, the patients did not want to remove the ICRSs, because there was an increase of UCVA. No late postoperative complications were observed during the follow-up period., Conclusions: ICRS (KeraRing) implantation using femtosecond laser for tunnel creation is a minimally invasive procedure for improving visual acuity (both UCVA and BSCVA) in keratoconic patients.

Purpose: To evaluate the accuracy and consistency of corneal flap thickness, horizontal diameter, and hinge size with the Moria M2 90-microm single use head., Methods: Fifty-two myopic patients (104 eyes), mean age 32.6 years, underwent bilateral LASIK with a superior hinged flap using the Moria M2 microkeratome (90-microm single use head). Prospective evaluation included flap thickness (subtraction method), diameter, hinge size, interface particles, intraoperative complications, and visual recovery., Results: The mean preoperative spherical equivalent refraction was -5.72 +/- 2.59 diopters (D) (range: -2.88 to -10.75 D) and -5.84 +/- 2.73 D (range: -3.13 to -9.38 D) for right and left eyes, respectively. The mean preoperative central corneal thickness was 548 +/- 24 microm and 547 +/- 25 microm for right and left eyes, respectively. The mean preoperative steepest K was 44.12 +/- 1.28 D and 44.41 +/- 1.27 D for right and left eyes, respectively. Corneal diameter (white-to-white) was 12 +/- 0.4 mm and 11.9 +/- 0.4 mm for right and left eyes, respectively. The mean postoperative flap thickness was 109 +/- 18 microm (range: 67 to 152 microm) and 103 +/- 15 microm (range: 65 to 151 microm) for right and left eyes, respectively. The mean postoperative flap diameter was 9.4 +/- 0.3 mm (expected mean according to the nomogram given by the company was 9.5 mm). The mean postoperative hinge chord was 4.4 +/- 0.4 mm (expected mean 4.2 mm). No interface particles were detected on slit-lamp examination., Conclusions: The Moria M2 90-microm single use head is safe with reasonable predictability for LASIK flap creation.

Purpose: To study the long-term efficacy, safety, and stability of laser in situ keratomileusis (LASIK) after unintentional ultrathin flap creation less than 80 mum., Setting: University refractive surgery center., Methods: This retrospective case series comprised 25 patients (33 eyes) who had LASIK after flap creation less than 80 mum with the Moria M2 disposable microkeratome (head 90 microm). Flap thickness was measured with intraoperative ultrasound pachymetry. Manifest refraction, uncorrected visual acuity (UCVA), best spectacle-corrected visual acuity, predictability, stability, complications, patient satisfaction, and confocal microscopy images were studied., Results: The mean follow-up was 14.58 months +/- 3.73 (SD) (range 12 to 25 months). The mean flap thickness was 72.1 +/- 5.9 microm (range 58 to 80 microm), and the mean preoperative spherical equivalent (SE) refraction was -5.59 +/- 2.01 diopters (D) (range -10.25 to -3.25 D). On the first postoperative day, the UCVA was 20/25 or better. The SE manifest refraction was 1.00 D or less in all patients. The mean SE manifest refraction was -0.75 +/- 0.55 D (range -1.00 to +0.75 D) (P < .01). At the last follow-up, changes in visual acuity and manifest refraction were not statistically significant; no late postoperative complications were observed. All patients were satisfied with the final outcome. Qualitative analysis of confocal microscopy images revealed interface particles and activated keratocytes., Conclusions: Despite the small sample and retrospective nature of the study, superficial LASIK seemed to be a safe and predictable technique for myopic refractive corrections. Patients were satisfied with the results and had rapid visual recovery with no intraoperative or early or late postoperative complications. If the safety and efficacy of an ultrathin flap are confirmed by additional studies, superficial LASIK could represent a new approach that combines the advantages of surface and lamellar procedures.

Purpose: To determine the morphologic changes in human corneas over time following radiofrequency-based conductive keratoplasty (CK) treatment., Design: Prospective, observational case series., Methods: In a single-center study six human corneas of six patients with localized peripheral keratoconus underwent CK treatment followed by penetrating keratoplasty. Three spots were applied in the periphery of each cornea (6 mm optical zone). Corneal buttons were examined with light and electron microscopy at different postoperative intervals up to 6 months post-CK., Results: In samples assessed on day one post-CK, small areas of detachment between the basal layer of epithelial cells and Bowman's layer were observed. At 1 week after the CK procedure, the epithelium appeared almost normal. Endothelium and Descemet's membrane had no alterations. In all samples, thermally induced misconfiguration of collagen fibers, described as "crumpled" changes of collagen layers, was observed reaching 75% to 80% of the stromal depth. The area of alterations had a cylindrical shape with a diameter of 120 mum., Conclusions: The conductive keratoplasty procedure produced collagen "crumpling and splitting" changes in human corneas, which were observed during the follow-up of 6 months. Areas adjacent to treatment site were minimally damaged.

Purpose: To report the intraoperative management of incomplete microkeratome LASIK flaps., Design: Small, retrospective, noncomparative, interventional case series., Participants: Five outpatients., Methods: In 5 eyes of 5 patients, the premature stop of the microkeratome resulting from mechanical obstacles resulted in an incomplete flap that would not allow for the completion of the LASIK procedure. After the careful realignment of the partial flaps and the removal of the identified obstacles, the second pass of the microkeratome resulted in flaps of the intended size., Main Outcome Measures: Final creation of a flap as intended, completion of the procedure, and visual and refractive outcome of the operative eyes., Results: The second pass of the microkeratome resulted in flaps as planned. All the procedures were completed, and the operative eyes that were followed up for at least 3 months had excellent visual and refractive results., Conclusions: The second pass of the microkeratome can successfully manage incomplete flaps resulting from microkeratome premature stop as a result of mechanical obstacles.

Purpose: To assess the long-term safety, efficacy, predictability, and stability of conductive keratoplasty (CK) for the treatment of low to moderate hyperopia and to evaluate the impact of the procedure on the quality of vision., Setting: University of Crete, Medical School, Vardinoyannion Eye Institute of Crete, Crete, Greece., Methods: In this prospective nonrandomized noncontrolled single-center study, 38 eyes of 26 patients (13 women and 13 men) were treated for hyperopia with a Refractec ViewPoint CK system and followed for 30 months. Preoperatively, the mean manifest refraction spherical equivalent (MRSE) was +1.89 diopters (D) +/- 0.6 (SD) (range +1.00 to +3.25 D), and the mean follow-up was 30.9 +/- 1.1 months. All eyes were treated with the regular CK nomogram for the treatment of spherical hyperopia. The treatment consisted of 8 to 32 spots applied to the periphery of the cornea. Mean age was 50.3 +/- 8.8 years (range 31 to 71 years). All treated eyes were analyzed for safety, efficacy, predictability, and stability., Results: At 12 months, the MRSE was -0.06 +/- 0.8 D and at 30 months was -0.02 +/- 0.7 D. At 30 months, the mean MRSE was within +/-0.50 D in 68%, within +/-1.00 D in 92%, and within +/-2.00 D in all eyes. At 30 months, uncorrected visual acuity was 20/20 or better in 52.5% and 20/40 or better in 89% of eyes. No eye lost 2 or more Snellen lines or had an induced cylinder of 2.00 D or greater. The procedure did not cause statistically significant changes in contrast sensitivity., Conclusion: Results show that CK for low to moderate hyperopia is a safe, effective, predictable, and stable procedure.

A 42-year-old man had Intacs (Addition Technology Inc.) implantation for early pellucid marginal degeneration (PMD). Two Intacs segments (0.45 mm thickness) were inserted uneventfully in the fashion typically used for low myopia correction (nasal-temporal). Eleven months after the procedure, the uncorrected visual acuity was 20/200, compared with counting fingers preoperatively, while the best spectacle-corrected visual acuity improved to 20/25 from 20/50. Corneal topographic pattern also improved. Although the results are encouraging, concern still exists regarding the long-term effect of this approach for the management of patients with PMD.

A 56-year-old woman had conductive keratoplasty (CK) for residual hyperopia and astigmatism. Three years before the procedure, the patient had arcuate keratotomy, followed by laser in situ keratomileusis 2 years later for high astigmatism correction in both eyes. During CK, a corneal perforation occurred in the right eye; during the postoperative examination, an iris perforation and anterior subcapsule opacification were seen beneath the perforation site. The perforation was managed with a bandage contact lens and an antibiotic-steroid ointment; it had a negative Seidel sign by the third day. The surgery in the left eye was uneventful. Three months after the procedure, the uncorrected visual acuity was 20/32 and the best corrected visual acuity 20/20 in both eyes with a significant improvement in corneal topography. Care must be taken to prevent CK-treated spots from coinciding with areas in the corneal stroma that might have been altered by previous refractive procedures.

Background: To compare the corneal healing response between conventional and phototherapeutic keratectomy through a masking agent, in rabbit corneas., Methods: 24 adult rabbits underwent phototherapeutic keratectomy. Animals were divided in two groups: 12 received photoablation through a masking agent (PALM gel) and the remaining 12 received conventional phototherapeutic keratectomy of equal depth and served as control. Light and transmission electron microscopy was performed in specimens of both groups obtained: immediately after, four hours, one week, one, three and six months after treatment., Results: Reepitheliazation was complete within five days in all eyes. Light and transmission electron microscopy did not reveal any differences of the healing process in the experimental eyes compared to the controls., Conclusion: Photoablation through the PALM technique did not result any evident alterations of the reepithelisation and stromal healing process.

Purpose: To prospectively study the effects of the use of Intacs microthin prescription inserts for the management of keratoconus., Design: Prospective nonrandomized clinical trial., Methods: Thirty-three eyes of 26 keratoconus patients (17 males and 9 females) ages 21 to 51 years (mean age, 32 +/- 9.7 years) were included in the current study. All patients had clear central corneas and contact lens intolerance. Patients were excluded if any of the following criteria applied after the preoperative examination: previous intraocular or corneal surgery; history of herpes keratitis; diagnosed autoimmune disease; and systemic connective tissue disease. Two Intacs segments of 0.45-mm thickness were inserted in the cornea of each eye, aiming at embracing the keratoconus area to try to achieve maximal flattening. Preoperative examination included uncorrected visual acuity (UCVA), best-corrected visual acuity (BCVA), manifest refraction, keratometric data, and corneal topography., Results: Intacs were successfully implanted in all eyes. In one eye Intacs were removed after 3 months because of their improper (superficial) placement. The follow-up ranged from 1 to 24 months (mean: 11.3 months). The mean UCVA significantly improved from 0.13 +/- 0.14 (range, counting fingers [CF]-0.5) to 0.39 +/- 0.27 (range, CF-1.0) (P <.01). Of 33 eyes, 2 eyes lost 1 line of UCVA, and 3 eyes maintained the preoperative UCVA, whereas the rest (28 eyes) experienced a 1- to 10-line gain. The mean BCVA also improved from 0.47 +/- 0.31 (range, CF-1.0) to 0.64 +/- 0.26 (range, 0.1-1.0) (P <.01). Of 33 eyes, 4 eyes experienced 1- to 2-line loss of BCVA, 4 eyes maintained the preoperative BCVA, whereas the rest (25 eyes), experienced a 1- to 6-line gain. Of 3 patients (3 eyes) with unsatisfactory results, 1 patient improved with one segment removal and in 2 patients the segments were permanently removed. One of these eyes underwent successful PKP., Conclusions: With mean follow-up of 11.3 months, intracorneal ring segments implantation improved UCVA and BCVA in the majority of the keratoconus patients. Even though the results are encouraging, concern still exists regarding the predictability as well as the long-term effect of such an approach for the management of keratoconus.

Purpose: To determine how refractive error, visual acuity, and high-order aberrations (3rd- and 4th-order) are affected by the formation of a lamellar corneal flap during laser in situ keratomileusis (LASIK)., Setting: University refractive surgery center., Methods: The effect of lamellar corneal flap formation was analyzed in 15 myopic eyes (mean preoperative refraction -4.72 diopters [D] [range -1.25 to -7.25 D]). The flap was created using a 2-step procedure: (1). a nasally hinged lamellar corneal flap was created; (2). the flap was lifted and stromal ablation performed 2 months after the flap was made. A Hartmann-Shack aberrometer was used to measure the aberrations., Results: There was no significant change in the refractive error (spherical equivalent pre-flap -4.72 +/- 1.99 D and post-flap -4.62 +/- 1.99 D [P =.28]) or visual acuity (pre-flap uncorrected visual acuity [UCVA] 0.07 and best corrected visual acuity [BCVA] 0.96; post-flap UCVA 0.08 and BCVA 0.95 [P =.16 and P =.33, respectively]). A statistically significant increase in total higher-order wavefront aberrations was observed following flap formation (root mean square pre-flap 0.344 +/- 0.125 and post-flap 0.440 +/- 0.221 [P =.04])., Conclusion: Flap formation during LASIK can modify the eye's existing natural higher-order aberrations (especially spherical and coma-like aberrations along the axis of the flap's hinge), while visual acuity and refractive error remain unaffected.

Purpose: To evaluate the safety and efficacy of intrastromal corneal ring segments (ICRS, INTACS, Keravision) for the correction of corneal ectasia after laser in situ keratomileusis (LASIK)., Methods: In this prospective, noncomparative case series, INTACS were implanted in three eyes (two patients) that were ectatic after LASIK. Mean follow-up was 8.7 months (range, 8 to 10 mo)., Results: No intraoperative complications occurred. After INTACS implantation, uncorrected visual acuity ranged from 20/20 to 20/25 at 6 months and remained stable until 9 months. Mean postoperative spherical equivalent refraction was 0.50 D at 1 month and no significant changes appeared up to 9 months after INTACS implantation. After INTACS implantation, no eye lost any lines of Snellen spectacle-corrected visual acuity and two eyes gained from one to two lines. There was an increase in topographical regularity in all three eyes., Conclusion: Implantation of INTACS in eyes with corneal ectasia after LASIK resulted in good refractive outcome, absence of complications, and improvement in visual acuity.
