Purpose: Neutrophils may be involved in the local pathophysiology of chronic graft-versus-host disease (cGVHD). We evaluated neutrophil infiltration in cGVHD using conjunctival impression cytology (IC) and its clinical correlation with ocular surface status and neutrophil enzyme levels in tears., Methods: This cross-sectional observational study included 76 patients with cGVHD. The ocular surface was assessed for the tear break-up time, Schirmer I test, corneal and conjunctival staining score, meiboscore, and the ocular surface disease index questionnaire. Conjunctival IC was performed at the temporal, superior bulbar, and upper palpebral conjunctiva, and the number of neutrophils (cells/high power field [HPF]) was calculated. Neutrophil elastase (NE), myeloperoxidase (MPO), and matrix metalloperoxidase-8 and -9 levels in tear washes were measured in 20 patients., Results: The number of neutrophils was significantly greater at the upper palpebral conjunctiva (median [range], 16.5 [0 to 147] cells/HPF) than at the temporal and superior bulbar conjunctiva (0 [0 to 70] and 0 [0 to 105] cells/HPF; P < 0.0001). The number of neutrophils at the upper palpebral conjunctiva showed moderate correlations with the corneal staining score and the NE and MPO levels in tears (r = 0.668, 0.553, and 0.563, respectively; P < 0.0001, P = 0.014, and 0.012)., Conclusions: Our results suggest that neutrophils at the upper palpebral conjunctiva associate with the clinical manifestations and inflammatory status of the ocular surface in cGVHD. Conjunctival neutrophils should be addressed when assessing the inflammatory activity of ocular cGVHD and exploring its pathogenesis., (Copyright © 2018 Elsevier Inc. All rights reserved.)

To evaluate surgically induced refractive change (SIRC) by manifest refraction and corneal power changes using an automated keratometer and Scheimpflug rotating camera, and to find the best keratometric measurements reflecting SIRC after hyperopic laser-assisted in situ keratomileusis (LASIK).This retrospective study included 18 eyes of 18 patients undergoing hyperopic LASIK using the Schwind Amaris 750S excimer laser. All measurements were performed preoperatively and 12 months postoperatively. Cycloplegic manifest refractions were performed and keratometric measurements were obtained via an RK-5 automated keratometer and a Pentacam rotating Scheimpflug camera. Sim K, true net power (TNP), and total corneal refractive power (TCRP) at 2.0 to 5.0 mm were analyzed using a Scheimpflug camera.The mean manifest refractive changes in the spherical equivalent (SE) at the corneal plane were 2.32 ± 1.65 D at 12 months postoperatively. The refractive power changes by the automated keratometer and Sim K were significantly less than SIRC (P = .043 and P = .048, respectively). Both TNP and the TCRP in the 5.0 mm zone produced lesser mean differences with SIRC (0.05 D and 0.06 D) and showed closer agreements with SIRC on Bland-Altman plots and higher correlation coefficients with SIRC.Corneal power measured on the anterior corneal surface underestimated SIRC. TCRP at the 5.0 mm zone provided by a Pentacam Scheimpflug camera reflected the SIRC accurately and precisely, and would be applicable for prediction of intraocular power before cataract surgery and follow-up measurement of corneal refractive power.

Purpose: To analyze internal spherical aberration in pseudophakic eyes that underwent aspheric intraocular lens (IOL) implantation, and to investigate the relationships between biometric data and the effectiveness of aspheric IOL implantation., Methods: This retrospective study included 40 eyes of 40 patients who underwent implantation of an IOL having a negative spherical aberration of -0.20 μm (CT ASPHINA 509M; Carl Zeiss Meditec Inc., Germany). The IOLMaster (version 5.0; Carl Zeiss AG, Germany) was used for preoperative biometric measurements (axial length, anterior chamber depth, central corneal power) and the measurement of postoperative anterior chamber depth. The spherical aberrations were measured preoperatively and 3 months postoperatively using the iTrace (Tracey Technologies, Houston, TX, USA) at a pupil diameter of 5.0 mm. We investigated the relationships between preoperative biometric data and postoperative internal spherical aberration, and compared biometric measurements between 2 subgroups stratified according to internal spherical aberration (spherical aberration ≤ -0.06 μm vs. spherical aberration > -0.06 μm)., Results: The mean postoperative internal spherical aberration was -0.087 ± 0.063 μm. Preoperative axial length and residual total spherical aberration showed statistically significant correlations with internal spherical aberration (p = 0.041, 0.002). Preoperative axial length, postoperative anterior chamber depth, IOL power, and residual spherical aberration showed significant differences between the 2 subgroups stratified according to internal spherical aberration (p = 0.020, 0.029, 0.048, 0.041 respectively)., Conclusion: The corrective effect of an aspheric IOL is influenced by preoperative axial length and postoperative anterior chamber depth. Not only the amount of negative spherical aberration on the IOL surface but also the preoperative axial length should be considered to optimize spherical aberration after aspheric IOL implantation.

Purpose: We sought to analyze surgically induced refractive change (SIRC) and change in high-order aberration after Raindrop corneal inlay insertion (ReVision Optics, Lake Forest, CA), and assess the extent to which Raindrop corneal inlay insertion could correct presbyopia., Design: Interventional case series., Methods: Seventeen patients were included if they had a corneal thickness ≥500 μm and a stable manifest spherical equivalent refraction between 0.50 and +1.00 diopters (D). The Raindrop corneal inlay was implanted on the stromal bed of a femtosecond laser-assisted generated flap of nondominant eyes. Manifest refraction, corneal powers, and corneal high-order aberrations were measured preoperatively and at 3 and 12 months postoperatively., Results: The SIRC by manifest refraction was 0.99 ± 0.26 D. The changes derived from simulated keratometry (K), true net power, and equivalent K reading (EKR) at 1.0-4.0 mm were greater than the SIRC (all P < .01) while the change in EKR at 6.0 mm was less than the SIRC (P < .01). The changes in EKR 5.0 mm, automated K, and EKR 4.5 mm did not differ significantly from the SIRC (P = .81, .29, and .09, respectively), and the difference was the least for EKR 5.0 mm. In analysis of high-order aberrations, only spherical aberration showed statistically significant difference between preoperative and postoperative on both anterior cornea and total cornea (all P < .01)., Conclusion: Raindrop corneal inlay corrects presbyopia via increasing negative spherical aberration. The equivalent K reading at 5.0 mm accurately reflected the SIRC, and would be applicable for intraocular power prediction before cataract surgery., (Copyright © 2016 Elsevier Inc. Purpose: To report the long-term outcomes of corneal astigmatism after cataract surgery., Methods: The study included 55 eyes of 46 patients who underwent cataract surgery with temporal 3-mm clear corneal incisions from January 2001 to December 2003. All patients underwent complete ophthalmological examination including keratometry at the preoperative visit and at 2 months and 10 years after surgery. Only those eyes that underwent a follow-up of 10 years or longer from the time of cataract surgery were enrolled. Arithmetic and vector analyses were performed to obtain the change in corneal astigmatism with advancing age., Results: The mean age of the patients was 59.11 ± 12.33 years (range: 18 to 75 years), and 58.7% of the patients were women. The mean follow-up period was 131.28 ± 14.36 months. The average magnitudes of surgically induced astigmatism and long-term astigmatism change were 0.66 ± 0.42 and 0.57 ± 0.47 diopters (D), respectively. The vector difference was 0.56 ± 0.55 D at 90° ± 0° between the preoperative and 2-month visits. However, there was no difference in vector values between postoperative 2 months and 10 years (0.00 ± 0.74 D at 0° ± 0°)., Conclusions: Patients with corneal astigmatism who underwent cataract surgery did not show a long-term against-the-rule change in corneal astigmatism during a 10-year follow-up. These results provide useful information when planning toric intraocular lens implantation in patients with corneal astigmatism. Purpose: To evaluate changes in intraocular pressure (IOP) in recipient and donor eyes during femtosecond laser-assisted keratoplasty (FLAK) and to assess for differences in the diameter of trephinated corneal buttons according to changes in pressure., Materials and Methods: Twenty porcine whole eyes (recipient model) and 20 porcine-corneoscleral rims (donor model) were prepared, and anterior chamber pressures were measured using a fiberoptic sensing device (Opsens, Quebec, Canada) during the femtosecond laser corneal cutting process. To determine the diameter of corneal buttons, 10 porcine whole eyes (recipient model) and 12 corneoscleral rims (donor model) of each baseline IOP were cut with the femtosecond laser programmed to the following pattern: "vertical side cut"; 1200 μm (depth), 8 mm (diameter). Digital photographs were obtained using microscopy and subsequently analyzed., Results: The IOP (mean ± SD) for the recipient model was 10.2 (±0.9) mm Hg at baseline and ranged from 96.6 (±4.5) to ∼138.4 (±3.8) mm Hg during the corneal cutting process. This shows that the maximum IOP during FLAK increased 13.5 times compared with baseline. In the donor model, the mean pressure elevation from baseline artificial anterior chamber (AAC) pressure to corneal cutting was 15.8 (±5.4) mm Hg. This showed a positive correlation with baseline IOP [correlation coefficient (CC) = 0.827, P = 0.006]. As the baseline IOP in the recipient eye increased, trephinated corneal button size was reduced by up to 3.9% in diameter (CC = -0.945, P = 0.015). In addition, in donor eyes, the diameter was decreased by up to 11.7% as the baseline AAC pressure increased (CC = -0.934, P = 0.006)., Conclusions: During the FLAK procedure, the IOP increases in both recipient and donor eyes. The diameter of the trephinated donor and recipient corneal buttons was decreased as the initial baseline IOP increased. Ophthalmic surgeons can determine the AAC pressure based on the baseline IOP in the recipient patient.

Background:   To compare keratometric values obtained with a manual keratometer (Topcon), an automated keratometer (Canon), an Orbscan II (Bausch & Lomb), the IOLMaster keratometer (Carl-Zeiss) and the Pentacam rotating Scheimpflug camera (Oculus) in cataract surgery, and to characterize the refractive outcomes generated using each device., Design:   Retrospective study conducted at a tertiary university hospital., Participants: Sixty-nine eyes of 69 patients were analysed., Methods:   The keratometric values obtained with different devices (manual keratometer, automated keratometer, corneal topography, IOLMaster keratometer and Scheimpflug camera) were employed for intraocular lens power calculation. Multiple comparisons of averaged keratometric value were conducted, and the averaged keratometric value was used to calculate the predicted refraction. The absolute values of corneal astigmatism were calculated and also compared., Main Outcome Measures:   Mean keratometric value, absolute value of astigmatism, mean error and mean absolute error from each device., Results:   The mean keratometric values generated by manual keratometer, automated keratometry, corneal topography, IOLMaster keratometer and the Pentacam Scheimpflug system were 43.95 ± 1.39, 43.91 ± 1.39, 44.67 ± 1.53, 44.03 ± 1.41 and 42.96 ± 1.39 diopter, respectively. The absolute value of astigmatism determined via manual keratometer, automated keratometer, corneal topography, IOLMaster keratometer and the Pentacam Scheimpflug system were 0.95 ± 0.60, 0.99 ± 0.69, 1.14 ± 0.74, 1.11 ± 0.65 and 1.03 ± 0.73 diopter, respectively. The corneal topography showed statistically significant differences with other devices and produced the greater value in mean absolute errors (all P < 0.05)., Conclusion:   Keratometric values with standard devices are a good choice for cataract surgery, whereas the corneal topography is not an appropriate method for the assessment of preoperative keratometric values., (© 2012 The Authors.
