Purpose: To develop and evaluate the performance of a 3-dimensional (3D) deep-learning-based automated digital gonioscopy system (DGS) in detecting 2 major characteristics in eyes with suspected primary angle-closure glaucoma (PACG): (1) narrow iridocorneal angles (static gonioscopy, Task I) and (2) peripheral anterior synechiae (PAS) (dynamic gonioscopy, Task II) on OCT scans., Design: International, cross-sectional, multicenter study., Participants: A total of 1.112 million images of 8694 volume scans (2294 patients) from 3 centers were included in this study (Task I, training/internal validation/external testing: 4515, 1101, and 2222 volume scans, respectively; Task II, training/internal validation/external testing: 378, 376, and 102 volume scans, respectively)., Methods: For Task I, a narrow angle was defined as an eye in which the posterior pigmented trabecular meshwork was not visible in more than 180° without indentation in the primary position captured in the dark room from the scans. For Task II, PAS was defined as the adhesion of the iris to the trabecular meshwork. The diagnostic performance of the 3D DGS was evaluated in both tasks with gonioscopic records as reference., Main Outcome Measures: The area under the curve (AUC), sensitivity, and specificity of the 3D DGS were calculated., Results: In Task I, 29.4% of patients had a narrow angle. The AUC, sensitivity, and specificity of 3D DGS on the external testing datasets were 0.943 (0.933-0.953), 0.867 (0.838-0.895), and 0.878 (0.859-0.896), respectively. For Task II, 13.8% of patients had PAS. The AUC, sensitivity, and specificity of 3D DGS were 0.902 (0.818-0.985), 0.900 (0.714-1.000), and 0.890 (0.841-0.938), respectively, on the external testing set at quadrant level following normal clinical practice; and 0.885 (0.836-0.933), 0.912 (0.816-1.000), and 0.700 (0.660-0.741), respectively, on the external testing set at clock-hour level., Conclusions: The 3D DGS is effective in detecting eyes with suspected PACG. Purpose: To examine the influence of anterior segment optical coherence tomography imaging protocols on the intraobserver and intrasession repeatability of epithelial, stromal, and total corneal thickness measurements., Methods: Repeated anterior segment optical coherence tomography (AS-OCT) images (Spectralis, Heidelberg) were obtained from 15 adults using single 8.3 mm wide horizontal line scans with an average of 2, 10, 20, 30, 50 and 100 B-scans. Volumetric scans consisting of nine 8.3 mm horizontal line scans encompassing a 1.3 mm vertical region were also captured (with 20 B-scans per line scan). Single point thickness measures (at the normal to the tangent of the anterior corneal surface) were compared with thickness measures averaged over the central 6 mm. The impact of B-scan averaging and intraobserver variability were examined for single line scans. For volumetric scans, the impact of the number of line scans upon intraobserver and intrasession variability were calculated., Results: Intraobserver repeatability did not vary significantly as a function of the number of averaged B-scans per line scan, but was lowest for 20-30 averaged B scans. For volumetric scans, increasing the number of line scans did increase scan duration (p < 0.001), with minimal impact upon the average scan quality index (p = 0.06). Averaging more than 3 line scans did not significantly improve intraobserver or intrasession repeatability for either single point or average thickness measurements., Conclusion: AS-OCT volumetric scans with 3 lines each consisting of 20 B-scans with measurements averaged over a central 6 mm of the cornea provide highly repeatable measures of epithelial, stromal and total corneal thickness (95 % LoA ≤ ±3.2 μm for intraobserver repeatability and ≤ ±3.7 μm for intrasession repeatability). Purpose: To assess the use of a hydrogel ocular bandage (HOB) on clear corneal incisions in phacoemulsification cataract surgery and determine whether HOB reduces ocular surface contaminants ingress after routine surgery., Methods: In this prospective randomized controlled study, thirty eyes of patients undergoing uneventful phacoemulsification were recruited consecutively and randomly assigned to have a HOB applied to the corneal incision at the end of the surgery or not. At the end of the surgery, trypan blue (TRB) was instilled on the ocular surface, aqueous fluid was aspirated from the anterior chamber (AC) and its optical density was measured using ultraviolet spectrophotometry. The corneal incisions were examined postoperatively using anterior segment optical coherence tomography. Main outcome measures were concentration of TRB in the AC, corneal incision architecture, intraocular pressure (IOP) and Seidel test., Results: All incisions were Seidel negative. The mean IOP in the immediate postoperative period was 18.1 ± 5.48 mmHg for the intervention group and 16.9 ± 5.7 mmHg for the control group (p > 0.05). No architectural differences of the incisions between the two groups were noted. The total mean length of the three-step corneal incisions in the two groups was 2261.2 ± 96.92 μm and 2263 ± 119.75 μm, respectively (p > 0.05). No trace of TRB was detected in any of the samples., Conclusion: Proper surgical wound construction without the use of a HOB is efficient in preventing the ingress of fluid through the main corneal incision postoperatively., (© 2014 Acta Ophthalmologica Scandinavica Foundation. Context: Chemical injury to eyes causes limbal stem cell deficiency (LSCD), which leads to conjunctival epithelium and underlying stromal changes. Simple limbal epithelial transplant helps to restore corneal epithelium and corneal stromal clarity. These changes are documented in this prospective eye study. Purpose: To report the changes in epithelial/stromal (E/S) reflectivity ratio and corneal stromal densitometry following simple limbal epithelial transplantation (SLET) in patients with unilateral chemical burn-induced LSCD. Methods and Material: It is a prospective imaging study of cornea before and after SLET. Corneal densitometry with Pentacam and epithelial and stromal reflectivity on anterior segment optical coherence tomography were analyzed for five patients (n = 5 eyes), who underwent autologous SLET for unilateral chemical burn-induced LSCD. Statistical Analysis Used: Mann–Whitney U-test and Pearson correlation. Results: A significant improvement in E/S reflectivity was noted at 1 month (P < 0.05) after SLET, and this was maintained until the end of 6 months. The densitometry measurements decreased in the entire cornea at every level, that is, anterior, central, and posterior cornea. However, the values did not normalize till the last follow-up at 6 months. Conclusions: A trend of normal corneal epithelization exists in eyes undergoing SLET. Densitometry at all the levels, the anterior, central, and posterior cornea, also decreases after SLET. Objective: To quantify rate of post-lens tear thickness change under scleral contact lenses in keratoconus patients and normal cornea volunteers. Materials and Methods: We conducted a prospective observational study where semi-scleral lenses were fitted to 20 eyes (5 subjects, 10 eyes in each group). After insertion, post-lens tear thickness was measured at 0, 30 minutes and every hour up to 6 hours using Anterior Segment OCT. To analyze post-lens tear thickness and its rate of change at each time point, both within and between groups, a linear mixed model was used. Results: The initial mean post-lens tear thickness (μm) was 742 ± 50 and 440 ± 50 in keratoconus and normal cornea group, respectively. The mean rate of change (μm/hr) was highest in the first 30 minutes in both groups (80.8 ± 8.7, 132.2 ± 8.8 in keratoconus and normal cornea group). Following the first four hours in keratoconus and the first hour in normal cornea group, the reduction rate in post-lens tear thickness exhibited no statistically significant difference from the rate of change observed during the 5-6 hour period. The mean percentage of total change over 6 hours after lens insertion was higher in normal cornea compared to keratoconus group (36.6% vs 22.5%). Conclusion: The reduction in post-lens tear thickness follows a nonlinear pattern. The mean rate of change was highest after insertion and remained stable after four hours in keratoconus and one hour in normal cornea group. Intraoperative OCT (iOCT) is an innovative imaging modality that provides ophthalmic surgeons with real-time cross-sectional views of the surgical field. Recent advances in OCT technology, such as higher acquisition scanning rates, enable real-time video visualization. iOCT systems are widely used in both vitreoretinal and anterior segment surgeries. In corneal surgeries, iOCT imaging aims to optimize efficacy and safety by improving depth perception and enhancing visualization in cases of opaque optical media. iOCT is a valuable tool not only for experienced corneal surgeons, but also for training novice surgeons. This review summarizes the emerging applications of iOCT in corneal surgery, particularly in technically demanding lamellar keratoplasty procedures, as well as in various other corneal diseases and complications that require surgical intervention. BackgroundDescemet membrane endothelial keratoplasty (DMEK) has been utilized more frequently during recent years to treat penetrating keratoplasty (PKP) graft failures. The perioperative evaluation technique of anterior segment optical coherence tomography (AS-OCT) is increasingly significant. Our goal is to discuss DMEK surgical and clinical for subsequent PKP graft failure, along with significant surgical modifications and adjustments in accordance with preoperative assessment utilizing AS-OCT. Materials and MethodsPatients’ records who performed DMEK for PKP failure were retrospectively reviewed. Demographic information, PKP graft size determined by postoperative problems, corneal donor endothelial cell density (ECD), AS-OCT, central pachymetry, visual acuity (VA) evaluated in Snellen units, intraoperative surgical procedure modifications, and postoperative ECD were all included in the data collection. ResultsThe observation was conducted with 16 patients with 16 eyes, nine males and seven females. The observation period is 18 months. DMEK was performed at an average age of 63. Preoperative AS-OCT was performed on all patients, and based on cases, surgical plans were created. Before processing DMEK, the mean VA is 0.04, and central pachymetry is 685 m. They improved considerably to 0.3 (P value = 0.001) and 542 m (P value = 0.008) at the most recent follow-up. About 93.75% of the grafts were adhered to after the procedure. Late decompensation caused a 6.25% graft failure rate. Graft detachment rates and cases requiring rebubble rates were respectively 18.75%. ConclusionIn DMEK for failed PKP, a good case-specific preoperative assessment by AS-OCT is essential. As a result, it relies on developing a surgical strategy that can improve surgical outcomes, lower the risk of complications, and quicken visual recovery.

To describe the changes in corneal epithelial thickness and reflectivity following simple limbal epithelial transplantation (SLET) using anterior segment optical coherence tomography (ASOCT). This was a prospective imaging study of 31 eyes of 29 patients who had undergone autologous or allogeneic SLET for limbal stem cell deficiency and had stable, avascular surfaces postoperatively. ASOCT scans of all four quadrants were taken preoperatively and at 1-week, 1-month, 3-month, 6-month, and 1-year postoperative time points in the eyes undergoing SLET. Baseline scans were also taken from the normal eye in unilateral cases. The following parameters were obtained from the scans: (i) epithelial thickness (ET), (ii) stromal thickness, (iii) total corneal thickness, (iv) epithelial reflectivity (ER), (v) stromal reflectivity, and (vi) epithelial/stromal (ES) reflectivity ratio. Chemical injury (24 eyes, 77.4%) was the most common indication for surgery. There was a significant improvement in the ET (184.8 ± 117.1 vs 60.3 ± 10 µm, p <.0001) and ER (144.5 ± 26.4 vs 120.9 ± 28.9, p <.0001) within the initial postoperative period following SLET, which remained stable at the end of 1 year follow-up. There was no difference in the ET of the post-SLET and normal eyes after the 3-month timepoint and this normalization was maintained until the end of the first year. A significant normalization was also noted in the ES reflectivity ratio at the end of 1 year (2.1 ± 0.8 vs 1 ± 0.2, p <.001). A significant correlation was found between the final visual acuity and the total corneal thickness (r = 0.942, p =.005). There is a significant improvement in the epithelial thickness and reflectivity of the cornea in eyes undergoing SLET. To discuss and evaluate new technologies for a better diagnosis of corneal diseases and limbal stem cell deficiency, the outcomes of a consensus process within the European Vision Institute (and of a workshop at the University of Cologne) are outlined. Various technologies are presented and analyzed for their potential clinical use also in defining new end points in clinical trials. The disease areas which are discussed comprise dry eye and ocular surface inflammation, imaging, and corneal neovascularization and corneal grafting/stem cell and cell transplantation. The unmet needs in the abovementioned disease areas are discussed, and realistically achievable new technologies for better diagnosis and use in clinical trials are outlined. To sum up, it can be said that there are several new technologies that can improve current diagnostics in the field of ophthalmology in the near future and will have impact on clinical trial end point design. Purpose: The purpose of the study was to describe findings demonstrated by anterior segment spectral domain optical coherence tomography (AS-OCT) in various types of deeply embedded corneal foreign bodies. Methods: In this experimental study, an ex vivo model of calf eyes was used and seven different foreign bodies were deeply embedded in the cornea, consisting of five different materials: glass, plastic, metal, wood, and pencil graphite. The eyes were photographed and then scanned by AS-OCT. The images were analyzed to determine distinguishing characteristics for each material. Results: Various materials presented unique characteristics in AS-OCT. The opaque materials (pencil graphite, metals, and wood) demonstrated a hyper-reflective anterior border, whereas the posterior border signal could not be clearly identified due to the shadowing effect. Moreover, a chain of signals was characteristic of both pencil graphite and metals though a "mirroring effect" was unique for metals. Wood, as an opaque material, appears as a hyper-reflective mass with a spectrum of penetrability depending on the degree of concentration. Transparent materials demonstrated hyper-reflective sharp borders when surrounded by air or fluid, as opposed to when being embedded purely in the corneal stroma. Conclusions: This study shows that AS-OCT was used to systematically define a novel set of distinguishing characteristics specific to various materials extruding from the cornea, fully embedded in the cornea, and intruding into the anterior chamber. Hopefully, the described characteristics of each material can aid clinicians in diagnosing the type of the material embedded and the depth of its involvement in ocular injury. [ABSTRACT FROM AUTHOR]

Corneal oedema following cataract surgery during the immediate postoperative period is a common clinical problem, often due to endothelial injury due to multi-factorial mechanisms. Descemet’s membrane detachment (DMD) is an underrecognized cause of postoperative corneal oedema. The understanding of the pathogenic mechanisms of DMD is still evolving. We report a case of DMD in a 64-year-old female presenting with immediate postoperative corneal oedema, diagnosed and monitored using anterior segment optical coherence tomography (AS OCT). Various mechanisms have been proposed for surgically induced DMD. We believe that hydration of surgical wounds at the end of surgery can inadvertently cause DMD, if DMD has not been recognized previously. The association between myopia control efficacy in children treated with orthokeratology and corneal epithelial thickness is still unknown. The aim of this study was to explore the corneal epithelial thickness and its association with axial length changes in children treated with orthokeratology. This retrospective cohort study enrolled children aged from 9 to 15 years who had received orthokeratology for myopia control and had been followed up for at least 1 year. Anterior segment optical coherence tomography was performed to generate wide epithelial thickness maps of the patients. Annual axial length changes were calculated from the axial length at 6 months after the initiation of orthokeratology lens wear and at final measurements. Corneal epithelial thickness data were obtained from 24 sectors and a central 2 mm zone of the wide epithelial thickness map. Associations between annual axial length changes and corneal epithelial thickness for each sector/zone of the wide epithelial thickness map, and orthokeratology treatment data were determined by generalized estimating equations. Finally, a total of 83 eyes of 43 patients (mean age 11.2 years) were included in the analysis. The mean annual axial length change was 0.169 mm; when regressing demographic and ortho-k parameters to mean annual axial length changes, age and target power were both negatively associated with them (β = −14.43, p = 0.008; β = −0.26, p = 0.008, respectively). After adjusting for age and target power, the annual axial length changes were positively associated with the corneal epithelium thickness of IT1, I1, SN2, and S2 sectors of the wide epithelial thickness map, and negatively with that of the I3 sector. In conclusion, we identified associations between annual axial length changes and the corneal epithelium thickness of certain sectors in children treated with orthokeratology. Changes in the limbal microvasculature following a chemical eye injury are essential for prognosis and management. At the slit lamp, it can be difficult to assess, here using fluorescein and indocyanine green angiography we show that anterior segment angiography may be informative to assess objectively the limbal microvascular changes over the follow-up period. Background: Both pterygium ingrowth and excision determine alterations in corneal topography. The aim of this study was to evaluate the influence of pterygium removal combined with conjunctival autografts in addition to the use of human fibrin tissue glue on changes in corneal parameters as measured by 3-D swept-source anterior segment optical coherence tomography (AS-OCT) imaging. Methods: Sixteen eyes (16 patients) with pterygium that qualified for surgical treatment were enrolled in this study. Eye examination, slit lamp, and 3-D AS-OCT (CASIA 2) assessment were performed before the surgery and 7 days, 1 month, and 6 months after pterygium excision. Topographic parameters of both anterior and posterior surfaces of the cornea were analysed at each follow-up visit. Results: The gradual decrease in total astigmatism power from preoperative median 2.75 (6.15) D to 1.2 (1.1) D at 6-month follow-up (p = 0.034) was noted from the day 7 visit. Values were strongly influenced by variations of anterior cornea astigmatism. In contrast, a gradual total HOA reduction at the 1-month (from median 0.79 (1.3) D to 0.44 (0.27) D; p = 0.038) and at 6-month visits (0.25 (0.09); p = 0.001) was observed. Similarly, values were strongly influenced by variations of the anterior. Additionally, total average keratometry values increased from preoperative 44.05 (2.25) D to 44.6 (1.9) (p = 0.043) 1 month after the surgery. Conclusions: Significant steepening of the anterior cornea and a reduction in both astigmatism and HOA were observed after pterygium excision. The anterior corneal surface was an essential component of the total postoperative corneal topography values. Three-dimensional swept-source AS-OCT imaging seems to be a valuable tool for monitoring both the progression of the disease and postoperative effects in pterygium eyes. [ABSTRACT FROM AUTHOR]
