The systematic investigation of susceptibility-induced contrast in MRI is important to better interpret the influence of microvascular and microcellular morphology on DSC-MRI derived perfusion data. Recently, a novel computational approach called the Finite Perturber Method (FPM), which enables the study of susceptibility-induced contrast in MRI arising from arbitrary microvascular morphologies in 3D has been developed. However, the FPM has lower efficiency in simulating water diffusion especially for complex tissues. In this work, an improved computational approach that combines the FPM with a matrix-based finite difference method (FDM), which we call the Finite Perturber the Finite Difference Method (FPFDM), has been developed in order to efficiently investigate the influence of vascular and extravascular morphological features on susceptibility-induced transverse relaxation. The current work provides a framework for better interpreting how DSC-MRI data depend on various phenomena, including contrast agent leakage in cancerous tissues and water diffusion rates. In addition, we illustrate using simulated and micro-CT extracted tissue structures the improved FPFDM along with its potential applications and limitations.

Purpose: We determined whether reperfusion damage was sufficient to allow extravasation of a large molecular weight contrast agent into infarcted pig myocardium., Material and Methods: Five pig hearts were subjected to in situ occlusion of the left anterior descending coronary artery (2 h) followed by reperfusion (1 h). The hearts were excised and perfused in the Langendorff mode for ex vivo MR imaging. Polylysine-Gd-DTPA (50,000 Da) and Gd-DTPA (500-700 Da) were injected into the aorta (alternately) and followed by measurements of T1 relaxation and mean transit time (MTT)., Results: In the normal myocardium, MTT of Gd-DTPA (56.8+/-23.2 s) was significantly (p=0.02) longer than that of polylysine-Gd-DTPA (29.0+/-7 s). However, both normal and infarcted myocardium showed similar MTT (29.0+/-7.0 vs. 28.0+/-5.0 s, p>0.05) when using polylysine-Gd-DTPA., Conclusion: The results indicate that the permeability of capillaries to polylysine-Gd-DTPA was not significantly higher in infarcted regions of the myocardium compared to normal tissue. However, infarcted myocardium displayed an increased permeability to the small molecular weight Gd-DTPA. We conclude that microvascular damage may not be sufficient to allow the extravasation of polylysine-Gd-DTPA in infarcted myocardium.

Study Design: A comparison of contrast enhancement in the intervertebral disc from two magnetic resonance imaging contrast media in experimental animals., Objectives: To test the effect of molecular weight on the diffusion of ionic contrast media into the intervertebral disc., Summary of Background Data: Intravenously administered gadopentetate diffuses similarly into the fibrocartilage of intervertebral discs and herniated disc fragments. Differentiation between recurrent disc fragments and scar tissue via magnetic resonance imaging is optimized by using contrast media, which result in different contrast enhancement of these two tissues. Contrast media of higher molecular weight diffuse more slowly into cartilage; hypothetically, therefore, such media will produce better contrast between scar tissue and recurrent disc fragments., Methods: Gadopentetate (molecular weight 546) or gadolinium-polylysine (molecular weight 40,000) was injected intravenously into rabbits. The signal intensities of intervertebral disc and muscle tissue were recorded by magnetic resonance imaging at baseline and at pre-determined intervals for 2 hours after injection of the contrast medium. Contrast enhancement in these tissues was calculated in each animal for each contrast medium, and differences in enhancement were tested for significance by a growth-curve model., Results: Contrast enhancement in the intervertebral disc was significantly less with gadolinium-polylysine than with gadopentetate. In muscle, no significant difference between the two media was observed., Conclusions: Molecular weight affects the diffusion of paramagnetic contrast media into the intervertebral disc. Contrast media of a high molecular weight may produce better contrast between recurrent herniated disc fragments and scar tissue than contrast media of lower molecular weight. This possibility should be rested in further studies.

Purpose: To obtain better insight into the process of dye extravasation in the human choroid during sodium fluorescein angiography of the ocular fundus., Methods: Sodium fluorescein angiography using a scanning laser ophthalmoscope was performed on the right eye of five healthy volunteers. The dose of sodium fluorescein was chosen to avoid quenching in the ocular vessels. The time-intensity curve of background fluorescence was measured and its shape compared with that of the time-intensity curve of a retinal artery, representing an intravascular compartment. Significant dye extravasation is expected to cause a marked change in shape of the time-intensity curve of background fluorescence as compared with the intravascular time-intensity curve due to long washout of dye from the interstitial space of the choroid., Results: In all five subjects, the time-intensity curve of background fluorescence and the retinal artery proved to be quite similar in shape until several seconds after peak intensity of the first bolus passage., Conclusion: Background fluorescence originates mainly from intravascular compartments during the early phase of the sodium fluorescein angiogram.
