Full-arch fixed implant-supported prostheses have shown high biologic success rates after long-term follow-up, along with technical and prosthetic complications. Adequate planning is necessary for immediate implant occlusal loading in a completely edentulous maxilla or mandible. For totally edentulous patients, the main prosthetic problem is the transfer of diagnostic information to the master cast when an immediate loading approach is used. The aim of this clinical case report is to illustrate a new prosthetic protocol for immediate implant loading in edentulous or potentially edentulous patients. This innovative procedure can successfully guide the clinician step by step in a predictable way from diagnosis to delivery of an immediately loaded implant-supported full-arch rehabilitation.

Purpose: The aim of this in vitro study was to verify whether or not stock and computer-aided design/computer-aided manufacturing (CAD/CAM) abutments show similar precision in the connection with the respective implants., Materials and Methods: Ten CAD/CAM titanium abutments were compared with 10 stock titanium abutments. Each abutment fit a regular-platform implant (Institute Straumann). Implants and abutments were measured independently and then connected. During the connection procedure, the torque was measured using a six-axes load cell. Then, outer geometric features of the implant-abutment connection were measured again. Finally, the assembly was sectioned to provide the analysis of inner surfaces in contact. The geometric measurements were performed using a multisensored opto-mechanical coordinate measuring machine. The following parameters were measured and compared for the CAD/CAM and stock titanium abutment groups, respectively: width of interference and interference length between the conical surfaces of the implant and abutment; and volume of material involved in the implant-abutment connection., Results: Interference width mean ± SD values of 18 ± 0.5 and 14 μ 0.5 μm were calculated for the stock and CAD/CAM titanium abutment groups, respectively. The difference was statistically significant (P = .02). Furthermore, the interference length mean ± SD values of 763 ± 10 and 816 ± 43 μm were calculated for stock and CAD/CAM titanium abutment groups, respectively. The difference was also statistically significant (P = .04). Finally, the volume of material involved in the implant-abutment connection was compared between stock and CAD/CAM titanium abutment groups; the mean ± SD values of 0.134 ± 0.014 and 0.108 ± 0.023 mm 3 were significantly different (P = .009)., Conclusion: Both standard and CAD/CAM abutment groups showed a three-dimensional (3D) seal activation after the screw tightening. Nevertheless, stock titanium abutments showed a significantly higher volume of material involved in the implant-abutment connection compared with that of CAD/CAM titanium abutments.

The aim of this study was to compare the interproximal papilla stability of restorations supported by computer-aided design/computer-assisted manufacture (CAD/CAM) abutments to those supported by prefabricated stock abutments in anterior areas over a 2-year follow-up. Abutments were selected depending on implant inclination and thickness of buccal peri-implant soft tissues from the following: zirconia stock, titanium stock, zirconia CAD/CAM and titanium CAD/CAM. Differences between the height of the papilla tip were measured (REC)., Results: REC values of titanium and zirconia CAD/CAM abutments were significantly lower than those of titanium and zirconia stock. The use of titanium and zirconia CAD/CAM abutments is related to better interproximal papillae stability.

Purpose: Different types of tooth preparations influence the marginal precision of zirconium-oxide based ceramic single crowns. In this in vivo study, the marginal fits of zirconium-oxide based ceramic single crowns with vertical and horizontal finish lines were compared., Materials and Methods: Forty-six teeth were chosen in eight patients indicated for extraction for implant placement. CAD/CAM technology was used for the production of 46 zirconium-oxide-based ceramic single crowns: 23 teeth were prepared with vertical finishing lines, 23 with horizontal finishing lines. One operator accomplished all clinical procedures. The zirconia crowns were cemented with glass ionomer cement. The teeth were extracted 1 month later. Marginal gaps along vertical planes were measured for each crown, using a total of four landmarks for each tooth by means of a microscope at 50× magnification. On conclusion of microscopic assessment, ESEM evaluation was completed on all specimens. The comparison of the gap between the two types of preparation was performed with a nonparametric test (two-sample Wilcoxon rank-sum test) with a level of significance fixed at p < 0.05. All data were analyzed with STATA12., Results: In the group with horizontal finish line preparations, the median value of the gap was 35.45 μm (Iqr, 0.33); for the vertical finish line group, the median value of the gap was 35.44 μm (Iqr, 0.40). The difference between the two groups was not statistically significant (two-sample Wilcoxon rank-sum test, p = 0.0872)., Conclusions: Within the limitations of this study, the gaps of the zirconium-oxide-based ceramic CAD/CAM crowns with vertical and horizontal finish line preparations were not different., (© 2015 by the American College of Prosthodontists.)

Purpose: The purpose of this study was to investigate the marginal precision of computer numeric control-milled frameworks fabricated of grade 4 commercially pure titanium or cobalt-chrome alloy through digital technology and to compare them with conventional cast frameworks., Material and Methods: A titanium cast of a mandibular arch with six implant analogs was used as a master. The master cast was measured with a coordinate measuring machine. Fifteen rigid anatomic frameworks were created on the master cast in cast gold alloy and milled in titanium or cobalt-chrome material. The fifteen anatomic frameworks were measured in the same manner as the master cast. While the milled frameworks were measured once, at the end of the milling process, the cast anatomic frameworks were measured twice: immediately after the casting and divesting procedures and again after a technical adaptation procedure. Each anatomic framework was weighed. To compare the measurements obtained from each group of frameworks, descriptive statistics were calculated and one-way analysis of variance was performed, with values considered statistically significant at P < .05., Results: The mean weight of the cast frameworks was 33.41 g, the cobalt-chrome frameworks weighed 18.12 g on average, and the titanium frameworks averaged 8.7 g. The mean values for three-dimensional deviation of the center point position for each group of frameworks were 261 μm (cast frameworks before adaptation), 49 μm (cast frameworks after adaptation), 26 μm (milled frameworks in cobalt-chrome), and 26 μm (milled frameworks in titanium)., Conclusions: Within the limitations of this in vitro study, absolute passive fit cannot be achieved, regardless of material and fabrication technique. Anatomic milled frameworks fabricated in titanium or cobalt-chrome presented reduced center point deviation compared to cast frameworks. Titanium frameworks weighed less than cobalt-chrome and cast gold alloy frameworks.

This clinical report presents a case treated by a currently available customized orthodontic treatment system. The use of patient-specific brackets, indirect bonding transfer devices, and customized archwires decreases treatment and chairside time, making orthodontic cases more predictable, accurate, and efficient. The need for time-consuming adjustments is greatly reduced, and appliance customization further facilitates the achievement of the final desired occlusion from the first day of treatment.

Purpose: To evaluate patient perception and operating time of digital (DW) and conventional (CW) workflows for the rehabilitation of a screw-retained, single-implant crown.Materials and Methods: A convenience sample of 10 patients with single implants in posterior sites was recruited for treatment with screw-retained single crowns, fabricated with either a DW or a CW protocol. Operating time and clinical adjusting time were recorded with a stopwatch, and patient preference and self-perception of the esthetic outcome were evaluated with a visual analog scale.Results: The mean operating time for the DW crowns was 16:21 minutes and for the CW crowns was 28:28 minutes. The mean total adjustment times were 118.1 seconds for the DW protocol and 181.5 seconds for the CW protocol. The mean score regarding self-perception of the esthetic outcome was 73 for DW crowns and 69 for CW crowns; for discomfort, the mean score was 15.5 for DW and 62 for CW.Conclusion: The DW approach resulted in a time reduction for both the impression phase and operative time.
