Amaç: Bu tez çalışmasında, Bilgi Çağı teknolojisinin günlük yaşam ile durmaksızın kesiştiği başlıca alanlardan biri olan, `Hastane Bilgi Sistemleri` (HBS) ele alınmaktadır. Her gün milyonlarca insana sağlık hizmeti sunulmasına aracılık eden bu sistemler, yine Bilgi Çağı sürecinde yeşeren disiplinlerden biri olan `Denetim` yönünden incelenmektedir. Günümüzde neredeyse HBS kullanmayan bir sağlık kuruluşunun olmaması ve bu sistemlerin işleyişindeki artan karmaşıklık, klinik denetimin yanında bilgi sistemleri ve teknolojileri denetimini de giderek daha önemli hale getirmiştir. Bununla beraber, sistemlerdeki aksaklıkların tespit edilerek sistemlerin iyileştirilmesi de ayrı bir gereksinimdir. Bu araştırmanın temel amacı, Türkiye'de bankacılık ve sermaye piyasalarında hali hazırda yasal bir gereklilik olarak uygulanmakta olan Bilgi Sistemleri Denetimi (BSD) süreçlerinin, Hastane Bilgi Sistemleri (HBS) açısından gerekliliğini vurgulamak ve bu hususta geliştirilebilecek genel bir çerçeve hakkında bilgi vermektir.Yöntem: Karma araştırma yöntemi izlenerek çalışılan bu tezde, BSD konusu salt HBS açısından ve bir denetçinin bakış açısıyla incelenmiştir. Bu doğrultuda, uygulanabilir bir denetim çerçevesi önerilmeye çalışılmış, buradan hareketle Hastane Bilgi Sistemleri Denetimi (HBSD) kavramı türetilmiş ve bu tez yazım tarihi itibariyle henüz uygulaması bulunmayan oldukça bakir bir konu, bir sağlık bilimci ve denetçi gözüyle irdelenmiştir.Bulgular: Çalışma kapsamında, BSD uygulamalarına ilişkin Türkiye yerel mevzuatında yürürlükte olan ilgili düzenlemelere dayalı olarak ve ISACA'nın COBIT rehberi kullanılarak HBSD için formal bir çerçevenin geliştirilebilir olduğu bulunmuştur.Sonuç: Halihazırdaki Bilgi Sistemleri Denetimi (BSD) uygulamalarının Hastane Bilgi Sistemleri (HBS) yönünden sağlık hizmetleri alanına uyarlanabilir olduğu ve bu doğrultuda genel nitelikli bir çerçeveye ihtiyaç duyulduğu sonucuna varılmıştır. Bununla birlikte, Hastane Bilgi Sistemleri Denetimi (HBSD) konusu kavramsal olarak ilk kez akademik bir çalışmaya konu edilmiş ve literatüre kazandırılmaya çalışılmıştır.Anahtar Kelimeler: hastane bilgi sistemleri denetimi, denetim çerçevesi, COBIT Objective: In this thesis, Hospital Information Systems (HIS), which is one of the main areas where the Information Age technology intersects with daily life, is taken into consideration. These systems, which mediate the provision of health services to millions of people every day, are examined from IS Auditing perspective, that is also an emerging concept. The factual knowledge that any form of the health organizations of today cannot be expected to render healthcare services without HIS as well as the increasing complexity on the functioning of these systems has made the audit of information systems and technologies increasingly important besides clinical supervision. However, it also brings in a separate need to identify the failures in HIS in order to improve the systems. The main objective of this research to emphasize the necessity of Information Systems Auditing (ISA), which is already being applied as a statutory requirement in financial and capital markets in Turkey, for a possible adaptation to Hospital Information Systems (HIS) and provide information about a general framework to be developed in this regard.Method: In this thesis, which has a mixed research method, ISA is examined in terms of HIS and from an auditor's point of view. In this respect, a viable audit framework has been tried to be developed and from this point, the concept of Hospital Information Systems Auditing (HISA) has been derived to examine a virgin topic, which has not yet been studied as of the date of this paper, from the eye of a health scientist and an auditor.Results: Within the scope of the study, the developability of a formal audit framework has been found applicable to promote HISA using ISACA's COBIT guide and based on local ISA regulations that are already in force in the related legislation in Turkey.Conclusion: It was concluded that the existing ISA applications are adaptable to HIS and health services field in line with the need for a general framework. Alongside this, the subject of Hospital Information Systems Auditing (HISA) has been conceptualized for the first time in an academic study and it has been tried to contribute to the literature.Key words: hospital information systems auditing, audit framework, COBIT 88

Amaç: Meme kanserlerinin erken tanısı için değişik görüntüleme yöntemleri kullanılmaktadır. Bu yöntemlerden bazıları, Manyetik Rezonans Görüntüleme (MRG), Pozitron Emisyon Tomografisi (PET) ve ultrasondur. En sık kullanılan yöntem ise mamografidir. Mamografi sisteminin maliyetinin yüksek olması ve bazı tümör yapılarını ayırt edememesi gibi dezavantajları vardır. Son yıllarda, dokuya zarar vermeyen dalga boyunda ışık kullanılarak, gönderilen ışığa dokunun verdiği yanıtı inceleyerek görüntü oluşturabilen Difüz Optik Tomografi (DOT) sistemleri kullanılmaya başlanmıştır. Bu tezde, bir klinik DOT sistemin elektronik devre tasarımı ve uygulamasının yapılması amaçlanmıştır.Yöntem: Bir Difüz Optik Tomografi sisteminin, görüntüleme oluşturabilmesi için gerekli olan veriyi elde etmek üzere, gelişen teknolojinin sunduğu yeni devre elemanları ve yazılım geliştirme olanaklarını kullanarak, örnek bir elektronik donanım, mikrodenetleyici yazılımı ve donanımı kontrol edip elde edilen ham görüntüleme verisini bilgisayar ortamına aktaran bir bilgisayar yazılımı geliştirildi. Mikrodenetleyici yazılımı Rowley Crossworks For Arm geliştirme ortamında, bilgisayar yazılımı Visual Studio geliştirme ortamında C# dili kullanılarak oluşturuldu.Bulgular: Geliştirilen sistemin mikrodenetleyici tabanlı donanımı ve bu mikrodenetleyicide çalışan yazılım ile bilgisayarda çalışan C# tabanlı olarak geliştirilen yazılımın, benzetilmiş çalışma şartları altındaki performansı incelendi.Sonuç: Arm cortex M3 temelli donanım sistemi ve yazılımı yapıldı. Optik seçici düzenekle birlikte test edildi. Bilgisayar arayüz programı yazıldı. Elektriksel performans ölçümleri yapıldı ve ham görüntü veri dosyaları elde edildi. Objective: Several different imaging techniques have been used for early diagnosis of breast cancer. Magnetic resonance imaging (MRI), positron-emission tomography (PET) and ultrasound imaging are some of them. Most often used technique is mammography. The mammography technique has some disadvantages as having high cost and not being able to differentiate some types of tumors. In recent years, diffuse optical tomography (DOT) systems that use near infrared light that are not harmful to tissue and can generate images by examining the response of the tissue to the light, have been put into use. In this theses, circuit design and implemention of a clinical DOT system have been targeted.Method: In this study, in order to obtain the necessary data for a Diffuse Optical Tomography system to generate an image, and electronical hardware system, a microcontroller firmware, and a computer software which controls the hardware and conveys obtained raw imaging data to computer environment have been developed and implemented by taking advantages of the new circuit components and software development tools presented by developing technologies. Microcontroller software was developed in Rowley Crossworks development environment and computer software was developed in Visual Studio development environment by using C# language.Results: System performance of the developed microcontroller based hardware, the code that runs in the microcontroller and the C# based computer program, have been examined under simulated working conditions.Conclusion: Arm cortex M3 based hardware system and it's firmware implemented. The system tested with an optical switching device. Electrical performance measurements of the system was carried out and raw imaging data files obtained. 73

Amaç: Şizofreni hastalarının günlük yaşamı ve günlük sosyal aktiviteleri hastanın iyileşme sürecinde ve takibinde oldukça önemlidir. Bu aktivitelerin takibi geleneksel olarak kâğıt tabanlı kayıtlarla yapılmaktadır. Bu çalışmada akıllı telefon üzerinde çalışabilen bir mobil uygulama ile bu aktivitelerin kaydı ve takibinin yapılması ve sonuçların değerlendirilmesi hedeflenmiştir.Yöntem: Geliştirdiğimiz uygulama hastanın günlük aktivitelerini kayıt etmesini sağlayan, yapmayı unuttuğu aktivitelerin hatırlatan, ilaç kullanımını takip eden ve hatırlatan, bu kayıtların canlı olarak doktor panelinden görüntülemesini sağlayan hibrit yapıda bir Andorid OS uygulamasıdır. Hastaların çalışmadan önce ve sonra klinik değerlendirmesi ve psikiyatrik testleri yapılmıştır. Geliştirilen mobil uygulama kullanılabilirlik çalışması sonrası 14 hasta tarafından bir ay kullanılmıştır. Çalışmayı tamamlayan 12 hastanın verileri değerlendirilmiştir.Bulgular: Çalışma süresince teknik açıdan bir sorun yaşanmamıştır. Uygulamaya, bir aylık sürede 12 kullanıcı tarafından toplam 1901 veri kaydedilmiştir. Hastaların günlük ortalama veri giriş sayısı 5,1'dir. Hastaların bir ay içinde uygulamayı kullanma sayıları minimum 22, maksimum 388'dir. Çalışmadan önce ve sonra PANSS, CGI, IADL, ŞİLÖ, FROGS toplam puanlarında anlamlı bir değişim görülmemiştir. İki alt ölçekte değişim görülmüştür, bunlardan biri olumlu biri olumsuz yöndedir, ancak kullanım sayısının yetersizliği nedeni ile bu değişimi uygulama kullanımına bağlamak zordur. Hastaların uygulamayı kullanma sayıları ile PANNS Pozitif belirtiler, PANNS Toplam, Klinik Global İzlenim arasında negatif korelasyon, ŞİLO Günlük yaşam becerileri ve ŞİLO toplam puan arasında pozitif korelasyon izlenmektedir.Sonuç: Teknik açıdan sorun olmamakla birlikte, uygulamanın kullanım sıklığı açısından bir sorun vardır. Bu sıklığı artırmak için neler yapılmasının araştırılması gerekmektedir.Anahtar Kelimeler: Mobil uygulama; Sosyal aktivite; Şizofreni; Hasta takibi; Sosyal izolasyon Objective: Daily life and social activities of schizophrenic patients are important because these activities both help the patients getting better and they are also indicator of being functional for the patients. Traditionally, these activities are recorded on paper based media. The purpose of this study is to evaluate the use of a mobile application which records the activities of the patients. Method: We have developed a hybrid Android OS application which is used for recording daily activities of the patients, reminds the required activities and medications, follows up medication usage, and permits the physican to view this data in a separate doctor panel. Clinical evaluation and pshyciatric tests of the patients were performed before and after the study. After usability study, the application was used by 14 patients. We evaluated the data of 12 patients who completed the study. Results: We had no technical problems during the study. Twelve users recorded a total of 1901 activities in one month. Average data entry per day is 5,1 for each patient. They used the application minimum 22, maximum 388 times in a month. We observed no statistically significant changes between pre and post PANSS, CGI, Lawton&Brody instrumental activities of daily living, Functional Remission of General Schizophrenia (FROGS) Scale, or Katz Index of Independence in Activities of Daily Living. We observed statistically significant changes in two sub-scales, one in a positive, one in a negative way, however it is difficult to link these differences to use of application, due to low usage of the application. There are negative correlations between number of use of the application and PANNS Positive scale, PANNS Total, and CGI. There are positive correlations between number of use of the application and FROGS total and FROGS daily life.Conclusion: We had no technical problems during the study, however we observed a problem of frequency of use of the application.
